背景技术
药物注射装置可采用各种形式。一种形式的注射装置使用药物药筒。典型情况下,患者在注射前将双头针连接到药筒,从而用双头针的近端尖刺穿药筒的密封件。
尽管药筒相对于注射筒可提供处理和储存上的优势,但它们并非没有缺点。例如,将针附接到药筒上需要额外的步骤。对于灵巧受限、协调性差或者他们的手失去一定程度的感觉的患者来说,这一步骤可能会有问题。即使有这样的缺点,在某些情况下,仍希望提供一种注射装置,其中针保持与药物分开,直至患者希望开始注射的时刻。本申请所述的注射装置旨在克服与现有装置相关的一个或多个问题。

具体实施方式
本公开的实施例提供了一种用于将药物药筒插入注射装置的可枢转药筒保持器中的机构。然后,在可枢转药筒保持器被闭合之后,可由患者或护理人员将注射装置的针插入到含有药物的药筒中以供注射。该机构允许药物药筒保持密封,直到用户希望开始注射时。
这些实施例的优点在于,容纳在药筒内的药物在装置的储存过程中不与针持续接触。这可防止药物在针的尖端周围堵塞。
当可枢转的药筒保持器打开时,所述可枢转的药筒保持器为使用者插入药筒提供了引导。
在可枢转的药筒保持器已经闭合之后将针插入药物药筒中的机构的自动化也减少了在注射之前使用者对针的操作量。实际上,在下面描述的一些实施例中,在针插入药物药筒中时,使用者不接触针。在一些实施例中,防止针移动,直至可枢转的药筒保持器已被闭合。
在本公开的实施例中,针起始联接到装置的帽,而在储存期间(即在制造之后和在使用之前)被隔离。
储存该装置并随后将针插入药物药筒的步骤可在不暴露针的情况下进行。
如本文所述的药物输送装置可被构造成将药物注射到患者体内。例如,输送可为皮下的、肌内的或静脉内的。这样的装置可由患者或护理人员(例如护士或医师)操作,并且可包括各种类型的安全注射筒、笔式注射器或自动注射器。该装置可包括需要在使用前刺穿密封安瓿的、基于药筒的系统。用这些各种装置输送的药物的体积可在约0.5ml至约2ml的范围内。另一种装置可包括大体积装置(“LVD”)或贴片泵,其被构造为在一段时间(例如,约5、15、30、60或120分钟)上附着于患者的皮肤,以输送“大”体积的药物(通常约2ml至约10ml)。
结合特定药物,目前描述的装置也可被定制,以在要求的规范内操作。例如,该装置可被定制成在一定时间段(例如,对于自动注射器约3秒至约20秒,对于LVD约10分钟至约60分钟)内注射药物。其它的规范可包括低程度或最低程度的不适,或某些与人为因素、保质期、失效期、生物相容性、环境考虑等有关的条件。这些变化可由于各种因素而产生,例如粘度在约3cP至约50cP范围内的药物。因此,药物输送装置通常会包括尺寸从约25号到约31号规格(Gauge)的空心针。常见的尺寸是27号和29号。
本文所述的输送装置还可包括一个或多个自动化功能。例如,针插入、药物注射和针退回中的一个或多个可自动化。用于一个或更多个自动化步骤的能量可由一个或多个能量源提供。能量源可包括例如机械能、气动能、化学能或者电能。例如,机械能量源可包括弹簧、杆件、弹性体或者储存或释放能量的其它机械机构。一个或多个能量源可组合成为单个装置。装置可进一步包括齿轮、阀,或将能量转换成为装置的一个或多个部件的移动的其它机构。
自动注射器的一个或多个自动化功能每一个可经由激活机构来激活。这样的激活机构可包括按钮、杆件、针套筒或其它激活部件中的一个或多个。自动化功能的激活可为单步或多步过程。即,使用者可需要激活一个或多个激活部件以产生自动化功能。例如,在单步过程中,使用者可将针套筒压靠于它们的身体以引起药物的注射。其它装置可需要自动化功能的多步激活。例如,使用者可需要压下按钮并使针护罩退回以引起注射。
