发明内容
本发明的多方面涉及一种药剂递送装置。

具体实施方式
本发明的实施方案提供了一种药剂递送装置,其包括:主注射针,该主注射针用于将药剂注射到患者体内;以及疼痛减轻装置,该疼痛减轻装置用于在药剂注射之前将疼痛减轻剂释放到该患者体内。提供这种疼痛减轻装置允许在药剂注射之前将疼痛减轻剂释放到注射部位中,并由此当主注射针被触发并插入患者的皮肤时允许减轻患者感觉到的疼痛或不适。

在本申请的上下文中,术语“近侧”和“远侧”在此分别指的是相对更靠近患者和相对更远离患者。

一些递送装置可以包括安全注射筒、笔式注射器或自动注射器的一个或多个功能。例如,递送装置可以包括机械能量来源,其被配置成自动注射药剂(如通常发现于自动注射器中)和剂量设定机构(如通常发现于笔式注射器中)。
如上文所述的装置10被配置成将药剂(例如,流体药剂)注射到患者体内。装置10包括主体或外壳11,该主体或外壳通常包含含有待注射的药剂的储器(例如,注射筒或药筒)和促进递送过程的一个或多个步骤所需的部件。装置10还可以包括能够可拆卸地安装到外壳11的帽组件12。通常,在可以操作装置10之前,使用者必须将帽组件12从外壳11移除。
装置10包括预填充有流体药剂的药筒15,以及包括主注射针17的笔式针或针组件,该主注射针用于将药剂从药筒15注射到患者体内。外壳11包括窗口11a,通过该窗口可以看到药筒15的内容物。
装置10包括疼痛减轻装置16(在图2A中更详细地示出),用于在药剂注射之前将疼痛减轻剂注射到患者体内。
装置10还包括用于控制疼痛减轻剂和药剂注射过程的激活机构14。
如所示,外壳11基本上是圆柱形的并且沿纵向轴线X具有基本上恒定的直径。外壳11具有近侧区域20和远侧区域21。术语“近侧”是指相对更靠近注射部位的位置,并且术语“远侧”是指相对更远离注射部位的位置。
装置10还可以包括联接到外壳11的针套筒13,以允许套筒13相对于外壳11移动。套筒13可缩回地安装在外壳11中。例如,套筒13可以在与纵向轴线X平行的纵向方向上移动。具体地,套筒13在远侧方向上的移动可以允许主注射针17从外壳11的近侧区域20延伸。
主针17的插入可以经由几种机制发生。例如,主针17可以相对于外壳11固定地定位并且最初位于延伸的针套筒13内。通过将套筒13的近端放置在患者身体上并在近侧方向上移动外壳11来使套筒13发生的远侧移动将使主针17的近端露出。这种相对移动允许主针17的近端延伸到患者体内。这种插入被称为“手动”插入,因为经由患者相对于套筒13手动移动外壳11而将主针17手动插入。
另一种插入形式是“自动化”,由此主针17相对于外壳11移动。这种插入可以通过套筒13的移动或通过另一种激活形式(如例如按钮22)引发。如图1A和图1B中所示,按钮22位于外壳11的远端。然而,在其他实施方案中,按钮22可以位于外壳11的一侧。
其他手动或自动化特征可以包括药物注射或针缩回、或两者。注射是如下过程:塞子或活塞23从药筒内的远侧位置移动到药筒内的更近侧位置,以便迫使来自药筒的药剂通过主针17。在一些实施方案中,驱动弹簧(未示出)在装置10被激活之前处于压缩状态。驱动弹簧的远端可以被固定在外壳11的远侧区域21内,并且驱动弹簧的近端可以被配置成将压缩力施加到活塞23的远侧表面上。在激活后,储存在驱动弹簧中的至少一部分能量可以被施加到活塞23的远侧表面上。该压缩力可以作用在活塞23上以使其在近侧方向上移动。这种近侧移动用于压缩药筒内的流体药剂,从而迫使其从主针17离开。
在注射后,主针17可以缩回到套筒13或外壳11内。当使用者从患者身体移除装置10时,当套筒13向近侧移动时可以发生缩回。这可以在主针17相对于外壳11保持固定定位时发生。一旦套筒13的近端移动通过主针17的近端,并且主针17被盖住,就可以锁定套筒13。这种锁定可以包括锁定套筒13相对于外壳11的任何远侧移动。

在本文所述的实施方案中,微针19基本上彼此相同。单独微针19的长度包括在约0.2至约3毫米的范围内。每根微针19的长度取决于微针的材料和/或取决于所使用的疼痛减轻剂。在本文所述的实施方案中,每个微针19由钢制成。在本文所述的实施方案中,每个微针19至少部分地基本上是圆柱形的。具体地,每根微针19具有圆形截面,并且具有在约40微米与约150微米之间范围内的直径。每根微针19的直径还取决于微针的材料和/或取决于所使用的疼痛减轻剂。应该注意的是,本发明并不意在限于这种特定类型的微针。例如,微针19可以具有不同的形状,例如圆锥形、管形或金字塔形。在替代实施方案中,阵列18包括不同微针的混合物。例如,阵列18可包括具有各种长度、直径或形状的微针。

