Description
本発明は、がん抗原ジシアロガングリオシド(GD2)に結合する、キメラ抗原受容体(CAR)に関する。そのようなCARを発現するT細胞は、神経芽腫などのがん性疾患の処置において有用である。
ジシアロガングリオシド(GD2、pubchem:6450346)は、主として細胞表面において発現する、シアル酸を含有するスフィンゴ糖脂質である。この糖鎖抗原の機能は、完全には理解されていないが、腫瘍細胞の細胞外マトリクスタンパク質への接着において重要な役割を担うと考えられている。GD2は、神経芽腫において密に、均一でかつほぼ普遍的に発現する。正常な組織において、GD2発現は、皮膚色素細胞および末梢疼痛線維ミエリン鞘に大きく限定される。CNS内で、GD2は、胎児性抗原であるようだが、散在したオリゴデンドロサイトにおいて、および下垂体後葉内で不明瞭に発現することが見出されている。これは、GD2を、標的抗腫瘍治療に十分に適切にさせる。
キメラ抗原受容体は、通常の構成において、モノクローナル抗体(mAb)の特異性を、T細胞のエフェクター効果へ移植したタンパク質である。その通常の構成は、抗原認識アミノ末端、スペーサー、膜貫通ドメインが全て、T細胞生存および活性化シグナルを伝達する混成エンドドメインに連結した、I型膜貫通ドメインタンパク質の形態である(図1aを参照)。
14g2a−CD28−OX40−ζ CARを発現するヒトT細胞は、いくらかの抗腫瘍活性を有するが、疾患を完全に根絶することができないことが示された(上記と同様にYvon et al (2009))。
本発明者らは、改善した性質を有する代替的なGD2標的化CARを作製しようとした。
本発明者らは、K666抗体に基づくGD2結合ドメインを含む、GD2を標的とする新しいキメラ抗原受容体(CAR)を構築した。

キメラ抗原受容体(CAR)
キメラ抗原受容体(CAR)(キメラT細胞受容体、人工T細胞受容体、およびキメラ免疫受容体としても公知)は、免疫エフェクター細胞上に任意の特異性を移植する、改変された受容体である。古典的なCARにおいて、モノクローナル抗体の特異性は、T細胞上に移植される。CARをコードする核酸配列は、例えばレトロウイルスベクターを使用して、T細胞に転移され得る。このようにして、多数のがん特異的T細胞は、養子細胞移入のために生成させることができる。このアプローチのフェーズI臨床試験は、効果を示す。

エンドドメインは、CARのシグナル伝達部である。抗原認識の後に、受容体は集合し、そしてシグナルは細胞へ伝達される。最も一般的に使用されるエンドドメイン構成要素は、3つのITAMを含むCD3ゼータのエンドドメイン構成要素である。これは、抗原が結合した後に、T細胞に活性化シグナルを伝達する。CD3ゼータは、完全に反応力を有する活性化シグナルを提供しないことがあり、そして追加の共刺激性シグナリングが必要となり得る。例えば、キメラのCD28およびOX40は、増殖/生存シグナルを伝達するために、CD3ゼータと使用することができ、または3つ全てを一緒に使用することができる。
本発明のCARは、CARがT細胞などの細胞の中で発現されたときに、新生タンパク質が小胞体へと、そしてそれに続きそれが発現される細胞表面へと指向されるように、シグナルペプチドを含み得る。

本発明のCARは、GD2結合ドメインと膜貫通ドメインとを連結させ、かつGD2結合ドメインをエンドドメインから空間的に離すための、スペーサー配列を含み得る。柔軟なスペーサーは、GD2結合を可能にするために、GD2結合ドメインを異なる角度に配向させる。
GD2は、ヒト神経芽腫およびメラノーマを含む神経外胚葉起源の腫瘍において発現するジシアロガングリオシドであり、ヒトにおける正常な組織においては、主として小脳および末梢神経に高度に限定された発現を有する。
本発明の第二の態様は、本発明の第一の態様のCARをコードする核酸配列に関する。
T細胞は生着しかつ自律的であるため、抗GD2 CAR T細胞のレシピエントにおいて選択的にCAR T細胞を消去する手段が望まれる。自殺遺伝子は、許容可能でない毒性に直面すると、注入されたT細胞の選択的な破壊をもたらす、遺伝学的にコード可能なメカニズムである。自殺遺伝子を用いた最も早期の臨床経験は、T細胞をガンシクロビルに対して感受性にさせるヘルペスウイルスチミジンキナーゼ(HSV−TK)を用いるものである。HSV−TKは、高度に効果的な自殺遺伝子である。しかしながら、事前に形成された免疫応答は、その使用をハプロ同一幹細胞移植などの考慮すべき免疫抑制の臨床的セッティングに限定し得る。誘導性カスパーゼ9(iCasp9)は、カスパーゼ9の活性化ドメインを改変されたFKBP12で置換することによって構築された自殺遺伝子である。iCasp9は、それ以外には不活性の小分子二量体化化学誘導剤(CID)によって活性化される。iCasp9は、ハプロ同一HSCTのセッティングにおいて最近試験されており、GvHDを抑えることができる。iCasp9の最も大きな制約は、臨床グレード専売のCIDの入手可能性に依存することである。iCasp9とHSV−TKとの両方は、細胞内タンパク質であり、そのため、単独の導入遺伝子として使用される場合、これらはマーカー遺伝子とともに共発現し、形質導入された細胞の選択を可能にする。
