发明背景
慢性肾功能衰竭的特征在于肾功能的渐进丧失,并且最终可进行至终末期肾功能衰竭,此时肾不再以维持身体的水平发挥功能作用。终末期肾功能衰竭是牵涉受累者的多个器官的破坏性疾病。在美国终末期肾病的最常见病因是糖尿病。

发明概述
在本发明的实施方案中本文提供了从已进行了传代和/或体外扩增的肾的分化细胞收获的分离的肾细胞群体。在一些实施方案中,肾细胞包括肾脏的肾小管周围间质细胞和/或内皮细胞。在一些实施方案中,肾细胞由或基本上由获自肾组织的并且体外传代的肾的肾小管周围间质细胞和/或内皮细胞组成。在一些实施方案中,细胞产生促红细胞生成素(EPO)。在另外的实施方案中,选择肾细胞来进行EPO产生。

本发明的另一个方面是如本文所述的方法用于制备组合物或药剂或用于制造如本文所述的制品的用途,所述组合物或药剂用于治疗或用于进行如本文所述的治疗方法(例如,用于治疗导致减少的EPO产量的肾病或其他不适)。

优选实施方案的详述
可以以两个方向:总体地和选择性地进行用于肾功能衰竭的基于细胞的疗法。本文所描述的是用于实现特定功能器官组分的恢复的选择性细胞疗法。

受试者还可包括用于例如兽医学和/或药物开发目的动物受试者,特别地哺乳动物受试者例如犬科动物、猫科动物、牛族动物、山羊科动物(caprines)、马科动物、羊科动物(ovines)、猪、啮齿类动物(例如,大鼠和小鼠)、兔类动物、非人灵长类动物等。
细胞可以是同基因的(即,遗传上相同的或密切相关的,以使组织移植排斥降至最低)、同种异体的(即,来自相同物种的非遗传相同的成员)或异种的(即,来自不同物种的成员)。同基因细胞包括为自体的(即,来自待治疗的患者的)和等基因的(即,遗传上相同但不同受试者,例如来自相同双胞胎的)细胞。细胞可获自例如供体(即活的或尸体的)或来源于已建立的细胞株或细胞系。细胞可以例如使用本领域内已知的标准活检技术从供体收获。
“原代培养”是在将分离的细胞或原代外植体接种入培养容器后建立的首次培养。如本文所使用的,“扩增”是指活细胞的数目的增加。扩增可以通过例如将细胞“培养”通过一个或更多个细胞周期来实现,其中至少一部分细胞分裂,从而产生增加的细胞。
“体外传代的”或“传代的”是指将细胞培养物转移或再培养至第二培养容器,通常牵涉机械或酶促分离、再接种和取决于增殖速度,通常至两个或更多个子培养物的分配。如果就特定的基因型或表型选择群体,培养物在传代培养后变成“细胞株”,即,培养物是同质性的并且具有期望的特征(例如,表达EPO的能力)。
EPO的“表达”或“表现”意指编码EPO的基因被转录,和优选被翻译。通常,根据本发明,EPO编码区的表达将导致编码的多肽产生,这样细胞是“产生EPO的细胞”。在一些实施方案中,细胞产生EPO而无需另外的操作例如外源基因的导入。在一些实施方案中,本发明的条件是:不通过导入外源基因和/或利用刺激EPO的产生的外源化学试剂来处理产生EPO的细胞。
在一些实施方案中,收获的细胞不进行传代。在其他实施方案中,将细胞传代一次、二次或三次。在其他实施方案中,将细胞传代3次以上。在一些实施方案中,将细胞传代0至1次、0至2次或0至3次。在一些实施方案中,将细胞传代1至2次、1至3次或1至4次或更多次。在一些实施方案中,将细胞传代2至3次、2至4次或更多次。在另外的实施方案中,将细胞传代5、8、10、12或15或更多次。在一些实施方案中,将细胞传代0、1、2、3或4至8、10、15或20或更多次。所用的传代次数可根据例如在每一次传代后细胞群体中测量的相对EPO产量来选择。
肾细胞的生长和扩增逐渐受到明显挑战,因为此类细胞易于停止生长和早期分化。在本发明的一些实施方案中通过使用包含促进它们生长的添加剂的肾细胞特异性培养基(kidney cell specific media)来克服该挑战。因此,在一些实施方案中,将肾细胞培养在包含添加剂例如生长因子和促进它们生长的其他补充剂的培养基中。此外,在一些实施方案中,将产生EPO的细胞与其他肾细胞类型共培养。
