我们可以快速轻松地对许多微生物类型,特别是细菌和单细胞真菌,进行质谱鉴定:将少量微生物从普通方法培养的菌落或从营养培养基中的菌落转移到质谱样品载板,并在载板中与含有基质的溶液混合,然后在基质辅助激光解吸电离后进行质谱测量。如果在微生物中的蛋白质有足够的浓度,则质谱图可显示特征蛋白质的质量和强度。该质谱图每次可显示微生物中大约40至80种蛋白质的峰值,可通过利用质谱库中成千上万的参照质谱进行相似性分析来确定其身份。这里"鉴定"一词表示分类学鉴定,例如,科、属和种的确定。许多地方正在进行研究,以便对具有成千上万微生物的参照质谱库进行整理,这些是可靠的且经过法律批准的医用质谱(所谓的"已验证"的质谱库)。
在许多研究中以及微生物实验室的日常工作中,这种鉴定微生物的方法已被证明非常成功。这种方法十分快速,成本低,而且误差率极低,远低于传统微生物鉴定方法。
这些方法的特殊版本不仅可以用于鉴定微生物种类,而且还经常用于鉴定亚种,有时甚至用于鉴定株系,如果它们与常见且可质谱检测的蛋白质在质量和强度方面存在不同。关于更多详细描述,请参阅专利申请例如DE 10 2009 032 649 A1(T.Maier和M.Kostrzewa,2009,US2011/0012016 A1;GB 2 471 746 A),其不仅提供了该方法的详细说明,而且还提供了更好的品种搜索方法。
微生物的鉴定对于传染病特别是败血症非常关键。重要的一点是其能够非常快速的鉴定病原体的种类,以便立即采取正确的治疗措施。质谱鉴定已在这些方面接受了试验和验证,目前正在逐渐获得临床和微生物实验室的认可。
然而,在医学领域不仅需要快速鉴定,而且还需要检测出对常用抗生素的抗药性。在不知道抗药性的情况下将无法快速控制疾病。因此,不仅有必要进行快速鉴定,而且还有必要快速确定并辨别微生物物种的抗药性。人们已经知道一些微生物物种对特定抗生素几乎具有完全的抗药性,因此在精确鉴定后才确定抗药性毫无意义。然而,大多数情况下,物种会有无抗药性的株系,还会有轻微抗药性,特别是高抗药性的株系,抗药性会随着抗生素种类的不同而不同。因此,确定抗药性的类型和强度非常重要。
似乎显而易见的是,质谱仪不仅可用于分类学鉴定,而且还可用于确定微生物对特定抗生素的抗药性,特别是细菌的抗药性。但是,已证明这一任务非常艰巨。尽管抗药性必须通过新蛋白质或改性蛋白质的存在来表示,但迄今为止还未证明可在质谱仪测量的蛋白质表达谱中直接进行鉴定。毕竟,在微生物的成百上千或甚至成千上万的蛋白质中,质谱图内仅能测量40至80种蛋白质。因此必须间接地确定抗药性。这种抗药性确定的首次尝试在文件DE 10 2006 021 493 B4(V.Govorun和J.Franzen;GB 2 438 066 B;US 2008/0009029 A1)中介绍;但是这种方法迄今为止还未得到认可。该方法主要基于在加入抗生素后因细胞死亡而引起的蛋白质表达谱的变化,亦或基于与抗药性参照微生物的比较而确定的繁殖停止。
抗生素抗药性一词归类了微生物种的特征(这里主要是细菌),即减弱或完全中和抗生素活性物质的作用。抗药性现在很普遍,在美国,医院内获得的大约70%的传染性细菌都至少对一种抗生素具有抗药性。患者经常会传染那些对若干种抗生素具有抗药性的细菌株系(多抗药性)。所谓的问题细菌是指抗甲氧西林性金黄色葡萄球菌(MRSA)、假单胞菌属、带有超广谱β-内酰胺酶(ESBL)抗药性的大肠杆菌及结核分枝杆菌。由CDC(美国疾病控制与预防中心)所做的评估认为2004年在医院有两百万传染病例,大约有九万人死亡,远远高于因交通事故或家庭或工业事故而导致的死亡人数。
日常使用的抗生素一词通常表示用于治疗细菌传染性疾病的药剂或药物产品。抗生素在医学界取得的巨大成功首先是青霉素。虽然青霉素取得了成功,但也首次出现了抗药性,这促使研究人员去寻找和发现更多的抗生素:链霉素、氯霉素、金霉素、四环素及许多其他抗生素。现在已知的大部分抗生素都源自于自然物质。通俗的讲,青霉素现在是抗生素的代名词。
