Description
本發明係有關於一種用於保護對抗HIV曝露及HIV感染治療的去免疫性單株抗體。
自發現HIV為引起「後天免疫不全症候群(Acquired Immunodeficiency Syndrome)」之20年中,以及自分子選殖與鑑定AIDS病毒開始起18年來,儘管已經進行了相當多關於AIDS疫苗之研究,HIV免疫療法的發展依然存在著許多困難。這些困難包括了第一類型HIV的高度變異性,病毒傳播途徑的多樣性,以及對於抑制HIV所需的免疫反應知識的缺乏。
在Moore等人所發表的一篇文獻中(1)指出,對抗不同變異性抗原決定區(variable exposed epitopes)的抗體只會對由自身取得的病毒(autologous virus)或是外套膜蛋白(envelop protein,env)相當接近的病毒分離株產生有效的中和作用。再者,可辨識更多env保留區段(conserved region)的單株抗體,乃是具有更佳的的免疫治療潛力之研究目標,不過它們幾乎不會有效中和多種不同之原始分離株(primary isolate)。對於可以產生廣泛抗各種病毒的抗體所需的知識依然不甚清楚,因此,目前在HIV疫苗這個領域內,只能把焦點放在至少可以提供部分保護作用的
方法上(2)。
HIV-1病毒原始分離株是由AIDS病人的週邊血液單核淋巴細胞(peripheral blood monoclear cells,PBMC)或病患未被感染PBMCs血清的培養所獲得。它們和感染人體的HIV品系很相近(3)。原始分離株可以很快的和實驗室中常用的T趨性(T-tropic)病毒,例如IIIb/LAI,SF2與MN加以區別出來,而這些病毒已被完全馴化到可以在人類T淋巴細胞株中生長與繁殖。
首先,大多數的HIV-1病毒原始分離株是M-趨性的-它們不容易在T細胞株上生長。反而,它們是屬於單核細胞(monocyte)或巨噬細胞(macrophage)-趨性的,具備感染初代T細胞之能力(4)。其次,原始分離株相當不容易在體外(in vitro)被重組可溶性病毒受體蛋白rsCD4中和(neutralization),和實驗室的病毒株相比,必須要多到200-2700倍之rsCD4的量才具有相當的中和效果(5)。第三,原始分離株對由gp120疫苗的產生的中和抗體有阻抗作用(6)。
在PBMC細胞培養中,原始分離株包括融合誘發分離株(syncytium-inducing isolate,SI)以及非融合誘發分離株(non-syncytiun-inducing,NSI)兩類。大多數之SI原始分離株可在高度HIV容納性之T細胞株MT2中進行複製,僅少數可以在較不容納(less permissive)之T細胞株CEM或H9中複製,CEM或H9乃實驗室中最常用來培養已經馴化過(adapted)的分離株。NSI原始分離株只能在來自週邊血液之初代培養T細胞(primary T cell)中培養。
有一些過於樂觀的誤解存在著--認為來自重組HIV-1病毒外套膜之疫苗(如gp120與gp160疫苗)可以產生有效的抗體反應。從實驗的外套膜疫苗臨床試驗所獲得的疫苗血清,通常只能夠在體外中和實驗室分離的HIV-1病毒病毒(7,8)。在那時,已在實驗室馴化過的HIV與原始分離株之差別仍然不是很清楚。當疫苗血清中之抗體被發現無法中和來自病人之HIV-1病毒原始分離株時,這個樂觀的想法才有所動搖。
利用疫苗來產生抗體之樂觀想法也部分源自於有關猿猴免疫缺乏病毒(Simian Immunodeficiency Virus,SIV)之不活化(inactivation)研究。由於和HIV-1病毒在型態,遺傳構造,以及感染與發病模式上的相似,SIV感染恆河猴(rhesus monkeys)成為研究AIDS疫苗與抗HIV抗體的良好模式。早期在恆河猴的研究顯示,將於人類T細胞培養出的SIV去活化與佐劑(adjuvent)製備成的疫苗,可以保護恆河猴免於被高度傳染性,且致病性的以人類T細胞培養出之SIV變異所感染(10)。出乎意料地是,若在將此SIV培養在同源的猴細胞中,這層保護作用就喪失了。
之後,有關在猿猴細胞生長之SIV與未感染之人類細胞的免疫研究顯示,先前的實驗所顯示不被SIV感染的保護作用可能是由於人類宿主細胞對異種(xenogeneic)蛋白所產生的免疫反應,而不是因為病毒抗原本身之刺激而產生(11)。涉及SIV之被動免疫(passive immunization)實驗指出,在缺乏細胞免疫反應(cell-mediated immunity)存在的情況之
下,特定之抗細胞抗體(anti-cell antibody)可能可提供保護宿主不被感染的作用(12)。
抗細胞抗體可以提供免疫保護作用之機制尚未被清楚描述,其中之一個可能的作用模式是經由阻礙CD4與HIV之結合位(binding site)來完成。長久以來,已知抗CD4抗體的預防感染作用是與CD4抗原決定區(epitope)有關,而非病毒株(13,14)。5A8是一個辨識CD4區塊2(domain 2)的單株抗體,實驗顯示該抗體在被SIV感染的恆河猴身上具有療效(15)。另外兩個抗CD4單株抗體,Leu3A與P1,它們是辨識CD4區塊1之類CDR-2環(CDR2-like loop),可以阻斷原始分離株對細胞的感染(5,14)。
抗CD4抗體可抑制SI與NSI品系HIV-1病毒的病毒株以病毒對細胞(virus-to-cell)或已感染細胞對未被感染細胞間的傳播(16)。宿主T細胞表面似乎具有一個與CD4連結在一起的抗原複合體(antigen complex)可以加速病毒與細胞的結合與進入,此成為一個抗細胞抗體的新研究目標(17,18)。
鼠源單株抗體B4(MAb B4)(17,18)也是一種抗細胞抗體,它乃是利用表現CD4分子之T淋巴球細胞作為一種免疫原而製造出來的。這些表現CD4之T細胞包括有:週邊血液單核T細胞,胸腺細胞,脾臟細胞(splenocyte),血癌(leukemia)或淋巴癌(lymphoma)衍生出來的T細胞株,如HPB-ALL或SUP-T1。MAb B4已經證實不論在體內(in vivo)或體外(in vitro)皆具有中和HIV-1病毒原始分離株及相關免
疫缺乏病毒(Immunideficiency virus)的能力。特別的是,MAb B4可以阻斷HIV與CD4分子之結合,所以它是一種進入阻斷劑(entry blocker),這是另一類可以治療HIV感染之抗反轉錄病毒藥物,它可以作為「高活性抗反轉錄病毒療法(Highly active anti-retroviral therapy,HAART)」的垂直治療藥物。
MAb B4乃直接對抗宿主細胞表面的抗原複合體--包含CD4蛋白與化學激素(chemokine)受體。B4辨識之位置乃位於CD4細胞外的區塊1之類CDR2環(CDR2-like loop),並與Leu3A之結合區不相同。依據美國5912176號專利指出,有強烈的證據顯示B4抗體具有廣泛的中和活性--對許多不同之HIV-1病毒原始分離株與HIV-2原始分離株及SIV都具有中和力。當針對不同細胞表面抗原的單株抗體混合在一起時,僅那些包含MAb B4的混合物會顯現有效之中合作用(column 24,line 1-10;column 47,line 5-10)。再者,在針對HIV-2原始分離株與SIV的中和力分析中,MAb B4對所有的品系均呈現出強效的中和作用,但對另一抗體anti-N-terminal V3 MN,已知可有效抗實驗室病毒株HIV-1病毒MN在此一試驗中卻沒有有效之中和效果。(column 45,line 19-35)。
其他中和性實驗也證實MAb B4在小鼠、黑猩猩與恆河猴皆具有保護作用。例如,若在帶有人類週邊淋巴細胞(PBL)之SCID小鼠感染由PBL培養出的HIV-1病毒之後,注射MAbB4抗體可干擾病毒對該細胞株之感染(18)。此外,在黑猩
