Abstract

本发明提供了制备适于治疗诸如湿疹和牛皮癣 一类的皮肤病的草药组合物的方法。该方法包括通 过水蒸汽蒸馏、煎煮，然后降低萃取物中多糖或糖的 含量来制备草药的一种或多种萃取物，所用的草药可 提供抗炎剂、肾上腺皮质兴奋剂及可的松保护剂。

Description

本发明涉及用于治疗皮肤病，例如湿疹和牛皮癣的草药组合物萃取物及它们的制备方法。
湿疹和牛皮癣的病因学尚不完全清楚。可是使疾病特征化的症状学却描述得很透彻。最重要的特征是：炎症和瘙痒，后者导致进一步刺激、发炎和瘙痒循环。
湿疹和牛皮癣是不恰当的免疫应答的表达，通过这种不恰当的免疫应答，机体把它的一些组织成分视为异物并与之反应。治疗的目的是减少免疫反应的严重性和减轻症状。在常规的西医治疗中，使用局部外用可的松激素以减轻炎症和抑制免疫应答。润肤药用来保护机体并滋润皮肤，出现皮肤感染时则使用抗菌素。
然而中医医生使用草药煎剂口服和外用，治疗包括湿疹和牛皮癣的皮肤病。已经使用了许多种试剂，而且在中医里通常使用复合处方，该处方是医生细心检查了各个病人之后开出的。通过临床实验已经发现，一些草药混合物可以用来提供对大部分患湿疹和牛皮癣的病人都有效的组合物，而不依赖于逐个治疗。虽然中药的作用模式尚不完全清楚，但已经为干性、渗出型、感染型和苔癣化湿疹设计了不同的处方。
表Ⅰ给出了一系列传统使用的治疗皮肤病的中药和它们的主要化学成分及药理作用（Chang        and        But;Pharmacology        and        Applica-tions        of        Chinese        Materia        Media.World        Scientific        Publishi-ng        1986，Volumes        I        and    Ⅱ）。该表说明，许多这种传统使用的药物都具有适合于治疗湿疹和牛皮癣症状的药理性质即消炎、镇痛、解热、止痒、抗菌和免疫抑制活性。在列出的药物中，一些药物可以兴奋肾上腺皮质产生内源性的可的松激素，另一些则可以抑制一些组织例如皮肤和肺中的可的松衰竭。即使在目前，仍不确切地知道何种组分是混合物的有益治疗效应的组分，对湿疹和牛皮癣症状提供联合进攻的多种草药的组合也是合理的。
因为制备萃取物的传统方法是熬，即在水中煮沸，所以为了增加一些活性组分在水中的溶解度，混合物中的某些草药可能是必需的。在中药的处方中包括了一些草药，这些草药作为缓和药发挥作用，即它们具有润滑胃肠道和促进病人吸收的功效。中医医生坚信，草药混合物的毒性低于单味草药。虽然这点尚需在控制的临床试验下严格地加以证明，但常识指出一些草药成分具有加和性生理活性，而另外一些草药成分则拮抗活性成分的毒性。直到确定了活性成分，才证实了使用多种草药的煎煮液、萃取液或分馏萃取物是稳妥的。为了有实际用途，必需有一种或多种固定组分的混合物。令人惊奇的是，已经发现特殊草药的固定组分能用于治疗不同类型的湿疹和牛皮癣。
