[1] レンバチニブまたはその薬剤学的に許容される塩と組み合わせて投与されることを特徴とする、(6S,9aS)-N-ベンジル-8-({6-[3-(4-エチルピペラジン-1-イル)アゼチジン-1-イル]ピリジン-2-イル}メチル)-6-(2-フルオロ-4-ヒドロキシベンジル)-4,7-ジオキソ-2-(プロプ-2-エン-1-イル)ヘキサヒドロ-2H-ピラジノ[2,1-c][1,2,4]トリアジン-1(6H)-カルボキサミドを含有する医薬組成物。
[2] (6S,9aS)-N-ベンジル-8-({6-[3-(4-エチルピペラジン-1-イル)アゼチジン-1-イル]ピリジン-2-イル}メチル)-6-(2-フルオロ-4-ヒドロキシベンジル)-4,7-ジオキソ-2-(プロプ-2-エン-1-イル)ヘキサヒドロ-2H-ピラジノ[2,1-c][1,2,4]トリアジン-1(6H)-カルボキサミドと組み合わせて投与されることを特徴とする、レンバチニブまたはその薬剤学的に許容される塩を含有する医薬組成物。

[8] レンバチニブまたはその薬剤学的に許容される塩と組み合わせて投与されることを特徴とする腫瘍治療用医薬組成物を製造するための、(6S,9aS)-N-ベンジル-8-({6-[3-(4-エチルピペラジン-1-イル)アゼチジン-1-イル]ピリジン-2-イル}メチル)-6-(2-フルオロ-4-ヒドロキシベンジル)-4,7-ジオキソ-2-(プロプ-2-エン-1-イル)ヘキサヒドロ-2H-ピラジノ[2,1-c][1,2,4]トリアジン-1(6H)-カルボキサミドの使用。
[9] (6S,9aS)-N-ベンジル-8-({6-[3-(4-エチルピペラジン-1-イル)アゼチジン-1-イル]ピリジン-2-イル}メチル)-6-(2-フルオロ-4-ヒドロキシベンジル)-4,7-ジオキソ-2-(プロプ-2-エン-1-イル)ヘキサヒドロ-2H-ピラジノ[2,1-c][1,2,4]トリアジン-1(6H)-カルボキサミドと組み合わせて投与されることを特徴とする腫瘍治療用医薬組成物を製造するための、レンバチニブまたはその薬剤学的に許容される塩の使用。
[10] レンバチニブまたはその薬剤学的に許容される塩と組み合わせて投与されることを特徴とする、腫瘍を治療するための(6S,9aS)-N-ベンジル-8-({6-[3-(4-エチルピペラジン-1-イル)アゼチジン-1-イル]ピリジン-2-イル}メチル)-6-(2-フルオロ-4-ヒドロキシベンジル)-4,7-ジオキソ-2-(プロプ-2-エン-1-イル)ヘキサヒドロ-2H-ピラジノ[2,1-c][1,2,4]トリアジン-1(6H)-カルボキサミド。
[11] (6S,9aS)-N-ベンジル-8-({6-[3-(4-エチルピペラジン-1-イル)アゼチジン-1-イル]ピリジン-2-イル}メチル)-6-(2-フルオロ-4-ヒドロキシベンジル)-4,7-ジオキソ-2-(プロプ-2-エン-1-イル)ヘキサヒドロ-2H-ピラジノ[2,1-c][1,2,4]トリアジン-1(6H)-カルボキサミドと組み合わせて投与されることを特徴とする、腫瘍を治療するためのレンバチニブまたはその薬剤学的に許容される塩。

[15] レンバチニブまたはその薬剤学的に許容される塩、および(6S,9aS)-N-ベンジル-8-({6-[3-(4-エチルピペラジン-1-イル)アゼチジン-1-イル]ピリジン-2-イル}メチル)-6-(2-フルオロ-4-ヒドロキシベンジル)-4,7-ジオキソ-2-(プロプ-2-エン-1-イル)ヘキサヒドロ-2H-ピラジノ[2,1-c][1,2,4]トリアジン-1(6H)-カルボキサミドを含有する腫瘍治療用医薬組成物。
