Abstract

Description
技术领域
本发明属于药物疗法领域。特别地,本发明涉及用于治疗存在或不存在气流限制的肺气肿和其他形式的慢性阻塞性肺病(COPD)的组合物和方法。
背景技术
COPD是最普遍的非传染性疾患之一。COPD是一种复杂的临床状况,这种状况具有与吸烟相关的固定气流限制作为共同的因素,在若干个月的观察期内,这种状况没有显著改变。此外,气流阻塞随着年龄的增长显示异常快速的进行性恶化。该病程最终导致慢性呼吸症状。
COPD发病机制的特征在于慢性炎症和弹性纤维的加速损失[1]1。COPD中的慢性气流限制是由小气道疾病、肺实质破坏(即,气肿)或两者混合引起的[1](方括号中的数字是指说明书末尾的参考文献清单中的数字)。气肿是COPD表型,气肿的特征在于由于蛋白酶/抗蛋白酶和弹性蛋白降解/修复不平衡导致肺实质中弹性蛋白纤维的过度损失。尽管不如弹性蛋白降解在气肿发病机制中的作用受重视,但胶原蛋白在肺实质中的积累是气肿的另一个重要发病特征[2]。
弹性蛋白,即弹性纤维的主要组分,是可为肺提供弹性、弹力和变形性的独特蛋白质,因此弹性蛋白为呼吸的基本要求[3]。弹性蛋白主要在子宫内和儿童早期时产生[4]。
弹性蛋白纤维的产生始于由若干种细胞类型合成弹性蛋白前体弹性蛋白原[4]。随后弹性蛋白原分泌于细胞外基质中,传输至原纤维支架上,与多种其他弹性蛋白原蛋白联合得到聚合物,最后与其他弹性蛋白原聚合物交联得到要求持续终生(lifetime)的成熟且持久的弹性蛋白纤维[4]。该交联过程通过酶LOX和LOX样蛋白(LOXL)1至4促进[4]。腓骨蛋白4和5在弹性蛋白纤维的发育和维持中也起重要作用。原型LOX和腓骨蛋白-4主要参与弹性蛋白纤维的初始发育,而LOXL1和腓骨蛋白-5对于弹性蛋白修复是必要的。
弹性蛋白降解会损害肺的弹性[4],由于抗蛋白酶的保护作用与蛋白酶的破坏性之间的不平衡,COPD患者中的弹性蛋白降解增强[3]。鉴于受损的弹性蛋白纤维比天然纤维更容易被蛋白酶进一步破坏,弹性蛋白降解的另一个驱动因素是弹性蛋白降解与弹性蛋白修复之间的不平衡[5]。此外,通过LOX酶交联的弹性蛋白纤维对蛋白酶具有相对抗性,而未交联的蛋白则容易降解[6-8]。加速的肺弹性蛋白降解是导致肺功能丧失的气肿的重要发病机制[9]。
除了加速的弹性蛋白损失,气肿患者的细胞外基质还存在另一个问题。已经证明,与对照相比,气肿患者的肺中胶原蛋白的水平升高[10],并且与一秒用力呼气量(FEV1)成反比[11]。
铜是活化LOX酶(即,原型LOX和LOXL1-4)的辅因子[12]。小鸡中诱导的铜缺乏(copper deficiency)会由于LOX活性降低而破坏弹性蛋白交联,并导致弹性蛋白含量的净减少[12]。铜缺乏中降低的弹性蛋白含量的原因似乎是由于降解增强引起,因为未交联的弹性蛋白原比适当交联的弹性蛋白对蛋白酶的敏感性高得多[12]。缺铜小鸡中铜的补充可将抗蛋白酶的弹性蛋白纤维的沉积恢复到接近正常值[7]。
弹性蛋白纤维损失会引起肺COPD,同时其会引起皮肤皱纹的形成[13]。健康对照中的含铜乳剂会诱导皮肤中弹性蛋白交联的增加[14]。
有证据表明肺气肿中存在铜缺少(copper shortage)。通过向大鼠和仓鼠喂食缺铜饮食,可以在大鼠和仓鼠中诱导气肿性改变[15,16]。铜缺乏使大鼠肺中弹性蛋白含量减少17%,并且肺泡间隙增大35%[15]。铜的补充使肺弹性蛋白的超微结构恢复到接近正常水平[15]。
在气肿中促炎性细胞因子肿瘤坏死因子α(TNF-α)的表达增强[17,18]。肺特异性TNF-α过表达的转基因小鼠产生气肿性病变[19]。结论是慢性TNF-α诱导的肺炎症后发生铜缺乏,并且此可能在炎症诱导的肺损伤中起必要的作用。
还有一项人研究表明,气肿区中存在局部铜缺乏。蛋白铜代谢结构域包含体-1(COMMD1)是铜代谢的关键调节因子[20]。已经证明气肿肺中COMMD1以及活性LOX、LOXL1和LOXL2的水平减少[21]。
COPD患者呼出的呼吸冷凝物中的铜浓度减少,并且与FEV1成反比[22]。此可以表明COPD肺中铜缺乏的存在。与此相符,患有门克斯病(Menkes disease)(铜传输的遗传病症)的个体可发展为严重的气肿[23]。
尽管现有技术可能建议铜是气肿肺中弹性蛋白修复和发育的有用刺激物,但是存在一个关键问题,其排除了将铜用作气肿患者的疗法。LOX酶不仅是弹性蛋白交联的刺激物,而且是胶原蛋白交联的刺激物。胶原蛋白交联的增加将引起组织、成熟增强,从而引起气肿肺中增强的胶原蛋白积累,鉴于气肿患者中的胶原蛋白水平已经增加,并且会引起肺气肿向肺纤维化的转变(这是另一种破坏性肺病),所以增强的胶原蛋白积累是非常不期望的。因此,铜诱导的胶原蛋白积累的刺激反向教导了使用铜作为气肿患者的疗法。
COPD患者最重要的主诉是劳累性呼吸困难,并且在后期还会发生休息时呼吸困难和运动不耐症。从机制的角度来看,将COPD视为综合症而不是统一的疾病实体似乎更为适当。COPD患者的气流阻塞是由小气道疾病、肺气肿或两者合并引起的。未患COPD(即,没有气流阻塞)的(前)吸烟者中的计算机断层扫描术(CT)上也经常存在严重的气肿。
由于呼吸短促、咳嗽和喘息的类似症状,临床上难以辨别气肿与慢性支气管炎。在大部分患者中,存在归因于慢性支气管炎或气肿的特征的组合。
Fleischner胸腔成像学会(Fleischner Society for Thoracic Imaging)发表了一份声明,描述了CT可定义的COPD亚型。可以辨别的主要病理学分类是气道壁增厚、支气管扩张、小气道疾病和气肿。
