Abstract
Description
そうすると、リング状弾性鋼線31’が自身の弾性復帰力で拡がり、図43に示すように、フィルタ32’の開口が狭窄部の末梢側において、血流に直交するようにして配置される。
その結果、リング状弾性鋼線31’は、図45に示すように、各糸4’が取り付けられている4箇所の中間部位において山谷に交互に折れ曲がり始める。
このとき、リング状弾性鋼線31’の一部は、図46に示すように、操作管13’の先端部を径方向外側からくわえ込むこととなる。
かといって、この引っかかりを解消すべく複雑な構造にすると、操作が難しくなったり、破損やトラブルを招いたりして、手術に悪影響を及ぼしかねない。
血管内に配置される芯材と、
開口が血流の上流側を向くように血管内に配置される袋状のフィルタ及び該フィルタの開口縁部に取り付けられて展開状態では自身の弾性復帰力によって拡がった環状となるリング状弾性線材からなるフィルタ部材であって、前記芯材の先端部に取り付けられて回収可能な折畳状態と血管内で拡がる前記展開状態との2状態を取り得るフィルタ部材と、
一端部が前記リング状弾性線材の複数箇所にそれぞれ取り付けられるとともに、他端部が前記一端部よりも芯材の根元側に取り付けられて、前記フィルタ部材を前記芯材に接続する複数本の線状体と、
前記芯材が内部を通るスライドチューブと、
前記スライドチューブを芯材に沿って移動させるアクチュエータ部材とを具備し、
前記展開状態から前記スライドチューブを芯材の先端側にアクチュエータ部材によって移動させることによって前記線状体をその他端部からスライドチューブ内に引き込むと、各線状体の一端部及びそれらにそれぞれ取り付けられているリング状弾性線材の複数箇所が互いに接近して、前記リング状弾性線材がスライドチューブの先端部外周面を径方向外側からくわえ込むように折れ曲がり、前記折畳状態となるように構成されたものである。
ガイド材の具体的実施態様としては、前記ガイド材が所定の曲げ剛性を有した弾性鋼線であるものを挙げることができる。
血管内に配置される芯材と、
開口が血流の上流側を向くように血管内に配置される袋状のフィルタ及び該フィルタの開口縁部に取り付けられて展開状態では自身の弾性復帰力によって拡がった環状となるリング状弾性線材からなるフィルタ部材であって、回収可能な折畳状態と血管内で拡がる前記展開状態との2状態を取り得る、前記芯材の先端部に取り付けられたフィルタ部材と、
自然状態では自身の弾性復帰力によって拡がった環状となる弾性鋼線であって、基端側の1箇所が前記芯材に取り付けられるとともに、前記芯材に対する取付箇所よりも先端側における対向箇所が前記リング状弾性線材の対向箇所にそれぞれ取り付けられた弾性鋼線とを具備し、
前記弾性鋼線が、前記芯材に沿って収束することによって前記フィルタ部材を前記展開状態から前記折畳状態に変形させ得るものであるとともに、前記展開状態では、前記弾性鋼線が前記リング状弾性線材と略直交または斜めに交差しながら環状に拡がり、前記対向箇所より先端側がフィルタの内部に位置するように構成したものであることを特徴とするものである。
しかも、その構成は、弾性鋼線を設けただけであり、極めてシンプルである。

第1実施形態の血管内遊離物捕獲器具100は、図1に示すように、血管B内に挿入された管状体1の内部を通る芯材たるワイヤ22と、その先端部に取り付けたフィルタ部材3とを具備するものであって、例えば手術中にこのフィルタ部材3を血管B内に配置することにより、手術で剥離した血栓等をこのフィルタ部材3で捕獲し、これよりも下流側に流れることを阻止するものである。
管状体1は、カテーテルと称されるもので、所定のトルク伝達性を有した中空の柔らかい素材を用いて形成されている。この管状体1は、体外から皮膚を貫通して血管B内に挿入され、例えば、その先端がフィルタ部材3を設置すべき位置の近傍となるように配置される。
