本発明のエマルション組成物は、有効成分として局所麻酔薬を含有する。
局所麻酔薬としては、アミド型の局所麻酔薬又はエステル型の局所麻酔薬等が挙げられるが、アミド型の局所麻酔薬が好ましく、例えば、下記一般式(I)
本発明における一般式(I)で表される化合物が不斉炭素原子を1個有する場合、絶対配置R又はSで表される純粋な光学活性体であってもよく、その任意な割合の混合物であってもよく、ラセミ体であってもよい。
本発明の局所麻酔薬としては、水溶性の塩であることが好ましい。
上記例示された局所麻酔薬は原薬として市販されているものを用いてもよく、また、それが含まれている製剤をそのまま若しくは希釈、抽出等加工して用いてもよい。
局所麻酔薬の含有量は特に限定されないが、組成物全体に対して通常0.01~5質量%であり、好ましくは0.01~3質量%であり、特に好ましくは0.05~2質量%である。
本発明のエマルション組成物は、本発明の効果を損なわない限りその他の有効成分を含んでいてもよい。併用できる有効成分は局所麻酔薬と併用禁忌でない成分であればよい。
本発明のエマルション組成物は、炭素数6から12の脂肪酸がエステル結合している脂肪酸エステルを含有する。脂肪酸エステルは、例えば、グリセリンに炭素数6から12の脂肪酸がエステル結合しているグリセリド、又はプロピレングリコールに炭素数6から12の脂肪酸がエステル結合しているプロピレングリコール脂肪酸エステルであることが好ましい。
炭素数6から12の脂肪酸は、炭素数6、7、8、9、10、11若しくは12の脂肪酸(例えば、カプロン酸、エナント酸、カプリル酸、ペラルゴン酸、カプリン酸、又はラウリン酸)又はこれらの組み合わせであってよい。
グリセリドは、構成脂肪酸として、1以上の炭素数6から12の脂肪酸を任意の割合で含む、脂肪酸モノグリセリド、脂肪酸ジグリセリド若しくは脂肪酸トリグリセリドを単一又は複数種含む組み合わせであってもよく、中鎖脂肪酸トリグリセリド(以下、「MCT」という場合がある。好ましくは構成脂肪酸の炭素数が8及び10である。)が好ましい。グリセリドの含有量は、本発明の効果を妨げない限り特に限定されないが、組成物全体に対して通常0.01~20質量%であり、好ましくは0.5~15質量%であり、特に好ましくは1~10質量%である。
グリセリドは公知の方法で製造することができ、また中鎖脂肪酸トリグリセリドとしてココナード(登録商標)MT(花王株式会社製;主な構成脂肪酸がカプリル酸及びカプリン酸であるトリグリセリド)、ココナードRK(花王株式会社製;主な構成脂肪酸がカプリル酸であるトリグリセリド)、Captex1000(Abitec社製;主な構成脂肪酸がカプリン酸であるトリグリセリド)、サンファットGDC-S(太陽化学株式会社製;主な構成脂肪酸がカプリル酸であるジグリセリド)又はパナセート(登録商標、日油株式会社製)等の市販品を用いてもよい。
構成脂肪酸として、飽和脂肪酸であってもよく、不飽和脂肪酸であってもよいが、飽和脂肪酸であることが好ましい。
プロピレングリコール脂肪酸エステルは、構成脂肪酸として、1以上の炭素数6から12の脂肪酸を任意の割合で含む、プロピレングリコール脂肪酸モノエステル若しくプロピレングリコール脂肪酸ジエステルを単一又は複数種含む組み合わせであってもよい。好ましくは構成脂肪酸の炭素数が8及び10である。プロピレングリコール脂肪酸エステルの含有量は、本発明の効果を妨げない限り特に限定されないが、組成物全体に対して通常0.01~20質量%であり、好ましくは0.5~15質量%であり、特に好ましくは1~10質量%である。
プロピレングリコール脂肪酸エステルは公知の方法で製造することができ、Captex100(Abitec社製;主な構成脂肪酸がカプリン酸であるプロピレングリコールジエステル)等の市販品を用いてもよい。
