Description
本發明係關於γδ T細胞之擴增和單離及γδ T細胞表現嵌合抗原受體用於過繼性T細胞療法(adoptive T cell therapy)之用途。
以目標特異性(target-specific)的方式特異性嚙合及殺死腫瘤細胞的T細胞之基因工程在為癌症患者建立新的治療選項方面已有結果,稱為經工程改造之T細胞療法(engineered T cell therapy)。此靶向一般係藉由用編碼嵌合抗原受體(chimeric antigen receptor,CAR)的重組DNA分子對患者衍生之T細胞進行基因操作而形成。CAR係合成受體,其包含連接至連接肽之胞外靶向結構域、跨膜(transmembrane,TM)結構域、及一或多個胞內傳訊結構域。傳統上,胞外結構域由對給定的腫瘤相關抗原(tumor-associated antigen,TAA)或細胞表面目標具有特異性的抗體之單鏈Fv片段(single chain Fv fragment,scFv)組成。該胞外scFv結構域賦予CAR之腫瘤特異性,而該傳訊結構域在TAA/目標嚙合時活化T細胞。將這些經工程改造之T細胞(CAR-T cell)重新輸注至癌症患者,其中彼等特異性嚙合並殺死表現該CAR之TAA目標的細胞(Maus et al.,Blood.2014 Apr 24;123(17):2625-35;Curran and Brentjens,J Clin Oncol.2015 May 20;33(15):1703-6)。
自體的、患者特異性CAR-T療法已成為一種對癌症,特別是對於CD19-陽性血液惡性疾病之強大且潛在有療效的療法。然而,CAR-T技術之發展及其廣泛應用由於許多關鍵缺點而部分受限,其包括a)實體腫瘤中無效的抗腫瘤反應、b)過繼性轉移CAR T細胞對免疫抑制型腫瘤微環境(immunosuppressive tumor microenviromment,TME)之有限的滲透及易感性、c)CAR-T細胞在體內之持久性不佳、d)患者中嚴重的不良事件,其包括細胞激素釋放症候群(cytokine release syndrome,CRS)及由CAR-T所介導之移植物抗宿主疾病(graft-versus-host disease,GVHD)、及e)製造所需之時間。

在一個大致態樣中,所提供者係一種自人類周邊血液單核細胞(human peripheral blood mononuclear cell,PBMC)擴增並單離γδ T細胞之方法。該等方法包含(a)獲得人類PBMC;(b)使人類PBMC在包含唑來膦酸、介白素-2(IL-2)、及介白素-15(IL-15)之培養基中培養以擴增該等γδ T細胞;及(c)單離該等γδ T細胞。在某些實施例中,該唑來膦酸之濃度係約1μM至約20μM。在某些實施例中,唑來膦酸之濃度係約5μM。
在某些實施例中,該IL-2之濃度係約50IU/mL至約5000IU/mL。該IL-2之濃度可係例如約100IU/mL至約1000IU/mL。在某些實施例中,該IL-2係重組人類IL-2(recombinant human IL-2,rhIL-2)。

適合用於判定序列同一性及序列相似性百分比之演算法的實例係BLAST及BLAST 2.0演算法,其係描述於Altschul et al.(1990)J.Mol.Biol.215:403-410及Altschul et al.(1997)Nucleic Acids Res.25:3389-3402。執行BLAST分析之軟體由美國國家生物技術資訊中心(National Center for Biotechnology Information)供大眾使用。此演算法涉及首先藉由在查詢序列中識別長度為W之短字組而識別高分序列對(HSP),其在與資料庫序列中具有相同長度之字組排比時匹配或滿足某個正值閾值評分T。T係指鄰近字組評分閾(neighborhood word score threshold)(Altschul et al.,如前述)。此等初始的鄰近字組命中作用為種子而啟動搜尋以發現含有其等之較長HSP。接著將字組命中沿著各序列向兩方向延伸,只要可增加累積排比評分便繼續進行。
針對核苷酸序列,累積評分係使用參數M(匹配殘基對之獎勵評分(reward score);總是>0)及N(錯配殘基之罰分;總是<0)來計算。針對胺基酸序列,使用評分矩陣以計算累積評分。字組命中在各方向之延伸在下列情況下停止:累積排比評分自其最大達成值下滑X之數量時;累積評分因為累積一或多個負分殘基排比而變成零或以下時;或達到任一序列之末端時。BLAST演算法參數W、T、及X判定排比之敏感度及速度。BLASTN程式(針對核苷酸序列)使用以下作為預設值:字組長度(W)為11、期望值(E)為10、M=5、N=-4、及兩股之比較。針對胺基酸序列,BLASTP程式使用以下作為預設值:字組長度(W)為3、期望值(E)為10、及BLOSUM62評分矩陣(參見Henikoff & Henikoff,Proc.Natl.Acad.Sci.USA 89:10915(1989))。
除了計算序列同一性百分比之外,BLAST演算法亦執行兩序列間之相似性的統計分析(參見例如,Karlin & Altschul,Proc.Nat’l.Acad.Sci.USA 90:5873-5787(1993))。其中一種由BLAST演算法所提供之相似性量度係最小總和機率(smallest sum probability,P(N)),其提供兩核苷酸或胺基酸序列之間隨機發生匹配之機率的指標。舉例而言,如在測試核酸與參考核酸之比較中,最小總和機率小於約0.1、更佳地小於約0.01、且最佳地小於約0.001,則將該核酸視為與參考序列相似。