例如，已经发现在治疗干性（“全发红”）早期湿疹中有效的由十味草药组成的特别有用的制剂，可以这样配制：往盛有草药的长柄带盖（非铝质）平底锅中加水（每38.75g草药加约300ml水），煮沸并煨1.5小时。该操作期间，液体的体积减至每38.75g原草药约50ml液体。煎煮液的最终体积无关紧要，而且当从耗尽的草药中移走时可以稀释以供服用。从草药得到的煎煮液是每天现制备的。一些草药含挥发油，它们应于煎煮结束的前3分钟加入，以便减少挥发油的损失。
该煎煮方法虽然足以把活性组分萃取出来，但另外的大量非活性物质也会以多糖、色素、糖和鞣酸的形式被萃取出来。所以需要一种制备草药萃取物的方法，以该法制得的萃取物中活性成分更浓，所含的无关物质的量则比迄今所知的那些萃取物所含的量低很多。
本发明人已经设计了一种方法，此法可达到所述目的并可服用较小的剂量。
因此，按照本发明的制备治疗皮肤病组合物的方法包括：
（a）制备多种草药的一种或多种萃取物，所用草药经水蒸汽蒸馏和煎煮可以提供抗炎症剂、肾上腺皮质兴奋剂和可的松保护剂;
（b）以下述一种或多种方法降低多糖和/或糖的量，使之在一种或多种萃取物中的重量百分数至少低于5%，条件是基本上不降低存在于所述物质中的苷类的含量：
（ⅰ）发酵或酶解作用;
（ⅱ）用极性范围E°0.4至0.99的溶剂萃取，或用溶剂混合物萃取，该混合溶剂中至少一种溶剂的极性在所述的范围之内;
（ⅲ）用无机化合物和/或胶体沉淀;和
（c）用E°0.4至0.99极性范围内的溶剂或至少有一种溶剂的极性处于该范围的混合溶剂进一步萃取，以浓缩存在于萃取物中的活性组分。
通过在水中煮草药的传统煎煮方法，即有利于进行上述方法中步骤（a）的萃取物的制备。煎煮之前，任何挥发油或其它挥发性成分通过蒸汽蒸馏而除去，如果需要的话，保留下来，再加入到最后的萃取物中。在治疗湿疹和牛皮癣的制剂中，这些挥发性成分可以是特别有用的。
在本发明的方法中，步骤（b）中进行的减少多糖和/或糖的操作极为重要。含大量糖的萃取物极难挥发成自由流动的粉末，而且常常形成固态粘块。按重量计，多糖和/或糖的量最好减少至低于5%，若减少至低于25%则尤其好。因为一种或多种活性成分可能是苷，所以在基本上不降低苷含量的条件下进行降低过量的多糖和糖的步骤也很重要。
如果通过发酵来降低多糖和/或糖的含量，则优选方法是，把从步骤（a）中制备的煎煮液得到的全部水萃取物与大麦芽温育或与酵母例如啤酒酵母温育，或与真菌例如米曲霉温育，或者与其它能提供淀粉酶和/或糖解酶的微生物温育。分离的和纯化的淀粉分解酶和糖分解酶也可以使用，可任意选择地结合到膜上或其它载体上。如果这些酶也会破坏苷类，最好使用苷酶抑制剂。
发酵过程中可同时生成乙醇，它可用于活性成分的进一步分离。
如果用一种溶剂或至少有一种溶剂的极性处于E°0.4至0.99范围的混合溶剂进一步萃取开初的萃取物，以降低多糖和/或糖的含量，则溶剂最好是一种或多种下述溶剂：氯仿、二氯甲烷、乙酸乙酯、四氢呋喃、甲基乙基酮、丙酮、乙腈、丙醇、乙醇、甲醇、甲基化的工业酒精、或一种或多种上述化合物的不低于70%的水溶液中的一种或多种。特别优选的是70%的乙醇、70%的丙醇或70%的甲基化的工业酒精。
