背景技术
TT场(TTFields)为一种有效且经FDA批准的治疗肿瘤的方法。使用TT场的治疗涉及将低强度、中频、交变电场连续、非侵入地施加到包括肿瘤的目标区域。先前的研究已表明TT场破坏了微管和隔膜蛋白(septin)丝,这两者在有丝分裂中起关键作用。

胶质母细胞瘤细胞侵入邻近脑组织的能力使得难以控制胶质母细胞瘤。因此,阻碍胶质瘤癌细胞运动的治疗方式的发展可促进疾病控制。
发明人已经确定,除了具有抗有丝分裂作用之外,TT场还对需要适当微管动力学的其它细胞过程具有影响。对胶质母细胞瘤进展具有重大影响的一种此类过程为细胞运动性。本申请记录了TT场对细胞运动性的影响,包括胶质瘤细胞的迁移和侵袭特性两者。本申请也描述了通过将TT场施加于目标区域中的癌细胞来降低细胞运动性的方法和系统。
进行一组实验以评估TT场对人胶质瘤细胞运动性的影响,包括这些细胞的迁移和侵袭特性。

图3总结了该第二实验的定量结果。在所有TT场处理的细胞系中,侵袭细胞的数量显著减少:与未处理的对照细胞相比,U-87MG(54%)、A-172(68%)、LN-229(38%)和LN-18(55%)(平均值±SEM;*p<0.05,**p<0.01,***p<0.001,来自对照组,斯氏t检验)。这些结果表明TT场阻碍了不同胶质瘤细胞系中的细胞侵袭。值得注意的是,这种侵袭实验的持续时间(24小时)比TT场的抗有丝分裂效应通常所需的持续时间(72小时)短。还值得注意的是,致使这种侵袭减少的电场强度(即0.6V/cm RMS)低于TT场的抗有丝分裂效应通常所需的电场强度(即>1V/cm RMS)。
进行第三实验来研究TT场抑制U-87胶质瘤癌细胞的侵袭潜力的频率依赖性。使用如上结合第二实验所述的改进的博伊登室执行U-87侵袭测定。使用如上结合第一实验所述的相同的0.6V/cm RMS标称强度和相同方向切换以各种频率(100kHz、200kHz、300kHz和400kHz)施加TT场。在37℃下在5%CO2培养箱中温育24小时后,如上结合第二实验所述,将侵入底部过滤器的细胞固定、成像和定量。
图4总结了该第三实验的定量结果。对于以频率依赖方式测试的所有四个频率的TT场,侵袭细胞的数量减少(相对于对照);并且在300kHz的频率下观察到侵袭细胞数量的最大减少。也许最值得注意的是,提供最高侵袭潜力降低的频率不同于在U-87细胞中提供(基于先前实验)最高抗有丝分裂效应的200kHz频率。
进行第四实验来研究TT场抑制胶质瘤癌细胞迁移的方向依赖性。使用通过定位于皮氏培养皿中的高介电常数陶瓷(ε>5000)(犹他州盐湖城的埃克里斯公司)绝缘的两对电极施加TT场(200kHz,1.75RMS)。电子对的几何形状类似于图1中所示的几何形状。使用IBIDI培养插入物执行体外伤口测定,其中每个培养皿中放置2个插入物,一个平行于电场,一个垂直于电场方向。将电极连接到改进的系统,该系统将AC信号施加到电极,以便在该实验所需的各个方向上产生电场。通过并入到培养皿中的热敏电阻测量温度。
在两个垂直方向上对A-172细胞进行划痕并且成像。如所描述的,在延时系列中观察到迁移24小时。使用Image Pro Premier(Media Cybernetics,美国)软件进一步评估所获得的图像以定量伤口愈合期间的细胞迁移率。迁移率的定量分析计算为细胞从划痕边缘向划痕中点移动的总距离随时间的变化。
