Abstract
本发明涉及一种治疗精子DNA损伤的中药,由以下重量计的原料药物:黄芪30‑50g、枸杞子20‑30g、五味子15‑25g、车前子12‑22g和川牛膝15‑25g制成;本发明采用纯天然的原料中草药物制成,原料丰富,成本低,各药物组分间有机配合,协同增效,配伍合理,益气、补肾类药能清除自由基,抗氧化;活血类药物能够清除自由基,改善组织血液循环,制备方法简单,服用方便,疗效佳,是治疗精子DNA损伤并导致不育症药物上的一大创新,经济和社会效益显著。

技术领域
本发明涉及中药,特别是一种治疗精子DNA损伤的中药。
背景技术
育龄夫妇,性生活正常且未采取任何避孕措施,女方正常,由于男方因素致女方在一年内不能自然受孕者,称为男性不育症。在男性不育症患者中,有15%的患者精液常规检查是正常的。精子DNA碎片指数(DNA fragmentation index,DFI)能够反映遗传物质DNA的完整性或损伤程度,精子DNA损伤能影响卵子受精及胚胎发育,甚至导致不良的妊娠结局,可作为一个新的评价精液质量和预测生育能力的重要指标。精子正常功能的发挥依赖于精子DNA的完整。任何形式精子染色体异常或DNA损伤都会降低男性生育潜能,并能影响后代健康。
研究表明,精子DNA完整性受生活方式、环境、身体健康状况及不明原因等诸多因素的影响,病因复杂。现代医学对本病的治疗多采用抗氧化剂、抗感染等治疗方法,由于用药单一,又不能采取针对性的治疗措施,故临床疗效不佳。中医药在本病的治疗中发挥了较好的作用,但临床各医家对本病的治疗多凭各自经验,采用自拟方药,既无成熟的理论,亦无效果满意的成方,可复制性差,缺乏循证医学支持。
精子DNA损伤性不育病机为“正虚易感”,正气是机体维持自身正常运行(阴阳平衡、脏腑协调)的能力,是机体对抗外界干扰、适应内外环境变化的能力,是机体抗病的能力以及病后康复和预防复发的能力。《黄帝内经·素问》:“正气存内,邪不可干;邪之所凑,其气必虚。”具体到脏腑,肾气是正气在脏腑功能中的具体体现。正气充足与否,决定着肾中精气盛衰。祖国医学认为,肾为先天之本,主人体生长、发育、生殖。《素问·六节脏象论》曰:“肾者主蛰,封藏之本,精之处也。”肾中精气盛衰,决定着人体强弱和生殖机能正常与否。肾主外,藏先天之精,化生元气,有调节机体适应环境、御邪防病的功能。正气维持着精子正常发生与形成的全过程:①正气协调脏腑、经络、气血,为精子生成创造稳定的环境;②正气监视、管理精子的生成过程,维持精子的新陈代谢;③正气可抵抗内外环境变化对精子的影响,保证生精过程的稳定;④正气及时发现生成的畸形率、DNA碎片率高的精子,并进行修复或清除。正气亏虚,肾中精气不足,一则脏腑功能紊乱,经络、气血运行失常,精子生成环境受到影响;二则正气监视、管理功能下降,且不足以抵抗内外环境变化,生精过程受到影响;最后修复或清除功能下降。这些均可致精子DNA碎片率增高,影响生育;因此,提供一种治疗精子DNA损伤的中药是必须要解决的技术问题。
发明内容
针对上述情况,为克服现有技术之缺陷,本发明之目的是提供一种治疗精子DNA损伤的中药,可有效解决由于精子DNA损伤以及造成的不孕不育症的问题。
