Abstract
本発明は、表面の電荷が異なる複数の炎症性サイトカインを高効率に吸着除去できる血液浄化材料を提供することを目的とする。本発明は、水不溶性基材と、酸性官能基を含むリガンドと、塩基性官能基を含むリガンドとを有し、上記酸性官能基を含むリガンド及び上記塩基性官能基を含むリガンドは、上記水不溶性基材に結合しており、上記酸性官能基を含むリガンドの分子量(MwA)を、上記塩基性官能基を含むリガンドの分子量(MwB)で除した値(MwA/MwB)が、1.000未満である、血液浄化材料を提供する。
Description
本発明は、血液浄化材料に関する。
近年、血液から炎症性サイトカイン等の血液成分を選択的に分離、吸着する目的で、種々の血液浄化材料及び当該材料を充填した血液浄化カラムが開発されている。
血液成分に対する血液浄化材料の吸着性能を向上させる手段としては、対象物質(例えば、炎症性サイトカイン)との相互作用が強い材料を基材として用いることや、対象物質との相互作用の強いリガンドを基材表面に付与することが一般的に知られている。
例えば、特許文献1には、セルロースからなるビーズ表面にデキストラン硫酸とトリプトファンを含む材料が、血液接触材料が有する補体活性化作用を抑制し、血漿中のC3a濃度の増大を低減させることが開示されている。
特許文献2には、水不溶性の基材表面に、酸性官能基を含む化合物がアミド結合を介して結合した改質重合体を有する血液成分の吸着材料が開示されており、改質重合体の含水率と、酸性官能基の導入量を規定することで、サイトカインを高効率に吸着することが開示されている。
特許文献3には、カチオン性重合体を表面処理したアクリロニトリルとアニオン性モノマーからなる中空糸膜が、アナフィラキシー様反応の原因となる、接触相の活性化を抑制することが開示されている。
例えば、急性呼吸窮迫症候群(ARDS)や敗血症のような炎症性疾患を発症した患者では、サイトカインストームと呼ばれる現象が発生し、インターロイキン6(IL-6)やインターロイキン8(IL-8)に代表される複数の炎症性サイトカインが過剰に産生されることが知られている。これらの炎症性サイトカインは分子量が5~20kDaと様々であり、表面の電荷も異なるが、炎症性疾患の患者を治療するためには、これらの複数のサイトカインを除去することが望ましい。
本発明は、表面の電荷が異なる複数の炎症性サイトカインを高効率に吸着除去できる血液浄化材料を提供することを目的とする。
本発明者らは、上記課題を解決すべく、鋭意検討を重ねた結果、酸性官能基を含むリガンドの質量平均分子量が塩基性官能基を含むリガンドの質量平均分子量よりも小さい血液浄化材料が、表面の電荷が異なる複数の炎症性サイトカインを高効率に吸着できることを見出し、本発明を完成するに至った。
すなわち、本発明は、以下の(1)~(7)を提供する。
(1) 水不溶性基材と、酸性官能基を含むリガンドと、塩基性官能基を含むリガンドとを有し、上記酸性官能基を含むリガンド及び上記塩基性官能基を含むリガンドは、上記水不溶性基材に結合しており、上記酸性官能基を含むリガンドの質量平均分子量(MwA)を、上記塩基性官能基を含むリガンドの質量平均分子量(MwB)で除した値(MwA/MwB)が、1.000未満である、血液浄化材料。
(2) 上記MwA/MwBは、0.001~0.430である、(1)記載の血液浄化材料。
(3) 上記MwAは、0.5×102~90.0×103であり、前記MwBは、1.0×102~66.6×103ある、(1)又は(2)記載の血液浄化材料。
(4) 上記酸性官能基の含有量は、乾燥重量1g当たり0.02~3.00mmolであり、上記塩基性官能基の含有量は、乾燥重量1g当たり0.50~2.00mmolである、(1)~(3)のいずれかに記載の血液浄化材料。
(5) 上記酸性官能基の含有量は、乾燥重量1g当たり0.02~1.50mmolであり、上記塩基性官能基の含有量は、乾燥重量1g当たり0.70~1.50mmolである、(1)~(3)のいずれかに記載の血液浄化材料。
(6) 炎症性サイトカイン吸着用である、(1)~(5)のいずれかに記載の血液浄化材料。
(7) (1)~(6)のいずれかに記載の血液浄化材料を備える、血液浄化カラム。

本発明の血液浄化材料は、表面の電荷が異なる複数の炎症性サイトカインを高効率に吸着除去できる。
本発明の血液浄化材料は、水不溶性基材と、酸性官能基を含むリガンドと、塩基性官能基を含むリガンドとを有し、上記酸性官能基を含むリガンド及び上記塩基性官能基を含むリガンドは、上記水不溶性基材の表面に結合しており、上記酸性官能基を含むリガンドの質量平均分子量(MwA)を、上記塩基性官能基を含むリガンドの質量平均分子量(MwB)で除した値(MwA/MwB)が、1.000未満であることを特徴とする。
「血液浄化材料」とは、血液成分中にある有機物を吸着する材料を意味し、当該材料の少なくとも一部に水不溶性基材を含み、水不溶性基材単独のもの及び水不溶性基材に適当な補強材が固定化又は混合されたものも含む。固定化又は混合の操作は、所望の形状に加工する前に行ってもよいし、加工した後に行ってもよい。
「血液成分」とは、血液を構成する成分を意味し、例えば、血液中の液性因子や血液中の細胞が挙げられる。本発明の血液浄化材料が吸着対象物質とする血液成分に特に制限はないが、血液成分の中でも血液中の液性因子が吸着対象物質として好ましい。
「血液中の液性因子」とは、血液中に溶解している有機物を意味する。具体的には、尿素、β2-ミクログロブリン、炎症性サイトカイン、IgE若しくはIgG等のタンパク質又はlipopolysaccharide(以下、LPSと略す)等の多糖類が挙げられる。これらの中でも、尿素、炎症性サイトカイン等のタンパク質又はLPS等の多糖類が吸着対象物質として好ましく、本発明の血液浄化材料を炎症性疾患の治療を目的として使用する場合は、炎症性サイトカインが吸着対象物質としてより好ましい。