擴增及純化γδ T細胞之方法
本文中所提供者係基於γδ T細胞之同種異體現成的CAR-T細胞產品。使用γδ T細胞能夠開發高品質的產品,其結合δγ T細胞固有的通用性(versatile nature)與彼等高效地溶細胞功能以減少腫瘤逃逸(tumor escape)之風險。此方法可有助於減少由CRS/GVHD所介導之副作用並預防長期的自體免疫,同時提供優異之療效,特別是在實體腫瘤中。γδ在血液中有豐富的T細胞,其具有良好之分選標記且可藉由良好定義之配體輕易地活化並大量擴增。由於γδ T細胞之辨識與MHC無關,γδ T細胞不會參與GVHD,且沒有同種異體辨識之風險,所以γδ T細胞可作為CAR-T之同種異體來源以用於更廣大的患者群體。現成的產品可具有數種優勢,其包括一致的品質、細胞可經分選以用於100%轉導、對劑量大小沒有限制、降低成本、並為患者的啟始治療節省大量時間。

在本發明之治療方法中,γδ T細胞對彼等所投予之對象可係自體的或非自體的。自體細胞係自要投予重組表現CAR的經工程改造之免疫細胞的對象中單離。替代地,可使用來自不是對象之非自體捐贈者的同種異體細胞。在非自體捐贈之情況下,將該等細胞針對人類白血球抗原(human leukocyte antigen,HLA)進行分型及匹配以判定適當的相容性程度。對於自體及非自體細胞,可選地可將該等細胞凍存直至準備使用。

在某些實施例中,CAR之胞外結構域係以位於胺基端的信號肽為先導。任何適合的信號肽皆可用於本發明中。信號肽可例如係衍生自天然、合成、半合成、或重組來源。根據一個實施例,信號肽係人類CD8α信號肽、人類CD3δ信號肽、人類CD3ζ信號肽、人類GMCSFR信號肽、人類4-1BB信號肽、或其衍生物。根據具體實施例,信號肽係人類CD8α信號肽。人類CD8α信號肽包含與SEQ ID NO:4至少90%同一性的胺基酸序列,較佳的是SEQ ID NO:4之胺基酸序列。信號肽可在CAR之易位期間或在CAR之完成易位之後,藉由信號肽酶切割以生成不含信號肽的成熟CAR。

材料和方法
試劑及抗體
細胞培養
將Kasumi-3細胞在RPMI+20% FBS+1x青黴素-鏈黴素中培養。每3天繼代細胞。簡言之,自燒瓶收集Kasumi-3細胞且以1500rpm離心5分鐘。丟棄上清液,且將細胞以5×105個細胞/mL之密度播種回新鮮培養基中。
自整個PBMC中選擇性擴增Vγ9陽性γδ T細胞
在第0天,將一小瓶的冷凍PBMC解凍,並將其添加至在50mL錐形離心管中之49mL溫熱的完整RPMI(RPMI+10%FBS+1x Pen/Strep))以稀釋冷凍的培養基。將PBMC以1500 RPMI離心5分鐘。藉由使PBMC再懸浮於35ml的完整RPMI培養基(RPMI+10%FBS+1xPen/Strep)中將PBMC洗滌一次。將細胞沉澱物再懸浮於完整RPMI培養基中然後計數細胞。

同時,製備γδ T細胞培養基(RPMI-10%;RPMI補充有10%FBS,1x Pen/Strep)補充有重組人類IL-2(rhIL-2)至最終濃度為1000IU/mL;重組人類rhIL-15至最終濃度為10ng/mL;及唑來膦酸(Zol)至最終濃度為5μM。將細胞密度用所製備之γδ T細胞培養基調整至1×106個細胞/mL。
在T-25燒瓶中將4×106個細胞播種在4mL培養基中。在第2天,將4mL的新鮮γδ T細胞培養基添加至該T-25燒瓶中,該培養基補充有濃度為800IU/mL的rhIL-2及濃度為20ng/mL的rhIL-15(rhIL-2及rhIL-15之最終濃度分別係400IU/mL及10ng/mL)。

