Abstract

Description
本発明は、血管等の生体管腔内にステントを送達、留置するためのステントデリバリーシステムに関する。
従来において、血管、胆管、気管、食道、尿道等の生体管腔内に生じた狭窄部や閉塞部の改善のためにステントが用いられる。ステントは、金属線材等によって全体として円筒形状に形成され、多数の側壁開口を有するように構成されたものであり、生体管腔内で拡張可能である。
ステントの1つの型式として、バルーンカテーテルのバルーンに載置され、バルーンによって内側から押し広げられることで生体管腔内に留置されるバルーン拡張型ステントがある。また、ステントの他の型式として、弾性力による自己拡張機能を備え、生体管腔内でシース等から放出されて自己拡張することで生体管腔内に留置される自己拡張型ステントがある。このようなバルーン拡張型ステントや自己拡張型ステントを、生体管腔内の所望の位置に送達し、留置するためにステントデリバリーシステムが用いられる。
一般に、従来の自己拡張型ステントのステントデリバリーシステムは、長尺状の内管と、内管の先端部近傍の外側に載置されたステントと、内管の外側に軸方向に変位可能であり収縮状態のステントを収容する外管と、外管の基端部に接続されたグリップとを備える。ステントデリバリーシステムを用いた手技では、ステントを内管と外管の間の間隙に圧縮・収納した状態で生体管腔内へと送達し、内管に対して外管を基端方向に移動させることで、生体管腔内にステントを放出して留置する。
ステントデリバリーシステムでは、さらに、外管を内管に対して軸方向に相対移動させるための移動機構がグリップに設けられる。移動機構は、回転可能な操作ホイールと、操作ホイールに設けられたピニオンに噛み合うラックが設けられたラック部材とを有する。そして、外管の基端部がラック部材に接続されている。このような移動機構の操作ホイールを所定方向に回転させると、ラック部材が後退移動し、これに伴って内管に対して外管が後退移動し、ステントが外管から生体管腔内へと放出される。なお、従来のステントデリバリーシステムを開示する文献としては、例えば、特表2007-504897号公報がある。
ところで、ステントには、治療部位に応じて全長が相当に長い(例えば、20cm以上)ものがある。また、複数のステントを内管の軸方向に並べて配置し、生体管腔内で複数のステントを放出し留置する場合がある。全長が相当に長いステントや、複数のステントを生体管腔内に放出するためには、ラック部材の初期位置からの基端方向への移動距離を長くする必要がある。従来のステントデリバリーシステムでは、1つのピニオンでラック部材を移動させる構成であるため、ラック部材をその必要移動距離だけ後退移動させるためには、必要移動距離に見合うだけのラック部材の全長が必要である。そして、ラック部材は、初期位置では、ピニオン側から先端方向に延在して配置されるため、ラック部材の長さに応じてグリップを先端方向に延ばさなければならない。その結果、グリップの全長が長くなり、手技中における取り扱い易さが低下するという問題がある。
このような課題を考慮し、本発明は、全長が相当に長いステントや複数のステントを用いる場合でも、グリップの全長を長くすることなく、生体管腔内にステントを放出し、留置することができるステントデリバリーシステムを提供することを目的とする。
上記の目的を達成するため、本発明は、長尺状の内側部材と、前記内側部材の外側に前記内側部材に対して軸方向に移動可能に配置されたシースと、前記内側部材と前記シースとの間に配置され自己拡張可能なステントと、前記内側部材に対して前記シースを軸方向に移動させるための移動機構を含むグリップと、を備え、初期状態で前記シース内に前記ステントを収納し、前記内側部材に対する前記シースの基端方向への移動に伴って前記ステントを生体管腔内に放出するステントデリバリーシステムであって、前記移動機構は、前記シースに接続され前記ステントデリバリーシステムの軸方向に移動可能であると共に前記軸方向に延在する変位部材と、前記変位部材の延在方向に互いに間隔をおいて配置され、回転に伴って前記変位部材を前記延在方向に移動させる複数の駆動回転体と、を有することを特徴とする。
上記のステントデリバリーシステムによれば、変位部材の延在方向に沿って互いに間隔をおいて設けられた複数の駆動回転体により、変位部材を基端方向に順次送るようになっている。この構成により、1つの駆動回転体により変位部材を移動させる構成と比較して、変位部材の全長を短くすることができる。従って、長いステントや軸方向に並んだ複数のステントを放出する構成の場合でも、変位部材の全長を長くすることなく、変位部材に必要な移動距離を確保することができる。このため、変位部材の長さを短くできる分、グリップの長さを節約することができ、手技中のグリップの取り扱い易さを向上できる。
上記のステントデリバリーシステムにおいて、前記複数の駆動回転体を連動させる連動機構と、前記連動機構を介して前記複数の駆動回転体を回転させる回転操作部と、を備えてもよい。この構成によれば、1つの駆動回転体の回転に連動して他の駆動回転体が回転する構成であるため、回転操作部の総数は駆動回転体の総数よりも少なくて済み、ステントデリバリーシステムの操作が簡便である。
上記のステントデリバリーシステムにおいて、前記回転操作部に、前記複数の駆動回転体の1つが設けられていてもよい。この構成によれば、複数の駆動回転体を簡便な構造で確実に連動させることができる。
上記のステントデリバリーシステムにおいて、前記連動機構は、前記複数の駆動回転体の各々の同軸上に設けられた複数の回転部と、前記複数の回転部に動力伝達可能に接触する少なくとも1つの伝達部材とを有してもよい。この構成によれば、複数の駆動回転体を簡便な構造で確実に連動させることができる。
上記のステントデリバリーシステムにおいて、前記伝達部材によって前記複数の回転部が連動する際、最も基端側の前記回転部は、最も先端側の前記回転部に対して速く回転してもよい。この構成によれば、ステントの放出初期では、ゆっくり放出することで位置決めをしやすくすることができ、ステントの放出終期では、早く放出することで早く手技を終えることができる。

上記のステントデリバリーシステムにおいて、前記第1連動部は、前記複数の第1駆動回転体の各々の同軸上に設けられた複数の第1回転部と、前記複数の第1回転部に動力伝達可能に接触する少なくとも1つの第1伝達部材とを有し、前記第2連動部は、前記複数の第2駆動回転体の各々の同軸上に設けられた複数の第2回転部と、前記複数の第2回転部に動力伝達可能に接触する少なくとも1つの第2伝達部材とを有してもよい。この構成によれば、複数の第1回転駆動体を簡便な構造で確実に連動させることができると共に、複数の第2回転駆動体を簡便な構造で確実に連動させることができる。
上記のステントデリバリーシステムにおいて、前記変位部材は、互いに反対側に第1及び第2被駆動部を有し、前記複数の第1駆動回転体は、前記第1被駆動部に当接して前記変位部材を駆動し、前記複数の第2駆動回転体は、前記第2被駆動部に当接して前記変位部材を駆動してもよい。この構成によれば、第1駆動回転体と第2駆動回転体とが変位部材の移動経路を挟んで互いに反対側に配置されるため、複数ずつ設けられる第1駆動回転体と第2駆動回転体とをグリップ内に効率的に配置できる。
上記のステントデリバリーシステムにおいて、前記グリップに回転可能に支持された第1回転操作部に、前記複数の第1駆動回転体の1つが設けられ、前記グリップに回転可能に支持された第2回転操作部に、前記複数の第2駆動回転体の1つが設けられ、前記第1回転操作部と、前記第2回転操作部とは、前記グリップにおける互いに反対側の外側面において露出してもよい。この構成によれば、第1回転操作部と第2回転操作部とが互いに異なる側でグリップから露出するため、使用者にとって、第1回転操作部と第2回転操作部の区別が容易であり、使い分けがし易い。
上記のステントデリバリーシステムにおいて、複数の前記ステントが、前記内側部材の軸方向に沿って並んで配置され、前記駆動回転体の数は、前記ステントの数と同じであり、先端側から順に前記駆動回転体を回転させる毎に、先端側から順に前記ステントが前記シースから放出されてもよい。この構成によれば、ステントが軸方向に沿って複数設けられる場合に、先端側から順に駆動回転体を回転させることで、先端側から順にステントがシースから放出される。すなわち、複数のステントの配置に対応して、複数の駆動回転体が設けられるため、複数のステントを放出させる各段階で、放出させたいステントだけを簡便な操作で確実に放出することができる。
上記のステントデリバリーシステムにおいて、前記駆動回転体の単位回転角度当たりの前記変位部材の移動距離が、少なくとも1つの前記駆動回転体と他の前記駆動回転体とで異なってもよい。この構成によれば、変位部材を移動させる際の移動速度が一定ではなく、駆動回転体によって異なる。このため、駆動回転体の配置によって、ステントをシースから放出する各段階での放出速度を制御することができ、適切且つ効率的に手技を遂行することができる。
上記のステントデリバリーシステムにおいて、複数の前記駆動回転体は、複数の第1駆動回転体と、複数の第2駆動回転体とを含み、前記変位部材の可動範囲において常に、少なくとも1つの前記第1駆動回転体と、少なくとも1つの前記第2駆動回転体とが、前記変位部材を駆動可能であり、前記複数の第1駆動回転体の単位回転角度当たりの前記変位部材の移動距離と、前記複数の第2駆動回転体の単位回転角度当たりの前記変位部材の移動距離とが異なってもよい。この構成によれば、第1駆動回転体と第2駆動回転体とで、変位部材を移動させる速度が異なる。このため、ステントを放出させたい速度に応じて、第1駆動回転体又は第2駆動回転体を回転させることにより、ステントのシースからの放出速度を容易に制御することができる。
上記のステントデリバリーシステムにおいて、前記変位部材は、互いに反対側に第1及び第2被駆動部を有し、前記複数の第1駆動回転体は、前記第1被駆動部に当接して前記変位部材を駆動し、前記複数の第2駆動回転体は、前記第2被駆動部に当接して前記変位部材を駆動してもよい。この構成によれば、第1駆動回転体と第2駆動回転体とが変位部材の移動経路を基準として互いに反対側に配置されるため、複数ずつ設けられる第1駆動回転体と第2駆動回転体とをグリップ内に効率的に配置できる。
上記のステントデリバリーシステムにおいて、前記グリップに回転可能に支持された複数の第1回転操作部の各々に、前記第1駆動回転体が設けられ、前記グリップに回転可能に支持された複数の第2回転操作部の各々に、前記第2駆動回転体が設けられ、前記複数の第1回転操作部と、前記複数の第2回転操作部とは、前記グリップにおける互いに反対側の外側面において露出してもよい。この構成によれば、第1回転操作部と第2回転操作部とが互いに異なる側でグリップから露出するため、使用者にとって、第1回転操作部と第2回転操作部の区別が容易であり、使い分けがし易い。
上記のステントデリバリーシステムにおいて、最も基端側に設けられた前記駆動回転体の単位角度当たりの前記変位部材の移動距離は、最も先端側に設けられた前記駆動回転体の単位角度当たりの前記変位部材の移動距離よりも大きくてもよい。この構成によれば、ステントの放出初期では、ゆっくり放出することで位置決めをしやすくすることができ、ステントの放出終期では、早く放出することで早く手技を終えることができる。
上記のステントデリバリーシステムにおいて、最も先端側に設けられた前記駆動回転体は、前記グリップに回転可能に支持された第1回転操作部に設けられ、最も基端側に設けられた前記駆動回転体は、前記グリップに回転可能に支持された第2回転操作部に設けられ、前記第1回転操作部と、前記第2回転操作部とは、前記グリップにおける互いに反対側の外側面において露出してもよい。この構成によれば、第1回転操作部と第2回転操作部とが互いに異なる側でグリップから露出するため、使用者にとって、第1回転操作部と第2回転操作部の区別が容易であり、使い分けがし易い。
上記のステントデリバリーシステムにおいて、前記変位部材の位置が、前記グリップの外側から視認可能であってもよい。この構成によれば、手技中に変位部材の位置が見えるため、回転させるべき駆動回転体を変更するタイミングを容易に知ることができ、効率的な操作でステントの放出手技を行うことができる。
本発明のステントデリバリーシステムによれば、全長が相当に長いステントや複数のステントを用いる場合でも、グリップの全長を長くすることなく、生体管腔内にステントを放出し、留置することができる。
以下、本発明に係るステントデリバリーシステムについて好適な実施形態を挙げ、添付の図面を参照しながら説明する。
[第1実施形態]
図1は、本発明の第1実施形態に係るステントデリバリーシステム10の一部省略側面図である。図1では、図示の便宜上、ステントデリバリーシステム10の軸方向の途中部分の図示を省略すると共に、ステントデリバリーシステム10の基端側(グリップ18側)に対して先端側(ステント12側)を大きく描いている。なお、この点は、第2~第4実施形態を示す図6~図8においても同様である。
図1は、本発明の第1実施形態に係るステントデリバリーシステム10の一部省略側面図である。図1では、図示の便宜上、ステントデリバリーシステム10の軸方向の途中部分の図示を省略すると共に、ステントデリバリーシステム10の基端側(グリップ18側)に対して先端側(ステント12側)を大きく描いている。なお、この点は、第2~第4実施形態を示す図6~図8においても同様である。
ステントデリバリーシステム10は、血管、胆管、気管、食道、尿道等の生体管腔内の所望位置にステント12を送達し、留置するための医療器具である。ステントデリバリーシステム10は、長尺状の内側チューブ14(内側部材)と、内側チューブ14の外側に配置された外側チューブ16(シース)と、内側チューブ14と外側チューブ16との間に配置された自己拡張可能なステント12と、外側チューブ16の基端側に設けられたグリップ18とを備える。
図1におけるステントデリバリーシステム10に関し、グリップ18が設けられた側又は方向を「基端側」又は「基端方向」と呼び、その反対側又は反対方向を「先端側」又は「先端方向」と呼び、他の各図についても同様とする。