猩予以感染黑猩猩馴化的HIV-1病毒後施打B4抗體,可阻止HIV-1病毒之感染(18)。再者,若於恆河猴身上施予一劑SIVmac251
,接著再施打4mg/Kg劑量的MAb B4,結果發現有75%之恆河猴可有效被保護不被SIV原始分離株感染(美國5912176號專利,column 50,line 15-22)。
由於HIV之感染過程中需要以CD4作為接受器,此現象暗示只要不產生不當的免疫調節作用,CD4分子應可成為免疫治療很好之標的。
然而,使用像B4這樣的鼠源抗體來治療人體或作為體內診斷試劑應用,人體會產生對抗鼠源抗體的免疫反應。此免疫反應所產生之「人類抗鼠源抗體之抗體(human anti-murine antibody,HAMA)」,會使原來抗體之效用大為減低。
抗體擬人化(humanization)之技術被設計來減少HAMA反應。例如,鼠源抗體可以被改造成人鼠嵌合(chimeric)抗體,亦即將來自鼠源之變異區(variable region)的DNA序列與人類的恆定區(constant region)之DNA序列相連接(19)。另外可藉由鼠源DNA序列可藉由互補決定區移植(CDR grafting)的方法得到較好的擬人化效果。此方法首先將剪出(excise)鼠源負責轉譯抗體重鏈(heavy chian)與輕鏈(light chain)變異區(variable region)之六組CDR的DNA,並將之移植到人類抗體架構區(framwork)之DNA序列上,此被改造後的鼠源Fv區段可用來作為人類免疫治療。此被改造後之變異區DNA序列再接著被接合至人類抗體恆定區之區
塊(20)。
然而,不論是嵌合抗體或移植抗體,對人體而言可能還是具有免疫性。並且,移植CDR至不相關之架構區(framework)可能會導致目標抗原與抗體間之親合力(affinity)喪失,有人便進一步利用序列同源性(sequence homology)與分子模型計算(molecular modeling)將鼠源抗體擬人化。這些方法被用來選擇對鼠源CDR與人類架構區(framework)更具同源性而可以保留高度結合親合力(binding affinity),並且減少結合所需之鼠源殘基序列。
既然將鼠源抗體擬人化是為了減少在人體的免疫刺激性(immunogenicity),去免疫性(deimmunization)乃成為另一個有用的方法。去免疫性技術乃是藉由移除鼠源抗體變異區(variable domain)可能在人體引發免疫反應之抗原決定位(epitope),來達到減少在人體產生免疫刺激性之目的。
事實上,一個有效的初級免疫反應(primary immune response)對抗治療用抗體包括了:處理(process)外來抗原,藉由MHC class II分子將抗原呈現T細胞抗原呈現位,然後刺激輔助型T細胞(helper T cell)。然後這些T細胞誘發並促進B細胞產生對抗外來治療用抗體的抗體。接著,倘若這些治療用抗體上一或多個抗原決定區被不成熟B細胞表面之免疫球蛋白(sIg)結合,同時又有適當的細胞激素(cytokine)存在,則B細胞會被刺激而進行分化、增殖,以提供可溶性之sIg,這些可溶性sIg將與治療用抗體結合而限制其效用,並且加速其由病人體內清除。因此,為避免這樣的初級免
疫反應發生,治療用抗體的B抗原決定區與T抗原決定區都應加以減少或修飾。
沒有了B細胞或T細胞免疫反應,治療用抗體的初級免疫反應將變得較為緩和或不存在。由Biovation公司(Aberdeen,UK)所發展出來之去免疫性技術(22,23)乃是在移除抗體分子可能會產生免疫性的B與T細胞抗原決定區。此技術被利用至本發明以去除Mab B4之免疫性。移除B細胞抗原決定區乃是利用「veneering技術」將一些替代性胺基酸來取代可能與引發免疫反應有關的胺基酸(24)。而T細胞抗原決定區的移除則是以胜肽通透(peptide threading)法,先辨識出治療用抗體變異區內的可能抗原決定區,再加以修改(25)。此外,鼠源抗體之恆定區則以人類抗體恆定區來取代(19)。
以人類、擬人化或去免疫性抗體來進行的被動免疫治療在HIV感染的治療或預防上,扮演了一個重要的角色。已知人類抗HIV單株抗體2F5、2G12可以中和HIV之原始使分離株,並且在被HIV感染之病人身上做研究,可以觀察到病毒量的暫時性下降與CD4 T細胞數目的暫時性增加(26)。這些抗體亦被使用在非人類靈長類動物模式中進行預防母體-嬰兒垂直感染之研究。不論是在胎兒生出前透過靜脈注射母體,或是在小恆河猴出生後再直接施打(infusion)抗體,這些初生動物均得到不被SHIV(simian/human immunodeficiency virus)感染的保護作用。作者的結論是,利用結合抗病毒單株抗體來進行的免疫預防作用
(immunoprophylaxis)是預防母體-胎兒垂直感染HIV的方法(27)。
既然特定的抗CD4單株抗體可以有效阻止淋巴球細胞與巨噬細胞不被HIV-1病毒感染,而且所具備的中和力遠好過其它單株抗體(5,18,28),像MAb B4這樣以受體為導向的單株抗體可能是更好的預防性抗體及治療性抗體。為達此目的,5A8已被重新設計過--利用CDR移植之技術--成為擬人化之IgG4(28)。作為IgG4的isotype,擬人化5A8抗體在CH2區塊(CH2 domian)上缺少醣化點(glycosylation site),而此處之醣鏈與補體固定作用(complement fixation)有很大的關係。這項特性可以令該抗體較不容易導致病人體內之CD4數下降(deplete)。擬人化5A8抗體目前已羥進入人體臨床試驗(29)。
此外,抗CD4單株抗體已在人體上顯示治療類風濕性關節炎之臨床效用(30)。另一個人類單株抗體HiMax-CD4亦已羥進入治療風濕性關節炎與牛皮癬之人體臨床試驗(31)。這些應用顯示出,像mAbB4這樣的去免疫性抗體,具有治療HIV感染以及與CD4分子有關之自體免疫疾病的潛力。

此發明有關於針對來自鼠源抗體B4之去免疫性抗體,該抗體具備極高的潛力於免疫治療AIDS及其他與CD4相關之疾病。此發明同時也指出以此抗體為主之免疫治療方法。更特別的是,由#7、#16、#21細胞株所表現的抗體包含了來自鼠源B4單株抗體之重組基因B4DIVHv1/VK1#7,B4DIVHv1/VK1#16,與B4DIVHv1/VK1#21。鼠源單株抗體B4已經被證實對位於CD4+細胞上HIV病毒之CD4受體複合體具有專一性,對多種HIV之原始分離株與相關免疫缺乏病毒株均具有中和能力(17,18)(B4已於1999年7月15日取得美國5912176號專利)。
此重組去免疫抗體之表達是將一段MAb B4 Fv片段與人類IgG1 Fc片段之cDNA嵌合而來。接下來將嵌合基因(chimeric gene)之鼠源Fv片段之核甘酸(nucleotide)序列進行去免疫作用--利用點突變方法將「對人體而言外來的」T細胞與B細胞抗原決定區(epitope)予以移除,如此這抗體才能用於人體治療。共設計4段不同之去免疫VH
cDNA與3段去免疫VK
多核甘酸鏈(polynucleotide chain)產生出來。其中一抗體並以定點突變之基因工程技術被加以改造,使成為不具醣鏈(aglycosylated)之IgG1。此種之去醣化抗體將無法與補體(complement)結合,因此也就不會引發CD4細胞之「補體媒介細胞溶解(complement-mediated lysis)」的現象。
這個新的去免疫性抗體保留了和原來鼠源單株抗體一樣的專一性與中和力。
這些由NSO或CHO細胞所表現的抗體已經成功地被馴化成無血清培養模式。這些去免疫性抗體也可能可以轉殖植物(transgenic plant)大量生產以減低生產成本。此抗體之發明可以用於預防HIV感染,治療HIV感染或治療與CD4分子有關的自體免疫疾病,如類風濕性關節炎。

