Definitions
- the present invention pertains to devices for introducing agents into the body, especially designed for medical use. More specifically the present invention is framed as devices for draining wounds or the like (instruments for keeping wounds open).
- catheters for intravenous infusion in which, depending on the aggressiveness and duration of treatment, a peripheral venous catheter, a central venous catheter or a central arterial catheter will be used.
- Central venous catheters for short-term treatments are the most suitable for perfusing large volumes and for performing hemodynamic measurements, being used in hospitals, in intensive care, emergency and operating room services.
- Central venous catheters for long-term treatments can be implanted as a reservoir or peripheral.
- the Hickman catheter and the central peripheral insertion catheter are the most suitable for long-term and aggressive therapies, since they can remain implanted for long periods of time.
- the preferred veins for catheter implantation are those of the head (jugular vein), upper extremities (basilic vein, cephalic vein, brachial vein, axillary vein, cubital vein, radial vein, palmar vein) and lower (iliac vein, vein vein) femoral, saphenous vein), thorax, pelvis, and abdomen (upper and lower vena cava).
- the placement of these catheters is performed cutaneously, after inspection and palpation of the skin that covers the vein, discarding those areas where there are bruises, scars, lymphatic drainage problems, or members susceptible to infection.
- the technique is performed with sterilization measures, introducing the catheter needle, progressively, forming an angle of 30 to 40 degrees with the direction of blood flow and placing the bevel of the catheter up.
- the blood output indicates that the catheter is in the vein lumen.
- an x-ray is performed to verify its correct placement.
- the catheter is fixed, by a rigid support, to the patient's skin.
- the catheter may have one or more lights, for the perfusion of different solutions simultaneously and / or for blood extraction without the need to puncture the patient's vein.
- the means of fixation to the skin of a peripheral venous catheter, of a central venous catheter or of a central arterial catheter have the following serious drawbacks: being the site of infections at the puncture site level, causing saturation of the anatomical area which is very difficult to support and uncomfortable for the patient, and pose a risk to nurses when sutures are used to fix the catheter support.
- Patent ES 2350887 incorporates a device for fixing a peripheral venous catheter, a central venous catheter or a central arterial catheter to the body of a patient.
- the aforementioned patent has the limitation that the patient must carry a large number of components of the catheter itself and more specifically the fixation device to the patient's body, the distal lumen, the proximal lumen, the lumen for the inflation system, etc., it is still difficult to apply apositos at the puncture site level to avoid possible infections that could trigger septicemia, in addition to saturating the anatomical area, making it difficult and uncomfortable for the carrier patient.
- Patent ES 2063823 presents a central venous catheter that tends to reduce erosion or perforation of the vasculature by means of a tube and lumens of flexible plastic material with a size and shape that are adequately detailed with reference to US patent 4650472, but with a system of fixation by means of a bucket of wings with openings and adapters of closing type Luer female that still maintains a high level of saturation of the anatomical area where it is inserted.
- There is no known system of closing, coupling and fixing a catheter to the body of a patient which does not imply that the patient carries with it a large number of components of the catheter itself, solving the drawbacks described above.
- the present invention presents a solution to the problems of risk of infection and saturation of the anatomical area of the patients where the skin puncture is performed in which peripheral venous catheters, central venous catheters or central arterial catheters are fixed.
- the system for attaching these catheters to the skin of a patient is characterized by having one or several lumens attached, by the end opposite to its part inserted in a venous access of a patient, to a piece (2) immediately before leaving at the skin level, said piece being further characterized by constituting the joint support with one or a plurality of external ducts, to allow simultaneous perfusion of fluids of different therapeutic activity, by having a plurality of holes (5) for its fixation by the inner part of the patient's skin and by having one or a plurality of ducts (3) for lumens where they will be coupled, by mechanical adjustment, with one or a plurality of external ducts.
- Said ducts for lumens are housed in the inner bottom of a hollow piece (6), which is left on the outside of a patient's skin and carries on its outside a removable mechanical joint system, easily designed and manufactured for any expert in the art, whose function is to keep the remaining components of the present invention fixed.
- the closure system of a peripheral venous catheter, of a central venous catheter or of a central arterial catheter for the home environment is characterized by having a sealing plug (7) and a blind screw cap (8).
- the sealing cap is characterized by having one or a plurality of closure tubes (9) that, by mechanical adjustment, couple the ducts for lumens housed inside the hollow part (3), which is left outside of the skin of a patient, of the fixation system of the present invention.
- the blind hollow plug is characterized by having in its inner part a removable mechanical joint system that keeps it attached to the hollow part (6), which is left on the outside of the skin of a patient, of the fixation system of the present invention, to prevent the displacement of the cap.
- the sealing plug is characterized by having, in addition, a handle (1 1) to facilitate its removal and a guide and position channel (10) to favor its correct coupling with a position pin (4 ) of the hollow piece (6).
- the coupling system of a peripheral venous catheter, of a central venous catheter or of a central arterial catheter for the hospital environment is characterized by having an intermediate sealing plug (12) and a hollow threaded plug (13 ).
- the intermediate sealing cap is characterized by being crossed by one or a plurality of lumens tubes (14), which protrude on both sides, said lumens tubes being characterized by coupling, by mechanical adjustment, with the lumens ducts housed in the inside the hollow part (3), which remains on the outside of the skin of a patient, of the fixation system of the present invention.
- the hollow screw cap is characterized by having in its interior part a removable mechanical joint system that keeps it attached to the hollow part (6), which is on the outside of the skin of a patient, of the fixation system of the present invention, to prevent displacement of the intermediate sealing plug (12). Additionally and preferably the intermediate sealing plug, is characterized by having a handle to facilitate its extraction and a guide and position channel (10) to favor its correct coupling with a position pin (4) of the hollow piece (6).
- Figure 1 depicts elevation and profile views, in the European system, of a preferred embodiment of the catheter fixation system of the present invention.
- Figure 2 represents a perspective view of a preferred embodiment of the closure system of a catheter of the present invention, for a home environment.
- Figure 3 represents a perspective view of a preferred embodiment of the lumen coupling system of a catheter of the present invention, for a hospital environment.
- a preferred embodiment of the fixation system of a peripheral venous catheter, of a central venous catheter or of a central arterial catheter to the skin of a patient shown in Figure 1, has a catheter (1) that houses one or several lumens attached , by the end opposite to its part anchored in a large caliber vein of the patient, to a round piece (2), made of hypo-allergic material, said round piece (2) being characterized by having at least two holes (5 ) for fixation on the inside of the patient's skin, by having one or more ducts for lumens (3) where, by mechanical adjustment, one or more tubes for external lumens are coupled and by having a hollow threaded part ( 6), which remains on the outside of the skin of a patient, and inside which the aforementioned ducts for lumens (3) and one or several position lugs (4) are housed to favor and position their coupling with the plugs of closure (7 ) or sealing (12) by coincidence with its guide and position channel (10).

- the blind screw cap (8) is characterized by remaining attached, by means of a threaded joint, to the hollow threaded part (6) of the previous preferred embodiment and by preventing the removal of the sealing plug (7).
- a third preferred embodiment of the present invention of the coupling system for application in the hospital environment as shown in Figure 3 has an intermediate plug (12) and a hollow threaded plug (13).