「炎症性サイトカイン」とは、感染や外傷等の刺激により、免疫担当細胞を始めとする各種の細胞から産生され、細胞外に放出されて作用する一群のタンパク質を意味し、例えば、インターフェロンα、インターフェロンβ、インターフェロンγ、インターロイキン1~インターロイキン15、腫瘍壊死因子-α、腫瘍壊死因子-β、ハイモビリティーグループボックス-1、エリスロポエチン又は単球走化因子が挙げられる。これらの中でも、炎症性サイトカインは、表面の電荷の違いに基づいて、陽性荷電サイトカインと陰性荷電サイトカインに分類される。
「陽性荷電サイトカイン」とは、等電点が7.5以上であるサイトカインであり、インターロイキン8(以下、IL-8)、及びインターフェロンβ等が挙げられる。
「陰性荷電サイトカイン」とは、等電点が6.5以下であるサイトカインであり、インターロイキン6(以下、IL-6)、及びインターロイキン-12(IL-12)等が挙げられる。
本実施形態に係る血液浄化材料は、タンパク質吸着用であることが好ましく、サイトカイン吸着用であることがより好ましい。サイトカインの中でも、IL-8及びIL-6が炎症性疾患の主要な原因物質であることから、IL-8及び/又はIL-6吸着用であることがさらに好ましく、IL-8及びIL-6吸着用であることが最も好ましい。
「吸着」とは、物質が材料に付着し、容易に剥離しない状態又は吸着平衡状態を意味する。吸着の原理に特に制限はないが、例えば、静電相互作用、疎水性相互作用、水素結合又はファンデルワールス力等の分子間力が挙げられる。
「水不溶性基材」とは、水に不溶性の基材である。ここで、水に不溶とは、水不溶性基材を水に入れた前後の乾燥重量変化が1%以下であることを意味する。この乾燥重量変化は、水不溶性基材を乾燥重量の9倍量の37℃の水に1時間浸漬した後にピンセット等で引き上げ、残った水を50℃以下で真空乾燥させた後に残った固形分の乾燥重量の、浸漬前の水不溶性基材の乾燥重量に対する割合である。水に不溶でない場合は、使用時に溶出物が多くなる危険性があり、安全上好ましくない。
「乾燥重量」とは、乾燥状態の固体の重量を意味する。ここで乾燥状態の固体とは、当該固体中に含まれる液体成分の量が1重量%以下の状態の固体を表し、固体の重量を測定した後に80℃、大気圧で24時間加熱乾燥し、残存した固体の重量減少量が乾燥前の重量の1重量%以下であるとき、当該固体は乾燥状態とみなす。
水不溶性基材を構成する成分としては、例えば、ポリエチレンテレフタレート、ポリブチレンテレフタレート、ポリ芳香族ビニル化合物、ポリエステル、ポリスルホン、ポリエーテルスルホン、ポリスチレン及びそれらの誘導体(例えば、ポリカーボネート、ポリエーテルケトン、ポリエーテルエーテルケトン、ポリフェニレンサルファイド、ポリフェノール、ポリフェニレンエーテル、ポリフェニレンエチニレン、ポリアミドイミド、ポリスチレンスルホン酸、ポリ(4-メチルスチレン)、ポリ(4-エチルスチレン)、ポリ(4-イソプロピルスチレン)、ポリ(2-クロロスチレン)、ポリ(4-クロロスチレン)、ポリ(3-ヒドロキシスチレン)、ポリ(4-メトキシスチレン)、ポリ(4-カルボキシスチレン)、ポリ(4-ニトロスチレン)、ポリ(4-クロロメチルスチレン)、ポリ(2,4-ジメチルスチレン)、ポリ(2,5-ジクロロスチレン)、ポリ(2,4,5-トリブロモスチレン)、ポリ(2,3,4,5,6-ペンタフルオロスチレン)、スルホン化ポリスルホン、スルホン化ポリエーテルスルホン)、ポリビニルアルコール、酢酸セルロース、ポリアクリロニトリル、並びに、これらの単独重合体、共重合体、及び混合物からなる群から選択される化合物が挙げられる。酸性官能基又は塩基性官能基を、水不溶性基材の表面に固定させる場合には、水不溶性基材を構成する成分は、単位重量当たりの芳香環の数が多く、硫酸基、スルホン酸基又はアミノ基を固定化しやすいことから、ポリスチレン、ポリスチレンの誘導体、ポリスルホン、ポリスルホンの誘導体、ポリエーテルスルホン及びポリエーテルスルホンの誘導体並びにそれらの混合物からなる群から選択される化合物であることが好ましく、ポリスチレン、ポリスチレンの誘導体、ポリスルホン及びポリスルホンの誘導体並びにそれらの混合物からなる群から選択される化合物であることがより好ましく、ポリスチレンであることがさらに好ましい。ポリスチレンの誘導体としては、例えば、ポリスチレンスルホン酸、ポリ(4-メチルスチレン)、ポリ(4-エチルスチレン)、ポリ(4-イソプロピルスチレン)、ポリ(2-クロロスチレン)、ポリ(4-クロロスチレン)、ポリ(3-ヒドロキシスチレン)、ポリ(4-メトキシスチレン)、ポリ(4-カルボキシスチレン)、ポリ(4-ニトロスチレン)、ポリ(4-クロロメチルスチレン)、ポリ(2,4-ジメチルスチレン)、ポリ(2,5-ジクロロスチレン)が挙げられ、ポリスルホンの誘導体としては、例えば、スルホン化ポリスルホンが挙げられ、ポリエーテルスルホンの誘導体としては、例えば、スルホン化ポリエーテルスルホンが挙げられる。
水不溶性基材の形状としては、比表面積が大きく、取扱い性に優れる点で、繊維形状又は粒子形状が好適である。
水不溶性基材が繊維形状である場合、水不溶性基材の形状は、繊維を加工した糸束、ヤーン、ネット、編地、織物、フェルト、ネット等が好ましく、比表面積が大きく、流路抵抗が小さい、糸束、編地、織物、フェルト、及びネットがより好ましい。これらの中でも、編地、フェルト、及びネットは、繊維を原料として、公知の方法により製造することができる。例えば、編地及びネットの製造方法としては、平織り法又は筒編み法が挙げられる。特に、血液浄化器に充填する観点から、単位体積当たりの充填重量が多い、筒編み法により製造される編地が好ましい。