プロピレングリコール脂肪酸エステルは、その分子内にエステル結合を有する点でグリセリドと共通している。
構成脂肪酸として、飽和脂肪酸であってもよく、不飽和脂肪酸であってもよいが、飽和脂肪酸であることが好ましい。
本発明のエマルション組成物のpHはpH7.0未満の酸性領域に調整され、好ましくはpHが3以上7.0未満、より好ましくは3以上6.5以下、更に好ましくは3以上6.0以下に調整される。
本発明のエマルション組成物のpHを上記領域に調整することで、油相中に局所麻酔薬が含まれにくくなり、及び/又は水相中に局所麻酔薬が含まれやすくなる。また、本発明のエマルション組成物のpHを上記領域に調整することで、(1)投与後、徐放化することなく速効性を有し、且つ長期間薬効が持続し、(2)保存安定性を有し、及び/又は(3)高い安全性を有するという本発明の効果を奏し、及び/又は向上し得る。
本発明のエマルション組成物として用いる乳化剤は特に限定されないが、例えば、デオキシコール酸類、ポリオキシエチレンヒマシ油、ポリオキシエチレン硬化ヒマシ油類、ポリオキシエチレンポリオキシプロピレングリコール、ポリオキシエチレンソルビタンモノオレエート(ポリソルベート80)、モノラウリン酸ポリオキシエチレンソルビタン、ポリオキシエチレンヒドロキシステアレート、ポビドン類、レシチン(卵黄レシチン、大豆レシチン)等が挙げられ、レシチンが好ましく、卵黄レシチンがより好ましい。乳化剤の含有量は、エマルション組成物全体に対して、0.1~3.0質量%が好ましく、0.5~2.0質量%がより好ましく、1.0~1.5質量%が特に好ましい。
本発明のエマルション組成物は、さらに安定化剤を含んでもよく、含有しなくてもよい。安定化剤としては、特に限定されないが、例えばEDTA、オレイン酸Naなどの脂肪酸、コレステロール、カゼイン、カゼインナトリウム塩等が挙げられる。EDTAは、エマルション組成物の乳化安定性乃至外観を向上するという効果があるため好ましい。ここで、外観を向上するとは、例えば、経時によるエマルション組成物の色の変化(例えば赤色への変化)を抑制すること等をいう。EDTAとしては、EDTA-2Na、EDTA-4Na、EDTA・Ca・2Na、又はこれらの水和物等が含まれる。EDTAの含有量は、エマルション組成物全体に対して、遊離のEDTAとして0.001~0.1質量%が好ましく、0.001~0.05質量%がより好ましく、0.01~0.05質量%が特に好ましい。
EDTAは公知の方法で製造することができ、またクレワット(登録商標)N(EDTA・2Na・2H2O、ナガセケムテックス株式会社)等の市販品を用いてもよい。
本発明のエマルション組成物は、さらに長鎖脂肪酸グリセリドを含有していてもよく、含有しなくてもよい。長鎖脂肪酸グリセリドを構成する炭素数は、例えば約14~20程度等であってもよい。長鎖脂肪酸グリセリドの含有量は、本発明の効果を妨げない限り特に限定されないが、組成物全体に対して通常2~20質量%であり、好ましくは3~15質量%であり、特に好ましくは3~10質量%である。
長鎖脂肪酸グリセリドは公知の方法で製造することができ、また精製大豆油等の市販品を用いてもよい。
本発明のエマルション組成物は、さらに溶剤を含有していてもよい。溶剤としては、特に限定されないが、例えば、精製水、蒸留水、注射用水、グリセリン、エタノール、プロピレングリコール、ポリエチレングリコール、マクロゴール、食用油(ゴマ油、トウモロコシ油、オリーブ油等)等が挙げられ、中でも精製水、蒸留水、注射用水等が好ましい。
溶剤の含有量は、本発明の効果を妨げない限り特に限定されないが、組成物全体に対して通常80~99.9質量%であり、好ましくは80~98質量%であり、特に好ましくは80~95質量%である。
