背景技术
用抗原特异性T细胞进行的免疫疗法在临床前模型以及I期和II期临床研究中在恶性肿瘤的治疗方面显示出前景。产生肿瘤特异性T细胞的一种有吸引力的策略是通过用嵌合抗原受体(CAR)进行遗传修饰,该嵌合抗原受体包含含有抗原识别部分的胞外结构域,跨膜结构域和至少一个胞内信号传导结构域(例如源自T细胞受体CD3-ζ,常常与共刺激分子胞内域连接的胞内信号传导结构域)。

发明内容

本文所用的术语“载体”是指由单链、双链、环状或超螺旋DNA或RNA组成的多核苷酸。典型的载体可以由以下以适当距离可操作地连接以允许功能基因表达的元件组成:复制起点、启动子、增强子、5'mRNA前导序列、核糖体结合位点、核酸盒、终止和多聚腺苷酸化位点以及选择标记序列。在特定应用中可以省略这些元件中的一个或多个。载体还可含有核酸盒,其可包括用于插入待表达的核酸序列的限制性位点。在功能载体中,核酸盒含有待表达的核酸序列,包括翻译起始和终止位点。

本发明的药物组合物可以单独使用或与其他得到确认的用于治疗癌症的药剂组合使用。无论是单独递送还是与其他药剂组合递送,本发明的药物组合物都可以通过各种途径递送至哺乳动物,特别是人体的不同部位,以实现特定效果。本领域技术人员将认识到,虽然可以使用多于一种途径进行施用,但是特定途径可以提供比另一种途径更直接且更有效的反应。例如,皮内递送可以比吸入有利地用于治疗黑素瘤。局部或全身递送可以通过施用来实现,施用包括将制剂施加或滴注到体腔中,吸入或吹入气溶胶,或通过肠胃外引入来实现,肠胃外引入包括肌内、静脉内、门静脉内、肝内、腹膜、皮下或皮内施用。
本发明的组合物可以以单位剂型提供,其中每个剂量单位,例如注射剂,含有预定量的单独的或与其它活性剂适当组合的组合物。如本文所用的术语单位剂型是指适合作为人和动物受试者的单位剂量的物理离散单位,每个单位含有预定量的单独的或与其它活性剂组合的本发明组合物,以量计算在适当的情况下,与药学上可接受的稀释剂、载体或媒介物相结合,足以产生所需的效果。本发明的新型单位剂型的规格取决于与特定受试者中的药物组合物相关的特定药效学。
优选地,将治疗有效量或足够数量的单独存在或作为组合物的一部分存在的表达本发明的CTX-CAR的宿主细胞引入受试者体内,使得建立长期、特异性反应。在一个实施方案中,该反应包括癌症的抑制。在一个实施方案中,该反应是肿瘤尺寸的减小或肿瘤生长或再生的消除或转移的减少,达到高于不存在用本公开的CTX-CAR进行此类治疗所产生的程度。理想地,与其中不存在表达本公开的CTX-CAR的宿主细胞的其他相同条件相比,向受试者中引入所述量的表达本公开的CTX-CAR的宿主细胞引起肿瘤尺寸减小10%、20%、30%、40%、50%、60%、70%、80%、90%、95%、98%或100%。
因此,施用的表达本公开的CTX-CAR的宿主细胞的量应考虑施用途径,并且应当使得将会引入足够数量的表达本发明的CTX-CAR的宿主细胞,以达到所需治疗反应。此外,表达本公开的CTX-CAR的每种宿主细胞或本文所述组合物中所包括的其他细胞的量(例如,待接触的每种细胞的量或每特定体重的量)可以在不同的应用中不同。一般而言,表达本公开的CTX-CAR的宿主细胞的浓度理想地应足以在治疗的受试者中提供至少约1x105至约1x1010个宿主细胞,甚至更理想地,约1x107至约5x108个宿主细胞,但可以利用任何合适的量,高于例如,大于5x108个细胞或低于,例如小于1x107个细胞。施用方案可以基于得到确认的基于细胞的疗法,或者可以采用替代的连续输注策略。