在一些实施方案中,将肾细胞培养在补充有10%胎牛血清(FBS)或fetal calf serum(FCS)和任选地青霉素-链霉素(P/S)的达尔伯克氏改良伊格尔氏培养基(DMEM)中。在其他实施方案中,将肾细胞培养在角质细胞无血清培养基(KSFM)中。在另外的实施方案中,将肾细胞培养在含有一种或更多种下列添加剂的KSFM中:牛垂体提取物(BPE)(例如,50g/mL)、表皮生长因子(EGF)(例如,5ng/mL)、抗生素-抗真菌药溶液(GIBCO)(例如,5mL)、胎牛血清(FBS)(Gemini Bio-Product)(例如,12.5mL 2.5%)和胰岛素转铁蛋白硒(ITS)(Roche)(例如,对于5L培养基50mg)。如本领域技术人员所理解的,在上述培养基的一些实施方案中,青霉素-链霉素(P/S)与抗生素-抗真菌药溶液是可互换的。
根据本发明的肾细胞的传代可使用本领域内已知的标准方法来进行。例如,可使用胰蛋白酶/EDTA分解细胞,将其转移至其他培养皿。这是用于许多细胞类型的标准方法。简而言之,在一些实施方案中,这可使用下列步骤来进行:1)除去培养基;2)加入10ml PBS/EDTA(0.5M)进行4分钟,在相关显微镜下确认细胞连接的分离;3)除去PBS/EDTA,加入胰蛋白酶/EDTA;4)当在显微镜下80-90%的细胞浮起时加入5ml培养基;5)将细胞悬浮液抽吸入15ml试管中;6)以1000rpm离心细胞4分钟;7)除去上清液;8)将细胞重悬浮于5ml培养基中;9)用移液管吸出100μl细胞悬浮液,然后进行台盼蓝染色以进行存活率测定;10)在血细胞计数器上计数细胞的数目;11)在板上等分期望的细胞数目,使培养基的体积为总共10ml;12)将细胞置于培养箱中。
“选择”可基于将一种细胞类型与另一种相区别的任何独特性质,例如密度、大小、独特的标记、独特的代谢途径、营养需求、蛋白质表达、蛋白质分泌等。例如,可基于密度和大小使用离心梯度来选择细胞。可用荧光活化细胞分选术(fluorescent activated cell sorting)(FASC)、免疫磁珠分选术、磁性活化细胞分选术(magnetic activated cell sorting)(MASC)、淘选等来选择独特标记。可通过改变在其上(特别地在无血清环境中)培养细胞的培养基的营养成分的组成和/或量来利用独特的代谢途径和营养需求。可用不同的测定法例如ELISA检测蛋白质表达和/或分泌。
“产生EPO的细胞”是指其的至少一部分产生EPO(例如,至少20、30、40或50%或更多,或更优选60、70、80或90%或更多的细胞产生EPO)的分化细胞。在一些实施方案中,细胞产生EPO而无需另外的处理例如外源基因的导入。在一些实施方案中,本发明以不通过外源基因的导入和/或利用刺激EPO的产生的外源化学试剂来处理产生EPO的细胞为条件。细胞可获自例如肾的肾小管周围间质细胞。在一些实施方案中,就它们产生EPO的能力来选择细胞。在其他实施方案中,通过细胞培养技术例如传代来在数量上扩增细胞。可使用来自肾和来自其他来源的具有产生EPO的特定功能的细胞。例如,EPO也可在肝中正常产生。
在肾中,通常已知EPO由肾小管周围间质细胞产生(图1)。在一些实施方案中,分离的分化肾细胞群体包括,由或基本上由肾的肾小管周围间质细胞组成,其由或基本上由80、90、95或99%或更多肾小管周围间质细胞和按数量计算不超过20、10、5或1%或更少的其他细胞类型组成。在其他实施方案中,分离的分化肾细胞群体包括其他细胞类型例如内皮管周细胞(endothelial peritubular cell)。
在一些实施方案中,分化肾细胞的分离群体包括,由或基本上由就EPO的产生而选择的肾细胞组成,其由或基本上由80、90、95或99%的或更多的产生EPO的细胞和按数目计算不超过20、10、5或1%的不表达EPO的细胞组成。选择可通过使用特异性标记选择表达EPO的细胞来实现。在一些实施方案中,细胞可包括不同的肾细胞类型,只要所述细胞表达EPO。在另外的实施方案中,可将整个肾细胞集落用于扩增和治疗。
在一些实施方案中,分离的分化肾细胞群体具有“长寿命”,这样当体外培养时它们就能够生长通过至少5、10、15、20、25或30或更多次的群体倍增。在一些实施方案中,当在体外培养时,细胞能够通过40、50或60或更多次群体倍增来增殖。
