Abstract
Description
この光強度検出工程により検出された前記光強度に基づき生体内における光の散乱係数を算出する散乱係数算出工程と、この散乱係数算出工程により算出された光の散乱係数に基づき生体内における脂質濃度を算出する脂質濃度算出工程とを有する。
本実施形態における照射工程は、非侵襲型生体脂質濃度計測器1の照射手段2を用いて実行される。光強度/距離算出部41を用いた濃度計測の場合、照射手段2は照射位置21に対して所定の光強度S0の連続光を体外から体内に向けて照射する。生体に照射する光を連続光とすることで、光強度検出手段3により検出される光強度が、時間による減衰の影響を含まれないようにしている。
照射工程では、光強度/距離算出部41と同様に、照射手段2を用いて照射位置21に対して連続光を照射する。
照射工程では、光強度/距離算出部41や光強度比算出部42と同様に、照射手段2を用いて照射位置21に対して連続光を照射し、光強度検出工程では、第一光強度検出手段31を用いて第一検出位置331における光強度を検出するとともに、第二光強度検出手段32を用いて第二検出位置332の光強度を検出する。第一検出位置331および第二検出位置332で検出された光強度は、散乱係数算出工程へと送られる。
照射工程では、照射手段2を用いて照射位置21に対してパルス状の光を照射する。生体に照射する光をパルス状の光とすることで、光強度検出手段3により検出される光強度が時間による減衰の影響を含むようにしている。
照射工程では、減衰時間算出部44と同様に、照射手段2を用いて照射位置21に対してパルス状の光を照射する。光強度検出工程では、第一光強度検出手段31を用いて第一検出位置331の光強度を時間的に連続して検出する。そして、第一検出位置331で検出された光強度は所定時間毎に散乱係数μs算出工程へと送られる。
照射工程では、照射手段2を用いて照射位置21に対して照射する光の強度または光の位相を変調させた光を照射する。光強度検出工程では、第一光強度検出手段31を用いて第一検出位置331の光強度を検出する。第一検出位置331で検出された光強度は所定時間毎に散乱係数算出工程へと送られる。
1.光強度から生体内における散乱係数μsを求めることにより、非侵襲により血中脂質の濃度を得ることができる。
2.採血等の人を傷つける処置が必要なくなり、被験者への苦痛や負担を軽減することができる。
3.採血等の医療行為を必要としないため家庭でも血中脂質を計測することができる。
4.光強度から散乱係数μsや脂質濃度の算出過程が単純であるため、即時的なデータ取得が可能である。
5.時間的に連続した血中脂質やこの血中脂質に相関性の良い散乱係数μsを算出することが可能であるため、食後高脂血症などの代謝異常の検査に応用することができる。
6.生体に照射する光の波長範囲を血漿の無機物により吸収される光の波長範囲以外として、ノイズとなる吸収の影響を抑え、血中脂質濃度の計測精度を高めることができる。
7.血液の細胞成分により吸収される光の波長範囲も除外することで、赤血球等の細胞成分による吸収の影響も抑制され、より正確な血中脂質濃度を測定することができる。
8.血中脂質による光の散乱と血中脂質の濃度との関係を考慮することにより、様々な計算手法により散乱係数μsおよび脂質濃度を算出することができる。
9.脂質濃度算出手段5または脂質濃度算出工程では、臨床現場のニーズに応じて濃度(mg/dL)等の単位のみではなく、粒子数への換算やホルマジン濁度などへの換算も可能である。
(1)全血の変化
まず、被験者に脂質を摂取し後の血中脂質等の濃度が時間変化することを確かめた。被験者はA氏およびB氏であり、それぞれに脂質(オフトクリーム;上毛食品社)を摂取させた。
次に、中性脂肪(TG)の上昇に寄与している物質を特定するため、本実施例1(1)の血液サンプルを高速液体クロマトグラフィー (High Performance Liquid Chromatography:HPLC)に供して、カイロミクロン(CM)/VLDL、LDLおよびHDLに分画し、CM/VLDL、LDLおよびHDL中の総コレステロール(TC)および中性脂肪(TG)の濃度を計測した。その結果を図8~図11に示す。これらの図において、横軸は被験者が脂質を摂取してからの時間、縦軸はCM/VLDL、LDLおよびHDL中の中性脂肪(TG)濃度または総コレステロール(TC)である。また、「カイロミクロン(CM)/VLDLの濃度」とは、カイロミクロン(CM)とVLDLとを合算した濃度である。