这些值提供了表达本公开的CTX-CAR的宿主细胞的范围的一般指导,供本领域技术人员在优化本发明的方法时用于本发明的实践。本文对此类范围的叙述绝不排除使用更高或更低量的组分,这在特定应用中可能是必要的。例如,实际剂量和方案可以根据组合物是否与其他药物组合物一起施用,或根据药代动力学、药物处置和代谢的个体间差异而变化。本领域技术人员可以容易地根据特定情形的紧急情况进行任何必要的调整。
为产生治疗效果的合适剂量将是每剂量约105至约1010个宿主细胞,优选地在一系列给药周期内。一种优选的施用方案包括四个为期一周的递增剂量的给药周期,从第0天以约105个细胞开始,到第5天逐渐增加到高达约1010个细胞的目标剂量。合适的施用模式包括静脉内、皮下、腔内(例如通过储库进入装置)、腹膜内和直接注射到肿瘤块中。
本发明还提供了抑制癌症生长的方法,其包括使癌细胞与表达本发明的CTX-CAR的宿主细胞接触。本发明还提供了治疗患者的癌症的方法,其包括向患者施用治疗有效量的表达本公开的CTX-CAR的宿主细胞或本发明的组合物,例如包含此类宿主细胞的药物组合物。优选地,待治疗的癌症是神经外胚层来源的。优选地,待治疗的癌症是恶性神经胶质瘤、黑素瘤、神经母细胞瘤、成神经管细胞瘤或小细胞肺癌。
本发明包括细胞疗法,其中宿主细胞(例如,免疫效应细胞,例如T细胞、NK细胞和优选γδ-T细胞)经遗传修饰以表达CTX-CAR和任选如上所述的用于实现DRI的基因,其中将此类宿主细胞输注给有需要的受者。输注的细胞能够杀伤受者中的肿瘤细胞。与抗体疗法不同,表达CTX-CAR的宿主细胞能够在体内复制,导致长期持久性,这可以导致持续的肿瘤控制。
本发明还包括细胞疗法,其中宿主细胞(例如,免疫效应细胞,例如T细胞、NK细胞和优选γδ-T细胞)经修饰以瞬时表达本发明的CTX-CAR和任选用于实现DRI的基因,其中将此类宿主细胞输注给有需要的受者。输注的细胞能够杀伤受者中的肿瘤细胞。因此,在各个方面,在将免疫效应细胞施用给患者后,施用给患者的免疫效应细胞(例如,免疫效应细胞,例如但不限于T细胞、NK细胞和优选γδ-T细胞)存在不到一个月,例如三周、两周、一周。
在用于细胞疗法的此类方法的各个方面,在将宿主细胞施用给患者后,施用给患者的宿主细胞或其后代在患者中持续存在至少四个月、五个月、六个月、七个月、八个月、九个月、十个月、十一个月、十二个月、十三个月、十四个月、十五个月、十六个月、十七个月、十八个月、十九个月、二十个月、二十一个月、二十二个月、二十三个月、两年、三年、四年或五年。
在一个方面,本发明的表达本公开的CTX-CAR的全人宿主细胞(例如,免疫效应细胞,例如T细胞、NK细胞和优选γδ-T细胞)可以是用于哺乳动物离体免疫和/或体内治疗的疫苗。一方面,哺乳动物是人。关于离体免疫,在将宿主细胞或包含宿主细胞的组合物(包括药物组合物)施用给哺乳动物之前,体外进行以下至少一项:i)扩增宿主细胞,ii)向宿主细胞引入编码CTX-CAR的核酸和/或iii)冷冻保存表达或能够表达CTX-CAR的宿主细胞。离体程序是本领域熟知的。简言之,从患者(例如,人)分离细胞并进行遗传修饰以表达本公开的CTX-CAR(即,用表达本文公开的CTX-CAR的载体在体外转导或转染)。可以将CTX-CAR修饰的宿主细胞施用给患者以提供治疗益处。患者优选为人,并且CTX-CAR修饰的宿主细胞相对于患者可以是自体的。或者,宿主细胞相对于患者可以是同种异体的、同基因的或异种的。
表达本公开的CTX-CAR的宿主细胞可以与其他已知的药剂和疗法组合使用。在一些实施方案中,一种治疗的递送在开始第二治疗的递送时仍在进行,使得在施用方面存在重叠。这有时在本文中称为“同时”或“并行递送”。优选地,一种治疗的递送在另一种治疗的递送开始之前结束。在任何一情形下,由于组合施用,治疗更有效。例如,第二治疗更有效,例如,相比于在没有第一治疗时施用第二治疗所见或用第一治疗所看到的类似情况,用更少的第二治疗看到同等效果,或第二治疗减少在更大程度上减轻症状。优选地,递送使得症状或与病症相关的其他参数的减小大于在不存在另一种治疗时递送一种治疗所观察到的减小。两种治疗的效果可以是部分加和的,完全加和的或大于加和的。递送可以使得当递送第二治疗时仍然可以检测到递送的第一治疗的效果。
