Abstract
Description
現在、手根管症候群や肘部管症候群などの絞扼性神経障害に対して神経剥離術などの減圧術が施行され、有効であると報告されているが、術後の神経周囲組織の瘢痕形成や神経線維束内の線維化などの問題が危惧されている。例えば、手根管開放術においては3%の失敗率があるが、これは術後の神経内外の癒着や線維化が症状再発の主な原因とされており、術後の癒着が少なければ神経機能障害修復の効果はより高まると考えられる。
一方、このような問題に対して生体吸収性の癒着防止材が提唱され、アドコン(グリアテック社)として米国において承認され、臨床的に使用されていた。しかし、この癒着防止材は、手術部位の治癒遅延による副作用が懸念されていた。すなわち、術後の癒着を防止できても、治癒遅延などにより減圧術後の神経機能障害の修復を妨げてしまう虞がある。
これまで、低下した神経機能を回復させるため、生体適合性材料である多糖類を使用した種々の提案がなされている。例えば、脂質を結合させたグリコサミノグリカンまたはその塩を有効成分とする神経疾患治療材が開示されている(特開平9−30979号公報)。しかし、この神経疾患治療材は任意の剤形に製剤化することが可能ではあるが、体内での滞留性に関わる適度な粘度を持つゲルについては記載されておらず、また、術後の癒着については記載も示唆もされていない。
また、カルボキシル基を有する多糖類および/またはその塩をアミン系化合物からなる架橋性試薬で共有結合架橋してなる架橋性多糖類からなる神経再生用材料が開示されている(特開2000−198738号公報)。しかし、残存する架橋性試薬による炎症反応など、その安全性については懸念が残る。また、化学架橋ゲルは不均一性による特性変化を起こすことがあり、その安定性には改善の余地がある。加えて、術後の癒着については記載も示唆もされていない。
さらに、ウサギ坐骨神経癒着モデルを用いて、ヒアルロン酸が手術後の末梢神経癒着を抑制するとともに、末梢神経の潜時遅延を抑制したとの報告がある(The British Association of Plastic Surgeons 56,pp342−347,2003)。しかし、このヒアルロン酸は手術の開始時から塗布しなければ効果がなく、実際に使用する際には手術の妨げとなり、その取扱い性には改善の余地がある。
一方、多糖類としてセルロースを用いたゲル材について、国際公開WO2007/015579号明細書には、水に溶解させるとゲルを形成する、カルボキシメチルセルロースをホスファチジルエタノールアミンで修飾した誘導体が開示されている。しかし、神経機能障害修復効果については記載も示唆もされていない。
末梢神経における術後の癒着防止材として、欧州において既に臨床的に使用されている自己架橋ヒアルロン酸であるHYALOGLIDE(登録商標)(フィディア社)は、手の末梢神経手術後の痛みやチネルサインの改善にも効果があることを報告している(Microsurgery 27(1),pp2−7,2007)。
上記したように、神経機能を回復させるために多糖類を用いた種々の提案がなされているが、リン脂質で修飾した多糖類誘導体を用いた、神経機能障害修復効果があり、適度な粘度を有し、かつ術後の癒着も少なく、取扱いやすいハイドロゲルについては何ら検討されていない。特に、神経伝達速度改善効果を示す多糖類誘導体は、従来知られていない。
本発明者らは、損傷や変性を受けた神経の機能を回復させるための神経機能障害修復剤について鋭意研究した。その結果、0.5重量%水溶液について動的粘弾性測定装置で角速度10rad/secにて求められる複素弾性率が1~1000N/m2であり、かつ損失係数が0.01~2.0である特徴を有する多糖類誘導体のハイドロゲルが存在することを見出した。そして、かかるハイドロゲルが損傷を受けた神経の機能回復に有用で、術後の癒着も少ないことを見出し、本発明を完成した。