本発明は、本発明にしたがう核酸配列を含むベクターも提供する。このようなベクターは、本発明の第一の態様にしたがう分子を発現し、かつ産生するよう、宿主細胞に核酸配列を導入するために使用され得る。
本発明は、本発明にしたがう核酸を含む宿主細胞も提供する。宿主細胞は、本発明の第一の態様にしたがうCARを発現する能力を有し得る。
本発明は、本発明のベクターまたはCAR発現T細胞を、薬学的に許容されるキャリア、希釈液、もしくは賦形剤、および任意に1つまたはそれより多くの更に薬学的に有効なポリペプチド、および/もしくは化合物を一緒に含む薬学的組成物にも関する。このような製剤は、例えば、静脈注入に適した形態であり得る。
本発明のCAR分子を発現するT細胞は、神経芽腫(neurobastoma)細胞などのがん細胞を殺傷する能力を有する。CARを発現するT細胞は、患者自身の末梢血由来(第一当事者)、またはドナー末梢血からの造血幹細胞移植の状態において(第二当事者)、またはつながりのないドナーからの末梢血(第三当事者)のいずれかの由来から、エキソビボで創出され得る。代替的に、CAR T細胞は、誘導性前駆細胞または胚性前駆細胞のT細胞へのエキソビボ分化に由来し得る。これらの例として、CAR T細胞は、ウイルスベクターを用いた形質導入、DNAもしくはRNAを用いたトランスフェクションを含む多くの手段の1つにより、CARをコードするDNAまたはRNAを導入することによって生成される。
本発明は、細胞にGM3シンターゼをコードする核酸およびGD2シンターゼをコードする核酸を導入する工程を含む、GD2発現細胞を作製するための方法も提供する。
CARを、Nakamuraら(2001−上記と同様)によって記載されたようなマウス抗体KM666またはそのヒト化型のhuK666のいずれか(図2上部においてバリアント(a)および(b))由来の配列を使用し、scFvを用いて構築した。これらの受容体を発現/安定性について比較し、両方の受容体について同等であることを見出した。次に、これらの受容体を用いて形質導入したT細胞の、GD2を発現しないまたはGD2を発現するいずれかの標的細胞によって負荷したときの殺傷、サイトカイン放出および増殖を試験した。両方の受容体の殺傷は同様であったが、ヒト化scFvに基づく受容体は、優れたIL2産生および増殖をもたらすことが結論付けられた(図3)。
Fcスペーサー、ヒンジ、ヒンジ−CD8ストークおよびCd8ストークを有する抗GD2 CARを生成した(それぞれ図2(b)、(d)、(e)および(f))。これらのCARを、正確な比較を可能にするために、2A口蹄疫自己切断ペプチドを用いて、絶対的な1:1様式でマーカー遺伝子、短縮型CD34とともに共発現させた(図4a)。さらに、CARに対する導入遺伝子発現の比較を可能にするために、huK666 scFvに、アミノ末端HAタグを用いてタグ付与した。
上記の実施例からの全般的なデータは、Fcスペーサーが全般的に最も良好に機能することを示唆した。しかしながら、Fcドメインは、インビボで、Fc受容体を発現する細胞からの非特異的な活性化を導き得る。この効果を廃するために、図5(a)に示されるように、変異をFc領域に導入した。これらの変異は、図5(b)に示されるように、CAR発現において有害な影響を有しなかった。
受容体の発現を最適化する観点で、以下:(a)カセット内への足場付着領域(SAR)の包含、(b)3’LTR内へのニワトリベータヘモグロビンクロマチンインスレーター(CHS4)の包含、および(c)オープンリーディングフレームのコドン最適化を試験した(図6a)。SARの包含は、コドン最適化したものと同様に発現の性質を改善したが、一方でCHS4は、ほとんど効果を有しないことが示された(図6b)。SARとコドン最適化との組み合わせは、発現を追加的に改善した(図6c)。
3種の異なるエンドドメインを有するコンストラクトを生成した:Fcスペーサー構成におけるCARでの、CD3ゼータエンドドメインを伴うCD28膜貫通ドメイン(CD28tmZ)、CD28エンドドメインおよびCD3ゼータエンドドメインを伴うCD28膜貫通ドメイン(CD28Z)、ならびにCD28膜貫通ドメイン、CD28エンドドメイン、OX30エンドドメインおよびCD3ゼータエンドドメイン(CD28OXZ)。増殖、IFNγ放出およびIL−2放出は、CD28tmZ<CD28Z<CD28OXZの順序で増大することを特筆する(図7)。
