Description

減少或去除牙齒敏感性之口腔保健組成物

 發明之背景
此專利申請案為2007年4月30日提出之美國專利申請案號11/742,039的部分連續申請案。
先前技術
齒質為琺瑯質和牙骨質的一部分牙齒內部，其具有大量被稱為牙本質小管之細管或小管的放射狀橫紋外觀。齒小管從髓腔通達齒質周圍及其基部通常為2微米直徑並且其周圍稍窄。齒小管由於被琺瑯質和牙骨質所覆蓋，因此通常未暴露於口腔環境。牙骨質通常同樣被牙齦所覆蓋。

 發明之摘要

發明之詳細說明
本發明包括含有至少(a)一黏附材料和(b)二氧化矽顆粒的一種口腔保健組成物。該二氧化矽顆粒具有不大於牙本質小管的直徑或者約8微米以下的平均粒徑。該顆粒的含量為約5%重量比或更高。該組成物含有附加治療和非治療性成分以及可被用於各種方法的實務上，其均屬於本發明的範圍內。本發明範圍內的組成物和方法可被用於例如減少或去除哺乳動物的敏感性牙齒，改善/維持全身性健康，及/或阻塞牙本質小管。
本發明的口腔組成物含有一黏附材料。該黏附材料為技術中任何已知或將被開發之可附著至哺乳動物牙齒表面及/或裝設於牙齒表面上異質性生物膜的材料。該附著可利用任何的方法，例如陰離子相互作用、凡德瓦爾力、疏水－親水性相互作用等。該黏附材料可為例如幾丁聚糖、幾丁質、樹膠或海洋膠質。其他可用的黏附材料包括可附著至牙齒表面的任何均聚物或共聚物(此後統稱為”聚合物”)。此類聚合物包括矽膠聚合物、具有聚乙烯磷酸、聚(1－磷酸丙烯)磺酸、聚(β－苯乙烯磷酸)、α－苯乙烯磷酸、合成陰離子聚合聚羧酸鹽、馬來酐、馬來酸和甲基乙烯醚之單體的聚合物。可使用任何分子量的聚合物包括，例如約1,000至約5,000(平均數目)的分子量。在各種的具 體實施例中，可使用例如約1：4至約4：1之單體比例的甲基乙烯醚和馬來酐共聚物。可使用其他聚合物作為黏附材料包括述於美國專利案4,521,551、4,485,090、4,138,477、4,138,914和3,956,480中者，將其內容併入於此以供參考。
本發明的口腔保健組成物亦含有平均粒徑不大於哺乳動物牙本質小管之平均直徑的顆粒，因此使一或多個顆粒被嵌入齒管內而可有效減少或去除敏感性牙齒的知覺。適當顆粒的平均粒徑為例如約8微米或以下，或者約3至約4微米，或約5至約7微米。組成物內的顆粒可在最初時即具有所欲的粒徑或較大的粒徑，但其在施予機械磨擦力如刷牙之後破裂或分裂成所欲的粒徑。
可藉由技術中任何已知或將被發展的方法製備該二氧化矽顆粒，以及需要時其可為表面改性劑以增加顆粒黏附牙齒表面的能力。其實例述於例如美國專利申請案序號11/271,306，將其內容併入於此以供參考。組成物內該二氧化矽顆粒的含量為總組成物重量比的約5%或更高。或者，該二氧化矽顆粒的含量為約5%、約10%、約15%、約20%，或約25%重量比。
可使用任何的研磨顆粒以及其可選自碳酸氫鈉、磷酸鈣(例如二水磷酸氫鈣)、硫酸鈣、沈澱碳酸鈣、二氧化矽(例如水合氧化矽)、氧化鐵、氧化鋁、珠光體(perlite)、塑膠顆粒如聚乙烯，以及其組合。明確而言，該研磨劑可選自磷酸鈣(例如二水磷酸氫鈣)、硫酸鈣、沈澱碳酸鈣、二氧 化矽(例如水合氧化矽)、焦磷酸鈣，以及其組合。可使用任何類型的二氧化矽，例如沈澱矽或矽膠。較佳的市售二氧化矽為例如供應自英國沃靈頓Ineos Silicas市的INEOS AC43。