次に、本実施例2では、非侵襲に光を体外から体内に向けて照射して、体外へと放射される光の光強度より散乱係数μsを求めるに当たり、ノイズとなる光の吸収する光の波長を検討した。検討には、被験者から血液を採取し、その全血を分光光度計に供して、波長300~3300nmの光に対する血液の吸光度を計測し、吸収スペクトルを得た。その結果を図12に示す。この図において、横軸は光の波長、縦軸は吸光度である。
次に、本実施例3では、非侵襲に光を体外から体内に向けて照射して、体外へと放射される光の光強度より散乱係数μsを求めることで血中脂質の濃度を計測すること、および透過光による計測の有効性について検討した。
(1)脂質濃度の変化に伴う血液、血球および血清の吸光度の変化
まず、A氏について、実施例1(1)に記載の方法により、血液サンプルを得て血中脂質の濃度を計測した。その結果を以下の表1に示す。
また、同様にA氏から血液を採取して、血液、血球および血清を得た。これらを分光光度計に供して、波長300~1000nmの光に対する血液の吸光度を計測し、吸収スペクトルを得た。なお、分光光度計の基本原理は、計測対象物に光を照射しその透過光を解析することで行うものである。血液、血球および血清の吸収スペクトルを重ねて表した結果を図16に示す。
本実施例4では、本発明の非侵襲型生体脂質濃度計測器1を用い、照射した光が散乱により時間的に減衰していく散乱現象に基づき脂質濃度を計測可能か否かを確かめた。
(1)光が出尽くすまでの時間(波形の拡がり)の検討
被験者の手の甲の中央部の血管の上の皮膚に、照射位置21を設定した。また、照射位置を設定したのと同じ血管の上の皮膚に、照射位置21からの間隔を10mmとして検出位置31を設定した。照射位置21の光源22には、Ti:Spphireレーザ Chameleon Ultra II(波長可変式、パルス幅:140fs FWHM、平均出力:400mW、繰り返し周波数:80MHz;コヒレント社)を、検出位置31の光強度検出手段3にはストリークカメラをそれぞれ用いた。
次に、散乱係数μsを求めるため、検出した光強度が最大となるまでの時間(peak top time)について検討した。その結果を、図19に示す。この図において、横軸は被験者が脂質を摂取してからの時間、縦軸は光強度である。
次に、本実施例5では、本発明の非侵襲型生体脂質濃度計測器1を用い、照射した光が散乱により距離に応じて減衰していく散乱現象に基づき脂質濃度を計測可能か否かを確かめた。
(1)複数の検出位置33における光強度の計測
被験者A氏およびB氏の手の甲の中央部の血管の上の皮膚に、照射位置21を設定した。また、照射位置21を設定したのと同じ血管の上の皮膚に、照射位置21からの間隔を10mm、15mmおよび20mmとして、計3箇所の検出位置33を設定した。照射位置21の光源22は、Ti:Spphire(Ti:S)レーザ Chameleon Ultra(自動波長掃引 フェムト秒レーザ;コヒレント社)を用い、検出位置31の光強度検出手段3にはフェムトワットフォトレシーバー FWPR-20-SI(フェムト社)を用いた。
続いて、各検出位置33において検出した光強度の比を算出し、散乱係数μsとした。この散乱係数μsの値は照射した光の血中脂質による散乱を示す指標である。散乱係数μsの値をグラフに表したものを図20および図21に示す。これらの図において、横軸は被験者が脂質を摂取してからの時間、縦軸は従来法であるHPLCにより計測されたCM/VLDLにおけるTG濃度または各検出位置33において検出した光強度の比から求めた散乱係数μsである。