本发明采用中医中药机理,紧抓精子DNA损伤性不育的主要病机—正虚易感,通过运用扶正益气,补肾活血类药物清除自由基,改善精子生成及成熟微环境,从而达到降低精子DNA损伤,改善生育的目的,据此,本发明解决的技术方案是,一种治疗精子DNA损伤的中药,由以下重量计的原料药物:黄芪30-50g、枸杞子20-30g、五味子15-25g、车前子12-22g和川牛膝15-25g制成;其中,先将车前子用纱布包住,再与其他原料药物混合在一起,加入500-700ml水,浸泡20-30min,武火煮沸,改用文火煎煮30-60min,过滤,得第一滤液,滤渣加500-700ml水,武火煮沸,改用文火煎煮30-60min,过滤,得第二滤液,合并两次滤液,浓缩至200-400ml。

实施例1:本发明一种治疗精子DNA损伤的中药,由以下重量计的原料药物:黄芪35-45g、枸杞子22-28g、五味子18-22g、车前子15-20g和川牛膝18-22g制成;其中,先将车前子用纱布包住,再与其他原料药物混合在一起,加入550-650ml水,浸泡20-30min,武火煮沸,改用文火煎煮30-60min,过滤,得第一滤液,滤渣加550-650ml水,武火煮沸,改用文火煎煮30-60min,过滤,得第二滤液,合并两次滤液,浓缩至200-400ml。
实施例2:本发明一种治疗精子DNA损伤的中药,由以下重量计的原料药物:黄芪30g、枸杞子20g、五味子15g、车前子12g和川牛膝15g制成;制备方法同实施例1。
实施例3:本发明一种治疗精子DNA损伤的中药,由以下重量计的原料药物:黄芪40g、枸杞子25g、五味子20g、车前子17g和川牛膝20g制成;制备方法同实施例1。
实施例4:本发明一种治疗精子DNA损伤的中药,由以下重量计的原料药物:黄芪50g、枸杞子30g、五味子25g、车前子22g和川牛膝25g制成;制备方法同实施例1。
在上述药物中,其中:
黄芪,系豆科植物蒙古黄芪或膜荚黄芪的根,主产于内蒙古、山西、黑龙江等地,春秋二季采挖,除去须根及根头,晒干,切片。本品甘温,归脾、肺经,具有补气健脾、益气固表、利尿消肿等功效。《本草汇言》:“补肺健脾,实卫敛汗,驱风运毒之药也。”主治脾气虚证、肺气虚证、气虚自汗、气血亏虚或疮疡难溃难腐等证。现代研究表明,本品含苷类、多糖、黄酮、氨基酸、微量元素等成分,具有促进机体代谢、增强和调节机体免疫功能、抗菌、抗衰老、抗缺氧、抗辐射、抗疲劳、利尿、兴奋呼吸、降血脂、保肝等多种作用。
枸杞子,系茄科植物宁夏枸杞的干燥成熟果实。西枸杞主产于主产宁夏,津枸杞主产于河北。此外,河南、陕西、四川、山西、江苏等地亦产。夏、秋果实成熟时采摘,除去果柄,置阴凉处晾至果皮起皱纹后,再暴晒至外皮干硬、果肉柔软即得。遇阴雨可用微火烘干。本品甘平,归肝、肾经,具有滋补肝肾,益精明目之功效。《药性论》:“能补益精诸不足,易颜色,变白,明目,安神。”用于虚劳精亏,腰膝酸痛,眩晕耳鸣,内热消渴,血虚萎黄,目昏不明等。现代研究表明,本品含枸杞多糖、胡萝卜素、硫胺素、核黄素、烟酸、抗坏血酸等,尚分离出β-谷甾醇、亚油酸等成分,具有调节免疫、保护生殖系统、抗衰老、抗肿瘤、降血糖、抗脂肪肝、降血压、提高视力、保肝等作用。
