对于RCC据估算,1995年在欧盟有41,325件新病例,其中21,728件由该疾病导致死亡(EUCAN资料库,1995)。按照美国健康和公共事业部门(the United States Department of Health and HumanServices)统计每年确诊约30,000件新病例,其中约12,000件为RCC相关的死亡(National Institute of Health,SEER CancerStatistics Review,1999)。
约50-60%的患者最初呈现在疾病的I期或II期,即为局部化的RCC。在原发肿瘤外科手术切除以后,这些患者预后良好,分别为60-80%(I期)和40-60%(II期)的5年生存率。其余的患者预后较差。尽管大多数III期疾病即在诊断时为非转移性疾病的患者(20-25%的总患者中)也将进行手术。他们中5年生存率仅为20-40%。这类患者,尽管不存在临床上可检测的肿瘤,但明显地处于肿瘤再发生的高危险之中。IV期疾病患者(转移性)(10-20%的总患者)5年生存率为0-20%(6)。各期可根据TNM分级定义,如下面的表1中给出:

一种在814名患者中进行结果评价的前瞻性群组研究能在疾病特异性和总体生存率两方面的定义五种不同的类别(7)。将这些类别转化为根据1997TNM分类、Fuhrman的分级和ECOG表现状态定义的风险组。在486名非转移性患者中,128(27%)名为低危险患者,190(41%)名为中度危险患者,150(32%)名为高危险患者。这些组之间5年总体生存率差别显著,分别为84%、72%和44%。在高风险组中,42.5%患者肾切除之后头两年内复发。Fuhrman的核分级可按下面的表2所示进行定义。

由于患者中RCC早期诊断增加以及手术后疾病复发的发病率高因而需要考虑辅助治疗。
癌可能呈现局部化的(仅在一个位点生长)而它实际上可能已经开始蔓延。癌细胞可能已经冒险出去进入躯体内,但只是小数量以致于还不能检测到它们。在初步治疗之后患者可能无症状。辅助治疗描述为以任何剩余的不能发现的癌细胞作为靶点的方法。辅助治疗在初步治疗如外科手术或放射之后运用,以预防癌症再发生。存在四种主要类型的辅助治疗,可基于癌症的类型及其进展进行选择:
-化疗
-激素治疗
-放射治疗
-免疫治疗。
辅助治疗的原理得到公认并在几种肿瘤如乳房癌和结肠直肠癌中沿用已久。对于后来复发的肾切除患者,复发时间的中位值为15至18个月,其中85%的复发在3年之内发生(8)。关于RCC的辅助治疗迄今为止还没有药物获得批准。
Pizzocaro等报道了以247名患者进行的RCC大量辅助治疗研究(9)。一半的患者接受干扰素-α(IFN-α),在外科手术的一个月之内开始,一周三次,肌内注射(i.m.)6个月。另一半患者只接受观察。5年总体的和无事件生存的概率显示出没有统计上显著的差异。在97名淋巴结阴性的(negative)患者中,在治疗组中观察到统计上显著的有害影响。当对3年的生存累积概率进行评价时,在13名接受pN2/pN3治疗的患者的小的分组(关于分类参见下文)中可观察到保护效应。由于该患者组数量少,因而这些数据没有统计上的显著意义。55%的患者表现出由IFN-α引起毒性的迹象,28%需要减小剂量和/或中止治疗。该结果表明在IFN治疗中死亡率更高且复发率更高,其中13%的患者经历4级中毒。
白细胞介素-2(IL-2)在辅助固定(setting)中的作用还没有定义。在美国的细胞因子工作组正进行的一项研究现在正在评价高剂量的IL-2与只是做观察相比较(10)。由于缺乏与显著的毒性相结合的正性研究结果,肾切除后护理的现行标准观察结果有限。