应该认识到,这些放射学异常也可在未患COPD的个体中鉴别到。气肿的特征在于不可逆的肺损伤。因此,肺组织的弹性丧失,从而导致气道塌陷并发生气流阻塞。慢性支气管炎是一种炎性疾病,这个疾病始于肺内较小的气道,逐渐发展至较大的气道。其增加了气道中的粘液,并增加了支气管中的细菌感染,从而阻碍了气流。
当前的COPD药理学疗法能够改善呼吸症状和恶化的频率,以及改进生活质量和运动能力[1]。还报道了长效支气管扩张剂和皮质类固醇吸入疗法对肺功能下降速率的减缓作用[2-4]。不幸的是,吸入性支气管扩张剂和皮质类固醇主要靶向COPD的气道组分,并且在以气肿为主的COPD患者中的作用不如在以气道为主的COPD患者中那么好。但是,CT上气肿的存在是一个重要发现,因为其与死亡率密切相关。
除肺移植以外,没有一种COPD干预已经证明能有效恢复肺功能[1]。因此,迫切需要为大量气肿个体建立特定的药理学疗法。
WO 03/068187 A1公开了糖胺聚糖,例如肝素,用于治疗呼吸病症诸如COPD,特别是慢性气流限制(CAL)的用途。
WO 2012/073025 A1公开了用于治疗和/或预防COPD的糖胺聚糖,诸如肝素,其中在施用于受试者之后,肝素减少了受试者肺中的炎症。
发明内容
本发明基于如下意外发现,铜与某些糖胺聚糖,特别是肝素的组合可用于治疗肺气肿和其他形式的COPD。该组合对气肿患者的肺中弹性蛋白纤维的修复和发育具有有益作用,并且同时预防铜诱导的胶原交联的刺激。
尽管现有技术公开了使用肝素作为COPD患者的吸入单一疗法[24,25],但现有技术没有教导或表明将肝素加入铜吸入疗法中以通过刺激弹性蛋白原交联来进一步刺激弹性蛋白修复/发育过程并且更重要地预防铜诱导的胶原蛋白交联的刺激的协同价值。
因此,本发明在一方面提供了一种用于肺气肿和其他形式的COPD的治疗方法中的组合物,所述组合物包含含有铜化合物的活性剂和糖胺聚糖或其生理学上可接受的盐。通过吸入使用施用是特别优选的。
在一个优选的实施方式中,根据本发明的组合物用作标准药理学COPD治疗的添加剂,所述标准药理学COPD治疗包括支气管扩张剂和免疫调节剂,诸如吸入性皮质类固醇和口服大环内酯。
在本发明的另一个方面,提供了一种治疗患有肺气肿或另一种形式的COPD的受试者的方法,所述方法包含向所述受试者施用治疗有效量的组合物,所述组合物包含含有铜化合物的活性剂和糖胺聚糖或其生理学上可接受的盐。
在以下详细描述中将更全面地概述根据本发明的组合物的活性成分对于弹性蛋白纤维的修复和发育以及预防胶原蛋白积累的重要性。

具体实施方式
肺弹性蛋白纤维产生的再活化和损伤的弹性蛋白纤维的修复是恢复肺功能所必要的。为了在成人中产生新的弹性蛋白纤维和修复损伤的弹性蛋白纤维,三个步骤至关重要:(a)弹性蛋白原合成的活化,(b)弹性蛋白原蛋白组装成聚合物链的活化,以及(c)赖氨酰氧化酶介导的交联的活化。
提供了用于治疗肺气肿和其他形式的COPD的组合物和方法。这些组合物包含含有铜的活性剂和糖胺聚糖或其生理学上可接受的盐。
本发明的组合物将用于治疗患有存在或不存在气流限制的肺气肿和其他形式的COPD或具有产生上述肺气肿和其他形式的COPD的风险的受试者。通常地,受试者是哺乳动物,特别是人类,但可以是脊椎动物。气流限制通常既是进行性的,又与肺弹性蛋白纤维的弹性降低相关。
提供了治疗肺气肿和其他形式的COPD的方法。此类方法包括诊断受试者肺的一种或多种病症和施用治疗有效量的组合物,所述组合物包含含有铜的活性剂和糖胺聚糖或其生理学上可接受的盐。
术语“治疗(treating/treatment)”是指执行如下方案,所述方案可以包括向患者(人或其他)施用一种或多种组合物或活性成分,以努力修复损伤的肺和/或预防疾病或病症进展的发生。“治疗”不要求完全停止疾病进展,不要求完全恢复所有肺损伤,并且特别包括仅对患者有轻微作用的方案。
术语“治疗有效量”是指在施用于患者时足以引起患者疾患改进的本发明组合物的量。改进并不是指治愈,并且可以仅包括患者疾患的轻微改变。其还包括预防疾患或终止或延迟其进展的活性剂的量。
出于本发明的目的,“其他形式的COPD”可以定义为如下的疾患:气道壁增厚、支气管扩张、慢性支气管炎和/或小气道疾病。
受试者通常地是成熟的成人。例如,受试者可以是21至85岁,优选25至70岁,更优选30至60岁,并且甚至更优选40至50岁。本文提及的任何或特定症状通常地在成人期发作。例如,受试者在经历特定症状之前可能已经至少25岁,更优选至少30岁,仍更优选至少35岁,并且甚至更优选至少40岁。特别地,与气肿的更晚期阶段相关的症状,诸如本文提及的任何症状,可在生命的此类后期阶段发作。具有产生气肿的遗传易感性的受试者,诸如α1-抗胰蛋白酶缺乏症受试者,可较早产生该疾病。例如,其可以在20至31岁,优选22至28岁,或更优选24至26岁时呈现出一种或多种症状或特定症状。替代地,他们可以在本文提及的任何年龄范围首次显示症状。受试者可以已经在本文指定的任何年龄或任何年龄范围内被诊断出。
在受试者不是人的情况下,受试者可以是家畜动物或农业上重要的动物。动物可以是例如绵羊、猪、母牛、公牛、禽鸟或其他商业上饲养的农畜。特别地,动物可以是母牛或公牛,并且优选是奶牛。动物可以是家养宠物,诸如狗、猫、鸟或啮齿动物。在一个优选的实施方式中,动物可以是猫或其他猫科动物。动物可以是猴子,诸如非人类灵长类动物。例如,灵长类动物可以是黑猩猩(chimpanzee)、大猩猩(gorilla)或猩猩(orangutan)。在本发明的一个优选的实施方式中,动物可以是马,并且例如可以是赛马。
