Abstract

Description
技术领域
本发明涉及一种增敏剂,以及在TKI治疗过程中、将其与酪氨酸激酶抑制剂(TKIs)结合用于肿瘤、癌细胞、或者癌前细胞增敏的方法。更加具体地讲,本发明涉及一种给药机制,将诸如吉非替尼或者埃克替尼等酪氨酸激酶抑制剂与TKI增敏剂DZ-1通过酯类和酰胺结合,形成他汀类或者铂类药物偶联物;或者形成青蒿素偶联物。其中包括但不限于:DZ1-辛伐他汀酰胺、DZ-1辛伐他汀酯、DZ-1顺铂酰胺和DZ-1顺铂酯、DZ-1青蒿素酯和DZ-1青蒿素酰胺。另外,本发明还涉及一种通过肿瘤、癌细胞、或者癌前细胞(尤其是对TKI具有耐药性的细胞,例如肺癌、胰腺癌和肾脏肿瘤)增敏,从而提高TKI癌症疗效的方法。

背景技术
当前已知的多种癌症疗法中,都需要使用各种方法来提高癌细胞对化疗药物的敏感性、以及/或者克服癌细胞对药物的耐药性。其中包括将诸如辛伐他汀等他汀类药物与各种治疗药物结合,以便克服对各种酪氨酸激酶抑制剂的耐药性。尽管如此,当前已知的各种方法都没有很好的效果,例如只对部分特定的癌细胞表型有效、以及/或者效果不稳定,对部分患者有效、但对其他部分患者效果很差。
近红外(NIR)染料与癌症药物的偶联物具有靶向性,通常用于引导癌症药物作用于癌细胞。尽管如此,近红外染料偶联物并不会提高癌细胞对TKI的敏感性,也无法克服癌细胞对TKI的耐药性。另外,当前尚没有任何已知的方法能够提高肺、胰腺和肾脏癌细胞的敏感性,或者克服其对TKI的耐药性。
因此,当前亟需一种新的药物,能够通过其配方或者使用方法来提高癌细胞对TKI化疗药物的敏感性。除此之外,还需要开发出能够克服癌细胞对TKI的耐药性的药物配方和方法。另外,还需要开发能够克服肺和胰腺癌细胞对TKI的耐药性的药物配方和方法。本发明能够满足上述要求、同时还有很多其他优点,我们将在下文中的“发明概述”一节中对此进行深入解释和介绍,这些特点对于领域内的专业人员而言具有显而易见性。
发明概述
让人意外的是研究结果表明特定近红外染料偶联物,尤其是特定的DZ1-药物酯类和酰胺类偶联物,在与TKI结合后具有癌细胞增敏效果;另外,在与一种TKI药物结合使用时,能够降低TKI的必要剂量、以及/或者克服细胞对TKI的耐药性。因此,本发明涉及一种增敏剂,以及在TKI治疗过程中、将其与酪氨酸激酶抑制剂(TKI)结合用于肿瘤、癌细胞、或者癌前细胞增敏的方法。更加具体地讲,本发明涉及一种给药机制,将诸如吉非替尼或者埃克替尼等酪氨酸激酶抑制剂与TKI增敏剂DZ1酯类和酰胺结合,形成他汀类或者铂类药物偶联物;或者形成青蒿素偶联物。其中包括但不限于:DZ1-辛伐他汀酰胺、DZ-1辛伐他汀酯、DZ-1顺铂酰胺和DZ-1顺铂酯、DZ-1青蒿素酯和DZ-1青蒿素酰胺。另外,本发明还涉及一种通过肿瘤、癌细胞、或者癌前细胞(尤其是对TKI具有耐药性的细胞,例如肺癌癌前细胞和肾脏肿瘤)增敏,从而提高TKI癌症疗效的方法。

在本发明的具体实施方式中,将提供一种药物偶联物增敏剂。其中药物部分是他汀类药物,可以包括:辛伐他汀(SIM)、阿托伐他汀、氟伐他汀、洛伐他汀、匹伐他汀、普伐他汀和瑞舒伐他汀。
在本发明的具体实施方式中,将提供一种药物偶联物增敏剂。其中药物部分是铂类抗肿瘤药物,可以包括:顺铂(CIS)、卡铂(也称为CBDCA)和双环铂(也称为DCP)、奥沙利铂、塞特铂、吡铂、奈达铂、三核铂、洛铂、庚铂和顺铂脂质体。
在本发明的具体实施方式中,将提供一种药物偶联物增敏剂。其中药物部分是酯类或酰胺偶联的辛伐他汀(SIM)、顺铂(CIS)和青蒿素(ART)。
在本发明的具体实施方式中,将提供一种药物配方。其中包括一种增敏剂、以及至少一种可以和药物兼容的载体。其中偶联物中的药物部分可以从他汀类药物、铂类抗肿瘤药物、以及青蒿素中选择;DZI部分可以从具有上文中所述的分子式I和II的酰胺或者酯类交联物质中选择。
在本发明的具体实施方式中,将提供一种药物配方。其中增敏剂和其载体具有合适的浓度,能够将充分浓度的增敏剂递送到使用TKI进行治疗的肿瘤、癌细胞或癌前细胞处,以增加癌细胞对TKI的敏感性。
在本发明的具体实施方式中,将提供一种可以作为一种或多种药物配方组合中的一部分的药物配方。在上述药物配方组合中包括:(1)增敏剂;(2)一种酪氨酸激酶抑制剂(TKI);(3)成分(1)或(2)、或者上述两种成分的一种或多种递送手段;以及(4)在常见给药方案中,协调完成增敏剂和TKI药物给药的具体规程和指令。其中增敏剂可以从他汀类药物、铂类抗肿瘤药物、以及青蒿素中进行选择。