包括在RCC中采用自体疫苗接种方法的辅助治疗的III期临床研究现在正在进行(National Cancer Institute,
www.cancer.gov;Antigenics,press release Dec.22,2003)。
通常认为评价RCC患者外科手术后预后的主要参数是依据TNM分类中所述的病理阶段。该分类已经在2002年修正如下(11):

在一家医院总共675名全肾切除的患者中进行所追溯评价,48%的患者有pT1,20%有pT2,10%有pT3a,20%有pT3b,2%有pT4(12)。
TNM分类只考虑宏观上可测定的血管和周围组织的侵入。运用多变量统计分析发现间变(anaplasia)和微观的血管侵入(MVI)的等级也能提供预后信息。后两个变量预后互相联系,因为MVI在高等级的间变之中特别频繁而在低等级肿瘤中较少,分别是核分等级的GII-GIII有56%的血管侵入而在GI肿瘤中为24%。有血管侵入的患者比没有血管侵入的患者肿瘤转移的频率高(47%对21%)(13)。
嵌合的单克隆抗体cG250是原型鼠单克隆抗体mG250的IgG1κ轻链嵌合体的模型,最初由Oosterwijk等描述(22)。在竞争性结合测定已经证明嵌合体G250(cG250)相当于鼠的mG250并对人癌细胞系表现出与mG250相似的结合反应活性。G250能检测肾癌细胞上的细胞表面抗原(MN抗原)。在新鲜冷冻组织的免疫组织化学测定中,G250与95%的透明细胞型的肾癌反应而与结肠直肠癌及其它癌反应的比例低得多。与肾癌的反应活性75%的肾癌中是均匀的(大于75%的反应细胞)。cG250与正常人组织的反应活性只限于胃上皮细胞和肝内胆管(23,24)。
该嵌合抗体可用碘-131进行放射性标记,其免疫反应性的损失极小。在对16名有转移性RCC患者的研究中,在肾切除前一周注入131I-标记的抗体(25)。注入后放射性标记的抗体逐渐局部化进入肿瘤内,其余的部分从体内排泄。局部化进入肿瘤内的标记抗体的百分率是曾经报道的用抗肿瘤抗体进行的临床试验中最高的之一。
在以未作标记的cG250抗体制剂的I期多次剂量的研究中,12名转移性RCC患者以剂量逐步升高的设值(setting)每周接受一定的剂量,持续6周。结果表明该抗体在5、10、25和50mg/m2的所有剂量水平上是安全的。观察到一项目的响应(one objective response),12名患者中有8名在第一个6周循环的治疗之后呈现疾病稳定(26)。
此外,在II期研究中未作标记的cG250抗体作为单一治疗给药,32名转移性RCC的可评价患者用50mg cG250治疗达到20周,每周一次,一次50mg cG250。该研究证实了用该抗体长期治疗的优良安全特性。没有严重的药物相关不良反应报道和没有过敏性反应发生。在两名患者中发现极低水平的人抗嵌合抗体(HACA)(27)。32名患者中,6名在进入研究项目时为进行性的患者,取得疾病的稳定至少6个月。此外,治疗结束4个月之后在随访期间发现两名肿瘤消退,一名完全性反应,一名轻微反应。在此期间这些患者当中没人接受任何肿瘤治疗。测得中值总体生存率为15.6个月,35%的患者在长达166周的随访时间之后仍活着。所有的研究证实了cG250的优良耐受性。
cG250的作用机理为抗体依赖性细胞毒性(ADCC),不过也可能是其它作用机理。体外研究表明0.5μg/ml的cG250对于诱导ADCC是足够的(28)。这表明至少0.5μg/ml cG250的临床给药方案递送水平应有效,条件是药物能达到靶点。此外,运用放射性标记的cG250的示踪剂剂量进行剂量逐步上升研究的结果表明每个患者10mg以上的单一剂量对于使所有抗原阳性的肿瘤细胞浸透一周的时间应是最佳的(24)。这些数据表明大于0.5μg/ml的血浆浓度将不会提供其它的临床益处。