青霉素是β-内酰胺类抗生素。这些β-内酰胺与青霉素结合蛋白(PBP)结合,即负责形成加固细胞壁的肽键的肽聚糖转肽酶。在β-内酰胺和PBP间的结合导致PBP失效。随着细菌的生长,有效PBP的数量不足会导致细胞壁损害,细胞膜会因此失去对渗透性的控制而无法调节细胞质的浓度。短时间后细菌就会死亡。在极端条件下,可在实验室内观察到完全"破裂"的细菌细胞。这是β-内酰胺的杀菌方式。
自从首次使用青霉素后,细菌已逐渐发展出不同类型的抗药性。细菌对β-内酰胺产生抗药性的一种重要类型是形成酶(β-内酰胺酶),这种酶通过水解,以催化的方式破坏β-内酰胺环,并因此致使β-内酰胺失效。现在已知的β-内酰胺酶有340多种,由多种类型的细菌形成。可根据细菌的总体结构或作用方式将它们分成不同类别。通过突变最初产生的酶合成遗传信息由染色体或质粒继承。质粒信息可通过各种途径在细菌之间转移,甚至可通过接触在不同物种的细菌之间转移("水平转移")。根据β-内酰胺酶的作用,可对青霉素酶和头孢菌素酶进行区别,但还有更多的类别。这些酶的催化作用意味着少量的β-内酰胺酶就足够破坏大量的β-内酰胺类抗生素。
现在存在大量β-内酰胺类抗生素的衍生物,在它们中有若干种青霉素(苄青霉素、口服青霉素、氨基青霉素、异恶唑青霉素、酰基氨基青霉素)、头孢菌素、单环β-内酰胺类和碳青霉烯类。这些衍生物通常具有较大的化学基团,以便对β-内酰胺酶产生空间位阻作用。反过来,超广谱β-内酰胺酶(ESBL)可分裂多种含有β-内酰胺的抗生素。ESBL最初由β-内酰胺酶上的点突变形成。ESBL的基因位于可从细菌到细菌水平转移的质粒上。
ESBL携带细菌对青霉素、头孢菌素(1-4代)及单环β-内酰胺类有抗药性。携带ESBL基因的主要是大肠杆菌和克雷伯氏菌(革兰氏阴性菌),但是微生物学家们焦虑地观察到这些ESBL抗药性的快速蔓延。除抗药性金黄色葡萄球菌(MRSA)外,超广谱β-内酰胺酶(ESBL)是传染研究中最令人担心的菌类之一。
β-内酰胺酶抑制剂是对抗β-内酰胺酶的一种手段,与β-内酰胺类一起使用以减弱存在于细菌中的β-内酰胺酶的作用。已确立的组合是克拉维酸+氨基青霉素、舒巴坦+氨苄青霉素、他唑巴坦+哌拉西林。不是所有的组合都有最佳的作用。这些手段仅可在仔细鉴定细菌并谨慎确定其抗药性后使用,因为这些手段也可能非常快速的失效。
在二十世纪的70年代和80年代,在抗生素领域进行了很多科研活动。现在对新抗生素的研发已经大大的减少了,尽管抗生素是世界上最常用的处方药之一,占有13%的市场份额,它们是我们使用药品的最大类别。现在已知的抗生素物质大概有8,000种,却仅仅大约有80种用于治疗,这主要是由于副作用和审批成本。根据德国联邦药品和医疗器械研究所(BfArM)的统计,2005年在德国总共有2,775种抗生素获得批准,但是仅仅涵盖了大约80种抗生素物质。
随着时间的推移,微生物种对诸如杀细菌剂或杀真菌剂等抗生素产生抗药性这一问题会变得越来越紧迫。一方面,微生物种对不同类型的抗生素产生抗药性的速度正在增加;而另一方面,正在研发的新的医用抗生素越来越少。由于没有作用,许多新抗生素不得不在投入市场一段时间后退出市场,对医药公司来说,研发新的抗生素越来越困难,而投资这一领域的利润也越来越少。根据世界卫生组织(WHO),在1990到2005年间仅正式推出了3种新的抗生素活性物质,与之对比,1940到1950年间推出了10种新的抗生素活性物质,1971到1980年间为5种新的抗生素活性物质。