本発明のエマルション組成物は、さらに等張化剤を含有していてもよい。特に限定されないが、例えば、ブドウ糖、D-ソルビトール、塩化ナトリウム、D-マンニトール、グリセリン等が挙げられ、グリセリンが好ましい。グリセリンの含有量は、エマルション組成物全体に対して等張にする量を計算により求めることができる。
本発明のエマルション組成物は、所望により、添加剤が好適に配合される。添加剤としては、例えば、溶解補助剤、酸化防止剤等が挙げられる。
本発明のエマルション組成物は、他の薬効成分を含んでもよい。麻酔効果の減弱化や製剤的な不安定化などの負の影響を与えるものでなければ、特に限定を受けないが、例えば、デキサメタゾン、そのエステル又はその塩(好ましくは、デキサメタゾンリン酸エステルナトリウム)、アドレナリン、デクスメデトミジン又はその塩(好ましくは塩酸デクスメデトミジン)などを含有していてもよい。これによりさらに薬効持続時間が延長したエマルション組成物を提供することが可能となる。
本発明のエマルション組成物は、局所麻酔薬、乳化剤、中鎖脂肪酸トリグリセリド又はプロピレングリコール脂肪酸エステル、水(例えば、蒸留水、精製水、又は注射用水等)に加えて、長鎖脂肪酸トリグリセリド、グリセリン及びEDTAからなる群より選択される1以上を含むことが好ましい実施態様の1つである。
本発明のエマルション組成物は、例えば、局所麻酔薬、中鎖脂肪酸トリグリセリド又はプロピレングリコール脂肪酸エステル並びに各成分を注射用水に任意の順に加えて加温攪拌後、ホモジナイジング処理することにより製造することができる。なお、たとえば、撹拌の際の温度は50~80℃である。すなわち、本発明のエマルション組成物は公知の方法又はそれに準じる方法により製造され得る。
本発明のエマルション組成物は、W/O型、O/W型のいずれであってもよいが、O/W型の方が好ましい。エマルション組成物に含まれる局所麻酔薬全体に対する、エマルション組成物の水相中に存在する局所麻酔薬の比率は通常10%以上、好ましくは15%以上、より好ましくは30%以上である。エマルション組成物に含まれる局所麻酔薬全体に対する、エマルション組成物の水相中に存在する局所麻酔薬の比率が上記範囲内であることで、投与後、(1)徐放化することなく速効性を有し、且つ長期間薬効が持続し、(2)保存安定性を有し、及び/又は(3)高い安全性を有するという本発明の効果を向上し得る。
本発明のエマルション組成物は、例えば、25℃、相対湿度60%保存下で6ヶ月以上局所麻酔薬の含量が初期値に対して90%以上維持され、製造時並びに保存中に析出物を認めないことが好ましい。本発明の組成物は保存安定性に優れている。
本発明のエマルション組成物は、例えば、ヒトや他の哺乳動物(例えば、ラット、マウス、モルモット、ウサギ、ヒツジ、ブタ、ウシ、ネコ、イヌ、サルなど)の疼痛制御に用いることができる。
本発明のエマルション組成物は、安全で低毒性であるので、例えば、ヒトや他の哺乳動物に対して十分量を投与することができる。
投与量は、局所麻酔薬の種類、投与対象、対象臓器、症状、投与方法等によっても異なるが、例えば注射剤では、通常例えば体重約60kgのヒトにおいては、1日当たり薬効成分に換算して約0.01~1000mg程度、好ましくは約0.01~500mg程度を公知の投与方法により投与するのが好都合である。1日当たりの総投与量は、単一投与量であっても分割投与量であってもよい。また、本発明のエマルション組成物を、経皮、皮下、皮内、筋肉内、神経近傍、歯髄内、脊髄内、硬膜外内、静脈内、又は点眼等の経粘膜から、全身又は局所へ投与できる。その投与量は当業者であれば局所麻酔薬の治療効果を得るため、及び/又は最大化させるために変えられる。また、目的とする疼痛制御を達成するために伝達麻酔、浸潤麻酔又は表面麻酔などの種々の麻酔法を適切な投与量により採用できる。伝達麻酔とは、例えば、末梢神経又はその周辺に麻酔薬を直接注射して、神経の刺激伝達を遮断する方法等をいう。浸潤麻酔とは、例えば、所望の組織(手術部位の周囲の皮内又は皮下等)に麻酔薬を直接注射すること等をいう。表面麻酔とは、例えば、皮膚又は粘膜などの表面にある神経の刺激伝達を遮断するための麻酔法等をいい、塗布、噴霧、又は表面冷凍等することにより行われる。