γδ T細胞之單離及增濃係使用EasySepTM人類δγ T細胞單離套組(Stem cell Technologies;Vancouver,Canada),根據製造商說明書來進行。
自整個PBMC單離γδ T細胞。
解凍冷凍的PBMC並將其添加至在50mL錐形離心管中之49mL溫熱的完整RPMI培養基中以稀釋冷凍的培養基。將PBMC以1500rpm離心5分鐘,且將PBMC用完整RPMI培養基(RPMI+10%FBS+1x Pen/Strep)洗滌一次。將細胞沉澱物再懸浮於1mL的Easy Sep緩衝液中,且藉由使用血球計來計數細胞。
自培養的PBMC單離及增濃γδ T細胞
採集細胞然後以1500rpm離心5分鐘。藉由使細胞再懸浮於純RPMI(無FBS、或Pen/Strep)培養基中將細胞洗滌一次,且以1500rpm離心5分鐘。將細胞沉澱物再懸浮於1mL的Easy Sep緩衝液中,且藉由使用血球計來計數細胞。在5mL聚苯乙烯圓底試管中,將細胞數在1mL的Easy Sep緩衝液中調整至50×106個細胞。
將50μL的生物素化調配混合物(cocktail)添加至再懸浮的細胞中。將細胞混合且在室溫下培養10分鐘。在培養期結束之前,將磁性粒子渦流震盪30秒以使彼等均勻分散。在培養期之後,將50μL的磁性粒子添加至1mL的再懸浮的細胞中。混合細胞懸浮液且在室溫下培養5分鐘。在培養期之後,將1.5mL的Easy Sep緩衝液添加至含有細胞的試管中。藉由上下溫和混合細胞使細胞再懸浮,且將試管置入磁鐵中並在室溫下培養5分鐘。在培養期結束時,藉由以一個連續動作倒轉磁鐵(含試管)收集含有增濃細胞懸浮液的培養基。
向增濃細胞懸浮液添加37μL的經渦流震盪之磁性粒子,且將該細胞懸浮液混合並在室溫下培養5分鐘。將含有細胞懸浮液的試管置入磁鐵中並在室溫下培養5分鐘。藉由使細胞懸浮液轉移入新的15mL試管中收集增濃細胞。然後將10mL的完整RPMI培養基添加至試管中,且將細胞以1500rpm離心5分鐘。將細胞藉由添加10mL的完整RPMI培養基再洗滌一次,且以1500rpm離心5分鐘。將細胞沉澱物再懸浮於1mL的完整RPMI培養基中,且藉由使用血球計來計數細胞。增濃細胞之純度係在流動式細胞儀上藉由用TCR γδ、TCR αβ、及Vγ9抗體染色細胞確認。
γδ T細胞之表型剖析
表面表型剖析

在Kasumi-3細胞上CD123表現之偵測
將五萬個Kasumi-3細胞在100μL的FACS緩衝液(1xPBS+2%FBS)中染色。將細胞以1500rpm離心5分鐘,且丟棄上清液。將抗人類CD123抗體及各個同型以2μg/mL於FACS緩衝液(1xPBS+2%FBS)中之濃度添加至細胞中。保留樣本的等分試樣未染色。將細胞在黑暗中在4℃下培養30分

電穿孔mRNA進入經活化T之細胞中
整個PBMC
收集在含有Zol+rhIL-2+rhIL-15的完整RPMI培養基中培養8天的PBMC且以1500rpm離心5分鐘。藉由將細胞沉澱物再懸浮於35mL的培養基中之方式,將該等細胞用純RPMI培養基(無FBS或Pen/Strep)來洗滌。將細胞以1500rpm離心5分鐘,將細胞沉澱物再懸浮於5mL的完整RPMI培養基中,且藉由使用血球計來計數細胞。

用CFSE標記目標細胞(Kasumi-3)
收集Kasumi-3細胞且用純RPMI培養基洗滌一次。計數細胞且將細胞之密度調整至1×106個細胞/mL。將細胞再懸浮於1mL的0.5μM CFSE於1xPBS中且在室溫(RT)下培養8分鐘,並伴隨偶爾的混合。加入1mL的FBS以停止標記反應。將細胞在完整RPMI培養基(RPMI+10%FBS+1xPen/Strep)中洗滌兩次。使用血球計來計數細胞且根據ET比(ET ratio)將細胞密度在100μL體積中調整。