加入这些溶剂，造成多糖和/或糖分步沉淀，而所需的活性组分则保留在溶液中。
作为对步骤（b）中的发酵、酶处理和/或溶剂萃取的替代方法，通过用无机化合物和/或胶体沉淀，也可以降低多糖和/或糖的含量。优选的无机化合物是氢氧化钙。
在降低糖含量的步骤之后，一般需要进一步浓缩萃取液。特别希望除去多苯烃和鞣酸萃取物。在本发明的方法中，通过使用一种溶剂或其中至少有一种溶剂的极性处在E°0.4至0.95范围内的混合溶剂进一步萃取步骤（c），可以做到这一点。合适的溶剂可以从上面步骤b（ⅱ）给出的清单中选择。
由本发明的方法生产的萃取液可以蒸发成糊状，与赋型剂混合，并压成颗粒用于口服，还可任意选择地加入调味剂。
制备的萃取液可以用水溶液稀释，以便口服。本发明特别提供了含草药萃取物的组合物，该草药萃取物用本发明的方法制备，含有抗炎症剂、肾上腺皮质兴奋剂和可的松保护剂，并与适于药用的赋型剂、稀释剂或载体掺和，它们中的许多实例是本专业技术熟练人员所周知的。特别有用的载体是硅胶。
例如，采用生产润滑软膏或水可分散型乳剂的既定处方，可以制备该组合物的外用剂型。该组合物也可以按单位剂量形式制备成口服的片剂或胶囊。如果在最后的萃取步骤之后，把草药萃取液通过喷雾干燥、冷冻干燥或蒸发，加工成粉末，则有利于制成口服剂型。
中医认为实质上组合物中的所有草药都必需是活性的，而且草药最好以临时制备的煎剂给药。然而现在惊奇地发现，组合的草药制品的抗湿疹活性，如果不是唯一地，也是主要地存在于有限数量的草药中。所以通过限制所用的草药数量，有可能减少来自于组合物的非必需材料的量，而且有可能通过按照本发明制备已经除去无关物质的浓缩型组合物来进一步减少给药量。
从表Ⅰ给出的清单中选择的草药适用于本发明，特别优选的组合物是含有来自地黄、白鲜皮、甘草、和或者防风或者荆芥穗的萃取物的组合物。还可任意选择地包括刺蒺藜（白蒺藜）。另一个优选的组合物是含表Ⅰ中列出的前十种草药的组合物。
如果包括在混合物中的一味草药提供止痒剂，那是较好的。因为有些草药味苦，作为口服组合物，最好加入甜味剂。
根据本发明，制备草药萃取物时，可以把许多草药先混合在一起，再进行步骤（a）、（b）和（c）的操作，也可以把其中的一些或所有草药逐个进行步骤（a）、（b）、（c）的操作，然后混合最终的萃取物。
前一种方法的弊端是，来自每种草药的活性成分的产率一批不同于一批，以致于无从知道混合物中每种活性成分的最终浓度。而用分别制备各种草药的萃取物，然后再混合的方法，则各萃取物都可以用对照物进行检测，有助于病人服用剂量标准化。该法的另一个优点是，减低糖含量的整个操作只需要对混合物中多糖或糖含量高的那些草药进行，正是多糖或糖的高含量使得它们难以处理。
本发明现在列出下述非限制性实施例。
实施例1
将下述组分粗切或粉碎，水蒸汽蒸馏除去挥发油：