正如以上的讨论,对于治疗转移性RCC的治疗最近几年中新的疗法已经获得批准,但肾细胞癌的生存率几十年中还没有显著的变化。因此,本发明旨在解决的问题是确定新的治疗方案,尤其是对于肾切除后有复发高危险又没有宏观上可检测的疾病证据的RCC患者广泛和容易应用的且无毒的治疗。
该问题的解决方案是本发明的对患者的辅助治疗方法,其中所述患者原发肿瘤的特征为G250表达。
在本发明的一实施方案中,治疗表达G250抗原的癌症的方法包括将G250抗原特异性抗体或/和其抗体片段作为辅助治疗给药于有原发肿瘤的患者,其中患者进行过原发肿瘤的切除和必要时进行过淋巴结切除和/或是由于进行原发肿瘤切除和必要时进行淋巴结切除的原因。
优选,患者已经被确诊为有非转移性疾病或/和已经被确诊有复发的高危险。非转移性疾病患者按照本发明的分类法(参见下文)可分为风险组I、II或III。
优选原发肿瘤为表达G250抗原的肿瘤,尤其是选自肾透明细胞癌、宫颈癌、胆癌、食管癌、结肠直肠癌和肺癌的原发肿瘤。
例如抗G250抗体描述于EP-B-0 637 336,在此引入作为参考。
在另一优选的实施方案中抗体或/和其抗体片段选自多克隆抗体、单克隆抗体、其抗原结合片段如F(ab′)2、Fab′、sFv、dsFv及其嵌合的、人源化的和完全人的变体。尤其优选,抗癌的抗体为嵌合的或人源化的G250抗体和/或其片段。这些抗体可按照PCT/EP02/01282和PCT/EP02/01283中描述的方法生产,在此引入作为参考。最优选的抗体为cG250。另一最优选的抗体为由杂交瘤细胞系DSM ACC 2526产生的单克隆抗体G250,其中已经保藏在DSMZ,Mascheroder Weg 1b,D-38124 Braunschweig。
更优选的实施方案为治疗肾透明细胞癌的方法,包括将G250抗原特异性抗体或/和其抗体片段作为辅助治疗给药于有原发肿瘤的患者,其中患者进行过原发肿瘤的切除和必要时进行过淋巴结切除和/或是由于进行原发肿瘤切除和必要时进行淋巴结切除的原因。
最优选本发明治疗肾透明细胞癌的方法包括给予抗体cG250。
例如,被分类为下列风险组之一的肿瘤患者有复发的高危险。这些风险组参考TNM分类法,第6版UICC(2002):
·风险组I:原发肿瘤已经在组织学上证实为阶段T3bN0M0
或 T3cN0M0或
T4N0M0
·风险组II:任何在组织学上证实的T阶段
和N1或N2疾病
·风险组III:原发肿瘤T1bN0M0或T2N0M0

和级别为III(Fuhrman或任何其它的含至少3个等级的核分级系统)
转移为癌细胞从一个器官或组织到另一个器官或组织的移动或蔓延。癌细胞通常经血流或淋巴系统蔓延。就原发癌的局部关系而论,将转移区分为局部转移(局部闭合或靠近原发癌)、区域转移(在区域淋巴系统的范围内)和远距离转移。
确诊为N0和M0的患者可分为风险组I或III类。在癌症的发展过程中,淋巴道转移可存在于单一的区域淋巴结中(N1,按照TNM分类法)或/和存在于一个以上的区域淋巴结中(N2),将其分类为高风险组II,其中有远距离转移的
患者将被分类为转移性疾病。因此,在优选的实施方案中,接受本发明方法治疗的患者为有复发高危险、被分类为风险组I、II或III的患者。