すなわち、本発明は、0.5重量%水溶液について動的粘弾性測定装置で角速度10rad/secにて求められる複素弾性率が1~1000N/m2であり、かつ損失係数が0.01~2.0である多糖類誘導体のハイドロゲルを含む神経機能障害修復剤である。
本発明で用いられる多糖類誘導体を水に溶解させると、注射可能な一定の弾性率および粘性を有するハイドロゲルとなる。かかるハイドロゲルを医療用インジェクタブルゲルとして使用すると、神経機能障害修復剤としての効果、例えば神経伝達速度改善効果を発現する。
また、本発明の神経機能障害修復剤は適度な粘弾性および/または優れた体内での滞留性を有するため、取扱い性に優れ、内視鏡を用いた手術の際など、複雑な形状の部位にも適用できる。

好ましい複素粘弾率範囲としては、1~200N/m2であり、より好ましくは1~100N/m2である。また、好ましい損失係数としては、0.01~1.5である。
本発明で用いる多糖類誘導体としては、好ましくはセルロース誘導体であり、より好ましくは、下記式で表される繰り返し単位からなるセルロース誘導体が挙げられる。
式中、R1、R2、およびR3はそれぞれ独立に下記式(a)、(b)、(c)、および(d)からなる群より選ばれるものであり、

式(c)中、Xはアルカリ金属またはアルカリ土類金属であり、
式(d)中、R4およびR5はそれぞれ独立に炭素数9~27のアルキル基またはアルケニル基であり、

(d)の置換度が0.001~0.05、より好ましくは0.005~0.015である。
上記式中、R4およびR5はそれぞれ独立に炭素数9~27のアルキル基またはアルケニル基を表す。なかでも、R4およびR5が炭素数9~19のアルケニル基であるものが好ましく、その中でもR4CO−および/またはR5CO−がオレオイル基であるもの、特にR4CO−およびR5CO−がオレオイル基であるものが好ましい。
本発明の神経機能障害修復剤としては、水100重量部に対し、本発明で用いる多糖類誘導体を0.1~1.5重量部含む、注入可能なハイドロゲルである神経機能障害修復剤であることが好ましい。さらに好ましくは、0.5~1.0重量部のものである。
そのなかでも、0.5重量%水溶液について動的粘弾性測定装置で角速度10rad/secにて求められる複素弾性率が1~200N/m2であるものが好ましい。さらに好ましくは、1~100N/m2のものである。また、このときの損失係数が0.01~1.5であるものが好ましい。
また、本発明の神経機能障害修復剤は多糖類誘導体ハイドロゲルからなり、生理的な塩濃度下において、1.0重量%水溶液について動的粘弾性測定装置で角速度10rad/secにて求められる複素弾性率が1~1000N/m2増加することが好ましい。より好ましい粘弾性の増加範囲としては、50~700N/m2増加するものであり、さらに好ましくは100~500N/m2増加するものである。
ここで、生理的な塩濃度とは、細胞の生存を維持させるように調整される生理的塩類溶液の塩濃度のことをいい、具体的な塩濃度をあげると、生理的食塩水(0.9%NaCl水溶液)、リンガー液、リン酸緩衝液などを例示することができる。
本発明は神経機能障害修復剤である。例えば、事故あるいは肉体の酷使などで損傷および/または変性した神経の機能回復などに好ましく用いられる。
本発明の神経機能障害修復剤調製のための多糖類誘導体としてセルロース誘導体を用いる場合、例えば次のようにして製造することができる。
<セルロース誘導体の製造方法>
上記した本発明で用いられるセルロース誘導体は、下記式で表される繰り返し単位からなり、分子量が5×103~5×106のカルボキシメチルセルロースと、
下記式で表されるホスファチジルエタノールアミンとを、
カルボキシメチルセルロースのカルボキシル基(すなわち(b)+(c)の置換基の合計)100当量に対し、ホスファチジルエタノールアミン0.1~100当量の割合にて、水および水と相溶する有機溶媒とからなり、水が20~70容量%含まれる混合溶媒に溶解し、縮合剤の存在下で反応させる工程を含む方法により製造することができる。
