背景技术

根据某些方面,本发明提供在个体中降低对变应性反应的易感性的方法,该方法包括对有需要的个体施用治疗有效量的IL-4R拮抗剂。在某些实施方案中,术语“有需要的个体”包括对变应性反应易感或处于发展对变应原的变应性反应的高风险的个体。在某些实施方案中,该个体可以处于由于变应原致敏而发展对该变应原的变态反应或变应性应答的高风险。例如,该术语包括显示对一种或多种变应原特异的血清IgE水平提高(“变应原致敏”)的个体。在本发明的背景中,术语“有需要的个体”还包括患有选自特应性皮炎、哮喘、变应性鼻炎、嗜酸细胞性食管炎和食物变态反应的疾病或障碍的个体。术语“个体”还包括具有提高的发展变应性应答的风险的血清总IgE和变应原特异性IgE、或血清趋化因子(例如CCL17或CCL27)的水平提高的个体。本发明提供在易感个体中降低发展变态反应或变应性应答的风险的方法。
根据某些方面,本发明提供在个体中降低血清变应原特异性IgE水平的方法,该方法包括施用治疗有效量的IL-4R拮抗剂。在某些实施方案中,施用IL-4R拮抗剂后,与基线相比,血清变应原特异性IgE水平降低至少10%、20%、30%、40%或50%。
用于检测和/或定量血清生物标志如变应原特异性IgE或总IgE的方法为本领域已知;用于测量这种生物标志的试剂盒可从多种商业来源获得;多种商业诊断实验室也提供提供这类生物标志的测量结果的服务。
例如,PhadiatopTM是引入用于筛查变应性致敏的血清特异性或抗原特异性IgE测定测试的市售变通形式(Merrett等1987,Allergy 17:409-416)。该测试提供对引起常见吸入性变态反应的相关变应原的混合物特异的血清IgE的同时测试。该测试提供定性结果,取决于所获得的荧光反应为阳性或阴性。在患者样品产生高于或等于参考的荧光反应时,指示阳性测试结果。较低荧光反应的患者样品指示阴性测试结果。本发明包括这样的方法,该方法包括选择显示阳性测试结果的个体,并对该个体施用治疗有效量的IL-4R拮抗剂。
本发明还包括用于测定个体是否是施用包含IL-4R拮抗剂的药物组合物有益的适宜个体的方法。例如,如果个体在接受包含IL-4R拮抗剂的药物组合物之前显示一定水平的预示变应原致敏的血清生物标志(例如变应原特异性IgE),则因此将该个体鉴定为施用本发明的药物组合物(包含抗IL-4R抗体的组合物)有益的适宜个体。
根据本发明的某些方面,提供用于预防或治疗变态反应的方法,其包括:(a)选择在治疗前或治疗时显示一定水平的预示变应性致敏的对至少一种变应原特异的IgE的个体;和(b)对该个体施用包含治疗有效量的IL-4R拮抗剂的药物组合物。在某些实施方案中,通过测定变应原特异性IgE水平是否提高来选择患者。通过从患者获取样品进行本领域已知的生物标志测定,来测定或定量变应原特异性IgE的水平。在某些其他实施方案中,通过从患者获取与变应原特异性IgE水平提高相关的信息来选择患者。在本发明的此方面的某些实施方案中,根据IgE或TARC或骨膜蛋白的水平提高选择个体。
如本领域普通技术人员将理解,可以通过将(i)施用IL-4R拮抗剂后确定时间点在个体中测量的生物标志水平与(ii)施用IL-4R拮抗剂前在患者中测量的生物标志水平(即“基线测量结果”)相比较来确定血清生物标志的增加或减少。该测量生物标志的确定时间点可以是例如施用IL-4R拮抗剂后约4小时、8小时、12小时、1天、2天、3天、4天、5天、6天、7天、8天、9天、10天、15天、20天、35天、40天、50天、55天、60天、65天、70天、75天、80天、85天、100天、150天或更长时间。
根据本发明的某些具体实施方案,在施用包含IL-4R拮抗剂(例如抗IL-4R抗体)的药物组合物后,个体可以显示对一种或多种变应原特异的血清IgE水平降低。例如,在施用一个或多个剂量的包含约75、150、200或300mg抗hIL-4R抗体(例如dupilumab)的药物组合物后约第8天、第15天、第22天、第25天、第29天、第36天、第43天、第50天、第57天、第64天、第71天、第85天或第112天,本发明的个体可以显示变应原特异性IgE从基线降低约1%、2%、5%、10%、15%、20%、25%、30%、35%、40%、45%、50%、55%、60%、65%、70%、75%、80%、85%、90%或更多(其中“基线”定义为第一次施用之前个体中的变应原特异性IgE水平)。
白介素-4受体拮抗剂