本发明组合物的主要治疗活性成分是铜和糖胺聚糖。下文将更详细地讨论这些成分。
铜
本发明的组合物使用包含铜化合物的活性剂。如本文所用的术语“活性剂”是指具有对肺弹性蛋白的修复和发育具有刺激作用的化合物的化学成分。活性剂包含铜化合物,特别是铜盐。各种铜盐可以提供铜化合物的来源。合适的铜盐包括但不限于硫酸铜、氯化铜、葡糖酸铜、乙酸铜、庚酸铜、氧化铜、蛋氨酸铜、氧化二铜、叶绿素铜和依地酸钙铜。其中,硫酸铜是优选的。
糖胺聚糖
本发明的组合物使用糖胺聚糖,并且特别是肝素。糖胺聚糖是具有特征性二糖重复序列的线性杂多糖,其通常地在D-葡糖胺、半乳糖胺和糖醛酸残基上被高度N-硫酸化和O-硫酸化。
任何合适的糖胺聚糖可以用于本发明中。适用于本发明的糖胺聚糖和糖胺聚糖盐的平均分子量为12至18kd。糖胺聚糖或盐可以以此范围内的各种分子量大小存在。对于其他细节,可以参考现有技术,特别是WO 03/068187及其对应的EP 1 511 466,其内容通过引用并入本文中。
糖胺聚糖可以是任何合适的可商购的糖胺聚糖,并且可以是例如未分级分离的糖胺聚糖。通常地,从天然来源诸如从动物分离糖胺聚糖。在一些情况下,糖胺聚糖可以是已经被合成,而不是天然存在的分子。
本发明中可以使用任何合适的生理学上可接受的糖胺聚糖盐,特别是金属盐,例如钠盐、碱金属或碱土金属盐。其他盐包括钙、锂和锌盐。还可以使用铵盐。该盐可以是糖胺聚糖钠或糖胺聚糖硫酸盐。本文提及的特定糖胺聚糖的衍生物的盐也可用于本发明。在提及糖胺聚糖的本申请中,此种提及还包括其生理学上可接受的盐。
在本发明的一个特别优选的实施方式中,所用的糖胺聚糖是以下中的任一种:硫酸软骨素A至E肝素、硫酸肝素、乙酰肝素、硫酸乙酰肝素、透明质酸、硫酸角质素、其任一种的衍生物或其生理学上可接受的盐或其任意两种的混合物。
肝素是一种天然存在的粘多糖,其存在于各种器官和组织,特别是肝、肺和大动脉中。肝素是由(1,4)糖苷键连接的交替的a-D-葡糖胺和己糖醛酸酯残基的聚合物。当天然合成糖胺聚糖时,通常地将其缀合至中心蛋白核心。然而,优选地,本发明中使用的糖胺聚糖缺乏此种中心核心。糖胺聚糖的可商购制剂通常缺乏该核心并且可以使用。
优选地,将未分级分离的肝素用于配制品中。可以将包含达肝素(dalteparin)和依诺肝素(enoxaparin)的低分子量肝素和糖胺聚糖家族的其他成员(包括硫酸乙酰肝素)代替未分级分离的肝素与铜化合物一起用于吸入配制品中来刺激弹性蛋白原聚合和/或预防铜诱导的胶原蛋白交联。
肝素在临床上用作抗凝剂,其中认为肝素通过与抗凝血酶III(AT-III)、肝素辅因子II和其他凝血因子相互作用来发挥其作用。通常地,肝素将保留一些抗凝活性,即能够增加个体的凝结时间。因此,优选地,肝素能够结合抗凝血酶III(AT-III)和/或肝素辅因子II(HCII)并因此抑制凝结。优选地,肝素能够与AT-III、凝血酶和凝结因子形成复合物。然而,在一些实施方式中,也可以使用缺乏抗凝活性或抗凝活性降低的肝素。因此,肝素可以已经被修饰,以使其具有未修饰形式的活性的0至80%,优选5至60%,更优选10至40%,并且甚至更优选10至30%,或与未修饰肝素相比,具有上述百分比的活性。其他糖胺聚糖,特别是硫酸皮肤素也具有抗凝活性。因此,优选地,如上文关于肝素及其衍生物所讨论,所使用的糖胺聚糖及其衍生物将保留一些抗凝活性。
其他组分

受试者评定
本发明提供了用于促进气肿患者的肺中弹性蛋白纤维的修复和发育并且预防铜诱导的胶原蛋白交联的刺激的组合物,所述组合物包含含有铜化合物的活性剂和糖胺聚糖或其盐。所用的铜化合物和糖胺聚糖或盐、递送途径以及组合物和所治疗的受试者的任何其他参数可以与本文对于本发明的任何其他实施方式所述相同。
本发明的组合物优选诱导受试者疾患的改进和/或疾病进展的预防/减缓。因此,所述组合物可以用于处理患有或易患如本文所定义的气肿和/或其他形式的COPD。所述组合物可以预防、改善、改进或治疗所述疾患。所述组合物可以减慢或阻止气肿和其他形式的COPD的进行性恶化特征,或在一些情况下甚至引起一些恶化的逆转。所述组合物可以预防、减少或逆转与气肿和其他形式的COPD相关的一种或多种症状。优选地,所述组合物还将增加受试者的幸福感和提高受试者生活质量。
本发明的组合物优选减少、消除或至少预防以下中的一种或多种的进一步增加:
-包括但不限于FEV1和扩散能力的肺功能参数的加速下降
-对肺结构的破坏。
用本发明的组合物治疗还可以指FEV1/FVC的比率不进一步下降或得到改进。例如,该比率可以接近健康受试者中的预期比率。
所述组合物可以减少肺弹性蛋白降解并促进肺弹性蛋白修复。所述组合物还可以对气肿肺中胶原蛋白的积累具有预防作用。

本发明的组合物可以通常地将肺功能参数,诸如扩散能力和FEV1的下降减小10至100%,优选20至80%,更优选30至60%,并且甚至更优选40至50%。所述组合物可以将FEV1的年下降减小10至100ml,优选每年20至60ml,并且甚至更优选每年30至40ml。在一些情况下,治疗后,受试者将呈现出肺功能参数的改进,使得FEV1和扩散能力为预计值的25至100%,优选40至100%,更优选60至100%,并且甚至更优选80至100%。