此處所述的口腔保健組成物可被配製成容許黏附材料和該顆粒接觸牙齒表面的傳遞型式。例如，該組成物可被配製成漱口液、牙膏、凝膠、糖錠(可溶解或可咀嚼)、噴霧劑、口香糖和薄膜(完全或部分溶解，或不溶解)。該組成物含有任何習知的賦形劑或載劑，但其視劑型或投藥方法而不同。賦形劑或載劑包括例如濕潤劑、著色劑、調味劑、甘油、山梨糖醇、木糖醇，及/或丙二醇、水或其他溶劑、膠質、增稠劑、表面活性劑、紅藻膠(富含苔蘚)、三仙膠及羧甲基纖維素鈉、澱粉、聚乙烯吡咯啶酮、羥乙基丙基纖維素、羥丁基甲基纖維素、羥丙基甲基纖維素，及羥乙基纖維素和非晶形矽。
需要時可含有表面活性劑。適當表面活性劑的實例包括較高脂肪酸單硫酸單酸甘油酯例如氫化椰子油脂肪酸之單硫酸單酸甘油酯的鈉鹽；較高烷基硫酸鹽例如月桂基硫酸鈉；烷基芳基硫酸鹽例如十二烷基苯磺酸鈉；較高烷基磺基乙酸鹽例如月桂基磺基乙酸鈉；1,2－磺酸二羥基丙烷的較高脂肪酸酯；以及較低脂族胺基甲酸化合物的的實質飽和較高脂族丙烯醯胺例如脂肪酸、烷基或醯基內具有12~16個碳原子者；及其類似物。該醯胺的實例包括N－月桂基肌胺酸，以及N－月桂醯基、N－肉豆蔻醯基或N－棕 櫚醯基肌胺酸的銨、鉀和乙醇胺鹽。其他包括例如非陰離子聚氧乙烯表面活性劑如Polyoxamer 407、Steareth 30、Polysorbate 20和蓖麻油；以及兩性離子表面活性劑如椰油醯基胺丙基甜菜鹼(tegobaine)和椰油醯基胺丙基甜菜鹼月桂基葡萄糖苷；氧化乙烯與其所反應及具有長烴鏈(例如從約12至約20個碳原子的脂族鏈)之各種含氫化合物的縮合產物，其縮合產物(ethoxamers)含有親水性聚氧乙烯基團例如聚(氧化乙烯)與脂肪酸、脂肪醇、脂肪醯胺和其他脂肪基團以及與氧化丙烯和聚氧化丙烯的縮合產物。
在一具體實施例中，該口腔組成物含有月桂基硫酸鈉(SLS)和甲基椰油醯基牛磺酸鈉(tauranol)的表面活性劑系統。需要時，該SLS和tauranol的比例為約1：5至約1：3。
以本發明產品組合物治療的牙質對侵蝕齒質以齒質傳導法 測量時可產生不超過約45%、約25%、約20%、約15%或約10%之流率值的液體流動速率。
齒質傳導法 ：拔出人類臼齒然後利用金剛石鋸從齒冠切開。除去齒髓然後固定獲得的齒質片段，例如固定於壓克力板上。從壓克力板內的孔連接管子至髓室下方。齒質片段被連接至測量液體流速(導水率)的儀器。
請看Zhang等人，”無痛去敏劑於體外對齒質通透性和管道阻塞在不同時間的效應”，Journal of Clinical Periodontol ，25(11Pt 1)：884~91(1998年11月)，將其內容併入於此以供參考。
以檸檬酸侵蝕齒質的上表面。在120釐米水壓下測量通過腐蝕齒質的液體流速。以3份去離子水稀釋的本發明口腔組成物研磨液將齒質表面刷洗2分鐘然後再一次測量液體流速。請看Pashley等人，”去敏潔齒劑於體外的效應”，J .Periodontol ，55(9)：522~525(1984年9月)。
