Abstract
De uitvinding heeft betrekking op een farmaceutische compositie met een voor nasale toediening geschikte samenstelling en vorm, waarin hydroxocobalamine in opgeloste vorm aanwezig is in een concentratie hoger dan 1 %.

FARMACEUTISCHE COMPOSITIE VOOR DE NASALE TOEDIENING VAN HYDROXOCOBALAMINE
De uitvinding betreft een farmaceutische compositie voor de nasale toediening van hydroxocobalamine, een stof uit de Vitamine B12 familie.
Vitamine BI2 is geïsoleerd in 1948 en wordt sindsdien op grote schaal toégepast bij de therapie van pernicieuze anemie en andere ziekten tengevolge van een Vitamine B12 deficiëntie. Deze ziekten ontstaan onder meer door een gebrekkige opname van Vitamine BI2 uit het maagdarmkanaal.
Vitamine B12 komt in het lichaam voor als methylcobalamine, adenosylcobalamine, hydroxocobalamine en cyanocobalamïne. In de geneeskunde worden uitsluitend cyanoco-balamine en hydroxocobalamine gebruikt voor toediening aan patiënten.
Orale, sublinguale en nasale toediening van Vitamine B12 is tot heden weinig effectief gebleken, vandaar dat intramusculaire toediening van cyanocobalamine en hydroxocoba-lamine nog steeds de enige algemeen aanbevolen therapie van Vitamine B12 deficiëntie is.
Aan de injectie van Vitamine B12 kleven echter vele nadelen, vooral omdat voor deze therapie oudere patiënten in aanmerking komen. Deze patiënten hebben slechts weinig spiermassa, waardoor de herhaalde injecties dikwijls ongerief veroorzaken, verder pijnlijk zijn en tussenkomst van een dokter vereisen. Onderhoudstherapie betekent maandelijkse injecties, meestal levenslang.
; Reeds in 1953 beschreven Monto et al.(Am.J.Med.Sci. 1953, volume 225, pag. 113-119) een studie waarbij als alternatief voor de injectie, een nasale inhalatie werd toegepast om cyanocobalamine te laten resorberen in respiratoir weefsel, voornamelijk in de longen.
Behalve cyanocobalamine in lactose poeder werd ook een fysiologische zoutoplossing, bevattend 5 tot 200 jig kristallijn cyanocobalamine per ml, gebruikt. De voorlopige resultaten van dit onderzoek werden beschreven als bemoedigend. Israels en Shubert (The Lancet, 1954 volume 1, pag.341-343) deden eveneens onderzoek met de inhalatie van 'cyanocobalamine in poeder en zij bevestigden de eerste resultaten van Monto et al..In een tweede publicatie beschrijven Monto et al. (Arch.of Int. Med.1954, volume 93, pag.219- 230) dat een goed haemopoietisch effect werd gezien bij de nasale toediening van cyanocobalamine in een dosis van 100 fig per ml., en ook na inhalatie van deze vloeistof en van lactose poeder, waarin cyanocobalamine was verwerkt. Hoewel beoordeeld door deze onderzoekers als een veilige, effectieve en niet kostbare therapie, heeft toch deze wijze van nasale toediening geen navolging gevonden in de daaropvolgende decennia. Blijkbaar waren de conclusies te optimistisch.
Andere voorstellen voor de intranasale toediening van cyanocobalamine zijn beschreven in PCT/US 86/00665 en PCT/US 86/00793. In beide publicaties wordt gesteld dat de eerdere experimenten uit de vijftiger jaren niet succesvol waren, vanwege het feit, dat de nasaal toegediende formulering van cyanocobalamine, in een vloeistof dan wel in een poeder, niet in de neus blijft maar de neusholte snel passeert en in de longen of de keelholte terecht komt. Daarom wordt in beide patentaanvragen als oplossing van dit probleem gegeven dat de formulering, een veel langere tijd in de neus moet blijven. Dit wordt bereikt door de formulering als aérosol toe te dienen (PCT/US 86/00665) of door 'de viscositeit te verhogen tot boven 2500 cP (PCT/US 86/00793). Daarmee zou een effectieve, consistente en uniforme absorptie van Vitamine B12 mogelijk zijn. Als kenmerkende concentratie voor een waterige oplossing wordt opgegeven 0.05-1% g/g.
Uitvoerig onderzoek is door ons verricht naar het mechanisme van de absorptie via de neus van zowel hydroxocobalamine als ook cyanocobalamine. Beide stoffen werden in isotonische waterige oplossingen, aan vrijwilligers toegediend.
Het uitgevoerde onderzoek resulteerde in het nieuwe inzicht, dat het nasaal absorberen van Vitamine B12 niet of slechts in zeer beperkte mate wordt bevorderd door een lange verblijfstijd van de compositie in de neus of door de hoeveelheid ingebrachte substantie, doch dat het absorberen in grote en overwegende mate bepaald wordt door de gebruikte concentratie van Vitamine B12.
De hoge intranasale absorptie van Vitamine B12, die bij de toediening grote voordelen biedt, blijkt bereikt te kunnen worden door hydroxocobalamine toe te passen, dat een aanzienlijk hogere oplosbaarheid vertoont dan die van cyanocobalamine.
Uitsluitend met hydroxocobalamine kan een superieure nasale toedieningsvorm in water worden geformuleerd, met veruit de hoogste concentratie en met als gevolg, een meer efficiënte nasale absorptie. Patiënten behoeven een dergelijke nasale formulering minder frequent te gebruiken, waardoor de therapie gemakkelijker en goedkoper wordt.
BESCHRIJVING EN VOORBEELD VAN DE UITVINDING
De volgende experimenten werden uitgevoerd in 6 gezonde mannelijke vrijwilligers om de snelheid van absorptie en het absorptieprofiel van Vitamine B12 te onderzoeken. Alleen niet-rokers werden als vrijwilliger geselecteerd, omdat bij rokers de bloedconcentraties van cyanocobalamine afwijken van het normale patroon. Serumconcentraties werden gemeten in pMol/L met een radioimmunoassay. De resultaten van het onderzoek worden samengevat in Tabel 1.