ここで、R1、R2、およびR3はそれぞれ独立に、下記式(a)、(b)、および(c)から選ばれるものであり、

式(c)中、Xはアルカリ金属またはアルカリ土類金属であり、

かかる原料としてのカルボキシメチルセルロースは、分子量が5×103~5×106であるものが好ましく、より好ましくは5×104~5×106、さらに好ましくは5×104~1×106である。
かかる原料としてのカルボキシメチルセルロースは、例えばパルプを水酸化ナトリウム溶液で溶解し、モノクロロ酢酸またはそのナトリウム塩でエーテル化し、精製することにより製造することができる。
上記式(c)におけるXのアルカリ金属としては、ナトリウム、カリウム、リチウムなどが好ましく、アルカリ土類金属としては、マグネシウム、カルシウムなどが好ましい。
(b)と(c)の置換度の合計は、0.3~2.0、好ましくは0.5~1.8、より好ましくは0.6~1.5である。(b)と(c)の割合は特に限定されないが、水に対する溶解性の点から、(c)が(b)よりも多く存在するほうが好ましい。
原料として好ましいカルボキシメチルセルロースの具体的な構造式は下記式で示す通りである。セルロース骨格におけるカルボキシメチル基の置換位置は、C−6位にあることが好ましい。
上記セルロース誘導体の製造方法で用いられる上記式で表されるホスファチジルエタノールアミンにおいて、R4およびR5はそれぞれ独立に、炭素数9~27のアルキル基またはアルケニル基である。R4およびR5としては、いずれも炭素数9~27のアルケニル基であることが好ましく、なかでもR4CO−および/またはR5CO−がオレオイル基であるものが好ましく、特にR4CO−およびR5CO−がオレオイル基であるものが好ましい。
かかる原料としてのホスファチジルエタノールアミンは、動物組織から抽出したもの、または合成して製造したものどちらでも使用できる。具体例としてジラウロイルホスファチジルエタノールアミン、ジミリストイルホスファチジルエタノールアミン、ジパルミトイルホスファチジルエタノールアミン、ジステアロイルホスファチジルエタノールアミン、ジアラキドイルホスファチジルエタノールアミン、ジベヘノイルホスファチジルエタノールアミン、ラウロオレオイルホスファチジルエタノールアミン、ミリストオレオイルホスファチジルエタノールアミン、パルミトオレオイルホスファチジルエタノールアミン、ジオレオイルホスファチジルエタノールアミン、ジリノレオイルホスファチジルエタノールアミン、ジリノレノイルホスファチジルエタノールアミン、ジアラキドノイルホスファチジルエタノールアミン、ジドコサヘキサエノイルホスファチジルエタノールアミンを挙げることができる。その中でも、合成する際に使用する有機溶媒への溶解性の面からジオレオイルホスファチジルエタノールアミンが好ましい。ホスファチジルエタノールアミンは生体由来の安全な物質である。
本発明で用いられるセルロース誘導体においては、セルロース誘導体分子間の疎水性相互作用を高める結果、本発明で用いられるセルロース誘導体はハイドロゲルを形成するものと考えられる。
本発明で用いられるセルロース誘導体の原料たるカルボキシメチルセルロースとホスファチジルエタノールアミンは、カルボキシメチルセルロースのカルボキシル基100当量に対し、ホスファチジルエタノールアミンを0.1~50当量、好ましくは1~40当量、より好ましくは3~30当量の割合で反応させる。0.1当量よりも少ないと生成されるセルロース誘導体がハイドロゲルを形成しない。また、40当量より多いと生理的塩濃度の状態での粘弾性上昇が観察されなくなる。
カルボキシメチルセルロースとホスファチジルエタノールアミンとの縮合反応は、縮合に用いる縮合剤の反応性や反応条件によっては反応効率が悪くなることがあるため、ホスファチジルエタノールアミンは、目的とする置換度の計算値よりも過剰に用いることが好ましい。