本发明的方法包括对有需要的个体施用包含白介素-4受体(IL-4R)拮抗剂的治疗组合物。本文所用的“IL-4R拮抗剂”(本文中也称为“IL-4R抑制剂”、“IL-4Rα拮抗剂”、“IL-4R阻断剂”、“IL-4Rα阻断剂”等)是与IL-4Rα或IL-4R配体结合或相互作用并抑制或减弱1型和/或2型IL-4受体的正常生物信号发放功能的任何活性剂。人IL-4Rα具有氨基酸序列SEQID NO:11。1型IL-4受体是包含IL-4Rα链和γc链的二聚体受体。2型IL-4受体是包含IL-4Rα链和IL-13Rα1链的二聚体受体。1型IL-4受体与IL-4相互作用并由IL-4刺激,而2型IL-4受体与IL-4和IL-13二者相互作用并由IL-4和IL-13二者刺激。因此,可用于本发明方法的IL-4R拮抗剂可以通过阻断IL-4介导的信号发放、IL-13介导的信号发放或IL-4和IL-13二者介导的信号发放来发挥功能。本发明的IL-4R拮抗剂因此可以阻止IL-4和/或IL-13与1型或2型受体的相互作用。
IL-4R拮抗剂种类的非限制性实例包括小分子IL-4R抑制剂、抗IL-4R适体、基于肽的IL-4R抑制剂(例如“peptibody”分子)、“receptor-body”(例如包含IL-4R的配体结合结构域成分的改造分子)和特异性结合人IL-4Rα的抗体或抗体的抗原结合片段。本文所用的IL-4R拮抗剂还包括特异性结合IL-4和/或IL-13的抗原结合蛋白。
抗IL-4Rα抗体及其抗原结合片段
根据本发明的某些示例性实施方案,该抗IL-4R拮抗剂是抗IL-4Rα抗体或其抗原结合片段。本文所用的术语“抗体”包括含有四条多肽链(通过二硫键相互连接的两条重(H)链和两条轻(L)链)的免疫球蛋白分子及其多聚体(例如IgM)。在典型抗体中,每条重链包含重链可变区(本文中缩写为HCVR或VH)和重链恒定区。重链恒定区包含CH1、CH2和CH3三个结构域。每条轻链包含轻链可变区(本文中缩写为LCVR或VL)和轻链恒定区。轻链恒定区包含一个结构域(CL1)。VH和VL区可进一步细分为称为互补决定区(CDR)的高变区,中间散布更保守的称为构架区(FR)的区域。每个VH和VL包含三个CDR和四个FR,从氨基端至羧基端按以下顺序排列:FR1、CDR1、FR2、CDR2、FR3、CDR3、FR4。在本发明的不同实施方案中,抗IL-4R抗体(或其抗原结合部分)的FR可以与人种系序列相同,或可以是天然或人工修饰的。可以基于两个或多个CDR的头对头分析来确定氨基酸共有序列。
本文所用的术语“抗体”还包括全抗体分子的抗原结合片段。本文所用的术语抗体的“抗原结合部分”、抗体的“抗原结合片段”等包括特异性结合抗原形成复合物的任何天然存在、可酶促获得、合成或遗传改造的多肽或糖蛋白。抗体的抗原结合片段可以例如用任意适宜的标准技术(如蛋白水解消化或涉及操作和表达编码抗体可变区和可选的恒定区的DNA的重组基因工程技术)从全抗体分子衍生。这种DNA是已知的和/或易于从例如商业来源、DNA文库(包括例如噬菌体抗体文库)获得,或可以合成。DNA可以进行测序和化学操作,或通过使用分子生物学技术例如来将一个或多个可变和/或恒定结构域排列为适宜的构型,或引入密码子、产生半胱氨酸残基、添加或缺失氨基酸等。
抗原结合片段的非限制性实例包括:(i)Fab片段;(ii)F(ab')2片段;(iii)Fd片段;(iv)Fv片段;(v)单链Fv(scFv)分子;(vi)dAb片段;和(vii)包含模拟抗体高变区的氨基酸残基的最小识别单位(例如分离的互补决定区(CDR),如CDR3肽),或受约束的FR3-CDR3-FR4肽。其他改造分子(如结构域特异性抗体、单结构域抗体、结构域缺失抗体、嵌合抗体、CDR移植抗体、双抗体、三抗体、四抗体、微型抗体、纳米抗体(例如单价纳米抗体、二价纳米抗体等)、小模块免疫药物(SMIP)和鲨鱼可变IgNAR结构域)也涵盖在本文所用的表述“抗原结合片段”内。
抗体的抗原结合片段通常将包含至少一个可变结构域。可变结构域可以具有任意大小或氨基酸组成,且通常将包含与一个或多个构架序列相邻或符合读框的至少一个CDR。在具有与VL结构域结合的VH结构域的抗原结合片段中,VH和VL结构域可以相对于彼此以任何适宜的排列定位。例如,可变区可以是二聚的,且包含VH-VH、VH-VL或VL-VL二聚体。备选地,抗体的抗原结合片段可以包含单体VH或VL结构域。

