塑膠末端灌注/抽吸尖頭

發明背景
人類眼睛最簡單之功能為提供視覺,藉由傳遞光線通過一清澈外部,稱之為角膜,可藉由水晶體將影像聚焦至視網膜上。聚焦影像之品質取決於許多因素,包括眼睛之大小與形狀,以及角膜與水晶體之透明度。
當年齡、疾病、創傷等因素導致水晶體變得較不透明,便會由於進入光線減少而使傳送至視網膜之視覺惡化。眼睛水晶體之缺損在醫學上稱之為白內障。此病症一般可接受之治療方法為手術移除,以及以人工水晶體(IOL)置換水晶體。
發明概要
於此係提供在眼科手術時灌注與抽吸之儀器。
本發明之一實施例係提供一種儀器,包括一灌注套管,一抽吸管,以及一灌注/抽吸尖頭。該灌注套管,包含一定義出灌注通道之延長體。該抽吸管位於該灌注通道內,並定義出抽吸通道。該灌注/抽吸尖頭,可組合至並吻合抽吸管之末端。該灌注/抽吸尖頭可密封在灌注套管末端與抽吸管末端間之空隙。此外,在某些實施例中,該灌注/抽吸尖頭包括一凸緣側面(如一斜形部分),對應於灌注套管側面。在某些實施例中,該灌注套管與灌注/抽吸尖頭可互相嵌合,使該套管可引導某一方向之流體,且該灌注/抽吸尖頭可由另一方向上吸出物質。該灌注與抽吸方向可互相垂直。該灌注/抽吸尖頭之抽吸通道可於抽吸部分末端延伸。在某些實施例中,該灌注/抽吸尖頭可沿著縱軸方向延伸至鄰近於灌注部分之點。

其中一實施例係提供一種可在白內障摘除與其他眼科手術中降低病患傷口之儀器。更特別的是,該實施例係提供一種可降低,若無法完全消除,由於各種儀器微刺物產生囊袋撕裂之機率。該實施例提供便宜且為拋棄式之眼科用儀器抽吸管。在某些實施例中,在灌注套管與灌注/抽吸尖頭間灌注流體之滲漏現象可大幅消除或降低。本實施例可免去手術後,眼科手術儀器用抽吸管與灌注/抽吸尖頭清洗之需要。

在習知技藝中,要自眼睛中摘除水晶體,手術人員有時會使用具有灌注套管與抽吸管置於其中之眼科手術儀器。手術人員亦會使用矽膠製成之套管,其中具有一個洞,用於抽吸流體。手術人員會將套管套在抽吸管上,之後使用該儀器進行手術。雖然該套管常常難以使用。例如,該套管會被撕裂,因而必須移除更換受損之套管。此外,一旦裝至抽吸管上,這些套管便會自抽吸管上滑落,使其必須自抽吸管上更換。此外,由於這些套管一般僅會部分填滿抽吸管與灌注管間之空間,因此某些灌注流體便會由儀器末端滲出,而向前流入眼中。此種情況是不希望的,因為手術人員一般希望儀器可引導流體流向垂直於儀器之方向上,當抽吸物質在儀器末端之縱軸方向上時。
請參照第1圖,為進行眼科手術時眼睛10之橫剖面圖。示範之手術為白內障摘除術。眼睛10包括鞏膜12、視神經14、視網膜16、水晶體18、囊袋19、虹膜20、角膜22,以及瞳孔24。一般而言,水晶體18可使通過角膜22與瞳孔24之光線聚焦至視網膜16。視網膜16可將光線轉換為神經脈衝,其中該視網膜16會將脈衝沿著視神經傳至大腦。虹膜20可調節通過瞳孔24與水晶體18之光線,因而使眼睛10可適應各種程度之光線。囊袋19可將水晶體18維持於原處,且為透明的,使光線可通過。因此,沿著視神經14傳送之神經脈衝係對應於眼睛10所見到之景象。
然而,各種疾病、病症、受傷等皆會導致水晶體18變得渾濁、半透明等,直至希望自眼睛10摘除水晶體18。在此情況下,受影響之病患可稱之為患有"白內障"。通常當水晶體18自眼睛10移除時(即白內障摘除),手術人員會將水晶體18置換為人工水晶體,因而可恢復病患視力。Alcon Laboratories,Inc.(Fort Worth,TX)提供了人工水晶體範例,如人工水晶體。為了移除水晶體18,手術人員有時會使用儀器100。如第1圖所示,儀器100可包括一延長灌注套管102、灌注/抽吸尖頭104,以及接頭113。眼科用導管115可於接頭113位置與儀器100聯結,並可自灌注/抽吸機器供應灌注流體至儀器100上,並將眼睛10吸出之物質送回至灌注/抽吸機器上。接頭113可提供眼科用導管115與灌注套管102與灌注/抽吸尖頭104間之聯絡通道。此外,接頭113可與灌注套管102結合,而間接與灌注/抽吸尖頭104結合(經由一或多個內部元件),因而將這些元件102與104維持於一互相固定之操作關係。