抗药性快速增加的原因多种多样:不负责任滥用抗生素,甚至没必要时也使用抗生素;在传染媒介完全被破坏之前不负责任地中断杀菌剂治疗;在农业和动物饲养中不负责任地、经常为纯粹预防而使用抗生素。与非抗药性物种相比,所有这些做法都促进了抗药微生物物种的选择和传播。
作为治疗传染病的新希望而闻名于上个世纪中叶的抗生素,现在正在快速地失去它的疗效。避免这一趋势的唯一希望就是有针对性地使用治疗手段,而且治疗必须全部完成,这需要快速鉴定传染病原体和快速鉴定其对不同类型抗生素的特定抗药性。
发明目的
发明目的是提供一种方法,即可以快速简单地通过质谱测量来确定微生物、特别是细菌对不同类型的β-内酰胺类抗生素的β-内酰胺酶抗药性。
发明概要
本发明提供了一种方法,即可非常方便快速地使用质谱仪测量微生物由于β-内酰胺酶而具有的抗药性。该方法可在微生物与相应的底物(β-内酰胺类抗生素或定制β-内酰胺类衍生物)混合几小时后,通过直接质谱测量β-内酰胺酶在底物上的水解破坏来确定其细菌的抗药性。细菌在底物上产生的β-内酰胺酶的催化作用导致β-内酰胺环水解分裂开。底物量减少,取而代之的是出现水解的分裂产物,其质量比抗生素多出18个原子质量单位。
酶的分裂反应非常迅速,但前提是其不会受到底物逐渐减少的影响,则每个分子反应时间大约是1到100毫秒,不同的β-内酰胺酶的特征差异也有所不同。测量反应速度可提供关于β-内酰胺酶类型和强度的初始信息。
原则上可使用任何质谱仪进行测量,但是最好使用进行细菌鉴定的同样的基质辅助激光解吸电离(MALDI)飞行时间质谱仪。由于用于在几百个原子质量单位的较低质量范围内对物质进行电离的MALDI过程(基质辅助激光解吸的电离)将会产生非常强的化学背景,因此最好使用位于低背景区域的底物。这可通过生成分子量在700到1,200原子质量单位的定制底物来实现。还有利的是,增加底物的质子亲和力以增加电离程度。不仅如此,如果能够使用较高浓度的底物以增加灵敏度,效果会更好。为了使高浓度的高杀菌作用不会立即杀死细菌,可定制底物使得其抗生素作用(MIC值)相对较小。MIC是在相应抗生素存在下通过β-内酰胺酶抑制剂抑菌的"最低抑制浓度",可作为测量抗药性强度和抗生素强度的衡量指标使用。
例如,较好的底物实例是通过共价键的方式将6-His标签与β-内酰胺结合。6-His标签由六个组氨酸组成,将分子量增加了大约800原子质量单位,提高了质子亲和力,并且还可从反应液体中提取纯形式的底物及其分解产物。例如,这种提取可借助使用市售的磁珠进行。在磁珠上有加载镍离子的螯合物,其可通过可逆的方式结合6-His标签。然后就可准备MALDI样品。该样品的基质包含经过高度富集且提纯的剩余底物及其分解产物,因此可实现非常灵敏的测量。
特别是,可通过引入若干种不同类型底物来测定特定的多抗药性,但是不同底物的定制方式以及它们的引入浓度都必须确保其分解方式可用来鉴定细菌的类别以及β-内酰胺酶的作用强度。例如,底物可模仿与不同类别抗生素的β-内酰胺环的接触。
附图说明
图1中顶部的质谱图显示了原子质量单位为349.41摩尔质量的混合氨苄青霉素对于无抗药性的DH5a株系大肠杆菌悬浮液的作用。氨苄青霉素(这里质量为350原子质量单位)和氨苄青霉素钠盐(质量为372原子质量单位)均未发生分解。相对而言,底部的质谱图显示了ESBL抗药性株系大肠杆菌对氨苄青霉素及其钠盐的作用:二者均被分解,分解产物的质量分别为368和390原子质量单位。
优选的实施方式
本发明可提供了一种根据微生物(特别是细菌)产生β-内酰胺酶来快速方便确定微生物抗药性的方法。
该方法主要是向细菌悬浮液中加入一种或多种合适的底物。底物可以是β-内酰胺类抗生素或更适宜的定制β-内酰胺类衍生物。如果细菌具有β-内酰胺酶抗药性,则在合适的培养条件下经过数分钟至数小时,β-内酰胺酶会通过水解破坏β-内酰胺环分解至少一种底物。β-内酰胺酶对底物的水解可直接进行质谱测量。底物量减少,取而代之的是水解的分裂产物,其质量比抗生素多出18个原子质量单位。