用于本发明方法的抗体可以是人抗体。本文所用的术语“人抗体”旨在包括具有源自人种系免疫球蛋白序列的可变和恒定区的抗体。但是,本发明的人抗体可以例如在CDR中尤其是在CDR3中包含并非由人种系免疫球蛋白序列编码的氨基酸残基(例如通过体外随机或位点特异性诱变或通过体内体细胞突变引入的突变)。但是,本文所用的术语“人抗体”并非旨在包括其中已将源自另一哺乳动物物种如小鼠种系的CDR序列移植在人构架序列上的抗体。

药物组合物
本发明包括这样的方法,该方法包括对患者施用IL-4R拮抗剂,其中IL-4R拮抗剂(例如抗IL-4R抗体)包含在药物组合物内。用适宜的载体、赋形剂和其他提供适宜的转移、递送、耐受等的活性剂配制本发明的药物组合物。大量适当的制剂可见于所有药物化学家已知的处方集中:Remington's Pharmaceutical Sciences,Mack Publishing Company,Easton,PA。这些制剂包括例如粉剂、糊剂、膏剂、凝胶剂、蜡、油、脂、含脂(阳离子型或阴离子型)微囊(如LIPOFECTINTM)、DNA缀合物、无水吸收糊剂、水包油和油包水乳剂、碳蜡(多种分子量的聚乙二醇)乳剂、半固体凝胶和含碳蜡的半固体混合物。参见例如Powell等"Compendium of excipients for parenteral formulations"PDA(1998)J Pharm SciTechnol52:238-311。
按照本发明方法对患者施用的抗体的剂量可以取决于患者的年龄和体型大小、症状、病症、施用途径等而变。通常按照体重或体表面积计算剂量。取决于病症的严重度,可以调整治疗的频率和持续时间。施用包含抗IL-4R抗体的药物组合物的有效剂量和方案可以根据经验确定;例如,可以通过定期评估监测患者进展,并相应地调整剂量。此外,可以用本领域公知的方法(例如Mordenti等,1991,Pharmaceut.Res.8:1351)进行剂量的物种间缩放。可用于本发明背景中的抗IL-4R抗体的具体示例性剂量和涉及抗IL-4R抗体的施用方案在本文中其他地方公开。
多种递送系统是已知的,且可以用于施用本发明的药物组合物,例如脂质体中包封、微粒、微囊、能够表达突变体病毒的重组细胞、受体介导的胞吞作用(参见例如Wu等,1987,J.Biol.Chem.262:4429-4432)。施用方法包括但不限于皮内、肌内、腹腔内、静脉内、皮下、鼻内、硬膜上和口服途径。组合物可以通过任何方便的途径施用,例如通过输注或推注,通过上皮或黏膜上皮吸收,且可以与其他生物活性剂一起施用。

剂量
按照本发明方法对个体施用的IL-4R拮抗剂(例如抗IL-4R抗体)的量通常是治疗有效量。本文所用的短语“治疗有效量”意指导致以下一项或多项的IL-4R拮抗剂的量:(a)预防变态反应;(b)治疗变应性反应或降低变应性反应严重度;(c)降低血清变应原特异性IgE水平;(d)减少变应原致敏;和/或(e)降低对变应性反应的易感性。

包含在单个剂量内的IL-4R拮抗剂的量可以按照毫克抗体/千克患者体重(即mg/kg)表示。例如,IL-4R拮抗剂可以按约0.0001至约10mg/kg患者体重的剂量对患者施用。
联合治疗
根据某些实施方案,本发明的方法包括与IL-4R拮抗剂组合对个体施用一种或多种附加治疗剂。本文所用的表述“与……组合”意指在包含IL-4R拮抗剂的药物组合物之前、之后或与之同时施用该附加治疗剂。术语“与……组合”还包括顺次或同时施用IL-4R拮抗剂和第二治疗剂。

施用方案
本发明包括这样的方法,该方法包括按约每周四次、每周两次、每周一次、每两周一次、每三周一次、每四周一次、每五周一次、每六周一次、每八周一次、每十二周一次的给药频率或更低频率对个体施用包含IL-4R拮抗剂的药物组合物,只要达到治疗反应。在涉及施用包含抗IL-4R抗体的药物组合物的某些实施方案中,可以利用按约75mg、150mg、200mg或300mg的量每周给药一次。
根据本发明的某些实施方案,可在确定时程内对个体施用多个剂量的IL-4R拮抗剂。本发明的此方面的方法包括对个体顺次施用多个剂量的IL-4R拮抗剂。本文所用的“顺次施用”意指在不同时间点,例如在相隔预先确定的间隔(例如小时、天、周或月)的不同天对个体施用每个剂量的IL-4R拮抗剂。本发明包括这样的方法,该方法包括对患者顺次施用单个起始剂量的IL-4R拮抗剂,然后施用一个或多个第二剂量的IL-4R拮抗剂,然后可选地施用一个或多个第三剂量的IL-4R拮抗剂。