图5总结了该第四个实验的定量分析结果。当细胞平行于(24%)电场迁移时,与对照相比,细胞迁移率显著降低,这与从两个不同方向施加TT场并且每秒切换时获得的降低相似(27%)。有趣的是,TT场从垂直于迁移过程的单一方向施加,导致迁移率降低最多(39%),这显著高于当TT场在平行于迁移过程的两个方向或单个方向上施加时的降低(***p<0.001,来自两组,斯氏t检验。实验重复3次)。这些结果表明,取决于迁移过程,施加电场的方向对迁移速率具有不同的影响。
进行第五个实验用于比较TT场和布美他尼在抑制胶质瘤细胞侵袭潜力方面的功效。布美他尼为用于治疗心力衰竭的氨磺酰类的袢性利尿剂,并且已知其影响细胞运动性。在大脑中,布美他尼阻断NKCC1阳离子-氯化物共转运蛋白,并且因此降低神经元中的内部氯化物浓度。NKCC1为胶质瘤细胞中Cl-积累提供了主要途径。NKCC1定位于侵袭过程的前沿,并且使用袢性利尿剂布美他尼的药理学抑制作用抑制体外转移孔迁移25%至50%。
如上文结合第二实验所述执行侵袭测定,并且进行一些微小的修改。更具体地,将U-87细胞(2×105个)悬浮在无血清DMEM中并且接种在腔室的上部隔室中。下部隔室含有10%FBS DMEM。两个隔室均含有浓度为200μM的布美他尼。如上所述施加TT场(200kHz,0.6V/cm RMS)。在37℃下在5%CO2培养箱中培育24小时后,将侵入底部过滤器的细胞固定,用结晶紫染色,并且在倒置显微镜下成像。使用ImageJ软件(NIH)执行侵袭细胞的定量。
图6总结了该第五实验的定量结果。在单独施加TT场(31%)或布美他尼(34%)治疗的细胞中,侵袭细胞的数量显著减少(***p<0.001,来自对照组,斯氏t检验)。并且值得注意的是,使用TT场和布美他尼两者的联合治疗导致侵袭细胞数量协同减少61%,这显著高于单独施加TT场或布美他尼所获得的减少量(***p<0.001,斯氏t检验)。这些结果表明,与单独的任一种治疗相比,暴露于低强度TT场与抑制细胞运动性的药物组合可导致对胶质瘤细胞侵袭性的协同增强的抑制作用。并且更具体地,与单独的任一种治疗相比,将TT场与布美他尼组合产生了协同增强的功效。
当前版本的Novocure 系统以单一频率在目标区域中产生AC电场,并且提前选择该频率以增加场在破坏目标区域内的癌细胞的有丝分裂的有效性。如上所表明,系统有时会降低电场强度,以防止患者皮肤过热,这使场破坏有丝分裂的有效性降低。下文所述的实施例利用了上述第二实验和第三实验的两种结果。更具体地,下文所述的实施例利用以下事实:(1)致使运动性降低所需的电场强度低于TT场的抗有丝分裂效应通常所需的电场强度;以及(2)对于至少某些细胞系,提供最高运动性降低的频率不同于提供最高抗有丝分裂效应的频率。
图7描绘了包括信号发生器20和四个电极41-44(也称为换能器阵列)的本发明的实施例。电极41-44定位成使得它们可在目标区域50中感应出电场。例如,如果目标区域为皮氏培养皿,则电极41-44可定位成与含有癌细胞55的皮氏培养皿的壁接触。另选地,如果目标区域为患者身体的一部分,则电极41-44可定位成与包括癌细胞55的肿瘤附近的患者皮肤接触。信号发生器20具有两个输出端Q1和Q2。第一输出端Q1跨电极41与电极42两端连接;并且第二输出端Q2跨电极43与电极44两端连接。当信号发生器20在输出端Q1处输出AC电压时,该电压将出现在电极41、电极42上,并且在第一方向D1上在目标区域50中感应出电场。当信号发生器20在输出端Q2处输出AC电压时,该电压将出现在电极43、电极44上,并且在第二方向D2上在目标区域50中感应出电场。温度传感器48检测电极41-44处的温度并且将这些温度报告回包含在信号发生器20内的温度传感器接口22。如果检测到的温度太高,则控制器25将命令信号发生器20的功率级28降低其输出电压,这将最终降低温度。