図2に示すように、内側チューブ14は、ガイドワイヤ20を挿通するためのガイドワイヤルーメン22が形成された内側先端チューブ24と、内側先端チューブ24の基端側に連結部材26を介して連結された内側基端チューブ28とを有する。内側チューブ14の先端、すなわち内側先端チューブ24の先端は、外側チューブ16の先端よりも先端側に突出している。
内側先端チューブ24の先端には先端方向に開口する先端開口24aが形成される。内側先端チューブ24の基端は、当該内側先端チューブ24の径方向外側に向かって湾曲しており、径方向外方に開口した基端開口24bが形成される。ステントデリバリーシステム10の初期状態(図2の状態)において、基端開口24bは、後述するガイドワイヤ導出孔44と連通する。なお、ガイドワイヤ20は、例えば、ステントデリバリーシステム10を生体管腔内の病変部に導くために用いられる。
内側先端チューブ24の先端部には、径方向外方に膨出し、外側チューブ16の先端方向への移動を規制するストッパ部32が形成される。ストッパ部32が外側チューブ16の先端方向への移動を規制することにより、外側チューブ16が内側チューブ14の先端に対して先端方向に突出することが阻止される。
連結部材26は、内側先端チューブ24の基端と、内側基端チューブ28の先端に連結される。内側基端チューブ28は、その先端から基端まで貫通するルーメン34を有する。図1に示すように、内側基端チューブ28は、グリップ18内で外側チューブ16の基端よりも基端方向に突出し、グリップ18内を直線状に延在する。
内側基端チューブ28の基端部は、グリップ18の基端部に設けられたコネクタ36に接続される。コネクタ36には、生理食塩水等の液体をステントデリバリーシステム10に供給するための液体注入器が接続可能である。内側基端チューブ28の基端部と、コネクタ36とは固定されており、これにより、内側チューブ14は、グリップ18に対して軸方向に移動しないように固定されている。
このような内側チューブ14は、樹脂材料、金属材料等により構成され得る。なお、内側先端チューブ24は、内側基端チューブ28よりも可撓性の高い材料で構成されるとよい。
外側チューブ16は、内側チューブ14及びグリップ18に対して軸方向に移動可能に設けられる。本図示例において、外側チューブ16は、外側先端チューブ38と、この外側先端チューブ38の基端に連結された外側基端チューブ40とを有する。外側先端チューブ38の内部には、内側チューブ14の内側先端チューブ24が配置される。
外側基端チューブ40の内部には、内側チューブ14の内側基端チューブ28が配置される。外側チューブ16の基端部、すなわち外側基端チューブ40の基端部は、グリップ18内に挿入される。
外側先端チューブ38の先端は、生体管腔内の病変部にステント12を留置する際のステント12の放出口として機能すると共に、途中まで放出されたステント12を回収(再収納)する際の収納口としても機能する。
外側先端チューブ38の基端近傍には、外側先端チューブ38の内外を連通するガイドワイヤ導出孔44が形成される。図2に示すように、ステントデリバリーシステム10の初期状態において、内側先端チューブ24の基端開口24bとガイドワイヤ導出孔44とは連通する。従って、内側先端チューブ24の先端開口24aを介してガイドワイヤ20をガイドワイヤルーメン22に挿入し、さらに、基端開口24b及びガイドワイヤ導出孔44を介してガイドワイヤ20を外側チューブ16の外部に導出することができる。
図2のように、外側先端チューブ38の先端部の外周面には、X線(放射線)不透過材料からなる造影マーカー42が設けられるとよい。
ステント12は、略円筒形状且つ自己拡張可能に形成されている。具体的には、ステント12は、ステントデリバリーシステム10の初期状態において、内側チューブ14と外側チューブ16との間に収縮状態で配置される。すなわち、ステント12は、外側チューブ16によって拡張が規制されて半径内方向に圧縮された状態で、外側チューブ16内に収納される。また、ステント12は、外側チューブ16が内側チューブ14に対して基端方向に移動することに伴って、外側チューブ16から放出されることにより、自己の弾性復元力によって半径外方向に拡張する。ステント12の長さLsは、治療対象部位によって様々であるが、例えば、10~300mm程度、好ましくは40~250mm程度である。
なお、ステント12の構成としては、例えば、多数の側孔が設けられたメッシュ状の構成、円形の環状体を軸方向に多数連結した構成、周方向にジグザグ状に延在する環状体を軸方向に多数連結した構成等が挙げられる。ステント12を構成する材料としては、例えば、Ni-Ti合金等の超弾性合金が好適である。ステント12の先端及び基端には、例えば、X線不透過材料からなる造影マーカーが設けられるとよい。
図2に示すように、内側チューブ14(具体的には、内側先端チューブ24)の外周部には、ステント12の軸方向への移動を規制するステント保持機構46が設けられる。ステント保持機構46は、ステント12の基端方向への移動を阻止する後退阻止ストッパ47と、ステント12の先端方向への移動を阻止する前進阻止ストッパ48とを有する。
具体的には、後退阻止ストッパ47は、ステント12を放出するために内側チューブ14に対して外側チューブ16を基端方向に移動させる際にステント12の縮径した基端部に係合することで、ステント12の後退移動を阻止する。前進阻止ストッパ48は、途中まで放出されたステント12を外側チューブ16内に再収納するために内側チューブ14に対して外側チューブ16を先端方向に移動させる際にステント12の縮径した基端部に係合することで、ステント12の前進移動を阻止する。
図1において、ステントデリバリーシステム10の基端部分を構成するグリップ18は、使用者が手で把持して操作するためのハンドルとして機能する部分である。グリップ18は、中空状のハウジング50を有し、ステントデリバリーシステム10の軸方向(前後方向)にやや長く構成されている。当該ハウジング50内に、内側チューブ14に対して外側チューブ16を軸方向に移動させるための移動機構52が設けられる。
図1及び図3に示すように、移動機構52は、外側チューブ16に接続されステントデリバリーシステム10の軸方向に移動可能であると共に当該軸方向に延在するラック部材54(変位部材)と、ラック部材54の延在方向に互いに間隔をおいて配置され、回転に伴ってラック部材54を延在方向に移動させる複数(図示例では3つ)のピニオン56(駆動回転体)と、複数のピニオン56を連動させる連動機構63と、連動機構63を介して複数のピニオン56を回転させる操作ホイール58(回転操作部)とを有する。なお、ピニオン56は2つ又は4つ以上設けられてもよい。
ラック部材54は、ハウジング50内に設けられた図示しないガイド部によってステントデリバリーシステム10の軸方向(グリップ18の前後方向)に摺動可能に支持される。ラック部材54は、外側チューブ16の基端部に固定されているため、ラック部材54に連動して、外側チューブ16も移動する。ラック部材54(図示例ではラック部材54の下面)には、ラック部材54の軸方向に沿って複数の歯部からなるラック55が設けられる。
なお、ラック部材54の位置確認手段として、ハウジング50の一部又は全部を、透明性を有する材料で構成し、あるいは、ハウジング50の一部にラック部材54を露出させる開口を設け、ハウジング50内のラック部材54の位置をハウジング50の外側から視認できるようにしてもよい。あるいは、位置確認手段の別の構成として、ラック部材54に突起を設け、この突起をハウジング50に設けたスリットを介してハウジング50から突出させ、突起の位置によってラック部材54の位置が分かる構成としてもよい。
各ピニオン56(56a~56c)は、ラック部材54のラック55に噛合可能である。3つのピニオン56の回転軸心は、互いに平行である。また、3つのピニオン56の回転軸心は、ラック部材54の移動可能方向(延在方向)に平行な1つの直線上に存在する。
図1において、3つのピニオン56a~56cにおける隣接する2つのピニオン56の中心間距離L2はすべて等しい。ラック部材54の長さL1(ラック55の長さ)は、ピニオン56の中心間距離L2よりも大きい。このため、ラック55は、少なくとも隣接する2つのピニオン56と噛み合うことが可能である。3つのピニオン56a~56cにおいて、歯数及びモジュールは、すべて同じである。なお、モジュール(m)は、ピッチ円直径(d)を歯数(z)で除した値である。なお、3つのピニオン56a~56cにおける隣接する2つのピニオン56の中心間距離L2は、異なっていてもよい。
最も先端側のピニオン56aが、操作ホイール58の径方向中央部に設けられる。図3に示すように、操作ホイール58の外周部には、溝60と突部61とが周方向に交互に配置された滑り防止形状が設けられる。操作ホイール58は、その軸方向に突出する軸部62を有する。軸部62は、ハウジング50の内側に設けられた軸受部によって支持される。これにより、操作ホイール58は、ハウジング50に回転可能に支持される。なお、他のピニオン56b、56cも、その軸方向に突出する軸部62を有し、各軸部62は、ハウジング50の内側に設けられた軸受部によって支持される。
図1において、ハウジング50には、操作ホイール58を部分的に露出させるための開口部39が設けられる。使用者は、操作ホイール58のうち開口部39から露出した部分に触れて、操作ホイール58を回転操作することができる。
図1及び図3に示すように、連動機構63は、複数のピニオン56の各々の同軸上に設けられた複数のプーリ64(回転部)と、複数のプーリ64に動力伝達可能に接触するベルト66(伝達部材)とを有する。図示例において最も先端側のプーリ64aは、操作ホイール58におけるピニオン56aの反対側に設けられる。従って、操作ホイール58、ピニオン56a及びプーリ64aは、同軸上に配置され、これらが一体的に回転する。
他のプーリ64b、64cは、連結軸68を介してそれぞれ他のピニオン56b、56cと連結されている。従って、中間に設けられたピニオン56bとプーリ64bは一体的に回転する。また、最も基端側に設けられたピニオン56cとプーリ64cは一体的に回転する。
本実施形態において、3つのプーリ64の直径(ピッチ円直径)は、すべて等しい。従って、ベルト66によって3つのプーリ64が連動して回転する際、3つのプーリ64の回転速度はすべて同じである。
ベルト66は、可撓性を有する無端部材(環状部材)であり、例えば、樹脂材料により構成され得る。本図示例のベルト66は、最も先端側のプーリ64aと最も基端側のプーリ64cに巻き掛けられると共に、中間に設けられたプーリ64bの外周部(上部及び下部)に接触する。なお、ベルト66は、中間に設けられたプーリ64bの少なくとも一部に接触していればよく、例えば、プーリ64bの上部のみ、あるいは下部のみに接触していてもよい。
本図示例において、各プーリ64は、外周部に複数の歯65が周方向に沿って形成された歯付きプーリであり、ベルト66は、各プーリ64と噛み合う複数の歯67が内周部に形成された歯付きベルトである。これにより、各プーリ64とベルト66との間でスリップが生じることがなく、3つのプーリ64を確実に連動させることができる。なお、各プーリ64の外周面とベルト66の内周面との接触によって摩擦力を十分に確保できる場合には、各プーリ64とベルト66は、それぞれ歯が形成されていない平プーリ、平ベルトであってもよく、あるいは、Vプーリ、Vベルトであってもよい。
上記のように連動機構63が構成されているため、操作ホイール58を回転操作すると、最も先端側のプーリ64aの回転がベルト66を介して他のプーリ64b、64cへと伝達される。従って、1つの操作ホイール58を回転操作するだけで、複数のピニオン56が連動して回転し、ラック部材54を前進又は後退させることができる。なお、本図示例では最も先端側のピニオン56aが操作ホイール58に設けられるが、他のピニオン56b、56cのいずれか1つが操作ホイール58に設けられてもよい。
図1及び図3に示すように、連動機構63には、ベルト66に適切な張力を付与する張力付与機構69が設けられるとよい。図示例の張力付与機構69は、ベルト66の外周面に接触する複数(2つ)のテンションピン69aを有する。テンションピン69aは、ハウジング50(図1参照)の内部に固定される。
一方のテンションピン69aは、ベルト66のうち、中間に設けられたプーリ64bと最も先端側に設けられたプーリ64aとの間の架け渡された部分に接触する。他方のテンションピン69aは、ベルト66のうち、中間に設けられたプーリ64bと最も基端側に設けられたプーリ64cとの間の架け渡された部分に接触する。これらのテンションピン69aにより、最も先端側と最も基端側の各プーリ64a、64cにベルト66が確実に巻き掛けられる。また、これらのテンションピン69aにより、ベルト66が中間のプーリ64bに接触するように矯正されるため、当該プーリ64bとベルト66とが確実に噛み合う。
なお、テンションピン69aは1つでもよい。また、張力付与機構69の他の構成として、テンションピン69aに代えて、ベルト66の外周面に接触する1つ又は複数のテンションローラが設けられてもよい。
グリップ18には、操作ホイール58の回転動作を規制することにより、ラック部材54の移動動作を規制可能なロック機構41が設けられる。ロック機構41は、ハウジング50の側面にスライド可能に設けられたスライド部材41aを有する。スライド部材41aは、操作ホイール58の回転動作を阻止するロック位置と、操作ホイール58の回転動作を許容する解除位置の間を移動可能である。スライド部材41aには、ハウジング50の内方に向けて突出したロックピン(図示せず)が設けられる。
一方、操作ホイール58には、ロックピンに係合可能な図示しない係合部(例えば溝部)が設けられる。スライド部材41aがロック位置にあるとき、ロックピンが係合部と係合することにより、操作ホイール58の回転動作が阻止される。一方、スライド部材41aが解除位置にあるとき、ロックピンが係合部から離脱することにより、操作ホイール58の回転動作が許容される。