“分化的”是指具有特化功能例如EPO产生和/或已知的分化细胞的标记(例如GLEPP1和/或Tamm Horsfall肾细胞标记)的表达的细胞或包含所述细胞的群体。在该意义上,它们不是祖细胞或干细胞。本发明的一些实施方案以不在产生特化程度不太高的细胞群体的条件下对收获的分化细胞进行传代为条件。
备选地,在其他实施方案中,培养细胞以产生细胞系,该细胞系之后可分化产生更特化的细胞。与细胞株相反,“细胞系”的建立总地来说是未分化的,虽然它们可被定型至特定谱系。繁殖天然地有利于增殖表型,在一些实施方案中细胞可能需要通过例如变更培养条件来重新导入分化。存在许多本领域中已知的可在细胞系中诱导分化的分化因子(例如,细胞因子例如表皮形态发生素和HGF、维生素等)。
治疗方法
在一些实施方案中,可将产生EPO的细胞施用(例如,通过注射)至有此需要的受试者的肾(例如,施用入皮质和/或髓质)。在其他实施方案中,将产生EPO的细胞施用至身体的其他部位,例如肝、腹膜等。在一些实施方案中,经皮下、被膜下(subcapsular)等施用产生EPO的细胞。在另外的实施方案中,可通过植入本文所述的含有所述产生EPO的细胞的基质(例如,胶原凝胶支架)来施用产生EPO的细胞。在其他实施方案中,通过血管通路(vascular access)(例如,全身性或局部地)施用产生EPO的细胞。
可用本文所公开的方法治疗的疾病包括但不限于贫血。贫血包括但不限于与肾功能衰竭或终末期肾病相关的贫血、由化疗或放射治疗所致的贫血、慢性障碍例如慢性感染、自身免疫性疾病、类风湿性关节炎、AIDS所致贫血、恶性肿瘤所致贫血,早产儿贫血、甲状腺功能减退所致贫血、营养不良所致贫血(例如,缺铁症)和与血液病相关的贫血。
“治疗”是指为患者例如遭受或处于发生疾病(例如,肾病、贫血等)风险中的患者带来益处的任何类型的治疗。治疗包括为了改善患者的状况(例如,一个或更多个症状的缓解)、延迟疾病的发生或进展等目的而采取的行为和克制不要采取的行为。
其他内分泌系统可受益于本文公开的治疗,例如,产生维生素D的细胞疗法或血管紧张肽系统。参见,例如,授予Atala等人的美国专利申请公开号2005/0002915,其以引用的形式合并入本文。可使用来自肾和其他来源的具有特定功能的细胞,即可产生靶功能的细胞。例如,EPO也在肝中正常产生。
优选在施用前将细胞与药学上可接受的载体混合或将细胞接种至其上。“药学上可接受的”意指化合物或组合物适合于根据疾病的严重度和治疗的必要性施用至受试者以获得本文公开的治疗而无过度有害副作用。可使用本领域内熟知的技术制备这样的制剂。参见,例如,美国专利申请2003/0180289;Remington:The Science and Practice ofPharmacy,Alfonso R.Gennaro,editor,第20版Lippincott Williams &Wilkins:Philadelphia,PA,2000。载体可以是固体或液体,或两者(例如,水凝胶),并且可以单位剂量制剂的形式与细胞一起配制。在一些实施方案中,以悬浮液的形式在载体中提供细胞来减少细胞的凝聚。在其他实施方案中,将细胞接种至生物可降解的支架或基质上。
在一些实施方案中,将细胞与适当的凝胶混合以便施用。可用于本发明的适当的凝胶包括但不限于琼脂、胶原、血纤蛋白、水凝胶等。除了凝胶以外,还可在本发明中使用其他支持性化合物。例如可将细胞外基质与支持性凝胶组合以最优化或功能化凝胶。还可将一种或更多种生长因子导入细胞悬浮液。
本发明的制剂包括通过注射或植入进行的胃肠外施用(例如,皮下、肌内、皮内、静脉内、动脉内、腹膜内注射)的制剂。在一个实施方案中,通过简单注射或通过置于适当的血管例如肾动脉中的导管进行的注射来进行施用。在一些实施方案中,通过“输注”进行施用,从而组合物经由静脉(例如,门静脉)被导入身体。在另一个实施方案中,以移植物的形式进行至如上所论述的待增强的器官或组织,例如肾和/或肝的施用。