注意,上一段中图7的描述与当前版本的Novocure系统的操作非常相似。但是因为场发生器单元总是输出相同的频率,所以图7实施例可与区分开。更具体地,场发生器单元总是输出基于其对相关癌症的抗有丝分裂效应而预先选择的频率(即,在胶质母细胞瘤的情况下为200kHz)。相比之下,图7实施例在选择用于破坏癌细胞有丝分裂的第一频率和选择用于降低癌细胞运动性的第二频率之间切换其输出Q1、Q2的频率。在图7实施例中,这由控制器25完成,该控制器25将适当的控制信号输出到功率级28,以便控制输出Q1和输出Q2的频率和振幅,如下文结合图8至图9描述。控制器25可使用对于相关领域的技术人员显而易见的各种技术中的任何一种来实现。示例包括但不限于基于微处理器或微控制器的控制器,其被编程以实施本文所述的序列;以及硬件定时器,其产生适当的控制信号以协调本文所述的序列。功率级28的硬件实现被配置成响应由控制器25产生的控制信号。
图8描绘了在控制器25的控制下由功率级28产生的输出Q1和输出Q2的第一事件序列。在该序列中,场的方向永远不会切换,这意味着只需要功率级的一个输出(即Q1或Q2),并且只需要一对电极(即41/42或43/44)。图8的下半部分的序列如下:首先,在t0和t1之间,在目标区域中强加第一频率的第一AC电场,其被选择以破坏癌细胞的有丝分裂。然后,在t1和t2之间,在目标区域中强加第二频率的第二AC电场,其被选择以降低癌细胞的运动性。然后重复该序列直至治疗过程结束。
在一些实施例中,除了切换频率之外(如图8的下半部分所示),信号发生器的输出的振幅也同步切换(如图8的上半部分所示)。在这些实施例中,在控制器25的控制下由功率级28产生的输出的事件序列如下。
在步骤(a)中,在第一时间间隔(例如,t0和t1之间的间隔)内在目标区域中强加第一AC电场。第一AC电场具有第一频率和第一振幅,并且选择第一频率和第一振幅使得第一AC电场破坏癌细胞的有丝分裂。在步骤(b)中,在第二时间间隔(例如,t1和t2之间的间隔)内在目标区域中强加第二AC电场。第二AC电场具有第二频率和第二振幅,并且选择第二频率和第二振幅使得第二AC电场降低癌细胞的运动性。第二振幅低于第一振幅。然后在治疗过程期间以交替顺序连续重复这些步骤(a)和步骤(b)。任选地,可在步骤(a)和步骤(b)之间或在步骤(b)和步骤(a)之间添加附加的中间步骤。例如,控制器25可命令功率级28在步骤(a)和步骤(b)的一个或多个实例之间输出第三频率。
如上所述,第二振幅(在抗运动性阶段期间)低于第一振幅(在抗有丝分裂阶段期间)。但这不是问题,因为致使运动性降低所需的电场强度低于TT场的抗有丝分裂效应通常所需的电场强度。并且有利地,系统可在抗运动性阶段期间依赖于减小的振幅来降低电极处的温度而不降低抗有丝分裂阶段(其需要更高的振幅)期间的场强。
此外,温度上升与振幅的平方成比例的事实使得这一优点更加复杂。因此,在抗运动性阶段期间将振幅降低X%将使该阶段期间的加热降低至(1-X)2%。例如,在抗运动性阶段期间将振幅从2V/cm降低到1V/cm将使该阶段期间的加热降低到(1-0.5)2%(即,其原始值的四分之一)。如果我们假设抗运动性阶段和抗有丝分裂阶段具有相同的持续时间,则该修改将使总加热负荷降低至其原始值的5/8。
在一些优选实施例中,第二振幅小于第一振幅的75%。优选地,第一时间间隔和第二时间间隔两者均小于五秒。这对于在不中断的情况下维持抗有丝分裂效应非常重要。优选地,治疗过程具有至少12小时的持续时间。在一些优选实施例中,第一频率与第二频率不同。在其它实施例中,第一频率和第二频率可为相同的(即,对于抗有丝分裂频率与抗运动性频率一致的细胞系)。在一些优选实施例中,第一频率(即,抗有丝分裂频率)为200kHz±10%,并且第二频率(即,抗运动性频率)为300kHz±10%。在一些优选实施例中,第二振幅为至少0.6V/cm RMS,以便维持抗运动性效应。任选地,可在治疗过程期间投予抑制细胞运动性的化合物(例如布美他尼)。