[16] レンバチニブまたはその薬剤学的に許容される塩、および(6S,9aS)-N-ベンジル-8-({6-[3-(4-エチルピペラジン-1-イル)アゼチジン-1-イル]ピリジン-2-イル}メチル)-6-(2-フルオロ-4-ヒドロキシベンジル)-4,7-ジオキソ-2-(プロプ-2-エン-1-イル)ヘキサヒドロ-2H-ピラジノ[2,1-c][1,2,4]トリアジン-1(6H)-カルボキサミドを含有する腫瘍治療剤。
[17] 腫瘍の治療に使用される、レンバチニブまたはその薬剤学的に許容される塩、および(6S,9aS)-N-ベンジル-8-({6-[3-(4-エチルピペラジン-1-イル)アゼチジン-1-イル]ピリジン-2-イル}メチル)-6-(2-フルオロ-4-ヒドロキシベンジル)-4,7-ジオキソ-2-(プロプ-2-エン-1-イル)ヘキサヒドロ-2H-ピラジノ[2,1-c][1,2,4]トリアジン-1(6H)-カルボキサミド。
[18] レンバチニブまたはその薬剤学的に許容される塩と組み合わせて、腫瘍の治療に使用される、(6S,9aS)-N-ベンジル-8-({6-[3-(4-エチルピペラジン-1-イル)アゼチジン-1-イル]ピリジン-2-イル}メチル)-6-(2-フルオロ-4-ヒドロキシベンジル)-4,7-ジオキソ-2-(プロプ-2-エン-1-イル)ヘキサヒドロ-2H-ピラジノ[2,1-c][1,2,4]トリアジン-1(6H)-カルボキサミド。
[19] (6S,9aS)-N-ベンジル-8-({6-[3-(4-エチルピペラジン-1-イル)アゼチジン-1-イル]ピリジン-2-イル}メチル)-6-(2-フルオロ-4-ヒドロキシベンジル)-4,7-ジオキソ-2-(プロプ-2-エン-1-イル)ヘキサヒドロ-2H-ピラジノ[2,1-c][1,2,4]トリアジン-1(6H)-カルボキサミドと組み合わせて、腫瘍の治療に使用される、レンバチニブまたはその薬剤学的に許容される塩。
[20] 腫瘍治療用医薬組成物を製造するためのレンバチニブまたはその薬剤学的に許容される塩、および(6S,9aS)-N-ベンジル-8-({6-[3-(4-エチルピペラジン-1-イル)アゼチジン-1-イル]ピリジン-2-イル}メチル)-6-(2-フルオロ-4-ヒドロキシベンジル)-4,7-ジオキソ-2-(プロプ-2-エン-1-イル)ヘキサヒドロ-2H-ピラジノ[2,1-c][1,2,4]トリアジン-1(6H)-カルボキサミドの使用。
[21] さらに賦形剤を含有する、上記医薬組成物または腫瘍治療剤。
[22] レンバチニブまたはその薬剤学的に許容される塩が、レンバチニブメシル酸塩である、上記医薬組成物、腫瘍治療剤、治療方法、使用、化合物またはキット。
[23] レンバチニブまたはその薬剤学的に許容される塩、および(6S,9aS)-N-ベンジル-8-({6-[3-(4-エチルピペラジン-1-イル)アゼチジン-1-イル]ピリジン-2-イル}メチル)-6-(2-フルオロ-4-ヒドロキシベンジル)-4,7-ジオキソ-2-(プロプ-2-エン-1-イル)ヘキサヒドロ-2H-ピラジノ[2,1-c][1,2,4]トリアジン-1(6H)-カルボキサミドが、それぞれ同時に、別々に、連続して、または時間差をおいて投与される、上記医薬組成物、腫瘍治療剤、治療方法、使用、化合物またはキット。
[24] 腫瘍が乳癌、甲状腺癌、肝細胞癌、大腸癌、腎細胞癌、頭頸部癌、子宮内膜癌またはメラノーマである、上記医薬組成物、腫瘍治療剤、治療方法、使用、化合物またはキット。
[25] 腫瘍が乳癌である、[24]に記載の医薬組成物、腫瘍治療剤、治療方法、使用、化合物またはキット。
[26] 腫瘍が甲状腺癌である、[24]に記載の医薬組成物、腫瘍治療剤、治療方法、使用、化合物またはキット。
[27] 甲状腺癌が甲状腺未分化癌である、[26]に記載の医薬組成物、腫瘍治療剤、治療方法、使用、化合物またはキット。