在本发明的一个示例性实施方案中,各第二和/或第三剂量在前一个剂量后1至14(例如1、11/2、2、21/2、3、31/2、4、41/2、5、51/2、6、61/2、7、71/2、8、81/2、9、91/2、10、101/2、11、111/2、12、121/2、13、131/2、14、141/2、或更多)周施用。本文所用的短语“前一个剂量”意指在多次施用的序列中,在施用序列中的下一个剂量之前对患者施用的IL-4R拮抗剂剂量无居间剂量。
本发明的此方面的方法可以包括对患者施用任意数目的第二和/或第三剂量的IL-4R拮抗剂。例如,在某些实施方案中,仅对患者施用单个第二剂量。在其他实施方案中,对患者施用两个或多个(例如2、3、4、5、6、7、8或更多个)第二剂量。同样,在某些实施方案中,仅对患者施用单个第三剂量。在其他实施方案中,对患者施用两个或多个(例如2、3、4、5、6、7、8或更多个)第三剂量。
在涉及多个第二剂量的实施方案中,各第二剂量可以按与其他第二剂量相同的频率施用。例如,各第二剂量可以在前一剂量后1至2周对患者施用。类似地,在涉及多个第三剂量的实施方案中,各第三剂量可以按与其他第三剂量相同的频率施用。例如,各第三剂量可以在前一剂量后2至4周对患者施用。备选地,对患者施用第二和/或第三剂量的频率在治疗方案的过程中可以改变。施用频率也可以在治疗过程中由医生在临床检查后根据个体患者的需要调整。

提出以下实施例,为本领域普通技术人员提供如何制备和使用本发明的方法和组合物的完整公开内容和描述,并非旨在限制发明人视为其发明的范围。关于所使用的数据(例如量、温度等),已努力确保准确性,但应说明一些实验误差和偏差。除非另有说明,部分是按重量计的部分,分子量是平均分子量,温度以摄氏度表示,压力为大气压或近大气压。
实施例1:研究Dupilumab在成年中度至严重AD患者中的有效性的临床试验
研究设计和目的
这是一项32周的随机化、双盲、安慰剂对照、平行组研究,评估对成年中度至严重AD患者连续16周每周施用dupilumab的安全性、有效性、生物标志特征、功能性浓度和免疫原性。Dupilumab是全人抗IL-4R抗体,包含含有氨基酸序列SEQ ID NO:9的重链和含有氨基酸序列SEQ ID NO:10的轻链;含有SEQ ID NO:1/2的HCVR/LCVR氨基酸序列对;及含有SEQID NO:3-8的重链和轻链CDR序列。
符合条件的患者按1:1的比例随机接受皮下(SC)dupilumab或SC安慰剂。随机化按疾病严重度(中度vs.严重AD)分层。提供知情同意书后,在筛查就诊时评估患者的研究合格性。符合合格标准的患者进行第1天/基线评估、随机化,并从第1周至第15周接受负荷剂量(400mg SC研究药物)及后续每周研究药物注射(200mg SC)。此期间,患者进行每周评估,大部分通过临床就诊,但一些通过电话联系。取决于患者的偏好和能力,患者和/或护理人员在前5次治疗就诊(就诊2、3、4、5和6)时在研究地点进行关于注射研究药物的培训,随后在就诊7、9、11、13和15时在门诊外施用研究药物,仅需电话联系。在具体门诊就诊时进行安全性、实验室和临床效果评估。治疗期就诊的结束发生在第16周最后一个剂量的研究药物之后,这时评估主要终点。从第18周至第32周每两周进行随访就诊。研究就诊的结束发生在第32周。
根据需要,向患者提供针对AD的挽救治疗(药物和/或光疗)。需要挽救治疗的患者停止研究治疗,但继续遵循研究评估的时间表。在施用任何挽救治疗之前获得有效性测量结果(例如IGA、EASI等)。
在研究期间的多种时间点采集用于临床化学和血液学、药物浓度和抗药抗体的样品。此外,采集1份样品用于DNA分析,多份样品用于RNA分析。
随机分配治疗,以避免将患者分配至特定治疗组的倾向,并最小化治疗组之间关于可影响结果的基线变量的系统性差异。双盲设计旨在最小化由研究人员或患者对治疗分配的了解引起的临床评估和患者报告结果中任何潜在倾向。安慰剂臂提供研究治疗的任何表观效应的可靠参考。分配至dupilumab臂的患者在第1天接受400mg负荷剂量后接受每周一次(qw)200mg。施用研究治疗16周,以稳定化功能性dupilumab的全身浓度。治疗后,所有患者随访16周(即约5个半衰期),以确保最终在研究就诊结束之前完成dupilumab清除(血浆浓度低于定量下限)。

探索性目的是:(1)评估dupilumab对表皮增生的作用;(2)评估dupilumab随时间推移对生物标志的药效动力学(PD)作用;和(3)评估胸腺和活化调节细胞因子(TARC)对EASI反应的预测价值。