フィルタ部材3は、図1、図2等に示すように、基端側、すなわち血流の上流側に開口が位置するように配置され、血流の下流側に位置する底部が窄まった形状である袋状のフィルタ32と、このフィルタ32の開口縁に取り付けられたリング状弾性線材31とを具備するものである。
このようにして、フィルタ部材3が設置される。
このとき、リング状弾性鋼線6の一部は、図8、図9に示すように、スライドチューブ21の先端部を径方向外側からくわえ込むこととなる。
その他に、以下のような効果も挙げることができる。
弾性鋼線6がループ状となっており、図1に示すように、その先端部6aがフィルタ32の内部に入り込んでいるので、展開状態において弾性鋼線6がフィルタ32を拡げる役割を果たし、該フィルタ32を、血栓等の捕獲に好適な、内部が拡がった状態で血管B内に配置することが可能になる。
第2線状体5を設けているので、フィルタ部材3を、その開口が血流に対して直交する好適な姿勢により確実に維持できる。
この変形例では、図14、図15に示すように中心にワイヤ22が通る中心孔8a及びその周囲に弾性鋼線6が通る一対の挿通孔8bをあけた薄厚円板体8を、スライドチューブ21と操作管13との間に介在させている。また、前記第1実施形態では、スライドチューブ21と操作管13との径を等しく設定していたが、この変形例では、操作管13の径よりもスライドチューブ21の径を小さくするとともに、操作管13の径と円板体8の径とほぼ等しくしている。挿通孔8bは、スライドチューブ21の外周よりも外側に位置するとともに操作管13の内周よりも内側に位置する。このことにより、弾性鋼線6が、操作管13とスライドチューブ21との間で挟まれることがなくなりその動きがより円滑になる。また、スライドチューブ21の径を小さくできるので、線状体4を接近させてリング状弾性線材31をより細く折り畳める。さらに、スライドチューブ21と操作管13とが重合して分離しなくなるといった不具合を回避できる。
この図50では、ワイヤ22とともにフィルタ32の底部をスライド可能に貫通させてその先端がフィルタ32から所定長さだけ突出するように構成してある。符号36は、フィルタ32の底部を貫通させて固着した管部材を示している。この管部材36を、ワイヤ22及び弾性鋼線両端部6bがスライド可能に挿通する。
この実施形態では、図29に示すように、前記ワイヤが、フィルタ部材3の取り付けられている先端ワイヤ2Aと、それより根元側の根元ワイヤ(搬送用根元ワイヤ2B1又は回収用根元ワイヤ2B2)とからなり、それら先端ワイヤ2Aと根元ワイヤ2Bとが連結構造を介して接離可能に構成してある。
このようにして、フィルタ部材3が設置されると、フィルタ搬送管12及び搬送用根元ワイヤ2B1は引き抜かれ回収される。
その後の動作は、前記第1実施形態と同様なので説明は省略する。
この実施形態では、フィルタ32及びリング状弾性線材31は前記第1実施形態と同じであるので、説明を省略する。
このようにして、フィルタ部材3が設置される。
前記操作管13が、図38、図39に示すように、外側操作管要素132とこの外側操作管要素132内をスライド可能に挿通する内側操作管要素131とからなり、前記内側操作管要素131内を芯材たるワイヤ22が通るように構成されたものであってもよい。
なお、前記実施形態で述べた管状体1を外側操作管要素、操作管を内側操作管要素とみなして、同様の動作をさせてもよい。
この実施形態は、第3実施形態にフィルタ部材3を分離させるという、第2実施形態のようなアイデアを盛り込んだものである。
また、第2実施形態同様、前記先端ワイヤ2Aの根元を折り曲げて、あるいは湾曲させてフック部9を形成している。
第1実施形態同様、まず、血流の流れ方向と同方向から管状体(図示しない)を血管内に挿入し、その先端をフィルタ部材3を設置すべき部位の上流側近傍に配置する。
