Abstract
本发明涉及一种处理微量全血的试剂及其应用,所述试剂含有以下成分:缓冲液、渗透压维持剂、保护剂、抗凝剂和防腐剂。本发明提供的试剂可以做到:以任意倍数(2-1000倍)稀释处理微量全血样本,得到血清或血浆的稀释液,而样本不被破坏、红细胞不会溶血。

本发明涉及全血试剂, 具体涉及一种处理微量全血的试剂及其应用。 背景技术
目前, 临床上, 血液样本的检测项目 (如: 25-羟基维生素 D3的检测、 乙 肝两对半的检测等项目) 绝大部分使用的是血清或血浆样本, 其中血清样本 获取方式是先釆集静脉血, 取血后离心, 吸取上清进行检测; 血浆样本的获 取方式是先用含有抗凝剂的釆血管釆集静脉血, 取血后离心, 吸取上清进行 检测。 而现有的制备方法都是釆集静脉血, 然后将全血离心, 分离血清和血 浆, 因此需要的血量比较多。
需要进行检测的人群中可能会有一些特殊人群, 比如某些釆血困难者如: 特殊疾病者、 吸毒人群、 老年人、 婴幼儿, 比如不能很好配合者如: 儿童, 则很难得到满意的、 足够量的全血标本。
因此, 急需一种可釆用较少的全血量, 即可满足临床检测需要的试剂及 方法。
在进行血样分析时, 需要添加一定量的血液稀释剂, 包括适当的电解质 成分, 而且相对于血细胞溶液必须是渗透压平衡的, 为抑制微生物的生长, 还要加防腐剂, 而防腐剂的加入有可能导致血细胞体积的变化, 为平衡或补 偿防腐剂对血细胞体积变化的影响, 还需要加入活化剂, 因血液 pH值一般在 7.4左右, 所以还要求所用的血液稀释剂的 pH值与血细胞溶液的 pH值是平衡
EP0049478 A1公开了一种血液稀释剂, 由平衡渗透剂(氯化钠、 氯化钾、 单和二氢硫酸钠或钾)、 乙二胺四乙酸、 氯化锂和苯乙醇组成, 除了配方成分 较复杂外, 均未解决抗凝问题, 实际使用的血液稀释剂在室温下储放一定时 间后会出现血细胞的凝集现象, 影响血细胞计数和测定的精确性。 另外, 氯 化锂作为红细胞体积稳定剂, 并引入较多的无机盐, 易对测量系统的金属零 部件产生腐蚀产物, 无机盐结晶, 造成导管和计数孔的堵塞。
CN1072020A (申请号为 91109885.2 )公开了一种多用途血液稀释剂, 包 括: 氯化钠、 枸櫞酸钠、 枸櫞酸钾、 庆大霉素。 釆用该发明检测结果偏低或 者是假阴性, 对病人会造成不好的影响。
CN1265196A (申请号为 98807459.1 )公开了一种血液稀释剂, 包括: a: 乙二胺四乙酸、 乙二胺四乙酸衍生物和这两类化合物的组合; b: 咪唑、 咪唑 衍生物 (如苯基咪唑、 甲基咪唑、 乙基咪唑和丁基咪唑)及两类化合物的组 合; c: 碱金属氯化物; d: 第三种化合物, 选自碱金属硫酸盐和碱金属有机 酸盐, pH值呈中性、 细胞稳定剂 (如 4-氨基苯甲酸酯及其衍生物)、 防腐剂、 预防血小板聚集的细胞稳定剂。
US4656139报道了一种用于血液分析的细胞的方法, 包括使用一种稀释 剂稀释血样, 该稀释剂由硼酸缓冲液、 乙二胺四乙酸和 2-硫代 -1-氧吡啶钠。

CN1238455A (申请号为 98102346.0 )公开了一种血液分析计数仪用稀释 液, 含有抗凝剂、 抗菌剂、 缓冲溶剂、 无机盐硫酸钠和氯化钠的水溶液, 其 中缓冲容剂为磷酸氢二钠、磷酸二氢钾; 防腐剂为叠氮化钠;抗凝剂为 EDTA。 该发明所使用保护剂硫酸钠的作用是防止破碎的红细胞碎片黏连, 影响其他 细胞的检测。
CN101485303A(申请号为 200910037434.8 )公开了一种脱落细胞保存液, 包括 pH缓冲剂、 渗透压维持剂、 防腐保鲜剂、 用于维持细胞形态的固定剂、 抗凝剂、 黏液软化剂、 保湿剂及破坏红细胞的成分, 其主要作用是保存细胞、 溶解粘液、 细胞形态完好, 但是破坏了部分红细胞。
CN 1683522A (申请号为 200510018341.2 )公开了一种全血质控物细胞生 物活性保护剂及其制法, 其中该保护剂中的全血细胞活性保养液包括以下成 分: 氯化钠、 硼酸、 硼酸钠、 海藻糖、 海藻酸钠、 葡萄糖、 环状糊精、 牛血 清白蛋白、 叠氮钠。
