Description
本発明は、2019年に発見された新型コロナウイルス(SARS-CoV-2)の検出方法に関し、さらに詳しくは遺伝子の高感度な検出法を利用した2019新型コロナウイルスによる急性呼吸器疾患(COVID-19)の診断補助方法に関するものである。
2019新型コロナウイルスによる急性呼吸器疾患(COVID-19)は、2019年12月に中国湖北省武漢市において原因不明の肺炎患者の発生として報告された。2020年1月7日に原因が新種のコロナウイルスであることが確認され、その翌週には、中国以外の複数の国で感染者が確認された。1月末の時点での感染者は1万人を越えたが感染拡大が止まらず、世界保健機構(WHO)は国際的に懸念される公衆衛生上の緊急事態(PHEIC)を1月31日に宣言した。
COVID-19の主な臨床症状は発熱、咳、及び全身倦怠感である。重症化すると肺炎や肺・心臓・腎の機能不全を引き起こし、死に至る場合がある。死亡率は2002年から2003年にかけて流行したSARSよりも低い3%程度であるが、感染者数がSARSの時よりもはるかに多いので、死亡者の数はSARS流行時を上回っている。
COVID-19を引き起こす新型ウイルス(SARS-CoV-2)はSARSコロナウイルスやMERSコロナウイルスと同様のベータコロナウイルスである。その遺伝子は1本鎖RNAのプラス鎖からなり、約30000塩基の長さがある(GENBANK Accession No.MN908947)。
SARS-CoV-2は、最初の肺炎患者発生の報告から約1ヶ月で全塩基配列が決定し公開された。この塩基配列に基づいてRT-PCRによるSARS-CoV-2の検出方法が開発された(非特許文献1他)。
しかしながら、これらの検出方法は短期間で開発されたため、中には検出精度に問題があるものも存在する。例えばアメリカ疾病予防管理センター(CDC)で開発された検査キットには一部に不具合があることがアナウンスされている(非特許文献2参照)。また、感染の急速な拡大により必要な検査件数も膨大な数に上っている。そのため、より迅速で精度の高いSARS-CoV-2の検出方法が求められている。

本発明は、COVID-19の診断のために、その病原ウイルスであるSARS-CoV-2を高感度に検出することを目的とする。
本発明者らは、上記課題を解決するために鋭意研究を行った結果、SARS-CoV-2に特異的な塩基配列と選択的にハイブリダイズするオリゴヌクレオチドプライマーを作製し、LAMP法によりSARS-CoV-2に特異的な塩基配列を増幅することで、SARS-CoV-2を高感度に検出できることを見出し、本発明を完成した。
すなわち、本発明は以下の構成からなる。

本発明によれば、SARS-CoV-2に特異的な塩基配列と選択的にハイブリダイズするオリゴヌクレオチドプライマーを作製し、LAMP法によりSARS-CoV-2に特異的な塩基配列を増幅することで、SARS-CoV-2を高感度かつ迅速に検出することができる。
本発明において使用される試料としては、SARS-CoV-2ウイルス感染を疑われる人間あるいは他の動物由来の検体、例えば喀痰、気管支肺胞洗浄液、鼻汁、鼻腔吸引液、鼻腔洗浄液、鼻腔拭い液、咽頭拭い液、うがい液、唾液、血液、血清、血漿、髄液、尿、糞便、組織などが挙げられる。また、感染実験などに用いられた細胞やその培養液、あるいは生体由来の検体や培養細胞などから分離したウイルスを含む検体なども試料となりうる。これらの試料は分離、抽出、濃縮、精製などの前処理を行ってもよい。
試料に含まれる核酸の増幅は、納富らが開発した、PCR法で不可欠とされる温度制御が不要な核酸増幅法:LAMP(Loop-mediated Isothermal Amplification)法と呼ばれるループ媒介等温増幅法(国際公開第00/28082号)で達成される。この方法は、鋳型となるヌクレオチドに自身の3’末端をアニールさせて相補鎖合成の起点とするとともに、このとき形成されるループにアニールするプライマーを組み合わせることにより、等温での相補鎖合成反応を可能とした核酸増幅法である。また、LAMP法では、プライマーの3’末端が常に試料に由来する領域に対してアニールするために、塩基配列の相補的結合によるチェック機構が繰り返し機能するため、その結果として、高感度にかつ特異性の高い核酸増幅反応を可能としている。
