Abstract
Es wird eine Knöchelorthese mit einem U-förmigen Element aus einem Paar von einander gegenüberliegenden Seitenwänden (1, 2) aus jeweils einem formstabilen Außenteil (3) und einer inneren Auskleidung (4), einem mit den Seitenwänden (1, 2) verbundenen flexiblem Bodenteil (6) und einer die Seitenwände (1, 2) umschlingenden bandartigen Befestigungseinrichtung (7, 8) geschaffen. Damit nicht nur eine seitliche Führung des Fußes sondern auch eine Führung um die Knöchelachse möglich wird, ist ein über dem Rist spannbarer erster Gurt (14) und ein über der Ferse spannbarer zweiter Gurt (15) vorgesehen. In einer besonderen Ausführungsform ist eine einschiebbare Fußschale (22) vorgesehen.
Description
Knochelorthese
Die Erfindung betrifft eine Knochelorthese mit einem U-förmi- gen Element aus einem Paar von einander gegenüberliegenden Seitenwänden aus jeweils einem formstabilen Außenteil und einer inneren Auskleidung, einem mit den Seitenwänden verbundenen flexiblen Bodenteil, einer die Seitenwände umschlingenden bandartigen Befestigungseinrichtung zum Verbinden der Seitenwände und einem fersenseitigen Gurt. Diese Knochelorthese wird in einem Halbschuh getragen. Sie weist in einem Abstand oberhalb des Halbschuhrandes Ösen auf, die es ermöglichen sollen, das Schnürband des Halbschuhes auch durch diese Ösen zu ziehen, so daß ein Nachaußenkippen der Vorderseiten der Seitenwände beim Tragen des Schuhs vermieden wird.
Aus der US-PS 5 199 941 ist eine Knochelorthese aus zwei Seitenwänden bekannt, die im unteren Teil durch eine Art Kreuzwickelverband zusammengehalten werden. Aus der US-PS 5 372 576 ist eine therapeutische Einrichtung mit einer Fußschale mit sich über die Ferse nach oben erstreckendem Fersenteil bekannt, die nach Art einer Sandale mit Riemen am Fuß befestigt ist und nach Art einer Sandale getragen wird.
Aufgabe der Erfindung ist es, eine Knochelorthese der eingangs beschriebenen Art zu schaffen, die eine besonders gute Halte- rung für den zu stützenden Fuß darstellt. Nach einer anderen Ausführungsform soll eine Knochelorthese geschaffen werden, die vorübergehend bzw. wahlweise dem Fuß einen zusätzlichen Halt geben kann.
Diese Aufgabe wird durch Patentanspruch 1 und die weitere Ausführungsform durch die Knochelorthese nach Patentanspruch 7 gelöst.
Weiterbildungen der Erfindung sind in den Unteransprüchen gekennzeichnet.
Weitere Merkmale und Zweckmäßigkeiten der Erfindung ergeben sich aus der Beschreibung eines Ausführungsbeispieles anhand der Figuren.
Von den Figuren zeigen:

Fig. 4 einen Schnitt entlang der Linie III - III in Fig. 3;
Fig. 5 ein Zusatzteil für die Knochelorthese; und
Fig. 6 eine Seitenansicht der Knochelorthese mit dem Zusatzteil.
Die Knochelorthese umfaßt eine erste Seitenwand 1 und eine dieser gegenüberliegende zweite Seitenwand 2. Jede Seitenwand umfaßt ein als harte Schale ausgebildetes formstabiles Außenteil 3. Auf der inneren Seite des Außenteiles 3 ist jeweils eine innere Auskleidung 4 vorgesehen. Auf der dem Außenteil abgewandten inneren Seite der Auskleidung 4 ist eine körperverträgliche Innenauskleidung 5 vorgesehen. Die beiden Seitenwände 1 und 2 sind bodenseitig mit einem biegsamen Bodenteil 6
verbunden und bilden so ein U-förmiges Element. Im mittleren und oberen Bereich der Seitenwände sind Verschlußbänder 7, 8, 7', 8' angebracht, die zum Befestigen bzw. Umschlingen der um ein Bein angelegten Knochelorthese dienen. Die Verschlußbänder können als Klettverschlußbänder ausgebildet sein.
Die Auskleidung 4 ist als eine thermoplastische Mittelschicht ausgebildet, deren Material so gewählt ist, daß es bei einer Erwärmung auf etwa 80 °C an den Fuß des Kunden angeformt werden kann. Nach Beendigung der Erwärmung behält die Schicht die Verformung bei und übt somit einen vorbestimmten Widerstand für die Stützfunktion aus. Die schalenartigen Außenteile 3 weisen im Bereich des Knöchels vorgeformte Vertiefungen auf. Darüber hinaus weisen die Außenteile eine erste Ausnehmung 9 im Zentrum der für den Knöchel vorgesehenen Vertiefung und im mittleren und oberen Bereich eine sich in Längsrichtung der Seitenwand erstreckende zweite Ausnehmung 10 auf. Die Ausnehmungen 9, 10 dienen dazu, der Auskleidung 4 aus thermoplastischem Material das Nachgeben bei innen anliegendem Druck zu erleichtern.
Das Bodenteil 6 ist in dem gezeigten Ausführungsbeispiel durch zwei miteinander verbindbare einen Klettverschluß bildende Bänder 11, 12 gebildet. Die beiden Bänder 11, 12 sind jeweils mit ihren dem Klettverschluß abgewandten Ende mit der Innenseite des jeweiligen Außenteiles 3 fest verbunden. Alternativ kann das Bodenteil 6 aus einem einstückigen Band gebildet sein, welches an wenigstens einem seiner freien Enden 13 über ein an der Innenseite des Außenteiles 3 angebrachtes Klettband in seiner Länge einstellbar mit diesem verbunden ist.