具体实施方式
本发明涉及一种增敏剂,以及在TKI治疗过程中、将其与酪氨酸激酶抑制剂(TKI)结合用于肿瘤、癌细胞、或者癌前细胞增敏的方法。更加具体地讲,本发明涉及一种给药机制,将诸如吉非替尼或者埃克替尼等酪氨酸激酶抑制剂与TKI增敏剂DZ1酯类和酰胺结合,形成他汀类或者铂类药物偶联物;或者形成青蒿素偶联物。其中包括但不限于:DZ1-辛伐他汀酰胺、DZ-1辛伐他汀酯、DZ-1顺铂酯、DZ-1顺铂酰胺、DZ-1青蒿素酯和DZ-1青蒿素酰胺。另外,本发明还涉及一种通过肿瘤、癌细胞、或者癌前细胞(尤其是对TKI具有耐药性的细胞,例如肺癌、胰腺癌和肾癌)增敏,从而提高TKI癌症疗效的方法。
根据本发明的具体实施方式,将一种增敏剂,即本发明所述的DZ1-药物酰胺或酯类偶联物与酪氨酸激酶抑制剂(TKI),特别是抑制表皮生长因子受体(EGFR)催化活性的TKI一起联合使用。TKI是一种抑制酪氨酸激酶的药物。酪氨酸激酶是一种通过信号转导激活尤其是EGFR激活多种蛋白的酶。通过在蛋白质中添加一种磷酸基(磷酸化)来激活蛋白质,即TKI抑制的一个步骤。TKI通常被用作治疗各种癌症的抗癌药物,通过抑制癌细胞的生长(并阻止或减缓肿瘤的生长),和/或诱导细胞凋亡(细胞死亡),通常可以导致肿瘤缩小。
根据本发明具体实施方式,TKI可以包括一个或多个吉非替尼、埃克替尼、埃罗替尼、阿法替尼、布加替尼、奥斯替尼、达克替尼、依维莫司、拉帕替尼、卡博替尼、舒尼替尼和凡德他尼。
根据本发明的具体实施方式,EGFR酪氨酸激酶抑制剂药物(TKI)可包括一种或多种吉非替尼、埃克替尼、埃罗替尼、阿法替尼、布加替尼、奥斯替尼、达克替尼和拉帕替尼(一种EGFR和ERBB2混合抑制剂)。首选的TKI可能是EGFR TKI,特别推荐吉非替尼和埃克替尼或它们的组合。
根据本发明的具体实施方式,通过将增敏剂运用到需要TKI治疗的癌症病人身上,尤其是对TKI具有耐药性或已产生耐药性的癌症病人,让肿瘤、癌症或癌前细胞可以接触到一种或几种增敏剂。
根据本发明的具体实施方式,DZ1-偶联增敏剂可能包括DZ1-顺铂酯、DZ1-顺铂酰胺、DZ1-辛伐他汀酯,DZ1-辛伐他汀酰胺,DZ1-青蒿素酯和DZ1-青蒿素酰胺,将其用于TKI治疗的肿瘤,肿瘤、癌症或癌前细胞増敏,将这类细胞暴露于増敏剂及TKI,也就是说将増敏剂和TKI给到癌症和肿瘤病人的癌细胞上。这些增敏剂的具体实例及其分子式如下所示。增敏剂包括

根据本发明的具体实施方式,替代顺铂(CIS)铂类增敏剂化合物包括铂类抗肿瘤药物或化疗药物,包括但不限于,顺铂、卡铂(也称为CBDCA)和双环铂(也称为DCP)、奥沙利铂、塞特铂、吡铂、奈达铂、三核铂、洛铂、庚铂和顺铂脂质体。

根据本发明的具体实施方式,为了增强肿瘤细胞、癌细胞或癌前细胞对TKI治疗的敏感性,具有TKI相关癌症的患者可优先选择在TKI给药之前或与TKI给药同时使用本发明的增敏剂。或者,对于已经产生TKI耐药性的癌细胞治疗,可以在一次或多次初始TKI给药后,在一次或多次TKI给药之前或同时给药。