“可生物降解的支架或基质”是对生物功能不具有毒性或有害作用并且能够被宿主降解成基本组成的任何物质。优选,支架或基质是多孔的,从而允许细胞沉积在基质的孔上和孔内。这样的制剂可通过将至少一个细胞群体提供至生物可降解支架以将该细胞群体接种在和/或接种入支架来制备。然后将接种的支架植入受体受试者的体内。
在一些实施方案中,通过将细胞(例如,存在于适当的载体中)直接注射入受试者的组织内来施用细胞。例如,可将细胞注射入肾(例如,肾的被膜下空间)。因为EPO产生的功能效应将是全身性的,因此还可通过注射入其他组织(例如,肝,皮下注射,等)来施用细胞。
还可被全身性递送细胞。在另外的实施方案中,将细胞递送至肾外组织(例如,肝),因为EPO产生的功能效应的结果将是全身性的。参见,例如,the″Edmonton protocol,″an established delivery method,where cellsare infused into a patient′s portal vein(Shapiro等人(2000)N Engl J Med343:230-238)。

在一些实施方案中,以治疗有效量施用细胞。细胞的治疗有效剂量在受试者个体间将会有少许变化,并且将取决于因素例如受试者的年龄、体重和状况以及递送途径。可根据本领域技术人员已知的方法来测定这样的剂量。通常,在一些实施方案中,可以以单次一起施用或以数次分开的施用来提供1x105、1x106或5x106至1x107、1x108或1x109个细胞或更多个细胞/受试者的剂量。在其他实施方案中,可以以单次一起施用或以数次分开的施用来提供1至100x108个细胞/千克受试者体重的剂量。当然,必要时可提供后续施用(follow-up administration)。
可按照一些实施方案施用细胞来获得靶血细胞比容范围。理想的或靶血细胞比容范围在受试者个体之间会有变化,其取决于例如特定共病(comorbidities)。在一些实施方案中,靶血细胞比容是30至40%,在一些实施方案中,靶血细胞比容是33至38%,以及在一些实施方案中,靶血细胞比容是33至36%。当施用根据本发明的细胞时,可测量血细胞比容以及,如果想要或必要时,通过例如细胞的进一步植入和/或本领域内已知的其他方法(例如,补充以重组EPO)来校正血细胞比容。可将用于贫血和/或肾病治疗的其他方法与本文提供的治疗方法例如定制的蛋白质-热量摄入饮食(adapted protein-caloric intake diet)结合使用。
在另外的实施方案中,如果想要或必要时,可按照已知的技术对受试者施用抑制施用的细胞的移植排斥的药剂例如雷帕霉素、氮杂硫代嘌呤(azathioprine)、皮质类固醇、环孢霉素和/或FK506。参见,例如,R.Calne,美国专利5,461,058、5,403,833和5,100,899;也参见美国专利6,455,518、6,346,243和5,321,043。一些实施方案使用植入和免疫抑制的组合,这可使移植物排斥降至最低水平。可按需要重复植入以产生足够量的移植组织。
本发明将在下面非限定性实施例中进行更详细地解释。
实施例
贫血是因肾减小的通过肾小管周围间质细胞产生促红细胞生成素(EPO)的能力而引起的慢性肾功能衰竭的不可避免的结果。我们通过检查选择和扩增产生促红细胞生成素的细胞以用于基于细胞的治疗的可行性来研究产生促红细胞生成素的细胞的补充是否是肾功能衰竭诱导的贫血的可能治疗选择。
下列实施例证明产生EPO产生的细胞存在于获自小鼠和大鼠肾的肾细胞中。此外,使用下面描述的方法分离和扩增的细胞包括在检查的每一个培养阶段表达EPO的细胞。此外,培养物中表达EPO标记的细胞的实际百分比与正常肾组织中存在的该细胞群体一致(参见Yamaguchi-Yamada等人,J Vet Med Sci,67:891,2005;Sasaki等人,BiosciBiotechnol Biochem,64:1775,2000;Krantz,Blood,77:419,1991)。