另外,一个自动化功能的激活可激活一个或更多个后续自动化功能,由此形成激活序列。例如,第一自动化功能的激活可激活针插入、药物注射和针退回中的至少两个。一些装置也可需要特定步骤顺序,以引起一个或多个自动化功能。其它装置可通过一系列独立步骤操作。
一些输送装置可包括安全注射筒、笔式注射器或者自动注射器的一个或多个功能。例如,输送装置可包括构造成自动地注射药物的机械能量源(如通常见于自动注射器中的)和剂量设定机构(如通常见于笔式注射器中的)。
根据本公开的一些实施例,示例性药物输送装置40在图1和2中示出。如上所述的装置10构造成将药物注射到患者体内。装置10包括主体11,主体11通常容纳包含要注射的药物的储存器(例如,注射筒)和促进输送过程的一个或多个步骤所需的部件。装置10还可包括帽组件13,帽组件13能够以可拆卸方式安装到主体11。通常,使用者必须先将帽13从主体11除去,然后才可操作装置10。
如所示的,主体11是大体圆筒形,并且具有沿着纵向轴线X大致恒定的直径。主体11具有远侧区120和近侧区121。术语“远侧”指距注射部位较近的位置,而术语“近侧”指距注射部位较远的位置。

插入的另一形式是“自动化”的,由此针17相对于主体11移动。该插入能够通过套筒14的移动或者通过另一形式的激活(例如,激活按钮22)来触发。按钮可位于主体11的近端。但是,在其它实施例中,按钮22能够位于在主体11的侧面上。
其它手动特征或自动特征可包括药物注射或者针退回,或包括两者。注射是塞头或活塞从注射筒(未示出)内的近侧位置向注射筒内的更远侧位置移动以迫使来自注射筒的药物通过针17的过程。在一些实施例中,驱动弹簧(未示出)在激活装置10之前处于压缩下。驱动弹簧的近端可固定在主体11的近侧区121内,并且驱动弹簧的远端可构造成对活塞的近侧表面施加挤压力。在激活之后,储存在驱动弹簧中的能量的至少一部分可被施加到活塞的近侧表面。此挤压力可作用在活塞上,使其沿远侧方向移动。该远侧移动起作用以压缩注射筒内的液体药物,将其压出针17。
在注射之后,针17可退回到套筒14或主体11内。退回可在套筒14随着使用者从患者身体除去装置10而在远侧方向上移动时发生。这可在针17保持相对于主体11以固定方式定位时发生。一旦套筒14的远端已经移过针17的远端,并且针17被覆盖,则可锁定套筒14。该锁定可包括锁定套筒14相对于主体11的任何近侧移动。
针退回的另一形式可在针17相对于主体11移动时发生。如果主体11内的注射筒相对于主体11在近侧方向上移动,则可发生该移动。该近侧移动能够通过使用位于远侧区120中的退回弹簧(未示出)实现。压缩的退回弹簧在被激活时,能够提供足够力至注射筒,使注射筒在近侧方向上移动。在足够退回之后,可用锁定机构锁定针17和主体11之间的任何相对移动。另外,装置10的按钮或其它部件可根据需要被锁定。

图2是图1中所示的装置10的横截面图。装置10还包括大致管状的针套筒14,针套筒14位于主体11的内部并与其同轴延伸。药筒保持器12容纳药物药筒15。装置10包括托架16,托架16布置成当药物药筒15已经插入后使用者闭合药筒保持器12时接收药筒保持器12。装置10可在药物药筒15已经插入的状态下在适当的包装中提供给使用者。作为替代,药物药筒15可分开提供,以在开始注射之前由使用者插入。

图4A是装置10的容纳药筒保持器12、药物药筒15和载架16的部分的横截面图。图4B示出了枢转点23,药筒保持器12围绕该枢转点23相对于载架16枢转,载架16相对于针套筒14固定。图4C示出了将载架16固定到主体11的夹子24。如此,枢转点位于横向于装置10的纵向轴线X朝向装置10的近端的轴承轴线上。