发明具体描述
在本发明的具体实施方式中,包括一种含有一种或多种增敏剂成分的药物配方。此药物配方可以供人用或者动物使用,其中含有一种或多种本发明所述成分(或者其盐类、溶液、代谢物或者衍生物),一种或多种可以和药物兼容的载体,以及/或者一种或多种赋形剂,以及/或者一种或多种活性成分。其中一种或多种载体、赋形剂、以及/或者活性成分可以从与配方中其他成分兼容、不会对受体产生不良影响的物质中进行选择。上述载体应该为相关领域内的已知载体,对于该领域内的普通技术人员具有显而易见性。
在本发明的具体实施方式中,相关成分和药物配方的给药途径包括但不限于:口服、腹膜内注射、皮下注射、肌肉注射、经皮注射、直肠给药、阴道给药、舌下给药、静脉注射、含服或者吸入。在部分实施方式中,本发明所述药物配方可以含有与药物成分兼容的赋形剂,以便让其形成能够通过上述途径给药的药物化合物或混合物。另外,作为替代方式,相关药物配方还可以通过泌尿生殖系统给药,即通过内部器官、局部病变、或者外部设施(例如膀胱、阴道插管)给药。
在本发明的具体实施方式中,药物活性成分可以和能够与药物成分兼容的赋形剂混合或者结合。其给药方式、溶剂、赋形剂和载体不会与药物活性成分发生反应,且能够被领域内的普通技术人员理解。例如,应该使用“雷明顿药剂科学”第18版(1990年)中所介绍的给药方式、溶剂、赋形剂和载体,上述著作应该视为本专利申请书的有机组成部分。赋形剂必须能够与药物配方中的其他成分兼容、且不会对药物受体产生任何不良影响。
在本发明的具体实施方式中,药物配方将能够使用制药行业内普通采用的方法、便捷地将其分为适用的单位剂型。通常而言,上述制备方法包括将药物配方制备为适合于递送的形式,例如,制备为水性悬液。在药物配方的剂型中,可以包括一种或多种制药过程中使用的辅剂或者附属成分,例如混合剂、缓冲剂、以及溶解增强剂。

在本发明的具体实施方式中,可以使用合适的溶剂来制备本发明所述药物配方的不经胃肠道剂型,其中包括但不限于:无菌水;“美国药典”规定的注射用水;生理盐水;葡萄糖溶液;诸如但不限于氯化钠注射液、林格注射液、右旋糖加氯化钠注射液、以及乳酸林格注射液等水性溶剂;诸如但不限于乙醇、聚乙二醇、以及丙二醇等水溶性溶剂;以及诸如但不限于玉米油、棉籽油、花生油、芝麻油、油酸乙酯、肉豆蔻酸异丙酯、以及苯甲酸苄酯等非水性溶剂。在本发明所述药物配方的不经胃肠道剂型(包括普通剂型和控释剂型)中,还可以使用会影响或者改变与本发明所述药物配方兼容的某种成分的盐类的溶解度的化合物。
在本发明的具体实施方式中,不经胃肠道给药的配方包括水性和非水性无菌注射液。在其中可以加入其他成分,例如抗氧化剂、缓冲剂、抑菌剂和溶质,以便制备得到与药物受体血液具有等张力的药物配方。药物配方中还可以包括水性和非水性无菌悬液,其中可以含有悬浮剂和增稠剂。
在本发明的具体实施方式中,可以使用行业当前已知的技术、利用合适的分散剂或润湿剂和悬浮剂,制备本发明所述药物配方的注射剂,例如无菌水性注射剂或者含油悬液注射剂。上述无菌注射剂可以为无菌注射溶液、悬液或者乳液,可以使用无毒、不经胃肠道的稀释剂或溶剂,例如使用1,3-丁二醇制备的溶液。可以使用的各种溶剂包括水、林格溶液、美国药典中规定的溶剂、以及等张氯化钠溶液。另外,通常也可以使用无菌、不具有挥发性的油作为溶剂或者悬浮介质。在这种情况下,可以使用任何温和、无挥发性的油,包括合成的单甘酯和双甘酯。另外,在注射剂的制备过程中,还可以使用诸如油酸等脂肪酸。
在本发明的具体实施方式中,通过直肠和阴道给药的药物配方应该优选使用栓剂。可以将本发明所述药物配方与不具有刺激性的合适赋形剂或载体混合来制备栓剂,例如可可油、聚乙二醇、或者栓剂蜡。这些赋形剂或载体在室温下为固体,但在体温下会变为液体,因此可以在直肠或者阴道中熔化并释放药物的活性成分。
在本发明的具体实施方式中,合适的口服或者舌下给药剂型包括:片剂、锭剂、胶囊、酏剂、悬液、糖浆、薄片剂、口香糖、或者按照行业当前惯例制备的其他剂型。其中活性成分应该具有合理的浓度,确保具有疗效的成分或者制剂能够达到合适的剂量。糖浆剂型通常是指添加有调味剂和着色剂的液体载体(例如:乙醇、丙三醇或水)中,相关药物成分或其盐类的悬液或溶液。

在本发明的具体实施方式中,在软质和硬质明胶胶囊中,可以使用类似类型的固体成分作为填充剂;同时使用诸如乳糖、高分子聚乙二醇和类似物质作为赋形剂。片剂、糖衣丸剂、胶囊、药片和颗粒等剂型表面可以带有糖衣层和外壳,例如肠溶衣层、或者制药行业中当前普遍采用的其他外层。在这些剂型中可以选择含有遮光剂。在优选实施方式中,可以选择包含能够让其只在胃肠道中的特定区域、以延迟的方式释放药物活性成分的其他成分。可以使用的填充成分的实例包括高分子聚合物和石蜡。在软质和硬质明胶胶囊中,可以使用类似类型的固体成分作为填充剂;同时使用诸如乳糖、高分子聚乙二醇和类似物质作为赋形剂。