本実施形態に係るステントデリバリーシステム10は、基本的には以上のように構成されるものであり、以下、その作用及び効果について説明する。
ステントデリバリーシステム10を用いて生体管腔内(例えば、血管内)に生じた病変部(例えば、狭窄部)を治療する手技では、ステントデリバリーシステム10の生体管腔内への挿入に先立って、ガイドワイヤ20(図2参照)を生体管腔内に挿入する。そして、ガイドワイヤ20の先端を、生体管腔内の病変部まで到達させた状態とする。
このような状態としたら、生体外にあるステントデリバリーシステム10における内側チューブ14及び外側チューブ16内を所定の液体(例えば、生理食塩水等)で満たすプライミングを行う。プライミングでは、先ず、術者がグリップ18の基端部に設けられたコネクタ36に対して液体注入具(図示せず)を接続し、当該液体注入具からコネクタ36へと液体を注入する。これにより、液体が内側チューブ14及び外側チューブ16の先端側へと流れ、内側チューブ14及び外側チューブ16の各先端から流出する。これにより、生体外における内側チューブ14及び外側チューブ16の内部のプライミングが完了する。
次に、生体外に露呈しているガイドワイヤ20の基端を、図2に示されるように、内側チューブ14の先端からガイドワイヤルーメン22へと挿通させ、ガイドワイヤ導出孔44を通じて外側チューブ16の外部へと導出させる。そして、ガイドワイヤ20に沿って内側チューブ14及び外側チューブ16を生体管腔内へと進めていく。
そして、外側チューブ16の先端が病変部に到達したことを造影マーカー42によって確認した後、グリップ18に設けられたスライド部材41aを先端側へと移動させることにより、操作ホイール58の回転規制状態を解除する。この時点では、図4Aのように、ラック部材54は、その可動範囲の最も先端側に位置している。図4Aにおいて、ステント12を完全に外側チューブ16の先端から放出する状態となったときのラック部材54の位置を点線で示している。参照符号L3は、ステント12を完全に外側チューブ16の先端から放出させるためのラック部材54の必要移動距離である。
操作ホイール58の回転規制状態を解除したら、操作ホイール58のうち、開口部39を介してハウジング50から露出した部分に触れて、操作ホイール58を所定方向(矢印A方向)へと回転させる。これにより、操作ホイール58に設けられたピニオン56aの回転に伴って、ラック部材54がハウジング50内で基端側へと移動し、これにより、ラック部材54に連結された外側チューブ16も、グリップ18の基端方向に移動していく。
この結果、外側チューブ16が内側チューブ14に対して基端方向に移動することに伴って、外側チューブ16内に収容されていたステント12(図1参照)が、先端部側から徐々に露出され始めるのと同時に半径方向外方に拡張し始める。すなわち、外側チューブ16の基端方向への移動に伴って、ステント12が放出されていく。なお、ステント12を途中まで放出させた段階で、生体管腔内でのステント12の位置を調整したい場合には、操作ホイール58を逆方向(矢印B方向)に回転操作すればよい。これにより、外側チューブ16が先端方向へと移動し、ステント12を外側チューブ16内に再収納することができる。
操作ホイール58の回転操作に伴ってラック部材54が基端方向へと移動していくと、やがてラック部材54のラック55は、図4Bのように、1つ基端側に設けられたピニオン56bと噛み合う。このとき、ステント12は途中(全長の3分の1程度)まで放出された状態である。
中間に設けられたプーリ64bはベルト66を介して最も先端側のプーリ64aと連動しているため、操作ホイール58に対する回転操作に伴って、中間に設けられたピニオン56bも回転させられる。従って、操作ホイール58を矢印A方向にさらに回転操作すると、ラック部材54と最も先端側のピニオン56aとの噛み合いが外れるが、ラック部材54は、中間に設けられたピニオン56bによって駆動され基端方向へと移動する。このようにピニオン56bの駆動によって、外側チューブ16を基端方向に移動させ、ステント12をさらに放出させていく。
操作ホイール58の回転操作に伴ってラック部材54が基端方向へと移動していくと、やがてラック部材54のラック55は、図4Cのように、その1つ基端側に設けられたピニオン56cと噛み合う。このとき、ステント12は未だ途中(全長の3分の2程度)まで放出された状態である。
最も基端側のプーリ64cはベルト66を介して最も先端側のプーリ64aと連動しているため、プーリ64aが設けられた操作ホイール58に対する回転操作に伴って、最も基端側のピニオン56cも回転する。従って、操作ホイール58を矢印A方向にさらに回転操作すると、ラック部材54と中間に設けられたピニオン56bとの噛み合いが外れるが、ラック部材54は、最も基端側のピニオン56cによって駆動され基端方向へと移動する。このようにピニオン56cの駆動によって、外側チューブ16を基端方向に移動させ、ステント12をさらに放出させていく。
そして、ステント12が、外側チューブ16に対して完全に露出した状態(ステント12の全長が放出された状態)となる。これにより、ステント12は円筒状に拡張した状態で病変部に留置されるに至る。
その後、ステントデリバリーシステム10を構成する内側チューブ14及び外側チューブ16を基端側へと引くことにより、ステント12のみを生体管腔内に残して、内側チューブ14及び外側チューブ16を生体外へと抜去する。
なお、図1に示したステントデリバリーシステム10では、ステント12が1つだけ設けられているが、本発明はこの構成に限らず、複数のステント12が軸方向に並べて配置された構成であってもよい。例えば、軸方向に並べて配置された2つのステント12(図1に示したステント12よりも短いもの)を備えるステントデリバリーシステム10の場合、次のように動作する。
すなわち、操作ホイール58の矢印A方向への回転操作によってラック部材54及び外側チューブ16が基端方向に移動することに伴って、先ず、1つ目(先端側)のステント12が外側チューブ16から放出される。そして、1つ目のステント12が放出された後、さらに操作ホイール58を矢印A方向に回転操作すると、ラック部材54及び外側チューブ16が基端方向に移動することに伴って、2つ目のステント12が外側チューブ16から放出される。
この場合、ステントデリバリーシステム10(内側チューブ14の先端)の位置を変えずに、2つのステント12を連続的に放出してもよい。あるいは、1つ目のステント12を放出した後に、ステントデリバリーシステム10(内側チューブ14の先端)の位置を移動し、生体管腔内の別の箇所で2つ目のステント12を放出してもよい。
以上説明したように、第1実施形態に係るステントデリバリーシステム10によれば、ラック部材54の延在方向に沿って互いに間隔をおいて設けられた複数のピニオン56(56a~56c)により、ラック部材54を基端方向に順次送るようになっている。このため、ラック部材54の長さL1は、ラック部材54の必要移動距離L3(図4A参照)よりも相当に短い。これに対し、1つのピニオンによりラック部材を移動させる従来構成の場合、当該ラック部材の長さは、少なくともその必要移動距離以上に設定される必要があるため、ラック部材の長さが相当に長くなり、ラック部材が収容されるグリップも長くならざるを得ない。
従って、ステントデリバリーシステム10によれば、長いステント12を放出する構成の場合、あるいは軸方向に並んだ複数のステント12を放出する構成の場合でも、ラック部材54の全長を長くすることなく、ラック部材54の必要移動距離L3を確保することができる。このため、ラック部材54の長さを短くできる分、グリップ18の長さを節約することができ、手技中のグリップ18の取り扱い易さを向上できる。
また、1つのピニオン56aの回転に連動して他のピニオン56b、56cが回転する構成であるため、操作ホイール58の総数はピニオン56の総数よりも少なくて済み、ステントデリバリーシステム10の操作が簡便である。
さらに、第1実施形態の場合、連動機構63は、複数の回転部(図示例ではプーリ64)と、当該複数の回転部に動力伝達可能に接触する少なくとも1つの伝達部材(図示例ではベルト66)とを有する。この構成によれば、複数の駆動回転体(ピニオン56)を簡便な構造で確実に連動させることができる。
上述した第1実施形態では、3つのプーリ64に対して1つのベルト66を掛け渡した構成を例示したが、プーリ64が3つ以上の場合、3つ以上のプーリ64に対して複数のベルトを掛け渡す構成を採用してもよい。例えば、図3のように3つのプーリ64がある場合、3つのプーリ64に対して2つのベルトを用いてもよい。具体的には、最も先端側のプーリ64aと中間のプーリ64bに一方のベルトを掛け渡し、中間のプーリ64bと最も基端側のプーリ64cに他方のベルトを掛け渡した構成としてもよい。この場合、中間のプーリ64bは、同軸に並んだ2層のプーリとし、各層を構成するプーリに一方のベルトと他方のベルトをそれぞれ掛け渡してもよい。
上述した第1実施形態では、グリップ18の軸方向に移動する変位部材としてラック部材54を例示すると共に、複数の駆動回転体として複数のピニオン56を例示したが、当該変位部材と当該駆動回転体は、ラック55とピニオン56との噛み合いにより動力を伝達する構成に限られない。例えば、変位部材の外側面と、駆動回転体の外周面との接触による摩擦抵抗により動力を伝達する構成であってもよく、この点は、後述する第2~第4実施形態においても同様である。
上述した連動機構63に代えて、図5に示す連動機構70を採用してもよい。この連動機構70は、ドライブシャフト74により動力を伝達する構成となっている。具体的には、連動機構70は、複数のピニオン56の各々の同軸上に設けられた複数(図示例では、3つ)の第1傘歯車72(回転部)と、複数の第1傘歯車72のそれぞれに噛み合う複数(図示例では、3つ)の第2傘歯車73が設けられたドライブシャフト74(伝達部材)とを有する。
操作ホイール58を回転操作すると、操作ホイール58と一体的に回転する最も先端側の第1傘歯車72aの回転が、ドライブシャフト74を介して他の第1傘歯車72b、72cに伝達される。これにより、操作ホイール58の回転操作に伴って、3つのピニオン56a~56cが連動して同一方向に回転するようになっている。
なお、図5の構成の場合、3つの第1傘歯車72a~72cの歯数及びモジュールの各積もすべて同じであり、3つの第2傘歯車73a~73cの歯数及びモジュールの各積はすべて同じである。従って、3つのピニオン56a~56cが連動して回転する際、これらのピニオン56a~56cの回転速度はすべて同じである。
[第2実施形態]
図6は、本発明の第2実施形態に係るステントデリバリーシステム80の一部省略側面図である。図6では、内側チューブ14の基端部及びグリップ18(ハウジング50)を仮想線で示している。なお、第2実施形態に係るステントデリバリーシステム80において、第1実施形態に係るステントデリバリーシステム10と同一又は同様な機能及び効果を奏する要素には同一の参照符号を付し、詳細な説明を省略する。
図6は、本発明の第2実施形態に係るステントデリバリーシステム80の一部省略側面図である。図6では、内側チューブ14の基端部及びグリップ18(ハウジング50)を仮想線で示している。なお、第2実施形態に係るステントデリバリーシステム80において、第1実施形態に係るステントデリバリーシステム10と同一又は同様な機能及び効果を奏する要素には同一の参照符号を付し、詳細な説明を省略する。
このステントデリバリーシステム80は、連動機構82の構成において、上述した第1実施形態に係るステントデリバリーシステム10と異なる。連動機構82は、複数のピニオン56の各々の同軸上に設けられた複数のプーリ84(回転部)と、複数のプーリ84に動力伝達可能に接触する少なくとも1つのベルト86(伝達部材)とを有する。
本実施形態では、複数のプーリ84のうち、最も先端側のプーリ84aの直径が最も大きく、最も基端側のプーリ84cの直径が最も小さい。従って、最も基端側のプーリ84cの直径は、最も先端側のプーリ84aの直径よりも小さい。中間に設けられたプーリ84bの直径は、最も先端側のプーリ84aの直径よりも小さく、最も基端側のプーリ84cの直径よりも大きい。
本実施形態では、各プーリ84は歯付きプーリであり、各プーリ84とベルト86とは噛み合っている。これらのプーリ84のモジュールは、すべて同じであるが、上述したように、最も基端側のプーリ84cの直径は、最も先端側のプーリ84aの直径よりも小さいことから、最も基端側のプーリ84cの歯数は、最も先端側のプーリ84aの歯数よりも少ない。中間に設けられたプーリ84bの歯数は、最も先端側のプーリ84aの歯数よりも少なく、最も基端側のプーリ84cの歯数よりも多い。
ベルト86は、可撓性を有する無端部材(環状部材)であり、例えば、樹脂材料により構成され得る。ベルト86は、複数のプーリ84に掛け渡されている。具体的には、ベルト86は、最も先端側と最も基端側の各プーリ84a、84cに巻き掛けられると共に、中間に設けられたプーリ84bの外周部(上部及び下部)に接触している。
なお、ベルト86は、中間に設けられたプーリ84bの少なくとも一部に接触していればよく、例えば、プーリ84bの上部のみ、あるいは下部のみに接触していてもよい。各プーリ84とベルト86は、それぞれ歯が形成されていない平プーリ、平ベルトであってもよく、あるいは、Vプーリ、Vベルトであってもよい。
このように構成された連動機構82では、ベルト86によって複数のプーリ84が連動する際、最も基端側のプーリ84cは、最も先端側のプーリ84aに対して速く回転する。すなわち、最も先端側のプーリ84aの回転は、増速されて最も基端側のプーリ84cへと伝達される。従って、操作ホイール58に対する回転操作速度が同じである場合、最も先端側のピニオン56aによってラック部材54を駆動する際には、ラック部材54と共に移動する外側チューブ16の移動速度は相対的に遅く、最も基端側のピニオン56cによってラック部材54を駆動する場合には、ラック部材54と共に移動する外側チューブ16の移動速度は相対的に速い。