カルボキシメチルセルロースとホスファチジルエタノールアミンとは、水および水と相溶する有機溶媒(A)とからなり、水が20~70容量%である混合溶媒に溶解させる。水の含有量が20容量%よりも少ないとカルボキシルメチルセルロースが溶解しにくくなり、また70容量%よりも多いとホスファチジルエタノールアミンが溶解しにくくなるため反応が進まない。水の含有量は、好ましくは30~60容量%である。
水と相溶する有機溶媒(A)としては、具体的にはテトラヒドロフラン、1,4−ジオキサン、1,3−ジオキサン、1,3−ジオキソラン、モルホリンなどの環状エーテル結合を有する有機溶媒、ジメチルアセトアミド、ジメチルホルムアミド、N−メチル−2−ピロリドンなどのアミド結合を有する有機溶媒、ピリジン、ピペリジン、ピペラジンなどのアミン類、ジメチルスルホキシドなどのスルホキシド類を挙げることができる。これらの中でも環状エーテル類あるいはスルホキシド類が好ましく、なかでもテトラヒドロフラン、ジオキサン、ジメチルスルホキシドがより好ましい。
かかる反応に用いる試薬としては、カルボキシル活性化剤や縮合剤が好ましい。カルボキシル活性化剤としては、N−ヒドロキシスクシンイミド、p−ニトロフェノール、N−ヒドロキシベンゾトリアゾール、N−ヒドロキシピペリジン、N−ヒドロキシスクシンアミド、2,4,5−トリクロロフェノール、N、N−ジメチルアミノピリジンなどが挙げられる。縮合剤としては4−(4,6−ジメトキシ−1,3,5−トリアジン−2−イル)−4−メチルモルホリニウムクロリド、1−エチル−3−(ジメチルアミノプロピル)−カルボジイミドやその塩酸塩、ジイソプロピルカルボジイミド、ジシクロヘキシルカルボジイミドやN−ヒドロキシ−5−ノルボルネン−2,3−ジカルボキシイミドなどが挙げられる。これらの中でも、カルボキシル活性化剤としてN−ヒドロキシベンゾトリアゾール、縮合剤として4−(4,6−ジメトキシ−1,3,5−トリアジン−2−イル)−4−メチルモルホリニウムクロリドや1−エチル−3−(ジメチルアミノプロピル)−カルボジイミド塩酸塩を用いるのが好ましい。
反応温度は、好ましくは0~60℃である。副生成物の産生を抑制するためには、反応を0~10℃で行うことがより好ましい。反応環境は弱酸性下が好ましい。さらに好ましくはpH6~7である。
<セルロース誘導体の精製方法>
本発明で用いられるセルロース誘導体の製造方法においては、得られたセルロース誘導体を、実質的にカルボキシメチルセルロースを溶解しないが水と相溶する有機溶媒(B)を用いてセルロース誘導体を精製する工程を加えてもよい。
ここで、実質的にカルボキシメチルセルロースを溶解しない有機溶媒とは、粉末状あるいは凍結乾燥状態で入手可能なカルボキシメチルセルロースナトリウム塩あるいはカルボキシメチルセルロース(COOH型)に関して、水が存在しない条件下でカルボキシメチルセルロースの有機溶媒に対する溶解性を調べたとき、溶解度が3%以下の有機溶媒をいう。具体的にはメタノール、エタノール、n−プロピルアルコール、イソプロピルアルコール、n−ブチルアルコール、t−ブチルアルコールなどのアルコール類、エチレングリコール、1,2−プロピレングリコール、1,3−プロピレングリコール、グリセリンなどの多価アルコール類、アセトンなどのケトン類、フェノールなどの芳香族アルコール類を挙げることができる。これらの中でも沸点100℃未満のものが好ましく、例えばメタノール、エタノール、イソプロピルアルコールが好ましい。生体内で使用することを考慮するとエタノールが特に好ましい。