鼠源單株抗體Fv片段的去免疫性是利用找尋出位於鼠源序列上可能於人體產生強烈的免疫性的T和B細胞抗原決定區並將之移除,而去除T細胞抗原決定區是利用一個三維的「胜肽通透」(peptide threading)法(22,25)分析抗體Fv內可能與MHC class II結合的區塊。並移除變異性區域上至少一個或全部的裸露在表面且不會妨礙抗體的辨識結合功能(22,24)的B細胞抗原決定區,接著,將鼠源單株抗體的恆定區(CH
和Cκ)整個移除,並用人類IgG1的恆定區的
DNA序列來取代,此嵌合抗體即含去免疫性的鼠源B4單株抗體變異性區域及人類IgG1的恆定區。

在例三詳細的敘述去免疫性變異性區域核苷酸序列片段的建構方法,包括了5’外側序列、領導訊號胜肽(the leader signal peptide)、領導插入序列(leader intron)和鼠源免疫球蛋白啟動子(promoter),3’外側序列、剪接位置(the splice site)以及插入序列(intron)等序列的建構方法。
去免疫性的重鏈和輕鏈變異性區域建構完成後,將它們移殖到含人類IgG1 CH
或區域的表現載體中,而篩選哺乳類細胞(第3圖,第4圖)的標籤基因也被包含在載體中,然後將此重組載體送入NS0細胞中表現,然後從培養基中純化出被表達的去免疫性抗體,並測試與重組可溶性CD4(rsCD4)的結合以及對抗HIV-1病毒的中和能力。
由ELISA結果顯示這十二個不同的B4單株抗體對於
rsCD4的結合親合力不同,利用從HIV-1病毒次群體A,B,C,D,E中的S1原始分離株以及從T細胞中分離出的HIV-1MN
做MT-2微溶體試驗(microplaque assay)(36)結果顯示這十二個不同的去免疫抗體也具有不同的病毒中和能力。比較四種重鏈的的變異(VHV
,1-4),所有與去免疫性的鏈結合的版本1(),顯示擁有版本1重鏈均具有較高的對抗所有HIV-1病毒原始分離株的病毒中和力,並接近鼠源B4單株抗體HIV-1VL

去免疫性B4抗體對於抗T細胞株馴化的HIV-1MN
病毒是無效的,這一點與鼠源抗體一致。具鏈版本2與3的去免疫性抗體其病毒中和能力較低(表2)。在這十二個不同抗體的比較結果顯示,含DIVHv.1及DIVKv.1的去免疫性B4抗體,對於所有的HIV-1病毒原始分離株具有高度的中和能力,且接近鼠源B4單株抗體對抗HIV-1病毒的能力。
為生產去免疫性的B4單株抗體,任何適當的表達系統都可能被使用。例如動物細胞之哺乳類細胞株,或植物細胞等真核細胞。較常見的表達去免疫性抗體系統有哺乳類細胞像NS0細胞或CHO細胞。NS0細胞是一株來自歐洲動物細胞保存中心(European Collection of Animal Cell Culture,Porton UK(ECACC no.85110505)本身不表達免疫球蛋白產生的小鼠骨髓瘤細胞,而CHO細胞是來自於美國種源保存中心(Type Culture Collection(ATCC no.CRL-9096)一種缺乏二羥化葉酸還原酶(dihydrofolate reductase)的細
胞(CHO dhfr-
)。
為了高度表達重組抗體,二羥化葉酸還原酶(dhfr)缺損的CHO細胞是最常被用來做基因複製、重組的哺乳類表達系統,因為這些細胞被發現會隨著基因重複數目的增加而增加產量,有文獻顯示可以產出專一性抗體產量高達100μg/106
細胞/天(42)。
在這些例子,傳統用來於哺乳類細胞表達的啟動子(promoter),如病毒系統中常被使用的人類cyomegalovirus immediate early(CMV)啟動子(37),其他哺乳類細胞表現啟動子在在專利中適合基因表現的也能被使用,如反轉錄病毒(retrovirus),多瘤病毒(polyomavirus),腺病毒(adenovirus)和猴類病毒40(SV40)或哺乳類細胞衍生的啟動子像是人類延長因子(elongation factor)1 α啟動子(38,39)。其中免疫球蛋白啟動子常被使用在像NS0細胞的淋巴細胞,此載體的設計包含了免疫球蛋白啟動子、領導訊息胜肽(leader signal peptide)、領導插入序列(leader intron)、剪切位置(splice site)、插入序列(intron)及含為了插入任何變異性區域的適合限制酵素切位,而其建構的描述見例二。這些載體可方便於NS0細胞以ELISA及生物活性篩選表達去免疫性抗體。
在哺乳類動物細胞中,重組蛋白質的高度表現可經由轉植DNA的放大而篩選出穩定的細胞株,所重組的基因在基因放大期間會跟著篩選標識基因共同放大。本發明中使用CHO dhfr-
細胞及DHFR基因作為放大篩選的標示;另一方面,由於骨髓瘤及融合瘤細胞株短缺的內生性GS活性,
因此麩胺醯胺對這些細胞生長是絕對需要的,因此麩胺醯胺合成酶(glutamine synthetase,GS)基因可被用來當作一個主要的篩選標識,轉植GS基因可使骨髓瘤及融合瘤細胞株生長在沒有麩胺醯胺的培養基中(41)。
例子5為表現抗體及DHFR在CHO dhfr-
細胞的質體建構描述,利用逐漸增加methotrexate(MTX)濃度(42)以放大DHFR基因表現的程度來增加去免疫性B4抗體的產量(42)。
經由幾次逐漸增加培養基中MTX濃度以培養細胞,篩選後得到具專一性抗體產量達10-30 pg/細胞/天的重組CHO(rCHO)細胞。
雖然去免疫性抗體多表現在像NS0或CHO細胞的哺乳類細胞,但為了降低大量的生產成本,也可能用轉植植物來表現;見例十。
所有的抗體都是屬於醣蛋白,IgG1在CH2(43)區域有一個雙叉的複雜N-端連結的碳水化合物,這些碳水化合物對抗體所擁有之作動器功能(effector function)像是補體固定作用(fixation)和依抗體性細胞毒性(antigen-dependent-cellular-cytotoxicity,ADCC)是重要的,這些作動器功能會使得抗體標的細胞(44)被移除(44)。而B4抗體目標是CD4接受器複合體,因此經由IgG1的作動器功能與補體結合可能造成CD4+

法可去除IgG1 Fc區域的醣基以阻止IgG1與人類FC
R1結合,進而活化的補體進而與C1 q結合(45)。
有兩個常用的去醣方法:利用tunicamycin處理抗體產生細胞,抑制其碳水化合物的前趨物吸附到天門冬醯胺(asparagines,Asn)(46);或用突變方法將第297位置的胺基酸Asn改成其它胺基酸,本發明中的去免疫性抗體與N連結醣化作用有關的第297Asn胺基酸已被移除。利用定點突變PCR將重鏈表現載體中第297 Asn胺基酸的AAC密碼改成His胺基酸的CAC密碼,並以CHO細胞表達去醣化去免疫抗體(第16圖)。
對於用來做人類治療藥物,去免疫性單株抗體的功能特性及安全性是重要的,所以它們需要被定性。在此發明中,已經在體外(in vitro)評估本去免疫性單株抗體的:(1)中和HIV-1病毒病毒;(2)補體固定作用;(3)對人類淋巴細胞引的免疫性。
以MT-2微溶體試驗法比較B4鼠源單株抗體及得自CHO細胞去免疫性單株抗體的HIV-1病毒中和力(36);表3顯示由21號細胞株表現去免疫性抗體DH1DK1CHO#
21,與由B4融合瘤細胞產生的鼠源抗體HIV-1病毒中和力比較結果。而50%中和五種品系HIV病毒(A-E品系)的效果與鼠源B4抗體比較,去醣去免疫性抗體DH1DK1CHO#
7顯示有明顯較高的中和品系B的病毒23135、品系D的病毒UG046以及品系E的病毒TH036能力(表3)。
補體消耗試驗(a complement consumption assay)是用