图5A是药筒保持器12和药筒15的特写图。药筒保持器12包括药筒保持器夹子25。夹子25具有锥形表面,当药筒保持器12闭合时,该锥形表面允许夹子25滑过载架16。在闭合药筒保持器12之前,夹子25提供足够阻力,以向用户提供触觉反馈,使得使用者不太可能意外地闭合药筒保持器12。夹子25也具有阶梯表面(见图5B),从而在药筒保持器12已经移动到闭合位置之后,夹子25阻止药筒保持器12向图1所示的打开位置的随后枢转移动。如此,闭合药筒保持器12的步骤是不可逆的。
图6A示出了药筒保持器12的近端的特写。药筒保持器12包括端部止挡26,用以支撑药物药筒15的近端。端部止挡26限定活塞在开始注射之前的位置。
图6B示出了药筒保持器12的远端的特写。药筒保持器12包括远端夹子27,用以支撑药物药筒15的远端。
图7示出了装置10的远端的特写横截面图。帽12包括臂28,每个臂28在其近端处设有扩展的端部或钩28a。每个臂28在内表面上由针套筒14支撑,并且在外表面上由针保持器18支撑。由于钩28a阻止帽12被沿轴向方向拉动,所以防止了从装置10移除帽12。如下面所解释的,只有在针17和针保持器18已经插入药物药筒15中之后,才能够移除帽。提供了针护罩夹子33,以确保当帽12被移除时,针护罩19也被移除。
图8A示出了针组件,其包括针保持器18、针护罩19、固体泡沫29、分隔件30、引导链接部31、臂34以及针(图中未示出)。针护罩19遮蔽针17的远端以提供无菌性,直到它在注射之前被移除为止。提供固体箔29以保护针17的近端,并为其提供无菌性。分隔件30可由泡沫形成,并且被设置成保护固体箔29免受针17损坏。引导链接部31被设置用于引导针保持器在套筒14中的轴向移动。
图8B是针组件的横截面图。在针保持器18和针护罩19之间设置密封连接32。通过固体箔29确保针17的近端的无菌,该固体箔29可被焊接或粘合到针保持器18。
图9示出了弹簧已被释放之前针套筒14和针保持器18之间的接口。臂34的近端由位于针套筒14上的突起部35支撑。突起部35防止针保持器18朝向药物药筒15的过早轴向移动。
图10A示出了当药筒保持器12处于打开位置时,位于药筒保持器12的远端上的凸耳36。图10B和图10C示出了在药筒保持器12已经移动到闭合位置之后的装置10。凸耳36推动臂34,这又使得臂34从突起部35脱离,由此释放弹簧20。在图10C中,出于例示目的已经移除了一些部件。
图11示出了在药筒保持器12闭合之前装置10的远端的横截面图。弹簧20处于其预应力位置。此外,由于帽12由针保持器18和针套筒14保持就位,所以帽12不能被移除。
图12示出了药筒保持器12已经闭合之后的装置10。如以上参照图10B所解释的,弹簧20被释放。弹簧20的释放引起针保持器被推向装置10的近端。针保持器18的移动引起分隔件30的压缩。引起针17的近端刺穿药物药筒15的隔膜37。此外,帽12的臂28不再由针保持器18支撑。因此,可从装置上移除帽12。一旦帽已被移除,即可开始注射。
注射可通过将装置10的远端推靠于患者的皮肤来开始,这引起针17的远端的暴露。
图13示出了根据替代实施例的装置100的简化横截面图。装置100包括:可枢转的药筒保持器101,其构造用以接收药筒15;和能沿轴向移动的针保持器102,其保持针17。装置100包括载架103,载架103构造成以在其闭合位置中接收可枢转的药筒保持器101。可枢转的药筒保持器101具有钩104,该钩104可由设置在载架103中的凹进105接收。一旦可枢转的药筒保持器101已经闭合,则钩104由凹进105接收并且不可能被移除。因此,关闭可枢转的药筒保持器101是不可逆的步骤。