根据本发明的具体实施方式,包含一个或多个増敏剂组成的药物按照有效剂量用于病人的肿瘤、癌症或癌前细胞,通过提高细胞对TKI的敏感性治疗或抑制癌症。TKI可以同时给药,也可以在増敏剂之前或之后不久给药,具体根据特定的给药方案决定,包括考虑增敏剂的浓度和血液中的TKI含量,以及活性成分的半衰期,这对普通技能人员具有显而易见性。
在本发明的具体实施方式中,可以让患者单次摄入含有增敏剂成分的药物配方的有效剂量。另外,作为替代方式,也可以让患者多次摄入含有增敏剂成分的药物配方的有效剂量。在特定实施方式中,可以通过皮肤贴片、滴注到膀胱中,通过尿道或者阴道、或者通过植入环向患者提供含有增敏剂成分的药物配方的有效剂量。可以一次性向患者提供含有本发明所述成分的药物配方的治疗剂量;也可以在一定的间隔时间内重复给药,例如间隔5分钟、10分钟、15分钟、20分钟或者25分钟。在必要时,还可以定期多次向患者给药,例如每三小时、六小时、十二小时或者更长时间给药一次;或者每两周给药一次,持续一个月、两个月、三个月、四个月或者更长时间。在部分情况下,在首次治疗之后,可以按照更低的频率给药。例如,在每两周给药一次,持续三个月之后,可以每月给药一次,持续六个月、一年或者更长时间。应该按照协调的给药机制,进行本发明中所述的一种或多种增敏剂成分与TKI的给药,确保癌细胞能够暴露、最好同时暴露在这两种成分之下,以便降低癌症症状生物标记物的水平。

根据本发明的具体实施方式,TKI抑制剂的浓度可能取决于特定TKI配方的典型剂量,可适用于将肿瘤、癌症或癌前细胞暴露于大约0.1uM~100uM的浓度。例如,吉非替尼的浓度可以在0.5~5uM之间,也可以与约1~10uM DZ1-辛伐他汀酰胺或酯,或1~10uM DZ1-顺铂酰胺或酯联合使用。

本发明中所述成分的剂量由医生根据实际观察到的疗效来确定或者调整。在治疗持续时间和频率方面,通常需要由专业的临床医生来监控患者,以便确定相关治疗方法是否有疗效,是否需要增加或者减少剂量,是否需要提高或者降低给药频率,是否需要中断治疗或者恢复治疗,或者对治疗方案做出其他修改。可以根据多种临床因素,例如患者对于本发明所述成分的敏感程度,每周或者每天对给药计划/方案进行修改。
除了增敏之外,对于还需要识别、成像、以及/或者定位癌细胞、癌前细胞或者肿瘤的患者而言,还可以通过多种具体实施方式来提供具有双重功能的方法,即在提高细胞对TKI癌症治疗方法的敏感性的同时,还能够提供成像功能。这种方法中可以包括一种或多种含有增敏剂成分的DZ1-染料偶联物。先让患者摄入一种或多种增敏剂成分,在让其摄入一种或多种TKI药物,之后执行光学成像操作。通过这种方法可以跟踪和控制肿瘤的生长以及/或者收缩过程,根据患者的具体情况确定更加优化的剂量,以及/或者在DZ1的近红外光谱区域内确定肿瘤以及/或者转移瘤的位置。在多种具体实施方式中都可以执行成像操作,例如在注射之后6到48小时内成像。还可以通过成像与正常组织/细胞进行比较。
还可以通过多种具体实施方式来提供具有双重功能的方法,即对本发明中所述的增敏剂成分进行现场的药代动力学和药效学分析、以及对肿瘤或正常细胞或组织中的药物有效载荷进行现场分析。在这种方法中,需要先摄入增敏剂,让其与癌细胞、肿瘤或者正常细胞或组织接触;之后对癌细胞、肿瘤或者正常细胞或组织进行成像,之后再进行药代动力学以及/或者药效学分析,即测定不同时间的荧光强度(或者其变化幅度)。