在一些实施例中,除了在针对破坏有丝分裂而优化的第一频率和针对降低运动性而优化的第二频率之间切换场的频率之外,目标区域中的电场的方向也可在两个或更多个方向之间切换。图9A和图9B描绘了两个示例,其中场的方向在图7中所示的两个方向D1和D2之间切换。当信号发生器20产生其输出Q1时,在方向D1上产生场,该场施加在电极41和电极42之间。当信号发生器20产生其输出Q2时,在方向D2上产生场,该场施加在电极43和电极44之间。
图9A描绘了在一个此类实施例中在控制器25的控制下由信号发生器20的功率级28产生的输出Q1和输出Q2的事件序列。在该序列中,方向在两个方向D1和D2之间以比切换频率的速率快的速率切换。这里的序列类似于上文结合图8描述的序列,其中频率和振幅两者彼此同步切换。但是,除了如上结合图8所述切换频率和振幅之外,也切换场的方向。更具体地,在第一时间间隔的第一部分(即,t0和t1之间的时间的第一部分)内在第一方向上在目标区域中强加第一AC电场,并且在第一时间间隔的第二部分(即,t0和t1之间的时间的最后部分)内在第二方向上在目标区域中强加第一AC电场。优选地,第一方向D1和第二方向D2之间的角度在60°和90°之间。(注意,本文使用的所有数字角度范围均包含端点。)
在一些实施例中,除了在第一时间间隔(其对应于抗有丝分裂阶段)期间切换方向之外,也在第二时间间隔(其对应于抗运动性阶段)期间切换方向。在这些实施例中,在第二时间间隔的第一部分(即,t1和t2之间的时间的第一部分)内在第一方向上在目标区域中强加第二AC电场,并且在第二时间间隔的第二部分(即,t1和t2之间的时间的最后部分)内在第二方向上在目标区域中强加第二AC电场。
图9B描绘了在另一个实施例中在控制器25的控制下由功率级28产生的输出Q1和输出Q2的事件序列。在该序列中,方向在两个方向D1和D2之间以比频率切换速率慢的速率切换。这里的序列类似于上文结合图8描述的序列,其中频率和振幅均彼此同步地切换。但是,除了上文结合图8描述的频率和振幅的切换之外,在重复步骤中的至少一些步骤期间改变第一AC电场的方向,并且在重复步骤中的至少一些步骤期间也改变第二AC电场的方向。在图9B的示例中,原始序列对应于t0和t2之间的时间间隔,并且在治疗过程期间重复原始序列,其中第一次重复占据t2和t4之间的时间间隔。如图9B所见,抗有丝分裂阶段和抗迁移阶段两者的场的方向在第一次重复期间相对于原始序列切换到D2。
注意,在上面图8至图9中描绘的序列中,抗有丝分裂阶段和抗运动性阶段相对于时间是互斥的。然而,在替代实施例中,那些阶段可部分地或完全地彼此重叠。此类实施例的一个示例为同时实现以下两个步骤(a)和(b):在步骤(a)中,在目标区域中强加第一AC电场。第一AC电场具有经选择使得第一AC电场破坏癌细胞的有丝分裂的第一频率和第一振幅。在步骤(b)中,在目标区域中强加第二AC电场。第二AC电场具有经选择使得第二AC电场降低癌细胞的运动性的第二频率和第二振幅。第二频率与第一频率不同,并且第二振幅低于第一振幅。当所有步骤(a)和步骤(b)同时实施时,第一AC电场和第二AC电场将通过叠加相加。