水不溶性基材を構成する繊維(例えば、海島複合繊維)の単糸径(以下、繊維径とも称する。)に特に限定はないが、吸着対象物質との接触面積の向上と材料の強度維持の観点から、3~200μmが好ましく、5~50μmがより好ましく、10~40μmがさらに好ましい。いずれの好ましい下限値もいずれの好ましい上限値と組み合わせることができる。
「単糸径」とは、繊維の小片サンプル10個をランダムに採取して、走査型電子顕微鏡を用いて1000~3000倍の写真をそれぞれ撮影し、各写真辺り10カ所(計100カ所)の繊維の直径を測定した値の平均値を意味する。
水不溶性基材が粒子形状である場合、対象物質を吸着させるための十分な比表面積を確保する観点から、粒子の直径は、1~500μmであることが好ましい。
血液浄化材料は、吸着対象物質と相互作用する必要があるため、少なくとも血液等に含まれる有機物と接触する表面にリガンドが結合していることが必要である。
「表面」とは、水不溶性基材の表面を意味し、表面に細孔を有する形状の場合は、細孔の凹凸に沿った最外層部分も表面に含まれる。さらに、繊維の内部に貫通孔を有する場合は、水不溶性基材の最外層部分だけではなく、当該基材の内部の貫通孔の外層も、表面に含まれる。
「結合」とは、水不溶性基材の表面に、リガンドが化学的相互作用、又は物理的相互作用によって存在する状態を意味する。ここで、化学的相互作用とは、共有結合、静電相互作用、又は水素結合を意味し、物理的相互作用とは、ファンデルワールス力を意味する。特に限定はされないが、血液と接触した際に安定して表面に存在できることから、水不溶性基材とリガンドとの結合は、共有結合が好ましい。
「リガンド」とは、水不溶性基材の表面に結合した化合物を意味し、酸性官能基又は塩基性官能基を少なくとも一つ含んでいれば、その化学構造は特に制限されるものではない。リガンドとしては、酸性官能基を含むリガンド及び塩基性官能基を含むリガンドが挙げられる。なお、上記官能基は、同一又は異なる官能基を複数組み合わせていてもよい。また、リガンドは、上記酸性官能基又は塩基性官能基を有していれば、さらに中性官能基を有していてもよい。中性官能基としては、例えば、メチル基若しくはエチル基等のアルキル基、又は、フェニル基、アルキル基で置換されたフェニル基若しくはハロゲン原子で置換されたフェニル基等のアリ-ル基が挙げられる。アルキル基で置換されたフェニル基としては、例えば、パラ(p)-メチルフェニル基、メタ(m)-メチルフェニル基、オルト(o)-メチルフェニル基、パラ(p)-エチルフェニル基、メタ(m)-エチルフェニル基、又はオルト(o)-エチルフェニル基等が挙げられる。ハロゲン原子で置換されたフェニル基としては、例えば、パラ(p)-フルオロフェニル基、メタ(m)-フルオロフェニル基、オルト(o)-フルオロフェニル基、パラ(p)-クロロフェニル基、メタ(m)-クロロフェニル基、又はオルト(o)-クロロフェニル基等が挙げられる。中性官能基と酸性官能基又は塩基性官能基とは、直接結合していてもよいし、スペーサーを介して結合していてもよい(当該結合に関与するスペーサーをスペーサー1と称する。)。スペーサー1としては、例えば、尿素結合、アミド結合、ウレタン結合が挙げられる。
水不溶性基材と、酸性官能基を含むリガンド又は塩基性官能基を含むリガンドとは、直接結合していてもよいし、反応性官能基由来のスペーサーを介して結合していてもよい(当該結合に関与するスペーサーをスペーサー2と称する。)。スペーサー2としては、尿素結合、アミド結合、エーテル結合、エステル結合、ウレタン結合等の電気的に中性の化学結合を有しているものであればよく、アミド結合又は尿素結合を有しているものが好ましい。なお、本発明の血液浄化材料において、スペーサー2は、酸性官能基を含むリガンド又は塩基性官能基を含むリガンドに含まれるものとみなす。
水不溶性基材と、酸性官能基を含むリガンド及び塩基性官能基を含むリガンドとの結合を媒介する反応性官能基としては、例えば、ハロアルキル基(ハロメチル基、ハロエチル基等)、ハロアシル基(ハロアセチル基、ハロプロピオニル基等)若しくはハロアセトアミドアルキル基(ハロアセトアミドメチル基、ハロアセトアミドエチル基等)等の活性ハロゲン基、エポキサイド基、カルボキシル基、イソシアン酸基、チオイソシアン酸基又は酸無水物基が挙げられる。適度な反応性を有する観点から、上記反応性官能基としては、活性ハロゲン基が好ましく、ハロアセトアミドアルキル基がより好ましく、ハロアセトアミドメチル基がさらに好ましい。反応性官能基を導入した水不溶性基材の具体的な例としては、表面にクロロアセトアミドメチル基を導入したポリスチレン、及び表面にクロロアセトアミドメチル基を導入したポリスルホンが挙げられる。
反応性官能基は、予め、水不溶性基材と適当な試薬とを反応させることで、水不溶性基材に結合させることができる。例えば、水不溶性基材を構成する成分がポリスチレンで、反応性官能基がクロロアセトアミドメチル基の場合は、ポリスチレンとN-ヒドロキシメチル-2-クロロアセトアミドを反応させることで、クロロアセトアミドメチル基が結合したポリスチレンを得ることができる。クロロアセトアミドメチル基が結合したポリスチレンに対し、例えば、アミノ基を有するテトラエチレンペンタミンを反応させることで、テトラエチレンペンタミンがアセトアミドメチル基を介して結合したポリスチレンが得られる。この場合、アセトアミドメチル基はスペーサー2に相当し、テトラエチレンペンタミンは、リガンドに相当する。水不溶性基材を構成する成分、スペーサー(スペーサー1及びスペーサー2)、及びリガンドは、任意に組み合わせることができる。リガンドが結合した水不溶性基材の例としては、エチレンジアミン、ジエチレントリアミン、トリエチレンテトラミン又はテトラエチレンペンタミン等のポリアミンを含むリガンドが、アセトアミドメチル基を介して結合したポリスチレン、エチレンジアミン、ジエチレントリアミン、トリエチレンテトラミン又はテトラエチレンペンタミン等のポリアミンを含むリガンドがアセトアミドメチル基を介して結合したポリスルホンが挙げられる。