实施例1.肾细胞原代培养物的扩增。培养扩增7至10天龄小鼠C57BL/6的肾细胞。将切碎的肾(小鼠的1个肾)置于装有15ml胶原酶/分散酶(0.2mg/ml)的50cc管中。将肾组织碎片与胶原酶/分散酶混合物(0.2mg/ml;15ml)一起在37℃振荡器中温育30分钟。向降解溶液中加入含有明胶的无菌PBS(20ml)(和明胶(DIFCO)2mg/ml)。将混合物通过70微米滤器过滤以除去未降解的组织碎片。充分混合收集的溶液(小心不要产生气泡),将其分配至2个50cc管中。以1000(-1500)RPM离心管5分钟。弃去上清液,将每个管的沉淀重悬浮于3ml KSFM培养基。将DMEM培养基(10%FBS,5ml P/S)用于基质细胞,和将具有BPE、EGF、5ml抗生素-抗真菌药、12.5ml FBS(Gemini Bio-Product,2.5%)、含有BPE和EGF的胰岛素转铁蛋白硒(Roche)(50mg,用于5L培养基)的KSFM用于上皮组分。还可加入P/S或抗生素-抗真菌药(GIBCO)。将各组织接种在25mm培养皿上,加入培养基(总共3ml)。
通过第二天更换培养基,然后取决于细胞密度每2天更换一次培养基来维持细胞。当细胞达到80-90%汇合时,通过使用胰蛋白酶/EDTA使其脱离,然后利用下列步骤将其转移至其他培养皿来传代细胞:1)除去培养基;2)加入10ml PBS/EDTA(0.5M)进行4分钟,在相关显微镜下确认细胞连接的分离;3)除去PBS/EDTA,然后加入7ml胰蛋白酶/EDTA;4)当在显微镜下80-90%的细胞浮起时,加入5ml培养基;5)将细胞悬浮液吸入15ml试管;6)以1000rpm离心细胞4分钟;7)除去上清液;8)将细胞重悬浮于5ml培养基中;9)用移液器吸取100μl细胞悬浮液,进行台盼蓝染色以进行存活率测定;10)在血细胞计数器上计数细胞的数目;11)在板上等分期望的细胞数目,使培养基的体积为总共10ml;12)将细胞置于培养箱中。
备选地,使用下列方案。在含有10%的抗生素/抗真菌药(GibcoInvitrogen,Carlsbad,CA USA)的Krebs缓冲液(Sigma Aldrich,St.Louis,MO USA)中收集10天龄雄性C57BL/6小鼠的肾以避免感染的风险。立即将肾运送至培养通风柜(hood),在培养通风柜中除去被膜。除去肾的髓状区,只将皮质组织用于分离之前已被鉴定为产生EPO的细胞的细胞(Maxwell等人,Kidney International,44:1149,1993)。切碎肾组织,使用Liberase Blendzyme(Roche,Mannheim,Germany)将其在37摄氏度下于摇动的水浴中酶促降解25分钟。移去上清液,将细胞沉淀通过100μm细胞过滤网以获得用于培养的单细胞悬浮液。
然后,将细胞以5x105个细胞/ml的密度涂板在10cm充满培养基的组织培养用培养皿中。培养基由比例为1∶1的角质细胞无血清培养基(KSFM)和预混合的达尔伯克氏改良伊格尔氏培(DMEM)的混合物组成。预混合的DMEM培养基包含3/4DMEM和1/4HAM’s F12营养混合物,其补充有10%胎牛血清(FBS)、1%青霉素/链霉素、1%谷氨酰胺100x(Gibco)、1ml的0.4μg/ml氢化可的松、5ml的10-10M霍乱毒素溶液、0.5ml的5mg/ml胰岛素溶液、12.5ml/500ml的1.2mg/ml腺嘌呤溶液、0.5ml的2.5mg/ml转铁蛋白+0.136mg/ml三碘甲腺原氨酸混合物和0.5ml的10μg/ml表皮生长因子(EGF)溶液。除非另外指出,否则所有组织培养试剂均购自Sigma-Aldrich(St.Louis,MO USA)。将细胞在37℃于5%CO2下温育,每3天更换培养基一次,当汇合时以1∶3的比率传代培养细胞以进行扩增。
实施例2.EPO产生的表征。利用特异性抗EPO抗体(兔多克隆抗EPO抗体,sc-7956,Santa Cruz Technologies,Santa Cruz,California)使用免疫细胞化学和western印迹分析就EPO表达来表征来自早期代(1、2和3)的细胞。