探索性终点包括:(1)具有组织学反应的患者的比例,组织学反应包括病灶皮肤表皮增生的显著减少,定义为表皮厚度从基线降低≥40%和/或通过免疫组织化学测定的K16表达的逆转;(2)TARC从基线至第16周的变化;(3)IgE从基线至第16周的变化;(4)变应原特异性IgE至第16周的变化;和(5)基线TARC和IgE在EASI反应上的相关性。
患者选择
目标群体包括用局部药物未充分控制或局部治疗对其不可取(由于例如副作用或安全性风险)的患有中度至严重AD的成人。
纳入标准:为了符合纳入研究的条件,患者必须满足以下标准:(1)男性或女性,18岁或以上;(2)在筛查就诊前已存在至少3年的慢性AD;(3)EASI评分在筛查就诊时≥12,在基线就诊时≥16;(4)IGA评分在筛查和基线就诊时≥3(在0-4IGA量表上);(5)在筛查和基线就诊时AD累及≥10%BSA;(6)患者具有对用局部药物进行的门诊患者治疗反应不足或局部治疗对其不可取(例如由于重要的副作用或安全性风险)的有记录的近期病史(筛查就诊前6个月内)[为了此方案的目的,反应不足表示尽管用中至高功效的局部糖皮质激素治疗(根据需要±局部钙依赖磷酸酶抑制剂),但未能达到和/或维持缓解或低疾病活动状态(例如IGA 0=清除至2=轻度)。为了评估对强化治疗的反应不足,每天应用局部治疗至少28天或产品处方信息推荐的最大持续时间(例如对于强效局部糖皮质激素为14天),以较短的时间为准。每日强化治疗后,根据尽管按较低强度的维持方案应用局部药物(即每周2天)也未能维持低疾病活动状态来确定反应不足。研究人员或患者主治医生评估的超过潜在治疗益处的重要副作用或安全性风险(例如超敏反应、皮肤明显萎缩、全身性效应等,或其危象)。可接受的记录包括记录局部糖皮质激素和/或局部钙依赖磷酸酶抑制剂处方的同期图表注释,治疗结果,基于与患者主治医生沟通的研究人员记录,或在其他形式的记录不可得的情况下由患者提供的病历(例如患者在前6个月内未针对AD就诊)];(7)患者必须在基线就诊前至少7天每天两次应用稳定剂量的局部润肤剂(增湿剂);(8)愿意且能够服从所有门诊就诊和研究相关流程;(9)能够理解和完成研究相关问卷;和(10)提供签字的知情同意书。
排除标准:以下是研究的排除标准:(1)之前参加过dupilumab的临床试验;(2)基线就诊前8周内或5个半衰期内(如果已知)(以较长者为准)的研究药物治疗;(3)基线就诊前4周内的以下治疗,或者患有在研究人员看来在研究治疗的前4周内可能需要这种(类)治疗的任何病症——全身糖皮质激素、免疫抑制/免疫调节药物(例如环孢霉素、吗替麦考酚酯(mycophenolate-mofetil)、IFNγ、Janus激酶(JAK)抑制剂、硫唑嘌呤或氨甲喋呤(methotrexate))或AD光疗;(4)基线就诊前1周内的局部糖皮质激素、他克莫司和/或吡美莫司治疗;(5)以下生物制品治疗:包括但不限于利妥昔单抗(rituximab)的任何细胞剔除剂:基线就诊前6个月内,或直至淋巴细胞和CD19+淋巴细胞计数回至正常,以较长者为准;英夫利昔单抗(infliximab)、阿达木单抗(adalimumab)、戈利木单抗(golimumab)、赛妥珠单抗(certolizumab pegol)、阿巴西普(abatacept)、益赛普(etanercept)、阿那白滞素(anakinra):基线就诊前16周内针对任何适应症,或5年内针对皮肤适应症;其他生物制品:基线就诊前5个半衰期(如果已知)或16周内,以较长者为准;(6)筛查期内起始处方增湿剂或含诸如神经酰胺、透明质酸、尿素或聚丝蛋白的添加剂的增湿剂的AD治疗(如果在筛查就诊前起始,则患者可继续使用稳定剂量的这类增湿剂);(7)基线就诊前4周内定期使用(每周2次以上就诊)日光浴浴房(tanning booth)/居室;(8)研究期间计划或预计使用任何禁止的药物和手术;(9)基线就诊前12周内的活(减毒)疫苗治疗;(10)筛查就诊前4周内需要用全身抗生素、抗病毒剂、抗寄生虫剂、抗原生动物剂或抗真菌剂治疗的慢性或急性感染,或筛查就诊前1周内的浅表皮肤感染;(11)已知或疑似免疫抑制,包括侵入性机会性感染(结核、组织胞浆菌病、利斯特菌病、球孢子菌病、肺囊虫病、曲霉病)史,尽管感染消退,或异常频率的其他复发性感染,或暗示无免疫应答状态的长期感染;(12)已知的人免疫缺陷病毒(HIV)感染史或筛查时为HIV血清阳性;(13)筛查就诊时乙型肝炎病毒表面抗原(HBsAg)、乙型肝炎病毒核心抗体(HBcAb)或丙型肝炎抗体为阳性或不明确;(14)筛查就诊时转氨酶(ALT和/或AST