非转移性疾病可能在区域淋巴结中没有组织学上证实的转移(N0,按照TNM)和没有远距离的转移(M0)。然而,患者存在有由肿瘤细胞带来的相当大的复发危险,例如在微小转移中,其中肿瘤细胞在原发肿瘤切除之后还有残存,其不能通过组织学方法检测,导致患者没有任何残留的肿瘤疾病证据的诊断结论。在优选的实施方案中接受本发明方法治疗的有复发高危险的患者有/曾经有被分类为N0和M0的原发肿瘤。在更优选的实施方案中,N0和M0患者,在原发肿瘤切除和任选的淋巴结切除之后,没有证据诊断患者有任何残留的肿瘤疾病。
另外,非转移性疾病可能在单一的区域淋巴结(N1)、或在一个以上的区域淋巴结(N2)中有淋巴道转移。在本发明优选的实施方案中,有复发高危险的患者有/曾经有被分类为N1或N2的原发肿瘤。
本发明各风险组选择的理论基础如下:
风险组I:
一旦Gerota氏筋膜穿透pT3子群的分析显示中值存活率的显著降低(pT3a和pT3b之间的过渡)。对于pT3a中值存活率为107个月,对于pT3b存活率降低至64个月而对于pT3c存活率降低至30个月(14)。此外,对814名患者进行前瞻性群组研究,其中RCC进一步分类为预测临床结果的风险组。名称为“非转移性高危险”(NM-HR)的一组,包括非转移性、T3或更大的N0肿瘤以及与其它因素的各种组合(操作量表,核分级)。在第24个月时复发评价为约42%的这组预后比中度和低风险组显著糟糕(7)。
风险组II:
在过去曾经显示淋巴结阳性的患者复发的概率显著高于其它患者类别。在淋巴结阳性组中,80%的患者在30个月之内复发。淋巴结阴性疾病的患者预后好得多,只有40%在第3年复发(15,16)。
风险组III:
若干研究已经表明微血管侵入的存在(MVI)对其预测值的意义。当在血管中发现肿瘤时认为存在微观的侵入,即认为至少一个或多个血管
中膜内皮细胞包围肿瘤细胞群。Lang等评价了255名通过全肾切除治疗的N0M0患者中在外科手术后至少5年的观察期期间的该参数(17)。通过双盲组织学研究测定MVI的存在,在29%的患者中有记录。在该研究中,MVI不是无关的和显著的预测因素,但与转移性发展危险增加有关。最近van Poppel已经追溯分析了180名全肾切除或部分肾切除的患者(18)。将微观的血管侵入的关联性与传统的肿瘤肿瘤分类、分级和肿瘤直径比较。在28.3%的患者中发现MVI。在有MVI但没有淋巴结介入或宏观血管侵入的患者中疾病级数的危险在一年之内为45%。
由于不能认为MVI的存在是显著性的预测因素,已经进行原发肿瘤切除且不显示MVI的患者毫无疑问被认为是有复发高危险的患者。因此,在优选的实施方案中,接受本发明方法治疗的患者不显示任何组织学上证实的MVI。
在进一步的研究中,将级别、血管侵入和年龄小确定为在肾切除后临床局部化的RCC中主要的独立预测因素(19)。
就分级而言,显示高级别的肿瘤(例如Fuhrman级别3和4)比低级别的肿瘤(级别1和2)有差的癌特异性存活率和无转移的存活率(21)。
MVI和分级结合当同时肿瘤尺寸大于4cm时视为阴性的预测因素。因此,这类患者也必须被认为是有复发高危险的患者。支持性证据来自对840名pT1肾细胞癌患者的评价(20)。该追溯性研究支持结论:对于透明细胞RCC的患者对pT1 7-cm肿瘤尺寸的切去可能太大,向危险增加的过渡发生在4.5-5cm的肿瘤尺寸。在该肿瘤尺寸下,发现RCC死亡的从低(比所期待的少)到高(比所期待的大)危险的过渡。实际上,在2002年UICC TNM分类法进行了修订,pT1a和pT1b之间的过渡定在4cm。
因此,在另一优选的实施方案中,接受本发明方法治疗的患者显示MVI。由于MVI和分级的结合(其可视为阴性的预测因素),更优选接受本发明方法治疗的显示MVI的患者的原发肿瘤核分级为至少GIII。