乳腺上皮細胞に局所的にWnt-1を発現させたトランスジェニックマウス(MMTV-Wnt-1、Jackson Laboratories)の自然発症乳癌を採取し、背景系統となるマウス(C57BL/6J、日本チャールズリバー)にトラカールで移植継代した。移植継代した腫瘍が1.5g程度になった時点で摘出し、30mg程度の断片にし、対照群、E7386 12.5、25または50mg/kg単独投与群、レンバチニブメシル酸塩 10mg/kg単独投与群、ならびにE7386 12.5、25または50mg/kgおよびレンバチニブメシル酸塩 10mg/kgの併用投与群の各群5例のマウス(C57BL/6J)の体側皮下に移植した。腫瘍の生着を確認した後、E7386(12.5、25または50mg/kg、1日2回、14日間、経口投与)およびレンバチニブメシル酸塩(10mg/kg、1日1回、14日間、経口投与)を単独で、または併用して、それぞれ単独投与群または併用投与群に投与した。投与の際に、E7386を0.1mol/L塩酸に溶解し、また、レンバチニブメシル酸塩を3mmol/L塩酸に溶解した。対照群には、0.1mol/L塩酸(1日2回、14日間)を投与した。
投与開始日を1日とし、以下、4日、8日、11日、および15日に、各マウスに発生した腫瘍の長径および短径を、デジマチックキャリパ(Mitsutoyo)で測定した。
以下の式に従って、腫瘍体積および比腫瘍体積(RTV)を算出した。

マウス乳癌4T1細胞(ATCC)を、37℃の条件下で、5%炭酸ガスインキュベーター内で、10%のFBSを含むRPMI1640培地(SIGMA社)を使用して培養した。細胞が約80%コンフルエントな状態となった時に、トリプシン-EDTAを使用して、細胞を回収した。ハンクス緩衝塩類溶液を加え、1.0×107cells/mLとなるように懸濁液を調製した。得られた細胞懸濁液0.1mLを、対照群、E7386 25mg/kg単独投与群、レンバチニブメシル酸塩 10mg/kg単独投与群、ならびにE7386 25mg/kgおよびレンバチニブメシル酸塩 10mg/kgの併用投与群の各群5例のマウス(C57BL/6J、日本チャールズリバー)の右第3乳腺脂肪体に移植した。移植後9日目より、E7386(25mg/kg、1日2回、14日間、経口投与)およびレンバチニブメシル酸塩(10mg/kg、1日1回、14日間、経口投与)を単独で、または併用して、それぞれ単独投与群または併用投与群に投与した。投与の際に、E7386を0.1mol/L塩酸に溶解し、また、レンバチニブメシル酸塩を3mmol/L塩酸に溶解した。対照群には薬剤を投与しなかった。
投与開始日を1日とし、以下3日、6日、9日および14日に、各マウスに発生した腫瘍の長径および短径を、デジマチックキャリパ(Mitsutoyo)で測定した。

投与開始日から定期的に、各マウスに発生した腫瘍の長径および短径をデジマチックキャリパ(Mitsutoyo)で測定し、腫瘍体積および比腫瘍体積(RTV)を算出する。RTVの結果から、E7386およびレンバチニブメシル酸塩の併用による抗腫瘍効果を評価することができる。

14日間の投与後にマウスから腫瘍組織を採取して、腫瘍組織のホルマリン固定を行い、腫瘍組織をパラフィンに包埋した。その後、パラフィン包埋腫瘍組織を、4μm厚で薄切し、スライドガラスに載せ、キシレン/エタノールで脱パラフィン処理を行った。血管周皮細胞(pericyte)のマーカーであるα平滑筋アクチン(α-SMA)抗体(SIGMA社製)および血管内皮細胞のマーカーであるCD31抗体(Dianova社製)を用いて免疫染色を行った。