本発明のエマルション組成物は、特に局所麻酔薬として優れている。
本発明のエマルション組成物は、速効性を示し得るが、徐放性を示さない。
具体的には投与後、徐放化により緩やかな薬効発現並びに長期間血中濃度が維持される徐放性組成物とは異なり、市販されている水溶液組成物と同様の速効性をもって薬効を発現し、血中からの消失もそれと同様若しくは安全性の観点からより優れた挙動を示す。
本発明のエマルション組成物は、薬効持続時間が延長され得る。
製造例
後掲の表1~2の組成(質量比)に従い各サンプルを作製した。具体的には、調製量300gの場合、500mLビーカーに各成分を量り採り、液温を50℃にしてホモミクサー(クレアミックス(登録商標)、エム・テクニック(株))で12000rpm、10分間粗乳化した。更に、高圧ホモジナイザー(Beryu、(株)美粒)を用い、圧力0.5~0.7Mpaで5回以上処理して乳化物を得た。なお比較例1、比較例2はレボブピバカイン塩酸塩を生理食塩液に溶解して用いた。
毛刈りしたモルモット(Hartley系、雄性、5週齢、日本エスエルシー株式会社)の背部に、実施例1及び比較例1の各サンプルをそれぞれ0.1mL皮内投与し、ピンプリック法即ち、シリンジ器具の針加圧(6回)によるモルモットの反応回数を観察し、局所麻酔効果として知覚神経の遮断時間を評価した(各群n=6)。各サンプル皮内投与後420分までの局所麻酔効果の結果を図1に示した。
図1の結果より、溶媒が生理食塩液である比較例1に対してMCTを含む脂肪乳剤である実施例1において局所麻酔効果の時間が延長されることが示された。
サンプルを実施例2~3及び比較例2とした以外は試験例1と同様にして、知覚神経の遮断時間を評価した(各群n=6)。各サンプル皮内投与後540分までの局所麻酔効果の結果を図2に示した。
図2の実施例2~3及び比較例2の結果より、レボブピバカイン塩酸塩はMCTの添加量に応じて、MCTの添加量が多いほど局所麻酔効果の時間が延長されることが示された。
0.05質量%レボブピバカイン塩酸塩水溶液5gに油相(大豆油:中鎖脂肪酸トリグリセリド=1:1)5gを加え、ボルテックスミキサーを用いて1分間撹拌した。その後、全量を遠沈管に移し、25℃で3600rpm、10分間遠心し、水相を分離した。得られた水相のレボブピバカイン濃度をHPLC法にて測定し、油水分配比率を求めた。その結果、レボブピバカイン塩酸塩は、その94.3%が水相に存在することがわかった。
Single-Use RED Plate, with Inserts(サーモフィッシャーサイエンティフィック株式会社)のバッファーチャンバーにグリセリン水溶液、透析膜チャンバーに実施例4及び比較例2の各サンプルを入れ、ユニットを接着フィルムで覆った。その後、25℃、100rpmで楕円振とうさせた。グリセリン水溶液を経時的にサンプリングし、レボブピバカイン濃度をHPLC法にて測定することにより徐放性を評価した。経時的に測定したグリセリン水溶液抽出率(%)の結果を図3に示した。
図3の実施例4及び比較例2の結果より、MCTの有無は、本発明の組成物の徐放性に影響を及ぼさないことが示された。すなわち、本発明の組成物は、公知技術とは異なり、MCTを含有するにもかかわらず、有効成分が速やかに放出され、さらに、試験例1~2の結果の通り、薬効持続時間が延長されるという驚くべき効果を奏することが示された。
