本发明属于医疗器械领域,公开了一种骨软骨缺损修复材料及其制备方法,本发明所述骨软骨缺损修复材料由关节骨软骨脱细胞和富集血小板血浆制成。其中所述关节骨软骨脱细胞来源广泛,不仅可以为自体来源的骨软骨微粒也可以为异体来源的骨软骨微粒,进行脱细胞处理获得关节骨软骨脱细胞,可避免多次手术,减少患者创伤。本发明所述骨软骨缺损修复材料中所述富集血小板血浆中含有大量促进软骨组织修复的生长因子,这些生长因子在局部的逐步释放,将诱导干细胞和其他修复细胞增殖,并一步产生修复功能。本发明所述骨软骨缺损修复材料不需要体外培养,可直接用于软骨缺损处的修复。且本发明所述骨软骨缺损修复材料中无细胞成分,免疫原性较低。

一种骨软骨缺损修复材料及其制备方法
技术领域
本发明属于医疗器械领域,具体涉及一种骨软骨缺损修复材料及其制备方法,尤其是一种复合自体PRP的软骨修复材料及其制备方法。
背景技术
软骨是胚胎早期的主要支架成分,随着胎儿发育逐渐被骨取代,取代过程一直延续到出生后一段时期。是特化的致密结缔器官,细胞间质坚固而有弹性。在胎儿和年幼期,软骨组织分布较广,后来逐渐被骨组织代替。在成体内,仅散在分布一些软骨,其作用依所处部位而异。如存在于骨的关节面关节软骨,在人体中起到承重负荷,减少关节间骨骼摩擦等重要的作用。
关节软骨属于透明软骨,表面光滑,呈淡蓝色,有光泽,它是由一种特殊的叫做致密结缔组织的胶原纤维构成的基本框架,这种框架呈半环形,类似拱形球门,其底端紧紧附着在下面的骨质上,上端朝向关节面,这种结构使关节软骨紧紧与骨结合起来而不会掉下来,同时当受到压力时候,还可以有少许的变形,起到缓冲压力的作用。在这些纤维之间,散在分布着软骨细胞,软骨细胞由浅层向深层逐渐由扁平样至椭圆或圆形的细胞组成,这些软骨细胞维持关节软骨的正常代谢。关节软骨没有神经支配,也没有血管,其营养成分必须从关节液中取得,而其代谢废物也必须排至关节液中,关节软骨的这种营养代谢必须通过关节运动,使关节软骨不断的受到压力刺激才行,所以关节运动对于维持关节软骨的正常结构起重要的作用。
关节软骨缺损在骨科临床十分常见,患者常常出现关节疼痛、活动受限,严重影响患者生活质量,甚至导致残废。目前临床治疗手段多样,往往根据患者病情发展的不同阶段,而选择不同的治疗手段。目前对于软骨大面积缺损的重度骨关节炎,且保守治疗无效时,可考虑行人工关节置换。软骨缺损面积<1.5cm2,可以选择关节镜下的“微骨折”技术。对于软骨缺损面积>1.5cm2可选用自体骨软骨复合组织移植。移植自体骨软骨复合组织,则需要在非负重部位取材,因此来源受限,且存在取材的并发症,增加了手术创伤和手术时间。选用异体骨软骨移植物,则面临免疫排斥和疾病传播的风险,同时异体组织在软骨缺损区域的改建过程也较缓慢。
发明内容
有鉴于此,本发明的目的在于针对现有技术中存在的问题,提供一种新的骨软骨缺损修复材料及其制备方法,以修复软骨缺损。
为实现本发明的目的,本发明采用如下技术方案。
一种骨软骨缺损修复材料,由关节骨软骨脱细胞和富集血小板血浆制成。
其中,富血小板血浆(Platelet-rich plasma,PRP),是全血经离心后得到的血小板浓缩物。所述富集血小板血浆中含有大量、多种生长因子,可以促进多种组织修复。以关节骨软骨脱细胞为基质载体,复合富集血小板血浆,具有较强的促进软骨缺损修复的能力,达到修复软骨缺损的目的。本发明所述骨软骨缺损修复材料中无细胞成分,因此免疫原性较低。