これらの群からなる有機溶媒(B)を用いて精製する場合、セルロース誘導体が水や有機溶媒(A)の混合液中に存在する状態で有機溶媒(B)を加えて沈殿を形成し、セルロース誘導体を取り出す方法を用いてもよい。また、上記により得られた沈殿、あるいは乾燥状態にある粉末、あるいは凍結乾燥により得られたスポンジなどの成型体に、有機溶媒(B)を添加し、洗浄する方法を用いてもよい。これらの精製方法により、反応に用いた縮合剤やカルボキシル活性化剤などの触媒類、反応せずに系中に残った未反応のリン脂質などを取り除くことができる。有機溶媒(B)中に懸濁している目的物を得るには、遠心分離、ろ過などの方法が利用される。また、ソックスレー抽出も、有機溶媒(B)による洗浄を行うために利用することができる。
<セルロース誘導体のハイドロゲル>
本発明の神経機能障害修復剤は、上述のセルロース誘導体を含有するハイドロゲルであり、水100重量部に対し、かかるセルロース誘導体を0.1~1.5重量部、好ましくは0.5~1.0重量部含むハイドロゲルである。
これらのハイドロゲルは、スパテルなどの金属へらで触ると容易に変形することが可能で、患部に塗布することが容易な状態であり、また注射器など細管を有する器具で注入することが可能である。
かかるゲルの好ましい複素弾性率としては、水中におけるポリマー濃度が0.5重量%、温度37℃の条件で、動的粘弾性測定装置を用い、角速度10rad/secで測定したときに1~200N/m2であるものが好ましく、1~100N/m2であるものがさらに好ましい。また、このときの損失係数が0.01~1.5であるものが好ましい。この範囲が損傷や変性を受けた神経の機能回復に最も効果があるからである。
また本発明のハイドロゲルは無色透明であり、製造の過程でごみなどの異物が混入した場合、これを検知することが可能であり、工業生産する上でのメリットを有する。
またハイドロゲル中に含まれる水以外の他の成分としては、触媒として用いた縮合剤類、縮合剤が所定の化学反応を経由することで生成するウレアなどの副産物類、カルボキシル活性化剤、未反応のホスファチジルエタノールアミン類、反応の各段階で混入する可能性のある異物、pHの調整に用いたイオン類などが考えられるが、これらの成分は、上記の有機溶媒(B)を用いた精製あるいは洗浄によって取り除かれており、いずれの化合物も、生体内に入れたときに異物反応として認識されない程度の低いレベルに抑えてあることが好ましい。
また本発明の神経機能障害修復剤の保存方法は限定されないが、例えば冷暗所に保管し、使用時に室温に戻して使用することができる。また本発明の神経機能障害修復剤の滅菌方法についても特に限定されないが、エチレンオキサイドガス滅菌、オートクレーブ滅菌、ガンマ線滅菌、電子線滅菌など一般的に医療機器、医療材料の滅菌に使用されている方法を用いることができる。
さらに、本発明の神経機能障害修復剤を手術後に用いる場合、手術をした部位とその周辺部に、例えば0.1~5.0mL程度を注射器で塗布し、術部全体を覆うことで損傷や変性を受けた神経の機能回復が期待できる。
(i)CMCNa:カルボキシメチルセルロースナトリウム(第一工業製薬(株)製、置換度0.73、または日本製紙ケミカル(株)製、置換度0.69)、
(ii)テトラヒドロフラン(和光純薬工業(株)製)、
(iii)0.1M HCl(和光純薬工業(株)製)、
(iv)0.1M NaOH(和光純薬工業(株)製)、
(v)4−(4,6−ジメトキシ−1,3,5−トリアジン−2−イル)−4−メチルモルホリニウムクロリド(国産化学(株)製)、
(vi)L−α−ジオレオイルホスファチジルエタノールアミン(COATSOME ME−8181、日本油脂(株)製)、
(vii)エタノール(和光純薬工業(株)製)、
(viii)注射用蒸留水(大塚製薬(株)製)、
(ix)消毒用エタノール(和光純薬工業(株)製)、
(x)ペントバルビタールナトリウム(ネンブタール注射液、大日本住友製薬製)、
(xi)NaCl(和光純薬工業(株)製)。
(2)セルロース誘導体中のリン脂質含量の測定