「酸性官能基を含むリガンド」とは、称する化学構造の一部に、硫酸基(-OSO2OH)、スルホン酸基(-SO2OH)及びカルボン酸基(-COOH)並びにそれらの塩からなる群から選ばれる酸性官能基を少なくとも一つ含む化合物を意味する。上記酸性官能基を有していればその化学構造に限定はないが、本実施形態において、水不溶性基材の成分がポリスチレンである場合、ベンゼン環に対して導入反応が容易なことから、酸性官能基としては、スルホン酸基又はその塩が好ましい。
「塩基性官能基を含むリガンド」とは、称する化学構造の一部に、アミノ基並びにそれらの塩からなる群から選ばれる塩基性官能基を少なくとも一つ含む化合物を意味する。アミノ基を有していればその化学構造に限定はないが、水不溶性基材に対して、アミド結合又はウレア結合等を介した導入反応が容易であり、MwBが大きいものを用いることができる観点から、塩基性官能基を含むリガンドとしては、ポリアミンが好ましい。
「ポリアミン」とは、称する化学構造の一部にアミノ基を2個以上有する化合物を意味し、例えば、エチレンジアミン、ジエチレントリアミン、トリエチレンテトラミン、テトラエチレンペンタミン、ペンタエチレンヘキサミン、ヘキサエチレンヘプタミン、ヘプタエチレンオクタミン、オクタエチレンノナミン、及びポリエチレンイミン等のポリエチレンアミン類が挙げられる。ポリアミンとしては、テトラエチレンペンタミンが好ましい。また、ポリアミン構造内のアミノ基は、1級アミン又は2級アミン由来のアミノ基であることが好ましい。上記ポリアミンは直鎖状、分岐状、及び環状のいずれでもよい。さらに、上記ポリアミンは、炭素数1~10のアルキル基、ビニル基、アリル基等の不飽和アルキル鎖、フェニル基、ナフチル基、アントラシル基等の芳香族置換基、イミダゾリル基、ピリジル基、ピペリジル基等の複素環式置換基を含んでいてもよい。
酸性官能基を含むリガンドの質量平均分子量(MwA)は、血液浄化材料の塩酸加水分解により抽出液を作製し、ゲル浸透クロマトグラフィー(以下、GPC)を行うことで同定することができる。具体的な測定方法の一例は次のとおりである。2cm×2cmの大きさに切り出した血液浄化材料をバイアル瓶に入れ、6M塩酸を2mL加えた後、乾熱滅菌機を用いて110℃で20時間加熱することで、抽出液を作製する。得られた抽出液を0.5mL採取し、水/メタノール=1/1(0.1N硝酸リチウム添加)1mLを加え、さらに、陽イオン交換樹脂を添加して静置する。静置後の上澄み液を測定溶液とする。得られた測定溶液を、ゲル浸透クロマトグラフ分析計[例えば、Prominence GPCシステム(島津製作所製)]を用いて測定することで、MwAを同定することができる。GPCの装置の構成及び測定条件の一例は以下のとおりである。

MwAをGPCによって測定した結果、得られたGPCチャート上にピークが認められなかった場合、すわわち、MwAが1000未満であると判断される場合は、GPCの代わりに、ガスクロマトグラフ質量分析(以下、GCMS)を行うことで、MwAを同定することができる。具体的な測定方法は次のとおりである。2cm×2cmの大きさに切り出した血液浄化材料をバイアル瓶に入れ、6M塩酸を2mL加えた後、乾熱滅菌機を用いて110℃で20時間加熱することで、抽出液を作製する。得られた抽出液を0.5mL採取し、水1.5mLを加えて、測定溶液とする。得られた測定溶液を、ガスクロマトグラフ質量分析計を用いて測定することで、MwAを同定することができる。GCMSの装置の構成及び測定条件の一例は以下のとおりである。

塩基性官能基を含むリガンドの質量平均分子量(MwB)は、血液浄化材料を塩酸によって加水分解した抽出液を、MwAの測定と同様の方法で作製した後に、これを6M水酸化ナトリウム水溶液で中和してから、MwAの測定と同様の方法でGPC又はGCMSを行うことで、同定することができる。
MwAが大きすぎると、塩基性官能基を含むリガンドとイオンコンプレックスを形成することで運動性が阻害され、血液成分との相互作用が低下する。MwAが小さすぎると、MwBが大きい場合に、酸性官能基を含むリガンドが塩基性官能基を含むリガンドに覆われてしまい、血液成分との相互作用が阻害されてしまう。こられのことから、MwAは0.5×102~90.0×103が好ましく、0.5×102~20.0×103がより好ましく、0.5×102~1.3×103がさらに好ましく、0.5×102~1.0×103がよりさらに好ましく、0.7×102~0.9×103が最も好ましい。好ましい下限値はいずれの好ましい上限値を組み合わせることができる。
MwBが大きすぎると、酸性官能基を含むリガンドとイオンコンプレックスを形成することで運動性が阻害され、血液成分との相互作用が低下する。MwBが小さすぎると、MwAが大きい場合に、塩基性官能基を含むリガンドが酸性官能基を含むリガンドに覆われてしまい、血液成分との相互作用が阻害されてしまう。こられのことから、MwBは1.0×102~66.0×103が好ましく、1.0×102~22.0×103がより好ましく、1.0×102~14.0×103がさらに好ましく、1.0×102~13.0×103がよりさらに好ましく、1.7×102~13.0×103が最も好ましい。好ましい下限値はいずれの好ましい上限値を組み合わせることができる。