)提高至正常上限(>3xULN)的3倍以上;(15)基线就诊前12个月内的临床寄生虫病史,经过治疗的阴道毛滴虫病除外;(16)存在可干扰研究评估的皮肤共患病;(17)基线就诊前5年内的恶性肿瘤史,完全治疗的原位宫颈癌及完全治疗并消退的非转移性皮肤鳞状或基底细胞癌除外;(18)非恶性淋巴增生性障碍史;(19)高风险寄生虫感染,如定居在寄生虫病流行区域或近期到寄生虫病流行区域旅行(基线就诊前12个月内),其中情况符合寄生虫暴露(例如长期停留,农村或贫民区,缺乏自来水,进食未烹煮、烹煮不充分或可能以其他方式受污染的食物,与携带者和载体亲密接触等),除非随后的医疗评估(例如粪检、验血等)排除了寄生虫感染/侵袭的可能性;(20)筛查就诊前2年内的酗酒或吸毒史;(21)对患者参与研究有不利影响的严重共存疾病。实例包括但不限于预期寿命短的患者,糖尿病未得到控制的患者(HbA1c≥9%),心血管病症(例如按照纽约心脏协会分类的III或IV期心力衰竭)、严重肾脏病症(例如透析患者)、肝胆病症(例如B或C类Child-Pugh)、神经病症(例如脱髓鞘疾病)、活动性主要自身免疫病(例如狼疮、炎性肠病、类风湿性关节炎等)、其他严重的内分泌、胃肠、代谢、肺或淋巴疾病患者;(22)筛查时提示新的和/或未充分理解的疾病的任何其他医学或心理病症,包括相关实验室异常,其可以由于研究患者参与此临床试验而对他/她是不合理的风险,可以使患者的参与不可靠,或干扰研究评估。这包括对局部麻醉剂过敏、出血障碍、使活检手术不可取的抗凝剂治疗或其他病症;(23)患者参与此研究期间有计划中的主要外科手术;(24)研究期间怀孕或哺乳的女性或计划怀孕或哺乳的女性;和(25)有生育能力和性生活的女性不愿意使用适当的避孕措施。
研究治疗
患者在第1天接受400mg dupilumab皮下负荷剂量,然后从第1周至第15周每周(qw)200mg。使用安慰剂的患者在第1天接受负荷剂量,然后从第1周至第15周每周接受皮下剂量的安慰剂。患者需从第-7天至第8天每天两次应用局部润肤剂。
流程和评估
通过AD疾病严重度评分、生活质量(QQL)问卷、瘙痒评估和患者报告结果来评估dupilumab在此群体中的功效。AD严重度评分包括美国专利申请公开号US2014/0072583(在此以其整体引入作为参考)中所述的AD相关临床参数,如湿疹区域和严重度指数(EASI)、研究人员总体评估(IGA)、瘙痒数值评分量表(NRS)、体表面积(BSA)、5-D瘙痒、特应性皮炎评分(SCORAD)、患者导向湿疹测量(POEM)和总体个体病征评分(GISS)。生活质量(QQL)问卷包括US2014/0072583(在此以其整体引入作为参考)中所述的患者对疾病状态的总体评估、皮肤生活质量指数(DLQI)、POEM、EQ-5D、Itchy QoL和医院焦虑和抑郁量表(HADS)。进行皮肤屏障功能测试,AD区域照相,并采集用于探索性微生物菌群分析的皮肤拭子样品。通过评价TEAE、详细病历、全面体检、生命体征、心电图(ECG)和临床实验室测试来评估dupilumab在此群体中的安全性。从获取知情同意书时至研究结束收集并行的药物和手术。随研究进行审核盲安全性数据。在预定时间点采集血样进行药物浓度和抗dupilumab抗体水平检测。采集研究样品和用于探索性生物标志分析的样品。还采集皮肤活检样品用于探索性生物标志分析。
统计分析
以治疗和随机化分层(中度vs.严重)用协方差分析(ANCOVA)模型分析主要和次要连续变量,相关终点基线值作为协变量。在使用挽救药物后或在患者退出研究后将有效性数据设为缺失。然后用末次观测值结转(LOCF)法输入所有缺失值。EASI和瘙痒NRS报告为从基线至第16周的最小二乘(LS)均值(标准误[SE])百分比变化,从LOCF法导出。
结果
患者安排和基线特征:将54名患者随机化至安慰剂(n=27)或dupilumab 200mgqw(n=27)。在治疗组之间平衡基线人口统计学和临床特征(表1)。超过75%的患者使用过先前药物,包括抗组胺、局部糖皮质激素(按功效分为I、II和III组)和阻塞性气道病药物。接受安慰剂的患者使用过糖皮质激素的比例高于接受dupilumab的患者。