セルロース誘導体中のリン脂質の割合は、バナドモリブデン酸吸光光度法による全リン含量の分析により求めた。
(3)ハイドロゲルの複素弾性率および損失係数の測定
ハイドロゲルの複素弾性率および損失係数は、動的粘弾性測定装置であるRheometer RFIII(TA Instrument)を使用し、37℃、角速度10rad/secで測定した。
複素弾性率とは、弾性体の応力とひずみの比を表す定数をいう。損失係数とは、貯蔵剪断弾性率と損失剪断弾性率の比を表す定数をいう。
[実施例1]
(セルロース誘導体)
CMCNa(第一工業製薬(株)製、置換度0.73)3000mgを水600mLに溶解し、さらにテトラヒドロフラン600mLを加えた。この溶液に、L−α−ジオレオイルホスファチジルエタノールアミン1405mg(1.889mol)(CMCNaのカルボキシル基100当量に対し20当量)、4−(4,6−ジメトキシ−1,3,5−トリアジン−2−イル)−4−メチルモルホリニウムクロリド575mg(2.08mol)を添加した後、終夜攪拌を行った。攪拌後、テトラヒドロフランを除去し、水をある程度蒸発させたところで、エタノール中に加え、沈殿させた。ろ過によりエタノールを除き、再度、エタノールにて洗浄し、ろ物を真空乾燥することでセルロース誘導体を得て、そのリン脂質含量を測定した。反応前のカルボキシメチルセルロースナトリウムの置換度は0.73であり、すべてのカルボキシメチル基はナトリウム化されていると仮定し、リン脂質含量を用いて計算により、式(d)の置換度を求めた。式(d)の置換度は0.78mol%/糖であった。
(ハイドロゲル)
真空乾燥したセルロース誘導体からなる組成物を滅菌後、注射用蒸留水に溶解し、濃度0.5重量%のハイドロゲルを調製した。得られたハイドロゲルの複素弾性率および損失係数を測定した結果、それぞれ18.3N/m2および0.63であった。
[実施例2]
(ラット坐骨神経の機能障害作製)
日本チャールス・リバー(株)のLewisラット(3肢)を使用し、Ohsumiらの方法に従って坐骨神経の機能障害を作製した[Hidehiko Ohsumi,Hitoshi Hirata,Takeshi Nagakura,Masaya Tsujii,Toshiko Sugimoto,Keiichi Miyamoto,Takeshi Horiuchi:Plastic and Reconstructive Surgery 116(3):823−30,2005]。すなわち、ラットをペントバルビタールナトリウムの腹腔内投与麻酔下で側臥位に固定し、臀部を剃毛した後、消毒用エタノールで消毒した。腹部から背部に向け、臀部を4~5cm切開して坐骨神経を露出させた。坐骨神経の上周膜を1.5cm剥離し、さらに周囲の筋組織を灼焼した。その後、上周膜を剥離した坐骨神経周囲に実施例1のハイドロゲル(0.5mL)を塗布し、切開部の筋層および皮膚を縫合した。創傷部をイソジン消毒液で消毒した後、ケージに戻した。術後20日に動物をペントバルビタールナトリウム麻酔下で再度、坐骨神経を露出させ、NeuroPack(日本光電工業(株))を用いて神経伝達速度を測定した。有意差については、Student’s t−testで検定した。その結果、神経伝達速度は、術後20日でそれぞれ18.8±3.3m/s(平均値±標準偏差)であった。
[比較例1]
コントロールとして、ハイドロゲルを塗布せずに、実施例2と同様の操作を行い、神経伝達速度を測定した。その結果、神経伝達速度は11.8±3.6m/s(平均値±標準偏差)であった。
実施例2および比較例1の、術後20日における神経伝達速度測定結果を図1に示す。
以上より、術後20日の神経伝達速度は、比較例1より実施例2において、統計学的に有意に大きくなった。よって、実施例1で得られたハイドロゲルには、生体内において損傷や変性を受けた神経機能を修復する効果が高いことが確認された。
[実施例3]
(塩添加による複素弾性率増加)