為了摘除白內障,手術人員可切開角膜22與囊袋19。經由此切口,手術人員可將儀器100上之灌注/抽吸尖頭104插入水晶體18中。使用儀器100,手術人員可引導灌注流體自灌注/抽吸尖頭104流至水晶體18,因而使水晶體18崩解。之後灌注/抽吸尖頭104可自囊袋19吸出灌注流體、皮質物質以及水晶體18之崩解部分。一段時間後,手術人員可將儀器100自眼睛10取出,置入人工水晶體於眼睛10之囊袋19中,並縫合切口。
在先前技藝中,此手術過程中所使用之儀器表面上會出現微刺物,會吸附並撕裂囊袋19。此外,先前所使用之儀器會有灌注液前滲現象,會干擾自囊袋19吸出材料。前滲會降低各種外科技術之效果,並增加手術所需時間。儀器100之實施例中,具有非常平滑、無微刺物之表面。因此,儀器100之實施例可降低,若未能完全消除,由於微刺物造成之囊袋19撕裂現象,而同時增進眼科手術之速度與效果。
第2圖更說明儀器100包括灌注套管102、灌注/抽吸尖頭104、灌注口106、抽吸管108、抽吸口110、灌注套管102之末端112、灌注/抽吸尖頭104之凸緣114、灌注/抽吸尖頭104之末端116、灌注/抽吸尖頭104之斜形部分118、灌注/抽吸尖頭104之末端120,以及儀器100之縱軸122。抽吸管108可與灌注套管102共軸套合,二者皆可於各自之末端與接頭113(請見第1圖)接合。接頭113可提供眼科用導管115與灌注套管102與抽吸管108間之聯絡通道(請見第1圖)。因此,注入流體便可經由末端引導流過灌注套管102,並於縱軸122之垂直方向上流出灌注口106。灌注/抽吸尖頭104之抽吸口110可自其周邊(如第1圖之水晶體)吸出物質,經由抽吸管108送回灌注/抽吸機器中。抽吸口110之吸出物質方向,可與灌注口106引導流體之方向互相垂直。
第3圖顯示儀器100之橫剖面。此外,第3圖亦說明灌注套管102;灌注通道103;沖洗/抽吸尖頭104;抽吸通道105;灌注口106;抽吸管108;抽吸通道109;抽吸孔110;灌注套管102之末端112;凸緣114;灌注/抽吸尖頭104之末端116;灌注/抽吸尖頭104之斜形部分118、末端120;抽吸管108之縱軸122與末端124。更特別的是,第3圖說明灌注/抽吸尖頭104可與抽吸管108之末端124結合並吻合。在某些實施例中,灌注/抽吸尖頭104可外覆於抽吸管108上。灌注/抽吸尖頭104之抽吸通道105可與抽吸管108之抽吸通道109對齊與配合。在某些實施例中,灌注/抽吸尖頭104之抽吸通道105可末端延伸至抽吸口110。灌注/抽吸尖頭104之抽吸通道105可與抽吸口110連通,而使儀器100可經由抽吸口110、灌注/抽吸尖頭104之抽吸通道105、以及抽吸管108之抽吸通道109(之後經由拋棄式眼科用導管115),將灌注/抽吸尖頭104附近之物質吸出。自抽吸管108之抽吸通道109抽出之物質,可經由眼科用導管115送回灌注/抽吸機器(或其他拋棄式系統)(請見第1圖)。