iCasp9自殺遺伝子を、抗GD2 CARとともに共発現させた(図8a−CARは、機能を実証するために任意に選択されたFcスペーサー、CD28OXZの構成であった)。CARは、iCasp9との共発現にも拘らず、良好に発現させることができた(図8b)。小分子二量体化剤を用いたiCasp9の活性化は、CAR陽性T細胞の消去を導いた(図8b)。この二量体化剤に曝露したiCasp9−GD2CAR T細胞は、二量体化剤に曝露したときに、これらのGD2特異性を失った(図8c)。
抗GD2 CARを、RQR8自殺分類遺伝子とともに共発現させた(図9a−CARは、機能を実証するために任意に選択されたFcスペーサー、CD28Zの構成であった)。受容体とCARとを共発現させることが可能であった(図9b)。リツキシマブおよび補体を用いたRQR8の自殺遺伝子機能の活性化は、形質導入されたT細胞の消去およびGD2認識の消失をもたらした(図9cおよびd)。
培養においてGD2CAR T細胞を刺激し、理想的なGD2−またはGD2+標的を得て、小動物モデルのためのシンジェニックな細胞を生成することを可能にするために、細胞株においてトランスジェニックにGD2を発現させることのできることが望ましい。GD2は、1つのタンパク質ではなく、酵素の複雑な組によって合成されることを必要とする。ここで、ちょうど2種の酵素:GM3シンターゼおよびGD2シンターゼのトランスジェニック発現発現は、これまでに形質導入した全ての細胞株において、明瞭なGD2発現をもたらすことが示されている(図10)。
CT26細胞株を、上記に記載されるように、GD2を発現するように改変した(短縮のために、CT26クローン#7またはCT25#7で示される)。2×105の野生型(wt)またはGD2陽性のいずれかのCD26細胞を、C57BL/6マウス(CT26とシンジェニック)のわき腹に接種した。腫瘍負荷の10日後、模造物を形質導入したシンジェニック脾細胞および抗GD2 CARを形質導入したシンジェニック脾細胞を調製した。マウスを以下の4種のコホート:抗GD2 CAR脾細胞を受容した、GD2を発現するCT26腫瘍を有するマウス、模造品を形質導入した脾細胞を受容した、GD2を発現するCT26腫瘍を有するマウス、抗GD2 CAR脾細胞を有する、GD2陰性(wt)CT26腫瘍を有するマウス、および脾細胞を受容していない、GD2を発現するCT26腫瘍を有するマウス、に分けた。腫瘍を、デジタルノギスを使用して三次元で測定し、それを用いて体積を推定した。図11は、腫瘍の成長曲線を示す。抗GD2 CAR T細胞を受容したマウスにおけるGD2陽性の腫瘍のみが、ほとんどまたは全く成長しなかった。
CARの抗原結合ドメインは、その機能に影響し得る。本研究において、CARとともにhuK666に基づく抗原結合ドメインを有する本発明のCARの機能を、14g2aに基づく抗原結合ドメインを有する同等のCARと比較した。
Claims (14)
- ジシアロガングリオシド(GD2)結合ドメインを含むキメラ抗原受容体(CAR)であって、
a)配列番号10として示される配列を有する重鎖可変領域(VH)ドメインを含むVH、および
b)配列番号12として示される配列を有する軽鎖可変領域(VL)ドメインを含むVL
を含む、キメラ抗原受容体(CAR)。 - 前記GD2結合ドメインが、配列番号8として示される配列を含む、請求項1に記載のCAR。
- 配列番号27〜35のいずれかとして示される配列、または少なくとも80%の配列同一性を有するが、i)GD2に結合する能力、およびii)T細胞シグナリングを誘導する能力を保持するそれらのバリアントを含む、請求項1または2に記載のCAR。
- 請求項1〜3のいずれか1項に記載のCARをコードする核酸。
- 配列番号25として示される配列または少なくとも90%の配列同一性を有するそのバリアントを含む、請求項4に記載の核酸。
- 自殺遺伝子もコードする、請求項4または5に記載の核酸。
- 請求項4〜6のいずれかに記載の核酸を含む、ベクター。
- 請求項1〜3のいずれかに記載のCARを発現する、細胞。
- 請求項1〜3のいずれかに記載のCARと自殺遺伝子とを共発現する、細胞。
- 前記自殺遺伝子が、iCasp9またはRQR8である、請求項9に記載の細胞。
- 請求項8〜10のいずれかに記載の細胞を作製するための方法であって、エクスビボでT細胞に、請求項4〜6のいずれかに記載の核酸を導入する工程を含む、方法。
- 請求項7に記載のベクターまたは請求項8〜10のいずれかに記載の細胞と、薬学的に許容できるキャリア、希釈剤または賦形剤とを一緒に含む、薬学的組成物。
- がんを処置するのに使用するための組成物であって、請求項7に記載のベクターまたは請求項8〜10のいずれかに記載の細胞を含む、組成物。
- 前記がんは、神経芽腫である、請求項13に記載の組成物。