CMCNa(日本製紙ケミカル(株)製、置換度0.69)3500mgを水100mLに溶解し、さらにテトラヒドロフラン100mLを加えた。この溶液に、L−α−ジオレオイルホスファチジルエタノールアミン413.7mg(0.0000795mol)(CMCNaのカルボキシル基100当量に対し5当量)、縮合剤として4−(4,6−ジメトキシ−1,3,5−トリアジン−2−イル)−4−メチルモルホリニウムクロリド169.4mg(0.0000874mol)を添加した後、終夜攪拌を行った。攪拌後、エタノール中に加え、沈殿させた。以下、実施例1と同様の操作を行い、セルロース誘導体を得た。置換度は1.0mol%/糖であった。セルロース誘導体からなる組成物20mgを注射用蒸留水1800mgに溶解後、最終濃度が0.9%となるように9%NaClを200mg添加し、最終濃度1.0重量%のハイドロゲルを調製した。得られたハイドロゲルの複素弾性率を測定した結果、134.5±1.4N/m2(平均値±標準偏差)であった。
[比較例2]
9%のNaClの代わりに注射用蒸留水200mgを添加すること以外、実施例3と同様の操作を行い、ハイドロゲルを調製した。得られたハイドロゲルの複素弾性率を測定した結果、8.0±0.5N/m2(平均値±標準偏差)であった。
実施例3および比較例2の複素弾性率結果を図2に示す。
以上より、比較例2より実施例3において複素弾性率が著しく増加し、複素弾性率5~200N/m2と低いハイドロゲルは生体内と同程度の0.9重量%となるようNaClを添加することで複素弾性率は著しく増加することが確認された。
[実施例4]
(セルロース誘導体)
CMCNa(第一工業製薬(株)製、置換度0.73)3000mgを水600mLに溶解し、さらにテトラヒドロフラン600mLを加えた。この溶液に、L−α−ジオレオイルホスファチジルエタノールアミン1405mg(1.889mol)(CMCNaのカルボキシル基100当量に対し20当量)、4−(4,6−ジメトキシ−1,3,5−トリアジン−2−イル)−4−メチルモルホリニウムクロリド575mg(2.08mol)を添加した後、終夜攪拌を行った。攪拌後、テトラヒドロフランを除去し、水をある程度蒸発させたところで、エタノール中に加え、沈殿させた。ろ過により、エタノールを除き、再度、エタノールにて洗浄し、ろ物を真空乾燥することでセルロース誘導体を得て、そのリン脂質含量を測定した。反応前のカルボキシメチルセルロースナトリウムの置換度は0.73であり、すべてのカルボキシメチル基はナトリウム化されていると仮定し、リン脂質含量を用いて計算により、式(d)の置換度を求めた。式(d)の置換度は0.78mol%/糖であった。
(ハイドロゲル)
真空乾燥したセルロース誘導体からなる組成物を滅菌後、注射用蒸留水に溶解し、濃度0.5重量%のハイドロゲルを調製した。得られたハイドロゲルの複素弾性率および損失係数を測定した結果、それぞれ18.3N/m2および0.63であった。

[比較例3]
コントロールとして、ハイドロゲルを塗布せずに、実施例5と同様の操作を行い、坐骨神経の組織学的観察を行った。その結果、術後1週での神経周膜の再生は不十分であった。
実施例5および比較例3の、術後1週におけるマッソン・トリクローム染色の結果をそれぞれ図3および図4に示す。以上より、術後1週での組織学的観察から、比較例3より実施例5において神経周膜の良好な再生が認められた。よって、実施例4で得られたハイドロゲルには、生体内において損傷や変性を受けた神経を修復する効果が高いことが確認された。
[実施例6]
(ラット坐骨神経変性作製)