また、各検出位置33において検出した光強度の差を算出し、散乱係数μsとした。その散乱係数μsの値をグラフに表したものを図23および図24に示す。これらの図において、横軸は被験者が脂質を摂取してからの時間、縦軸は各検出位置31において検出した光強度の差から求めた散乱係数μsである。また、ダイヤ型のプロットは照射位置21からの間隔が10mmにおける検出位置の光強度と、15mmにおける検出位置の光強度の差の値であり、四角型のプロットは、照射位置21からの間隔が10mmにおける検出位置の光強度と、20mmにおける検出位置の光強度の差の値であり、三角型のプロットは照射位置21からの間隔が15mmにおける検出位置の光強度と、20mmにおける検出位置の光強度の差の値である。
本実施例6では、生体組織模擬ファントムを用いた実験および理想的な散乱現象をモンテカルロシミュレーションを用いて計算し、その値を用いて下記式(1)および下記式(2)について検討を行った。
まず、モンテカルロシミュレーションにより、照射検出間距離ρと検出される光子数についてシミュレーションを行った。シミュレーションの条件は、吸収係数が0.01、照射する光子数が10×109個である。そして、散乱係数μsを0.5、1、1.5、2、2.5、3、3.5、4、4.5および5としてそれぞれ計算を行った。計算結果を図25に示す。この図において、横軸は照射検出間距離ρ、縦軸は検出される光子数である。
次に、イントラリピッドを用いて、散乱係数μsが既知の生体組織模擬ファントムを作成し、それに基づき実験を行った。具体的には、散乱係数μsを0.5、1、1.5、2、2.5、3、3.5、4、4.5および5の生体組織模擬ファントムを作成し、それら生体組織模擬ファントムに照射手段2により連続光を照射するとともに、光強度検出手段4により光強度を検出し、その検出した光強度を、本実施例(1)と同様に、式(1)および式(2)に代入することで、散乱係数μsを算出した。その結果を図29に示す。この図において、横軸は生体組織模擬ファントムの散乱係数μs、縦軸は、検出した光強度を式(1)および式(2)に代入したことによって算出された散乱係数μsである。
本実施例7では、生体組織模擬ファントムを用いた実験および理想的な散乱現象をモンテカルロシミュレーションを用いた計算により下記式(3)について検討を行った。
上記式(3)より、散乱係数μsは、第一検出位置における光強度R(ρ1)、R(ρ2)の比が大きいほど、感度がよいことが予想される。そのためには、ρ1、ρ2の距離差が大きいほどよい。しかし、照射検出間距離ρが大きくなるほど光強度R(ρ)は指数関数的に小さくなり、計測における信号量 (signal) と雑音量 (noise)との比であるSN比は急速に大きくなり計測精度は劣化する。またρ1、ρ2の距離差が大きくなると、それぞれの地点における光強度の散乱体内部伝搬領域の共通部分が小さくなり、さらに計測精度が劣化する。そこで、良好な計測条件を求めるべく、実用範囲内でρ1、ρ2を変化させて、計測条件について検討を行った。
まずは、イントラリピッドを用いて、散乱係数μsが既知の生体組織模擬ファントムを作成した。実験では、このイントラリピッドを用いて、散乱係数μsに対するR(ρ1)/R(ρ2)の変化を計測した。計測結果の代表例を図30に示す。この図において横軸は散乱係数であり、縦軸は光強度である。また、ダイヤ型のプロットは照射位置21からの間隔が10mmにおける検出位置の光強度と、15mmにおける検出位置の光強度の比の値であり、四角型のプロットは、照射位置21からの間隔が10mmにおける検出位置の光強度と、20mmにおける検出位置の光強度の比の値であり、三角型のプロットは照射位置21からの間隔が10mmにおける検出位置の光強度と、25mmにおける検出位置の光強度の比の値であり、バツ型のプロットは照射位置21からの間隔が10mmにおける検出位置の光強度と、30mmにおける検出位置の光強度の比の値である。
式(3)の妥当性を検証するため、モンテカルロシミュレーションにて検証を行った。シミュレーション条件は、吸収係数μaを0.01、0.05および0.1とした。そして、散乱係数μsを0.5、1、1.5、2、2.5、3、3.5および4としてそれぞれの組み合わせの計算を行った。また、照射検出間距離ρは、上述のとおり、ρ1=10 mm、ρ2 =20mmとした。そして、得られた値を上記式(3)に代入し、計算を行った。その計算結果を図31に示す。この図において、横軸は、シミュレーション条件でもある理論的な散乱係数μs、縦軸は式(3)を用いて計算された散乱係数μsである。