针套筒14设有锁定构件106,该锁定构件106与设置在载架103中的对应的凹进接合。如图13中所示,针套筒14可不相对于装置100的载架103或主体移动。药筒保持器101具有解锁构件107,用于将针套筒14从载架103分离。
图14示出了在可枢转的药筒保持器101已经闭合之后的装置100。药筒保持器101可设有狭缝(未示出),以使其颈部能够配合穿过载架103而到达图14所示的位置。钩104被凹进105接收,以将可枢转的药筒保持器101固定到载架103。解锁构件107推动锁定构件106,使得其不再与载架103接合。如图14中所示,载架103和主体现在可相对于针套筒14沿轴向移动。
装置100包括帽108。使用者可将帽朝着装置100的近端沿轴向推动,以使针刺穿药筒15的隔膜。然后,可以与关于前述实施例所讨论的类似的方式开始注射。
尽管已经关于自动注射器描述了本发明的实施例,应该牢记的是,本发明也适用于替代的注射装置,例如注射筒、笔式注射器、手动注射器、脊柱注射系统等。用于将针附接到药物药筒的机构可用在其中合意的是将针保持与药物分开直至即将注射前的任何注射装置中。
术语“药物”或“药剂”在本文中同义使用,并且描述了含有一种或多种活性药物成分或其药学上可接受的盐或溶剂化物和任选的药学上可接受的载体的药物制剂。最广义地说,活性药物成分(“API”)是对人或动物具有生物学效应的化学结构。在药理学中,药物或药剂用于疾病的治疗、治愈、预防或诊断,或用于以其他方式增强身体或精神健康。药物或药剂可在有限的持续时间使用,或定期地用于慢性疾病。
如下文所述,药或药物可包括至少一种API或其组合,以多种形式的制剂用于治疗一种或多种疾病。API的实例可包括具有500Da以下分子量的小分子;多肽;肽和蛋白(例如激素、生长因子、抗体、抗体片段和酶);糖和多糖;及核酸、双链或单链DNA(包括裸和cDNA)、RNA、反义核酸如反义DNA和RNA、小干扰RNA(siRNA)、核酶、基因和寡核苷酸。核酸可并入分子递送系统如载体、质粒或脂质体。还涵盖一种或多种药物的混合物。
术语“药物输送装置”应涵盖被构造成将药物分配到人或动物体内的任何类型的装置或系统。无限制地,药物输送装置可为注射装置(例如,注射筒、笔型注射器、自动注射器、大体积装置、泵、输注系统、或被构造成用于眼内、皮下、肌肉内、或血管内输送的其它装置)、皮肤贴片(例如,渗透性、化学品、微型针)、吸入器(例如,用于鼻或肺的)、可植入装置(例如,药物或API涂层支架、胶囊)、或用于胃肠道的供给系统。本文描述的药物与包括针(例如,具有24或更高规格号的皮下针)的注射装置一起可为特别有用的。
药物或药剂可被包含在适于与药物输送装置一起使用的初级包装或“药物容器”内。药物容器可为例如药筒、注射筒、存储器、或被构造成为储存(例如,短期或长期储存)一种或多种药物提供适当的腔室的其它固定或柔性容器。例如,在某些情况下,腔室可被设计成储存药物至少一天(例如,1天至至少30天)。在某些情况下,腔室可被设计成储存药物约1个月至约2年。储存可在室内温度(例如,约20℃)或冷冻温度(例如,约-4℃至约4℃)进行。在某些情况下,药物容器可为或可包括双腔室药筒,所述双腔室药筒被构造成分别储存待施用的药物制剂的两种以上组分(例如,API和稀释剂,或两种不同的药物),每个腔室一种组分。在这样的情况下,双腔室药筒的两个腔室可被构造成允许药物或药剂的两种以上组分之间在分配到人或动物体内之前和/或在分配到人或动物体内期间进行混合。例如,两个腔室可被构造成使得它们彼此流体连通(例如,借助两个腔室之间的导管)并且当在分配之前使用者需要时允许混合两种组分。可替代地或另外,两个腔室可被构造成允许在这些成分正被分配到人或动物体内时进行混合。