LAMP法で使用されるオリゴヌクレオチドプライマーは、鋳型核酸の塩基配列の計6領域、すなわち3’末端側からF3c、F2c、F1cという領域と、5’末端側からB3、B2、B1という領域の塩基配列を認識する少なくとも4種類のプライマーであって、各々フォワードインナープライマー及びバックワードインナープライマーとフォワードアウタープライマー及びバックワードアウタープライマーと呼ぶ。また、F1c、F2c、F3cの相補配列をそれぞれF1、F2、F3、またB1、B2、B3の相補鎖をB1c、B2c、B3cと呼ぶ。インナープライマーとは、標的塩基配列上の「ある特定のヌクレオチド配列領域」を認識し、かつ合成起点を与える塩基配列を3’末端に有し、同時にこのプライマーを起点とする核酸合成反応生成物の任意の領域に対して相補的な塩基配列を5’末端に有するオリゴヌクレオチドである。ここで、「F2より選ばれた塩基配列」及び「F1cより選ばれた塩基配列」を含むプライマーをフォワードインナープライマー(以下FIPプライマー)、そして「B2より選ばれた塩基配列」と「B1cより選ばれた塩基配列」を含むプライマーをバックワードインナープライマー(以下BIPプライマー)と呼ぶ。一方、アウタープライマーとは、標的塩基配列上の『「ある特定のヌクレオチド配列領域」の3’末端側に存在するある特定のヌクレオチド配列領域』を認識かつ合成起点を与える塩基配列を有するオリゴヌクレオチドである。ここで、「F3より選ばれた塩基配列」を含むプライマーをフォワードアウタープライマー(以下F3プライマー)、「B3より選ばれた塩基配列」を含むプライマーをバックワードアウタープライマー(以下B3プライマー)と呼ぶ。ここで、各プライマーにおけるFとは、標的塩基配列のセンス鎖と相補的に結合し、合成起点を提供するプライマー表示であり、一方Bとは、標的塩基配列のアンチセンス鎖と相補的に結合し、合成起点を提供するプライマー表示である。ここで、プライマーとして用いられるオリゴヌクレオチドの長さは、10塩基以上、好ましくは15塩基以上で、化学合成あるいは天然のどちらでも良く、各プライマーは単一のオリゴヌクレオチドであってもよく、複数のオリゴヌクレオチドの混合物であってもよい。
LAMP法においては、インナープライマーとアウタープライマーに加え、さらにこれとは別のプライマー、すなわちループプライマーを用いる事ができる。ループプライマー(Loop Primer)は、ダンベル構造の5’末端側のループ構造の一本鎖部分の塩基配列に相補的な塩基配列を持つプライマーである。このプライマーを用いると、核酸合成の起点が増加し、反応時間の短縮と検出感度の上昇が可能となる(特許文献国際公開第02/024902号パンフレット)。ループプライマーの塩基配列は上述のダンベル構造の5’末端側のループ構造の一本鎖部分の塩基配列に相補的であれば、標的遺伝子の塩基配列あるいはその相補鎖から選ばれても良く、他の塩基配列でもよい。また、ループプライマーは1種類でも2種類でも良く、本明細書中ではフォワードループプライマー(以下LF)とバックワードループプライマー(以下LB)と呼ぶ。
1本鎖RNAウイルスの遺伝子をLAMP法により増幅して検出する場合には、さらにアウタープライマーを追加することにより増幅反応初期段階の逆転写反応の効率を高めることができる。SARS-CoV-2はプラス鎖型1本鎖RNAウイルスであることから、B3プライマーの下流側にアウタープライマー(B4プライマー)を追加すればよい。
SARS-CoV-2はRNAウイルスである。LAMP法は鋳型がRNAの場合には、鋳型がDNAの場合の反応液に逆転写酵素を添加する事で、同様に核酸増幅反応を進めることができる(RT-LAMP法)。