用CT扫描测量肺密度是一种定量肺气肿严重性的便利方法。本发明的组合物可以减慢或阻止气肿患者CT-肺密度的进行性下降,或甚至可以增加肺密度。
本发明的组合物可以减少肺组织降解并且促进损伤的肺组织的修复。
本发明的组合物可以消除本文提及的气肿和其他形式的COPD的任何症状和特征,延迟其发作或减少其严重性。
施用和配制
本发明的药物组合物可以通过用药物技术中常规的生理学上并且特别是药学上可接受的标准载体和/或赋形剂配制至少一种铜化合物,优选硫酸铜,和优选肝素的糖胺聚糖来制备。
配制品的确切性质将取决于若干个因素,包括所用的特定铜化合物和糖胺聚糖以及期望的施用途径。合适类型的配制品在雷明顿药物科学(Remington's PharmaceuticalSciences),第22版,Mack Publishing Company,美国宾夕法尼亚州东部(EasternPennsylvania,USA)中充分描述,其全部公开内容通过引用包括在本文中。
在一个特别优选的实施方式中,包含铜化合物和糖胺聚糖的组合物作为吸入疗法施用,所述吸入疗法包括但不限于吸入雾化配制品、定剂量吸入器或以适用于干粉吸入器的形式吸入。所述组合物可以存在于泡罩包装或可破碎胶囊中。因此,施用可以通常地通过口进行。
由于根据本发明的组合物通常地通过吸入或通过装置施用,因此优选其为适用于通过此种途径施用的形式。特别地,所述组合物可以为适用于吸入和/或装置施用的形式。
用于配制和制备待通过吸入施用的组合物的合适方法是本领域中熟知的,并且可以用于本发明中。以硫酸铜和肝素作为雾化疗法为例的组合物可以与包含盐水的赋形剂一起使用。作为干粉配制品的所述组合物可以与包含乳糖的赋形剂一起使用。定剂量吸入器中的组合物可以与包含含有氢氟烷(HFA)的推进剂、含有乙醇的共溶剂和含有油酸的稳定剂的赋形剂一起使用。
待施用的必要剂量通常由医师决定,但取决于许多因素,诸如待治疗的疾患和患者的状况。下文将提供剂量和剂量范围的实例。优选的施用持续时间、优选的施用频率和优选的施用剂量取决于多种因素,包括但不限于年龄、体重和通过CT肺密度测定法测量和肺功能测试定量的气肿性病变的严重性。治疗时间通常地可以为两周、一个月、六个月、一年或更久。在许多情况下,受试者将永久或长期保持使用本发明的组合物。在患有更严重形式的气肿的患者中,使用本发明的优选持续时间是终生,并且优选的施用频率是每天一次。在较轻形式的气肿中,短暂的施用时间和低于每天一次的施用频率可足够。
肺中铜缺乏的严重性是治疗强度的另一个决定因素。呼出的呼吸冷凝物中的铜测量是计算肺中铜缺乏以指导铜吸入疗法的强度和持续时间的一种便利方法。
以硫酸铜和肝素为例的根据本发明的药物组合物优选且有效地以如下剂量施用,这尤其取决于诸如年龄、性别、体重和患者状况的因素。硫酸铜和肝素两者的优选剂量来源于下述用纤维母细胞进行的细胞培养研究(参见“实验”部分),其中评定了各种剂量和组合对弹性蛋白修复和发育的作用。

优选的用于吸入的组合物含有约0.5-1mg硫酸铜和约150,000IU肝素。
构成根据本发明的组合物的治疗活性组分优选同时施用,但是如果需要,也可以依序或分别给予。
在一些优选的实施方式中,本发明的组合物可以被配制成气雾剂。药物气雾剂的配制是本领域技术人员常规的,参见例如,Sciarra,J.,雷明顿(Remington)(同上)。可以将所述药剂配制成干粉、乳液或半固体制剂的溶液气雾剂、分散或悬浮气雾剂。可以使用本领域技术人员已知的任何推进剂系统来递送气雾剂。可以将气雾剂施加于下呼吸道。可以使用本领域技术人员已知的脂质体和纳米颗粒递送方法来递送包含铜化合物和肝素的组合物。脂质体,特别是阳离子脂质体,可以用于载体配制品中。
根据本发明使用的组合物,除活性成分以外,还可以包括药学上可接受的赋形剂、载体、缓冲剂、稳定剂或本领域技术人员熟知的其他物质。特别地,所述组合物可以包括药学上可接受的赋形剂。此类物质应该是无毒的并且不应干扰活性成分的功效。载体或其他物质的精确性质将取决于施用途径。合适的药物载体描述于雷明顿(Remington)(同上)中。
本发明的组合物可以通过适合于将一种或多种治疗组合物引入下呼吸道的任何装置来递送。在一些优选的实施方式中,本发明的装置可以是定剂量吸入器。所述装置可以适合于以液体、泡沫或粉末的精细分散雾滴的形式递送本发明的治疗组合物。所述装置可以使用压电效应或超声振动来取出附接在表面诸如胶带上的粉末,以产生适用于吸入的雾滴。所述装置可以使用本领域中已知的任何推进剂系统,其包括但不限于泵、液化气体、压缩气体等。
在铜化合物和肝素以颗粒或小滴的形式施用的情况下,可以选择颗粒/小滴的大小和/或颗粒/小滴的其他特性以确保将颗粒递送到呼吸道的特定区域。例如,所述组合物可以被设计成仅到达呼吸道的下部。在铜化合物和肝素以水溶液形式递送的情况下,溶液优选是等渗的,以帮助确保有效递送至受试者。特别地,认为直径为10μM的颗粒有效到达呼吸道的下部,因此所述颗粒可以在对于组合物而言是期望标靶的部位使用。在期望将组合物递送至呼吸道的下部诸如肺泡的实施方式中,所施用的颗粒的直径可以是小于10μM,优选小于8μM,更优选小于6μM,并且甚至更优选小于4μΜ。在一个优选的实施方式中,颗粒的直径可以是3μM或更小,并且更优选地,直径可以是2μM或更小。在一个特别优选的实施方式中,颗粒的直径是3至5μΜ。在一些情况下,所施用的颗粒的直径可以是小于1000nm,优选小于500nm,更优选小于250nm,并且仍更优选小于100nm。所述大小可以指固体物质的颗粒或溶液和悬浮液的小滴。