五味子,为木兰科植物五味子或华中五味子的干燥成熟果实。前者习称“北五味子”,后者习称“南五味子”。产于黑龙江、吉林、辽宁、内蒙古、河北、山西、宁夏、甘肃、山东等地,秋季果实成熟时采摘,晒干或蒸后晒干,除去果梗及杂质。本品酸、甘,温,归肺、心、肾经,具有收敛固涩,益气生津,补肾宁心的功效。《本经》:“主益气,咳逆上气,劳伤羸度,补不足,强阴,益男子精。”主治肺虚喘咳,口干作渴,自汗,盗汗,劳伤羸瘦,梦遗滑精,久泻久痢。现代研究表明,本品含挥发性成分、木脂素类和有机酸类。亦含柠檬醛、叶绿素、甾醇、维生素C、维生素E、糖类、树脂和鞣质。药理研究表明,五味子有增强机体免疫力、抗菌、强心、保肝等作用。
车前子,是大车前、车前、平车前、海滨车前、长叶车前的别称;药用车前子为车前科植物车前或平车前的干燥成熟种子。主产于黑龙江、辽宁、河北等地。夏、秋二季种子成熟时采收果穗,晒干,搓出种子。本品性味甘寒,入肾、膀胱、肝、肺经,功能利水通淋、渗湿止泻、清肝明目、清热化痰。《药性论》:“能去风毒,肝中风热,毒风冲眼目,赤痛障翳,脑痛泪出,去心胸烦热。”治小便不通,淋浊,带下,尿血,暑湿泻痢,咳嗽多痰,湿痹,目赤障翳等证。现代研究表明,本品含多量粘液质、桃叶珊瑚甙,并含车前子酸、胆碱、腺嘌呤、琥珀酸、树脂等,尚含维生素A、B1。药理研究表明,本品有利尿及促进消化液分泌增加、可促进关节囊滑膜结缔组织增生等作用。
川牛膝,系苋科植物川牛膝的干燥根,主要产于四川、云南、贵州、河北等地。秋、冬二季采挖,除去芦头、须根及泥沙,炕干或晒干。本品味甘、微苦,性平,归肝、肾经,有逐瘀通经,通利关节,利尿通淋等功效。《四川中药志》:祛风利湿,通经散血。治寒湿腰腿骨痛,足痿筋挛,妇女经闭及症瘕,淋病,尿血,阴痿、失溺。用于经闭症瘕,胞衣不下,关节痹痛,足痿筋挛,尿血血淋,跌扑损伤等证。现代研究表明,本品含β-蜕皮甾酮及微量元素钛(Ti)等成分。
上述药物,黄芪甘温,归脾、肺经,具有补气健脾、益气固表、利尿消肿等功效,为补中益气,主治脾气虚证、肺气虚证、气虚自汗、气血亏虚或疮疡难溃难腐等,也就是说,黄芪一可补益脾气,改善少气懒言、倦怠乏力等脾气虚衰症状;二可补益肾气,提高精子活力,故为君药。枸杞子甘、平,归肝、肾经,具有滋补肝肾,益精明目之功效,可用于虚劳精亏,腰膝酸痛,眩晕耳鸣,内热消渴,血虚萎黄,目昏不明等,五味子酸、甘,温,归肺、心、肾经,具有收敛固涩,益气生津,补肾宁心的功效,主治肺虚喘咳,口干作渴,自汗,盗汗,劳伤羸瘦,梦遗滑精,久泻久痢,车前子性味甘寒,入肾、膀胱、肝、肺经,功能利水通淋、渗湿止泻、清肝明目、清热化痰。治小便不通,淋浊,带下,尿血,暑湿泻痢,咳嗽多痰,湿痹,目赤障翳等证。三者共为臣药,有助君药补益肾精之效;川牛膝,味甘、微苦,性平,归肝、肾经,有逐瘀通经,通利关节,利尿通淋等功效。用于此方能补肝肾、活血化瘀通络、利尿,功兼佐使之药用。全方选药精细、组方严谨、有效组合、配伍得当、协同增效,共奏扶正益气,补肾活血之功。该方并经临床应用,取得了很好的效果,其有关资料如下。
1、西医诊断标准
1.1精子DNA损伤诊断标准
根据《世界卫生组织男性不育标准化检查与诊疗手册》和DFI检测指标执行:育龄夫妇,性生活正常且未采取任何避孕措施,女方正常,由于男方因素致女方在一年内不能自然受孕,并在符合精液分析检查的时间(禁欲2~7天)内所做精液分析报告中,15%≤DFI<80%的病症。
1.2精子DNA碎片化率异常诊断标准:经稀释、染色、酸处理及AO染色的标本。
依据精子染色质结构分析(SCSA)检测方法的参考标准:DFI<15%为正常,15%≤DFI<30%为中度碎片化,DF1≥30%为高度碎片化。