表达本公开的CTX-CAR的宿主细胞和至少一种附加治疗剂可以同时施用,在相同或单独的组合物中施用或依次施用。对于依次施用,表达本公开的CTX-CAR的宿主细胞可以第一施用,附加药剂可以第二施用,或者可以颠倒施用顺序。
CTX-CAR疗法、DRI-CTX-CAR疗法和/或其他治疗剂、程序或方式可以在活跃性病症期间或在缓解期或较不活跃的疾病期间施用。CTX-CAR疗法或DRI-CTX-CAR疗法可以在其他治疗之前,与其他治疗同时,在其他治疗之后(治疗后)或在病症缓解期间施用。
当组合施用时,CTX-CAR疗法或DRI-CTX-CAR疗法和附加药剂(例如,第二或第三药剂),前述一种或所有的量或剂量,可以以比单独使用(例如作为单一疗法)的每种药剂的量或剂量更高、更低或相同的量或剂量施用。在CTX-CAR疗法、DRI-CTX-CAR疗法、其他药剂(例如,第二药剂或第三药剂)的某些实施方案中,前述一种或全部的量或剂量比单独使用(例如作为单一疗法)的每种药剂的量或剂量低(例如,至少20%、至少30%、至少40%或至少50%)。在CTX-CAR疗法、DRI-CTX-CAR疗法、附加药剂(例如,第二或第三药剂)的其他实施方案中,前述一种或所有药剂产生所需效果(例如,癌症抑制)的量或剂量,比单独使用(例如作为单一疗法)的每种药剂达到相同治疗效果所需的量或剂量低(例如,至少20%、至少30%、至少40%或至少50%)。
优选地,表达本公开的CTX-CAR的宿主细胞可以与附加治疗性治疗组合使用,例如但不限于手术、化疗、检查点抑制剂、放射、免疫抑制剂(例如环孢菌素、硫唑嘌呤、甲氨蝶呤、霉酚酸酯和FK506)、抗体或其他免疫消融剂(例如CAMPATH、抗CD3抗体或其他抗体疗法)、细胞毒素、氟达拉滨、FK506、雷帕霉素、霉酚酸、类固醇和细胞因子。在某些实施方案中,表达本公开的CTX-CAR的宿主细胞经进一步修饰为如本文所述的DR并用于DRI。
本文描述的任何组合物均可包含在试剂盒中。在非限制性实例中,嵌合受体表达构建体,一种或多种产生嵌合受体表达构建体的试剂,用于表达构建体转染的细胞,和/或一种或多种获得用于表达构建体转染的自体细胞的仪器(此类仪器可以是注射器、移液管、镊子和/或任何此类医学上认可的装置)。试剂盒可包含一种或多种适当等分的本发明组合物或用于产生本发明组合物的试剂。试剂盒的组分可以在水性介质中或呈冻干形式包装。试剂盒的容器装置可包括至少一个小瓶、试管、烧瓶、瓶子、注射器或其他容器装置,其中可将组分放入其中并优选适当地等分。在试剂盒中存在多于一种组分的情况下,试剂盒通常还会含有第二、第三或其他附加容器,其中可以将附加组分单独放入其中。然而,小瓶中也可以包含组分的各种组合。本发明的试剂盒通常还将包括用于容纳严格限制用于商业销售的嵌合受体构建体和任何其他试剂容器的装置。此类容器可包括例如,将所需小瓶保留在其中的注射或吹塑塑料容器。
实施例
以下实施例是以说明的方式提供的,不应解释为以任何方式限制本发明。
实施例1-产生CTX-CAR和noCTX-CAR的方法
为了工程化表达目标CAR的T细胞,使用G-block技术(IDT)合成CTX-CAR和noCTX001-CAR基因。简言之,为合成CTX-CAR,使用公开可用的在线软件将氯毒素的氨基酸序列(蝎毒;PRF:445665)MCMPCFTTDHQMARKCDDCCGGKGRGKCYGPQCLCR(SEQ ID NO:1)转化为核苷酸并且使用集成DNA核苷酸技术(IDT)在线密码子优化工具进行密码子优化,所述工具具有序列ATGTGTATGCCTTGCTTTACGACCGATCATCAGATGGCTAGAAAGTGTGATGACTGTTGTGGAGGCAAGGGACGAGGGAAATGCTATGGACCTCAATGTTTGTGTCGC(SEQ ID NO:4)。