将肾细胞以每孔3000个细胞的密度涂板在8孔培养腔室玻片(chamber slide)中。将细胞在37℃于5%CO2下温育24小时以允许附着。然后在室温下用4%多聚甲醛固定10分钟。通过加入0.1%的PBS中的Triton-X 100在室温下进行3分钟来透化细胞膜。然后将细胞在室温下于山羊血清中温育30分钟。清洗后,将细胞在室温下与一抗一起温育1小时(1∶50)。第二次清洗细胞,加入生物素化山羊多克隆抗兔抗体(多克隆抗兔IgG,Vector Laboratories,Inc.,Burlingame,California)(1∶200),然后在室温下温育45分钟。按照厂商说明书使用Vector ABC试剂盒(VectorLaboratories,Inc.,Burlingame,California)进行终清洗步骤后的EPO的显色检测。无一抗的玻片用作内部阴性对照,正常小鼠肾组织用作阳性对照。
培养中的肾细胞在显微镜下显示多个表型。细胞在涂板的7至10天内达到汇合。与阴性对照相(其显示无背景或非特异性染色)相比较,在从肾分离后的前3代中观察到许多细胞对于EPO呈阳性染色(图2),这表明观察到的染色可能归因于EPO在培养物中的存在。对于EPO呈阳性染色的细胞的数目在研究的3代中保持恒定,甚至当在相同时期中在培养物中观察到表型变化时亦如此。肾组织的免疫组织化学染色显示与在培养细胞中观察到的相似的EPO表达量(图3)。
表达EPO的细胞的数目随着后续传代略微减少(图4)。这极可能归因于增加的传代次数和细胞/功能随时间流逝而丢失。然而,相对百分比在第1代后似乎保持稳定。
EPO表达也通过western印迹法来确认,示于图5中。
实施例3.小鼠和大鼠肾细胞的表征。将FACS分析用于在各代(1-3代)定量已建立的肾细胞培养物中产生EPO的细胞的数目。通过胰蛋白酶消化和离心收集细胞,将其重悬浮于培养基中,然后通过70μm细胞过滤网以确保单细胞悬浮物。在计数细胞后,将它们离心,然后以5-7.5x105个细胞/管重悬浮于PBS中以从细胞除去FBS。用2%多聚甲醛在4℃下固定细胞10分钟,在室温下用100%甲醇透化10分钟。然后,将细胞重悬浮于3%的PBS中的山羊血清中,之后在冰上与兔抗EPO一抗sc-7956(SantaCruz Biotechnology,Santa Cruz,California)一起温育45分钟。用3%的PBS中的山羊血清清洗细胞2次,然后将其与异硫氰酸荧光素(FITC)-缀合的山羊抗兔第二抗体一起温育1小时。然后用3%的PBS中的血清充分清洗细胞,将其转移至FACS机器(FACS Calibur E6204,Becton-Dickinson,Franklin Lakes,New Jersey)。
荧光活化细胞分选实验显示44%的第1代(P1)细胞是EPO阳性的。该百分比在第2代(P2)增加至82%,然后在第3代(P3)降回至42%。这可以表明,在培养的前几天中,产生EPO的细胞的增殖和/或EPO基因表达的上调响应培养基中的低氧浓度(与正常活组织相比较)而发生。然后此类反应在随后几天中可恢复正常,从而导致与在肾组织中发现的产生EPO的细胞的数目接近的产生EPO的细胞的数目(图6,顶行)。
FACS数据表明EPO表达在数代内得到维持。应当指出,在第2代培养中存在表达EPO的细胞数目(82%)的波动,这通过几次重复实验得以确认。虽然不希望受任何特定理论束缚,该现象的一个可能的解释可能是EPO表达是所有肾细胞的可在需要时打开和关闭的固有性状。在该情况下,在身体和培养皿之间的存活条件发生急剧改变后,细胞可能已被驱动至随时表达EPO直至培养的稳定化发生。与此相符,EPO的波动很快被逆转,第3代分析显示更低的产生EPO的细胞百分比(42%)。
通过免疫荧光进行的小鼠细胞表征确认了EPO表达(图7A)。细胞群体对于肾细胞标记GLEPP1和Tamm Horsfall是阳性的(图7B)。
也使用荧光活化细胞分选术(FACS)(图6,底行)就EPO的产生分析大鼠细胞第0、1和2代。培养的大鼠细胞具有不同的细胞形态学并且对于GLEPP1和Tamm Horsfall肾细胞标记呈阳性(图8)。