图1为实例1中使用DZ1-辛伐他汀酯和酰胺分别检测的人肺癌细胞系与辛伐他汀比较的IC50结果。结果表明辛伐他汀的药效与DZ1-辛伐他汀酯或DZ1-辛伐他汀酰胺相当或更有效,且DZ1-辛伐他汀酯的药效优于DZ1-辛伐他汀酰胺。
图2为实例2中使用DZ1-辛伐他汀酯和酰胺分别检测的非小细胞肺癌细胞系的IC50结果,表明EGFR-突变细胞对TKI具有敏感性。结果表明,DZ1-辛伐他汀酰胺有助于提高耐药和不耐药EGFR突变癌细胞对TKI的敏感性,且DZ1-辛伐他汀可克服TKI耐药和/或减少TKI剂量。
图3为实例3中使用DZ1-辛伐他汀酰胺检测的非小细胞肺癌细胞系的IC50结果,表明EGFR-野生型细胞对TKI具有敏感性。结果表明,在耐药细胞和非耐药细胞中,TKI的剂量可以在保持TKI效应的同时降低,而且在没有EGFR突变的癌细胞中,TKI的増敏作用也始终有效。
图4为实例4中使用DZ1-辛伐他汀酰胺检测肺癌细胞系的结晶紫实验结果。结果表明,运用增敏剂后,细胞对TKIs的增敏作用跟浓度有关,在浓度低至0.5uM(增敏剂浓度为5uM)时,吉非替尼对TKI的增敏效果较好。
图5为实例5中使用γ分泌酶抑制剂-ro4929097检测肺癌细胞的IC50结果。结果表明,RO4929097不能稳定提高细胞对TKI的敏感性,与浓度没有关系,对EGFR野生型细胞没有敏感性;只有A549具有敏感性,而A549/DDP、H-1975和H-1650没有増敏作用。
示例性实施方式
在本实施实例中使用的材料和方法如下所述:辛伐他汀从Ark Pharm,Inc.(阿灵顿高地,伊利诺伊州)处采购;顺铂从MedKoo Biosciences,Inc.(教堂山,北卡罗来纳州)处采购。所有其他化学品和试剂都是从Sigma-Aldrich以及/或者VWR处采购的,都是可以购买到的最高质量级别。去离子水(18.2欧姆)是从使用Millipore(比勒立卡,马萨诸塞州,美国)处采购的Milli-Q直接超纯水系统制备的。
所有中间产物都使用1H NMR和质谱进行分析,化合物的纯度使用高效液相色谱法分析。使用Bruker 400MHz核磁共振仪,在标准参数条件下采集1HNMR数据;相对于非氘化溶剂的化学位移使用ppm(δ)表征。新化合物的质谱使用Thermo Fisher LTQ Orbitrap Elite系统进行ESI质谱分析。
DZ1的合成方法如下所述:使用去离子超纯水(电阻率为18.2MΩcm)和高效液相色谱(HPLC)级溶剂制备。使用Apollo C18 RP液相色谱柱(5μm,150×4.6mm)和1260无限二极管阵列检测器,在Agilent系统中进行反相(RP)高效液相色谱(HPLC)分析。流动相从50%溶剂A(0.1%三氟乙酸和80%的水)和50%溶剂B(0.1%三氟乙酸和80%的水乙腈)经30分钟梯度变到100%溶液B,流量为1毫升每分钟(检测波长为254和780纳米)。使用ThermoFisher LTQ Orbitrap Elite质谱分析仪执行ESI质谱分析。使用Bruker 400MHz NMR核磁共振光谱仪测定1HNMR谱图数据。