在一优选的实施方案中,将本发明的抗体在单一治疗方案中给药。
在其它优选的实施方案中,可将抗体在联合治疗方案中给药。该辅助抗体治疗可与任何其它类型的辅助治疗例如辅助性化疗、辅助性激素治疗或/和辅助性放射治疗结合。更优选,可将细胞因子与该抗体一起共同给药以增加抗体依赖的细胞毒性(ADCC)和/或激活患者的免疫系统,例如NK细胞。
细胞因子优选选自白细胞介素,例如IL-2、3、4、5、6、7、8、9、10、11、12、13、14和15,干扰素,例如IFN-α、IFN-β和IFN-γ,TNF-α、TNF-β,神经生长因子(NGF),CD40、FAS、CD27和CD30的配体,巨噬细胞抑制蛋白,Rantes,活性片段和可药用类似物和衍生物以及它们的混合物。更优选细胞因子选自IL-2和IFN-α。
例如,医生将根据年龄、重量和疾病的严重程度确定G250抗原特异性抗体的剂量。例如,每个患者每周的周期20mg或50mg cG250的给药方案,将释放0.5μg/ml以上的浓度,因此应该有足够效力。因此,在优选的实施方案中,以5至250mg/周G250-特异性抗体、更优选10至100mg/周和最优选20mg/周至50mg/周的每周剂量给药。
在上述技术研究现状中所收集的药代动力学数据表明cG250的血浆水平在治疗的六至十周达到平高线水平。据发现,在剂量为每周20mg的情况下,波谷血浆在10周的治疗之后于5.5μg/ml变为水平。令人惊奇地,其波谷水平与50mg的单一剂量之后达到的波谷水平(4.2μg/ml)几乎相同。这表明通过给予预先50mg的负荷剂量可使每周20mg的剂量加速达到稳态血浆浓度水平。
因此,本发明的方法优选包括将G250抗原特异性抗体或/和其抗体片段在至少两个治疗阶段中给药于需要其的对象,其中以不同的量、优选渐减量的抗体给药。
更优选本发明的方法包括在两个阶段将G250抗原特异性抗体或/和其抗体片段给药于需要其的对象,其中
(a)在第一治疗阶段中所给G250抗原特异性抗体的剂量为10-250mg/周,优选20-100mg/周,更优选20-50mg/周,最优选50mg/周,和
(b)在第二治疗阶段中所给G250抗原抗体的剂量为5-100mg/周,优选10-50mg/周,更优选15-25mg/周,最优选20mg/周。
更优选第一治疗阶段包括给予50mg/周的G250-特异性抗体,第二治疗阶段包括给予20mg/周的G250-特异性抗体。
抗癌的抗体优选静脉内给药、优选通过输注或静脉注射。抗体通过输注的给药,优选在最高约30分钟、更优选约15分钟内进行。
每周20或50mg的cG250的输注达20周的给药计划似乎能较好耐受且不会引起显著的HACA发生。
因此优选,第一治疗阶段包括最多12周、优选最多6周、更优选最多一周,且第二治疗阶段包括最多156周、优选最多104周、更优选最多52周、尤其更优选最多12-24周。
在最优选的实施方案中,对于肾透明细胞癌第一治疗阶段包括最多一周和cG250的50mg/周的单一负荷剂量的给药,第二治疗阶段包括最多24周和cG250的20mg/周的给药。
在另一实施方案中,本发明涉及一种药物组合物或药盒,其包含以本发明如上所述的方法给药的G250抗原特异性抗体或/和其抗体片段。
在优选的实施方案中,药物组合物或药盒包含第一组合物,其中第一组合物包含第一治疗阶段中治疗的G250抗原特异性抗体或/和其抗体片段,进一步包含第二组合物,其中第二组合物包含用于第二治疗阶段中治疗的G250抗原特异性抗体或/和其抗体片段。

实施例1
包括在有透明细胞RCC和复发高危险的患者中进行辅助抗体-cG250治疗的临床试验。
1.1设计