上記モンテカルロシミュレーションの結果を踏まえて、生体組織模擬ファントムを用いた実験により式(3)の検討をおこなった。計測結果を図32に示す。横軸は、生体組織模擬ファントムの散乱係数μs、縦軸は実験によって光強度を算出しその結果を式(3)を用いて計算された散乱係数μsである。

Claims (21)
- 非侵襲により生体内における血中の脂質濃度を計測する非侵襲型生体脂質濃度計測器であって、
生体外から生体内に向けて所定の光強度で光を照射する照射手段と、
この照射手段による光の照射位置から所定間隔をあけて配置されて前記生体から放出される光強度を検出する光強度検出手段と、
この光強度検出手段により検出された前記光強度に基づき生体内における光の散乱係数を算出する散乱係数算出手段と、
この散乱係数算出手段により算出された光の散乱係数に基づき生体内における脂質濃度を算出する脂質濃度算出手段と
を有する非侵襲型生体脂質濃度計測器。 - 前記照射位置と前記光強度を検出する検出位置とが所定の照射検出間距離を隔てて設けられており、前記光強度検出手段が生体内の血中脂質により散乱された後方散乱光による光強度を検出する請求項1に記載の非侵襲型生体脂質濃度計測器。
- 前記散乱係数算出手段は、検出された光強度と、照射位置から光強度を検出する検出位置までの照射検出間距離との比に基づき生体内における光の散乱係数を算出する請求項1または請求項2に記載の非侵襲型生体脂質濃度計測器。
- 前記照射手段により連続光を照射するとともに、その照射位置を中心として各々異なる距離に設置された複数の前記光強度検出手段により各々の検出位置における光強度を検出し、
前記散乱係数算出手段は、各々の前記光強度検出手段により検出された前記光強度同士の比および/または前記光強度同士の差に基づいて生体内における光の散乱係数を算出する請求項1または請求項2に記載の非侵襲型生体脂質濃度計測器。 - 前記照射手段により連続光を照射するとともに、その照射位置を中心として各々異なる距離に設置された第一光強度検出手段および第二光強度検出手段が順に並べられており、
前記散乱係数算出手段は、前記照射位置から前記第一光強度検出手段による第一検出位置までの第一照射検出間距離ρ1と、前記照射位置から前記第二光強度検出手段による第二検出位置までの第二照射検出間距離ρ2と、前記第一光強度検出手段により検出された第一光強度R(ρ1)と、前記第二光強度検出手段により検出された第二光強度R(ρ2)とを下記式(3)に代入することで散乱係数μsを算出する請求項1、請求項2または請求項5のいずれかに記載の非侵襲型生体脂質濃度計測器。
- 前記照射手段によりパルス状の光を照射するとともに、前記光強度検出手段により所定の時間毎の前記光強度を検出し、
前記散乱係数算出手段は、前記照射手段によりパルス状の光を照射した時から、前記光強度検出手段により検出された前記光強度が所定の強度に減衰するまでの時間の長さに基づき生体内における光の散乱係数を算出する請求項1または請求項2に記載の非侵襲型生体脂質濃度計測器。 - 前記照射手段によりパルス状の光を照射するとともに、前記光強度検出手段により所定の時間毎の前記光強度を検出し、
前記散乱係数算出手段は、前記照射手段によりパルス状の光を照射した時から、前記光強度検出手段により検出された前記光強度が最も強くなる時までの時間の長さに基づき生体内における光の散乱係数を算出する請求項1または請求項2に記載の非侵襲型生体脂質濃度計測器。 - 前記照射手段により照射する光の強度または光の位相を変調させた光を照射するとともに、前記光強度検出手段により所定の時間毎の前記光強度を検出し、
前記散乱係数算出手段は、前記光強度検出手段により検出された前記光強度の時間変化に基づき光密度波形を算出し、この光密度波形に基づき前記血液の散乱係数および吸収係数を算出する請求項1または請求項2に記載の非侵襲型生体脂質濃度計測器。 - 請求項1~請求項9に記載のいずれかの非侵襲型生体脂質濃度計測器により算出された散乱係数および/または脂質濃度の時間変化から生体脂質代謝機能を計測する非侵襲型生体脂質代謝機能計測器。