口腔保健組成物需要時可含有任何其他的治療劑、化粧品及/或美學材料。實例包括去敏劑、硝酸鹽、精胺酸酯、碳酸氫鹽、硝酸鉀、精胺酸－碳酸氫鹽－植酸鹽複合物、化學潔白劑(例如過氧化物釋放化合物)、不透明潔白劑(例如羥磷灰石)和防牙石劑。用於本發明口腔保健組成物內的其他選擇包括三氯沙(triclosan)；亞錫離子劑；氯己定(chlorhexidine)；阿立西定(alexidine)；海克西定(hexetidine)；血根鹼(sanguinarine)；氯化芐烷銨；水楊醯苯胺；溴化杜每芬(domiphen bromide)；氯化鯨蠟基吡啶鹽(CPC)；十四烷基氯化吡啶(TPC)；N－十四烷基－4－乙基氯化吡啶(TDEPC)；奧替尼啶(octenidine)；地莫匹醇(delmopinol)；辛哌醇(octapinol)；尼生素(nisin)；鋅離子劑；銅離子劑；精油；呋喃酮；殺菌素(bacteriocins)；月桂醯精胺酸乙酯；木蘭花(magnolia)萃取物；金屬離子源；精胺酸碳酸氫鹽；厚朴酚(honokiol)；magonol；熊果酸(ursolic)；ursic酸；桑色素(morin)；沙棘(sea buckthorn)萃取物；酵素；山茶花萃取物；類黃酮；黃素母烷(flavan)；鹵化二苯基醚；肌酸；以及蜂膠。
可藉由技術中習知的任何方法製備本發明的口腔保 健組成物。例如，潔齒劑的習知製備方法已述於例如美國專利案3,966,863；3,980,767；4,328,205和4,358,437，將其內容併入於此以供參考。通常，任何濕潤劑(例如甘油、山梨糖醇、丙二醇及/或聚乙二醇)係在攪拌的習知混合機內被分散於水中。分散物內加入增稠劑如羧甲基纖維素(CMC)、卡拉膠或三仙膠；任何陰離子聚羧酸鹽；任何鹽類如氟化鈉抗齲齒劑；以及任何的甜味劑。
攪拌獲得的混合物直至形成均質的凝膠相為止。在凝膠相中加入任何的色素如二氧化鈦，以及調節組成物酸鹼度的任何附加酸或鹼。混合這些成分直至獲得均質相為止。
然後將該混合物置入高速/真空混合機，其中將表面活性劑加入該混合物內。接著加入二氧化矽。將任何水不溶性物質如三氯沙溶解於風味油而被加入潔齒劑內，該溶液與表面活性劑共同被加入該混合物內，然後在約20至50毫米汞柱的真空下高速混合約5至30分鐘。獲得的產品為均質、半固體、可擠壓膏狀或凝膠產物。
在整體原位凝膠/沈澱的製法中分析初步製成氧化矽膠的構造。這些分析中包括孔隙度。利用氮吸附－脫附等溫分析可獲得此類可通過孔隙的性質。利用取自喬治亞州Norcross市Micromeritics儀器公司的微孔分析儀(ASAP 2010)根據脫附分佈測定BJH(Barrett－Joiner－Halender)模型平均孔徑。樣本在150~200℃下脫氣直至約5 mu.米水銀真空壓時為止。此類分析儀在77K時為一種自動容量測定 型。獲得在壓力P/P.sub.0＝0.99時的孔體積。可從孔體積和假設為柱狀孔的表面積獲得其平均孔徑。利用BJH法計算孔大小分佈(.DELTA.V/.DELTA.D)，其提供在一孔徑範圍內的孔體積。利用無兩端開孔之具有1.7至300.0奈米直徑的Halsey厚度曲線型。本發明顆粒亦具有當乾燥顆粒的水分散性小於約0.45西西/克孔體積藉由BJH氮孔隙儀測量時具有600埃或更小孔徑之孔的孔隙度。
本發明範選亦包括數種相關的方法。例如，本發明方法包括減小牙敏感性及阻塞哺乳動物齒質管的方法。