所得挥发油保留，残渣与1升水混合，煮沸1.5小时，冷却至35℃形成煎剂。
往水和草药的混合物中加100ml新鲜烘烤醇母的活泼生长的培养液，搅拌8小时或直至等分试样的糖含量低于0.4%。
粗滤除去植物性物质，离心除去酵母。滤液蒸发至干。
这样制备的萃取物用200ml乙酸乙酯搅拌10分钟，离心分离，保留溶液。再加50ml乙酸乙酯，混合物搅拌并分离。滤液合并后蒸发至干。
保留的挥发油返加到所得的蒸干物中，产品为棕色萃取物，能自由流动，加入合适的调味剂即能作为剂型使用。该产品加入赋型剂即可配制成常规的药剂用于治疗干性湿疹。上面给出的量足以为成人治疗十天。
实施例2
将下述草药粗切或粉碎，用4升水煎煮1小时：

离心分离出水层。萃取液蒸发至含50%固体（W/V），并加入10%重量的氢氧化钙。加入三倍体积的异丙醇并混合，混合物室温放置。移去上清液，并加入一倍体积的75%异丙醇，混合，残留物离心。所有的上清液合并，蒸发至干。生成的精细萃取物可以与药用助剂混合，并装入胶囊或压成片剂。

采用实施例2中描述的方法，从下述草药的混合物制备萃取物：

该组合物适于治疗进行性牛皮癣，而且上面给出的量适合于成年病人作十天治疗。
实施例7
按照实施例2中给定的方法，从下述草药的混合物制备精细的萃取物，然后与乳化软膏基质混合，生成油/水型乳化剂：
黄柏（20g草药的萃取物）
黄岑（20g草药的萃取物）
黄连（20g草药的萃取物）
Cetomacrogol乳化软膏加至100g。
实施例8
按照实施例2中描述的方法，从苦参制备精细的萃取物，并按照下述配方掺入到乳化软膏基质之中：
苦参（20g草药的萃取物）
乳化蜡        30g
硬石蜡        5g
鱼肝油        15g
月见草油        15g
白色软石蜡加至100g
该软膏可以薄薄地涂于皮肤上，并能与水混合，生成水包油型的润肤乳剂。
实施例9
按照实施例2中描述的方法，从掌叶大黄制备精细的萃取物，并掺入到乳化软膏基质中：
掌叶大黄（Rheum        Palmatum）（30g草药的萃取物）
Cetomacrogol乳化软膏加至100g。
本例中，以及实施例7和8中草药萃取物的量是说明性的，并不限于给出的比例。
实施例10
采用实施例1中给出的方法，从列在实施例1中的38.75g草药制备煎剂，终体积为50ml。使用合适的测定盒测定该煎剂的近似稀释物在加入3ml新鲜的10%酵母悬浮液前和加入后的间隔里的含糖量。混合物在30℃温育，偶尔搅拌。

发酵之后，虽然还有含在混合物中的甘草造成的残余甜度，但煎剂已明显不甜。所得溶液过滤并蒸发至干，生成4.7g萃取物。
实施例11
将下述草药粗切，并用水蒸汽蒸馏除去挥发油：

蒺藜子        300
虫胶（Lacca）        450
木通        300
甘草        150
地黄        450
赤芍        300
淡竹叶        300
白鲜皮        450
荆芥        150
3100g
挥发油保留待用，残渣与1升水混合，煮沸1.5小时，冷却至35℃，形成煎剂。
100ml新鲜烘烤酵母的活泼生长的培养液加到水和草药的混合物中，保持搅拌8小时或直至一个等分试样的糖含量低于0.4%。
粗滤除去植物性物质，离心除去酵母。滤液蒸发至干。
这样制得的萃取物用200ml        70%的甲基化的工业酒精搅拌10分钟，离心分离，所得溶液储备待用。再加入50ml        70%的甲基化工业酒精，混合物搅拌并分离。合并的滤液蒸发成稠糖浆。测定每ml的重量，并加入该重量三分之一的胶态硅，萃取液蒸发至干。
生成的干萃取物显棕色，自由流动，加入适当的调味剂即能以粉剂、颗粒剂、胶囊剂或片剂使用，治疗干性湿疹。这些量足够成年病人治疗二十天。

Claims (25)