來評估比較去免疫性抗體及它們原本極相像的鼠源B4單株抗體補體固定作用(fixation)的特性(例子7),從這試驗結果得到B4去免疫性抗體可使補體固定作用(fixation)的特性降低(第17圖),由收集反應的上清液顯示,去免疫性抗體比鼠源單株抗體剩餘較多的補體,意味著去免疫性抗體比鼠源單株抗體在與人類CD4+
細胞和CD4複合體結合時,較不會引起補體固定作用(fixation),而原本的鼠源抗體在和CD4細胞表面複合體結合時,會較有效的固定補體;因此,從混合物的上清液試驗反應表示鼠源B4抗體剩餘較少的補體去溶解致敏化的羊紅血球(sensitized SRBC)。這個研究得到一個結果,從它們降低補體固定(fixation)的能力,可知去免疫性抗體DH1DK1CHO#
7和DH1DK1CHO#
21比鼠源B4單株抗體具較高的安全性,而其中以去醣去免疫性抗體最不會與補體結合。
鼠源單株抗體在當作人類治療用藥所產生的免疫性(immunogenicity),可能會引發人類T和B細胞的反應而產生對抗治療用抗體的抗體,而引起T細胞活化的結果會製造調節人類免疫反應的細胞激素,其可對抗特殊感染或癌症。因此由鼠源抗體引起的細胞激素的混亂,可能引發不平衡或損害對抗病源或癌症的免疫反應。
B細胞的活化會引起宿主產生人類對抗鼠源抗體的抗體,而導致抗原抗體免疫複合體形成及這些複合體在不該存在的組織中存積,進而可能造成人體的發炎或病變。另外,人類對抗鼠源抗體的抗體也會與所施予的抗體藥物結
合,而降低抗體有效濃度,進而降低治療用抗體藥物到達它所該達到的標的,例如CD4+
細胞。
基於以上的考量,我們測試去免疫性單株抗體和鼠源單株抗體B4,在體外(in vitro)人類週邊血液單核細胞(PBMC)的免疫性(見例8)。此研究結果如表4所示,純化的鼠源B4抗體會刺激人類週邊血液單核細胞(PBMC)產生IL-10和TNF-α
,而一般被認為可被抗原活化Th1 CD4細胞所產生的IFN-γ
和IL-2,以及被Th2 CD4細胞所產生的IL-4都沒有被偵測到。
相反的,去免疫性抗體DH1DK1CHO#
16和去醣DH1DK1CHO#
7在相同的試驗中顯示都不會刺激人類週邊血液單核細胞(PBMC)分泌這五種細胞激素,此結果顯示這些去免疫性單株抗體有較高的藥物安全性。
本發明中也建立無血清培養基以降低生產成本及法規對於由於血清的疑慮。由於細胞培養過程中常使用動物血清,有機會伴隨而來的一些不預期病原例如病毒或其他感染原(例如狂牛症)的污染。再者,使用動物血清當一個原始的材料對於大量細胞培養過程的經濟效益具有負面的衝擊。最後,若哺乳動物細胞分泌的蛋白質生物製劑是分泌到培養基中,不含血清培養基的使用則大大的簡化開發及下游蛋白質純化的製程。
重組CHO細胞依賴血清生長的特性需要馴化成為不需血清、懸浮的培養以利於產程放大。例子6描述成功的馴化重組CHO細胞株適應懸浮的生長,並於血清的培養基中所
表現的抗體量,與細胞在單層培養與含10%牛血清培養基時一樣,且抗體的HIV-1病毒中和力要與這些抗體在單層培養與含10%牛血清培養基時相同。
總體而言,這些結果強力地支持本去免疫性抗體可作為安全的治療性藥物以治療並預防HIV病毒感染。
特殊給藥的方式有各種技巧,抗體給藥的方式不限定在腸胃外的,和直接注射在罹患位置,包含腸胃外的給藥方式但不限定在靜脈、肌肉、腹腔以及皮下。
本專利包括上述去免疫性抗體的組成,適合腸胃外施藥方式,包含但不限定無菌等張溶液,這些溶液包括但不限定用於靜脈、肌肉、腹腔、皮下注射,或直接注射在罹患位置或其他位置的鹽類以及磷酸鹽溶液。
以本專利發明的去免疫性抗體用予接受治療的哺乳動物時,特別是人類,施打抗體的劑量與哺乳類動物的年齡、體重、身高、性別、一般醫療情況、過去醫療病史以及喜好等因子相關,而決定適當劑量、給藥方式及頻率等技術知識已知,其他治療性抗體的劑量可推算本抗體之用量,在此專利發明的範圍不需過多的實驗來決定。
本專利發明中的去免疫性抗體可能施打於病人,藥學上可接受的劑量形式,適合施打於靜脈、皮下以及肌肉的形式,此種劑量形式包含藥學上可接受的固有、無毒和無治療性的攜帶者,包括離子交換劑(ion exchangers)、鋁化物(alumina)、硬酯鋁酸(aluminum stearate)、卵磷脂(lecithin)以及血清蛋白(serum proteins),例如人類血清蛋白,緩衝溶
劑例如磷酸、甘胺酸、甘露醇、山梨酸、鹽酸、山梨酸鉀、飽和植物脂肪酸的乾油脂混合物、水、鹽類,或如硫酸鉀、磷酸氫鈉、磷酸氫鉀、氯化鈉以及磷酸鈉等電解質。去免疫性抗體常被配成0.1-100mg/ml的濃度。
藥物組成可以被製成已知技藝的配方,例如Remington’s Pharmaceutical Science(51)中所描述的。
一般而言,提供去免疫性抗體給治療者的最初劑量,範圍在1~100mg/kg,常用的劑量是在1~25mg/kg,更被常用的是在1~10mg/kg,最被常用的是在2~5mg/kg,例如,採用一或多次分別或持續的注射。一般而言,抗體以靜脈(IV)或肌肉(IM)注射,重複施打幾天或更久,依據情況重複治療直到疾病發生的症狀抑制,或至病人病情已達改善。此治療也可能重複施行一段期間從一週一次到每六個月一次。最適劑量、施打方式及頻率為現今之工藝,類似其他去免疫性抗體的劑量,在此專利發明的範圍中不需過多的實驗來決定。
此專利發明的去免疫性抗體亦被用於與其他抗HIV專一性的抗體結合使用,例如抗gp41和gp120的抗體。這些包括但不限定,如Hofmann-Lehmann等人的描述對gp41具專一性的2F5抗體及對gp120具專一性的2G12抗體(27)。
下面的例子闡述本專利發明應用性之不同過程,這些例子僅說明目的,並不被限定為此專利發明的範圍。
鼠源單株抗體B4基因的選殖及定性
1.總RNA的製備
以Promega(Madison,WI)之SV40 Total RNA Isolation System(Cat.No.Z3100),根據操作手冊,萃取5x106
個融合瘤B4細胞之總RNA。
2.將鼠源的重鏈變異區(VH)及輕鏈變異區(VL)之cDNA進行放大
使用GeneAmp RNA PCR Kit(Perkin Elmer,Norwalk,CT,part no.N8080017),根據操作手冊,合成單股cDNA並且放大重鏈變異區(VH)及輕鏈變異區(VL)簡言之,鼠源的重鏈變異區(VH)及輕鏈變異區(VL)的單股cDNA是從1 ug的總RNA藉由MuLV反轉錄酶(Reverse Transcriptase)及寡酸苷酸(oligo dT)當引子,進行RT-PCR而來的。反應混合物乃於42℃反應15分,在99℃反應5分,然後在5℃反應5分。
鼠源的重鏈變異區(VH)及輕鏈變異區(VL)的單股cDNA是以AmpliTaq DNA聚合酶進行放大,所使用的放大引子為”The mouse-specific Ig Primer Sets”(Novagen,Madison,WI,cat.no.69831-1)。反應混合物起始加熱溫度為95℃,反應1分45秒,之後在95℃反應15秒接下來60℃反應30秒,共35個循環。之後這反應混合物維持在72℃,10分鐘。所得到的放大DNA用洋酯瓊膠(gel)純化並將它轉殖至pGm T Easy(Promega,cat.no.A1360)的質體上。而所得到的這些VH及的細胞株,再以聚合酶連鎖反應(PCR)進
行篩選,來確認所要插入序列的大小。
利用雙去氧鏈終止法(Dideoxy chain termination method)將VH及的DNA插入序列進行DNA定序。互補決定區域(complementarity determining regions,CDR)的位置是參考其他抗體序列庫所決定的(32)。B4 VH被分配至鼠源重鏈次級群體V(B)而B4被分配至鼠源kappa鏈次級群體III(32)。DNA、胺基酸序列及VH及的互補決定區域分別表示於第1圖及第2圖。