本発明者らはSARS-CoV-2に特異的な塩基配列を迅速に増幅できるLAMP法のプライマーの塩基配列とその組み合わせを鋭意研究した結果、SARS-CoV-2の塩基配列(配列番号1)のN遺伝子領域とRNA依存性RNAポリメラーゼ(RdRP)遺伝子領域から、プライマーセットを設計した。すなわち、本発明は、SARS-CoV-2の塩基配列(配列番号1)のN遺伝子領域(配列番号1の28976塩基から29211塩基までの領域)とRNA依存性RNAポリメラーゼ(RdRP)遺伝子領域(配列番号1の15394塩基から15595塩基までの領域)から設計される少なくとも一つからなるプライマーセットである。本発明はF3、B3、FIP、及びBIPからなるプライマーセットであって、として次のa、bの2組から選択される少なくとも一つを含むプライマーセットを設計したである。加えて、本発明は、LFプライマー及びLBプライマーであって、次のa、bの2組から選択される少なくとも一つを含むプライマーセットである。
また、本発明者らはSARS-CoV-2に特異的な塩基配列を迅速に増幅できるLAMP法のプライマーの塩基配列とその組み合わせを鋭意研究した結果、SARS-CoV-2の塩基配列(配列番号1)のM遺伝子領域とS遺伝子領域から、プライマーセットを設計した。すなわち、本発明は、SARS-CoV-2の塩基配列(配列番号1)のM遺伝子領域(配列番号1の26767塩基から26977塩基までの領域)とS遺伝子領域(配列番号1の24660塩基から24916塩基までの領域)から設計される少なくとも一つからなるプライマーセットである。本発明はF3、B3(又はB3及びB4)、FIP、及びBIPからなるプライマーセットであって、次のc、dの2組から選択される少なくとも一つを含むプライマーセットである。加えて、本発明は、LFプライマー及びLBプライマーであって、次のc、dの2組から選択される少なくとも一つを含むプライマーセットである。
さらに、本発明は、上記プライマーセットc又はd及び蛍光標識プローブを含むキットである。プライマーセットcのプローブは配列番号20からなるプローブであり、プライマーセットdのプローブは配列番号28からなるプローブである。これらのプローブは3’末端のシトシン塩基がBODIPY(登録商標)FL色素で蛍光標識されている。これらのプローブはQプローブである。すなわち、プローブが標的核酸と結合すると標的核酸のグアニン塩基と蛍光色素が近接し、グアニン塩基の消光作用により、蛍光色素の蛍光強度が減少する。
本発明のSARS-CoV-2ウイルスの検出方法は、本発明のプライマーセットを用いてLAMP法により増幅反応を行う方法である。
核酸合成で使用する酵素は、鎖置換活性を有する鋳型依存性核酸合成酵素であれば特に限定されない。このような酵素としては、Bst DNAポリメラーゼ(ラージフラグメント)、Bca(exo-)DNAポリメラーゼ、大腸菌DNA ポリメラーゼIのクレノウフラグメント等が挙げられ、好ましくはBst DNAポリメラーゼ(ラージフラグメント)が挙げられる。
RT-LAMP法に用いる逆転写酵素としては、RNAを鋳型としてDNAを合成する活性を有する酵素であれば特に限定されない。このような酵素としては、AMV、Cloned AMV 、MMLVの逆転写酵素、SuperscriptII、ReverTraAce、Thermoscript等が挙げられ、好ましくは、AMVあるいはCloned AMV逆転写酵素が挙げられる。またBca DNAポリメラーゼのように、逆転写酵素活性とDNAポリメラーゼ活性の両活性を有する酵素を用いると、RT-LAMP反応を1つの酵素で行う事ができる。
核酸合成で使用する酵素や逆転写酵素は、ウイルスや細菌などから精製されたものでも良く、遺伝子組み換え技術によって作製されたものでもよい。またこれらの酵素はフラグメント化やアミノ酸の置換などの改変をされたものでもよい。