使用G-block技术(IDT)合成优化的核苷酸序列,与5'处的CD8-α前导序列和接头序列SGGG x3、包含IgD铰链和IgG1CH2和CH3结构域的胞外结构域、来自CD28的跨膜结构域和3'处包含CD28/Ox40/CD3ζ的胞内信号传导结构域框内克隆。使用输注技术(Clontech)将G-block克隆到编码CD8α-IgD铰链、CH2CH3(IgGH1)和TM结构域、CD28/OX40/CD3ζ的SFG载体中,并使用Sph1和Not1限制性核酸酶(NEB)线性化。noCTX-CAR使用相同分子骨架,但包括不相关的肽(SEQ ID NO:5)而不是CTX(SEQ ID NO:1),并且被设计用作功能实验中的阴性对照。图1显示了CTX-CAR(上图)和noCTX-CAR(下图)构建体的设计。
然后使用In-Fusion试剂盒(Clontech)将两种基因克隆到SFG逆转录病毒载体的SphI和NotI位点。用连接混合物转化细菌感受态细胞(Stellar,Clontech)并接种在琼脂上。挑选几个细菌克隆物以制备质粒小量制剂(minipreps)(Qiagen),对其进行测试以选择正确的质粒构建体。基于用限制酶AfeI、XhoI、NcoI和NotI切割的质粒DNA的限制性模式选择正确的克隆物;在琼脂糖凝胶上测定DNA片段的大小。
实施例2-产生表达CTX-CAR和noCTX-CAR的T细胞和Jurkat细胞。
CTX-CAR和noCTX-CAR载体用于与gag-pol和env(RDF)质粒一起转染293T细胞以获得病毒上清液。施加所得病毒以转导Jurkat E1-6T细胞系,以便使用CH2CH3抗体通过流式细胞术测试两种CAR的表达(检测CAR分子的IgD铰链)。随后,用编码两种CAR的逆转录病毒载体进行T细胞转导。
简言之,将293T细胞用a)CTX-CAR或b)noCTX-CAR质粒以及env(RDF114)和gag-pol(PEG-PAM)一起共转染以获得病毒上清液。72小时后收集293T并用CH2CH3抗体染色(表示CAR表达)。图4中的无阴影部分显示用CH2CH3抗体染色的未转染的293T细胞,阴影部分显示CTX-CAR转染的293T细胞(左图)和noCTX-CAR转染的293T细胞(右图)。结果显示293-T细胞经两种质粒构建体有效转导。
用CTX-CAR和noCTX-CAR病毒转导Jurkat和γδT细胞扩增。72小时后收集细胞并用CH2CH3抗体染色(以表示CAR表达)。用编码CTX-CAR和noCTX-CAR的病毒转导的Jurkat T细胞在细胞表面上表达两种转基因,如该实验所示,其中显示了目标基因的表达。用所得的逆转录病毒有效转导Gamma delta T细胞,并确认两种CAR的表达(图5)。
实施例3-表达CTX-CAR的Jurkat细胞选择性结合靶神经胶质瘤细胞
该实施例显示表达本公开的CTX-CAR的效应细胞选择性结合神经胶质瘤细胞。使用神经胶质瘤细胞系U87、U251和LN229以及使用原代人星形胶质细胞(ScienCellResearch Labs)(HA)和原代αβT-细胞(T LC)测试表达本公开的CTX-CAR或noCTX-CAR的效应细胞的结合特性。用编码图1所示和如实施例1中描述的CTX-CAR或noCTX-CAR构建体的逆转录病毒,使用实施例2中描述的方案转导Jurkat细胞。