このステントデリバリーシステム80によれば、操作ホイール58を回転操作することにより、連動機構82の作用下に連動して回転する複数のピニオン56でラック部材54を順次、基端方向に送り、内側チューブ14に対して外側チューブ16を基端方向に移動させることができる。外側チューブ16の基端方向への移動に伴って、ステント12がその先端側から徐々に放出され、図6において点線で描かれた位置までラック部材54が移動した段階では、ステント12は外側チューブ16の先端から完全に放出される。
この場合、ステント12の放出初期では、最も先端側のピニオン56aによりラック部材54を低速で基端方向に移動させるため、ステント12がゆっくり放出され、位置決めがし易い。一方、ステント12の放出終期では、最も基端側のピニオン56cによりラック部材54を基端方向に移動させる。最も基端側のプーリ84cの回転速度は、最も先端側のプーリ84aに対して加速されているため、ステント12の放出終期では、ステント12が素早く放出され、早く手技を終えることができる。
第1実施形態と同様に、第2実施形態においても、ラック部材54の延在方向に間隔をおいて配置された複数のピニオン56によりラック部材54が駆動される。このため、ラック部材54の必要移動距離L3に対してラック部材54の長さL1を相当に短くできる。従って、長いステント12や軸方向に並んだ複数のステント12を放出する構成の場合でも、ラック部材54を長大化する必要がない。この結果、グリップ18の長さを節約でき、手技中のグリップ18の取り扱い易さを向上できる。
上述した第2実施形態では、3つのプーリ84に対して1つのベルト86を掛け渡した構成を例示したが、プーリ84が3つ以上の場合、3つ以上のプーリ84に対して複数のベルトを掛け渡す構成を採用してもよい。例えば、図6のように3つのプーリ84がある場合、最も先端側のプーリ84aと中間のプーリ84bに一方のベルトを掛け渡し、中間のプーリ84bと最も基端側のプーリ84cに他方のベルトを掛け渡した構成としてもよい。この場合、中間のプーリ84bは、同軸に並んだ2層のプーリとし、各層を構成するプーリに一方のベルトと他方のベルトをそれぞれ掛け渡してもよい。
その他、第2実施形態において、第1実施形態と共通する各構成部分については、第1実施形態における当該共通の各構成部分がもたらす作用及び効果と同一又は同様の作用及び効果が得られることは勿論である。
[第3実施形態]
図7は、本発明の第3実施形態に係るステントデリバリーシステム90の一部省略側面図である。図7では、内側チューブ14の基端部及びグリップ78(ハウジング50)を仮想線で示している。なお、第3実施形態に係るステントデリバリーシステム90において、第1実施形態に係るステントデリバリーシステム10と同一又は同様な機能及び効果を奏する要素には同一の参照符号を付し、詳細な説明を省略する。
図7は、本発明の第3実施形態に係るステントデリバリーシステム90の一部省略側面図である。図7では、内側チューブ14の基端部及びグリップ78(ハウジング50)を仮想線で示している。なお、第3実施形態に係るステントデリバリーシステム90において、第1実施形態に係るステントデリバリーシステム10と同一又は同様な機能及び効果を奏する要素には同一の参照符号を付し、詳細な説明を省略する。
このステントデリバリーシステム90は、内側チューブ14に対して外側チューブ16を基端方向に移動させるための移動機構52aを有するグリップ78を備える。移動機構52aは、ラック部材54と、複数のピニオン92、94と、複数の操作ホイール96、98と、連動機構100とを有する。操作ホイール96、98の総数は、ピニオン92、94の総数よりも少ない。ラック部材54には、互いに反対側(図示例では、上側と下側)に平行に延在する第1ラック102(第1被駆動部)及び第2ラック104(第2被駆動部)が設けられる。
複数のピニオン92、94は、第1ラック102に噛合可能でありラック部材54の延在方向に間隔をおいて配置された複数の第1ピニオン92(駆動回転体)と、第2ラック104に噛合可能でありラック部材54の延在方向に間隔をおいて配置された複数の第2ピニオン94(駆動回転体)とを有する。
本実施形態では、第2ピニオン94の歯数とモジュールの積は、第1ピニオン92の歯数とモジュールの積よりも大きい。このため、第2ピニオン94の単位回転角度当たりのラック部材54の移動距離は、第1ピニオン92の単位回転角度当たりのラック部材54の移動距離よりも大きい。すなわち、回転角度が同じである場合、第2ピニオン94の駆動によるラック部材54の移動距離は、第1ピニオン92の駆動によるラック部材54の移動距離よりも大きい。
従って、第1操作ホイール96に対する回転操作速度と、第2操作ホイール98に対する回転操作速度とが同じである場合、第1操作ホイール96を回転操作した際には、ラック部材54と共に移動する外側チューブ16の移動速度は相対的に遅く、第2操作ホイール98を回転操作した際には、ラック部材54と共に移動する外側チューブ16の移動速度は相対的に速い。
複数の操作ホイール96、98は、グリップ78における互いに反対側の外側面(図7では、上面と下面)において開口部39を介して部分的に露出した第1操作ホイール96と、第2操作ホイール98とを含む。本図示例では、第1操作ホイール96に先端側の第1ピニオン92aが設けられ、第2操作ホイール98に最も先端側の第2ピニオン94aが設けられる。
なお、第1操作ホイール96は、基端側の第1ピニオン92bと一体的に回転するように設けられてもよい。また、第2操作ホイール98は、中間の第2ピニオン94b又は最も基端側の第2ピニオン94cと一体的に回転するように設けられてもよい。
ラック部材54の長さL1(第1ラック102及び第2ラック104の各長さ)は、第1ピニオン92同士の中心間距離L4よりも大きいと共に、第2ピニオン94同士の中心間距離L5よりも大きい。ラック部材54の可動範囲内で、ラック部材54は、少なくとも1つの第1ピニオン92と噛み合うと共に、少なくとも1つの第2ピニオン94とも噛み合う。
なお、複数の第1ピニオン92(92a、92b)において、歯数及びモジュールは同じであり、複数の第2ピニオン94(94a~94c)において、歯数及びモジュールは同じである。第1ピニオン92と第2ピニオン94のモジュールは同じでもよいが、この場合、図7のように、第2ピニオン94の歯数は、第1ピニオン92の歯数よりも多い。あるいは、第1ピニオン92の歯数と第2ピニオン94の歯数とが同じで、第2ピニオン94のモジュールが、第1ピニオン92のモジュールよりも大きくてもよい。あるいは、第2ピニオン94の歯数及びモジュールが、それぞれ第1ピニオン92の歯数及びモジュールよりも大きくてもよい。
連動機構100は、複数の第1ピニオン92を連動させる第1連動部106と、複数の第2ピニオン94を連動させる第2連動部108とを有する。
第1連動部106は、複数の第1ピニオン92の各々の同軸上に設けられた複数の第1プーリ110(第1回転部)と、複数の第1プーリ110に動力伝達可能に接触する第1ベルト112(第1伝達部材)とを有する。図示例において先端側の第1プーリ110aは、第1操作ホイール96における第1ピニオン92aの反対側に設けられる。従って、第1操作ホイール96、第1ピニオン92a及び第1プーリ110aは、同軸上に配置され、これらが一体的に回転する。
基端側の第1プーリ110bは、図3に示した連結軸68と同様の連結軸を介して基端側の第1ピニオン92bと同軸上に連結されている。従って、基端側の第1プーリ110bと第1ピニオン92bは一体的に回転する。本実施形態において、複数の第1プーリ110の直径は、すべて等しい。従って、第1ベルト112によって複数の第1プーリ110が連動して回転する際、複数の第1プーリ110の回転速度はすべて同じである。
第1ベルト112は、可撓性を有する無端部材(環状部材)であり、例えば、樹脂材料により構成され得る。第1ベルト112は、先端側の第1プーリ110aと基端側の第1プーリ110bに巻き掛けられる。本図示例において、各第1プーリ110は歯付きプーリであり、第1ベルト112は歯付きベルトである。なお、各第1プーリ110、第1ベルト112は、それぞれ歯が形成されていない平プーリ、平ベルトであってもよく、あるいは、Vプーリ、Vベルトであってもよい。
上記のように第1連動部106が構成されているため、第1操作ホイール96を回転操作すると、先端側の第1プーリ110aの回転が第1ベルト112を介して基端側の第1プーリ110bへと伝達される。従って、1つの第1操作ホイール96を回転操作するだけで、複数の第1ピニオン92が連動して回転し、ラック部材54を前進又は後退させることができる。
第2連動部108は、複数の第2ピニオン94の各々の同軸上に設けられた複数の第2プーリ114(第2回転部)と、複数の第2プーリ114に動力伝達可能に接触する第2ベルト116(第2伝達部材)とを有する。
本図示例において、最も先端側の第2プーリ114aは、第2操作ホイール98における第2ピニオン94aの反対側に設けられる。従って、第2操作ホイール98、第2ピニオン94a及び第2プーリ114aは、同軸上に配置され、これらが一体的に回転する。
他の第2プーリ114b、114cは、図3に示した連結軸68と同様の連結軸を介して他の第2ピニオン94b、94cと連結されている。中間に設けられた第2ピニオン94bと第2プーリ114bは一体的に回転する。最も基端側に設けられた第2ピニオン94cと第2プーリ114cは一体的に回転する。
本実施形態において、複数の第2プーリ114の直径は、すべて等しい。従って、第2ベルト116によって複数の第2プーリ114が連動して回転する際、複数の第2プーリ114の回転速度はすべて同じである。
第2ベルト116は、可撓性を有する無端部材(環状部材)であり、例えば、樹脂材料により構成され得る。本図示例の第2ベルト116は、最も先端側の第2プーリ114aと最も基端側の第2プーリ114cに巻き掛けられると共に、中間に設けられた第2プーリ114bの外周部(上部及び下部)に接触する。なお、第2ベルト116は、中間に設けられた第2プーリ114bの少なくとも一部に接触していればよく、例えば、第2プーリ114bの上部のみ、あるいは下部のみに接触していてもよい。
本図示例において、各第2プーリ114は歯付きプーリであり、第2ベルト116は歯付きベルトである。なお、各第2プーリ114、第2ベルト116は、それぞれ歯が形成されていない平プーリ、平ベルトであってもよく、あるいは、Vプーリ、Vベルトであってもよい。
上記のように第2連動部108が構成されているため、第2操作ホイール98を回転操作すると、最も先端側の第2プーリ114aの回転が第2ベルト116を介して中間に設けられた第2プーリ114b及び最も基端側の第2プーリ114cへと伝達される。従って、1つの第2操作ホイール98を回転操作するだけで、複数の第2ピニオン94が連動して回転し、ラック部材54を前進又は後退させることができる。
このように構成されるステントデリバリーシステム90によれば、第1操作ホイール96又は第2操作ホイール98を矢印A方向に回転操作することにより、複数のピニオン92、94が回転してラック部材54を順次、基端方向に送り、内側チューブ14に対して外側チューブ16を基端方向に移動させることができる。外側チューブ16の基端方向への移動に伴って、ステント12がその先端側から徐々に放出され、図7において点線で描かれた位置までラック部材54が移動した段階では、ステント12は外側チューブ16の先端から完全に放出される。
この場合、第1ピニオン92と第2ピニオン94とで、ラック部材54を移動させる速度が異なる。このため、ステント12を放出させたい速度に応じて、第1操作ホイール96又は第2操作ホイール98を矢印A方向に回転操作して、第1ピニオン92又は第2ピニオン94を回転させることにより、外側チューブ16からのステント12の放出速度を容易に制御することができる。
例えば、ステント12をゆっくり放出させたい場合には、第1操作ホイール96を回転操作して歯数が少ない第1ピニオン92によりラック部材54を駆動することにより、外側チューブ16を低速で基端方向に移動させればよい。一方、ステント12を素早く放出させたい場合には、第2操作ホイール98を回転操作して歯数が多い第2ピニオン94によりラック部材54を駆動することにより、外側チューブ16を高速で基端方向に移動させればよい。
また、例えば、長いステント12の場合、ステント12の放出初期では、第1操作ホイール96を回転操作して第1ピニオン92によりラック部材54を駆動することにより、外側チューブ16を基端方向に移動させる。そうすると、ステント12がゆっくり放出され、位置決めがし易い。一方、ステント12の放出終期では、第2操作ホイール98を回転操作して第2ピニオン94によりラック部材54を駆動することにより、外側チューブ16を基端方向に移動させる。そうすると、ステント12が素早く放出され、早く手技を終えることができる。
第1実施形態と同様に、第3実施形態においても、ラック部材54の延在方向に間隔をおいて配置された複数のピニオン92、94によりラック部材54が駆動される。このため、ラック部材54の必要移動距離L3に対してラック部材54の長さL1を相当に短くできる。従って、長いステント12や軸方向に並んだ複数のステント12を放出する構成の場合でも、ラック部材54を長大化する必要がない。この結果、グリップ78の長さを節約でき、手技中のグリップ78の取り扱い易さを向上できる。
また、第3実施形態によれば、第1ピニオン92と第2ピニオン94とで、ラック部材54を移動させる速度が異なる。このため、ステント12を放出させたい速度に応じて、第1ピニオン92又は第2ピニオン94を回転させることにより、ステント12の外側チューブ16からの放出速度を容易に制御することができる。