構築嵌合B4抗體基因(chimeric B4 antibody genes)及利用NS0細胞表現
1.構築嵌合B4抗體基因
嵌合B4抗體基因包含鼠源-人類的抗體鏈(mouse-human antibody chain),這是將鼠源的單株抗體B4的變異區域基因(variable region genes)與人類的免疫球蛋白恆定區域(constant region genes)結合,在某種程度上與Morrison等人所描述的內容相似。以這種方式,此完整地鼠源變異區域被連到人類的恆定區域。當要測試經過去免疫性變異區域(deimmunized variable regions)的B4抗體,此嵌合抗體提供了一個有用的含有相同人類恆定區域的無去免疫性控制組(undeimmunized control)作為比較。
為了構築嵌合B4抗體,VH-PCR1及VK-PCR1質體(20)被使用來當模板以引入5’邊端序列-包括領導訊息胜肽(leader signal peptide)、領導插入序列(leader intron)及鼠源
免疫蛋白啟動子(murine immunoglobulin promoter),及3’邊端序列(flanking sequence)-包括剪切位置(splice site)及插入序列(intron sequences),接於VH及的周圍。鼠源HV及VJ表現卡匣(expression cassettes)被接入pUC19而且整個DNA序列也進行確認。
鼠源VH及VL表現卡匣從pUC19以HindIII及BamI被切出。之後再轉至表現載體pSVgpt及pSVhyg中(20)(第3圖及第4圖),分別包括人類IgG1(48)或是恆定區域(49)及在哺乳動物細胞中的篩選標識(selection of marker)。利用DNA定序去確認在pSV表現載體上的VH及。
2.表現B4嵌合抗體的寄主細胞
表現抗體的寄主細胞為NS0,NS0這種細胞本身是不會產生免疫球蛋白的鼠源骨髓癌細胞,購自歐洲動物細胞培養保存中心(European Collection of Animal Cell Cultures,Porton UK(ECACC No.85110505))。藉由點轉染法,嵌合重鏈及輕鏈表現載體被共同轉染(cotransfect)至NS0。表現gpt基因的細胞乃於含有10%胎牛血清的Dulbecco’s Modified Eagle’s Medium(DMEM),0.8 ug/mL mycophenolic acid及250 ug/mL xanthine中被篩選。
3.利用人類IgG ELISA作抗體表現之偵測
藉由ELISA偵測人類IgG的產量。方法如下:將溶於carbonate/bicarbonate coating buffer pH 9.6(Sigma,cat.No.C3041)的抗人類k抗體覆蓋於ELISA微定量盤孔表面上,作用於37℃,1小時。再以PBST(PBS含0.05% Tween 20)洗3
次。此步驟是會形成固相免疫吸附層以捕捉培養基中的k鏈。
從轉染細胞培養出的培養基覆蓋於定量盤孔中,作用於37℃,1小時,然後倒掉。再以PBST洗3次。二級抗體為過氧化酶嵌合的羊抗人類IgG γ鏈的專一性抗體(Peroxidase-conjugated sheep anti-human IgG γ chain specific antibody)(The Binding Site,Cat.no.AP004),而o-phenylenediamine(OPD)溶液(Sigma,cat.No.P7288)為呈色指示劑。分泌人類抗體的細胞株以此被挑選及放大。
4.嵌合抗體的生產
分泌最高量的嵌合抗體之NS0轉植株被挑選及放大。抗體是用Prosep A親和色層管柱(Millipore Cat.No.113112722)從500 mL至1000 mL之培養基中純化而來。抗體以0.1M甘胺酸(glycine)pH 3.0引流出並中和以及以PBS透析。純化好的抗體經過過濾使呈無菌並儲存於4℃。抗體的濃度以IgG ELISA與參考標準品0.1,1,5,10 ng比較來定量(The Binding Site,cat.No.BP078)
5.比較rsCD4對嵌合抗體與鼠源抗體的相對結合親合力(relative binding affinity)
嵌合抗體對重組可溶性CD4(rsCD4)的相對結合親合力由ELISA所測定
將0.25 ug/ml rsCD4(American Biotechnologies,Columbia MD)溶於碳酸溶液(carbonate buffer)pH9.5並用含3% BSA,0.05% Tween 20的PBS進行去活化。加入不同
稀釋倍率的嵌合抗體、鼠源抗體及正性控制組。嵌合及鼠源抗體的偵測分別是用過氧化酶嵌合的羊抗人類IgG(peroxidase conjugated goat anti-human IgG)及羊抗鼠源的IgG(sheep anti-mouse IgG)。用OPD呈色。
當加入固相rsCD4,在所有的嵌合及鼠源抗體濃度下,A492
的吸光讀值增加。這個試驗確定所選殖的及定序的重鏈及輕鏈的可變區域仍保有正確的辨認位置。與原鼠源抗體相比較,嵌合抗體在A492
中顯示減少10倍。
去免疫可變區域序列之設計及構築
6.去免疫序列之設計
鼠源B4 VH及Vk的胺基酸序列與人類胚源VH及Vk的序列進行比對並且也與人類胚源J區域序列進行比對(49)。人類胚源VH,J區域,Vk序列對於鼠源B4 VH及Vk有高同源性,所以被選來作為人類架構區的參考序列。
隨著人類架構區參考序列的確認,在B4 VH及Vk架構區表面曝露的某些非完全相同的胺基酸及其殘基,被改成與人類參考序列相對應的胺基酸(24)。而對於抗體結構及結合相關的重要殘基,不被包括在此過程並且保持不變。舉例說明,在N端的鼠源殘基及位置靠近抗體CDRs,的重要殘基被保留下來。這過程所產生的序列大致上與「被修飾的抗體」(veneered antibody)過程相似,將抗體表面的殘基改為人類的殘基而埋藏在內的殘基則保持原始鼠源序列。
接著將上述序列之胜肽通透(peptide threading)法的電
腦計算方法與18種不同的人類’MHC class II allotypes進行分析以鑑定出具有與MHC class II分子有結合潛力的T細胞抗原決定區(25)。初級的去免疫的VH及Vk胺基酸及cDNA序列被設計出來(B4DIVHv.1,第5圖及第7圖;B4DIVk.1,第6圖及第8圖)。初級的去免疫序列的產生需要進行小量的胺基酸取代,此取代可能會影響最後的去免疫性分子的抗原結合力,因此另三種不同的VH
S(B4DIVHv.2,B4DIVHv4.,第5圖及第7圖)及二種VkS(B4DIVkv.2,B4DIVkv.3,第6圖及第8圖)同時被設計出來,以防止上述情形。
7.去免疫性抗體序列之構築
按照上述設計藉由定點突變PCR去構築去免疫性的變異區域(QuickChangeTM Site-Derected Mutagenesis Kit,Stratagene,Cat.No.200518)
將選殖的鼠源VH及Vk基因為模板,合成包含擬突變之序列引子以突變法(mutagenesis)將架構區轉成所需的去免疫性序列。所產生的去免疫性的V區域被選殖至pUC19並且將每一去免疫性的VH及Vk作完整的DNA序列分析並確定DNA序列的正確性。
將位於pUC19上包含去免疫性的重鏈及輕鏈之V-區之HindIII至BamHI的DNA片段切出來。並轉移至抗體表現載體pSVgpt及pSVhg上(第3圖及第4圖)。並確認於抗體表現載體上的去免疫性VH及VkDNA序列的正確性。