本发明所述骨软骨缺损修复材料中所述富集血小板血浆可以从外周静脉血中分离获得,可以从自体外周静脉血中分离。
作为优选,本发明所述的骨软骨缺损修复材料中,所述关节骨软骨脱细胞的制备方法为关节部位骨软骨复合组织切割成柱状骨软骨微粒,然后进行脱细胞处理获得关节骨软骨脱细胞。
其中,所述骨软骨微粒为0.5×0.5×0.5cm至1×1×1cm的柱状骨软骨微粒。
所述骨软骨微粒可以为自体来源的骨软骨微粒,也可以为异体来源的骨软骨微粒。
进一步的,所述脱细胞处理具体为骨软骨微粒清洗,消化酶37℃,消化24 h;所述消化酶由1 U/ml RNase A、50 U/ml DNaseⅠ组成。
其中,所述脱细胞处理中所述清洗为骨软骨微粒置于含蛋白酶抑制剂的10 mMTris-HCl 液中,离心清洗后加入含在1%Triton X-100 的pH 7. 5的Tris-HCl 缓冲液中震荡48 h。
作为优选,本发明所述骨软骨缺损修复材料中所述关节骨软骨脱细胞与富集血小板血浆的体积比为1:1。
本发明还提供了所述的骨软骨缺损修复材料的制备方法,将关节骨软骨脱细胞加入富集血小板血浆混合均匀使得富集血小板血浆充分进入关节骨软骨脱细胞。
其中,在一些实施方案中,所述混合具体包括以下步骤:
A、将关节骨软骨脱细胞浸泡在富集血小板血浆中1分钟,然后无菌纱布吸调液体,进行预处理;
B、将关节骨软骨脱细胞浸泡在另外富集血小板血浆中3-5分钟,使得PRP液体充分进入移植物中。
本领域技术人员可以理解本发明对步骤A中的富集血小板血浆的体积没有限制,只要能够保证关节骨软骨脱细胞完全浸入其中即可。在一些实施例中,步骤A中的富集血小板血浆的体积为1ml。
作为优选,所述关节骨软骨脱细胞预处理后可以在浸泡PRP液体中后进行振荡,使得PRP液体充分进入移植物中。
作为优选,所述步骤B中的富集血小板血浆体积与关节骨软骨脱细胞的体积比为1:1。
本发明所述骨软骨缺损修复材料在体外可以采用3D生物打印方式或者其它细胞-材料构建方式进行处理,形成具有一定形态和支撑强度的移植物。
采用该移植物对软骨缺损进行修复时首先需要对软骨损伤区域进行处理,刮除明显退变松动的残留软骨组织,保留软骨下骨板,避免骨髓大量渗透到修复创面。然后将上述骨软骨缺损修复材料固定在软骨缺损区域。本领域技术人员可以理解固定方式可以是生物蛋白凝胶黏合等。修复术后在一定时间应避免患肢负重,但可以早期屈伸关节。
由上述技术方案可知,本发明提供了一种骨软骨缺损修复材料由关节骨软骨脱细胞和富集血小板血浆制成。本发明所述骨软骨缺损修复材料中所述关节骨软骨脱细胞来源广泛,不仅可以为自体来源的骨软骨微粒也可以为异体来源的骨软骨微粒,进行脱细胞处理获得关节骨软骨脱细胞,可避免多次手术,对软骨缺损创面的形态也无特殊要求,减少患者创伤。本发明所述骨软骨缺损修复材料中所述富集血小板血浆中含有大量促进软骨组织修复的生长因子,这些生长因子在局部的逐步释放,将诱导干细胞和其他修复细胞增殖,并一步产生修复功能。本发明所述骨软骨缺损修复材料不需要体外培养,可直接用于软骨缺损处的修复。且本发明所述骨软骨缺损修复材料中无细胞成分,免疫原性较低。