このようにして、フィルタ部材3が設置される。また、搬送用根元ワイヤ2B1は引き抜かれて回収される。
その後は、第3実施形態と同様なので、説明は省略する。
Claims (16)
- 血管内に配置される芯材と、
開口が血流の上流側を向くように血管内に配置される袋状のフィルタ及び該フィルタの開口縁部に取り付けられて展開状態では自身の弾性復帰力によって拡がった環状となるリング状弾性線材からなるフィルタ部材であって、折畳状態と血管内で拡がる前記展開状態との2状態を取り得る、前記芯材に取り付けられたフィルタ部材と、
一端部が前記リング状弾性線材の複数箇所にそれぞれ取り付けられるとともに、他端部が前記一端部よりも芯材の根元側に取り付けられて、前記フィルタ部材を前記芯材に接続する複数本の線状体と、
前記芯材が内部を通るスライドチューブとを具備し、
前記展開状態から前記スライドチューブを芯材の先端側に移動させることによって前記線状体をその他端部からスライドチューブ内に引き込むと、各線状体の一端部及びそれらにそれぞれ取り付けられているリング状弾性線材の複数箇所が互いに接近して、前記リング状弾性線材がスライドチューブの先端部外周面を径方向外側からくわえ込むように折れ曲がり、前記折畳状態となるように構成されたものにおいて、
前記折畳状態において、前記リング状弾性線材におけるくわえ込み部分の先端部とそれよりも根元側におけるスライドチューブの外側周面との間に略形成されるガイド面を有したガイド材をさらに具備していることを特徴とする血管内遊離物捕獲器具。 - 前記スライドチューブの根元側に配置され、芯材に案内されて進退する操作管を芯材の先端側に移動させることによって、該操作管の先端がスライドチューブを押して芯材の先端側に移動させるように構成してある請求項1記載の血管内遊離物捕獲器具。
- 前記ガイド材が所定の曲げ剛性を有した弾性鋼線である請求項1記載の血管内遊離物捕獲器具。
- 前記弾性鋼線は、その基端部が前記芯材に取り付けられているとともに、その基端取付部位から前記スライドチューブの外側を通って芯材の先端方向に延び、または前記基端取付部位からスライドチューブの内部を通りその側周壁に設けた鋼線貫通孔から該スライドチューブの外側に延出して芯材の先端方向に延び、前記くわえ込み部分の先端部に、芯材の延伸方向に沿ってスライド可能に取り付けられている請求項3記載の血管内遊離物捕獲器具。
- 前記弾性鋼線は、前記くわえ込み部分先端部の径方向外側に取り付けられている請求項4記載の血管内遊離物捕獲器具。
- 前記リング状弾性線材が、折畳状態において一対の対向するくわえ込み部分を形成するものにおいて、
各くわえ込み部分に対応して設けられた一対の前記弾性鋼線が、各くわえ込み部分の先端部からフィルタの内部にそれぞれ延伸して一体につながり、ループ状をなすように構成してある請求項3記載の血管内遊離物捕獲器具。 - 一端部が前記リング状弾性線材の複数箇所にそれぞれ取り付けられるとともに、他端部が、前記一端部よりも芯材の先端側に取り付けられて、該リング状弾性線材を前記芯材に接続する複数本の第2線状体をさらに具備している請求項1記載の血管内遊離物捕獲器具。
- 前記リング状弾性線材が細鋼線を巻回して構成したものであり、前記細鋼線の両端余剰部がフィルタの内部へ延伸して芯材に取り付けられている請求項1記載の血管内遊離物捕獲器具。
- 前記芯材が、前記フィルタ部材が取り付けられているとともに前記スライドチューブの中を通る先端芯材と、それより根元側の根元芯材とを接離可能に構成したものにおいて、
前記根元芯材として、フィルタ部材を血管内に設置するときに用いる搬送用芯材と、フィルタ部材を血管から回収するときに用いる回収用芯材との2種類が設けてあり、
前記搬送用芯材の先端部には、前記先端芯材の基端部に設けた被係合部に係合することなくこれを押す押圧面が設けてあり、