一些实施例涉及通过将具有高转移摄取概率的原发性肿瘤或其附近、转移瘤或器官以最佳选择的频率暴露于交变电场以抑制肿瘤细胞的迁移或侵袭来治疗癌症的方法。至少对于某些细胞系,该频率与破坏有丝分裂所需的频率显著不同。场可使用Novocure的系统或美国专利7089054、6868289和7706890中描述的装置传递,所述美国专利通过引用方式并入本文。其可用于治疗GBM或其它类型的癌症。可在优选方向上传递场,并且选择优选方向以防止进入不可切除的区域或重要器官。
对于不同类型的癌症,可通过以下来确定抑制特定癌细胞类型的细胞迁移的最佳频率:在皮氏培养皿中培养细胞(或与感兴趣的细胞非常相似的细胞系),将细胞暴露于给定强度和不同频率的交变电场,并且找到对该特定细胞系的细胞迁移具有最大抑制作用的频率。可经由划痕测定和/或经由侵袭测定来确定电场的影响。
一些实施例涉及治疗癌症的方法,其中肿瘤暴露于包含两个频率分量的交变电场:一个频率为抑制细胞分裂的最佳频率,另一个频率为抑制迁移的最佳频率。注意,在美国专利8244345中描述了组合经调节以损坏分裂细胞的两个不同频率,所述美国专利通过引用方式并入本文。但是此处,其中一个频率被调节为防止迁移。可顺序地施加不同频率的信号,或者可将它们组合成单个场。可在一个方向上或多个不同方向上传递场。
虽然已经参考某些实施例公开了本发明,但是在不脱离如所附权利要求中所限定的本发明的范围和精神的情况下,可对所描述的实施例进行多种修改、变更和改变。因此,本发明不旨在限制于所描述的实施例,而是具有由所附权利要求的语言及其等同物限定的全部范围。
Claims (25)
1.一种抑制癌细胞在目标区域中扩散的方法,所述方法包括:
(a)在第一时间间隔内在所述目标区域中强加第一AC电场,所述第一AC电场具有第一频率和第一振幅,其中选择所述第一频率和所述第一振幅使得所述第一AC电场破坏所述癌细胞的有丝分裂;
(b)在第二时间间隔内在所述目标区域中强加第二AC电场,所述第二AC电场具有第二频率和第二振幅,其中选择所述第二频率和所述第二振幅使得所述第二AC电场降低所述癌细胞的运动性,并且其中所述第二振幅低于所述第一振幅;以及
(c)在治疗过程期间以交替顺序连续重复步骤(a)和步骤(b)。
2.根据权利要求1所述的方法,其中所述第二振幅小于所述第一振幅的75%。
3.根据权利要求2所述的方法,其中所述第一时间间隔小于5秒,并且其中所述第二时间间隔小于5秒。
4.根据权利要求3所述的方法,其中所述治疗过程的持续时间为至少12小时。
5.根据权利要求1所述的方法,其中所述第二频率与所述第一频率不同。
6.根据权利要求1所述的方法,其中所述第一频率为200kHz±10%,并且所述第二频率为300kHz±10%。
7.根据权利要求1所述的方法,进一步包括在所述治疗过程期间投予抑制细胞运动性的化合物的步骤。
8.根据权利要求7所述的方法,其中所述化合物包括布美他尼(Bumetanide)。
9.根据权利要求1所述的方法,其中所述第二振幅为至少0.6V/cm RMS。
10.根据权利要求1所述的方法,其中所述第二振幅小于所述第一振幅的75%,其中所述第二频率与所述第一频率不同,其中所述第一时间间隔小于5秒,其中所述第二时间间隔小于5秒,并且其中所述第二振幅为至少0.6V/cm RMS。
11.根据权利要求10所述的方法,其中所述第一频率为200kHz±10%,并且所述第二频率为300kHz±10%。
12.根据权利要求1所述的方法,其中在所述第一时间间隔的第一部分内在第一方向上在所述目标区域中强加所述第一AC电场,
其中在所述第一时间间隔的第二部分内在第二方向上在所述目标区域中强加所述第一AC电场,并且
其中所述第一方向与所述第二方向之间的角度在60°和90°之间。