上述发明专利均是全血细胞稀释液, 其用途在于保护细胞形态、 包括细
胞结构, 但没有保护血液中的其他蛋白和化学成分, 更有的发明破坏了部分 红细胞, 导致在检测过程中影响检测结果, 这不是本发明最终想要解决的问 题。 发明内容
本发明的目的是提供一种处理微量全血的试剂。
本发明提供的一种处理微量全血的试剂, 所述试剂含有以下成分: 缓冲 液、 渗透压维持剂、 保护剂、 抗凝剂和防腐剂。
具体的, 所述 100ml试剂为含有以下成分的水溶液: 浓度为 5-100mM, pH 为 6.8-7.6的缓冲液 0.01-5.0g、 渗透压维持剂 0.5-15g、 保护剂 0.05-3g、 抗凝剂 0.005-lg和防腐剂 0.03-150mg。
优选地, 所述 100ml试剂为含有以下成分的水溶液: 浓度为 5-60mM, pH 为 7.2-7.5的缓冲液 0.02-4.0g、 渗透压维持剂 0.6-11.5g、 保护剂 0.1-lg、 抗凝剂 0.005-0.7g和防腐剂 0.04-100mg。
本发明所述试剂还含有以下成分: 植物血球凝集素或红细胞抗体。
优选地, 本发明所述试剂还包含以下成分: 植物血球凝集素 0.1-0.5g或红 细胞抗体 0.02-0.2ml。
以上试剂中:
所述 mM为 mmol/L的简写。
所述缓冲液为磷酸缓冲系统、 硼酸缓冲系统、 tris-Hcl缓冲系统或巴比妥 缓冲系统, 所述磷酸缓冲系统为磷酸二氢钠和磷酸氢二钠组合、 磷酸二氢钾 和磷酸氢二钾组合; 硼酸缓冲系统为硼酸和氢氧化钠组合、硼酸和硼砂组合; tris-Hcl缓冲系统为 tris ( tris为三羟甲基氨基甲垸的缩写 )和盐酸; 巴比妥缓冲 系统为巴比妥钠和盐酸。
所述渗透压维持剂为氯化钠或蔗糖等; 其中渗透压维持剂的用量为: 氯 化钠为 0.6-0.9g或蔗糖为 10-11.5g。

櫞酸盐等中的一种或几种; EDTA盐为 EDTA二钠盐或 EDTA二钾盐; 肝素盐 为肝素钠、 肝素锂; 草酸盐为草酸钾盐、 草酸铵盐; 枸櫞酸盐为枸櫞酸钠盐 或枸櫞酸钾盐。 所述抗凝剂的用量范围即 100ml血液中的使用量为: EDTA或 其盐为 0.15-0.25g; 肝素或其盐为 0.005-0.2g; 草酸盐为 0.4-0.7g; 枸櫞酸盐为 0.2-0.5g。
所述防腐剂为 proclin300 (其主要成分是 2-甲基 -4-异噻唑啉 -3-酮(MCI ), 5-氯 -2-甲基 -4-异噻唑啉 -3-酮 (CMCI ) )或庆大霉素。
所述水为超纯水, 所述超纯水是指将水中的导电介质几乎完全去除, 又 将水中不离解的胶体物质、 气体及有机物均去除至很低程度的水。 电阻率大 于 18MQ *cm, 或接近 18.2MQ *cm极限值(25°C )。
本发明还提供了上述试剂的制备方法, 该方法包括以下步骤: 按照配比 称取各成分, 加入 80ml水, 混匀, 调节 pH值为 6.8-7.6, 定容, 即得。
上述制备方法中:
所述 pH调节剂为氢氧化钠和盐酸。
本发明还提供了上述试剂在处理微量全血中的应用。 该应用的具体方法 包括以下步骤: 根据临床检测需要的样本量, 从耳垂、 手指末端、 足跟等处 釆取到的微量全血(至少 20ul ), 然后取本发明试剂稀释 2-1000倍, 混匀, 然 后根据检测内容进行相关操作, 比如离心, 取上清液进行检测。
所述釆血量至少为 20 μ 1, 是为了满足临床检测时的最少用量。
所述临床检测是指: 1、 定量免疫试验, 如 25羟基维生素 D3的检测; 2、 定性免疫试验, 如乙肝表面抗原试验; 3、 生化方面的检测, 如 TBil (总胆红 素) 的检测; 4、 ICP-MS (电感耦合等离子体质谱) 的检测。 本发明可以应 用在以上方面的检测, 但不局限在以上的检测。
本发明提供的试剂及其应用具有以下优点:
1、 本发明提供的试剂中:
缓冲液: 提供合适的离子浓度和酸碱度, 保护红细胞不破碎, 维持血浆 恒定 pH值, 防止易变因子失活;
渗透压维持试剂: 提供合适的渗透压, 保持细胞内外的水平衡和细胞的
正常体积形态;
保护剂: 保护血液中含有的蛋白、 保持血液的稳定性;
抗凝剂: 使红细胞不凝固;
防腐剂: 抑制细菌生长, 延长试剂的保存期, 且不影响实验结果。
2、 本发明提供的试剂需要的釆血量可少至 20 μ 1 (这个釆血量为检测时 所需血清或血浆的最小值), 解决了幼儿、 老龄及重病人釆血困难的问题, 是 国内外首创的试剂。
3、 釆血后, 可以根据任意倍数加入本发明的试剂稀释, 得到血清或血浆 的稀释液, 而样本不被破坏、 红细胞不会溶血, 且使用方法方便, 简单, 快 速。 具体实施方式
以下实施例用于说明本发明, 但不用来限制本发明的范围。
本发明中的缓冲液的配制,以实施例 1、 2为例,所述磷酸二氢钠( NaH2P04 2H20 )、 磷酸氢二钠(Na2HPO4 12H20 )是以 5-100mM, pH为 6.8-7.6进行配比, 如 10mM , pH为 7.2 , 应该用 NaH2P04 · 2H20 0.04368g; Na2HP04 · 12H20 0.25776g。
实施例 1: 一种处理微量全血的试剂
1、 组成 ( 100ml ):
2、 制备方法: 取 1个 500ml的烧杯, 加入 80ml的超纯水, 按照配比称取各 成分, 加入到烧杯中, 使其溶解, 并用氢氧化钠或盐酸调节 pH值为 7.4, 定容 至 100ml, 即得。
实施例 2: —种处理微量全血的试剂
1、 组成 ( 100ml )
2、 制备方法: 取 500ml烧杯, 加入 80ml的超纯水, 按照配比称取各成分, 加入到烧杯中, 并分别调节 pH值为: 7.4 (对比例 1和对比例 2)、 6.8 (实施例 2-1)、 7.0 (实施例 2-2)、 7.2 (实施例 2-3)、 7.6 (实施例 2-4), 然后用水定容 至 100ml。
实施例 3: —种处理微量全血的试剂
1、 组成 ( 100ml):

实验例 1: 维生素 D3检测
1、 试剂盒来源: 25羟基维生素 D3 , 购自北京博晖创新光电技术股份有 限公司。
2、 样本来源: 某医院自愿接受检测的患者, 釆取血液。
3、 实验分组:
每个样本釆用 29种方法进行处理, 具体处理方法如下:
对照 1组:常规操作,按照说明书要求样本的获取方式:先釆集静脉血 (取 血量 3-5ml), 离心, 吸取上清液, 用样本稀释液稀释后, 混匀, 取混合液进 行实验。 静脉血要求不得溶血, 要求澄清、 透明。 如 400ul样本稀释液(试 剂盒内自带, 其成分是磷酸缓冲液)加入 lOul的血清, 震荡混匀, 即可进行
实验 1-24组: 实施例 1-6合计 24组, 使用本发明时样本获取方式: 刺 破无名指, 取 20ul的微量全血加入到试管中, 加入 400ul本发明实施例提供 的试剂, 即将 20ul的微量全血稀释 20倍, 震荡混匀, 离心, 取上清即可进 行实验。
对照 2组: CN1072020A (申请号为 91109885.2 ) 的优先选择的组分, 即: 氯化钠 8.10g/l, 枸櫞酸钠 2.98g/l, 枸櫞酸钾 0.82g/l, 庆大霉素 6000u/l ( u为国际单位, 1ιι=1 μ β ), 用双蒸馏水稀释至 1000ml, 配制稀释液来完成 检测。 处理血液的方法同实验组的方法。
对照 3组: CN1238455A (申请号为 98102346.0 ) 的实例 1配置的稀释 液, 即硫酸钠 10.8g、 氯化钠 3.6g、 EDTANa2 lg、 磷酸氢二钠 lg、 硫酸二氢 钾 0.6g、 叠氮钠 0.5g, 用蒸馏水 200g溶解, 继续加蒸馏水使其体积达到 1 升, 即得到稀释液。 处理血液的方法同实验组的方法。
对照 4组: 按照实施例 2中的对比例 1提供的组分, 即 0.3978g