渗透到呼吸道特定部分所必需的颗粒的大小是本领域中已知的,因此可以选择粒度(particle size)以适合靶标大小。可以使用诸如研磨的技术来产生所需的非常小的颗粒。在一些情况下,呼吸道的期望部分可以是上呼吸道,因此可以使用较大的粒度。还可选择颗粒的密度及其形状,以促进其递送至期望部位。
本发明的组合物可以采用多种形式。所述组合物可以是粉末、粉末微球、溶液、悬浮液、凝胶、纳米颗粒悬浮液、脂质体、乳液或微乳液的形式。存在的液体可以是水或其他合适的溶剂,诸如CFC或HFA。在溶液和悬浮液的情况下,所述组合物可以是水性的或包括除水以外的溶液。
本发明的装置通常地包含具有一个或多个阀的容器,治疗的组合物流行进(travel)穿过所述阀;以及用于控制所述流的致动器。用于本发明的合适装置可以见于例如雷明顿(Remington)(同上)中。适用于施用本发明组合物的装置包括吸入器(inhaler)和雾化器(nebulizer),诸如通常地用于将类固醇递送至哮喘患者的装置。在一些情况下,可以将间隔件与吸入器结合使用以帮助确保有效递送。
各种设计的吸入器可商购,并且可以用于递送本发明的组合物。这些吸入器包括Accuhaler、Aerohaler、Aerolizer、Airmax、Autohaler、Breezhaler、Clickhaler、Diskhaler、Easi-breathe吸入器、Easyhaler、Evohaler、Ellipta、Fisonair、Handihaler、Integra、喷射吸入器、Miat-haler、Nexthaler、Novolizer吸入器、Pulvinal吸入器、Respimat、Rotahaler、Spacehaler、Spinhaler、Syncroner吸入器和Turbohaler装置。产生特别理想的颗粒的许多配制技术是本领域中已知的并且可以使用。例如,可以使用纳米晶体、pulmosol和pulmosphere技术。
在一些情况下,组合物可以通过装置施用。在此类情况下,通常地所述组合物将为液体形式并且将通过人工气道诸如气管内导管施用。通常地将液体吸上来进入注射器中,然后通过人工气道排入受试者的呼吸道中。装置施用通常用于紧急情况下。在许多情况下,可以在受试者具有相对晚期的CAL形式并已入院的情况下使用它。
所述组合物可以包含各种成分以优化其对所选特定递送途径的适合性。使用药学上可接受的增稠剂可以将组合物的粘度维持在期望的水平。可以使用的增稠剂包括甲基纤维素、黄原胶、羧甲基纤维素、羟基丙基纤维素、卡波姆(carbomer)、聚乙烯醇、海藻酸盐、阿拉伯胶、壳聚糖及其组合。增稠剂的浓度将取决于所选药剂和期望的粘度。
在一些实施方式中,所述组合物可以包含湿润剂。此可以帮助减少或预防粘膜干燥并预防对膜的刺激。合适的湿润剂包括山梨糖醇、矿物油、植物油和甘油;舒缓剂;膜调节剂;甜味剂;及其组合。
所述组合物可以包含表面活性剂。合适的表面活性剂包括非离子、阴离子和阳离子表面活性剂。可以使用的表面活性剂的实例包括,例如,山梨糖醇酐的脂肪酸偏酯的聚氧乙烯衍生物,诸如Tween 80、聚氧乙烯40硬脂酸酯、聚氧乙烯50硬脂酸酯、梭链孢酸盐(fusieate)、胆汁盐和辛苯聚醇。
将在“实验”部分进一步证明根据本发明的组合物在吸入疗法中的协同作用,所述组合物包含含有铜化合物的活性剂和糖胺聚糖,分别以硫酸铜和肝素为例。
实验
本发明所基于的研究是遵循系统性方法的研究项目的一部分,其目的是为肺气肿患者建立特定疗法。
此项目的焦点在于肺细胞外基质大蛋白弹性蛋白和胶原蛋白,以及在弹性蛋白和胶原蛋白纤维的发育和修复过程中具有关键作用的其他蛋白:即,弹性蛋白原、腓骨蛋白-4、腓骨蛋白-5、“原型”LOX和LOXL1。
弹性蛋白交联的水平通过测量弹性蛋白特异性的交联氨基酸锁链素和异锁链素(统称为DES)来定量[3],并且胶原蛋白交联的水平通过测量胶原蛋白特异性的交联氨基酸羟脯氨酸来定量。
在申请人的研究项目中,按以下顺序进行实验:
1.气肿、特发性肺纤维化(IPF)、合并肺纤维化和气肿(CPFE)的患者以及无实质肺病的对照受试者的肺活检组织学检查。
2.用抗活性LOXL1和LOXL2抗体对肺活检进行染色。
3.来自肺气肿、IPF、CPFE患者以及无实质肺病的对照受试者的肺组织的基因表达研究。
4.测量气肿和IPF患者以及无肺病对照呼出的呼吸冷凝物中的铜水平。
5.测量气肿、IPF、CPFE患者以及无实质肺病的对照受试者肺组织中的铜水平。
6.用肺大鼠纤维母细胞进行细胞培养。
7.小鼠中猪胰蛋白酶诱导的气肿模型中的修复机制。
8.使用激光衍射分析方法分析肝素钠和硫酸铜溶液的雾化。
1.肺活检的组织学检查
基本原理:我们以检查来自气肿、IPF、CPFE患者以及对照受试者的细胞外基质开始此项目。还研究纤维化患者的肺的原因是我们期望阐明IPF和气肿的发病机制中所谓的“差异因子”可有助于确定负责气肿肺中弹性蛋白修复过程失败的缺陷,并促进针对气肿患者的特定疗法的确定。
方法:从气肿患者(n=10)和无COPD/气肿的健康对照(n=10)的手术肺切除标本中获得肺组织;获取距肿瘤适当距离的胸膜下区域中的无肿瘤肺组织。从IPF患者(n=10)的诊断性手术肺活检中获得肺组织。从外植的肺中获得CPFE患者尖部(气肿)和底部(纤维化)肺区的肺组织(n=4;图1)。我们首先通过组织学分析检查了肺细胞外基质:胶原蛋白为Masson三色染色剂,并且弹性蛋白为Verhoeff-Van Gieson染色剂。