「MwAをMwBで除した値(MwA/MwB)」は、MwAをMwBで除することで算出される。MwA/MwBを1.000未満にすることで、表面の電荷が異なる複数の炎症性サイトカインの吸着率を向上させることができる。これは、酸性官能基を含むリガンドと塩基性官能基を含むリガンドとが、イオンコンプレックスを形成することなく、それぞれが炎症性サイトカイン等の血液成分に対して相互作用を発現することができるためであると考えられる。また、MwA/MwBが1.00以上になると、酸性官能基を含むリガンド同士が水素結合を形成することで、炎症性サイトカイン等の血液成分に対して相互作用を発揮できないためであると考えられる。よって、MwA/MwBは、1.000未満であることが好ましく、0.001以上1.000未満であることがより好ましく、0.001~0.430であることがさらに好ましく、0.006~0.430であることが最も好ましい。いずれの好ましい下限値もいずれの好ましい上限値と組み合わせることができる。
酸性官能基の含有量は、血液浄化材料を酸塩基逆滴定することにより測定できる。測定方法としては、例えば、予め乾燥重量を測定した血液浄化材料の表面を、塩酸を用いて脱塩処理した後、イオン交換水を用いて洗浄液のpHが中性になるまで洗浄を繰り返す。続いて、真空乾燥により水分を除くことで乾燥状態にし、既知濃度の水酸化ナトリウム水溶液を添加して、当該材料表面の酸性官能基を塩にする。この際に消費された水酸化ナトリウムの濃度を滴定により測定することで、酸性官能基の含有量を求めることができる。
酸性官能基の含有量は、少なすぎると血液成分と相互作用できず、多すぎると抗凝固剤に対する吸着性能を発現してしまい、血液の凝固を亢進する懸念があることから、血液浄化材料の乾燥重量1g当たり0.02~3.00mmolであることが好ましく、0.02~2.00mmolであることがより好ましく、0.02~1.50mmolであることがさらに好ましく、0.80~1.50mmolであることが最も好ましい。いずれの好ましい下限値もいずれの好ましい上限値と組み合わせることができる。
塩基性官能基の含有量は血液浄化材料を酸塩基逆滴定することにより測定できる。測定方法としては、例えば、予め乾燥重量を測定した血液浄化材料の表面を、水酸化ナトリウム水溶液を用いて脱塩処理した後、イオン交換水を用いて洗浄液のpHが中性になるまで洗浄を繰り返す。続いて、真空乾燥により水分を除くことで乾燥状態にし、既知濃度の塩酸を添加して、当該材料表面の塩基性官能基を塩にする。この際に消費された塩酸の濃度を滴定により測定することで、塩基性官能基の含有量を求めることができる。
塩基性官能基の含有量は、少なすぎると血液成分と相互作用できず、多すぎると血液に対して溶血毒性を発現してしまい、安全性が低下する懸念があることから、血液浄化材料の乾燥重量1g当たり0.50~2.00mmolであることが好ましく、0.50~1.50mmolであることがより好ましく、0.70~1.50mmolであることがさらに好ましく、1.10~1.50mmolであることが最も好ましい。いずれの好ましい下限値もいずれの好ましい上限値と組み合わせることができる。
本実施形態に係る血液浄化材料は、血液浄化カラムに充填する材料として好ましく用いられ、特に、炎症性疾患の治療を目的として体外循環を行う場合は、炎症性サイトカインの吸着除去用の材料として好適に用いられる。血液浄化材料を用いた血液浄化カラムを血液浄化療法に用いる場合には、体外に導出した血液を直接カラムに通してもよいし、血漿分離膜等と組み合わせて使用してもよい。
「炎症性疾患」とは、体内で炎症反応が惹起される疾患全体を表し、例えば、全身性エリテマトーデス、悪性関節リウマチ、多発性硬化症、潰瘍性大腸炎、クローン病、薬剤性肝炎、アルコール性肝炎、A型肝炎、B型肝炎、C型肝炎、D型肝炎、E型肝炎、敗血症(例えば、グラム陰性菌由来の敗血症、グラム陽性菌由来の敗血症、培養陰性敗血症、真菌性敗血症)、インフルエンザ、急性呼吸窮迫症候群(Acute Respiratory Distress Syndrome:ARDS;急性呼吸促迫症候群、急性呼吸促進症候群とも表記される。)、急性肺傷害(Acute Lung Injury:ALI)、膵炎、特発性間質性肺炎(Idiopathic Pulmonary Fibrosis:IPF)、炎症性腸炎(例えば、潰瘍性大腸炎、クローン病)、臓器移植後の再灌流障害、胆嚢炎、胆管炎又は新生児血液型不適合等が挙げられる。
炎症性疾患の中でも、血液中に原因物質が放出され、血液浄化による治療効果が特に期待できることから、薬剤性肝炎、アルコール性肝炎、A型肝炎、B型肝炎、C型肝炎、D型肝炎、E型肝炎、敗血症(例えば、グラム陰性菌由来の敗血症、グラム陽性菌由来の敗血症、培養陰性敗血症、真菌性敗血症)、インフルエンザ、急性呼吸窮迫症候群、急性肺傷害、膵炎、又は特発性間質性肺炎が、血液浄化カラムによる治療対象として好ましい。本実施形態の血液浄化カラムの用途としては、例えば、上記の炎症性疾患の治療用途が好ましく、その中でも、薬剤のみでは治療が困難であり、炎症性サイトカインが関与している疾患と考えられている、敗血症(例えば、グラム陰性菌由来の敗血症、グラム陽性菌由来の敗血症、培養陰性敗血症、真菌性敗血症)、インフルエンザ、急性呼吸窮迫症候群、急性肺傷害、特発性間質性肺炎の治療用途がより好ましい。
血液浄化材料の血液浄化性能の評価方法としては、例えば、IL-6吸着率及びIL-8吸着率を測定する方法が挙げられる。IL-6及びIL-8は血液成分中に含まれる炎症性サイトカインの一種であり、炎症性疾患患者の血液において、顕著に増加することが知られていることから、血液浄化性能評価用の血液成分として好適である。IL-6吸着率及びIL-8吸着率が高いほど、血液浄化材料の血液浄化性能が高いと判断できる。