NaH2P04 2H20, 0.8771g Na2HP04 · 12H20, lOg蔗糖, 0.05g HSA, 0.005g 肝素锂, 0.04mg庆大霉素, 按照实施例 2中对比例 1的制备方法进行制备。 处理血液的方法同实验组的方法。
对照 5组: 按照实施例 2中的对比例 2提供的组分, 即 0.1014g
NaH2P04 2H20, 1.5573g Na2HP04 12H20, llg蔗糖, 1.5g HSA, 0.2g肝素 锂, O.lmg庆大霉素, 按照实施例 2中对比例 2的制备方法进行制备。 处理 血液的方法同实验组的方法。
4、 试剂盒操作:
在包被板上设置定标品(2孔*5 )、 质控品(3孔 *2 ), 定标品、 质控品每 孔均加 50ul, 其余孔设为样本孔, 每种样本加两孔, 每孔加入 50ul稀释好的 样本, 所有孔均加 50ul辣根过氧化物酶标记的 25羟基维生素 D3抗原, 严格 按照说明书进行操作。

结果表明: 釆用 25羟基维生素 D3的试剂盒进行检测, 本发明提供的试剂 对微量全血进行处理, 与现有的标准检测方法釆取较大量全血, 检测结果相 同, 方法优于现有技术的其他方法。 实验例 2: 乙肝表面抗原的检测
1、试剂盒来源: 乙肝表面抗原胶体金检测卡, 购自上海科华生物工程股 份有限公司。
2、 样本来源: 某医院自愿接受检测的患者, 釆取血液。
3、 实验分组:
每个样本釆用 29种方法进行处理, 具体处理方法如下:

对于人群来说, 检测结果是 "-" 才是正常的, 否则是病毒感染者, 如果 釆用对照 2组、 对照 4 5组的检测结果, 其检测结果出现差异, 造成检测结果 不准确, 使病人达不到早发现、 早诊断的目的, 造成误诊。
结果表面: 与现有标准方法相比, 釆用本发明稀释微量全血进行检测, 其结果与标准方法相同, 没有统计学差异, 效果优于现有技术。
实验例 3: TBil (总胆红素)的检测
1、试剂盒来源:总胆红素检测试剂盒,购自上海荣盛生物技术有限公司。
2、 样本来源: 某医院自愿接受检测的患者, 釆取血液。
3、 实验分组:
每个样本釆用 29种方法进行处理, 具体处理方法如下:
对照 1组:常规操作,按照说明书要求样本的获取方式:先釆集静脉血 (取 血量 3-5ml), 离心, 吸取上清液 lOOul进行试验。
实验 1-24组: 实施例 1-6合计 24组, 使用本发明时样本获取方式: 刺 破无名指, 取 lOOul的微量全血加入到试管中, 加入 lOOul本发明实施例提 供的试剂, 即将 lOOul的微量全血稀释 2倍, 震荡混匀, 离心, 取上清 lOOul 即可进行实验, 将所得实验结果乘以稀释倍数, 即为原血液样本的含量。
对照 2组: CN1072020A (申请号为 91109885.2 ) 的优先选择的组分, 即: 氯化钠 8.10g/l, 枸櫞酸钠 2.98g/l, 枸櫞酸钾 0.82g/l, 庆大霉素 6000u/l ( u为国际单位, 1ιι=1 μ β ), 用双蒸馏水稀释至 1000ml, 配制稀释液来完成 检测。 处理血液的方法同实验组的方法。
对照 3组: CN1238455A (申请号为 98102346.0 ) 的实例 1配置的稀释 液, 即硫酸钠 10.8g、 氯化钠 3.6g、 EDTANa2 lg、 磷酸氢二钠 lg、 硫酸二氢 钾 0.6g、 叠氮钠 0.5g, 用蒸馏水 200g溶解, 继续加蒸馏水使其体积达到 1 升, 即得到稀释液。 处理血液的方法同实验组的方法。
对照 4组: 按照实施例 2中的对比例 1提供的组分, 即 0.3978g

对于人群来说,谷丙转氨酶的检测值在 3.42-20umol/l之间是正常的, 如果 釆用对照 2和 3组的结果, 会使检测结果偏低, 造成检测结果不准确, 使处于 边缘的病人达不到早发现的目的, 以致于误诊。
6、 统计方法: 各组分别与对照 1组进行配对 T检验, 其统计结果见表 34。