また、第3実施形態によれば、第1ピニオン92と第2ピニオン94とがラック部材54を基準として互いに反対側に配置されるため、複数ずつ設けられる第1ピニオン92と第2ピニオン94とをグリップ78内に効率的に配置できる。
さらに、第3実施形態によれば、第1操作ホイール96と第2操作ホイール98とが互いに異なる側でグリップ78から露出するため、使用者にとって、第1操作ホイール96と第2操作ホイール98の区別が容易であり、使い分けがし易い。
第3実施形態の場合、第1連動部106は、複数の第1回転部(図示例では第1プーリ110)と、当該複数の第1回転部に動力伝達可能に接触する少なくとも1つの第1伝達部材(図示例では第1ベルト112)とを有する。この構成によれば、複数の第1駆動回転体(第1ピニオン92)を簡便な構造で確実に連動させることができる。また、第2連動部108は、複数の第2回転部(図示例では第2プーリ114)と、当該複数の第2回転部に動力伝達可能に接触する少なくとも1つの第2伝達部材(図示例では第2ベルト116)とを有する。この構成によれば、複数の第2駆動回転体(第2ピニオン94)を簡便な構造で確実に連動させることができる。
第3実施形態の場合、第1連動部106及び第2連動部108は、それぞれベルト機構で構成されるため、動力を効率的に伝達することができ、第1操作ホイール96又は第2操作ホイール98の回転操作によるラック部材54の前進又は後退を円滑に行うことができる。
その他、第3実施形態において、第1実施形態と共通する各構成部分については、第1実施形態における当該共通の各構成部分がもたらす作用及び効果と同一又は同様の作用及び効果が得られることは勿論である。
上述した第3実施形態では、3つの第2プーリ114に対して1つの第2ベルト116を掛け渡した構成を例示したが、第2プーリ114が3つ以上の場合、3つ以上の第2プーリ114に対して複数の第2ベルトを掛け渡す構成を採用してもよい。例えば、図7のように3つの第2プーリ114がある場合、3つの第2プーリ114に対して2つの第2ベルトを用いてもよい。具体的には、最も先端側の第2プーリ114aと中間の第2プーリ114bに一方の第2ベルトを掛け渡し、中間の第2プーリ114bと最も基端側の第2プーリ114cに他方の第2ベルトを掛け渡した構成としてもよい。この場合、中間の第2プーリ114bは、同軸に並んだ2層のプーリとし、各層を構成するプーリに一方の第2ベルトと他方の第2ベルトをそれぞれ掛け渡してもよい。
なお、図7では第1プーリ110は2つであるが、第1プーリ110が3つ以上設けられる場合には、3つ以上の第1プーリ110に対して複数の第1ベルトを掛け渡す構成としてもよい。
図7に示す構成では、第2ピニオン94の歯数とモジュールの積は、第1ピニオン92の歯数とモジュールの積よりも大きいが、この構成とは逆に、第1ピニオン92の歯数とモジュールの積が、第2ピニオン94の歯数とモジュールの積よりも大きくてもよい。この場合、第1ピニオン92が設けられた第1操作ホイール96を回転操作すると、ステント12が素早く放出され、第2ピニオン94が設けられた第2操作ホイール98を回転操作すると、ステント12がゆっくり放出される。
図7に示す構成では、ラック部材54に2つのラック(第1ラック102と第2ラック104)が設けられ、複数の第1ピニオン92と複数の第2ピニオン94はラック部材54の移動経路の互いに反対側に配置されているが、以下のように構成されてもよい。すなわち、ラック部材54には1つのラックのみが設けられ、複数の第1ピニオン92と複数の第2ピニオン94とがラック部材54の移動経路に対して同じ側に設けられてもよい。この場合、複数の第1ピニオン92と複数の第2ピニオン94とはラック部材54の軸方向に沿って交互に配置される。
[第4実施形態]
図8は、本発明の第4実施形態に係るステントデリバリーシステム120の一部省略側面図である。図8では、内側チューブ14の基端部及びグリップ122(ハウジング50)を仮想線で示している。なお、第4実施形態に係るステントデリバリーシステム120において、第1実施形態に係るステントデリバリーシステム10と同一又は同様な機能及び効果を奏する要素には同一の参照符号を付し、詳細な説明を省略する。
図8は、本発明の第4実施形態に係るステントデリバリーシステム120の一部省略側面図である。図8では、内側チューブ14の基端部及びグリップ122(ハウジング50)を仮想線で示している。なお、第4実施形態に係るステントデリバリーシステム120において、第1実施形態に係るステントデリバリーシステム10と同一又は同様な機能及び効果を奏する要素には同一の参照符号を付し、詳細な説明を省略する。
このステントデリバリーシステム120は、内側チューブ14に対して外側チューブ16を基端方向に移動させるための移動機構52bを有するグリップ122を備える。移動機構52bは、ラック部材54と、複数(図示例では、3つ)のピニオン124と、複数(図示例では、2つ)の操作ホイール126と、連動機構128とを有する。操作ホイール126の総数は、ピニオン124の総数よりも少ない。ラック部材54には、互いに反対側(図示例では、上側と下側)に第1ラック102及び第2ラック104が設けられる。
複数のピニオン124は、ラック部材54の延在方向に互いに間隔をおいて配置される。具体的には、複数のピニオン124は、第1ラック102に噛合可能でありラック部材54の延在方向に間隔をおいて配置された複数の第1ピニオン124a(駆動回転体)と、第2ラック104に噛合可能な第2ピニオン124b(駆動回転体)とを有する。第2ピニオン124bは、基端側の第1ピニオン124aよりもさらに基端側に配置される。
複数の操作ホイール126は、ラック部材54の延在方向(軸方向)に間隔をおいて回転可能に配置される。複数の操作ホイール126は、第1ラック102を介してラック部材54を駆動する第1操作ホイール126aと、第2ラック104を介してラック部材54を駆動する第2操作ホイール126bとを含む。
図示例では、1つの第1操作ホイール126aが設けられると共に、1つの第2操作ホイール126bが設けられる。第1操作ホイール126aと、第2操作ホイール126bは、グリップ122における互いに反対側の外側面(図8では、上面と下面)において開口部39を介して部分的に露出する。
本図示例では、第1操作ホイール126aは、先端側の第1ピニオン124aと一体的に回転するように設けられる。なお、第1操作ホイール126aは、基端側の第1ピニオン124aと一体的に回転するように設けられてもよい。第2操作ホイール126bには、第2ピニオン124bが設けられる。
第2ピニオン124bの歯数とモジュールの積は、第1ピニオン124aの歯数とモジュールの積よりも大きい。このため、第2ピニオン124bの単位回転角度当たりのラック部材54の移動距離は、第1ピニオン124aの単位回転角度当たりのラック部材54の移動距離よりも大きい。すなわち、回転角度が同じ場合、第2ピニオン124bの駆動によるラック部材54の移動距離は、第1ピニオン124aの駆動によるラック部材54の移動距離よりも大きい。
従って、第1操作ホイール126aに対する回転操作速度と、第2操作ホイール126bに対する回転操作速度とが同じである場合、第1操作ホイール126aを回転操作した際には、ラック部材54と共に移動する外側チューブ16の移動速度は相対的に遅く、第2操作ホイール126bを回転操作した際には、ラック部材54と共に移動する外側チューブ16の移動速度は相対的に速い。
複数の第1ピニオン124aにおいて、歯数及びモジュールは同じである。第1ピニオン124aと第2ピニオン124bのモジュールは同じでもよいが、この場合、図8のように、第2ピニオン124bの歯数は、第1ピニオン124aの歯数よりも多い。あるいは、第1ピニオン124aの歯数と第2ピニオン124bの歯数とが同じで、第2ピニオン124bのモジュールが、第1ピニオン124aのモジュールよりも大きくてもよい。あるいは、第2ピニオン124bの歯数及びモジュールが、それぞれ第1ピニオン124aの歯数及びモジュールよりも大きくてもよい。
ラック部材54の長さ(第1ラック102及び第2ラック104の各長さ)L1は、第1ピニオン124a同士の中心間距離L6よりも大きい。また、ラック部材54の長さL1は、基端側の第1ピニオン124aと第2ピニオン124bとの中心間距離L7よりも大きい。従って、ラック部材54の可動範囲内で、ラック部材54は、複数のピニオン124のうち、少なくともグリップ122の軸方向(ラック部材54の延在方向)に隣接する2つのピニオン124と噛み合う。なお、第1ピニオン124a同士の中心間距離L6と、基端側の第1ピニオン124aと第2ピニオン124bとの中心間距離L7とは、同じでもよく、異なっていてもよい。
連動機構128は、複数の第1ピニオン124aの各々の同軸上に設けられた複数のプーリ130(回転部)と、複数のプーリ130に動力伝達可能に接触するベルト132(伝達部材)とを有する。図示例において先端側のプーリ130aは、第1操作ホイール126aにおける第1ピニオン124aの反対側に設けられる。従って、第1操作ホイール126a、第1ピニオン124a及びプーリ130aは、同軸上に配置され、これらが一体的に回転する。
基端側のプーリ130bは、図3に示した連結軸68と同様の連結軸を介して基端側の第1ピニオン124aと同軸上に連結されている。従って、基端側のプーリ130bと基端側の第1ピニオン124aは一体的に回転する。本実施形態において、複数のプーリ130の直径は、すべて等しい。従って、ベルト132によって複数のプーリ130が連動して回転する際、複数のプーリ130の回転速度はすべて同じである。
ベルト132は、可撓性を有する無端部材(環状部材)であり、例えば、樹脂材料により構成され得る。ベルト132は、先端側のプーリ130aと基端側のプーリ130bに巻き掛けられる。本図示例において、各プーリ130は歯付きプーリであり、ベルト132は歯付きベルトである。なお、各プーリ130、ベルト132は、それぞれ歯が形成されていない平プーリ、平ベルトであってもよく、あるいは、Vプーリ、Vベルトであってもよい。
上記のように連動機構128が構成されているため、第1操作ホイール126aを回転操作すると、先端側のプーリ130aの回転がベルト132を介して基端側のプーリ130bへと伝達される。従って、1つの第1操作ホイール126aを回転操作するだけで、複数の第1ピニオン124aが連動して回転し、ラック部材54を前進又は後退させることができる。
このように構成されるステントデリバリーシステム120によれば、操作ホイール126を矢印A方向に回転操作することにより、複数のピニオン124が回転してラック部材54を順次、基端方向に送り、内側チューブ14に対して外側チューブ16を基端方向に移動させることができる。外側チューブ16の基端方向への移動に伴って、ステント12がその先端側から徐々に放出され、図8において点線で描かれた位置までラック部材54が移動した段階では、ステント12は外側チューブ16の先端から完全に放出される。
この場合、ステント12の放出初期では、第1操作ホイール126aを回転操作して先端側の第1ピニオン124aによりラック部材54を低速で基端方向に移動させるため、ステント12がゆっくり放出され、位置決めがし易い。一方、ステント12の放出終期では、第2操作ホイール126bを回転操作して第2ピニオン124bにより高速でラック部材54を基端方向に移動させるため、ステント12が素早く放出され、早く手技を終えることができる。
第1実施形態と同様に、第4実施形態においても、ラック部材54の延在方向に間隔をおいて配置された複数のピニオン124によりラック部材54が駆動される。このため、ラック部材54の必要移動距離L3に対してラック部材54の長さL1を相当に短くできる。従って、長いステント12や軸方向に並んだ複数のステント12を放出する構成の場合でも、ラック部材54を長大化する必要がない。この結果、グリップ122の長さを節約でき、手技中のグリップ122の取り扱い易さを向上できる。
また、第4実施形態によれば、第1操作ホイール126aと第2操作ホイール126bとが互いに異なる側でグリップ122から露出するため、使用者にとって、第1操作ホイール126aと第2操作ホイール126bの区別が容易であり、使い分けがし易い。
その他、第4実施形態において、第1実施形態と共通する各構成部分については、第1実施形態における当該共通の各構成部分がもたらす作用及び効果と同一又は同様の作用及び効果が得られることは勿論である。
なお、図8に示す構成では、ラック部材54に2つのラック(第1ラック102と第2ラック104)が設けられ、第1ピニオン124aと第2ピニオン124bはラック部材54の移動経路の互いに反対側に配置されているが、以下のように構成されてもよい。すなわち、ラック部材54にはラックが1つだけ設けられ、第1ピニオン124aと第2ピニオン124bとがラック部材54の移動経路に対して同じ側に設けられてもよい。
[第5実施形態]
図9は、本発明の第5実施形態に係るステントデリバリーシステム210の一部省略側面図である。図9では、図示の便宜上、ステントデリバリーシステム210の軸方向の途中部分の図示を省略すると共に、ステントデリバリーシステム210の基端側(グリップ218側)に対して先端側(ステント212側)を大きく描いている。なお、この点は、第6及び第7実施形態を示す図14、図15においても同様である。
図9は、本発明の第5実施形態に係るステントデリバリーシステム210の一部省略側面図である。図9では、図示の便宜上、ステントデリバリーシステム210の軸方向の途中部分の図示を省略すると共に、ステントデリバリーシステム210の基端側(グリップ218側)に対して先端側(ステント212側)を大きく描いている。なお、この点は、第6及び第7実施形態を示す図14、図15においても同様である。
ステントデリバリーシステム210は、血管、胆管、気管、食道、尿道等の生体管腔内の所望位置にステント212を送達し、留置するための医療器具である。ステントデリバリーシステム210は、長尺状の内側チューブ214(内側部材)と、内側チューブ214の外側に配置された外側チューブ216(シース)と、内側チューブ214と外側チューブ216との間に配置された自己拡張可能なステント212と、外側チューブ216の基端側に設けられたグリップ218とを備える。