腔室25包括可滑动地安装在腔室25中的塞子。塞子被配置成在腔室25内滑动以将疼痛减轻剂引向微针19,并从而将疼痛减轻剂排出装置10。可替代地,腔室25可以由柔性材料制成,使得当朝向注射部位对腔室25施加力时,疼痛减轻剂通过微针19排出腔室25。在另一替代实施方案中,腔室25包括可裂膜或薄膜。在使用中,腔室25被加压并且可裂膜破裂,使得疼痛减轻剂可以流过空心微针19。在再另一替代实施方案中,腔室25由柔软材料制成,并且在使用中,主针17刺穿腔室25,使得疼痛减轻剂可朝向微针19释放。
如图2B所示,可以在腔室25与空心微针19之间设置诸如压敏膜27等的渗透性膜,以在注射疼痛减轻剂之前保留疼痛减轻剂。在使用中,疼痛减轻剂可以按各种方式流过膜27,例如通过扩散和/或流体动力学地流过。当疼痛减轻剂通过扩散流动时,疼痛减轻剂首先溶解在膜27中。腔室25与膜27之间的化学势的梯度确保了疼痛减轻剂从腔室25朝向膜27扩散,直到腔室25中和膜27中的化学势变得相等,即直到在腔室25与膜27之间的界面处到达平衡。疼痛减轻剂还可以流体动力学地或通过对流朝向膜27流动。膜27可以是用于微过滤和/或超过滤的类型的膜,诸如用于从溶液中分离大分子的膜、从分散体中分离胶体的膜或用于去除细菌的膜。在此过程中,保留在膜27上的疼痛减轻剂颗粒或分子在膜27上形成浆状物质或滤饼。膜27的这种阻塞可能妨碍过滤以及疼痛减轻剂朝向微针19的流动。可以通过使用错流过滤来减少这种阻塞,其中疼痛减轻剂相对于膜27的渗透侧或背面以正压力沿膜27的正面切向地流动。小于膜孔径的一部分疼痛减轻剂颗粒作为渗透物或滤液通过膜27,并且其余部分作为渗余物保留在膜27的正面。正面的切向流产生剪切应力,该剪切应力使滤饼破裂并减少结垢。
阵列18相对于外壳11布置,使得微针19的近端从装置10突出,并且使得一旦装置10被放置在注射区域上,微针19就穿透皮肤和/或内部组织和/或患者血管壁。在本实施方案中,阵列18布置在装置10中,使得微针19相对于外壳11采取至少两个不同的位置。具体地,阵列18可相对于外壳11在缩回位置与展开位置之间移动。在缩回位置,阵列18缩回到装置10中,即微针19至少部分地缩回到外壳11内。在展开位置,阵列18从装置10突出,即微针19的尖端从外壳11的近侧区域20突出。在替代实施方案中,阵列18可相对于外壳11固定。
激活机构14设置在外壳11中。激活机构14被配置成在储存位置与使用位置之间驱动主针17并且驱动药筒15内的活塞23。同样地,激活机构14被配置为在缩回位置与展开位置之间驱动阵列18,并且驱动腔室25内的塞子。具体而言,激活机构14被配置成在将主针17从储存位置朝使用位置驱动之前将阵列18从缩回位置朝展开位置驱动,使得在药剂注射之前将疼痛减轻剂注射到患者体内。激活机构14被适配成在疼痛减轻剂已经注射到患者体内之后的预定时间段结束时驱动主针17。例如,根据皮肤类型、注射部位在体内的位置、药剂和/或药剂的压力,疼痛减轻剂注射结束与主针17的触发之间的时间段包括在一秒或多秒与1分钟之间。为此,激活机构14可以包括计时器,该计时器被配置为在疼痛减轻剂注射之后将主针17的触发延迟预定时间。这可以在药剂注射之前提供足够的时间供疼痛减轻剂在患者体内生效,例如以麻醉注射部位。通过按下按钮22来触发激活机构14。
现在将描述根据本发明的药剂注射装置10的操作。
最初,主针17处于储存位置,即主针17缩回到外壳11内。阵列18处于缩回位置,即微针19的尖端缩回到外壳11内。
在使用中,帽组件12被拉离外壳11,并且装置10被放置在注射部位上的患者皮肤上。然后,按下按钮22并触发激活机构14。激活机构14驱动阵列18,使得阵列18从缩回位置朝展开位置移动。当阵列18朝展开位置移动时,微针19朝注射部位移动,直到微针19到达并穿透患者的皮肤。覆盖阵列18的消毒涂层在疼痛减轻剂注射之前对注射区域进行消毒。然后,激活机构14驱动在腔室25内朝装置10的近侧极端滑动的塞子,以通过膜27并通过微针19将疼痛减轻剂排出到患者体内,例如进入患者的皮肤和/或内部组织和/或血管壁。