有效性:与安慰剂(SE)相比,dupilumab治疗显著改善(降低)从基线至第16周的EASI评分:-75.2%(8.15)vs-5.8%(8.16);P<0.0001(图1)。与安慰剂(SE)相比,dupilumab治疗还使每周峰值瘙痒NRS从基线至第16周显著降低:-51.5%(10.2)vs-6.3%(10.0);P=0.0027(图2)。与安慰剂组[1/27(3.7%)]相比,更高比例的dupilumab组患者[14/27(51.9%)]至第16周达到IGA降低≥2分。与安慰剂组中没有患者达到相比,37%的dupilumab组患者至第周达到0(清除)或1(几乎清除)的评分。如EASI-50(21/27[77.8%]dupilumab versus 6/27[22.2%]安慰剂,p<0.0001)、EASI-75(18/27[66.7%]dupilumabversus 4/27[14.8%]安慰剂,p=0.0001)和EASI-90(9/27[33.3%]dupilumab versus 0/27[0%]安慰剂,p=0.0011)患者的比例所证明,至第16周在其EASI评分中达到降低的患者的比例始终是dupilumab组高于安慰剂组。SCORAD评分从基线至第16周的LS均值(±SE)百分比变化与绝对均值SCORAD的变化一致(-54.8±5.40%dupilumab versus-8.2±5.41%,p<0.0001)。观察到dupilumab组中至第16周达到50%SCORAD降低(SCORAD-50)的患者的比例高于安慰剂组(15/27[55.6%]dupilumab versus 2/27[7.4%]安慰剂,p=0.0002)。dupilumab组显示BSA累及从基线至第16周偏离基线的LS均值(±SE)减少为-69.0±12.61%,而安慰剂组显示增加13.6±12.61%。
安全性:dupilumab安全且良好耐受,具有可接受的安全性特征。Dupilumab组中23/27(85.2%)患者和安慰剂组中24/27(88.9%)患者具有至少1例TEAE。安慰剂组中3/27(11.1%)患者报告严重TEAE,dupilumab组中没有一名患者报告严重TEAE。大多数TEAE严重度为轻度或中度。常见TEAE(按监管活动医学词典[MedDRA]优选术语)包括鼻咽炎(dupilumab:3/27[11.1%]的患者;安慰剂:5/27[18.5%]的患者)、上呼吸道感染(分别为4/27[14.8%]和4/27[14.8%])、病毒性上呼吸道感染(分别为3/27[11.1%]和2/27[7.4%])和注射部位反应(按MedDRA高级术语;分别为5/27[18.5%]和1/27[3.7%])。
实施例2:生物标志分析
A.研究A

表2显示两个治疗组中的基线生物标志评分。与安慰剂相比,用dupilumab治疗观察到了血清TARC、PARC和骨膜蛋白的快速且显著的降低(图3-5)。血清总IgE水平在治疗期间稳步下降,在第16周显示与安慰剂相比dupilumab组显著降低(图6)。
就针对所测试的所有变应原系列引发的IgE,注意到抗原特异性IgE从第1周至研究结束(第32周)偏离基线的均值和中位数降低,所测试的变应原系列包含灰赤杨、细极链格孢、狗牙根、白桦、猫毛、枝孢属、蟑螂(德国)、粉尘螨(螨)、狗毛、榆树、约翰逊草、白栎、短豚草、艾蒿、梯牧草(梯牧草属)、美国白蜡树、葡萄球菌肠毒素A和葡萄球菌肠毒素B。与安慰剂相比,dupilumab显著抑制一系列血清变应原特异性IgE(图7;从基线至第16周的中位数基线(IQR)和中位数百分比变化(IQR))。
B.研究B
在研究“B”中,测量来自涉及患有中度至严重AD的个体的临床试验的样品中的血清生物标志。患有AD的个体按1:1:1:1:1:1的比例随机化,接受16周的皮下安慰剂每周、或dupilumab 100mg每4周(q4w)、300mg q4w、200mg每2周(q2w)、300mg q2w或300mg每周(qw)治疗。在第1天,300mg剂量组患者接受600mg负荷剂量,而200和100mg剂量组接受400mg。16周治疗期后进行16周的安全性随访(总研究期32周)。AD相关血清生物标志如胸腺和活化调节趋化因子(TARC)、肺和活化调节趋化因子(PARC)、骨膜蛋白、乳酸脱氢酶(LDH)、嗜酸性粒细胞、总IgE和抗原特异性IgE描述于美国专利申请公开号US2014/0072583(以其整体引入作为参考)中。

对于变应原特异性IgE,定量下限(LLQ)为0.10kU/L;认为≥0.35kU/L的水平是变应原致敏的证据。
使用与基线值的协方差分析作为协变量,在第16周比较各dupilumab给药方案与安慰剂的血清生物标志物TARC,periostin和LDH的平均百分比变化以及总IgE和过敏原特异性IgE的中值百分比变化。