使神经胶质瘤细胞系和原代细胞系在标准条件下生长。用胰蛋白酶消化贴壁靶细胞并用绿色荧光染料钙黄绿素标记。表达本公开的CTX-CAR(CTX-CAR)或noCTX-CAR(noCTX-CAR)的Jurkat细胞用别藻蓝蛋白(APC)标记的抗CH2CH3抗体(其结合CTX-CAR和noCTX-CAR构建体中的铰链区)染色。染色后,洗涤所有标记的细胞,计数并以1:1的比率在200μl PBS+1%BSA中按总共500,000个细胞/份样品混合。将细胞混合物在温和搅拌下在室温下孵育1小时。温和洗涤后,将细胞混合物样品进行流式细胞术分析。通过对双重染色的FITC-APC群体进行门控来确定对应于效应子-靶标聚集物的双重标记细胞群的百分比。
图7显示表达本公开的CTX-CAR的效应细胞优先结合神经胶质瘤细胞系,与原代人星形胶质细胞或原代人T细胞的结合最少。与表达noCTX-CAR构建体的效应细胞相比,表达本公开的CTX-CAR的效应细胞的结合显示与神经胶质瘤细胞的结合增加。表达CTX-CAR构建体和no-CTX-CAR构建体的效应细胞的结合在原代人星形胶质细胞和原代人T细胞中是等同的。
结果证明,表达本公开的CTX-CAR的效应细胞优先结合人神经胶质瘤细胞,与所测试的非神经胶质瘤细胞系的结合最少。
实施例4-表达CTX-CAR的T淋巴细胞的细胞毒性。
该实施例显示表达本公开的CTX-CAR的效应细胞对神经胶质瘤细胞具有细胞毒性。使用Bright-Glo荧光素酶细胞毒性测定试剂盒(Promega)测量细胞毒性。用表达eGFP和萤火虫荧光素酶(GL)的融合蛋白的慢病毒感染靶神经胶质瘤细胞系U251和U87,以使细胞通过荧光可视化并通过生物发光成像(称为U251GL和U87GL)定量。
使用CD3/CD28珠粒(用于T细胞的活化和扩增)活化从全血中分离的PBMC一天,并用编码图1所示和如实施例1中所述的CTX-CAR(CTX)或noCTX-CAR(noCTX)构建体的逆转录病毒转导。转导后第4天评价CAR表达的效率,范围为40%-60%。将靶神经胶质瘤细胞系U251GL和U87GL按20,000或50,000个细胞/孔接种到96孔板中的100μl完全培养基中。将表达CAR的T细胞添加到孔中的100ml培养基中,靶细胞与效应细胞(T:E)比率为1:2、1:5、1:10、1:20,并在37℃下与靶细胞一起孵育过夜。作为对照,将靶细胞系与模拟转导的活化PBMC(Mock)和Jurkat细胞(Jurkat)一起孵育。孵育后,将96孔板短暂离心,弃去150μl培养基。然后按照制造商的说明添加Bright-Glo试剂盒(用于裂解细胞并提供荧光素酶底物)。孵育2分钟后,将细胞裂解物转移到黑色平底96孔板的孔中,用H1Hybrid发光计(Biotek)测量荧光。
图9A显示与用缺乏CTX的相同CAR构建体转导的效应细胞相比,用本公开的CTX-CAR转导的效应细胞对U251GL神经胶质瘤细胞具有增强的细胞毒性。在较低T:E比率下,增强的细胞毒性得到增强。图9B中示出了类似结果,说明用本公开的CTX-CAR转导的效应细胞对U87GL神经胶质瘤细胞的细胞毒性增强。
结果证明,表达本公开的CTX-CAR的效应细胞对人神经胶质瘤细胞具有细胞毒性。

1.一种包含至少一种载体的γδT细胞,所述至少一种载体指导嵌合抗原受体(CAR)和存活因子的表达,其中所述CAR由为氯毒素的胞外抗原识别结构域,与跨膜结构域连接的胞外间隔区、至少一个胞内信号传导结构域和肽接头组成,其中所述存活因子是赋予对一种或多种化学治疗剂的抗性的多肽,并且其中所述γδT细胞表达NKG2D受体,
其中所述肽接头将所述胞外间隔区连接至所述胞外抗原识别结构域,以及
其中所述跨膜结构域是CD28跨膜结构域。