2、中医辨证标准
参照《中药新药临床研究指导原则》及《中医男科学》所述气虚、肾虚、血瘀等辨证要点:主症:不育,性欲淡漠,少气懒言,精子稀少,精子活动力低下;次症:腰膝酸软,夜尿增多,舌淡苔薄白或舌暗红有瘀斑瘀点,脉沉细或弦涩。
以上主症“不育”必须具备,兼具其余各项中1项及次症中的2项(必兼舌脉),即可诊断。
3、纳入标准
①符合精子DNA损伤的西医诊断标准;
②符合中医证型诊断标准;
③符合年龄在22~45岁之间的患者;
④符合医学伦理学标准;
⑤签署知情同意书。
4、排除标准
①外生殖器先天性器质性病变者;
②泌尿生殖道梗阻及感染者;
③染色体异常者(包括核型异常及Y染色体微缺失);
④合并有导致内分泌功能异常的疾病者;
⑤配偶有全身严重器质性疾病,不能充分配合者;
⑥合并严重心、脑血管疾病,肝、肾功能不全等严重原发性疾病,及精神病患者;
⑦需同时应用可能影响药品疗效和安全性评价药物者。
5、脱落标准与剔除标准
5.1病例脱落标准
①发生严重不良事件、并发症和特殊生理变化,不宜继续接受试验者;
②试验过程中自行退出者;
③因其他各种原因疗程未结束退出试验、失访的病例;
④未按规定用药,无法判定疗效,或资料不全等影响疗效或安全性判断者。
5.2病例剔除标准
①未按医嘱应用药物治疗者;
②在随机化之后没有任何数据者给予剔除。
6、分组及实验方法
筛选80例合格进入随机化试验的受试者,按随机数字表法随机均分为2组,每组40例,治疗组采用本发明中药治疗,对照组采用左卡尼汀口服液治疗。本研究采用随机、平行、对照的临床试验方法,研究方案报请医院伦理委员会审批后进行。
7、治疗方案
治疗组采用本发明中药治疗,每日2次,分早晚两次饭后温服。对照组采用组卡尼汀口服液,每日3次,每次1支,口服。2组均12周为1个疗程,1个疗程后统计疗效。
注意事项:①治疗前2周开始到治疗结束停用其他治疗本病及影响本病的药物;②若期间有其他疾病必须治疗者,应尽量服用对本病无影响或影响较小的药物;③服药期间一定要戒除烟酒,禁止桑拿及盆(池)浴,尽量避免接触高温、放射线污染、化学毒物污染等环境;④治疗期间不禁房事。
8、疗效评定
8.1主要效应指标:总有效率,中医证候评分(见表1),治疗前、后精子DNA碎片率及精子活力、配偶妊娠率(包括生化妊娠率和临床妊娠率)。

8.2安全性评价指标:治疗前后监测患者生命体征及血常规、尿常规、肝肾功、电解质情况。8.3中医证候疗效评定标准:
依据《中药新药临床研究指导原则1993版》进行评定。
痊愈:中医临床症状、次症消失或基本消失,证候总评分减少≥90%;
显效:中医临床症状、次症明显改善,60%≤证候总评分减少<90%;
有效:中医临床症状、次症均有好转,30%≤证候总评分减少<60%;
无效:中医临床症状、次症无改善,甚至加重,证候总评分减少<30%。
积分变化公式(尼莫地平法)为:[(治疗前总评分-治疗后总评分)÷治疗前总评分]
×100%
8.4综合疗效评定标准:
参考《指导原则》中不育症相关内容拟定:
①痊愈:药物治疗期间精子DFI<15%或治疗期间配偶怀孕;
②显效:配偶未怀孕,但精液分析参数及精子DFI指数恢复正常,15%≤DFI<30%;
③有效:精液分析主要参数有改善,精子DFI≥30%且下降比例>30%者;