在图1中,在两个质谱图中使用β-内酰胺类抗生素氨苄青霉素时,显示仅在具有抗药性时发生的这种水解。上部的质谱图显示了原子质量单位为349.41摩尔质量的氨苄青霉素与DH5a株系大肠杆菌悬浮液混合的结果。该株系没有抗药性。因此,可以看到氨苄青霉素(这里质量为350原子质量单位)和氨苄青霉素钠盐(质量为372原子质量单位)均未发生分解。相对而言,底部的质谱图显示了ESBL抗药性株系大肠杆菌对氨苄青霉素及其钠盐的作用:二者均被分解,分解产物的质量分别为368和390原子质量单位。
氨苄青霉素是一种半合成产品,抗菌活性药物来自β-内酰胺类抗生素(青霉素)的基团。因为其可抑制革兰氏阳性病原体和一些革兰氏阴性杆菌,因此作为广谱抗生素而为人们所熟知。从化学角度上讲,氨苄青霉素是一种氨基青霉素。
与所有β-内酰胺类抗生素一样,氨苄青霉素的杀菌(杀死细菌)作用是对以多种形式存在于不同细菌中的D丙氨酸转肽酶的抑制作用。这种酶是细菌生长和分裂阶段形成新的坚固细胞壁的必须物质。这些转肽酶也叫做青霉素结合蛋白(PBP)。抑制作用以附着形式进行,β-内酰胺环为附着基序。附着可阻止新的坚固细胞壁的合成。细胞因此无法分裂,但最初仍能存活直至其生长导致足够多的细胞壁损坏从而导致细胞的死亡。然而人体细胞的分裂和生长却不会受到阻碍,因为人体细胞只有细胞膜,没有细胞壁,因此没有相应的转肽酶。
如图1的示例中,将10微升浓度10毫克/毫升的氨苄青霉素水溶液加入到Eppendorf试管内。从细菌中选取三组菌落来进行试验,然后将这些菌落放置于10微升的氨苄青霉素溶液中进行重新悬浮处理。然后将容器在搅拌作用下置于37°C下培养3小时。在完成培养后,将其以13000转/分的速度离心分离2分钟,以便将细胞分离出来。剩余的氨苄青霉素和水解反应产物现在处于上清液中。
原则上可使用任何质谱仪进行测量,但是最好使用进行细菌鉴定的同样的MALDI飞行时间质谱仪。为此,从该上清液中取1.5微升置于质谱样品载板上。样品干燥以后,将样品包涂1微升基质溶液。使用的基质是α-氰-4-羟基肉桂酸(HCCA),其与水混合,浓度为10毫克/毫升,其中含50%乙腈和2.5%三氟乙酸。样品再次干燥后,可采用普通的方法使用MALDI飞行时间质谱仪取得制剂的质谱。
作为该示例底物的氨苄青霉素的浓度非常高,要高出治疗所需浓度的1000多倍。事实是这个数量下的氨苄青霉素也被分解,显示了超广谱β-内酰胺酶(ESBL)的非凡作用。很难想象细菌可在这种高浓度下长时间存活,然而,在其寿命期间释放的少量β-内酰胺酶就足可以催化分裂大量的底物。选择高浓度是因为可以清楚地看到高于此质量范围内的高化学背景的信号。如果使用质量范围大约在800到1000的原子质量单位的底物,则可以使用减少100倍的浓度,这可通过带有较高分子量的定制底物来实现。
这也有利于增加底物的质子亲和力,以便增加电离率。带有低质量的β-内酰胺类没有高质子亲和力,因此仅有一小部分的β-内酰胺类在电离过程中发生电离。例如,可通过插入带有高质子亲和力胺基酸将灵敏度进一步增加10倍。
不仅如此,使用较高浓度的底物还往往有利于进一步增加灵敏度。但是为了避免那些带有较弱β-内酰胺酶的细菌被高杀菌效果立即杀死,可这样定制底物,使其抗生素作用(MIC值)相对较小。抗生素的作用通常随着分子尺寸的增加而降低,因为这些大分子渗入细胞壁孔隙而进入细菌内的过程被大大阻碍。
此外,以这种方法定制底物还有利于完全地、方便地从上清液中提取底物。为此,准备的底物可带有锚定基团,这样可使用经固定的配体(partner)来提取底物。生物素基团对底物的吸附是本文描述的第一个实例。然后可通过固定在壁上的链霉抗生物素蛋白从上清液中提取底物和分解产物。由于生物素和链霉抗生物素蛋白之间的结合可逆,因此,底物和分解产物可在使用已知方法进行浓缩后被进一步处理和测量。可在市面上买到内壁上涂有链霉抗生物素蛋白的合适容器,与涂覆如磁珠等微粒子一样。