日本チャールス・リバー(株)のLewisラット(3肢)を使用し、Ohsumiらの方法に従って坐骨神経を変性させた[Hidehiko Ohsumi,Hitoshi Hirata Takeshi Nagakura,Masaya Tsujii,Toshiko Sugimoto,Keiichi Miyamoto,Takeshi Horiuchi:Plastic and Reconstructive Surgery 116(3):823−30,2005]。すなわち、ラットをペントバルビタールナトリウムの腹腔内投与麻酔下で側臥位に固定し、臀部を剃毛した後、消毒用エタノールで消毒した。腹部から背部に向け、臀部を4~5cm切開して坐骨神経を露出させた。坐骨神経の上周膜を1.5cm剥離し、さらに周囲の筋組織を灼焼した。その後、上周膜を剥離した坐骨神経周囲に実施例4のハイドロゲル(0.5mL)を塗布し、切開部の筋層および皮膚を縫合した。創傷部をイソジン消毒液で消毒した後、ケージに戻した。術後6週に動物をペントバルビタールナトリウム麻酔下で坐骨神経を採取し、トルイジンブルー染色を行い、坐骨神経の組織学的観察を行った。その結果、術後6週で髄鞘の再生が認められた。
[比較例4]
コントロールとして、ハイドロゲルを塗布せずに、実施例6と同様の操作を行い、坐骨神経の組織学的観察を行った。その結果、術後6週での髄鞘の再生は不十分であった。
実施例6および比較例4の、術後6週におけるトルイジンブルー染色の結果をそれぞれ図5および図6に示す。以上より、術後6週での組織学的観察から、比較例4より実施例6において髄鞘の良好な再生が認められた。よって、実施例4で得られたハイドロゲルには、生体内において損傷や変性を受けた神経を修復する効果が高いことが確認された。
Claims (15)
- 0.5重量%水溶液について動的粘弾性測定装置で角速度10rad/secにて求められる複素弾性率が1~1000N/m2であり、かつ損失係数が0.01~2.0である多糖類誘導体のハイドロゲルを含む神経機能障害修復剤。
- 多糖類誘導体がセルロース誘導体である、請求項1に記載の神経機能障害修復剤。
- R4およびR5が炭素数9~19のアルケニル基である、請求項3に記載の神経機能障害修復剤。
- R4CO−および/またはR5CO−がオレオイル基である、請求項3に記載の神経機能障害修復剤。
- 水100重量部に対し、多糖類誘導体を0.1~1.5重量部含む、請求項1から5のいずれかに記載の神経機能障害修復剤。
- 多糖類誘導体のハイドロゲルから成り、生理的な塩濃度下において、1.0重量%水溶液について動的粘弾性測定装置で角速度10rad/secにて求められる複素弾性率が1~1000N/m2増加することを特徴とする神経機能障害修復剤。
- 多糖類誘導体が、下記式で表される繰り返し単位からなるセルロース誘導体であり、1.0重量%水溶液について動的粘弾性測定装置で角速度10rad/secにて求められる複素弾性率が5~200N/m2である、請求項7に記載の神経機能障害修復剤。
式中、R1、R2、およびR3はそれぞれ独立に下記式(a)、(b)、(c)、および(d)からなる群より選ばれるものであり、

−CH2−COOX (c)
式(c)中、Xはアルカリ金属またはアルカリ土類金属であり、
式(d)中、R4およびR5はそれぞれ独立に炭素数9~27のアルキル基またはアルケニル基であり、
(b)と(c)の置換度の合計が0.3~2.0であり、
(d)の置換度が0.001~0.05である。 - 末梢神経機能障害修復剤である、請求項1から8のいずれかに記載の神経機能障害修復剤。
- 末梢神経機能障害が絞扼性神経障害である、請求項9に記載の神経機能障害修復剤。
- 中枢神経機能障害修復剤である、請求項1から8のいずれかに記載の神経機能障害修復剤。
- 坐骨神経機能障害修復剤である、請求項1から8のいずれかに記載の神経機能障害修復剤。
- 神経伝達速度改善効果を有する、請求項1から8のいずれかに記載の神経機能障害修復剤。
- 神経周膜再生効果を有する、請求項1から8のいずれかに記載の神経機能障害修復剤。
- 髄鞘再生効果を有する、請求項1から8のいずれかに記載の神経機能障害修復剤。