④无效:精液分析各项参数及DFI与治疗前无明显变化,甚至加重者。
9、统计学处理
①计量资料:先进行正态性检验,符合正态分布者进行t检验或方差计算,方差计算前进行方差齐性检验。两组间比较采用t检验,不符合正态性分布者则采用两组间秩和检验;组内治疗前后自身比较采用配对t检验(Paired-Samples T Test),不符合正态性分布者则采用配对秩和检验(2Related Samples,Wilcoxon法)。多组间两两比较符合正态性分布、方差齐者采用方差计算(one-way ANOVA),两两组间比较用LSD法;不符合正态分布或方差不齐者用Wilcoxon秩和检验。
②计数资料:采用秩和检验,两两比较用Wilcoxon秩和检验;两个以上组间比较用Kruskal-Wallis检验。两个总体率(或构成比)之间比较,用χ2检验或确切概率法,多个样本率或构成比之间比较,采用列联表χ2检验。
③等级资料:采用Kruskal-Wallis H检验。α取0.05标准,以P<α为有显著性差异。
总有效率=(痊愈+显效+有效)/总例数×100%

注:两组于治疗前中医症候总评分比较无显著性差异,t=2.832,P>0.50;治疗后两组中医症候积分比较,t=0.159,P<0.05,故治疗后两组差别有统计学意义。
安全性指标异常率情况
经统计学处理,治疗组和对照组各项安全性指标血压、脉搏、心电图、血、尿常规化验,肝、肾功能,治疗前后比较均无显著差异(P>0.05)。

结论
由上述表明,本发明中药稳定可靠,治疗效果好,不良反应小,无毒副作用,在治疗精子DNA损伤性不育上,能明显改善患者精液质量及精子DNA碎片率,减少少气懒言、腰膝酸软等症状,降低中医证候总评分,增加综合疗效评分总评分,为精子DNA损伤性不育提供了良好的治疗条件,有效率达到92%以上,是中医药学上的一大创造,有实际的临床意义,是治疗精子DNA损伤药物上的一大创新,有良好的社会和经济效益。
Claims (5)
1.一种治疗精子DNA损伤的中药,其特征是:由以下重量计的原料药物:黄芪30-50g、枸杞子20-30g、五味子15-25g、车前子12-22g和川牛膝15-25g制成;其中,先将车前子用纱布包住,再与其他原料药物混合在一起,加入500-700ml水,浸泡20-30min,武火煮沸,改用文火煎煮30-60min,过滤,得第一滤液,滤渣加500-700ml水,武火煮沸,改用文火煎煮30-60min,过滤,得第二滤液,合并两次滤液,浓缩至200-400ml。
2.根据权利要求1所述的治疗精子DNA损伤的中药,其特征是:由以下重量计的原料药物:黄芪35-45g、枸杞子22-28g、五味子18-22g、车前子15-20g和川牛膝18-22g制成。
3.根据权利要求1所述的治疗精子DNA损伤的中药,其特征是:由以下重量计的原料药物:黄芪30g、枸杞子20g、五味子15g、车前子12g和川牛膝15g制成。
4.根据权利要求1所述的治疗精子DNA损伤的中药,其特征是:由以下重量计的原料药物:黄芪40g、枸杞子25g、五味子20g、车前子17g和川牛膝20g制成。
5.根据权利要求1所述的治疗精子DNA损伤的中药,其特征是:由以下重量计的原料药物:黄芪50g、枸杞子30g、五味子25g、车前子22g和川牛膝25g制成。