结果:我们发现,肺气肿患者的肺实质中弹性蛋白含量减少,而IPF患者的弹性蛋白含量增加(图2)。与对照肺相比,气肿患者和IPF患者两者的胶原蛋白含量均增加;然而,我们观察到IPF患者中胶原纤维的增加更明显。在CPFE患者中,底部纤维化肺实质中的弹性蛋白含量增加,而尖部气肿肺实质中的弹性蛋白含量减少。CPFE患者的胶原蛋白含量在尖部气肿肺实质和底部纤维化肺实质两者中均增加,但在底部纤维化肺区更明显。气肿肺的DES水平减少,而IPF肺的DES水平增加。气肿肺和IPF肺两者中的羟脯氨酸水平均增加,但后者高得多。明显地,气肿肺与IPF肺之间的胶原蛋白水平的相对差异远低于气肿肺与IPF肺之间的羟脯氨酸水平的相对差异,这指示与IPF肺相比,气肿肺中的胶原蛋白交联程度较低。
结论:我们从对纤维化肺和气肿肺的分析得出结论,由于气肿肺中存在大量胶原蛋白,因此我们针对气肿患者的特定疗法不仅应刺激弹性蛋白纤维的修复和发育,还应抑制胶原蛋白的成熟、组织和积累。
2.活性LOXL1和活性LOXL2的肺活检染色
基本原理:LOX酶不仅负责将弹性蛋白原前体交联成持久的弹性蛋白纤维,而且LOX酶还将胶原蛋白前体交联成持久的胶原蛋白纤维。弹性蛋白纤维提供弹性、弹力和变形性,而胶原蛋白纤维为肺提供拉伸强度。过多的胶原蛋白沉积是肺纤维化的标志。刺激肺纤维化的形成是LOX刺激的不良副作用。我们假设LOX酶在气肿中会减少而在纤维化中会增加。
方法:我们用活性LOXL1(诺伟司生物公司(Novus Biologicals);NBP1-82827)和活性LOXL2(诺伟司生物公司;NBP1-32954)抗体对与用于组织学分析相同的肺活检进行了染色。
结果:与对照受试者相比,IPF患者的活性LOXL1和活性LOXL2染色两者强度均增强,而气肿患者中两者的强度降低。在CPFE患者中,底部纤维化肺实质中的活性LOXL1和活性LOXL2染色强度增强,而尖部气肿肺实质中的强度降低。
3.肺组织中的基因表达分析
基本原理:我们通过在气肿、IPF和CPFE患者的肺中与对照的肺相比进行基因表达(定量实时聚合酶链反应;qRT-PCR)分析来进行系统性研究项目,以鉴别那些在气肿中上调不足且应进行刺激以实现有效弹性蛋白修复的弹性蛋白修复基因/蛋白。
方法:我们分析了上文提及的肺活检中以下基因的表达:弹性蛋白原(ELN)、LOX、LOXL1、LOXL2、腓骨蛋白-4和腓骨蛋白-5。
结果:出乎我们意料之外,我们发现气肿和IPF患者两者中的ELN和腓骨蛋白-5均大大上调,表明这些蛋白不是肺气肿和纤维化之间的“差异因子”。与对照相比,IPF患者中的LOXL1上调。气肿患者与对照受试者之间的LOXL1基因表达没有显著差异。
结论:我们从基因表达研究得出结论,刺激ELN和腓骨蛋白-5合成可以不是肺弹性蛋白修复疗法的必要靶标,因为这些蛋白在气肿患者的肺中已经上调。
中期分析
我们面临一个矛盾,即气肿肺中活化的LOXL1水平减少;然而,在qRT-PCR中LOXL1的表达没有减少。基于对我们系统性研究项目结果的中期分析,我们得出结论,患者不会因为蛋白LOXL1水平的减少而产生气肿,并且假设患者可因为LOXL1的必要辅因子(即铜)水平的减少而产生气肿。我们进行了若干个研究来检验此假设。
4.呼出的呼吸冷凝物和血清中的铜
基本原理:由于铜是活化LOX酶的必要辅因子,所以我们假设气肿患者中的铜浓度会减少。
方法:首先,我们测量了气肿患者(n=10)和对照(n=10)的血清中铜的水平。随后,我们通过RTubeTM(呼吸研究公司(Respiratory Research);www.repiratoryresearch.com)收集了呼出的呼吸冷凝物(EBC),并测量了铜水平。
结果:与我们的假设相反,气肿患者中的血清铜水平并未减少,而是增加了(图3)。然而,与对照相比,气肿患者中的EBC铜浓度减少了。
结论:气肿中存在局部肺铜缺乏且不存在全身铜缺乏。
5.肺活检中的铜浓度
基本原理:由于铜是活化LOX酶的必要辅因子,所以我们假设气肿患者中的铜浓度会减少并且IPF患者中会增加。
方法:我们测量了气肿、IPF和CPFE患者肺实质中的铜浓度。
结果:我们发现,与对照肺相比,气肿中铜浓度确实减少了,而纤维化中铜浓度确实增加了(图4)。我们还发现CPFE患者的肺中尖部气肿(低铜水平)与底部纤维化(高铜水平)实质之间的铜浓度存在大的梯度。对于CPFE肺中铜浓度的这些令人惊讶的差异,我们的解释是,铜向上肺区的递送远低于向下肺区的递送,鉴于尖部肺区的灌注非常差,这似乎是合乎逻辑的。
结论:与全身施用途径相比,铜吸入疗法是优选的,(a)因为肺尖部的通气远优于灌注,以及(b)因为存在局部铜缺乏并且无全身性铜缺乏。与口服施用相比,优选吸入性铜疗法还有第三个重要原因。血清铜水平与产生阿尔茨海默氏病(Alzheimer's disease)的风险呈正比[38]。为了在肺中实现相同的铜浓度(尤其在尖部肺区),比起口服疗法,吸入疗法需要更低剂量的铜。我们通过气管内向小鼠施用铜,并且实际上发现这种干预对脑铜浓度没有作用。
IPF患者的肺实质和CPFE患者的纤维化底部肺区中的高铜浓度构成了我们的理解的基础,即通过铜吸入疗法刺激LOX酶的活化会刺激胶原蛋白交联,从而刺激胶原蛋白的成熟/组织,因为LOX酶不仅是弹性蛋白的交联剂,而且还是胶原蛋白的交联剂[39]。气肿肺中铜诱导的胶原蛋白的积累将是有害的,因为(a)气肿肺中胶原蛋白水平已经增加,并且(b)其可以导致从气肿转变为纤维化(即,一种破坏性肺病向另一种破坏性肺病转变)。