図9におけるステントデリバリーシステム210に関し、グリップ218が設けられた側又は方向を「基端側」又は「基端方向」と呼び、その反対側又は反対方向を「先端側」又は「先端方向」と呼び、他の各図についても同様とする。
図10に示すように、内側チューブ214は、ガイドワイヤ220を挿通するためのガイドワイヤルーメン222が形成された内側先端チューブ224と、内側先端チューブ224の基端側に連結部材226を介して連結された内側基端チューブ228とを有する。内側チューブ214の先端、すなわち内側先端チューブ224の先端は、外側チューブ216の先端よりも先端側に突出している。
内側先端チューブ224の先端には先端方向に開口する先端開口224aが形成される。内側先端チューブ224の基端は、当該内側先端チューブ224の径方向外側に向かって湾曲しており、径方向外方に開口した基端開口224bが形成される。ステントデリバリーシステム210の初期状態(図10の状態)において、基端開口224bは、後述するガイドワイヤ導出孔244と連通する。なお、ガイドワイヤ220は、例えば、ステントデリバリーシステム210を生体管腔内の病変部に導くために用いられる。
内側先端チューブ224の先端部には、径方向外方に膨出し、外側チューブ216の先端方向への移動を規制するストッパ部232が形成される。ストッパ部232が外側チューブ216の先端方向への移動を規制することにより、外側チューブ216が内側チューブ214の先端に対して先端方向に突出することが阻止される。
連結部材226は、内側先端チューブ224の基端と、内側基端チューブ228の先端に連結される。内側基端チューブ228は、その先端から基端まで貫通するルーメン234を有する。図9に示すように、内側基端チューブ228は、グリップ218内で外側チューブ216の基端よりも基端方向に突出し、グリップ218内を直線状に延在する。
内側基端チューブ228の基端部は、グリップ218の基端部に設けられたコネクタ236に接続される。コネクタ236には、生理食塩水等の液体をステントデリバリーシステム210に供給するための液体注入器が接続可能である。内側基端チューブ228の基端部と、コネクタ236とは固定されており、これにより、内側チューブ214は、グリップ218に対して軸方向に移動しないように固定されている。
このような内側チューブ214は、樹脂材料、金属材料等により構成され得る。なお、内側先端チューブ224は、内側基端チューブ228よりも可撓性の高い材料で構成されるとよい。
外側チューブ216は、内側チューブ214及びグリップ218に対して軸方向に移動可能に設けられる。本図示例において、外側チューブ216は、外側先端チューブ238と、この外側先端チューブ238の基端に連結された外側基端チューブ240とを有する。外側先端チューブ238の内部には、内側チューブ214の内側先端チューブ224が配置される。
外側基端チューブ240の内部には、内側チューブ214の内側基端チューブ228が配置される。外側チューブ216の基端部、すなわち外側基端チューブ240の基端部は、グリップ218内に挿入される。
外側先端チューブ238の先端は、生体管腔内の病変部にステント212を留置する際のステント212の放出口として機能すると共に、途中まで放出されたステント212を回収(再収納)する際の収納口としても機能する。
外側先端チューブ238の基端近傍には、外側先端チューブ238の内外を連通するガイドワイヤ導出孔244が形成される。図10に示すように、ステントデリバリーシステム210の初期状態において、内側先端チューブ224の基端開口224bとガイドワイヤ導出孔244とは連通する。従って、内側先端チューブ224の先端開口224aを介してガイドワイヤ220をガイドワイヤルーメン222に挿入し、さらに、基端開口224b及びガイドワイヤ導出孔244を介してガイドワイヤ220を外側チューブ216の外部に導出することができる。
図10のように、外側先端チューブ238の先端部の外周面には、X線(放射線)不透過材料からなる造影マーカー242が設けられるとよい。
ステント212は、略円筒形状且つ自己拡張可能に形成されている。具体的には、ステント212は、ステントデリバリーシステム210の初期状態において、内側チューブ214と外側チューブ216との間に収縮状態で配置される。すなわち、ステント212は、外側チューブ216によって拡張が規制されて半径内方向に圧縮された状態で、外側チューブ216内に収納される。また、ステント212は、外側チューブ216が内側チューブ214に対して基端方向に移動することに伴って、外側チューブ216から放出されることにより、自己の弾性復元力によって半径外方向に拡張する。ステント212の長さLsは、治療対象部位によって様々であるが、例えば、10~300mm程度、好ましくは40~250mm程度である。
なお、ステント212の構成としては、例えば、多数の側孔が設けられたメッシュ状の構成、円形の環状体を軸方向に多数連結した構成、周方向にジグザグ状に延在する環状体を軸方向に多数連結した構成等が挙げられる。ステント212を構成する材料としては、例えば、Ni-Ti合金等の超弾性合金が好適である。ステント212の先端及び基端には、例えば、X線不透過材料からなる造影マーカーが設けられるとよい。
図10に示すように、内側チューブ214(具体的には、内側先端チューブ224)の外周部には、ステント212の軸方向への移動を規制するステント保持機構246が設けられる。ステント保持機構246は、ステント212の基端方向への移動を阻止する後退阻止ストッパ247と、ステント212の先端方向への移動を阻止する前進阻止ストッパ248とを有する。
具体的には、後退阻止ストッパ247は、ステント212を放出するために内側チューブ214に対して外側チューブ216を基端方向に移動させる際にステント212の縮径した基端部に係合することで、ステント212の後退移動を阻止する。前進阻止ストッパ248は、途中まで放出されたステント212を外側チューブ216内に再収納するために内側チューブ214に対して外側チューブ216を先端方向に移動させる際にステント212の縮径した基端部に係合することで、ステント212の前進移動を阻止する。
図9において、ステントデリバリーシステム210の基端部分を構成するグリップ218は、使用者が手で把持して操作するためのハンドルとして機能する部分である。グリップ218は、中空状のハウジング250を有し、ステントデリバリーシステム210の軸方向(前後方向)にやや長く構成されている。当該ハウジング250内に、内側チューブ214に対して外側チューブ216を軸方向に移動させるための移動機構252が設けられる。
図9及び図11に示すように、移動機構252は、外側チューブ216に接続されステントデリバリーシステム210の軸方向に移動可能であると共に当該軸方向に延在するラック部材254(変位部材)と、ラック部材254の延在方向に互いに間隔をおいて配置され、回転に伴ってラック部材254を延在方向に移動させる複数(図示例では3つ)のピニオン256(駆動回転体)と、各ピニオン256が設けられた複数(図示例では3つ)の操作ホイール258とを有する。なお、ピニオン256は2つ又は4つ以上設けられてもよい。この場合、操作ホイール258も、2つ又は4つ以上設けられてもよい。
ラック部材254は、ハウジング250内に設けられた図示しないガイド部によってステントデリバリーシステム210の軸方向(グリップ218の前後方向)に摺動可能に支持される。ラック部材254は、外側チューブ216の基端部に固定されているため、ラック部材254に連動して、外側チューブ216も移動する。ラック部材254(図示例ではラック部材254の下面)には、ラック部材254の軸方向に沿って複数の歯部からなるラック255が設けられる。
なお、ラック部材254の位置確認手段として、ハウジング250の一部又は全部を、透明性を有する材料で構成し、あるいは、ハウジング250の一部にラック部材254を露出させる開口を設け、ハウジング250内のラック部材254の位置をハウジング250の外側から視認できるようにしてもよい。あるいは、位置確認手段の別の構成として、ラック部材254に突起を設け、この突起をハウジング250に設けたスリットを介してハウジング250から突出させ、突起の位置によってラック部材254の位置が分かる構成としてもよい。
各ピニオン256(256a~256c)は、ラック部材254のラック255に噛合可能である。3つのピニオン256の回転軸心は、互いに平行である。また、3つのピニオン256の回転軸心は、ラック部材254の移動可能方向(延在方向)に平行な1つの直線上に存在する。
図9において、3つのピニオン256a~256cにおける隣接する2つのピニオン256の中心間距離L9はすべて等しい。ラック部材254の長さL8(ラック255の長さ)は、ピニオン256の中心間距離L9よりも大きい。このため、ラック255は、少なくとも隣接する2つのピニオン256と噛み合うことが可能である。3つのピニオン256a~256cにおいて、歯数及びモジュールは、すべて同じである。なお、モジュール(m)は、ピッチ円直径(d)を歯数(z)で除した値である。
これら複数のピニオン256が、複数の操作ホイール258(258a~258c)の中央部にそれぞれ設けられる。操作ホイール258a~258cは、グリップ218の前後方向に間隔をおいて配置される。図11に示すように、各操作ホイール258の外周部には、溝260と突部261とが周方向に交互に配置された滑り防止形状が設けられる。各操作ホイール258は、その軸方向に突出する軸部262を有する。各軸部262は、ハウジング250の内側に設けられた軸受部によって支持される。これにより、各操作ホイール258は、ハウジング250に回転可能に支持される。
図9において、ハウジング250には、各操作ホイール258を部分的に露出させるための複数の開口部264が設けられる。使用者は、操作ホイール258のうち開口部264から露出した部分に触れて、操作ホイール258を回転操作することができる。本図示例では、ハウジング250における同じ側(上側)にすべての開口部264が設けられる。
グリップ218には、複数の操作ホイール258のうち最も先端側に配置された操作ホイール258aの回転動作を規制することにより、ラック部材254の移動動作を規制可能なロック機構266が設けられる。ロック機構266は、ハウジング250の側面にスライド可能に設けられたスライド部材268を有する。スライド部材268は、操作ホイール258の回転動作を阻止するロック位置と、操作ホイール258の回転動作を許容する解除位置の間を移動可能である。スライド部材268には、ハウジング250の内方に向けて突出したロックピン(図示せず)が設けられる。
一方、操作ホイール258aには、ロックピンに係合可能な図示しない係合部(例えば溝部)が設けられる。スライド部材268がロック位置にあるとき、ロックピンが係合部と係合することにより、操作ホイール258aの回転動作が阻止される。一方、スライド部材268が解除位置にあるとき、ロックピンが係合部から離脱することにより、操作ホイール258aの回転動作が許容される。
本実施形態に係るステントデリバリーシステム210は、基本的には以上のように構成されるものであり、以下、その作用及び効果について説明する。
ステントデリバリーシステム210を用いて生体管腔内(例えば、血管内)に生じた病変部(例えば、狭窄部)を治療する手技では、ステントデリバリーシステム210の生体管腔内への挿入に先立って、ガイドワイヤ220を生体管腔内に挿入する。そして、ガイドワイヤ220の先端を、生体管腔内の病変部まで到達させた状態とする。
このような状態としたら、生体外にあるステントデリバリーシステム210における内側チューブ214及び外側チューブ216内を所定の液体(例えば、生理食塩水等)で満たすプライミングを行う。プライミングでは、先ず、術者がグリップ218の基端部に設けられたコネクタ236に対して液体注入具(図示せず)を接続し、当該液体注入具からコネクタ236へと生理食塩水等の液体を注入する。これにより、液体が内側チューブ214及び外側チューブ216の先端側へと流れ、内側チューブ214及び外側チューブ216の各先端から流出する。これにより、生体外における内側チューブ214及び外側チューブ216の内部のプライミングが完了する。
次に、生体外に露呈しているガイドワイヤ220の基端を、図10に示されるように、内側チューブ214の先端からガイドワイヤルーメン222へと挿通させ、ガイドワイヤ導出孔244を通じて外側チューブ216の外部へと導出させる。そして、ガイドワイヤ220に沿って内側チューブ214及び外側チューブ216を生体管腔内へと進めていく。
そして、外側チューブ216の先端が病変部に到達したことを造影マーカー242によって確認した後、グリップ218に設けられたスライド部材268を先端側へと移動させることにより、操作ホイール258aの回転規制状態を解除する。この時点では、図12Aのように、ラック部材254は、その可動範囲の最も先端側に位置している。図12Aにおいて、ステント212を完全に外側チューブ216の先端から放出する状態となったときのラック部材254の位置を点線で示している。参照符号L10は、ステント212を完全に外側チューブ216の先端から放出させるためのラック部材254の必要移動距離である。
操作ホイール258aの回転規制状態を解除したら、操作ホイール258aのうち、開口部264を介してハウジング250から露出した部分に触れて、操作ホイール258aを所定方向(矢印A方向)へと回転させる。これにより、操作ホイール258aに設けられたピニオン256aの回転に伴って、ラック部材254がハウジング250内で基端側へと移動し、これにより、ラック部材254に連結された外側チューブ216も、グリップ218の基端方向に移動していく。