13.根据权利要求12所述的方法,其中在所述第二时间间隔的第一部分内在所述第一方向上在所述目标区域中强加所述第二AC电场,并且
其中在所述第二时间间隔的第二部分内在所述第二方向上在所述目标区域中强加所述第二AC电场。
14.根据权利要求13所述的方法,其中所述第二振幅小于所述第一振幅的75%。

16.根据权利要求1所述的方法,其中在所述重复步骤中的至少一些步骤期间改变所述第一AC电场的方向,并且其中在所述重复步骤中的至少一些步骤期间改变所述第二AC电场的方向。
17.根据权利要求16所述的方法,其中所述第二振幅小于所述第一振幅的75%。
18.根据权利要求17所述的方法,其中所述第二频率与所述第一频率不同,其中所述第一时间间隔小于5秒,并且其中所述第二时间间隔小于5秒。
19.一种抑制癌细胞在目标区域中扩散的方法,所述方法包括:
(a)在所述目标区域中强加第一AC电场,所述第一AC电场具有第一频率和第一振幅,其中选择所述第一频率和所述第一振幅使得所述第一AC电场破坏所述癌细胞的有丝分裂;
(b)在所述目标区域中强加第二AC电场,所述第二AC电场具有第二频率和第二振幅,其中选择所述第二频率和所述第二振幅使得所述第二AC电场降低所述癌细胞的运动性,其中所述第二频率与所述第一频率不同,并且其中所述第二振幅低于所述第一振幅。
20.根据权利要求19所述的方法,其中所述第二振幅小于所述第一振幅的75%。
21.一种用于抑制癌细胞在目标区域中扩散的设备,所述设备包括:
AC信号发生器,所述AC信号发生器具有(i)第一可控输出端,所述第一可控输出端被配置成在定位于所述目标区域附近的第一组电容耦合电极上施加AC电压,以便在第一方向上在所述目标区域中强加电场,以及(ii)第二可控输出端,所述第二可控输出端被配置成在定位于所述目标区域附近的第二组电容耦合电极上施加AC电压,以便在第二方向上在所述目标区域中强加电场;以及
控制器,所述控制器被配置成(a)控制所述第一可控输出端,使得在第一时间间隔内产生第一AC信号,所述第一AC信号具有第一频率和第一振幅,其中选择所述第一频率和所述第一振幅以强加破坏所述癌细胞的有丝分裂的电场,以及(b)控制所述第一可控输出端,使得在第二时间间隔内产生第二AC信号,所述第二AC信号具有第二频率和第二振幅,其中选择所述第二频率和所述第二振幅以强加降低所述癌细胞的运动性的电场,并且其中所述第二振幅低于所述第一振幅,(c)控制所述第二可控输出端,使得在第三时间间隔内产生第三AC信号,所述第三AC信号具有第三频率和第三振幅,其中选择所述第三频率和所述第三振幅以强加破坏所述癌细胞的有丝分裂的电场,以及(d)控制所述第二可控输出端,使得在第四时间间隔内产生第四AC信号,所述第四AC信号具有第四频率和第四振幅,其中选择所述第四频率和所述第四振幅以强加降低所述癌细胞的运动性的电场,并且其中所述第四振幅低于所述第三振幅,以及(e)在治疗过程期间以交替顺序连续重复(a)至(d)。
22.根据权利要求21所述的设备,其中所述第二振幅小于所述第一振幅的75%,并且其中所述第四振幅小于所述第三振幅的75%。
23.根据权利要求21所述的设备,其中所述第二频率与所述第一频率不同,并且其中所述第四频率与所述第三频率不同。
24.根据权利要求21所述的设备,其中所述第二振幅小于所述第一振幅的75%,并且其中所述第四振幅小于所述第三振幅的75%,其中所述第二频率与所述第一频率不同,其中所述第四频率与所述第三频率不同,其中所述第三频率与所述第一频率相同,其中所述第四频率与所述第二频率相同,其中所述第一时间间隔小于5秒,其中所述第二时间间隔小于5秒,其中所述第三时间间隔小于5秒,并且其中所述第四时间间隔小于5秒。