1、制备治疗皮肤病组合物的方法，该方法包括：
(a)制备多种草药的一种或多种萃取物，所说草药以水蒸汽蒸馏和煎煮，能提供抗炎剂、肾上腺皮质兴奋剂和可的松保护剂；
(b)采用一种或多种下述方法降低多糖和/或糖的量，使之在一种或多种萃取物中的重量百分含量至少低于5%，条件是基本上不降低存在于所说物质中的苷类的含量；
(i)发酵或酶解作用，
(ii)用极性范围E°0.4至0.99的溶剂萃取，或用混合溶剂萃取，该混合溶剂中至少一种溶剂的极性在所述范围内，
(iii)用无机化合物和/或胶体沉淀，
(c)用E°0.4至0.99极性范围内的溶剂，或至少有一种溶剂的极性处于该范围内的混合溶剂进一步萃取，以浓缩存在于萃取物中的活性组分。
2、根据权利要求1的方法，其中在步骤b（ⅰ）中，使用大麦芽或诸如啤酒酵母或米曲霉一类的酵母发酵来降低多糖和/或糖的含量。
3、根据权利要求1的方法，其中在步骤b（ⅰ）中，使用分离的淀粉分解酶或糖分解酶，任意选择地在苷酶抑制剂存在下，来降低多糖和/或糖的含量。
4、根据权利要求3的方法，其中酶被固定在膜或其它载体上。
5、根据前面任何一项权利要求的方法，其中当在步骤b（ⅱ）的方法中，通过活性剂的溶剂萃取降低多糖和/或糖的含量时，溶剂为一种或多种下述溶剂：氯仿、二氯甲烷、乙酸乙酯、四氢呋喃、甲基乙基酮、丙酮、乙腈、丙醇、乙醇、甲醇、甲基化工业酒精或一种或多种上述溶剂的浓度不低于70%的水溶液。
6、根据权利要求5要求的方法，其中所说的溶剂为乙酸乙酯或70%的乙醇、70%的丙醇或70%的甲基化工业酒精的溶液。
7、根据前面任一项权利要求的方法，其中当在步骤（c）中用沉淀法降低多糖和/或糖的含量时，无机化合物为氢氧化钙。
8、根据前面任一项权利要求的方法，其中在步骤（c）的溶剂萃取法中，溶剂为一种或多种下述溶剂：氯仿、二氯甲烷、乙酸乙酯、四氢呋喃、甲基乙基酮、丙酮、乙腈、丙醇、乙醇、甲醇、甲基化工业酒精或一种或多种上述溶剂的浓度不低于70%的水溶液。
9、根据权利要求8的方法，其中所述的溶剂为乙酸乙酯或70%的乙醇、70%的丙醇或70%的甲基化工业酒精的溶液。
10、根据权利要求8或9的方法，其中从萃取物中除去鞣酸和多苯类化合物。
11、根据前面任一项权利要求的方法，其中在水蒸汽蒸馏中除去的挥发油或其它挥发性组分被保留下来，并再加入到组合物中。
12、根据前面任一项权利要求的方法，其中得到的草药萃取物例如通过蒸发、喷雾干燥或冷冻干燥被加工成粉末。
13、根据权利要求1至12中任一项的方法，其中得到的草药萃取物干燥成糊剂，与赋型剂混合并压成颗粒。
14、根据前面任一项权利要求的方法，其中用于制备权利要求1的步骤（a）的一种或多种萃取物的草药选自表1中列出的草药。
15、根据权利要求14的方法，其中一种或多种萃取物含有来自下列草药的成分：
（a）地黄，提供可的松保护剂和肾上腺皮质兴奋剂，
（b）白鲜皮，提供生物碱成分，
（c）甘草，提供肾上腺皮质兴奋剂，
（d）防风或荆芥穗，提供抗炎剂。
16、根据权利要求15的方法，其中也使用：
（e）刺蒺藜。
17、根据权利要求14的方法，其中用来制备萃取物的草药是表1中的前十味草药。
18、含有抗炎剂、肾上腺皮质兴奋剂和可的松保护剂的草药萃取物，它是用前面权利要求中任一项所要求的方法制备的。
19、含有权利要求18所要求的萃取物的治疗皮肤病的组合物。
20、与药用赋型剂、稀释剂或载体掺合得到的如权利要求19所要求的组合物。
21、权利要求20所要求的组合物，其中载体为硅胶。
22、权利要求19或20所要求的组合物，该组合物按单位剂型制备。
23、权利要求19所要求的组合物，它的剂型是水溶液。
24、权利要求19或20所要求的组合物，它的剂型是软膏或乳剂。
25、权利要求18所要求的草药萃取物在制备口服或外用治疗皮肤病的药物中的用途。