灌注/抽吸尖頭104之凸緣114可緊鄰於灌注套管102之末端112,並可密封注入通道103,以對抗灌注套管102末端112之滲漏。灌注/抽吸尖頭104可延伸入灌注通道103某些距離,因而亦可密封灌注套管102之內壁。此外,灌注/抽吸尖頭104可延伸入灌注套管102至灌注口106之鄰近點,因而阻斷灌注通道103之流體,並引導灌注流體流出灌注口106。在某些情況下,灌注套管102之內表面可自灌注/抽吸尖頭104逐漸成斜形,直至接近灌注口106,因而允許流經灌注口106之流體通過灌注/抽吸尖頭104。在些實施例中,灌注/抽吸尖頭104可藉由灌注/抽吸尖頭104與灌注套管102內壁間之摩擦力,而維持於灌注套管102中,儘管灌注通道103間有壓力。例如,灌注/抽吸尖頭104與灌注套管102可成形並定型,以在灌注/抽吸尖頭104插入於灌注套管102時產生干涉配合。或者,在灌注/抽吸尖頭104與灌注套管102之間可有某些空隙。在某些實施例中,灌注/抽吸尖頭104可經由抽吸管108間接地與接頭113(請見第1圖)結合,而使其可維持於灌注套管102中,儘管其中有壓力在。灌注/抽吸尖頭104與接頭113之間接結合可使灌注/抽吸尖頭104維持於灌注套管102之末端,因而在元件104與102之間產生一密封。因此,灌注/抽吸尖頭104可預防灌注流體自灌注套管102,沿著縱軸方向122滲漏出。
第3圖亦說明了包含斜形部分118之灌注/抽吸尖頭104。斜形部分118中,灌注/抽吸尖頭104末端116之直徑,係與灌注套管102末端112之直徑約相同。因此,灌注/抽吸尖頭104之側面可對應於灌注套管102之側面。斜形部分118可往另一面逐漸變細,在距離灌注/抽吸尖頭104末端116某些距離處具有較小直徑。因此,灌注/抽吸尖頭104與灌注套管102間之界面可為平滑,並在儀器100插入眼睛10中時(請見第1圖)僅產生些微阻力或甚至無阻力。自斜形部分118之末端,灌注/抽吸尖頭104之表面可平行於縱軸122,自接近斜形部分118至接近抽吸口110之末端邊緣。
灌注/抽吸尖頭104可形成自各種塑膠、彈性體等,而灌注套管102與抽吸管108可形成自不銹鋼、鈦或其他適當之生物相容材料。在某些實施例中,灌注/抽吸尖頭104係形成自2558(其可得自Bayer MaterialScience L.L.C.,Pittsburg,PA)。因此,灌注/抽吸尖頭104可具有一光滑表面,無尖銳邊緣、微刺物等。因此,各實施例之灌注/抽吸尖頭104可預防囊袋19撕裂,因而加速病患復原,並降低病患由於某些眼科手術過程造成之不舒服。
此外,儀器100可由手術人員快速組合。儀器100可經由將抽吸管108(具有灌注/抽吸尖頭104包覆於上)滑入灌注套管102而組合。由於灌注/抽吸尖頭104到達灌注套管102之末端112,手術人員可將灌注/抽吸尖頭104與灌注套管102之末端112對齊。手術人員可將灌注/抽吸尖頭104推入灌注通道103,因而密封灌注套管102之末端112。若希望的話,手術人員可將灌注套管102與抽吸管108聯結至接頭113、眼科用導管115等(請見第1圖)。
手術人員可引導儀器100至眼睛10附近,並開始將灌注/抽吸尖頭104之插入端120插至切口處。灌注/抽吸尖頭104進入眼睛10,灌注/抽吸尖頭104之平滑表面可摘除所遇到的組織,而不會撕裂囊袋19,或使眼睛10受傷。當手術人員將儀器100置入眼睛10中時,斜形部分118亦可摘除組織,而不會撕裂囊袋19或使眼睛10受傷。因此手術人員可操作儀器100摘除白內障與其他希望之組織。當希望時,手術人員可將儀器100由眼睛10中移出。
手術人員可拆解儀器100,並丟棄灌注/抽吸尖頭104與抽吸管108。灌注/抽吸尖頭104與抽吸管108之製造費用比較低,因而可單次使用灌注/抽吸尖頭104與抽吸管108。因此,此實施例可免除手術人員在各種手術流程後,清洗與滅菌灌注/抽吸尖頭104與抽吸管108之步驟。此外,由於灌注/抽吸尖頭104與抽吸管108可預先滅菌,因此可免去手術人員在某些眼科手術前必須之灌注/抽吸尖頭104與抽吸管108(包括灌注套管102與灌注/抽吸尖頭104間之任何可能縫隙)清潔與滅菌步驟。