この結果、外側チューブ216が内側チューブ214に対して基端方向に移動することに伴って、外側チューブ216内に収容されていたステント212(図9参照)が、先端部側から徐々に露出され始めるのと同時に半径方向外方に拡張し始める。すなわち、外側チューブ216の基端方向への移動に伴って、ステント212が放出されていく。なお、ステント212を途中まで放出させた段階で、生体管腔内でのステント212の位置を調整したい場合には、操作ホイール258aを逆方向(矢印B方向)に回転操作すればよい。これにより、外側チューブ216が先端方向へと移動し、ステント212を外側チューブ216内に再収納することができる。
操作ホイール258aの回転操作に伴ってラック部材254が基端方向へと移動していくと、やがてラック部材254のラック255は、図12Bのように、1つ基端側に設けられたピニオン256bと噛み合う。このとき、ステント212は途中(全長の3分の1程度)まで放出された状態である。ラック部材254のラック255が、ピニオン256bと噛み合ったら、当該ピニオン256bが設けられた操作ホイール258bを所定の方向(A方向)に回転操作することにより、外側チューブ216を基端方向に移動させ、ステント212をさらに放出させていく。
なお、操作ホイール258aの回転操作中に、1つ基端側の操作ホイール258bに設けられたピニオン256bがラック部材254と噛み合うと、当該操作ホイール258bが連動して回転させられる。これにより、使用者は、ラック部材254のラック255がピニオン256bと噛み合ったことを認識することができる。また、操作ホイール258aの回転操作に伴ってラック部材254が基端方向へと移動していくと、やがてピニオン256aとラック255との噛み合いが外れ、操作ホイール258aが空回りすることになる。このため、使用者は、ラック部材254が次のピニオン256bと噛み合ったことを認識し、次の操作ホイール258bを操作すべきことを認識することができる。また、ラック部材254の位置を視認できる位置確認手段がグリップ218に設けられる場合には、使用者はラック部材254の位置を見ることで、ラック部材254が次のピニオン256bと噛み合ったことを認識し、次の操作ホイール258bを操作すべきことを認識することができる。
操作ホイール258bの回転操作に伴ってラック部材254が基端方向へと移動していくと、やがてラック部材254のラック255は、図12Cのように、その1つ基端側に設けられたピニオン256cと噛み合う。このとき、ステント212は未だ途中(全長の3分の2程度)まで放出された状態である。ラック部材254のラック255が、ピニオン256cと噛み合ったら、当該ピニオン256cが設けられた操作ホイール258cを所定の方向(A方向)に回転操作することにより、外側チューブ216を基端方向に移動させ、ステント212をさらに放出させていく。
そして、ステント212が、外側チューブ216に対して完全に露出した状態(ステント212の全長が放出された状態)となる。これにより、ステント212は円筒状に拡張した状態で病変部に留置されるに至る。
その後、ステントデリバリーシステム210を構成する内側チューブ214及び外側チューブ216を基端側へと引くことにより、ステント212のみを生体管腔内に残して、内側チューブ214及び外側チューブ216を生体外へと抜去する。
以上説明したように、本発明の第5実施形態に係るステントデリバリーシステム210によれば、ラック部材254の延在方向に沿って互いに間隔をおいて設けられた複数のピニオン256(256a~256c)により、ラック部材254を基端方向に順次送るようになっている。このため、ラック部材254の長さL8は、ラック部材254の必要移動距離L10(図12A参照)よりも相当に短い。これに対し、1つのピニオンによりラック部材を移動させる従来構成の場合、当該ラック部材の長さは、少なくともその必要移動距離以上に設定される必要があるため、ラック部材の長さが相当に長くなり、ラック部材が収容されるグリップも長くならざるを得ない。
このように、ステントデリバリーシステム210によれば、長いステント212を放出する構成の場合でも、ラック部材254の全長を長くすることなく、ラック部材254の必要移動距離L10を確保することができる。このため、ラック部材254の長さを短くできる分、グリップ218の長さを節約することができ、手技中のグリップ218の取り扱い易さを向上できる。
また、ラック部材254は、グリップ218内に設けられるものであるが、上述したように、ステントデリバリーシステム210では、ラック部材254の位置がグリップ218の外側から視認可能となるように構成してもよい。このように構成した場合、回転させるべきピニオン256、すなわち操作ホイール258を変更するタイミングを容易に知ることができ、効率的な操作でステント212の放出手技を行うことができる。
なお、上述した実施形態では、グリップ218の軸方向に移動する変位部材としてラック部材254を例示すると共に、複数の駆動回転体として複数のピニオン256を例示したが、当該変位部材と当該駆動回転体は、ラック255とピニオン256との噛み合いにより動力を伝達する構成に限られない。例えば、変位部材の外側面と、駆動回転体の外周面との接触による摩擦抵抗により動力を伝達する構成であってもよく、この点は、後述する変形例、第6及び第7実施形態においても同様である。
図13A~図13Cは、変形例に係るステントデリバリーシステム210aを示す模式図である。なお、図13A~図13Cでは、グリップ218を簡略的に示しているが、図9に示したグリップ218と同様に、把持に適した形状を有し、且つ複数の操作ホイール258(258d、258e)が部分的にグリップ218から露出している。
この変形例に係るステントデリバリーシステム210aでは、ステント212(212a、212b)が内側チューブ214及び外側チューブ216の軸方向に沿って複数(図示例では、2つ)並べて配置されている。
また、ステントデリバリーシステム210aは、内側チューブ214に対して外側チューブ216を基端方向に移動させるための移動機構252aを備える。移動機構252aにおいて、操作ホイール258は、ステント212と同数の複数個、すなわち本変形例では2つ設けられる。各操作ホイール258に、ラック部材254と噛合可能なピニオン256(256d、256e)が設けられる。ステントデリバリーシステム210aでは、先端側から順にピニオン256を回転させる毎に、先端側から順にステント212が外側チューブ216から放出されるようになっている。
具体的には、図13Aのように、ラック部材254が初期位置(可動範囲の最先端位置)にあるとき、すべてのステント212は収縮状態で外側チューブ216内に収納されている。1つ目の操作ホイール258dを回転操作してピニオン256dを回転させ、ラック部材254と共に外側チューブ216を内側チューブ214に対して基端方向に移動させると、1つ目のステント212aが外側チューブ216から放出されていく。そして、図13Bのように、ラック部材254が所定位置まで後退した段階で、1つ目のステント212aが完全に外側チューブ216から放出される。このとき、ラック部材254のラック255は、1つ目の操作ホイール258dに設けられたピニオン256dと噛み合わないため、これ以上操作ホイール258dを回転操作しても、操作ホイール258dは空回りする。
1つ目のステント212aが放出された段階で、ラック部材254のラック255は、2つ目の操作ホイール258eに設けられたピニオン256eと噛み合っている。そこで、次に、2つ目の操作ホイール258eを回転操作することにより、ピニオン256eの回転に伴ってラック部材254と共に外側チューブ216を内側チューブ214に対して基端方向に移動させる。これにより、図13Cのように、2つ目のステント212bが外側チューブ216から放出される。
なお、ステントデリバリーシステム210a(内側チューブ214の先端)の位置を変えずに、2つのステント212a、212bを連続的に放出してもよい。あるいは、1つ目のステント212aを放出した後に、ステントデリバリーシステム210a(内側チューブ214の先端)の位置を移動し、生体管腔内の別の箇所で2つ目のステント212bを放出してもよい。
ステントデリバリーシステム210aでは、軸方向に沿って3つ以上のステント212が並べて配置されてもよい。この場合、ステント212の個数と同数の3つ以上のピニオン256及び操作ホイール258が設けられ、操作ホイール258の回転操作毎に、ステント212が順次、外側チューブ216から放出される。
変形例に係るステントデリバリーシステム210aによれば、ラック部材254の延在方向に沿って互いに間隔をおいて設けられた複数のピニオン256により、ラック部材254を基端方向に順次送るようになっている。この構成により、1つのピニオンによりラック部材を移動させる従来構成と比較して、ラック部材254の全長を短くすることができる。従って、軸方向に並んだ複数のステント212を放出する構成の場合でも、ラック部材254の全長を長くすることなく、ラック部材254の必要移動距離L10を確保することができる。このため、ラック部材254の長さを短くできる分、グリップ218の長さを節約することができ、手技中のグリップ218の取り扱い易さを向上できる。
[第6実施形態]
図14は、本発明の第6実施形態に係るステントデリバリーシステム270の一部省略側面図である。図14では、内側チューブ214の基端部及びグリップ272(ハウジング250)を仮想線で示している。なお、第6実施形態に係るステントデリバリーシステム270において、第5実施形態に係るステントデリバリーシステム210と同一又は同様な機能及び効果を奏する要素には同一の参照符号を付し、詳細な説明を省略する。
図14は、本発明の第6実施形態に係るステントデリバリーシステム270の一部省略側面図である。図14では、内側チューブ214の基端部及びグリップ272(ハウジング250)を仮想線で示している。なお、第6実施形態に係るステントデリバリーシステム270において、第5実施形態に係るステントデリバリーシステム210と同一又は同様な機能及び効果を奏する要素には同一の参照符号を付し、詳細な説明を省略する。
このステントデリバリーシステム270は、内側チューブ214に対して外側チューブ216を基端方向に移動させるための移動機構252bを有するグリップ272を備える。移動機構252bは、ラック部材254と、複数のピニオン282、284と、複数の操作ホイール278、280(回転操作部)とを有する。ラック部材254には、互いに反対側(図示例では、上側と下側)に第1ラック274(第1被駆動部)及び第2ラック276(第2被駆動部)が設けられる。
複数の操作ホイール278、280は、ラック部材254の延在方向(軸方向)に間隔をおいて配置され回転可能な複数(図示例では、2つ)の第1操作ホイール278(第1回転操作部)と、ラック部材254の延在方向(軸方向)に間隔をおいて配置され回転可能な複数(図示例では、3つ)の第2操作ホイール280(第2回転操作部)とを含む。
複数の第1操作ホイール278(278a、278b)と、複数の第2操作ホイール280(280a~280c)とは、グリップ272における互いに反対側の外側面(図14では、上面と下面)において露出する。具体的には、複数の第1操作ホイール278に対応して、グリップ272の上面に複数の開口部264が設けられ、開口部264を介して各第1操作ホイール278が部分的に露出する。また、複数の第2操作ホイール280に対応して、グリップ272の下面に複数の開口部264が設けられ、開口部264を介して各第2操作ホイール280が部分的に露出する。
第1操作ホイール278には、第1ラック274と噛合可能な第1ピニオン282(駆動回転体)が設けられる。ラック部材254の長さ(第1ラック274及び第2ラック276の長さ)L8は、第1ピニオン282同士の中心間距離L11よりも大きい。第2操作ホイール280には、第2ラック276と噛合可能な第2ピニオン284(駆動回転体)が設けられる。ラック部材254の長さL8は、第2ピニオン284同士の中心間距離L12よりも大きい。ラック部材254の可動範囲内で、ラック部材254は、少なくとも1つの第1ピニオン282と噛み合うと共に、少なくとも1つの第2ピニオン284と噛み合う。
本実施形態では、第2ピニオン284の歯数とモジュールの積は、第1ピニオン282の歯数とモジュールの積よりも大きい。このため、第2ピニオン284の単位回転角度当たりのラック部材254の移動距離は、第1ピニオン282の単位回転角度当たりのラック部材254の移動距離よりも大きい。すなわち、回転角度が同じである場合、第2ピニオン284の駆動によるラック部材254の移動距離は、第1ピニオン282の駆動によるラック部材254の移動距離よりも大きい。
従って、第1操作ホイール278に対する回転操作速度と、第2操作ホイール280に対する回転操作速度とが同じである場合、第1操作ホイール278を回転操作した際には、ラック部材254と共に移動する外側チューブ216の移動速度は相対的に遅く、第2操作ホイール280を回転操作した際には、ラック部材254と共に移動する外側チューブ216の移動速度は相対的に速い。
なお、複数の第1ピニオン282(282a、282b)において、歯数及びモジュールは同じであり、複数の第2ピニオン284(284a~284c)において、歯数及びモジュールは同じである。第1ピニオン282と第2ピニオン284の各モジュールは同じでもよいが、この場合、図14のように、第2ピニオン284の歯数は、第1ピニオン282の歯数よりも多い。あるいは、第1ピニオン282の歯数と第2ピニオン284の歯数が同じで、第2ピニオン284のモジュールが、第1ピニオン282のモジュールよりも大きくてもよい。あるいは、第2ピニオン284の歯数及びモジュールが、それぞれ第1ピニオン282の歯数及びモジュールよりも大きくてもよい。