包含在如本文所述的药物输送装置中的药物或药剂可用于治疗和/或预防许多不同类型的医学病症。病症的例子包括例如糖尿病或与糖尿病相关的并发症,例如糖尿病性视网膜病、血栓栓塞病症如深静脉或肺血栓栓塞症。病症的其它实例是急性冠状动脉综合征(ACS)、心绞痛、心肌梗塞、癌症、黄斑变性、炎症、枯草热、动脉粥样硬化和/或类风湿性关节炎。API和药物的实例是在手册如Rote Liste 2014中所述的那些,例如但不限于主要组12(抗糖尿病药物)或86(肿瘤药物),以及Merck Index,第15版中所述的那些。
用于治疗和/或预防1型或2型糖尿病或与1型或2型糖尿病有关的并发症的API的实例包括胰岛素,例如人胰岛素或者人胰岛素类似物或者衍生物,胰高血糖素样肽(GLP-1),GLP-1类似物或者GLP-1受体拮抗剂或其类似物或衍生物,二肽基肽酶-4(DPP4)抑制剂,或者其药学上可接受的盐或溶剂合物,或其任何混合物。如本文使用的,术语“类似物”和“衍生物”涉及与原始物质在结构上充分相似以具有基本上相似功能或活性(例如,治疗有效性)的任何物质。特别是,术语“类似物”是指具有分子结构的多肽,所述分子结构可通过缺失和/或交换天然存在的肽中存在的至少一个氨基酸残基和/或通过添加至少一个氨基酸残基,而在形式上源自天然存在的肽的结构,例如人胰岛素的结构。添加和/或交换的氨基酸残基可为可编码氨基酸残基,或其他天然存在的残基,或纯合成氨基酸残基。胰岛素类似物也被称为“胰岛素受体配体”。特别是,术语“衍生物”是指具有形式上可源自天然存在的肽的结构的分子结构的多肽,例如源自人胰岛素的结构,其中一个或多个有机取代基(例如脂肪酸)与一个或多个氨基酸结合。任选地,天然存在的肽中存在的一个或多个氨基酸可已被缺失和/或被其他氨基酸替换,包括不可编码的氨基酸,或者氨基酸(包括不可编码的氨基酸)已被添加至天然存在的肽。
示例性的胰岛素类似物是Gly(A21)、Arg(B31)、Arg(B32)人胰岛素(甘精胰岛素);Lys(B3)、Glu(B29)人胰岛素(赖谷胰岛素);Lys(B28)、Pro(B29)人胰岛素(赖脯胰岛素);Asp(B28)人胰岛素(门冬胰岛素);人胰岛素,其中位置B28处的脯氨酸替换为Asp、Lys、Leu、Val或Ala且其中位置B29处的Lys替换为Pro;Ala(B26)人胰岛素;Des(B28-B30)人胰岛素;Des(B27)人胰岛素和Des(B30)人胰岛素。

示例性的寡核苷酸是例如mipomersen sodium(米泊美生钠)一种用于治疗家族性高胆固醇血症的降低胆固醇的反义治疗。
示例性的DPP4抑制剂是Vildagliptin(维达列汀)、Sitagliptin(西他列汀)、Denagliptin(地那列汀)、Saxagliptin(沙格列汀)、Berberine(小檗碱)。
示例性的激素包括垂体激素或下丘脑激素或调节性活性肽及其拮抗剂,如促性腺激素(Gonadotropine)(促滤泡素(Follitropin)、促黄体激素(Lutropin)、绒毛膜促性腺激素(Choriongonadotropin)、促生育素(Menotropin))、Somatropine(生长激素)(促生长激素(Somatropin))、去氨加压素(Desmopressin)、特利加压素(Terlipressin)、戈那瑞林(Gonadorelin)、曲普瑞林(Triptorelin)、亮丙瑞林(Leuprorelin)、布舍瑞林(Buserelin)、那法瑞林(Nafarelin)和戈舍瑞林(Goserelin)。