这是评价在外科手术后没有残留疾病证据且有复发高危险的透明细胞RCC患者中与安慰剂比较辅助cG250治疗的效力和安全性的预期的、多中心、III期研究。
主要目的是通过与安慰剂供给比较评价在治疗供给中的无病生存和总体生存来评价治疗效力。此外,还评价抗体治疗的安全性和对生活质量的影响。
1.2终点标准
首要目的是:
a)评价cG250治疗与安慰剂相比较的无病存活率
b)评价cG250治疗与安慰剂相比较的总存活数
第二目的是:
c)利用核实问卷评价在治疗和安慰剂供给中的生活质量

1.3研究的时间
在两种供给中对个体患者治疗的时间为连续24周。存活的监测将一直持续到310名死亡发生或最后一名患者参加后60个月,以较后的时间为期限。

1.5研究药物
研究药物通过静脉输注一周(±两天)一次给药,连续24周。在第1周时给予单一负荷剂量的50mg的cG250,之后在第2-24周时每周输注20mg的cG250。对于在第1周的第一次给药将总共50mg抗体以溶液的形式调配。对于2-24周的进一步的连续给药将总共20mg抗体以溶液的形式调配。该溶液用注射器抽取,在加入到100ml生理盐水(无菌的0.9%氯化钠水溶液)之前必须用0.2μm滤过器预过滤。另外2ml的生理盐水用于冲洗滤过器以避免研究药物在滤过器上损失。将包含抗体的生理盐水注入到输注溶液中。将输液在15分钟内给药到坐着或仰卧的患者。

认为与安慰剂比较在治疗供给中显著较好的无病存活率是治疗效力的证据。这可通过测定总存活数(中值存活率和5年的存活率)随后证实。放射学评价用来记录肿瘤复发的存在。对转移性疾病或局部复发的两种独立的放射学检查(reviews)用于中心评价在基线时和在研究期间的螺旋CT。
1.6.2无病存活率的评价
肿瘤复发的评价是基于胸、腹部和骨盆(静脉相)的对比增强的螺旋计算机体层摄影(CT),试验点的放射部门做的相邻切片厚度≤7.5mm。为了保证质量将扫描用数字技术储存在CD-ROM上。在特殊情况下以胶片的形式提供数据也可接受,例如若在某个评价时点不能获得数字数据时。
1.7统计
第一研究终点为无病存活率和总存活数。
·从随机化的日期直到有记录的复发日期计算无病存活率。当在所有患者当中50%已经复发时则达到中值。没有复发记录的患者在他们最后的研究评价日期接受检查。
·复发定义为有转移性疾病的体征或被计算机X射线体层摄影证实有局部复发、死亡(排除与该疾病无关的死亡)或开始新的抗肿瘤治疗。
·从随机化的日期到所记录的死亡日期计算总存活数。没有死亡记录的患者在他们最后的研究评价日期接受检查。
第二终点为:
·生活质量(EORTC QLQ-C30问卷)由EORTC开发的QLQ-C30既包括多项等级又包括单项测量(24,25)。这些包括五个功能等级、三个症状等级、整体的健康状态/QoL等级以及六个单项。每一多项等级包括不同的项目组-没有项目存在多于一个的等级。编码该评分方法的统计程序包已由EORTC提供并在SAS中运行。

·根据NCI CTC标准分等级的实验室值。
1.7.1统计模型
对无病存活率和总存活数应用分级测试以保持5%的总体显著水平。
对无病存活率,第一研究终点之一,在cG250供给和安慰剂供给之间采用组序贯对数-秩和检验(a group sequential log-ranktest),该检验基于总体5%的α水平上的O′Brien和Fleming类型边界值。模型估计值采用Kaplan-Meier法导出。对无病存活率的主要分析是基于打算处理的人群(定义为所有随机化的患者)。