CD31/α-SMA共染色した10倍拡大免疫染色像(カラー写真では茶色(CD31)および赤色(α-SMA)で表示している)を図17に示す。図17の(a)、(b)、(c)および(d)はそれぞれ、対照群、E7386単独投与群、レンバチニブメシル酸塩単独投与群、ならびにE7386およびレンバチニブメシル酸塩の併用投与群の免疫染色像である。その結果、E7386およびレンバチニブメシル酸塩の併用は、対照群およびそれぞれを単独投与した場合と比較して、MVDが顕著に減っていることを観察した。
CD31/α-SMA共染色した200倍拡大免疫染色像(カラー写真では茶色(CD31)および赤色(α-SMA)で表示している)を図18に示す。図18の(a)、(b)、(c)および(d)はそれぞれ、対照群、E7386単独投与群、レンバチニブメシル酸塩単独投与群、ならびにE7386およびレンバチニブメシル酸塩の併用投与群の免疫染色像である。黒矢印で示しているのは、CD31で染色された微小血管である。白抜き矢印で示しているのは、CD31/α-SMA両陽性血管である。その結果、E7386およびレンバチニブメシル酸塩の併用は、対照群およびそれぞれを単独投与した場合と比較して、PCIが顕著に減っていることを観察した。
上記実施例7に記載されているように、ヒト肝細胞癌株HepG2細胞(JCRB細胞バンク)を用いて調製した細胞懸濁液0.1mLを、対照群、E7386 50mg/kg単独投与群、レンバチニブメシル酸塩 10mg/kg単独投与群、ならびにE7386 50mg/kgおよびレンバチニブメシル酸塩 10mg/kgの併用投与群の各群5例のヌードマウス(CAnN.Cg-Foxn1nu/CrlCrlj、日本チャールズリバー)の体側皮下に移植した。移植後12日目より、E7386(50mg/kg、1日1回、14日間、経口投与)およびレンバチニブメシル酸塩(10mg/kg、1日1回、14日間、経口投与)を単独で、または併用して、それぞれ単独投与群または併用投与群に投与した。投与の際に、E7386を0.1mol/L塩酸に溶解し、また、レンバチニブメシル酸塩を3mmol/L塩酸に溶解した。対照群には薬剤を投与しなかった。
14日間の投与後にマウスから採取した腫瘍組織を分割し、血管解析用の腫瘍サンプルとした。分割した腫瘍組織は、ホルマリン固定を行い、パラフィンに包埋した。その後、パラフィン包埋腫瘍組織を、4μm厚で薄切し、スライドガラスに載せ、キシレン/エタノールで脱パラフィン処理を行った。血管周皮細胞(pericyte)のマーカーであるα平滑筋アクチン(α-SMA)抗体(SIGMA社製)および血管内皮細胞のマーカーであるCD31抗体(Dianova社製)を用いて免疫染色を行った。

- レンバチニブまたはその薬剤学的に許容される塩と組み合わせて投与されることを特徴とする、(6S,9aS)-N-ベンジル-8-({6-[3-(4-エチルピペラジン-1-イル)アゼチジン-1-イル]ピリジン-2-イル}メチル)-6-(2-フルオロ-4-ヒドロキシベンジル)-4,7-ジオキソ-2-(プロプ-2-エン-1-イル)ヘキサヒドロ-2H-ピラジノ[2,1-c][1,2,4]トリアジン-1(6H)-カルボキサミドを含有する腫瘍治療用医薬組成物。
- (6S,9aS)-N-ベンジル-8-({6-[3-(4-エチルピペラジン-1-イル)アゼチジン-1-イル]ピリジン-2-イル}メチル)-6-(2-フルオロ-4-ヒドロキシベンジル)-4,7-ジオキソ-2-(プロプ-2-エン-1-イル)ヘキサヒドロ-2H-ピラジノ[2,1-c][1,2,4]トリアジン-1(6H)-カルボキサミドと組み合わせて投与されることを特徴とする、レンバチニブまたはその薬剤学的に許容される塩を含有する腫瘍治療用医薬組成物。