实施例4.产生EPO的培养物对低氧条件的暴露。虽然表型特征的维持在细胞扩增阶段是必需的,但确保细胞治疗的成功的关键组分是产生EPO的细胞调节和维持正常EPO水平的能力。EPO属于造血细胞因子家族,其控制骨髓中红细胞生成,并且通过EPO受体(EPOR)介导的信号转导调控红系祖细胞(erythroid progenitor cell)的增殖、分化和存活。EPO主要在肾中产生,当该器官功能衰竭时,EPO产量下降,从而导致贫血。身体中EPO的表达极大地依赖于能够产生EPO的细胞的周围环境中的氧张力。影响氧水平的因素包括环境空气中氧的缺乏和减少的肾血流量。
为了确定培养中的EPO表达性细胞是否可对变化的氧水平作出反应,进行了其中将细胞血清饥饿24小时,然后将其在体外暴露于不同的氧水平的实验。将Lewis大鼠肾细胞和HepG2(人肝细胞肝癌细胞系)细胞在正常和低氧条件下培养,利用细胞的western印迹法估量EPO产量和确认其。还通过使用 Erythropoietin ELISA试剂盒(R&DMinneapolis,Minnesota),利用双抗体夹心酶联免疫吸附测定分析来自在含氧量正常和低氧条件下培养的肾细胞的上清液来测量和确认EPO在培养基中的存在。
在实验前,将细胞置于无血清培养基中进行24小时。然后将培养皿转移至低氧箱并且暴露于不同低氧条件(1%、3%、5%和7%的氧)。HepG2细胞用作正对照,因为之前已报导它们在培养中产生高水平的EPO(Horiguchi等人,Blood,96:3743)。通过EPO western印迹法确认HepG2的EPO表达(图9)。在含有NP-40的裂解缓冲液中收获所有细胞。使用Bio-Rad蛋白质测定法测量各样品中蛋白质浓度。通过使用SDS-PAGE,将40μg总蛋白在10%丙烯酰胺凝胶上进行电泳。然后将蛋白质转移至PVDF膜(Millipore Corp.)上。裂解物中β-肌动蛋白表达的检测用作上样对照。以1∶200使用EPO抗体(兔多克隆sc-7956,Santa CruzBiotechnology)以及以1∶2000使用二抗(山羊抗-兔7074,Cell SignalingTechnology,Beverly,Massachusetts)。为了测量原代肾培养物分泌入培养基的EPO的量,收集培养基,使用Amicon超速离心过滤装置(MilliporeCorporation,Billerica,Massachusetts)将其浓缩减少至500ul。将该培养基的样品在10%聚丙烯酰胺凝胶上进行电泳。以1∶100使用EPO抗体(兔多克隆sc-7956,Santa Cruz Biotechnology)以及以1∶2000使用二抗(山羊抗-兔7074,Cell Signaling Technology,Beverly,MA,USA)。
Western印迹显示在低氧后细胞裂解物中EPO表达的少量增加(图10)。然而当将培养基浓缩物用于测量EPO时未看到这些结果O(图11)。培养基检测表明所有培养基浓缩物(低氧和含氧量正常的条件)包含相同的低EPO量。
备选地,在第1和2代从大鼠肾原代细胞制备总蛋白质裂解物。处理来自含氧量正常的样品(NC)、3%O2和7%O2中的样品的培养皿,然后在10%SDS-PAGE上进行电泳。KNRK细胞系用作正对照。结果示于图12中。
不希望受任何特定理论束缚,这可表明24小时可能是不足以使分泌的EPO水平升高至可被western印迹法检测到的水平的时间。细胞开始分泌EPO可能需要更长的暴露时间,因为从头的蛋白质产生可能要花费数小时才变得明显。因此进行下列实验,其中将细胞置于低氧条件进行24、48和72小时。
在各代在10cm培养皿中培养来自Lewis大鼠的原代培养细胞至接近汇合。将细胞置于低氧箱(1%O2)进行24、48或72小时。在低氧温育后,收集培养基,并且用10K分子量筛截Amicon超速离心装置(Millipore)进行浓缩。然后将40μg总蛋白上样于10%聚丙烯酰胺凝胶上。KNRK细胞用作正对照。结果示于图13。
概括地,所有实验表明原代培养细胞中的EPO水平高于或等于在HepG2阳性对照中测量的水平,并且产生EPO的细胞能够响应变化的环境。