对总存活数,另一个第一研究终点,在cG250供给和安慰剂供给之间采用对数-秩和检验和Kaplan-Meier法进行比较。采用组序贯法的O′Brien-Fleming法调整总存活数分析内的显著性水平以保持总显著性水平为5%。对总存活数的主要分析是基于打算治疗的人群(定义为所有随机化的患者)。
对各部分的95%置信区间采用精确方法计算(Pearson-Clopper)。
无病存活率和总存活数的Kaplan-Meier曲线由处理组显示。运用描述统计学。漏失值没有被代替。
采用时间对事件参数的Cox比例危险模型研究预测因素对无病存活率和总存活数两者可能的影响。目的是探究调整主要的预测因素后有统计显著性的敏感性。认为有可能的预测值的主要参数反映在TNM分类和研究项目的疾病阶段中。迄今为止研究项目定义的三个高风险标准充分地反映了危险因素,其预测值在肾细胞癌中确定。
进行美国和欧洲地点的分离分层法。
当主要分析设定时利用打算治疗的人群对所有的效力进行分析并利用每个方案人群重复。
第二统计分析试验在5%的α水平下进行并被认为是探测性的,因此没有对多重性进行调整。

Claims (21)

5.权利要求1至4任一项的方法,其中抗体或/和其抗体片段选自多克隆抗体、单克隆抗体、其抗原结合片段如F(ab′)2、Fab′,sFv、dsFv及其嵌合的、人源化的和完整人的变体。
6.权利要求1至5任一项的方法,其中原发肿瘤按照TNM分类法被分类为N0和M0。
7.权利要求6的方法,其中在原发肿瘤切除和任选的淋巴结切除之后,患者被诊断为没有任何残留的肿瘤疾病的迹象。
8.权利要求1至5任一项的方法,其中原发肿瘤按照TNM分类法已被分类为N1或N2。
9.权利要求1至8中任一项的方法,其中原发肿瘤不显示微血管侵入。
10.权利要求1至8中任一项的方法,其中原发肿瘤显示微血管侵入。
11.权利要求10的方法,其中原发肿瘤显示至少GIII的核等级。
12.前述权利要求中任一项的方法,其中受治疗的患者分类为风险组I、II或III。
13.前述权利要求中任一项的方法,其包括将G250抗原特异性抗体或/和其抗体片段在至少两个治疗阶段中给药于需要其的对象,其中施用不同的量、优选渐减量的抗体。
14.权利要求13的方法,其包括将G250抗原特异性抗体或/和其抗体片段给药于需要其的对象,其中
(a)在第一治疗阶段中所给G250抗原特异性抗体的剂量为10-250mg/周,优选20-100mg/周,更优选20-50mg/周,最优选50mg/周,和
(b)在第二治疗阶段中所给抗G250抗体的剂量为5-100mg/周,优选10-50mg/周,更优选15-25mg/周,最优选20mg/周。
15.前述权利要求中任一项的方法,其中G250抗原特异性抗体或/和抗体片段经静脉内途径给药。
16.权利要求15的方法,其中静脉内给药为输注,优选在最高约30分钟、更优选约15分钟内进行。
17.权利要求13至16中任一项的方法,其中第一治疗阶段包括最多12周、优选最多6周、更优选最多一周,且第二治疗阶段包括最多156周、优选最多104周、更优选最多52周、更优选最多12-24周。
18.前述权利要求中任一项的方法,其中癌症为肾透明细胞癌。
19.权利要求18的方法,其中抗体为cG250或由杂交瘤细胞系DSMACC 2526产生的抗体。
20.一种药物组合物或药盒,其包含用于权利要求1至19任一项的方法中给药的G250抗原特异性抗体或/和其抗体片段。
21.权利要求20的药物组合物或药盒,其包含第一组合物,其中第一组合物包含用于第一治疗阶段中治疗的G250抗原特异性抗体或/和其抗体片段,进一步包含第二组合物,其中第二组合物包含用于第二治疗阶段中治疗的G250抗原特异性抗体或/和其抗体片段。