- 請求項1~請求項9に記載のいずれかの非侵襲型生体脂質濃度計測器により算出された散乱係数および/または脂質濃度の時間変化を取得し生体脂質代謝機能を検査する非侵襲による生体脂質代謝機能検査方法。
- 非侵襲により生体内における血中の脂質濃度を計測する非侵襲による生体脂質濃度計測方法であって、
生体外から生体内に向けて所定の光強度で光を照射する照射工程と、
この照射工程による光の照射位置から所定の間隔をあけて前記生体から放出される光強度を検出する光強度検出工程と、
この光強度検出工程により検出された前記光強度に基づき生体内における光の散乱係数を算出する散乱係数算出工程と、
この散乱係数算出工程により算出された光の散乱係数に基づき生体内における脂質濃度を算出する脂質濃度算出工程と
を有する非侵襲による生体脂質濃度計測方法。 - 前記照射位置と前記光強度を検出する検出位置とが所定の照射検出間距離を隔てて設けられており、前記光強度検出工程では生体内の血中脂質により散乱された後方散乱光による光強度を検出する請求項12に記載の非侵襲による生体脂質濃度計測方法。
- 前記散乱係数算出工程において、検出された光強度と、照射位置から光強度を検出する検出位置までの照射検出間距離との比に基づき生体内における光の散乱係数を算出する請求項12または請求項13に記載の非侵襲による生体脂質濃度計測方法。
- 前記照射工程において生体外から生体内に向けて連続光を照射するとともに、前記光強度検出工程において照射された光の照射位置を中心として各々異なる距離に配置された複数の前記検出位置における前記光強度を検出し、
前記散乱係数算出工程では、各々の前記光強度検出工程において検出された前記光強度同士の比および/または前記光強度同士の差に基づいて生体内における光の散乱係数を算出する請求項12または請求項13に記載の非侵襲による生体脂質濃度計測方法。 - 前記照射工程において生体外から生体内に向けて連続光を照射するとともに、前記光強度検出工程において照射された光の照射位置を中心として各々異なる距離に配置された第一検出位置および第二検出位置における前記光強度を検出し、
前記散乱係数算出工程では、前記照射位置から前記第一検出位置までの第一照射検出間距離ρ1と、前記照射位置から前記第二検出位置までの第二照射検出間距離ρ2と、前記第一検出位置において検出された第一光強度R(ρ1)と、前記第二検出位置において検出された第二光強度R(ρ2)とを下記式(3)に代入することで散乱係数μsを算出する請求項12、請求項13または請求項16のいずれかに記載の非侵襲による生体脂質濃度計測方法。
- 前記照射工程において生体外から生体内に向けてパルス状の光を照射するとともに、前記光強度検出工程において前記検出位置における所定の時間毎の前記光強度を検出し、
前記散乱係数算出工程では、パルス状の光を照射した時から、検出された前記光強度が所定の強度に減衰するまでの時間の長さに基づき生体内における光の散乱係数を算出する請求項12または請求項13に記載の非侵襲による生体脂質濃度計測方法。 - 前記照射工程において生体外から生体内に向けてパルス状の光を照射するとともに、前記光強度検出工程において前記検出位置における所定の時間毎の前記光強度を検出し、
前記散乱係数算出工程では、パルス状の光を照射した時から、検出された前記光強度が最も強くなる時までの時間の長さに基づき生体内における光の散乱係数を算出する請求項12または請求項13に記載の非侵襲による生体脂質濃度計測方法。 - 前記照射工程において照射する光の強度または光の位相を変調させた光を照射するとともに、前記光強度検出工程において前記検出位置における所定の時間毎の前記光強度を検出し、
前記散乱係数算出工程では、検出された前記光強度の時間変化に基づき光密度波形を算出し、この光密度波形に基づき前記血液の散乱係数および吸収係数を算出する請求項12または請求項13に記載の非侵襲による生体脂質濃度計測方法。 - 請求項12~請求項20に記載のいずれかの非侵襲による生体脂質濃度計測方法により算出された散乱係数および/または脂質濃度の時間変化を取得し生体脂質代謝機能を検査する非侵襲による生体脂質代謝機能検査方法。