這些方法的步驟分別包括將上述任何的組成物塗抹至牙齒表面。祇要黏附材料和顆粒能接觸齒表面可藉由任何方法進行塗抹。塗抹可藉由刷牙、牙線、灌注、擦拭、沖洗(漱口)、泡沫/凝膠和盤內處理、咀嚼、噴霧、塗抹等，或藉由薄膜或條帶施予。
或者，本發明包括藉由將複合材料施予至口腔表面(至口腔的硬和軟組織)以增加或維持哺乳動物全身健康的方法。可利用上述任何方法施予該組成物，但其必需含有至少一種三氯沙；三氯沙單磷酸鹽；氯己定；阿立西定；海克西定；血根鹼；氯化芐烷銨；水楊醯苯胺；溴化杜每芬；氯化鯨蠟基吡啶鹽(CPC)；十四烷基氯化吡啶(TPC)；N－十四烷基－4－乙基氯化吡啶(TDEPC)；奧替尼啶；地莫匹醇；辛哌醇；尼生素；鋅離子劑；銅離子劑；精油；呋喃酮；殺菌素；月桂醯精胺酸乙酯；木蘭花萃取物；金屬離子源；精胺酸碳酸氫鹽；厚朴酚；magonol；熊果酸；ursic 酸；桑色素；沙棘萃取物；酵素；山茶花萃取物；類黃酮；黃素母烷；鹵化二苯基醚；肌酸；以及蜂膠。塗抹時至少每天一次，但以每天高至五次較佳，以及於一段時間內投予例如一週、長至一年、長至三年或終身。
利用表1的材料和數量以及下述的方法製備兩種牙膏狀的組成物。組成物B~D為本發明的組成物以及組成物A為不含指定氧化矽顆粒的對照組成物。Ineos AC43氧化矽具有0.29西西/克的孔體積。
糖精鈉和氟化鈉為溶解於水中。三氯沙為溶解於香料內。
甘油和丙二醇被共同混合。CMC鈉和Iota卡拉膠為分散於其間。二氧化矽被加入該混合物內。接著加入山梨糖醇。然後加入水中的糖精鈉和氟化鈉以及在49℃之下混合15分鐘。然後在49℃之下加入PMV/MA共聚物和氫氧化鈉(50%)(混合5分鐘)。將全部混合物置入攪拌機內進行混合。接著在完全真空的高轉速之下加入研磨型二氧化矽和Ineos AC43二氧化矽顆粒。
加入預拌香料和三氯沙以及硫酸鈉粉末。在完全真空的中轉速之下混合10分鐘。解除真空狀態然後檢查整批產品的均勻度。
利用上述方法測量各組成物(A~D)通過齒質樣本的液體流速。
以組成物C~D(聚合物及小顆粒二氧化矽)處理牙本質可產生明顯較單獨含聚合物之組成物A為低的侵蝕齒質液體流動值之5~22%的液體流動速率。無小顆粒二氧化矽/聚合物的典型市售牙膏之值為侵蝕齒質液體流動值的50~100%(請參考Pashley DH等人，去敏牙膏的效應，J.Periodontol，1984，55：522~525)。因此，組成物C~D可明顯降低液體流動速率。
同樣，利用共焦顯微鏡影像攝取經組成物C處理的侵蝕齒質，當與經組成物A處理的侵蝕齒質比較顯示可明顯阻塞/塗佈開啟的齒質管。此外，藉由組成物C所產生的阻塞塗層可對抗可樂造成的酸溶解。

Claims (23)

  1. 一種用於減少或去除哺乳動物牙齒敏感性之口腔保健組成物，其包含：(a)一黏附材料；(b)具有小於7微米之平均粒徑的二氧化矽顆粒，其中該二氧化矽顆粒在組成物內的含量為約5%重量比至25%重量比，以及該組成物提供之液體流率不大於約45%的侵蝕齒質(etched dentin)液體流率。
  2. 如申請專利範圍第1項之組成物，其中該顆粒在約600埃或更小孔內具有小於約0.45西西/克的孔隙度。
  3. 如申請專利範圍第1項之組成物，其中該黏附材料為具有平均分子量約1,000至約5,000的聚合物。