例如,较好的可提取底物实施方式是通过共价键的方式将6-His标签与β-内酰胺结合。每个6-His标签由六个组氨酸组成,将分子量增加了大约800原子质量单位,提高了质子亲和力,并且提供一种从反应液体中提取纯净的底物及其分解产物的简单过程。例如,该提取可通过磁珠执行。可在市面上买到涂有螯合物的磁珠。这些螯合物可加载镍离子。镍离子以可逆方式与6-His标签结合。这便于采用已知的方法准备MALDI样品,该样品仅包含嵌入基质晶体中的纯净的剩余底物及其分解产物,因此测量的灵敏度将很高。

另一个较好的实施方式是在一个多抗药性的测定中使用若干种不同的定制底物。例如,底物可携带妨碍β-内酰胺酶攻击的不同类型的立体位阻,如同存在于不同的抗生素中一样。从分解方式和分解速度就可得出β-内酰胺酶的类型和不同类型抗生素的效果。通过使用合适的底物和选择正确的浓度,便有可能确定β-内酰胺酶的有效程度。图1是两种底物(氨苄青霉素及其钠盐)同时分解的简单示例,尽管此例中没有使用可抑制分解的带有不同抗药性的特制底物。
特别是,微生物抗药性的测量也可用于从血液或血液培养液中获得的纯净微生物,如DE 10 2009 033 368 A1(T.Maier;WO 2011/006911A3)中所述。
除了在MALDI飞行时间质谱仪内使用基质辅助激光解吸(MALDI)进行电离对底物分解进行分析,当然还可以使用其他类型的电离,如电喷雾电离(ESI),及其他类型的质谱,如正交离子注入时间飞行质谱(OTOF)、离子回旋共振质谱(ICR-MS)、Kingdon静电质谱或,或者尤其是低成本的离子阱质谱。本领域技术人员熟悉所有这些质谱仪和电离方法,因此我们在这里不再进行阐述。
用于测量底物分解和分解产物增加的一个特别合适的选择就是三重四极杆质谱仪,该质谱仪主要用于对底物和分解产物进行比较测量。这种三重四极杆质谱仪可达到非常高的灵敏度,所以很小量的底物就足以使用这种方法的测量。
为了简化抗药性的确定,可采用消毒用品包(packs of consumables)(试剂盒)的形式提供所需的材料中的一些或是全部。特别是,用品包可包含精确量的定制底物,如有需要,还包含相应的基质。它们还可含有在质谱测量后丢弃的一次性MALDI样品载板。用品包可进行商业化生产。
可对质谱图进行直观评估,还可以通过合适的计算机程序进行评估。特别是,还可研发并使用可评估多抗药性测定的程序。这些程序可根据分解方式立即确定微生物β-内酰胺酶抗药性的类型和强度,并提供建议性治疗方案。
Claims (16)
1.基于通过微生物生成β-内酰胺酶,确定微生物β-内酰胺抗药性的方法,其中通过获得剩余底物和分解产物的质谱对微生物β-内酰胺酶对底物的酶促分解进行质谱测量。
2.根据权利要求1的方法,其中底物分子包含β-内酰胺环。
3.根据权利要求2的方法,其中底物为β-内酰胺类抗生素或β-内酰胺类衍生物。
4.根据权利要求1的方法,其中底物分子量为700到1200原子质量单位。
5.根据权利要求1的方法,其中底物仅具有微弱的抗菌效果。
6.根据权利要求1的方法,其中底物分子具有可用于从溶液中提取底物的锚定基团。
7.根据权利要求6的方法,其中锚定基团为生物素基团或6-His标签。
8.根据权利要求1的方法,其中同时测量几种类型底物的分解。
9.根据权利要求8的方法,其中定制不同类型的底物,从而它们的分解方式使得能够鉴定β-内酰胺酶的不同类别。

(d)获得溶液的质谱图。
15.用于基于微生物产生β-内酰胺酶而质谱确定微生物β-内酰胺酶抗药性的用品包(试剂盒),其中用品包提供可被微生物β-内酰胺酶酶促分解的底物。
16.用于评估用根据权利要求1的方法获得的质谱图的程序。