NS0細胞表現的去免疫性抗體之定性
8.去免疫性抗體之表現
用來表現抗體的寄主細胞NS0,來自歐洲動物細胞培養保存中心(European Collection of Animal Cell Cultures,Porton UK(ECACC No.85110505)),其為一不會產生免疫球蛋白的鼠源骨髓癌細胞。
藉由電轉染法(electroporation)將去免疫性重鏈及輕鏈表現載體之各種的組合(12種組合)共同轉染至NS0。表現gpt基因之細胞株在含有10%胎牛血清,0.8ug/ml mycophenolic acid及250ug/ml xanthine之Dulbecco’s Modified Eagle’s Medium(DMEM)中進行篩選。
藉由ELISA定量由轉染的細胞株產生的人類IgG產量,挑選其中會分泌抗體的細胞株並放大培養。
9.製造去免疫性B4抗體
放大表現去免疫性B4抗體之NS0。挑選抗體產量最高的細胞株放大培養。並以ProSep A(Millipore,cat.No.113112722)親合力層析管柱純化500ml-1000ml培養基中的去免疫性B4抗體。用0.1M甘胺酸(Glycine)pH 3.0洗提,中和後並以PBS透析。用過濾的方式使純化好的抗體無菌並保存在4℃。以ELISA定出抗體濃度。
10. B4去免疫性抗體的rsCD4結合親合力
4種去免疫性B4重鏈結合3種去免疫性輕鏈之12種去免疫性B4抗體與rsCD4結合力結果顯示於第9-11圖。
由去免疫性重鏈版本1結合去免疫性輕鏈版本1,2,或3所
構成之抗體與嵌合抗體比較結果,顯示與rsCD4結合力相當。
由去免疫性重鏈版本2,3或4結合去免疫性輕鏈版本1或2所構成之抗體與嵌合蛋白比較,其結合力減低3倍。由去免疫性重鏈版本2,3或4結合去免疫性輕鏈版本3所構成之抗體與嵌合抗體比較顯示與rsCD4結合力減低5倍。
11.病毒中和性分析
病毒種株。A次型病毒分離株UG/92/029是由世界衛生組織HIV分離及定性組織(WHO Network for HIV Isolation and Characterization)所獲得。次型C病毒分離株ZIM748是由Stanford大學之D.Katzenstein,所贈。次型D病毒分離株UG266與次型ESI病毒分離株TH32036是由U.S.Military HIV Research Program(Silver Spring,MD)所提供。次型E病毒分離株TH 32036是由NIAID(NIH,Bethesda,MD)之J.Bradac所贈。從病人分離之病毒株在PBMC中進行繼代培養。除了一個MN樣品在PBMC中繼代培養之外,T細胞馴化之HIV-1病毒分離株MN是在H9細胞中生長。
將NSO細胞所產生的B4去免疫性抗體進行HIV-1病毒中和活性分析。MT-2微溶體分析法如(36)描述,除了以下條件稍有不同,其中熱不活化血清是以50%高葡萄糖DMEM其含15%胎牛血清,抗生素,2%麩胺醯胺(Glutamine)及碳酸氫鈉溶液(bicarbonate buffer),加入50%去纖維化的人類血清進行系列稀釋。而抗體對於HIV-1病毒於mitogen-stimulated PBMC之中和力分析方法如(50)所描述。
由NS0細胞所產生之去免疫性B4 IgG1之病毒中和力與不加抗體的控制組比較其50% plaque inhibition。50% plaque inhibition的終端抗體濃度是由抗體稀釋濃度之內插法所算出。比較4個重鏈變異株(VHv.1-4),與去免疫化k鏈版本1(VKv.1)結合之4個去免疫性B4抗體,顯示有版本1重鏈抗體對於所有的HIV-1病毒原始分離株有較高的中和力,接近鼠源B4單株抗體對抗HIV-1VL135
,HIV-1UG029
及HIV-1TH036
的活性(表一)。
去免疫性B4抗體對於T細胞株馴化的HIV-1MN
病毒無效。去免疫性抗體包含k鏈版本2或3,其病毒中和力相較較低。12個不同免疫性B4抗體,其中有著DIVHv.1重鏈以及DIVKv.1之抗體對HIV-1病毒原始分離株有較高的病毒中和力,活性接近鼠源B4單株抗體。
穩定且高表達去免疫性B4抗體之CHO細胞株之建立
B4DIVHv.1及B4DIVKv.1分別被選為於CHO細胞中表達之抗體重鏈及輕鏈。
12.細胞株
以CHO dhfr-之中國倉鼠卵巢細胞為生產級的抗體表達系統,CHO dhfr-之中國倉鼠卵巢細胞由取自台灣食品工業研究所之細胞及菌種保存中心(Culture Collection and Research Center(CCRC,60113)of Food Industry Research and Development Institute,Taiwan)。
細胞培養在含10%透析過的胎牛血清(DFCS,Gibco cat
no.26400-44)、hypoxanthine及thymidine(Gibco,cat no.11067-03m)之Isocoves Modified Dulbecco’s Medium(IMDM,Sigma cat.No.I-3390)。利用二羥化葉酸還原酶(dihydrofolate reductase)做為放大及篩選的標識。藉由dhfr抑制劑metrotrexate(MTX)的刺激,放大dhfr基因表現。
當dhfr基因被大量放大時,其他鄰近基因時常會一起被放大。所以在每一回放大之後,經由篩選可篩得高產率的細胞株。
13.載體建構
表現dhfr基因的載體;DHFR基因的cDNA乃利用GENEAMP RNA PCR KIT(PERKIN ELMER PART NO.N808-0017)合成自NS0的細胞株。將合成的DHFR基因cDNA以EcoRI限制酶切位放入在動物細胞表現載體pSI(PROMEGA CAT.NO.E172A)中,位於SV-40增進子(enhancer)與早期啟動子及SV-40終止子(terminator)中間。
B4DIVH.1重鏈表達載體:表現重組B4重鏈的DNA片段是來自用DNA合成酶以pSVgpt B4IgG(第3圖)當作模版所合成出來的。將崁合IgG(chimeric IgG)基因的cDNA利用HinDIII和EcoRI限制酶切過之後,放入動物細胞表現載體pcDNA3.1neo(INVITROGEN,CARLSBAD,CA,CAT.NO.V790-20)中位於CMVSV-40增進子與早期啟動子後面,及BGH poly A訊號和轉錄終端子前面,以促進高表現量及增加mRNA的穩定性。然後,以限制酶HinDIII和BamHI切除崁合IgG基因的變異區,並接入B4DIVHv.1的變
異區而成為B4DIVHv.1重鏈表現載體(pcDNA3.1neo DeIgG-S,第13圖)。
B4DIVH.1去-醣化重鏈表達載體:利用定點突變的技術將IgG1Fc中的N-醣化位置移除,已經被證實可以將IgG1和Fcγ
R1結合的能力、激發補體的能力,以及和補體中C1q結合的能力去除。因此,去醣化及去免疫性的B4單株抗體其導致CD4的細胞受損的免疫毒害可能性比醣化抗體少。
為了去除N端醣鏈,B4DIVH.1重鏈中的Asn297基因碼ACC以定點突變的技術(site direct mutagenesis PCR,Invitrogen)被改為His基因碼CAC。
B4DIVH.1輕鏈表達載體:B4DIVH.1輕鏈全長由DIVKv.1的變異區及人類的免疫球蛋白中輕鏈恆定區組成。以PUC19-B4DIVH.1為模版利用DNA合成酶將DNA放大複製DIVKv.1區域是;人類k鏈恆定區則是以B4崁合重組k鏈pSVhyghuck為模版複製而來。B4DIVH.1全部輕鏈DNA片段利用限制酶HindIII和EcoRI切位被接入pcDNA-zeo(Invitrogene cat no.v860-20),接入的位置介於CMV增進子(enhancer)與早期啟動子及BGH poly A訊號和轉錄終端子前面,以促進高表現量及增加mRNA的穩定性。