血液浄化材料のサイトカイン吸着性能としては、炎症性疾患患者の血中に産生したサイトカインの濃度を低下させ、全身性ショック及び血圧降下等の重篤な症状から一刻も早く回復させる観点から、IL-6吸着率が10%以上であり、IL-8吸着率が20%以上であることが好ましい。また、IL-6吸着率が10%以上であり、IL-8吸着率が40%以上である、又は、IL-6吸着率が20%以上であり、IL-8吸着率が20%以上であることがより好ましく、IL-6吸着率が20%以上であり、IL-8吸着率が40%以上であることがさらに好ましい。
また、本発明の血液浄化カラムは、上記の血液浄化材料を備えることを特徴としている。
「血液浄化カラム」とは、少なくとも、液体入口部、ケース部、及び液体出口部を有しており、ケース部に血液浄化材料が充填されているものを意味する。カラムとしては、例えば、ラジアルフロー型のカラムが挙げられる。
血液浄化カラムの容器形状としては、血液成分等を含む液体(以下、液体)の入口部及び出口部、並びにケース部を有し、当該ケース部内に血液浄化材料を充填できる形状であればよい。一つの実施形態としては、血液浄化材料をパイプに巻きつけ、円筒状にしたもの(以下、円筒)を内部に充填できる容器で、液体が円筒の外周より入り、円筒の内側へと流れた後に当該液体が容器外に出る容器、又は液体が円筒の内側より入り、円筒の外側へと流れた後に当該液体が容器外に出る容器が挙げられる。製造効率や処理液のショートパス抑制の観点から、血液浄化カラムの容器としては、側面に孔を持つパイプに対して血液浄化材料が巻きつけられている構造が好ましく、具体的には、供給された液体を流出するために設けられた孔を長手方向の側面に備える中心パイプと、当該中心パイプの周りに充填され、当該液体に含まれる標的物質を吸着させる血液浄化材料と、流入してきた当該液体が当該中心パイプの中を通るように当該中心パイプの上流端に連通され、当該液体が当該中心パイプを通過せずに当該血液浄化材料と接触するのを防ぐように配置されたプレートと、当該中心パイプの下流端を封鎖し、当該血液浄化材料を当該中心パイプの周りの空間に固定するように配置されたプレートと、を備えるラジアルフロー型の容器が挙げられる。また、容器の形状は、円柱状、又は、三角柱状、四角柱状、六角柱状若しくは八角柱状等の角柱状が挙げられるが、これらの形状に限定されるものではない。また別の実施形態としては、円形に切り取った血液浄化材料を充填可能な円筒状の空間を内部に有し、液体導入口及び液体排出口を有した容器が考えられる。具体的には、液体を導入するために設けられた液体導入口を備えるプレートと、導入された液体を排出するために設けられた液体排出口を備えるプレートとを有し、円形に切り取った血液浄化材料が充填された円筒状のケース部を内部に有する容器が挙げられる。なお、血液浄化材料の形状は、円形に限られず、血液浄化カラムの容器形状に合わせて、楕円形、三角形や四角形等の多角形、台形等の任意の形状に適宜変更することができる。
血液浄化カラムの容器としては、ガラス製、プラスチック・樹脂製、ステンレス製等の容器が挙げられ、臨床現場や測定場所での操作性及び廃棄の容易さを考慮すると、プラスチック・樹脂製の容器が好ましい。容器のサイズは使用目的に応じて適宜選択されるが、臨床現場や測定場所での操作性・廃棄の容易さを考慮すると、手で握りやすいサイズであることが好ましく、血液浄化カラム全体の高さは1cm以上30cm以下、外径は1cm以上10cm以下、内容積は200mL以下であることが好ましい。
血液浄化材料は、血液浄化カラム内に積層されて充填されていることが好ましい。ここで、積層とは、血液浄化材料を2枚以上密着させて重ねることを意味し、積層されて充填する方法としては、例えば、アキシャルフローカラムのように、シート形態に加工した血液浄化材料を複数枚重ねていく方法や、ラジアルフローカラムのように、孔を持つパイプにシート形態に加工した血液浄化材料を巻きつけていく方法が挙げられる。
血液浄化カラム内に充填するものは、血液浄化材料単独でもよく、他の水不溶性基材及び/又は各種スペーサーを組み合わせて充填してもよい。スペーサーとしては、例えば、編地、織物、不織布、シート形状にした繊維、膜、ビーズ、及びハイドロゲル等が挙げられる。
以下、本発明の血液浄化材料について実施例により具体的に説明するが、本発明はこれらの例によって限定されるものではない。
(実施例1)
海成分としてポリスチレン、島成分としてポリプロピレンを用いて、別々に溶融計量し、1つの吐出孔当たり700の島成分用分配孔が穿設された海島複合口金が組み込まれた紡糸パックに流入させて、海島複合流とし、溶融吐出した。島比率を50wt%に制御し、海島複合繊維の表面から最外島成分までの距離を2μmに調整し、単繊度が3.0dtex(繊維径20μm)である海島複合繊維を得た。
海成分としてポリスチレン、島成分としてポリプロピレンを用いて、別々に溶融計量し、1つの吐出孔当たり700の島成分用分配孔が穿設された海島複合口金が組み込まれた紡糸パックに流入させて、海島複合流とし、溶融吐出した。島比率を50wt%に制御し、海島複合繊維の表面から最外島成分までの距離を2μmに調整し、単繊度が3.0dtex(繊維径20μm)である海島複合繊維を得た。
当該海島複合繊維を用いて、筒編み機(機種名:丸編み機 MR-1、丸善産業株式会社)の度目調整目盛りを調整し、目付けが60g/m2、嵩密度が0.20g/mLの編地Aを作製した。