なお、実施例2及び4は、同程度の速効性及び比較例2に対する同程度の薬効持続時間の延長が確認された(データ示さず)。このことより、EDTA・2Na・2水和物の有無が薬効持続時間及び徐放性に与える影響はほとんどないものと考えられる。
ラット(SD系、雄性、7週齢、日本エスエルシー株式会社)の右側の坐骨神経周囲に実施例4、並びに比較例2及び3の各サンプルを0.1mL投与し、経時的に頸静脈より採血した(各群n=3~4)。特許第5723567号に記載の方法に従ってレボブピバカインの血中濃度を測定した。また、麻酔効果の評価として、経時的に両側の後肢中指をピンセットで挟み忌避反応の有無を確認することにより知覚神経の遮断時間を測定した。サンプル投与後360分間の血中濃度推移の結果を図4に、知覚神経遮断効果の持続時間の結果を図5にそれぞれ示した。
血中濃度の過度な増大は副作用(例えば、循環抑制、痙攣等)につながるところ、図4より、水溶性組成物(比較例2)では急激な血中濃度の上昇が示されたが、本発明のエマルション組成物(実施例4)は、水溶性組成物(比較例2)よりもCmax(最高血中濃度)は低く、より安全性が高いことが示された。また両者の血中からの消失は同様の傾向を示した。
一方、脂溶性組成物(比較例3)は、エマルション組成物(実施例4)及び水溶性組成物(比較例2)と比較して、血中濃度の上昇は緩やかであり、徐放性が示された。
図5より、本発明のエマルション組成物(実施例4)の薬効発現時間は、脂溶性組成物(比較例3)で遅延したのに対して遅延することなく、水溶性組成物(比較例2)と同様に速やかな薬効発現を示した。また、図5より、本発明のエマルション組成物(実施例4)では、水溶性組成物(比較例2)と比較して薬効持続時間が延長された。
また、脂溶性組成物(比較例3)では、200分以後知覚神経遮断効果を示さないにもかかわらず(図5)、血中濃度が他のサンプルと比較して高く(図4)、薬効を示さなくなってからも比較的高濃度で血中に存在していた。一方、本発明のエマルション組成物(実施例4)は、水溶性組成物(比較例2)と同様に、知覚神経遮断効果を示さなくなってから血中濃度の減少が見られ、より安全な血中濃度の推移が確認された。
以上より、本発明の組成物は、有効成分が速やかに局所麻酔作用を発現し、さらに、薬効持続時間が延長され、尚且つ血中動態は急激な血中濃度の上昇を伴わず水溶液組成物(比較例2)よりも安全な血中濃度の推移を示すという、驚くべき効果を奏することが示された。
実施例1と比較例4の25℃、相対湿度60%での保存安定性を確認した。表3より、実施例1では6ヵ月保存後も残存率は98%以上を維持したが、初期pHが約7.7を示した比較例4では経時的に残存率は低下し6か月後には約85%まで低下した。
実施例2を基本処方として、さらにEDTA・2Na水和物を0.0055質量%、0.016質量%、若しくは0.03質量%又はEDTA・Ca・2Naを0.03質量%含有し、その分の蒸留水の量を減らしたサンプルを作製し、滅菌処理(121℃、20分)後、一定温度条件下で6ヶ月間保存し、定期的に外観を目視で確認することにより安定性を確認した。
(1)基本処方のサンプルの結果
25℃で3ヶ月間、外観に変化がなく、長期保存安定性を有することが確認された。
(2)EDTA・2Na・2水和物0.0055質量%又は0.016質量%含有サンプルの結果
25℃で4ヶ月間、外観に変化がなく、EDTA添加によりさらに長期間の保存安定性が得られることが確認された。
(3)EDTA・2Na・2水和物0.03質量%(実施例4)又はEDTA・Ca・2Na・2水和物0.03質量%(実施例5)含有サンプルの結果
5℃、25℃又は40℃で6ヶ月間、外観に変化がなく、高濃度のEDTAによりさらに長期間の保存安定性が得られることが確認された。また、EDTAの添加により、外観の色調の変化も抑制されることが確認された(表4参照)。