在细胞和细胞系测试中,除非另有说明,否则均是在含有10%FBS的介质中进行培养的,让其生成单层的细胞,之后再使用不同浓度的增敏剂成分以及/或者TKI处理72小时。之后测定这些培养细胞的半数抑制浓度IC50(定义为细胞增生程度降低50%时的浓度,使用结晶紫分析法)。数据表达方式为至少三次独立实验的平均值±标准差。
在结晶紫分析中,使用96孔板分析,测定每个孔中细胞吸收和洗脱的结晶紫染料量。实际使用的是Falcon 96孔板,将细胞注入到每个孔中(孔中装有T介质,含有1%的活性炭解吸CS和2%的TCM)。经过24小时之后,将细胞转移到无血清环境中,使用不同浓度的测试药物进行测试。为了避免在每次更换介质时附着不佳的细胞脱离,只是轻轻地部分洗掉之前的介质,再按照50微升的整数倍加入100微升的新鲜介质。每两天更换一次介质。在7-10天之后,使用1%的戊二醛(Sigma)将细胞固定,再使用0.5%的结晶紫(Sigma)进行染色。之后冲洗板块并吹干,再使用100微升的索伦森缓冲液(9毫克柠檬酸三钠溶解在305毫升的水、195毫升0.1摩尔每升的盐酸、以及500毫升90%的乙醇中)进行染料洗脱。之后使用Titertek Multiskan TCC/340(Flow Laboratories,麦克莱恩,弗吉尼亚州)、在560纳米波长下测定每个孔的吸收率。对照试验结果表明,吸收率与每个孔中的细胞数量成正比。
实例1
人类肺癌细胞系包括非小细胞肺癌(NSCLC)细胞系和小细胞肺癌(SCLC)细胞系。非小细胞肺癌细胞系包括亲代A-549、A-549/DDP(顺铂耐药细胞系)、95D(亲代非转移性95C的转移变体)、鳞状YTMLC细胞系。H446是SCLC细胞系。细胞在含10%胎牛血清的培养基中作为单层培养,用不同浓度的药物处理72h。在这些培养的细胞中测定了IC50(根据结晶紫法测定的导致细胞增殖下降50%的浓度)。数据为至少从三个独立实验得出的平均值±标准差(SEM)。检测和比较的药物为辛伐他汀、DZ1-辛伐他汀酯(DZ1-辛伐他汀E)、DZ1-辛伐他汀酰胺(DZ1-辛伐他汀A)、顺铂(CIS)、DZ1-顺铂。这些药物作为单一药剂使用。
如下表所示,结果表明辛伐他汀与DZ1-辛伐他汀酯或DZ1-辛伐他汀酰胺作用相同或更有效,且在所有体外检测的肺癌细胞系中,DZ1-辛伐他汀酯均优于DZ1-辛伐他汀酰胺。DZ-顺铂酯衍生物在所有检测的人类肺癌细胞系中始终比顺铂更有效。
图1A的A-F图显示了用DZ1-辛伐他汀酯和酰胺分别对例1中检测的细胞系计算出的50%抑制浓度(IC50)对数刻度;图1B的A-F图显示了顺铂和DZ1-顺铂的情况。图中数据以平均值+/-SEM的形式表示,采用两种四份复本的检测方法。用二甲基亚砜处理的控制细胞的存活率赋值为100。上述分析的综合数值结果如表1所示。
实例2
实例2显示了在人体肺癌体外模型中,EGFR-突变非小细胞肺癌细胞系H1975和H1650摄入EGFR酪氨酸激酶抑制剂药物(TKI),使用或不使用不同浓度的DZ1-辛伐他汀酰胺増敏剂的检测结果。人肺细胞系H1975和H1650用作EGFR突变肺癌细胞的模型。如实例1所示,使用不同的细胞系,不同的药物及其浓度。将H-1975和H-1650在含10%胎牛血清的培养基中培养,在使用或不使用不同浓度的増敏剂DZ1-辛伐他汀酰胺(浓度为3-7uM)的情况下,用不同浓度的TKI(吉非替尼或埃克替尼)处理72h。在这些培养的细胞中测定了IC50(根据结晶紫法测定的导致细胞增殖下降50%的浓度)。数据代表了至少三个独立实验的平均值。在图2中,图A-D图中数据采用两种四份复本的检测方法。用二甲基亚砜处理对照细胞的存活率赋值为100。
IC50结果见下表和图2(见图A-D),比较了DZ1-辛伐他汀酰胺増敏细胞和非增敏细胞,发现DZ1-辛伐他汀酰胺可使癌细胞对EGFR传导的抗肿瘤效果和TKI(图中为吉非替尼和埃克替尼)细胞毒性更为敏感。与单独使用TKI相比,与DZ1-辛伐他汀酰胺联合使用,显著降低了这些细胞针对吉非替尼或埃克替尼的半抑制浓度(IC50)。这表明,使用DZ1-辛伐他汀酰胺,可使对TKI耐药的EGFR突变癌细胞对TKI的抗肿瘤效果更加敏感,而在耐药和非耐药细胞中,有DZ1-辛伐他汀存在时,TKI的剂量可大大降低,同时保留足够的抗肿瘤作用。