在先前技藝中,該手術儀器具有全面性直線側面。而在第4圖中,儀器300包括抽吸管302、灌注/抽吸尖頭304,以及用於與握把聯結之聯結部分306。抽吸管302彎曲半徑可經選擇,以延伸抽吸管302之完整長度或長度部分。抽吸管302之一或更多部分可維持直線,當軟管彎曲時。如第5圖所示,該彎曲部分可相對小,並具有小半徑,而抽吸管302之其餘部分則維持直線,使儀器呈現彎折狀態。例如,一直線部分可由聯結部分306延伸出至彎曲部分,而另一直線部分則由彎曲部分延伸出至灌注/抽吸尖頭304。在其他實施例中,彎曲側面可使用直線部分間之非彎曲界面達成。

Claims (13)

  1. 一種眼科用手術儀器,包括:一灌注套管,包含一定義出灌注通道之延長體,並具有一近端、一末端,以及一沿著該延長體長度方向之縱軸;一定義出抽吸通道之抽吸管,具有一近端與一末端,且位於該灌注通道內;以及一由塑膠製成之灌注/抽吸尖頭,可耦合至並吻合抽吸管之末端,其中該灌注/抽吸尖頭係外覆於該抽吸管上,其中在灌注套管末端與抽吸管末端之間存在有一空隙,且其中該灌注/抽吸尖頭會密封住該空隙。
  2. 如申請專利範圍第1項之儀器,其中該灌注/抽吸尖頭係定義出一斜形部分。
  3. 如申請專利範圍第1項之儀器,其中該灌注套管定義出一灌注口,其經定位以自垂直於縱軸之第一方向之灌注通道引導灌注流體。

  7. 一種眼科用手術儀器,包括:一灌注套管,包含一定義出灌注通道之延長體,並具有一近端、一末端,以及一沿著該延長體長度方向之縱軸,該灌注套管定義出一灌注口,其經定位以自垂直於縱軸之第一方向之灌注通道引導灌注流體;一定義出抽吸通道之抽吸管,具有一近端與一末端,且位於該灌注通道內;以及一灌注/抽吸尖頭,定義出一斜形部分,可耦合至並吻合抽吸管之末端,其中在灌注套管末端與抽吸管末端之間存在有空隙,且其中該灌注/抽吸尖頭可密封住該空隙,且其中該灌注/抽吸尖頭定義出一抽吸口,其經定位以自垂直於縱軸之第二方向之環境中抽吸出物質,且其中該第一與第二方向係互相垂直,其中該抽吸管定義出一抽吸通道,且其中該灌注/抽吸尖頭定義出抽吸通道之延伸,其向末端延伸至抽吸口,以及其中該灌注/抽吸尖頭沿著縱軸方向延伸至鄰近於灌注口之點。
  8. 一種單次使用、可拋棄式組件,可耦合至眼科用手術儀器上,其中該儀器包括一灌注套管,包含一定義出灌注通道之延長體,並具有一近端、一末端,以及一沿著該延長體長度方向之縱軸,該組件包含:一定義出抽吸通道之抽吸管,具有一近端與一末端,且位於該灌注通道內;以及 一由塑膠製成之灌注/抽吸尖頭,可耦合至並吻合抽吸管之末端,其中該灌注/抽吸尖頭係外覆於抽吸管上,其中當該可拋棄式組件耦合至儀器上時,在灌注套管末端與抽吸管末端之間會存在有空隙,而其中灌注/抽吸尖頭會密封住該空隙。
  9. 如申請專利範圍第8項之可拋棄式組件,其中該灌注/抽吸尖頭係定義出一斜形部分。
  10. 如申請專利範圍第8項之可拋棄式組件,其中該灌注套管定義出一灌注口,其經定位以自垂直於縱軸之第一方向之灌注通道引導灌注流體。
  11. 如申請專利範圍第10項之可拋棄式組件,其中該灌注/抽吸尖頭定義出一抽吸口,其經定位以自垂直於縱軸之第二方向之環境中抽吸出物質,其中,當該拋棄式組件耦合至該儀器上時,該第一與第二方向係互相垂直。
  12. 如申請專利範圍第11項之可拋棄式組件,其中該抽吸管定義出一抽吸通道,且其中該灌注/抽吸尖頭定義出抽吸通道之延伸,其向末端延伸至抽吸口。
  13. 如申請專利範圍第10項之可拋棄式組件,其中,當該拋棄式組件耦合至儀器上時,該灌注/抽吸尖頭沿著縱軸方向延伸至鄰近於灌注口之點。