同時表現dhfr和B4DIVH.1輕鏈的表達載體:同時擁有dhfr基因和B4DIVH.1輕鏈的表現單元是由dhfr在前,k輕鏈在後的方式建構而成。DHFR基因表現單元是以pSI-dhfr當模版,用PCR方式放大表現而來。且從Bg1II限
制酶切位轉殖進載體pcDNA3.1zeo B4DIVH.1(第15圖)。
14.表現B4抗體的卵巢母細胞
將表達重鏈的pcDNA3.1zeoB4DIVHv.1和表達DHFR和輕鏈基因載體pcDNA3.1zeoB4DIVkv.1/dhfr同時利用電轉染法送入缺乏dhfr基因的CHO細胞,利用含有10%胎牛血清及抗生素geneticin(GIBCO,CAT,NO.10131-027)和0.05 uM MTX(SIGMA,St.LOUIS,MO,Cat.no.M8047)及缺乏HT補充的培養基中篩選有DHFR表現的細胞株,將表現DHFR和重鏈基因的細胞株篩選出並放大培養。
15.利用MTX媒介DHFR基因放大的方式建立B4去免疫性的高產量細胞株
利用漸進式增加MTX濃度的方式來提升DHFR和IgG基因的放大,藉以提高去免疫性B4單株抗體的產量。將細胞株培養在含MTX的培養基中,濃度逐量從0.05μ
M增加到5μ
M,且每個處理濃度持續約3至4週,在每個基因被放大的循環之間,細胞株再一次被篩選以增加IgG的產量。
16利用單一細胞的篩選以建立穩定去免疫性B4高產量細胞株
利用限制細胞數稀釋的方式來挑選產量最好的細胞株。將細胞懸浮在選擇性培養基中,並稀釋成每200μ
l中含有約0.1,0.5,和0.25個細胞濃度後接種在96孔盤中,每個孔注入200μ
l,再一次選擇細胞株。將細胞培養約3至4週後進行篩選,每個孔中取出10μ
l培養基,利用ELISA的方法來偵測人類IgG的表現量。經由此篩選過程獲得高產量細
胞株,並放大培養建立了低溫保存的細胞庫,這株細胞在放大後IgG產量約為10-20pg/細胞/天,這種產量已經由原來最早的0.1pg/細胞/天被放大了100到200倍了。
17 以CHO細胞生產N-醣化抗體和N-去醣化抗體
將野生型重鏈基因及在第297號胺基酸由Asn變成His的突變種基因轉染入CHO細胞中並加以放大培養。抗體由500至1000ml培養末期的細胞培養液以prosep A親和性的層析管柱(Millipore cat.no.113112722)加以純化。並以0.1mol濃度的甘胺酸(glycine)及pH3.0的緩衝液析出抗體後以磷酸鹽緩衝液加以中和及透析純化後的抗體,利用過濾達到無菌並儲存在4℃,以ELISA方法偵測抗體的濃度。
18 利用含有SDS的瓊膠來確認N醣化的情形
將野生型重鏈基因及在第297號胺基酸由Asn變成His的抗體純化出來,以N去醣酶處理過後,利用含有SDS的10%的電泳膠並以Commassie blue及醣化蛋白染色後加以分析(Gelcode Glycoprotein Stain,cat.no.24562,Pierce,Rockford Illinois)。
在第297號胺基酸由Asn變成His的抗體比醣化去免疫的抗體在膠中擁有較塊的游動能力,當N去醣化酵素F處理過之後,對於Asn突變抗體在膠中游動能力及醣蛋白染色並沒有影響,但是對野生型抗體卻有加速在膠中游動能力及降低醣蛋白的染色情形。N去醣酶處理過後(第16A圖),野生和Asn突變抗體在膠中游動能力及醣蛋白染色幾乎一樣(第16B圖),此結果確認Asn突變抗體的去醣化情況。
19 由CHO細胞生產的去免疫性B4抗體病毒中和力的測試
MT-2中和病毒能力的測試法和之前陳述的一樣。由CHO細胞生產的去免疫性B4抗體,以含有抗體和未含抗體比較下在降低百分之50的溶菌斑其所需抗體濃度(表3)定義出其中和病毒能力,並由一系列稀釋抗體中計算其降低百分之50的溶菌斑所需抗體濃度。
馴化去免疫性B4抗體CHO細胞株成為無血清的懸浮培養
20 懸浮細胞培養的馴化
在T75規格的培養瓶中長滿會去免疫性B4抗體CHO細胞,利用蛋白水解酶將細胞懸浮化並種入50 mlIMDM(IMDM,Sigma,cat.no.I-3390)含有10%透析過的胎牛血清(DFCS,Gibco cat.no.26400-044)培養液的旋轉培養瓶中(Spinner Flask,Bellco,Vineland,NJ),旋轉培養瓶培養是由磁力控制旋轉,並置於和培養單層貼壁細胞一樣溼度、溫度及二氧化碳濃度的培養箱中培養。
每二至三天,利用加入新鮮培養基將活著的細胞數調整至每ml約1-3×105
個細胞,緊密監視細胞生長速度和細胞存活率數週。
21 馴化細胞至無血清培養基
一但懸浮細胞培養穩定之後,細胞將被離心並打散接種至無血清培養基(SFMII,Gibco BRL,Rockville,MD,cat no.31033-020)。每2至3天利用加入新鮮培養基,將活著的
細胞數調整至每ml約1-3×105
個細胞。緊密監視細胞生長速度和細胞存活率數週。
22去免疫性B4抗體CHO細胞株馴化到無血清的懸浮細胞後的抗體生產
將已馴化成無血清的懸浮培的去免疫性B4抗體CHO細胞。以每毫升2×105
個細胞濃度種在含有Gibco SFMII培養基的100 ml旋轉培養瓶中(Bellco),當細胞濃度達到每毫升6×105
個細胞時將培養細胞轉移到250至500毫升的旋轉培養瓶中,並一系列慢慢將培養體積放大。抗體由500 ml的培養細胞液中利用prosep A親和性的層析管柱(Millipore cat.no.113112722)加以純化,抗體由含有0.1 mol濃度的甘胺酸(glycinc)及pH3.0的緩衝液析出,並用磷酸鹽緩衝液加以中和跟透析。純化後的抗體利用過濾達到無菌並儲存在4℃,利用ELISA的方法偵測抗體的濃度。
CHO細胞株生產的去免疫性B4抗體在補體固定力分析
補體消耗測試被利用於評估鼠源B4抗體和去免疫性抗體之補體固定力。此測試是需要B4單株抗體對新鮮準備且含有人類CD4複合體於在細胞表面的CD4+
細胞進行結合,這個結合可使抗體和一部分已知量的兔子補體結合,接著以敏感化的羊紅血球細胞分析在反應中剩下的補體量。
此分析試驗以來自正常捐獻者靜脈中抽出以抗凝血因子處理過的週邊血中含有CD4+
複合體的人類細胞來進行,全血在50 ml的離心管(購自NUNC cat.no.373660)中以等倍
體積不含血清RPMI-1640(Gibco,cat.no.21870-076)培養基加以稀釋,10.0 ml稀釋過的全血隨後加在9.0毫升的Ficoll HagueTM Plus(購自Amersham Biosciences,cat.no.17-1440-02)上面。
利用在室溫將全血以每分鐘2000轉離心30分鐘後,達到分離淋巴球效果,在Ficoll hagueTM
內面的buffy外層為週邊血單核球,其與稀釋的血清一起被收集起來。利用pH7.6含有鎮定劑(Veronal buffer)的緩衝液洗三次,此緩衝液由9.1mM sodium barbital(merck cat.no.1.06318.0100);15.6 mM barbital(Merck,cat.no.1.00276.0100);0.73 M sodium chloride;2.5 mM magnesium chloride;2.5 mM calcium chloride所組成。週邊血單核球(PBMC)隨即以17.0×107
/ml個細胞懸浮在同樣的鎮定劑緩衝液中。
補體分析步驟,首先於準備好裝有200μ
l共3.6×106
個PBMC細胞的微量離心管中加入30μ