本発明のCOVID-19の検査方法は、本発明のプライマーセットを用いて、SARS-CoV-2ウイルスの標的核酸領域の増幅を検出することにより、SARS-CoV-2ウイルス感染の有無を検査する方法である。LAMP反応後の核酸増幅産物の検出には公知の技術が適用できる。例えば、増幅された塩基配列を特異的に認識する標識オリゴヌクレオチドや蛍光性インターカレーター法(特開2001-242169号公報)を用いる方法や、あるいは反応終了後の反応液をそのままアガロースゲル電気泳動にかけても容易に検出できる。アガロースゲル電気泳動では、LAMP増幅産物は、塩基長の異なる多数のバンドがラダー(はしご)状に検出される。また、LAMP法では、核酸の合成反応により副産物である不溶性のピロリン酸マグネシウムが生成するため、反応液が肉眼でも確認できる程に白濁する。この白濁を、光学的に測定することにより、核酸増幅反応を検出することも可能である(国際公開第01/83817号)。さらに、反応液に金属指示薬であるカルセインを添加して、増幅反応の進行に伴う金属イオン濃度の変化を蛍光の変化として検出することもできる(特開2004-283161号公報)。
COVID-19の診断方法において、本発明のプライマーセットは、核酸増幅の検出を行う際に必要な各種の試薬類とともに、あらかじめパッケージングしてキット化する事ができる。具体的には、本発明のプライマーやループプライマーとして必要な各種のオリゴヌクレオチド、核酸合成の基質となる4種類のdNTP、核酸合成を行うDNAポリメラーゼ、逆転写酵素、酵素反応に好適な条件を与える緩衝液や塩類、酵素や鋳型を安定化する保護剤、さらに必要に応じて反応生成物の検出に必要な試薬類がキットとして提供される。
以下に実施例を挙げて本発明を具体的に説明するが、本発明はこれらにより何ら限定されるものではない。
実施例1.プライマーセットa及びbの検出感度の確認
LAMP法による検出感度の確認を行った。
1.試料及び試薬の調製
1)試料(転写RNA)
RNA鋳型は、SARS-CoV-2遺伝子からRT-PCRにより作製したcDNAをプラスミドに組み込み、該プラスミドDNAからRNAを転写及び精製することにより作製した。転写には、Script Max(登録商標)Thermo T7 Transcription Kit(東洋紡株式会社製、コード番号:TSK-101)を用い、RNA精製にはRNeasy(登録商標)Mini Kit(キアゲン社製、カタログ番号:No.74104)を用いた。精製したRNAから1μLあたり10コピーから103コピーまでの希釈液を調製して試料溶液とした。またYeast RNA溶液を0コピーの試料溶液(Negative Control)とした。
実施例1.プライマーセットa及びbの検出感度の確認
LAMP法による検出感度の確認を行った。
1.試料及び試薬の調製

0.2mLの試薬チューブに、次の組成を有するLAMP反応試薬25μLを調製した。プライマーは、表1に示すプライマーセットaまたはbを用いた。

実施例2.ゲノムRNAを鋳型とした場合の検出感度
感染研から分与されたウイルスゲノムRNAを用いて検出感度の検討を行った。併せてPCRも実施して検出感度の比較を行った

各方法での測定結果を表5に示す。どちらの増幅方法も50コピーまでのゲノムRNAを検出できたが、LAMP法の方がPCRよりも短時間で検出することができた。

実施例3.プライマーセットc及びdの検出感度の確認
LAMP法による検出感度の確認を行った。
1.試料及び試薬の調製
1)試料(転写RNA)
M領域及びS領域の配列をそれぞれ組み込んだ人工遺伝子を合成し、その転写RNAを鋳型とした。人工遺伝子の合成はEurofins社に委託した。人工遺伝子の配列を配列番号29(M領域検出用人工遺伝子)及び30(S領域検出用人工遺伝子)に示す。
LAMP法による検出感度の確認を行った。

この反応試薬に、プライマーセットc又はd、プライマーセットに対応する蛍光標識プローブ、SYTOTM 63 Red Fluorescent Nucleic Acid Stainを添加し、15μL/testとなるように精製水(DW)で調製したマスターミックス(MM)を用い、そこにDW5μL、鋳型5μLの計25μLで1反応とした。反応試薬は氷上にて調製した。

LAMP反応試薬 12.5μL

実施例2.SARSウイルスとの交差性の確認