用dupilumab vs安慰剂观察到血清中多种抗原特异性IgE(包括对金黄色葡萄球菌肠毒素特异的那些)从基线至第16周降低(图8和9)。
实施例3:金黄色葡萄球菌皮肤建群
对从参与dupilumab临床试验的个体采集的样品进行了皮肤微生物建群分析。患有中度至严重特应性皮炎(AD)的个体施用16个每周剂量的dupilumab(200mg)或安慰剂;使用dupilumab的患者在第1天接受400mg负荷剂量。在筛查和第32周之间的AD病灶和非病灶皮肤上测定金黄色葡萄球菌建群和感染。从预先测量的皮肤区域(~10cm x 10cm)采集皮肤拭子(预先用Tris-EDTA缓冲液湿润),并测试金黄色葡萄球菌的存在。裂解包含在拭子中的细菌细胞,并纯化总基因组DNA。用定量实时PCR(qPCR)测定来自总细菌基因组DNA的金黄色葡萄球菌femA DNA的丰度。使用用来自已知CFU的金黄色葡萄球菌的基因组DNA产生的标准曲线,测定金黄色葡萄球菌的相对菌落形成单位(rCFU)。将金黄色葡萄球菌丰度报告为偏离基线的中位数(四分位距[IQR])百分比变化。挽救药物后将数据设为缺失。未针对多样性调整变量,因此提供名义p值。
Dupilumab显著降低AD病灶皮肤中的金黄色葡萄球菌丰度(与安慰剂相比从基线至第16周的中位数%变化[P=0.0125;表3]),与安慰剂相比在第16周观察到中位数金黄色葡萄球菌丰度偏离基线的整体降低(图10)。

在非病灶AD皮肤中,与安慰剂相比dupilumab组显示金黄色葡萄球菌丰度在数值上更大的降低(从基线至第16周的中位数%变化[表3;P=0.9865]),及与安慰剂相比在第16周偏离基线的整体中位数降低(图11)。
实施例4:dupilumab在共患PAR的不受控持续性哮喘患者中改善常年性变应性鼻炎(PAR)症状
Dupilumab(抗白介素(IL)-4受体-α单克隆抗体)抑制IL-4和IL-13信号发放(2型炎症的主要驱动信号)。在一项关键性2b期研究(NCT01854047)中,dupilumab改善1秒钟用力呼气量、减少严重哮喘恶化、改善生活质量,且在尽管使用了中至高剂量的吸入型糖皮质激素加长效β2-激动剂(ICS+LABA)也未控制住的持续性哮喘患者中通常很好地耐受。这项事后分析检查dupilumab对鼻窦-鼻腔结果测试(SNOT-22)总得分以及通常与常年性变应性鼻炎(PAR)(哮喘的常见共病)患者中的变应性鼻炎(鼻塞、流鼻涕、打喷嚏和后鼻道卡他(post-nasal discharge))相关的单个项目的影响。PAR定义为在进入研究时存在≥0.35Ku/L的抗常年性抗原(烟曲霉(Aspergillus fumigatus)、猫毛、粉层螨、屋尘螨、狗毛、德国蟑螂、或东方蟑螂)的特异性IgE。由于可能的混淆效应,从分析中排除鼻息肉共患病患者。报告目前在3期研究(NCT02414854)下接受安慰剂和dupilumab方案200或300mg每2周[q2w]的意向治疗群体的数据。终点是SNOT-22总评分以及单个项目后鼻道卡他、鼻塞、流鼻涕和打喷嚏从基线至第24周的变化。在392名接受dupilumab(200或300mg q2w)或安慰剂的患者中,241(61%)名患有PAR。在PAR患者中,dupilumab 300mg q2w相对于安慰剂显著改善SNOT-22总评分(LS均值差-5.98[95%CI,-10.45至-1.51],P=0.009vs.安慰剂)及上文定义的全部4种变应性鼻炎相关症状(鼻塞:-0.60[95%CI,-0.96至-0.25];流鼻涕:-0.67[95%CI,-1.04至-0.31];打喷嚏:-0.55[95%CI,-0.89至-0.21];后鼻道卡他:-0.49[95%CI,-0.83至-0.16];全部P<0.01vs.安慰剂);dupilumab 200mg q2w显示SNOT-22总评分(-1.82[95%CI,-6.46至2.83],P=0.443)以及4种变应性鼻炎相关症状的数值上但非统计显著的降低。在非PAR患者中未观察到SNOT-22总评分和4种变应性鼻炎相关症状相对于安慰剂的差异。综上所述,dupilumab 300mg q2w在不受控持续性哮喘和共患病PAR患者中显著改善鼻窦鼻腔症状。
本发明的范围不限于本文所述的具体实施方案。实际上,对本领域技术人员而言,本发明的除本文所述的那些之外的多种修改将从前述描述和附图变得显而易见。这类修改也旨在落在所附权利要求书的范围之内。