Am bodenseitigen Ende jedes Außenteiles 3 ist zu dem einen Rand gegenüber der Mittenachse des länglichen Außenteiles versetzt ein erster Gurt 14 und in etwa der gleichen Höhe zur anderen Seite gegenüber der Mittellinie versetzt ein zweiter Gurt vorgesehen. Jeder der Gurte ist derart zweiteilig ausgebildet, daß ein Gurtteil 16, 16' mit dem Außenteil 3 der er-
sten Seitenwand 1 und der zweite Gurtteil 17, 17' mit dem Außenteil der zweiten Seitenwand 2 verbunden ist. Die Verbindung des ersten Gurtes mit den Seitenwänden erfolgt über ein als Niet ausgebildetes Drehgelenk 18, die des zweiten Gurtes über ein entsprechendes Drehgelenk 19. Jeweils eines der Gurtteile des ersten bzw. zweiten Gurtes weist ein Auflagepolster 20 bzw. 21 auf.
Im Betrieb erfolgt als erstes eine Voreinstellung der Länge des Bodenteiles 6 derart, daß die beiden Seitenwände 1, 2 mit ihrer vorgeformten Vertiefung für die Knöchel höhenrichtig angeordnet sind. Anschließend wird das thermoplastische Material auf etwa 80 °C vorerwärmt und durch Anlegen an das Bein des Patienten angeformt. In der so vorbereiteten Weise kann die Knochelorthese dann durch Anlegen und Festziehen der Verschlußbänder 7, 8 bzw. 7', 8' und Anlegen der Gurte 14 und 15 verwendet werden. Auf diese Weise erfolgt nicht nur eine seitliche Stabilisierung, sondern es wird zur Stabilisierung auch die Flexions- und Extensions-Kippbewegung des Sprunggelenkes eingeschränkt.
Es tritt häufig bei der Benutzung das Problem auf, daß der Benutzer in Situationen, in denen er keinen Schuh anhat, plötzlich aufstehen und ohne Schuh gehen muß. Auch in dieser Situation muß ein fester Halt zwischen Orthese und Bein gewährleistet sein. Damit er in dieser Situation trotzdem eine gute Führung haben kann, ist eine in Fig. 5 gezeigte Fußschale 22 vorgesehen. Diese ist aus einem Hartkunststo f geformt und weist ein bis zum Ballen reichendes Fußbett 23 mit einem daran anschließenden Rückenteil 24 zum Anliegen an Ferse und unterem Teil des Unterschenkels auf. Diese Fußschale 22 wird im Bedarfsfalle unter den fersenseitigen zweiten Gurt 15 und die beiden Verschlußbänder 7, 8 geschoben, so daß sie eine ausreichende Halterung besitzt.
Claims
1. Knochelorthese mit einem U-förmigen Element aus einem Paar von einander gegenüberliegenden Seitenwänden (1, 2) aus jeweils einem formstabilen Außenteil (3) und einer inneren Auskleidung (4) , einem mit den Seitenwänden (1, 2) verbundenen flexiblen Bodenteil (6), einer die Seitenwände (1, 2) umschlingenden bandartigen Befestigungseinrichtung (7, 8) zum Verbinden der Seitenwände (1 , 2) , und einem Fersengurt (15) und einem Ristgurt (14) .
2. Knochelorthese nach Anspruch 1, dadurch gekennzeichnet, daß die innere Auskleidung (4) eine thermoplastische Mittelschicht aufweist.
3. Knochelorthese nach Anspruch 2, dadurch gekennzeichnet, daß auf der dem Außenteil (3) abgewandten Seite der Mittelschicht (4) eine Innenauskleidung (5) aus körperverträglichem Material vorgesehen ist.
4. Knochelorthese nach einem der Ansprüche 1 bis 3 , dadurch gekennzeichnet, daß die Außenteile (3) jeweils Ausnehmungen (9, 10) aufweisen.
5. Knochelorthese nach einem der Ansprüche 1 bis 4, dadurch gekennzeichnet, daß das Bodenteil (6) als in seiner Länge einstellbares Band (11, 12) ausgebildet ist.
6. Knochelorthese nach einem der Ansprüche 1 bis 5, gekennzeichnet durch eine einschiebbare Fußschale (22) mit einem wenigstens einen Teil des Fußes umfassenden Fußbett (23) und einem an der Ferse und dem anschließenden Teil des Unterschenkels anliegenden Rückenteil (24) .
7. Knochelorthese mit einem U-förmigen Element aus einem Paar von einander gegenüberliegenden Seitenwänden (1, 2) aus jeweils einem formstabilen Außenteil (3) und einer inneren Auskleidung (4) , einem mit den Seitenwänden (l, 2) verbundenen flexiblen Bodenteil (6) , einer die Seitenwände (l, 2) umschlingenden bandartigen Befestigungseinrichtung (7, 8) zum Verbinden der Seitenwände (1, 2) , einem fersenseitigen Gurt (15) , und einer unter den fersenseitigen Gurt (15) schiebbaren Fußschale (22) mit einem Fußbett (23) und einem daran anschließenden Rückenteil (24).
8. Knochelorthese nach Anspruch 7, dadurch gekennzeichnet, daß die Fußschale im eingeschobenen Zustand von dem fersenseitigen Gurt (15) und wenigstens einem Verschlußband (7, 8) umfaßt ist.