このように構成されるステントデリバリーシステム270によれば、第1操作ホイール278又は第2操作ホイール280を先端側から順に回転操作することにより、複数のピニオン282、284が回転してラック部材254を順次、基端方向に送り、内側チューブ214に対して外側チューブ216を基端方向に移動させることができる。外側チューブ216の基端方向への移動に伴って、ステント212がその先端側から徐々に放出され、図14において点線で描かれた位置までラック部材254が移動した段階では、ステント212は外側チューブ216の先端から完全に放出される。
この場合、第1ピニオン282と第2ピニオン284とで、ラック部材254を移動させる速度が異なる。このため、ステント212を放出させたい速度に応じて、第1操作ホイール278又は第2操作ホイール280を回転操作して、第1ピニオン282又は第2ピニオン284を回転させることにより、外側チューブ216からのステント212の放出速度を容易に制御することができる。
例えば、ステント212をゆっくり放出させたい場合には、第1操作ホイール278を回転操作して歯数が少ない第1ピニオン282によりラック部材254を駆動することにより、外側チューブ216を低速で基端方向に移動させればよい。一方、ステント212を素早く放出させたい場合には、第2操作ホイール280を回転操作して歯数が多い第2ピニオン284によりラック部材254を駆動することにより、外側チューブ216を高速で基端方向に移動させればよい。
また、例えば、長いステント212の場合、ステント212の放出初期では、先端側の第1操作ホイール278aを回転操作して第1ピニオン282aによりラック部材254を駆動することにより、外側チューブ216を基端方向に移動させる。そうすると、ステント212がゆっくり放出され、位置決めがし易い。一方、ステント212の放出終期では、基端側の第2操作ホイール280cを回転操作して第2ピニオン284cによりラック部材254を駆動することにより、外側チューブ216を基端方向に移動させる。そうすると、ステント212が素早く放出され、早く手技を終えることができる。
第5実施形態と同様に、第6実施形態においても、ラック部材254の延在方向に間隔をおいて配置された複数のピニオン282、284によりラック部材254が駆動される。このため、ラック部材254の必要移動距離L10に対してラック部材254の長さL8を相当に短くできる。従って、長いステント212や軸方向に並んだ複数のステント212a、212b(図13A~図13C参照)を放出する構成の場合でも、ラック部材254を長大化する必要がない。この結果、グリップ272の長さを節約でき、手技中のグリップ272の取り扱い易さを向上できる。
また、本実施形態によれば、第1ピニオン282と第2ピニオン284とで、ラック部材254を移動させる速度が異なる。このため、ステント212を放出させたい速度に応じて、第1ピニオン282又は第2ピニオン284を回転させることにより、ステント212の外側チューブ216からの放出速度を容易に制御することができる。
また、本実施形態によれば、第1ピニオン282と第2ピニオン284とがラック部材254を基準として互いに反対側に配置されるため、複数ずつ設けられる第1ピニオン282と第2ピニオン284とをグリップ272内に効率的に配置できる。
さらに、本実施形態によれば、第1操作ホイール278と第2操作ホイール280とが互いに異なる側でグリップ272から露出するため、使用者にとって、第1操作ホイール278と第2操作ホイール280の区別が容易であり、使い分けがし易い。
その他、第6実施形態において、第5実施形態と共通する各構成部分については、第5実施形態における当該共通の各構成部分がもたらす作用及び効果と同一又は同様の作用及び効果が得られることは勿論である。
なお、図14に示す構成では、第2ピニオン284の歯数とモジュールの積は、第1ピニオン282の歯数とモジュールの積よりも大きいが、この構成とは逆に、第1ピニオン282の歯数とモジュールの積が、第2ピニオン284の歯数とモジュールの積よりも大きくてもよい。この場合、第1ピニオン282が設けられた第1操作ホイール278を回転操作すると、ステント212が素早く放出され、第2ピニオン284が設けられた第2操作ホイール280を回転操作すると、ステント212がゆっくり放出される。
また、図14に示す構成では、ラック部材254に2つのラック(第1ラック274と第2ラック276)が設けられ、複数の第1ピニオン282と複数の第2ピニオン284がそれぞれラック部材254の互いに反対側に配置されているが、以下のように構成されてもよい。すなわち、ラック部材254には1つのラックのみが設けられ、複数の第1ピニオン282と複数の第2ピニオン284とがラック部材254に対して同じ側に設けられてもよい。この場合、複数の第1ピニオン282と複数の第2ピニオン284とはラック部材254の軸方向に沿って交互に配置される。
[第7実施形態]
図15は、本発明の第7実施形態に係るステントデリバリーシステム290の一部省略側面図である。図15では、内側チューブ214の基端部及びグリップ292(ハウジング250)を仮想線で示している。なお、第7実施形態に係るステントデリバリーシステム290において、第5実施形態に係るステントデリバリーシステム210と同一又は同様な機能及び効果を奏する要素には同一の参照符号を付し、詳細な説明を省略する。
図15は、本発明の第7実施形態に係るステントデリバリーシステム290の一部省略側面図である。図15では、内側チューブ214の基端部及びグリップ292(ハウジング250)を仮想線で示している。なお、第7実施形態に係るステントデリバリーシステム290において、第5実施形態に係るステントデリバリーシステム210と同一又は同様な機能及び効果を奏する要素には同一の参照符号を付し、詳細な説明を省略する。
このステントデリバリーシステム290は、内側チューブ214に対して外側チューブ216を基端方向に移動させるための移動機構252cを有するグリップ292を備える。移動機構252cは、ラック部材254と、複数(図示例では、3つ)のピニオン299と、複数(図示例では、3つ)の操作ホイール294とを有する。ラック部材254には、互いに反対側(図示例では、上側と下側)に第1ラック274及び第2ラック276が設けられる。
複数の操作ホイール294は、ラック部材254の延在方向(軸方向)に間隔をおいて回転可能に配置される。複数の操作ホイール294は、第1ラック274を介してラック部材254を駆動する第1操作ホイール296と、第2ラック276を介してラック部材254を駆動する第2操作ホイール298とを含む。図示例では、2つの第1操作ホイール296が設けられると共に、1つの第2操作ホイール298が設けられる。
第1操作ホイール296と、第2操作ホイール298とは、グリップ292における互いに反対側の外側面(図15では、上面と下面)において露出する。具体的には、複数の第1操作ホイール296に対応して、グリップ292の上面に複数の開口部264が設けられ、開口部264を介して各第1操作ホイール296が部分的に露出する。また、第2操作ホイール298に対応して、グリップ292の下面に開口部264が設けられ、当該開口部264を介して第2操作ホイール298が部分的に露出する。
第1操作ホイール296には、第1ラック274と噛合可能な第1ピニオン300が設けられる。第2操作ホイール298には、第2ラック276と噛合可能な第2ピニオン302が設けられる。第2ピニオン302の歯数とモジュールの積は、第1ピニオン300の歯数とモジュールの積よりも大きい。このため、第2ピニオン302の単位回転角度当たりのラック部材254の移動距離は、第1ピニオン300の単位回転角度当たりのラック部材254の移動距離よりも大きい。すなわち、回転角度が同じ場合、第2ピニオン302の駆動によるラック部材254の移動距離は、第1ピニオン300の駆動によるラック部材254の移動距離よりも大きい。
従って、第1操作ホイール296に対する回転操作速度と、第2操作ホイール298に対する回転操作速度とが同じである場合、第1操作ホイール296を回転操作した際には、ラック部材254と共に移動する外側チューブ216の移動速度は相対的に遅く、第2操作ホイール298を回転操作した際には、ラック部材254と共に移動する外側チューブ216の移動速度は相対的に速い。
複数の第1ピニオン300において、歯数及びモジュールは同じである。第1ピニオン300と第2ピニオン302の各モジュールは同じでもよいが、この場合、図15のように、第2ピニオン302の歯数は、第1ピニオン300の歯数よりも多い。あるいは、第1ピニオン300の歯数と第2ピニオン302の歯数が同じで、第2ピニオン302のモジュールが、第1ピニオン300のモジュールよりも大きくてもよい。あるいは、第2ピニオン302の歯数及びモジュールが、それぞれ第1ピニオン300の歯数及びモジュールより大きくてもよい。
ラック部材254の長さ(ラック274、276の長さ)L8は、第1ピニオン300同士の中心間距離L13よりも大きい。また、ラック部材254の長さL8は、基端側の第1ピニオン300と第2ピニオン302との中心間距離L14よりも大きい。従って、ラック部材254の可動範囲内で、ラック部材254は、複数のピニオン300、302のうち、少なくともグリップ292の軸方向(ラック部材254の延在方向)に隣接する2つのピニオンと噛み合う。なお、第1ピニオン300同士の中心間距離L13と、基端側の第1ピニオン300と第2ピニオン302との中心間距離L14とは、同じでもよく、異なっていてもよい。
このように構成されるステントデリバリーシステム290によれば、複数の操作ホイール294を先端側から順に回転操作することにより、複数のピニオン300、302が回転してラック部材254を順次、基端方向に送り、内側チューブ214に対して外側チューブ216を基端方向に移動させることができる。外側チューブ216の基端方向への移動に伴って、ステント212がその先端側から徐々に放出され、図15において点線で描かれた位置までラック部材254が移動した段階では、ステント212は外側チューブ216の先端から完全に放出される。
この場合、ステント212の放出初期では、先端側の第1操作ホイール296を回転操作して先端側の第1ピニオン300によりラック部材254を低速で基端方向に移動させるため、ステント212がゆっくり放出され、位置決めがし易い。このように、先端側の第1ピニオン300は、ステント212の放出初期においてラック部材254を移動させる初期放出用駆動回転体として機能する。
一方、ステント212の放出終期では、第2操作ホイール298を回転操作して第2ピニオン302により高速でラック部材254を基端方向に移動させるため、ステント212が素早く放出され、早く手技を終えることができる。このように、第2ピニオン302は、ステント212の放出終期においてラック部材254を移動させる終期放出用駆動回転体として機能する。
第5実施形態と同様に、第7実施形態においても、ラック部材254の延在方向に間隔をおいて配置された複数のピニオン300、302によりラック部材254が駆動される。このため、ラック部材254の必要移動距離L10に対してラック部材254の長さL8を相当に短くできる。従って、長いステント212や軸方向に並んだ複数のステント212a、212b(図13A~図13C参照)を放出する構成の場合でも、ラック部材254を長大化する必要がない。この結果、グリップ292の長さを節約でき、手技中のグリップ292の取り扱い易さを向上できる。
また、本実施形態によれば、第1操作ホイール296と第2操作ホイール298とが互いに異なる側でグリップ292から露出するため、使用者にとって、第1操作ホイール296と第2操作ホイール298の区別が容易であり、使い分けがし易い。
その他、第7実施形態において、第5実施形態と共通する各構成部分については、第5実施形態における当該共通の各構成部分がもたらす作用及び効果と同一又は同様の作用及び効果が得られることは勿論である。
図15に示す構成では、ラック部材254に2つのラック(第1ラック274と第2ラック276)が設けられ、第1ピニオン300と第2ピニオン302はラック部材254の互いに反対側に配置されているが、以下のように構成されてもよい。すなわち、ラック部材254には1つのラックのみが設けられ、第1ピニオン300と第2ピニオン302とがラック部材254に対して同じ側に設けられてもよい。
上記において、本発明について好適な実施形態を挙げて説明したが、本発明は前記実施形態に限定されるものではなく、本発明の要旨を逸脱しない範囲において、種々の改変が可能なことは言うまでもない。

前記回転操作部(58、96、98、126)に、前記複数の駆動回転体(56、92、94、124)の1つが設けられている、

前記連動機構(63、70、82、100、128)は、前記複数の駆動回転体(56、92、94、124)の各々の同軸上に設けられた複数の回転部(64、84、110、114、130)と、前記複数の回転部(64、84、110、114、130)に動力伝達可能に接触する少なくとも1つの伝達部材(66、86、112、116、132)とを有する、

前記伝達部材(86)によって前記複数の回転部(84)が連動する際、最も基端側の前記回転部(84c)は、最も先端側の前記回転部(84a)に対して速く回転する、

前記連動機構(100)は、前記複数の第1駆動回転体(92)を連動させる第1連動部(106)と、前記複数の第2駆動回転体(94)を連動させる第2連動部(108)とを有し、
前記複数の第1駆動回転体(92)の単位回転角度当たりの前記変位部材(54)の移動距離と、前記複数の第2駆動回転体(94)の単位回転角度当たりの前記変位部材(54)の移動距離とが異なる、

前記第2連動部(108)は、前記複数の第2駆動回転体(94)の各々の同軸上に設けられた複数の第2回転部(114)と、前記複数の第2回転部(114)に動力伝達可能に接触する少なくとも1つの第2伝達部材(116)とを有する、

前記第1回転操作部(96)と、前記第2回転操作部(98)とは、前記グリップ(78)における互いに反対側の外側面において露出する、