因此,我们得出结论,在我们的吸入配制品中应将铜与一种或多种其他成分组合,以预防铜诱导的胶原蛋白积累。
6a.具有额外铜的纤维母细胞培养物
基本原理:基于铜缺乏是气肿患者弹性蛋白修复过程无效的最可能原因,我们假设补充铜会通过活化更多的LOX酶来刺激弹性蛋白的发育/修复过程。
方法:使纤维母细胞生长21天,之后将其裂解并提取mRNA。培养基每周补充两次。进行qPCR以测量LOX、LOXL1和弹性蛋白(编码弹性蛋白原的ELN)基因的表达。使用AmpliteFluorimetrix LOX测定试剂盒(美国加利福尼亚州桑尼维尔的AAT Bioquest公司(AATBioquest,Sunnyvale,CA,USA))测量LOX活性。用FastinTM弹性蛋白测定试剂盒(英国的Biocolor公司(Biocolor,UK))测量沉积在细胞层中的总不溶性弹性蛋白和可溶的弹性蛋白原。如前所述,在Canisius-Wilhelmina医院(荷兰奈梅亨(Nijmegen,TheNetherlands)),使用液相色谱-串联质谱法测量了DES水平[9]。使用SircolTM不溶性胶原蛋白测定法(英国的Biocolor公司)对培养基和基质中的胶原蛋白进行定量。我们首先测量了纤维母细胞培养基中的铜水平。随后,我们以递增的浓度添加了额外的硫酸铜,即+0.5*纤维母细胞培养基中的初始铜浓度、+1*初始铜浓度、+2*初始铜浓度、+4*初始铜浓度、+8*初始铜浓度、+16*初始铜浓度和+32*初始铜浓度,以获得铜浓度与其他变量之间的剂量-反应。
结果:硫酸铜以剂量依赖性方式增加了LOX和LOXL1基因表达、LOX活性、DES水平(均有利;图5)以及不溶性胶原蛋白水平(不利)。硫酸铜对ELN基因表达没有任何作用。
结论:向细胞培养基中添加额外硫酸铜对交联的弹性蛋白纤维的积累具有有利的刺激作用,但是,其还对不溶性胶原蛋白水平的积累具有不利的刺激作用。关于DES水平的剂量反应曲线,在铜浓度为约+8*纤维母细胞培养基中的初始铜浓度时达到最高(图5)。

胞培养物
基本原理:在细胞培养研究的第二部分中,我们评定了向硫酸铜中添加其他物质是否会进一步刺激弹性蛋白的发育/修复过程。
方法:我们向富含铜的纤维母细胞培养基(铜浓度为+8*纤维母细胞培养基中的初始铜浓度)中添加了视黄酸、米诺地尔和肝素。
结果(图6):与硫酸铜单一疗法相反,向硫酸铜中添加视黄酸对ELN基因表达和弹性蛋白原水平具有刺激作用。将视黄酸添加到硫酸铜中还对不溶性弹性蛋白水平具有额外的刺激作用;但是,视黄酸对DES水平没有额外作用。相较于硫酸铜单一疗法,视黄酸对LOX和LOXL1基因表达没有额外作用。向硫酸铜中添加米诺地尔对LOX、LOXL1、ELN和腓骨蛋白-5基因表达具有刺激作用。与硫酸铜单一疗法相比,向硫酸铜中添加米诺地尔对弹性蛋白原、不溶性弹性蛋白和DES水平具有额外的刺激作用。与硫酸铜单一疗法相比,向硫酸铜中添加米诺地尔对胶原蛋白的积累没有额外的刺激作用;但是,添加米诺地尔对胶原蛋白积累也无遏制作用。与硫酸铜单一疗法相比,向硫酸铜中添加肝素对LOX、LOXL1、ELN和腓骨蛋白-5基因表达均没有额外作用;并且对弹性蛋白原水平没有作用。与硫酸铜单一疗法相比,向硫酸铜中添加肝素对总不溶性弹性蛋白水平具有很小的刺激作用;但是,其对DES水平没有额外作用。更重要且令人惊讶地,与硫酸铜单一疗法相比,向硫酸铜中添加肝素对胶原蛋白积累具有强遏制作用。
结论:与硫酸铜单一疗法相比,添加视黄酸、米诺地尔和肝素对弹性蛋白的发育和修复过程具有一些额外作用。令人惊讶地,向硫酸铜中添加肝素对胶原蛋白水平具有强抑制作用。我们从此研究得出结论,肝素似乎是用于治疗气肿患者以预防铜诱导的胶原蛋白积累的铜的理想辅助物。
6c.用于测试在硫酸铜之上维生素K和硫酸镁的潜在协同作用的纤维母细胞培养

结果:向硫酸铜中添加维生素K1、K2和硫酸镁对ELN基因表达、弹性蛋白原水平或对LOX和LOXL1基因表达没有刺激作用。但是,向硫酸铜中添加维生素K1、K2和硫酸镁对不溶性弹性蛋白水平和DES积累具有额外的刺激作用(图7)。与硫酸铜单一疗法相比,向硫酸铜中添加维生素K1、维生素K2和硫酸镁对胶原蛋白的积累没有额外的刺激作用;但是,添加维生素K1、维生素K2和硫酸镁对胶原蛋白积累也无遏制作用。
结论:与硫酸铜单一疗法相比,添加维生素K1、维生素K2和硫酸镁对弹性蛋白和DES积累具有额外作用。此作用的最可能的机制原因是维生素K1、维生素K2和硫酸镁对弹性蛋白降解的抑制作用,因为维生素K1、维生素K2和硫酸镁对弹性蛋白的发育过程没有任何作用。我们从此研究得出结论,维生素K1、维生素K2和硫酸镁似乎是用于治疗气肿患者以抑制弹性蛋白降解速率的铜的有用辅助物。
7.通过气管内施用猪胰弹性蛋白酶诱导的气肿
基本原理:基于细胞培养研究中向硫酸铜中添加肝素对弹性蛋白和胶原蛋白代谢两者的非常有前景的作用,我们在气肿动物模型中进一步评定了这些作用。