实施例5.产生EPO的细胞的体内施用。为了确定产生EPO的细胞在体内是否存活和形成组织,将与胶原凝胶混合的肾细胞以1x106和5x106的浓度皮下植入于无胸腺小鼠,然后在植入后第14和28天取回以进行分析。使用处于1至5代的不同代的细胞。将细胞悬浮于胶原凝胶以便容易注射(浓度:0.1mg/ml)。
在组织学上,取回的植入物显示存活的肾细胞继续表达EPO蛋白,使用EPO特异性抗体在组织学上对其进行了确认(图14)。
这些结果证明在培养中生长和扩增的产生EPO的肾细胞稳定地体内表达EPO。因此,产生EPO的细胞可用作由慢性肾功能衰竭所致贫血的治疗选择。
实施例6.使用实时PCR进行的EPO表达的分析。进行实时PCR来估量响应低氧条件的EPO的大鼠细胞表达。
为了检测培养基的作用,将培养在KSFM和DMEM中的细胞暴露于低氧条件(3%O2)。将肾原代细胞(第0代)于10cm培养皿中培养至80%汇合。用无血清KSFM或DMEM培养3个培养皿的细胞,将其置于3%O2的低氧箱。在24小时后,处理样品以进行总RNA和cDNA合成。以一式三份重复进行实时PCR,相对于含氧量正常的样品定量样品。结果示于图15中。
利用实时PCR横跨24、48和72小时比较大鼠肾培养物的EPO表达。将肾原代细胞(第0和2代)在10cm培养皿中培养至80%汇合。然后将细胞培养在无血清KSFM中,并且置于1%O2的低氧箱中。在24、48或72小时后,处理样品以进行总RAN和cDNA合成。以一式三份重复进行实时PCR,相对于含氧量正常的样品定量样品。结果示于图16。
检测时间点直至24小时,将肾原代细胞(第0代)在10cm板中培养至80%汇合。然后将细胞置于1%O2的低氧箱中进行长达24小时。然后处理样品以进行总RNA和cDNA合成。以一式三份重复进行实时PCR,相对于含氧量正常的样品定量样品。结果示于图17中。
实施例7.人肾细胞的扩增。也使用上述培养基和条件证明原代人肾细胞的生长和可扩增性。来自第2、4、7和9代的培养物示于图18中。证明了人原代肾细胞可保持通过20次倍增(图19)。就EPO和GLEPP1表达表征人肾细胞培养物(图20)。
实施例8.通过胶原注射的人肾细胞递送。如上面实施例5中所述将在胶原凝胶中混合的人肾细胞皮下植入无胸腺小鼠。比较1mg/ml、2mg/ml和20mg/ml的胶原浓度。在1和2mg/ml上,在施用后体内体积消失。在20mg/ml上,体内注射体积得到保持,并且看到新血管形成图21。组织学确认在体内形成了表达EPO的组织(图22)。
实施例9.使用磁性细胞分选术进行的产生EPO的细胞的选择。使用磁性细胞分选术选择表达EPO的细胞。使用标准制备方法分离单细胞悬浮物。在制备单细胞悬浮物后,计数细胞的总数,离心细胞样品以获得沉淀。用10%的山羊血清(在其中制备二抗的动物的)封闭细胞,进行10分钟。加入1或2mL封闭溶液。在10分钟的离心后,将细胞重悬浮于EPO的一抗中(使用1μg的一抗/百万细胞)。通常,在4-8℃下标记15分钟就足够了。通过每107个细胞1-2mL缓冲液和在300×g下离心10分钟来清洗细胞2次以除去任何多余的一抗。在两次连续清洗后,以80μL的PBS(0.5%的BSA和2mM EDTA,pH 7.2)/107个细胞重悬浮细胞。每107个细胞加入20μL山羊抗兔MicroBead。充分混合,在4-8℃下温育15分钟。通过每107个细胞加入1-2mL缓冲液和在300×g下离心10分钟来清洗细胞两次。用移液器完全吸出上清液。于500μL缓冲液中重悬浮至108个细胞(注意:对于更高的细胞数目,相应地按比例扩增缓冲液体积;为了使用LD柱耗尽,将细胞沉淀重悬浮于500μL缓冲液至1.25×108个细胞).进行磁性细胞分离。
注意:操作要快,使细胞保持冷却,以及使用预冷溶液。这可防止抗体在细胞表面上的罩盖和非特异性细胞标记。下面给出的用于磁性标记的体积达到107个总细胞。当用少于107个细胞进行时,使用如所指定的相同体积。当用更高的细胞数目进行时,相应地按比例扩大所有试剂体积和总体积(例如,对于2×107总个细胞,使用2倍体积的所有指定的试剂体积和总体积)。在冰上的操作可能需要增加的温育时间。更高的温度和/或更长的温育时间导致非特异性细胞标记。