- レンバチニブまたはその薬剤学的に許容される塩、および(6S,9aS)-N-ベンジル-8-({6-[3-(4-エチルピペラジン-1-イル)アゼチジン-1-イル]ピリジン-2-イル}メチル)-6-(2-フルオロ-4-ヒドロキシベンジル)-4,7-ジオキソ-2-(プロプ-2-エン-1-イル)ヘキサヒドロ-2H-ピラジノ[2,1-c][1,2,4]トリアジン-1(6H)-カルボキサミドが、それぞれ同時に、別々に、連続して、または時間差をおいて投与される、請求項1~4のいずれか1項に記載の医薬組成物。
- 腫瘍が乳癌、甲状腺癌、肝細胞癌、大腸癌、腎細胞癌、頭頸部癌、子宮内膜癌またはメラノーマである、請求項1~5のいずれか1項に記載の医薬組成物。
- 腫瘍が乳癌である、請求項6に記載の医薬組成物。
- 腫瘍が甲状腺癌である、請求項6に記載の医薬組成物。
- 甲状腺癌が甲状腺未分化癌である、請求項8に記載の医薬組成物。
- 腫瘍が肝細胞癌である、請求項6に記載の医薬組成物。
- 腫瘍が大腸癌である、請求項6に記載の医薬組成物。
- 腫瘍が腎細胞癌である、請求項6に記載の医薬組成物。
- 腫瘍が頭頸部癌である、請求項6に記載の医薬組成物。
- 腫瘍が子宮内膜癌である、請求項6に記載の医薬組成物。
- 腫瘍がメラノーマである、請求項6に記載の医薬組成物。
- レンバチニブまたはその薬剤学的に許容される塩と組み合わせて投与されることを特徴とする、(6S,9aS)-N-ベンジル-8-({6-[3-(4-エチルピペラジン-1-イル)アゼチジン-1-イル]ピリジン-2-イル}メチル)-6-(2-フルオロ-4-ヒドロキシベンジル)-4,7-ジオキソ-2-(プロプ-2-エン-1-イル)ヘキサヒドロ-2H-ピラジノ[2,1-c][1,2,4]トリアジン-1(6H)-カルボキサミドを含有する腫瘍治療剤。
- (6S,9aS)-N-ベンジル-8-({6-[3-(4-エチルピペラジン-1-イル)アゼチジン-1-イル]ピリジン-2-イル}メチル)-6-(2-フルオロ-4-ヒドロキシベンジル)-4,7-ジオキソ-2-(プロプ-2-エン-1-イル)ヘキサヒドロ-2H-ピラジノ[2,1-c][1,2,4]トリアジン-1(6H)-カルボキサミドと組み合わせて投与されることを特徴とする、レンバチニブまたはその薬剤学的に許容される塩を含有する腫瘍治療剤。
- レンバチニブまたはその薬剤学的に許容される塩、および(6S,9aS)-N-ベンジル-8-({6-[3-(4-エチルピペラジン-1-イル)アゼチジン-1-イル]ピリジン-2-イル}メチル)-6-(2-フルオロ-4-ヒドロキシベンジル)-4,7-ジオキソ-2-(プロプ-2-エン-1-イル)ヘキサヒドロ-2H-ピラジノ[2,1-c][1,2,4]トリアジン-1(6H)-カルボキサミドを、それらを必要とする患者に投与する腫瘍の治療方法。
- レンバチニブまたはその薬剤学的に許容される塩と組み合わせて投与されることを特徴とする腫瘍治療用医薬組成物を製造するための、(6S,9aS)-N-ベンジル-8-({6-[3-(4-エチルピペラジン-1-イル)アゼチジン-1-イル]ピリジン-2-イル}メチル)-6-(2-フルオロ-4-ヒドロキシベンジル)-4,7-ジオキソ-2-(プロプ-2-エン-1-イル)ヘキサヒドロ-2H-ピラジノ[2,1-c][1,2,4]トリアジン-1(6H)-カルボキサミドの使用。
- (6S,9aS)-N-ベンジル-8-({6-[3-(4-エチルピペラジン-1-イル)アゼチジン-1-イル]ピリジン-2-イル}メチル)-6-(2-フルオロ-4-ヒドロキシベンジル)-4,7-ジオキソ-2-(プロプ-2-エン-1-イル)ヘキサヒドロ-2H-ピラジノ[2,1-c][1,2,4]トリアジン-1(6H)-カルボキサミドと組み合わせて投与されることを特徴とする腫瘍治療用医薬組成物を製造するための、レンバチニブまたはその薬剤学的に許容される塩の使用。