N-ヒドロキシメチル-2-クロロアセトアミド(以下、NMCA)8gを、ニトロベンゼン62mLと98重量%硫酸41mLとの混合溶液に添加し、NMCAが溶解するまで5℃で攪拌して、NMCA溶液を調製した。次に、ニトロベンゼン5mLと98重量%硫酸3mLとの混合溶液にパラホルムアルデヒド(以下、PFA)0.5gを添加し、PFAが溶解するまで50℃で攪拌し、PFA溶液を調製した。PFA溶液8mLを5℃に冷却後、NMCA溶液111mLに混合し、5分間攪拌した。PFA溶液とNMCA溶液との混合液に、編地A5gを2時間浸漬させた。
次いで、PFA溶液とNMCA溶液との混合液に浸漬させた編地Aを、ニトロベンゼン62mLと98重量%硫酸41mLとの混合溶液に、50℃で4時間浸漬させた。編地Aを0℃のニトロベンゼン100mL中に浸して反応を停止させた後、ガラスフィルターを用いてろ別し、1000mLのメタノールで洗浄した。
メタノールで洗浄した編地Aを、テトラエチレンペンタミン(TEPA;シグマアルドリッチ製;品番:30-0850-05)1.3mLとトリエチルアミン2mLとジメチルスルホキシド(DMSO)93mLとの混合液に、40℃で3時間浸漬させた。ガラスフィルターを用いて編地Aをろ別し、1000mLのDMSOで洗浄した。当該編地に付着しているDMSOを1000mLのメタノール、2000mLのイオン交換水で洗浄することで、血液浄化材料1を得た。
(実施例2)
実施例1(血液浄化材料1の製法)を基準として、ニトロベンゼンと98重量%硫酸との混合溶液への編地Aの浸漬を実施せず、TEPAの添加量を1.3mLから0.7mLに変更する以外は、同様の操作を行うことで、血液浄化材料2を得た。

実施例1(血液浄化材料1の製法)を基準として、TEPA1.3mLを30%ポリエチレンイミンP-70溶液(富士フィルム和光純薬製;品番:169-11955;平均分子量:約70000(カタログ値))5gに変更する以外は、同様の操作を行うことで、血液浄化材料4を得た。
(比較例1)
編地Aを、実施例1と同様の操作で作製した。次に、実施例1と同様の操作でNMCA溶液を作製し、編地A5gを2時間浸漬させた。編地Aを、0℃のニトロベンゼン100mLに浸して反応を停止させた後、ガラスフィルターを用いてろ別し、1000mLのメタノールで洗浄した。
編地Aを、実施例1と同様の操作で作製した。次に、実施例1と同様の操作でNMCA溶液を作製し、編地A5gを2時間浸漬させた。編地Aを、0℃のニトロベンゼン100mLに浸して反応を停止させた後、ガラスフィルターを用いてろ別し、1000mLのメタノールで洗浄した。
エチレンジアミン(富士フィルム和光純薬製)0.5mLとDMSO199mLとの混合液に、メタノールで洗浄した後の編地Aを、40℃で3時間浸漬させた。ガラスフィルターを用いて編地Aをろ別し、200mLのDMSOで洗浄した。編地Aに付着しているDMSOを1000mLのメタノールで洗浄した。
1.0質量%となるようにポリアクリル酸25000(富士フィルム和光純薬株式会社製;品番:162-18581;平均分子量:約25000(カタログ値))をメタノール180mLに溶解させた1.0質量%ポリアクリル酸25000溶液に、編地Aをそのまま添加し、さらにDMT-MM(富士フィルム和光純薬製)0.9gを添加し、40℃で2時間浸漬させた。編地Aを、ガラスフィルターを用いてろ別し、1000mLのメタノールで洗浄した。
メタノールで洗浄した後の編地Aを、TEPA1.5mLとメタノール93mLとの混合溶液に、40℃で3時間浸漬させた。編地Aを、ガラスフィルターを用いてろ別し、1000mLのメタノール、2000mLのイオン交換水で洗浄することで、血液浄化材料5を得た。
(比較例2)
比較例1(血液浄化材料1の製法)を基準として、TEPA1.5mLをポリエチレンイミン(富士フィルム和光純薬製;品番:166-17825;平均分子量:約10000(カタログ値))5gに変更する以外は、同様の操作を行うことで、血液浄化材料6を得た。

2cm×2cmの大きさに切り出した血液浄化材料をバイアル瓶に入れ、6M塩酸を2mL加えた後、乾熱滅菌機を用いて110℃で20時間加熱することで、抽出液を作製した。得られた抽出液を0.5mL採取し、水/メタノール=1/1(0.1N硝酸リチウム添加)1.5mLを加え、さらに、陽イオン交換樹脂(オルガノ製;品番:IR120B H)を添加して1時間静置した。静置後の上澄み液を測定溶液とした。得られた測定溶液を、ゲル浸透クロマトグラフ分析計(Prominence GPCシステム;島津製作所製)を用いて測定することで、MwAを同定した。GPCの装置の構成及び測定条件は以下のとおりである。

<血液浄化材料のMwBのGCMSによる測定>
血液浄化材料のMwBをGPCによって測定した結果、得られたGPCチャート上にピークが認められなかった場合、GCMSによって、以下の方法でMwBを測定した。2cm×2cmの大きさに切り出した血液浄化材料をバイアル瓶に入れ、6M塩酸を2mL加えた後、乾熱滅菌機を用いて110℃で20時間加熱することで、抽出液を作製した。得られた抽出液を0.5mL採取し、6M水酸化ナトリウム水溶液を0.5mLで中和した後、水1mLを加えて、測定溶液とした。得られた測定溶液を、MwAのGCMSによる測定と同様の操作で、ガスクロマトグラフ質量分析計を用いて測定することで、MwBを同定した。
血液浄化材料のMwBをGPCによって測定した結果、得られたGPCチャート上にピークが認められなかった場合、GCMSによって、以下の方法でMwBを測定した。2cm×2cmの大きさに切り出した血液浄化材料をバイアル瓶に入れ、6M塩酸を2mL加えた後、乾熱滅菌機を用いて110℃で20時間加熱することで、抽出液を作製した。得られた抽出液を0.5mL採取し、6M水酸化ナトリウム水溶液を0.5mLで中和した後、水1mLを加えて、測定溶液とした。得られた測定溶液を、MwAのGCMSによる測定と同様の操作で、ガスクロマトグラフ質量分析計を用いて測定することで、MwBを同定した。