Het experiment zoals beschreven in Tabel 1, werd herhaald met 500 cyanocobalamine. Tevens werden formuleringen onderzocht als aérosol en formuleringen die methylcellulbse 400 mPas 1-2% en hydroxypropylmethylcellulose 4000 inPas 1% als verdikkingsmiddel bevatten. Geen van deze formuleringen gaf een significant ander resultaat dan hetgeen in Tabel 1 is weergegeven. Uit deze onderzoekingen hebben wij de zeer verrassende conclusies getrokken dat: de absorptie van hydroxocobalamine en cyanocobalamine in de menselijke neus plaats vindt tussen 0-60 minuten na toediening en dat de maximale bloedconcentra-tie (Cmax) wordt bereikt tussen 30-60 minuten (Tmax) na toediening; en dat toediening per aérosol en/of toediening in aanwezigheid van verdikkingsmiddelen, zoals beschreven in PCT/US 86/00665 en respectievelijk PCT/US 86/00793, geen invloed van betekenis hebben op de snelheid en de hoeveelheid Vitamine B12 die in de neus geabsorbeerd wordt.
Voortgezet onderzoek naar het mechanisme van de absorptie van cyanocobalamine en hydroxocobalamine leverde een verrassend resultaat. Er werd vastgesteld dat: de hoeveelheid die via de neus geabsorbeerd wordt, voornamelijk bepaald wordt door hoogte van de concentratie van het Vitamine B12 in opgeloste vorm, die ter plaatse van het neusepitheel wordt aangeboden; en dat zeer effectief werkende concentraties, die via de neus kunnen worden toegediend en geabsorbeerd, boven de 1 % liggen. Deze concentratie kan bij hydroxocobalamine verbindingen (door een goede oplosbaarheid in water) oplopen tot ongeveer 10%, hetgeen ongeveer lOx meer is dan maximaal met cyanocobalamine kan worden bereikt.
Ter illustratie van deze uitvinding vermeldt Tabel 2 de resultaten van een studie bij 6 vrijwilligers naar de absorptie van hydroxocobalamine uit een formulering die veel meer hydroxocobalamine HC1 (2%) bevat dan maximaal mogelijk is met cyanocobalamine. Duidelijk blijkt, dat de efficiëntie van de absorptie sterk toeneemt bij toename van de concentratie in de nasale formulering. De stijging van de Cmax in Tabel 2 is enkele malen groter dan in Tabel 1. De concentratie blijkt dus van doorslaggevende betekenis voor de absorptie. Een superieure absorptie blijkt derhalve mogelijk bij toediening van èen hógé concentratie, veel hoger dan maximaal met cyanocobalamine kan worden bereikt.

Een geschikte concentratie van hydroxocobalamine voor nasale toediening ligt hoger dan 1%, bij voorkeur aanzienlijk hoger. De maximale oplosbaarheid van hydroxocobalamine is ongeveer 10%, hetgeen ongeveer lOx hoger is dan bereikt kan worden met cyanocobala-mine. De toediening van de formulering aan de patiënt kan geschieden middels een methode die gebruikelijk is voor nasale toediening. Ook mogelijk zijn capsules, tampons of sponsjes waarin de hydroxocobalamine formulering is verwerkt. De formulering bevat dus hydroxocobalamine in een waterige oplossing en bezit als gehele compositie een lage viscositeit. Deze formulering kan tevens een aantal hulpstoffen bevatten, die uit de farmaceutische literatuur bekend zijn en die gebruikt worden om aan nasale formuleringen toe te voegen, zoals een conserveermiddel, een pH-instellend middel, een toniciteitsmid-del, een oppervlakte-actieve stof, een complexerende stof, een stabiliserende stof en een oplossingsbevorderende stof.
Hydroxocobalamine komt voor als hydrochloride, sulfaat, acetaat en dergelijke verbindingen. In water is hydroxocobalamine in evenwicht met aquocobalamine. Voor farmaceutische doeleinden dient hydroxocobalamine te voldoen aan de eisen van een Farmacopee. Het door ons gebruikte hydroxocobalamine HC1 voldeed aan de eisen van de Britse Farmacopee (1988).
Claims (5)
- 2. Farmaceutische compositie volgens conclusie 1, met het kenmerk dat de concentratie van hydroxocobalamine ligt tussen 1.1% en ongeveer 10 %.
- 3. Farmaceutische compositie volgens conclusie 1 en 2, met het kenmerk dat de viscositeit kleiner is dan 100 cP.
- 4. Farmaceutische compositie volgens conclusie 1, met het kenmerk dat het tevens een conserveermiddel en/of een pH-instellend middel en/of een toniciteitsmiddel en/of een oppervlakte-actieve stof en/of een complexerende stof en/of een stabiliserende stof en/of een oplossingsbevorderende stof bevat. 5.Inrichting omvattende een dispenser voor nasale toediening, welke een bepaalde hoeveelheid van een farmaceutische compositie volgens één der voorgaande conclusies bevat.
- 6. Capsule voor nasale toediening inhoudende een farmaceutische compositie volgens één der voorgaande conclusies.
- 7. Nasale tampon of sponsje of een dergelijk element, waarin een bepaalde hoeveelheid van een farmaceutische compositie volgens één der voorgaande conclusies is verwerkt.