GGGenome(https://gggenome.dbcls.jp/ja/)にて、呼吸器関連菌や病原微生物38種(Human coronavirus 229E, Human coronavirus OC43, Human coronavirus HKU1, Human coronavirus NL63, SARS-coronavirus, SARS-coronavirus-2, MERS-coronavirus, Adenovirus, Human Metapneumovirus, Parainfluenza virus 1-4, Influenza A, Influenza B, Enterovirus, Respiratory syncytial virus, Rhinovirus, Chlamydia pneumonia, Haemophilus influenzae, Legionella pneumophila, Mycobacterium tuberculosis, Streptococus pneumonia, Streptococcus pyrogenes, Bordetella pertussis, Mycoplasma pneumoniae, Pneumocystis jirovecii (PJP), Influenza C, Parechovirus, Candida albicans, Corynebacterium diphtheriae, Legionella non-pneumophila, Bacillus anthracosis (Anthrax), Moraxella cararrhalis, Neisseria elongate and miningitidis, Pseudomonas aeruginosa, Staphylococcus epidermis, Staphylococcus salivarius, Leptospirosis, Chlamydia psittaci, Coxiella burneti (Q‐Fever), Streptococcus aureus)との交差性を、M領域及びS領域のそれぞれについて、in silicoにて確認した。

結果、プローブ設計領域のミスマッチが5塩基以内かつF1,F2,B1,B2プライマー設計領域のミスマッチがそれぞれ3塩基以内かつそれらがLAMP反応する位置となっていたのは、SARS-Coronavirus(SARSウイルス)のM領域のみであった。そこで、SARSウイルスとの交差反応性を確認した。
SARSウイルスの鋳型は、領域別に相同性の高いSARSウイルス(GenBank NC004718.3)の配列を組み込んだ人工遺伝子の転写RNAを用いた(合成依頼先:Eurofins社)。鋳型量は1.0×107コピー/テストとした。
両検出系でSARSウイルスとの交差反応は見られなかった。また、SARS-CoV-2とSARSウイルスに共感染した場合を想定し、SARSウイルスの転写RNA 1.0×106コピー/テストとSARS-CoV-2の転写RNA 25又は1000コピー/テストを混合した試験も行った。
混合試験においても、SARS-CoV-2の検出が可能であった。
Claims (15)
- SARS-CoV-2ウイルスをLAMP法により増幅して検出するためのプライマーセットであって、前記プライマーセットは、以下の(a)~(b)の領域から設計される少なくとも一つからなるプライマーセット。
(a)配列番号1の28976塩基から29211塩基までの領域
(b)配列番号1の15394塩基から15595塩基までの領域 - SARS-CoV-2ウイルスをLAMP法により増幅して検出するためのプライマーセットであって、前記プライマーセットは、以下の(a1)~(b1)の群から選択される少なくとも一つを含むプライマーセット。
(a1)配列番号2からなるF3プライマー、配列番号3からなるB3プライマー、配列番号4からなるFIPプライマー、及び配列番号5からなるBIPプライマーからなるプライマーセット。
(b1)配列番号8からなるF3プライマー、配列番号9からなるB3プライマー、配列番号10からなるFIPプライマー、及び配列番号11からなるBIPプライマーからなるプライマーセット。 - SARS-CoV-2ウイルスをLAMP法により増幅して検出するためのプライマーセットであって、前記プライマーセットは、以下の(a2)~(b2)の群から選択される少なくとも一つを含むプライマーセット。