製造例2
後掲の表5の組成(質量比)に従い各サンプルを作製した。具体的には、調製量300gの場合、500mLビーカーに各成分を量り採り、液温を50℃にしてホモミクサー(クレアミックス(登録商標)、エム・テクニック(株))で12000rpm、10分間粗乳化した。更に、高圧ホモジナイザー(Beryu、(株)美粒)を用い、圧力0.5~0.7Mpaで5回以上処理して乳化物を得た。また、比較例として、前記表1に記載の比較例2を製造し、使用した。
ラット(SD系、雄性、7週齢、日本エスエルシー株式会社)の右側の坐骨神経周囲に実施例6~8並びに比較例2の各サンプルを0.1mL投与した(各群n=6)。麻酔効果の評価として、経時的に両側の後肢中指をピンセットで挟み忌避反応の有無を確認することにより知覚神経の遮断時間を測定した。サンプル投与後の知覚神経遮断効果の持続時間の結果を表6及び図6に示した。
図6より、本発明のエマルション組成物(実施例6~8)の薬効発現時間は、水溶性組成物(比較例2)と同様に速やかな薬効発現を示した。また、図6より、本発明のエマルション組成物(実施例6~8)では、水溶性組成物(比較例2)と比較して薬効持続時間が延長された。
以上より、本発明の組成物は、有効成分が速やかに局所麻酔作用を発現し、さらに、薬効持続時間が延長されるという、驚くべき効果を奏することが示された。
なお、上記実施例の25℃、及び40℃で75%RH下、2ヶ月以上のpH、残存率(%)、粒子径(nm)及び外観の保存安定性も確認されている(データ示さず)。
Claims (16)
- 局所麻酔薬、及び炭素数6から12の脂肪酸がエステル結合している脂肪酸エステルを含有する酸性エマルション組成物。
- 脂肪酸エステルが、グリセリドである請求項1に記載の酸性エマルション組成物。
- 局所麻酔薬が、アミド型の局所麻酔薬である請求項1又は2に記載の酸性エマルション組成物。
- 局所麻酔薬が、レボブピバカイン、ブピバカイン、ロピバカイン及びそれらの塩からなる群より選択される1以上である請求項1~3のいずれかに記載の酸性エマルション組成物。
- グリセリドが、いずれも構成脂肪酸として炭素数6から12の脂肪酸を1以上含有する、脂肪酸モノグリセリド、脂肪酸ジグリセリド及び脂肪酸トリグリセリドからなる群より選択される1以上の脂肪酸グリセリドである請求項2~4のいずれかに記載の酸性エマルション組成物。
- エマルション組成物のpHが3以上7.0未満である請求項1~5のいずれかに記載の酸性エマルション組成物。
- エマルション組成物のpHが3以上6.5以下である請求項1~5のいずれかに記載の酸性エマルション組成物。
- エマルション組成物のpHが3以上6.0以下である請求項1~5のいずれかに記載の酸性エマルション組成物。
- さらにEDTAを含有する請求項1~8のいずれかに記載の酸性エマルション組成物。
- さらに長鎖脂肪酸グリセリドを含有する請求項1~9のいずれかに記載の酸性エマルション組成物。
- エマルションがO/W型である請求項1~10のいずれかに記載の酸性エマルション組成物。
- 水相中に存在する局所麻酔薬の比率が組成物に含まれる局所麻酔薬総量の15%以上である請求項11に記載の酸性エマルション組成物。
- 脂肪酸エステルが、プロピレングリコール脂肪酸エステルである請求項1、3、4、及び6~12のいずれかに記載の酸性エマルション組成物。
- 哺乳動物の疼痛制御に用いられる請求項1~13のいずれかに記載の酸性エマルション組成物。
- 疼痛制御のための投与が、経皮、皮下、皮内、筋肉内、神経近傍、歯髄内、脊髄内、硬膜外内、静脈内、又は点眼等の経粘膜から、全身又は局所への投与である請求項14に記載の組成物。
- 疼痛制御のための麻酔が、伝達麻酔、浸潤麻酔又は表面麻酔である請求項14に記載の組成物。