<血液浄化材料の乾燥重量1g当たりの酸性官能基の含有量の測定>
50mLのポリプロピレン製遠沈管に血液浄化材料0.5gを入れ、6Mの塩酸40mLを添加し、室温で30分間転倒混和した。混和後、溶液のみをデカンテーションし、イオン交換水を20mL添加して5分間転倒混和した。混和後、溶液のみをデカンテーションし、イオン交換水を20mL添加して5分間転倒混和する操作を10回繰り返し、最後にデカンテーションした溶液のpHをリトマス試験紙にて確認し、pH6以上であることを確認した。洗浄後の血液浄化材料を40℃に昇温した真空乾燥機に入れて24時間真空乾燥した。乾燥した血液浄化材料を50mLのポリプロピレン製遠沈管に入れ、40mLの0.1M水酸化ナトリウム水溶液を添加し、30分間転倒混和した。当該遠沈管から5mLの上澄み溶液を抜き取り、別の15mLのポリプロピレン製遠沈管に入れて、さらに0.1M塩酸5mLを添加した後、メチルレッド水溶液とフェノールフタレイン水溶液をそれぞれ0.02mL添加した。ビュレットを用いて当該遠沈管に0.05Mの水酸化ナトリウム水溶液を0.02mL滴下し、10回転倒混和した後に溶液の色調を確認した。水酸化ナトリウム水溶液の滴下と転倒混和、色調確認を繰り返し、溶液の色調が赤橙色から黄色へ変化したと最初に認められた時の水酸化ナトリウム水溶液の滴下量を滴定量とした。以下の式1を用いて、得られた値の小数点第3位を四捨五入して、血液浄化材料の乾燥重量1g当たりの酸性官能基の含有量を算出した。

<血液浄化材料の乾燥質量1g当たりの塩基性官能基の導入量の測定>

<血液浄化材料のIL-6吸着率の測定>
ポリプロピレン製のアシストチューブに、直径8mmの円板状に切り抜いた血液浄化材料を6枚入れた。このアシストチューブに、IL-6の濃度が2000pg/mLなるように調製した牛胎児血清(Fetal Bovine Serum:FBS)を、1cm3の血液浄化材料に対して24mLになるように換算(固液比:0.04cm3/mL)して添加し、37℃のインキュベータ内で1時間転倒混和した後、酵素結合免疫吸着(ELISA)法にて、FBS中のIL-6濃度を測定した。転倒混和前後のIL-6濃度から、以下の式3により、IL-6吸着率を算出した。なお、IL-6吸着率は、小数点第1位を四捨五入した値を用いた。

ポリプロピレン製のアシストチューブに、直径8mmの円板状に切り抜いた血液浄化材料を6枚入れた。このアシストチューブに、IL-6の濃度が2000pg/mLなるように調製した牛胎児血清(Fetal Bovine Serum:FBS)を、1cm3の血液浄化材料に対して24mLになるように換算(固液比:0.04cm3/mL)して添加し、37℃のインキュベータ内で1時間転倒混和した後、酵素結合免疫吸着(ELISA)法にて、FBS中のIL-6濃度を測定した。転倒混和前後のIL-6濃度から、以下の式3により、IL-6吸着率を算出した。なお、IL-6吸着率は、小数点第1位を四捨五入した値を用いた。

<血液浄化材料のIL-8吸着率の測定>
ポリプロピレン製のアシストチューブに、直径6mmの円板状に切り抜いた血液浄化材料を4枚入れた。このアシストチューブに、IL-8の濃度が2000pg/mLなるように調製したFBSを、1cm3の血液浄化材料に対して286mLになるように換算(固液比:0.004cm3/mL)して添加し、37℃のインキュベータ内で1時間転倒混和した後、ELISA法にて、FBS中のIL-8濃度を測定した。転倒混和前後のIL-8濃度から、以下の式4により、IL-8吸着率を算出した。なお、IL-8吸着率は小数点第1位を四捨五入した値を用いた。

血液浄化材料1~9における、MwA、MwB、酸性官能基の含有量、塩基性官能基の含有量、IL-6吸着率、及びIL-8吸着率を測定した結果を、表1に示す。
表1の結果より、MwA/MwBが1.000未満である血液浄化材料1~4及び7、すなわち、MwAがMwBよりも小さい血液浄化材料は、表面電荷の異なる複数のサイトカイン(IL-6及びIL-8)に対する吸着率が高いことが明らかとなった。これに対して、MwAがMwBよりも大きい血液浄化材料5、6、8、及び9では、表面電荷の異なる複数のサイトカインに対する吸着率が低いことが明らかとなった。
本発明の血液浄化材料は、高効率に炎症性サイトカインを吸着できるため、体外循環用の吸着材料として利用できる。
Claims (7)
- 水不溶性基材と、酸性官能基を含むリガンドと、塩基性官能基を含むリガンドとを有し、
前記酸性官能基を含むリガンド及び前記塩基性官能基を含むリガンドは、前記水不溶性基材に結合しており、
前記酸性官能基を含むリガンドの分子量(MwA)を、前記塩基性官能基を含むリガンドの分子量(MwB)で除した値(MwA/MwB)が、1.000未満である、血液浄化材料。 - 前記MwA/MwBは、0.001~0.430である、請求項1記載の血液浄化材料。
- 前記MwAは、0.5×102~90.0×103であり、前記MwBは、1.0×102~66.0×103ある、請求項1又は2記載の血液浄化材料。
- 前記酸性官能基の含有量は、乾燥重量1g当たり0.02~3.00mmolであり、
前記塩基性官能基の含有量は、乾燥重量1g当たり0.50~2.00mmolである、請求項1~3のいずれか一項記載の血液浄化材料。 - 前記酸性官能基の含有量は、乾燥重量1g当たり0.02~1.50mmolであり、
前記塩基性官能基の含有量は、乾燥重量1g当たり0.70~1.50mmolである、請求項1~3のいずれか一項記載の血液浄化材料。 - 炎症性サイトカイン吸着用である、請求項1~5のいずれか一項記載の血液浄化材料。
- 請求項1~6のいずれか一項記載の血液浄化材料を備える、血液浄化カラム。