7、 统计结果分析: 查配对 T检验界值表, TaQ5Q9)=2.093 , 上表实验组的 结果均小于 2.093 , 说明釆用现有技术的方法和本发明的方法最终处理结果非 常接近, 没有统计学差异; 而釆用对照 2组和对照 3组的方法进行试验, 其结 果与对照实验方法检测结果有差异(>2.093), 说明对照 2和 3组的发明不同于 本发明; 对于对照 4和 5组来说, 均与对照 1组接近。
结果表明: 釆微量全血后, 用本发明提供的试剂处理, 可以与现有技术
方法釆取较大量血液检测的结果相同,效果优于其他现有技术。对照 2组与本 发明相比, 最大的区别在于没有缓冲系统, 使整个系统没有缓冲作用, 从而 不稳定;对照 3组与本发明最大的区别在于使用了叠氮钠作为防腐剂,叠氮钠 对于某些生化反应是有很大影响的; 对照 4和 5组与本发明最大的区别在于使 用了超出本发明的保护剂的范围, 这样会使本底过高或抑制过度, 从而影响
虽然, 上文中已经用一般性说明、 具体实施方式及试验, 对本发明作了 详尽的描述, 但在本发明基础上, 可以对之作一些修改或改进, 这对本领域 技术人员而言是显而易见的。 因此, 在不偏离本发明精神的基础上所做的这 些修改或改进, 均属于本发明要求保护的范围。 工业实用性
本发明公开了一种处理微量全血的试剂, 在进行临床检测时, 可根据临 床检测需要的样本量, 从耳垂、 手指末端、 足跟等处釆取到的微量全血(至 少 20ul ), 然后取本发明试剂稀释 2-1000倍, 混匀, 然后根据检测内容进行相 关操作, 与常规的临床检测结果相同, 且使用方法方便, 简单, 快速。 解决 了现有临床检测需全血量大, 幼儿、 老龄及重病人釆血困难的问题。
Claims
2、 根据权利要求 1所述的试剂, 其特征在于, 所述 100ml试剂为含有以下 成分的水溶液: 浓度为 5-100mM, pH为 6.8-7.6的缓冲液 0.01-5.0g、 渗透压维 持剂 0.5-15g、 保护剂 0.05-3g、 抗凝剂 0.005-lg和防腐剂 0.03-150mg。
3、 根据权利要求 1所述的试剂, 其特征在于, 所述 100ml试剂为含有 以下成分的水溶液: 浓度为 5-60mM, pH为 7.2-7.5的缓冲液 0.02-4.0g、 渗 透压维持剂 0.6-11.5g、保护剂 0.1-lg、抗凝剂 0.005-0.7g和防腐剂 0.04-100mg。
4、 根据权利要求 1-3任一项所述的试剂, 其特征在于, 所述试剂还含有 以下成分: 植物血球凝集素 0.1-0.5g或红细胞抗体 0.02- 0.2ml。
5、 根据权利要求 1-4任一项所述的试剂, 其特征在于, 所述缓冲液为磷 酸缓冲系统、 硼酸缓冲系统、 tris-Hcl缓冲系统或巴比妥缓冲系统; 优选地, 所述磷酸缓冲系统为磷酸二氢钠和磷酸氢二钠的组合、 磷酸二氢钠和磷酸氢 二钾的组合; 硼酸缓冲系统为硼酸和氢氧化钠的组合、 硼酸和硼砂组合; tris-Hcl缓冲系统为 tris和盐酸; 巴比妥缓冲系统为巴比妥钠和盐酸。
6、 根据权利要求 1-4任一项所述的试剂, 其特征在于, 所述渗透压维持 剂为氯化钠或蔗糖。
7、 根据权利要求 1-4 任一项所述的试剂, 其特征在于, 所述保护剂为 BSA、 海藻糖或 HSA中的一种或几种。
8、 根据权利要求 1-4 任一项所述的试剂, 其特征在于, 所述抗凝剂为 EDTA或其盐、 肝素或其盐、 草酸盐或枸橼酸盐中的一种或几种; 所述防腐 剂为 proclin300或庆大霉素中的一种或两种。
9、 一种制备权利要求 1-8任一项所述试剂的方法, 其特征在于, 该方法 包括以下步骤: 按照配比称取各成分, 加入 80ml水, 混匀, 然后调节 pH值 为 6.8-7.6, 定容, 即得。
10、 权利要求 1-8任一项所述试剂在处理微量全血中的应用; 优选地, 所
述应用包括以下步骤: 根据临床检测需要的样本量, 从耳垂、 手指末端、 足 跟等处釆取到的微量全血, 然后直接用试剂稀释 2-1000倍, 混匀。