实施例1、骨软骨缺损修复材料的制备。
1、骨软骨移植物-关节骨软骨脱细胞的准备。
将捐赠的关节部位骨软骨复合组织进行切割处理,使之形成0.5×0.5×0.5cm至1×1×1cm的柱状骨软骨微粒。然后进行脱细胞处理,具体为:骨软骨块置于含蛋白酶抑制剂的10 mM Tris-HCl 液中,3000 转离心机清洗3 遍后置于10 mM Tris-HCl 液中备用。将骨软骨组织块中加入1%Triton X-100 Tris-HCl 缓冲液( pH 7. 5) ,恒温震荡机上震荡48h; 骨软骨块在消化酶液( 1 U/ml RNase A,50 U/mlDNaseⅠ) ,37℃,消化24 h,0. 01 MPBS 液反复冲洗,37℃烘干后,密封包装,辐照灭菌备用。
2、PRP制备:
梯度离心制备富血小板血浆。详细步骤如下。
全血采集完毕后,放22℃±2℃的无菌环境下振摇30min;接着进行样本离心:200ml全血要求离心转速为2000转、离心13分钟、温度22℃;第1次离心后采用白膜法分出白膜层(即少量血浆及白膜),若需要保存,可放置在22℃±2℃振摇箱上进行振摇保存,不宜超过24小时。也可以直接取出白膜层进行离心,要求离心转速为600转、离心10分钟、温度22℃±2℃。白膜层离心后再次分出上层的血浆及血小板,弃去下面白膜即制备成富血小板血浆(PRP),称重量后留取标本做计数并记录。
3、骨软骨缺损修复材料的体外制备。
首先将所述骨软骨移植物关节骨软骨脱细胞浸泡在1mlPRP液体中1分钟,然后无菌纱布吸调液体,进行预处理。然后将关节骨软骨脱细胞浸泡在PRP液体中3-5分钟,可以震荡,使得PRP液体充分进入移植物中,备用。其中,所述PRP液体体积与关节骨软骨脱细胞的体积比为1:1。

在贵州小香猪膝关节股骨滑车区域做个直径8mm骨软骨缺损,深度8mm(含软骨下骨5mm),植入同实施例1制备的直径8mm的骨软骨移植物,深度8mm,然后关闭切口。术后自由活动。3月后切开膝关节,肉眼大体观,见软骨缺损修复满意,无软骨脱落和分层,关节腔积液也不明显。取出修复区域的组织标本,固定标本,脱钙处理,然后进行常规的HE染色分析,结果见图3和图4。
图3和图4结果可见软骨缺损充填软骨组织,移植骨与宿主骨整合好,整个病变被移植材料较好的修复。

Claims (7)

1.一种骨软骨缺损修复材料,由关节骨软骨脱细胞和富集血小板血浆制成;
所述关节骨软骨脱细胞的制备方法为关节部位骨软骨复合组织切割成柱状骨软骨微粒,然后进行脱细胞处理获得关节骨软骨脱细胞;
所述柱状骨软骨微粒为0.5×0.5×0.5cm至1×1×1cm的柱状骨软骨微粒。
2.根据权利要求1所述的骨软骨缺损修复材料,其特征在于,所述脱细胞处理具体为柱状骨软骨微粒清洗,消化酶37℃,消化24 h;所述消化酶由1 U/mL RNase A,50 U/mL DNaseⅠ组成。
3.根据权利要求2所述的骨软骨缺损修复材料,其特征在于,所述清洗为柱状骨软骨微粒置于含蛋白酶抑制剂的10 mM Tris-HCl 液中,离心清洗后加入含在1%Triton X-100 的pH 7. 5的Tris-HCl 缓冲液中震荡48 h。
4.根据权利要求1-3任意一项所述的骨软骨缺损修复材料,其特征在于,所述关节骨软骨脱细胞与富集血小板血浆的体积比为1:1。
5.权利要求1-4任意一项所述的骨软骨缺损修复材料的制备方法,其特征在于,将关节骨软骨脱细胞加入富集血小板血浆混合均匀使得富集血小板血浆充分进入关节骨软骨脱细胞。
6.根据权利要求5所述的制备方法,其特征在于,所述混合具体包括以下步骤:
A、将关节骨软骨脱细胞浸泡在富集血小板血浆中1分钟,然后无菌纱布吸调液体,进行预处理;
B、将关节骨软骨脱细胞浸泡在另外富集血小板血浆中3-5分钟,使得PRP液体充分进入移植物中。
7.根据权利要求6所述的制备方法,其特征在于,所述步骤B中的富集血小板血浆体积与关节骨软骨脱细胞的体积比为1:1。