方法:为了评定硫酸铜加肝素在体内对弹性蛋白和胶原代谢两者的作用,我们使用了猪胰弹性蛋白酶(PPE)诱导的气肿模型。在7周龄,起始体重约为25g的雄性BALB/c小鼠中进行了研究。在研究期间,将所有小鼠都安置在滤盖笼中进行12/12h明暗循环的常规动物舍中,并随意提供颗粒状食物和水。在第1天在轻度麻醉下气管内施用25μL含1.5U猪胰弹性蛋白酶的盐水。在第1、8、15、22和29天在轻度麻醉下气管内施用25μL硫酸铜单一疗法(12.5μg,在25μL盐水中;n=4)、硫酸铜(12.5μg,在12.5μL盐水中)/肝素(1,000IU,在12.5μL盐水中;n=4)的组合或安慰剂(25μL盐水;n=4)。在第35天,用甲苯噻嗪(8.5mg/kg)和氯胺酮(130mg/kg)的混合物腹膜内麻醉小鼠,将其气管切开并置于全身体积描记器中以评定肺功能。在测量肺功能后,通过心内施用戊巴比妥(pentobarbital)使小鼠安乐死。将左肺在液氮中速冻,并储存在-80℃下以用于测量ELN、LOX和LOXL1的后续基因表达研究。将右肺在25cm流体柱的恒定静水压力下在6%的多聚甲醛中固定24h。脱水并包埋在石蜡中后,用各种染色剂对矢状切面染色并用于组织学分析以测量空域扩大(平均线性截距);随后,将此肺用于测量DES和不溶性胶原蛋白两者的浓度。取出脑以测量铜浓度。
结果:安慰剂组小鼠的肺功能测试中的过度充气现象比硫酸铜和硫酸铜/肝素组多(图8)。接受硫酸铜和硫酸铜/肝素的小鼠肺组织中的DES水平高于接受安慰剂的小鼠(图9)。接受硫酸铜和硫酸铜/肝素的小鼠平均线性截距低于接受安慰剂的小鼠(图8、10和11)。与安慰剂相比,接受硫酸铜单一疗法的小鼠的不溶性胶原蛋白和羟脯氨酸水平升高(图9)。与接受硫酸铜单一疗法的小鼠相比,接受硫酸铜/肝素的小鼠的不溶性胶原蛋白和羟脯氨酸水平显著降低,而与接受安慰剂的小鼠相比则低一些。肝素是众所周知的抗凝剂;但是,气管内施用肝素对小鼠的全身凝结没有任何作用。
结论:我们发现硫酸铜非常有效地刺激了弹性蛋白的修复过程,而且还诱导了肺中胶原蛋白纤维的积累和成熟。硫酸铜加肝素的组合非常有效地促进了损伤的弹性蛋白纤维的修复(甚至比硫酸铜单一疗法更好),并且我们发现,与硫酸铜单药相反,硫酸铜加肝素并没有引起胶原蛋白的积累。硫酸铜/肝素处理后的胶原蛋白和羟脯氨酸水平甚至低于安慰剂处理后。因此,吸入性肝素是作为含铜吸入配制品的佐剂以预防铜诱导的胶原蛋白积累并刺激弹性蛋白修复过程的理想化合物。
8.使用激光衍射分析方法分析肝素钠和硫酸铜溶液的雾化。
基本原理:有必要知道使用常用雾化器系统雾化由肝素和铜两者组成的溶液是否可行,以及是否此产生适当百分比的<5μm的颗粒。
方法1:我们用相对低浓度的铜和肝素开始雾化实验。将26mg肝素钠(191IU/mg)溶于1mL 0.9%氯化钠中,并将12.5mg硫酸铜(5mg铜)溶于10mL 0.9%氯化钠中,其中使用1mL。将3mL 0.9%的氯化钠添加到1mL肝素钠(5,000IU)溶液和1mL硫酸铜(0.5mg铜)溶液中。将5mL雾化溶液装入可重复使用的雾化器(PARIPlus)中,并用压缩机((PARISX)雾化。使用激光衍射分析(LDA)每30秒分析一次气雾剂,直到雾化器开始喷溅。
结果1:雾化时间为约3分钟。X10是0.81μm,X50是2.34μm并且X90是6.58μm。<5μm的颗粒的百分比是82.44%(图12)。重复进行实验1的测量:雾化时间约为3分钟,X10是0.80μm,X50是2.29μm,X90是6.34μm,并且<5μm的颗粒的百分比为83.58%(图13)。
方法2:将100,000IU硫酸肝素和1mg铜组合在溶液中,并添加0.9%氯化钠,总体积为5mL。
结果2:雾化时间为约4分钟。X10是0.80μm,X50是2.32μm并且X90是6.86μm(图14)。<5μm的颗粒的百分比为82.13%。重复进行实验2的测量:雾化时间约为5分钟,X10是0.80μm,X50是2.33μm,X90是7.05μm,并且<5μm的颗粒的百分比为81.19%(图15)。
结论:将肝素和铜组合在雾化配制品中是可行的,并且使用常用雾化器系统产生了高百分比的<5μm的颗粒,这些颗粒会有效地到达人肺的靶标肺泡区。

Claims (16)

12.根据前述权利要求中任一项所述的组合物,其中所述组合物还包含至少一种对脉管系统中的弹性蛋白代谢起作用的药物或物质,所述药物或物质选自多酚表没食子儿茶素-(3-)没食子酸酯(EGCG)和五没食子酰葡萄糖(PGG);ATP依赖性钾通道开放剂,例如米诺地尔、尼可地尔、二氮嗪、吡那地尔和克罗卡林;镁;维生素K1;维生素K2;动脉中的AGE的断裂剂,例如氨基胍、吡哆胺、N-苯甲酰甲基噻唑鎓溴化物、阿拉格布和类黄酮,例如,山奈酚、染料木素、槲皮苷、槲皮素和表儿茶素;和/或至少一种对肺中的弹性蛋白代谢有潜在作用的化合物,其选自维生素A、维生素D和五没食子酰葡萄糖。
13.根据前述权利要求中任一项所述的组合物,其中所述组合物通过吸入和/或通过滴注施用。
14.根据前述权利要求中任一项所述的组合物,其中所述组合物:
(a)用于再活化受试者中的肺弹性蛋白纤维产生;
(b)修复损伤的弹性蛋白纤维;
(c)减缓弹性蛋白降解的速率;和/或
(d)抑制晚期糖基化终末产物(AGE)。
15.根据前述权利要求中任一项所述的组合物,其中所述组合物用作标准药理学COPD治疗的添加剂,所述标准药理学COPD治疗包括支气管扩张剂和免疫调节剂,包括吸入性皮质类固醇和口服大环内酯。
16.一种治疗患有肺气肿或另一种形式的COPD的受试者的方法,所述方法包含向所述受试者施用治疗有效量的组合物,所述组合物包含含有铜化合物的活性剂和糖胺聚糖或其生理学上可接受的盐。