实例3
实例3显示了在人体肺癌体外模型中,EGFR-野生型非小细胞肺癌细胞系A549和A549/DDP摄入EGFR酪氨酸激酶抑制剂药物(TKI),使用或不使用不同浓度的DZ1-辛伐他汀酰胺増敏剂的检测结果。人肺细胞系A549和A549/DDP用作没有EGFR突变的肺癌细胞的模型。可以看出TKI诱导生长抑制作用。将EGFR-野生型人体肺癌细胞系A549和A549/DDP在含10%胎牛血清的培养基中培养,在使用或不使用不同浓度的増敏剂DZ1-辛伐他汀酰胺(浓度为5-9.5uM)的情况下,用不同浓度的TKI(吉非替尼或埃克替尼)处理72h。在这些培养的细胞中测定了IC50(根据结晶紫法测定的导致细胞增殖下降50%的浓度)。数据代表了至少三个独立实验的平均值。在图3中,数据以平均值+/-SEM的形式表示,采用两种四份复本的检测方法。用二甲基亚砜处理控制细胞的存活率赋值为100。由下表和图3可以看出,与单独使用TKI相比,DZ1-辛伐他汀酰胺可显著降低无EGFR突变的TKI耐药和非耐药肺癌细胞针对TKI的IC50值,并与吉非替尼或埃克替尼的浓度有关。这说明DZ1-辛伐他汀酰胺可以使野生型EGFR肺癌细胞对TKIs的抗肿瘤作用更加敏感,而在耐药和非耐药细胞中,有了DZ1-辛伐他汀酰胺,TKI的剂量可以大大降低,同时保留足够的抗肿瘤作用。结果与肺癌患者的临床观察结果一致(未显示详细结果),其中EGFR突变的人肺癌细胞系(IC50s=4.8-10.2uM)对TKI的敏感性高于野生型EGFR人肺癌细胞系(IC50s=10.2-22uM)。结果表明,无论有无EGFR突变,对癌细胞都有效。
实例4
对人肺癌细胞系(H-1975,EGFR突变细胞系)进行晶体紫染色法测定,将细胞单独暴露于不同浓度的吉非替尼,与联合暴露于DZ1-辛伐他汀酰胺的结果进行比较。如图4所示,使用吉非替尼的浓度为64、32、16、8、4、2、1、0.5、0uM(列1-6,行A),将相同浓度的吉非替尼与浓度为5uM的DZ-辛伐他汀酰胺(行B)和浓度为6uM的DZ-辛伐他酰胺(行C)结合使用。结果显示,在与DZ1-辛伐他汀合并使用的情况下,TKI吉非替尼的细胞毒性作用和协同机制显著增强,并与浓度相关,比如0.5uM吉非替尼结合6uM的DZ1-辛伐他汀酰胺。
实例5
实例5显示仅采用γ分泌酶抑制剂RO4929097对EGFR突变细胞H1975缺乏増敏作用。RO4929097与EGFR TKI(吉非替尼或埃克替尼)联合用于人肺癌体外模型(EGFR突变H1975、H-1650(数据未显示)、EGFR野生型A549和A549/DDP肺癌细胞系)。所有细胞均在含10%胎牛血清的培养基中培养,在存在或不存在γ-分泌酶抑制剂-ro4929097(浓度为10-20uM)的情况下,用不同浓度的TKI(吉非替尼或埃克替尼)处理72h。在这些培养的细胞中测定了IC50(根据结晶紫法测定的导致细胞增殖下降50%的浓度)。表中数据为至少三个独立实验的平均值。图中数据以平均值+/-SEM的形式表示,采用两种四份复本的检测方法,并将用二甲基亚砜处理的控制细胞的存活率赋值为100。下表和图5的结果显示,无论是EGFR突变还是EGFR野生型人肺癌细胞系,RO4929097并不能持续强化细胞对TKI诱导抗肿瘤作用的敏感性。
结果表明,RO4929097增强细胞对TKI诱导生长抑制作用的敏感性是可变的,特别是RO4929097可以增强吉非替尼在A549细胞中的作用,但对H1975细胞和A549/DDP细胞没有增强作用,且増敏作用与浓度没有连续相关性,与EGFR突变无关。如图5所示的结晶紫实验显示,无论是EGFR突变细胞系还是野生型非小细胞肺癌细胞系,RO4929097(RO)并没有持续降低TKI的IC50。在RO4929097増敏成功后,只有使用吉非替尼治疗的A549有反应,而A549/DDP、H-1975和H-1650(数据未显示)对吉非替尼和埃克替尼均无持续反应。
实例6