本文使用的术语“抗体”指免疫球蛋白分子或其抗原结合部分。免疫球蛋白分子抗原结合部分的实例包括F(ab)和F(ab')2片段,其保留结合抗原的能力。抗体可为多克隆、单克隆、重组、嵌合、去免疫或人源化、全长人、非人(例如鼠类)或单链抗体。在一些实施方案中,抗体具有效应物功能且可固定补体。在一些实施方案中,抗体不具有或具有减少的结合Fc受体的能力。例如,抗体可为同型或亚型、抗体片段或突变体,其不支持与Fc受体的结合,例如其具有诱变或缺失的Fc受体结合区。术语抗体还包括基于四价双特异性串联免疫球蛋白(TBTI)的抗原结合分子和/或具有交叉结合区域定向(CODV)的双重可变区抗体样结合蛋白。

术语“互补决定区”或“CDR”指在重链和轻链多肽两者可变区内的短多肽序列,其主要负责介导特异性抗原识别。术语“框架区”指在重链和轻链多肽两者可变区内的氨基酸序列,其并非CDR序列,且主要负责维持CDR序列的正确定位以允许抗原结合。如本领域已知,尽管框架区它们自己通常不直接参与抗原结合,一些抗体框架区内的一些残基可直接参与抗原结合或可影响CDR中一个或多个氨基酸与抗原相互作用的能力。
抗体的实例为抗PCSK-9mAb(例如阿利库单抗(Alirocumab))、抗IL-6mAb(例如Sarilumab)和抗IL-4mAb(例如Dupilumab)。
也涵盖本文所述的任何API的药学上可接受的盐用于药物输送装置中的药物或药剂。药学上可接受的盐例如是酸加成盐和碱式盐。

Claims (12)
1.药物注射装置,包括:
针保持器,所述针保持器保持针,
可枢转的药筒保持器,所述可枢转的药筒保持器能够从打开状态枢转到闭合状态,
其中所述可枢转的药筒保持器布置成在所述打开状态下接收药物药筒,并且其中当所述可枢转的药筒保持器处于所述闭合状态时,所述针保持器能够沿轴向朝向所述可枢转的药筒保持器移动,并且
其中所述可枢转的药筒保持器向所述闭合状态的移动引起所述针保持器在近侧方向上朝向所述药筒保持器的轴向移动。
2.根据权利要求1所述的装置,其中所述可枢转的药筒保持器能够围绕轴承轴线朝向所述近端枢转。
3.根据任一项先前权利要求所述的装置,还包括针套筒和预应力的弹簧,所述预应力的弹簧联接到所述针套筒和所述针保持器,
其中所述可枢转的药筒保持器向所述闭合状态的移动引起所述预应力的弹簧的释放,由此引起所述针保持器朝向所述药筒保持器的所述轴向移动。
4.根据任一项先前权利要求所述的装置,还包括帽和针套筒,其中所述帽联接到所述针套筒并且包括夹子,所述夹子防止所述帽的移去,直至所述弹簧被释放。
5.根据任一项先前权利要求所述的装置,还包括布置成防止所述可枢转的药筒保持器的意外闭合的一个或多个夹子。
6.根据任一项先前权利要求所述的装置,还包括夹子,所述夹子构造成在闭合所述可枢转的药筒保持器之后,将所述可枢转的药筒保持器固定到所述装置的主体。
7.根据任一项先前权利要求所述的装置,还包括用于保护所述针的远端的针护罩。
8.根据任一项先前权利要求所述的装置,还包括用于保护所述针的近端的固体箔。
9.根据任一项先前权利要求所述的装置,还包括用以保护所述固体箔的可压缩分隔件。
10.根据任一项先前权利要求所述的装置,其中所述可枢转的药筒保持器容纳包含药物的药筒。
11.根据任一项先前权利要求所述的装置,其中所述装置是自动注射器。
12.装载药物注射装置的方法,所述药物注射装置包括保持针的针保持器,所述方法包括:
将药物药筒插入到可枢转的药物药筒保持器中,所述药物药筒在其一端上具有可穿透的隔离物;并且
闭合所述可枢转的药物药筒保持器;
其中闭合所述可枢转的药物药筒保持器引起所述针保持器朝向所述药筒保持器的轴向移动,并引起所述针穿透所述可穿透的隔离物。