l之各個不同的純化單株抗體。這些混合物將在室溫中培養兩個小時。90ul的兔子補體(Rockland,cat.no.C304-0010),先以含有鎮定劑緩衝液稀釋16倍,再加入反應混合物中。補體的消耗將在最終產物於37℃培養時被啟動。準備致敏化的羊紅血球細胞,取3.0 ml 100%的羊紅血球細胞(購自Rockland,cat.no.R406),以含有100μ
l抗體血清(購自Rockland cat.no.113-4139)及含有鎮定劑的緩衝液在37℃下洗兩次,每次30分鐘。
最後直接加入致敏化羊紅血球(SRBC)懸浮細胞(成份
以10.0 ml含有鎮定劑的緩衝液組成),所有的反應混合物將在37℃下培養1個小時後,反應的管子以微量離心機在5000轉離心一分鐘將完整的羊紅血球細胞(SRBC)離心下來以測定補體在反應混合物中殘存的量。
羊紅血球細胞(SRBC)溶解是以利用150μ
l的上清液分管自每一管反應管中,置入96孔盤中(Becton Dickinson,cat.no.353915),以波長540nm的吸收光波加以偵測讀值。結果以相對於控制組(未處理單株抗體)細胞溶解的百分比來計算(第17圖)。
去免疫性抗體的免疫力分析
利用體外培養人類PBMC系統測試單株抗體(如B4單株抗體,DIVHv1/VK1 7號單株抗體,或是DIVHv1/VK1 16號單株抗體),對人類PBMC細胞的免疫刺激性,在這個測試中所使用到的PBMC分離和以上範例7中所陳述的方法一樣。
體外培養的PBMC細胞以2.5×106
個細胞於每孔含有1.5ml添加抗生素Penicillin及Streptomycin(Gibco,cat.no.10378-016)和胎牛血清(Gibco,cat.no.10099141)的RPMI-164完全培養基的24孔盤(Corning,cat.no.3254)中培養。加入10.0μ
g/ml的個別單株抗體至PBMC細胞培養中。以加入2.0μ
g/ml的Concanavalin A(Sigma,cat.no.C5275)或10.0μ
g/ml的Pokeweed mitogen(Sigma,cat.no.L-8777),作為正面控制組而以未加入mitogen及單株抗體的當作負
面控制組,每一組不同測試的刺激條件均重複兩組,在第3,5,7,9天時收集培養上清液並存放在-70℃直到被用來檢驗。
利用購自eBioscience(USA)的細胞激素試驗組來測試在抗原或mitogen刺激後的人類PBMC細胞培養的上清液中,是否含有Interleukin-2(IL-2),Interleukin-4(IL-4),Interleukin-10(IL-10),干擾素gamma(IFN-r),跟腫瘤細胞溶解因子(TNF-a)。這些細胞激素試驗組分別是IL-2(cat.no.88-7026-22);IL-4(cat.no.88-7046-22);IL-10(cat.no.88-7106-22);IFN-γ
(cat.no.88-7316-22);跟TNF-α
(cat.no.88-7346-22)。
此試驗是按照廠商提供的步驟來執行,結果顯示在表4。
去免疫性抗體的臨床評估
去免疫性單株抗體如DH1DK1#16或DH1DK1#7將在人類單一劑量及劑量試驗中研究其安全性、免疫刺激性、藥物動力及靜脈注射的效力。此試驗使用兩組已有兩次進行HAART治療失敗經驗的愛滋病人,每一組有兩人。此次試驗沒有只給安慰劑的組別,第一和第二組是分別施與相當1 mg/kg跟5 mg/kg DH1DK1NS0#16或DH1DK1NS0#7的抗體,在第0天施以大劑量的靜脈注射。
血液及血清的樣品在第零天注射前及大約注射後10分鐘及第1和12小時後收集起來,隨後血液跟血清在24跟48小時及3、7、14、21、28、35、42、49、56天被收集起來。
安全性則是檢測生理功能,血液化學、血球細胞計數跟副作用的紀錄,每週間隔的血樣品以免疫螢光化學分析淋巴球表面標的物的方法加以分析其免疫抑制力。
已有驗證過的方法檢驗以下的標的物:CD3/CD4,CD3/CD8,CD14/CD45,跟FITC嵌合的抗人類IgG1(anti-human IgG1-FITC)。DIH1DK1#7或DIH1DK1#16於血漿的量在每一次取血後,利用酵素螢光免疫方式測試得知。使用rsCD4為免疫接受器的固相來作為藥物動力的偵測。免疫刺激性是以利用ELISA方法分析血清中對抗DIH1DK1#7或DIH1DK1#16抗體的抗體含量的程度。
對於有效性試驗,利用定量的RT-PCR偵測每一週愛滋病毒RNA含量。
在轉殖基因植物的生產B4去免疫性抗體
以轉殖基因植物生產去免疫性抗體是一種較經濟的方法。煙草(Nicotiana tabacum),一種雙子葉植物,可利用根瘤桿菌中轉殖帶有去免疫性及去醣化的重鏈及輕鏈的7號cDNA質體,是一種有用的生產B4去免疫性抗體的植物生物反應器。
利用專一性酵素HindIII和EcoRI由載體pcDNA3.1(+)Neo-DeIg1(B4DIVHv.1)-S(第13圖)上切下帶有完整表現B4去免疫性抗體DIH1DK1#7的重鏈cDNA,其包含去醣化IgG1 CH
,B4DIVHv.1 VH
,跟前導序列。另一方面利用專一性酵素HindIII和EcoRI由載體pcDNA3.1(+)
Zeo-HuK(B4DIVKv.1)-S(第14圖)上切下帶有完整表現B4去免疫性抗體輕鏈的k cDNA,其包含人類k CH
,B4 DIVKv.1 VK
,跟前導序列。將表現免疫球蛋白鏈的cDNA片段被接入pMON530,此載體是一個包含花椰菜鑲嵌病毒35S啟動子在植物中持續表現的載體。
這個重組載體將被送入大腸桿菌DH5-α
而轉染過的大腸桿菌將利用有放射性的PCR產物來篩選免疫球蛋白鏈cDNA存在與否。轉化成功的轉殖載體將被引入根瘤菌中,利用帶有重組基因的根瘤菌培養液接種在煙草植物的葉面上,從轉殖基因植物的細胞重新建立莖和根植株。
萃取葉片並以酵素免疫螢光方式偵測人類的重鏈和輕鏈表現,將那些表現重鏈和表現輕鏈的植物交叉受粉,這些步驟已經在(52)陳述過了。
萃取葉片並以酵素免疫螢光方式偵測有完整表現去免疫性抗體DIH1DK1NS0#7的轉殖基因植物,選出表現7號抗體的殖株並萃取抗體,測量其rsCD4結合力跟病毒中和力。在例子4已經陳述作rsCD4結合力跟病毒中和力方法。
另一個選擇,玉蜀黍(Zea mays),一種單子葉植物,更勝於雙子葉植物,常被用於改良轉殖基因植物產量和穩定度。此植物特別有利於seed-storage promoter於特定之在玉米內胚層組織表現。
玉蜀黍蛋白基因的啟動子對於這種特定之在玉米內胚層組織表現效果很好。玉蜀黍蛋白是一種儲存蛋白,它是合成發生在發展玉蜀黍內胚層時,玉蜀黍蛋白基因的啟動
子被提高調控直到穀粒成熟。依據此情形,分別可從pcDNA3.1(+)Neo-DeIg1(B4DIVHv.1)-S(第13圖)和pcDNA3.1(+)Zeo-HuK(B4DIVKv.1)-S(第14圖)切下表現去免疫性抗體DIH1DK1#7的重鏈和k輕鏈的cDNA。
這個cDNA片段將被接入帶有玉蜀黍蛋白基因啟動子跟選擇標識蛋白如發光酶的植物轉殖載體中。這個重組載體將送入大腸桿菌中,並篩選免疫球蛋白cDNA的存在。利用粒子轟擊的方式送入載體致年輕的玉蜀黍種子中。粒子準備、DNA包覆,以PDS1000/He放射元素轟擊(Bio-Rad Laboratories,Hercules CA),並利用螢光計偵測轉染過的玉蜀黍的蛋白萃取物的方法如(55)所述。
個別轉殖的基因玉蜀黍來自正反應之植胚並培育至成熟。隨後做交叉受粉產生可生產完整去免疫性B4抗體的植株,去免疫性抗體的選擇跟特徵跟之前所陳述的相同。

第4圖係為含插入輕鏈變異區的輕鏈表達載體pSVhygHuK,用於NS0細胞表現鼠源單株抗體B4輕鏈變異區與人類輕鏈CH1融合之chimeric IgG1輕鏈。Ig enh和Kappa enh分別是重鏈及輕鏈加強子。