前記回収用芯材の先端部には、前記被係合部に係合する係合部が設けてある請求項1記載の血管内遊離物捕獲器具。 - 前記被係合部が、前記先端芯材の基端部を先端側に折り曲げて形成したフック部であり、該フック部が、その弾性復帰力により、フィルタ部材を血管内に設置した際に、先端芯材とのなす角度が広がるように構成してある請求項9記載の血管内遊離物捕獲器具。
- 血管内に配置される芯材と、
開口が血流の上流側を向くように血管内に配置される袋状のフィルタ及び該フィルタの開口縁部に取り付けられて展開状態では自身の弾性復帰力によって拡がった環状となるリング状弾性線材からなるフィルタ部材であって、折畳状態と血管内で拡がる前記展開状態との2状態を取り得る、前記芯材に取り付けられたフィルタ部材と、
自然状態では自身の弾性復帰力によって拡がった環状となる弾性鋼線であって、その1箇所を基端側にして前記芯材に取り付けられるとともに、前記芯材に対する取付箇所よりも先端側における対向箇所が前記リング状弾性線材の対向箇所にそれぞれ取り付けられた弾性鋼線とを具備し、
前記弾性鋼線が、前記芯材に沿って収束することによって前記フィルタ部材を前記展開状態から前記折畳状態に変形させ得るものであるとともに、前記展開状態では前記リング状弾性線材と交差するように環状に拡がり、前記対向箇所より先端側がフィルタの内部に位置するように構成したものであることを特徴とする血管内遊離物捕獲器具。 - 前記芯材が内部を通ることで該芯材に沿って進退可能に設けられた管部材を利用するものであって、
前記展開状態から、前記弾性鋼線の対向箇所が管部材の先端に位置するまで、前記芯材を管部材内に向かって相対的に移動させ、弾性鋼線を管部材内に引き込むと、当該弾性鋼線の対向箇所が接近するのに伴って前記リング状弾性線材の対向箇所が接近した状態となり、さらにそこから前記芯材が管部材内に引き込まれるようにそれらを相対的に移動させると、前記リング状弾性線材がその各対向箇所において前記芯材に沿うように折れ曲がった折畳状態となり前記管部材内にその一部又は全部が収納されるように構成してあることを特徴とする請求項11記載の血管内遊離物捕獲器具。 - 前記管部材が、内側操作管要素と外側操作管要素とからなり、前記内側操作管要素内を芯材が通るように構成されたものであって、
前記展開状態から、前記弾性鋼線の対向箇所が内側操作管要素の先端に位置するまで、前記芯材を内側操作管要素内に向かって相対的に移動させ、弾性鋼線を内側操作管要素内に引き込むと、当該弾性鋼線の対向箇所が接近するのに伴って前記リング状弾性線材の対向箇所が接近した状態となり、さらにそこから前記外側操作管要素を内側操作管要素及び芯材に対して相対的に繰り出すと、前記リング状弾性線材がその各対向箇所において前記芯材に沿うように折れ曲がった折畳状態となり前記外側操作管要素内にその一部又は全部が収納されるように構成してある請求項12記載の血管内遊離物捕獲器具。 - 前記リング状弾性線材が細鋼線を巻回して構成したものであり、前記細鋼線の両端余剰部をフィルタの内部へ延伸して芯材に取り付けている請求項11記載の血管内遊離物捕獲器具。
- 前記芯材が、フィルタ部材及び弾性鋼線が取り付けられている先端芯材と、それより根元側の根元芯材とを接離可能に構成したものであり、
前記根元芯材として、フィルタ部材を血管内に設置するときに用いる搬送用芯材と、フィルタ部材を血管から回収するときに用いる回収用芯材との2種類が設けてあり、
前記搬送用芯材の先端部には、前記先端芯材の基端部に設けた被係合部に係合することなくこれを押す押圧面が設けてあり、
前記回収用芯材の先端部には、前記被係合部に係合する係合部が設けてある請求項11記載の血管内遊離物捕獲器具。 - 前記被係合部が、前記先端芯材の基端部を先端側に折り曲げて形成したフック部であり、該フック部が、その弾性復帰力により、フィルタ部材を血管内に設置した際に、先端芯材とのなす角度が広がるように構成してある請求項15記載の血管内遊離物捕獲器具。