实例7

1.一种与酪氨酸激酶抑制剂结合使用的癌细胞增敏剂,其特征在于,所述增敏剂的成分为DZ1-药物酰胺或酯类偶联物,其中药物部分可以从他汀类药物、铂类抗肿瘤药物和青蒿素中选择;DZ1部分可以从具有下列分子式I和II的酰胺或者酯类交联物质中选择:
2.根据权利要求1所述的一种与酪氨酸激酶抑制剂结合使用的癌细胞增敏剂,其特征在于,所述药物部分为他汀类药物,可以从辛伐他汀、阿托伐他汀、氟伐他汀、洛伐他汀、匹伐他汀、普伐他汀和瑞舒伐他汀中进行选择。
3.根据权利要求1所述的一种与酪氨酸激酶抑制剂结合使用的癌细胞增敏剂,其特征在于,所述药物部分为铂类抗肿瘤药物,可以从顺铂、卡铂和双环铂、奥沙利铂、塞特铂、吡铂、奈达铂、三核铂、洛铂、庚铂和顺铂脂质体中进行选择。
4.根据权利要求1所述的一种与酪氨酸激酶抑制剂结合使用的癌细胞增敏剂,其特征在于,所述药物部分可以从酯类或酰胺偶联的辛伐他汀、顺铂和青蒿素中选择。
5.一种药物配方,其中含有一种增敏剂成分、以及至少一种可以与药物兼容的载体;其中药物部分可以从他汀类药物、铂类抗肿瘤药物和青蒿素中选择;DZI部分可以从具有下列分子式I和II的酰胺或者酯类交联物质中选择:
6.权利要求5中所述的药物配方,其中增敏剂和其载体具有合适的浓度,能够将充分浓度的增敏剂输送到使用酪氨酸激酶抑制剂进行治疗的肿瘤、癌细胞或癌前细胞处,以增加癌细胞对酪氨酸激酶抑制剂的敏感性。
7.权利要求5中所述的药物配方,作为一种或多种药物配方组合的一部分使用,在上述药物配方组合中包括:(1)增敏剂;(2)一种酪氨酸激酶抑制剂;(3)成分(1)或(2)、或者上述两种成分的一种或多种递送手段;以及(4)在常见给药方案中,协调完成增敏剂和酪氨酸激酶抑制剂药物给药的具体规程和指令,其中增敏剂可以从他汀类药物、铂类抗肿瘤药物、以及青蒿素中进行选择。
8.权利要求7中所述的药物配方,其中酪氨酸激酶抑制剂可以从吉非替尼、埃克替尼、埃罗替尼、阿法替尼、布加替尼、奥斯替尼、达克替尼、依维莫司和拉帕替尼中进行选择。
9.权利要求7中所述的药物配方,其中酪氨酸激酶抑制剂可以从吉非替尼和埃克替尼中进行选择;增敏剂成分中的偶联药物部分可以从辛伐他汀、顺铂和青蒿素中选择,增加吗成分可以从DZ1-辛伐他汀酯、DZ1-辛伐他汀酰胺、DZ1-顺铂酯、DZ1-顺铂酰胺、DZ1-青蒿素酯和DZ1-青蒿素酰胺中选择。
10.一种用于提高使用酪氨酸激酶抑制剂治疗的癌细胞或癌前细胞敏感性的方法,其特征在于,所述方法包括向使用酪氨酸激酶抑制剂治疗的患者进行下列给药:
a)一定剂量的酪氨酸激酶抑制剂,具有充分的浓度,能够产生细胞生长抑制以及/或者细胞凋亡等酪氨酸激酶抑制剂效果,酪氨酸激酶抑制剂与一种或者多种增敏剂同时给药、或者在患者体内有一种或多种增敏剂时给药;以及
b)一定剂量的增敏剂,具有充分的浓度,能够提高癌细胞对酪氨酸激酶抑制剂的敏感度,使得酪氨酸激酶抑制剂的疗效高于在没有进行增敏剂给药时的酪氨酸激酶抑制剂疗效;
其中所述增敏剂是一种DZI-1药物酰胺或酯类偶联物,其中药物部分可以从他汀类药物、铂类抗肿瘤药物和青蒿素中选择;DZ1部分可以从具有下列分子式I和II的酰胺或者酯类交联物质中选择:
11.根据权利要求9中所述的一种用于提高使用酪氨酸激酶抑制剂治疗的癌细胞或癌前细胞敏感性的方法,其特征在于,所述药物部分可以从辛伐他汀、阿托伐他汀、氟伐他汀、洛伐他汀、匹伐他汀、普伐他汀和瑞舒伐他汀中进行选择。
12.根据权利要求9中所述的一种用于提高使用酪氨酸激酶抑制剂治疗的癌细胞或癌前细胞敏感性的方法,其特征在于,所述药物部分可以从顺铂、卡铂和双环铂、奥沙利铂、塞特铂、吡铂、奈达铂、三核铂、洛铂、庚铂和顺铂脂质体中进行选择。
13.根据权利要求9中所述的一种用于提高使用酪氨酸激酶抑制剂治疗的癌细胞或癌前细胞敏感性的方法,其特征在于,所述药物部分可以从酯类或酰胺偶联的辛伐他汀、顺铂和青蒿素中选择。
14.根据权利要求9中所述的一种用于提高使用酪氨酸激酶抑制剂治疗的癌细胞或癌前细胞敏感性的方法,其特征在于,其中酪氨酸激酶抑制剂可以从吉非替尼、埃克替尼、埃罗替尼、阿法替尼、布加替尼、奥斯替尼、达克替尼、依维莫司和拉帕替尼中进行选择。
15.根据权利要求9中所述的一种用于提高使用酪氨酸激酶抑制剂治疗的癌细胞或癌前细胞敏感性的方法,其特征在于,其中酪氨酸激酶抑制剂可以从含有吉非替尼和埃克替尼的物质中进行选择。
16.根据权利要求9中所述的一种用于提高使用酪氨酸激酶抑制剂治疗的癌细胞或癌前细胞敏感性的方法,其特征在于,可以向患者递送下列一种或多种药物组合:
a)吉非替尼和DZ1-辛伐他汀酯类增敏剂组合;
b)吉非替尼和DZ1-辛伐他汀酰胺增敏剂组合;
c)吉非替尼和DZ1-顺铂酯类增敏剂组合;
d)吉非替尼和DZ1-顺铂酰胺增敏剂组合
e)吉非替尼和DZ1-青蒿素酯类增敏剂组合;
f)吉非替尼和DZ1-青蒿素酰胺增敏剂组合。
17.根据权利要求9中所述的一种用于提高使用酪氨酸激酶抑制剂治疗的癌细胞或癌前细胞敏感性的方法,其特征在于,可以向患者递送下列一种或多种药物组合:
a)埃克替尼和DZ1-辛伐他汀酯类增敏剂组合;
b)埃克替尼和DZ1-辛伐他汀酰胺增敏剂组合;
c)埃克替尼和DZ1-顺铂酯类增敏剂组合;以及/或者
d)埃克替尼和DZ1-顺铂酰胺增敏剂组合;
e)埃克替尼和DZ1-青蒿素酯类增敏剂组合;以及/或者
f)埃克替尼和DZ1-青蒿素酰胺增敏剂组合。
18.根据权利要求9中所述的一种用于提高使用酪氨酸激酶抑制剂治疗的癌细胞或癌前细胞敏感性的方法,其特征在于,可以向患者递送下列一种或多种药物组合:
g)依维莫司和DZ1-辛伐他汀酯类增敏剂组合;
h)依维莫司和DZ1-辛伐他汀酰胺增敏剂组合;
i)依维莫司和DZ1-顺铂酯类增敏剂组合;
j)依维莫司和DZ1-顺铂酰胺增敏剂组合;
k)依维莫司和DZ1-青蒿素酯类增敏剂组合;以及/或者
l)依维莫司和DZ1-青蒿素酰胺增敏剂组合。
19.根据权利要求9中所述的一种用于提高使用酪氨酸激酶抑制剂治疗的癌细胞或癌前细胞敏感性的方法,其特征在于,所述方法包括与从下列增敏剂中选择的增敏剂同时给药、或者在其存在的情况下,向患者递送从吉非替尼、埃克替尼、埃罗替尼、阿法替尼、布加替尼、奥斯替尼、达克替尼、依维莫司和拉帕替尼中选择的一种或多种酪氨酸激酶抑制剂类药物:
a)DZ1-辛伐他汀酯类增敏剂;
b)DZ1-辛伐他汀酰胺增敏剂;
c)DZ1-顺铂酯类增敏剂;以及
d)DZ1-顺铂酰胺增敏剂;
e)DZ1-青蒿素酯类增敏剂;以及
f)DZ1-青蒿素酰胺增敏剂。
20.根据权利要求9中所述的一种用于提高使用酪氨酸激酶抑制剂治疗的癌细胞或癌前细胞敏感性的方法,其特征在于,其中癌细胞或者癌前细胞包括肺癌、非小细胞肺癌、小细胞肺癌、胰腺癌、肾脏肿瘤、直肠结肠癌、前列腺癌、皮肤癌、肝癌、乳腺癌、肺部鳞状细胞癌、肛门癌、胶质母细胞、以及头部和颈部的上皮肿瘤中的一种或多种癌细胞或癌前细胞。