(a2)請求項2に記載の(a1)のプライマーセットに加えて、配列番号6からなるLFプライマー、配列番号7からなるLBプライマー。
(b2)請求項2に記載の(b1)のプライマーセットに加えて、配列番号12からなるLFプライマー、配列番号13からなるLBプライマー。 - 請求項1~3のいずれか1項に記載のプライマーセットを用いてLAMP法により増幅反応を行うことを特徴とするSARS-CoV-2ウイルスの検出方法。
- 請求項1~3のいずれか1項に記載のプライマーセットを用いて、SARS-CoV-2ウイルスの標的核酸領域の増幅を検出することにより、SARS-CoV-2ウイルス感染の有無を検査することを特徴とするCOVID-19の検査方法。
- 請求項1~3のいずれか1項に記載のプライマーセットを含むことを特徴とするCOVID-19を検査するためのキット。
- SARS-CoV-2ウイルスをLAMP法により増幅して検出するためのプライマーセットであって、前記プライマーセットは、以下の(c)~(d)の領域から設計される少なくとも一つからなるプライマーセット。
(c)配列番号1の26767塩基から26977塩基までの領域
(d)配列番号1の24660塩基から24916塩基までの領域 - SARS-CoV-2ウイルスをLAMP法により増幅して検出するためのプライマーセットであって、前記プライマーセットは、以下の(c1)~(d1)の群から選択される少なくとも一つを含むプライマーセット。
(c1)配列番号14からなるF3プライマー、配列番号15からなるB3プライマー、配列番号16からなるFIPプライマー、及び配列番号17からなるBIPプライマーからなるプライマーセット。
(d1)配列番号21からなるF3プライマー、配列番号22からなるB3プライマー、配列番号23からなるB4プライマー、配列番号24からなるFIPプライマー、及び配列番号25からなるBIPプライマーからなるプライマーセット。 - SARS-CoV-2ウイルスをLAMP法により増幅して検出するためのプライマーセットであって、前記プライマーセットは、以下の(c2)~(d2)の群から選択される少なくとも一つを含むプライマーセット。
(c2)配列番号14からなるF3プライマー、配列番号15からなるB3プライマー、配列番号16からなるFIPプライマー、配列番号17からなるBIPプライマー、配列番号18からなるLFプライマー、及び配列番号19からなるLBプライマーからなるプライマーセット。
(d2)配列番号21からなるF3プライマー、配列番号22からなるB3プライマー、配列番号23からなるB4プライマー、配列番号24からなるFIPプライマー、配列番号25からなるBIPプライマー、配列番号26からなるLFプライマー、及び配列番号27からなるLBプライマーからなるプライマーセット。 - 請求項7~9のいずれか一項に記載のプライマーセット及び蛍光標識プローブを含む、SARS-CoV-2ウイルスをLAMP法により増幅して検出するためのキット。
- プライマーセットが(c1)又は(c2)のプライマーセットであり、蛍光標識プローブが配列番号20からなるプローブである、請求項10に記載のキット。
- プライマーセットが(d1)又は(d2)のプライマーセットであり、蛍光標識プローブが配列番号28からなるプローブである、請求項10に記載のキット。
- COVID-19を検査するための、請求項10~12のいずれか一項に記載のキット。
- 請求項7~9のいずれか一項に記載のプライマーセット又は請求項10~13のいずれか一項に記載のキットを用いて、LAMP法により増幅反応を行うことを特徴とするSARS-CoV-2ウイルスの検出方法。
- 請求項7~9のいずれか一項に記載のプライマーセット又は請求項10~13のいずれか一項に記載のキットを用いて、SARS-CoV-2ウイルスの標的核酸領域の増幅を検出することにより、SARS-CoV-2ウイルス感染の有無を検査することを特徴とするCOVID-19の検査方法。