效應對目標比

取得及分析

結果
實例1:γδ T細胞亞群在整個新鮮PBMC中之分佈。

Vγ9+ γδ T細胞主要顯示中樞記憶(CD27+CD45RA-)及效應子記憶(CD27- CD45RA-)表型。然而經Zol活化之Vγ9+ γδ T細胞僅顯示效應子記憶(CD27- CD45RA-)表型(圖3A)。與此一致的是,休眠Vγ9+ γδ T細胞(來自新鮮PBMC)顯示活化表型(CD44表面表現),伴隨明顯的胞內顆粒酶B及穿孔素表現(圖3B)。同樣地,經Zol活化之Vγ9+ γδ T細胞顯示在彼等表面上明顯的CD44表現及胞內顆粒酶B及穿孔蛋表現(圖3B)。有趣的是,休眠Vγ9+ γδ T細胞沒有顯示CD69(早期活化標記)之任合表面表現。然而,部分(28%)的經Zol活化之Vγ9+ γδ T細胞確實顯示在彼等表面上存在CD69(圖3B)。
在用各種抑制性受體將Vγ9+ γδ T細胞染色後,休眠Vγ9+ γδ T細胞顯示沒有PD1、Tim-3、及CTLA-4之表面表現。另一方面,經活化T之Vγ9+ γδ T細胞顯示在細胞表面上Tim-3之明顯的上調。然而,在經活化T之Vγ9+細胞上,PD1及CTLA-4表現相較於休眠Vγ9+ γδ T細胞似乎沒有改變。有趣的是,大部分休眠Vγ9+ γδ T細胞在彼等表面上表現2B4且實際上所有Vγ9+ γδ T細胞在用Zol活化後在彼等表面上皆表現2B4(圖3C)。
實例4:負向選擇以耗盡αβ T細胞
γδ T細胞係自第14天之Zol+rhIL-2+rhIL-15培養物中經由負向選擇使用EasySep人類γδ T細胞單離套組濃縮。γδ T細胞濃縮去除了殘留的TCRαβ T細胞污染物(圖4)。用最佳化電穿孔參數(1400V、20ms脈寬、及1脈衝)將CAR mRNA轉染入γδ T細胞。
實例5:CAR-T之設計及構建
CAR-T構建體由抗原特異性單鏈Fv(scFv)部份組成,該部份以可撓的人類CD8鉸鏈序列錨定在膜上。人類CD8之單向跨膜結構域,接續的共刺激部份(4-1BB)及傳訊結構域(CD3ζ)亦係構建體之一部分(圖7)。
實例6:CAR轉導γδ T細胞之細胞毒性
為了分析CAR介導的γδ T細胞細胞毒性,將經CFSE標記之目標(Kasumi-3)細胞與來自培養第14天的CAR轉染γδ T細胞(效應子)以1:1之效應子對目標(ET)比共培養24小時。作為對照組,經CFSE標記之目標(Kasumi-3)細胞亦與GFP轉染γδ T細胞以1:1之效應子對目標(ET)比培養。在電穿孔之後,將細胞休眠40小時期間。
在細胞休眠期間藉由在轉染後2、20、及40小時分別測量彼等排除LIVE/DEAD染色及EGFP表現的能力來取得這些細胞的細胞存活力及轉染效率(圖5A及圖5B)。細胞存活力在電穿孔後2小時接近96%且在電穿孔後40小時下降到60%附近。轉染效率(EGFP+細胞的%)在電穿孔後2小時係在35%且在20小時略微增加至42%。作為轉染對照組,藉由添加CAR/GFP mRNA來類似地處理γδ T細胞且保留未轉染之彼等(圖5A,右)。
在目標(Kasumi-3)細胞上的CD123表現係藉由使用市售可得之抗CD123抗體分析。藉由用1μg的CD123生物素化純化蛋白探測細胞來測量CAR表面表現。使用鏈黴親和素PE/Cy7以標記生物素化蛋白。未觀察到在CAR轉染γδ T細胞上的PE/Cy7信號,其可能由於CD123純化蛋白之生物素化失效或其他技術原因所致。
將目標抗原表現細胞(諸如針對CD123的Kasumi-3、針對GPRC5D的H929細胞、針對CD33的KG1細胞)及目標剔除細胞或非目標表現性細胞(諸如針對CD123的K562)用CFSE標記以在流動式細胞量術分析期間
將彼等識別為CFSE+。共培養期後,添加7-AAD以分析7-AAD+CFSE+細胞之百分比作為細胞毒性的量度。減去用未轉染γδ T細胞觀察到的基礎細胞毒性(basal cytotoxicity)以獲得由在γδ T細胞上CAR表現所介導之特異性細胞毒性。用mRNA構建體轉染的γδ T細胞觀察到最大的細胞毒性為自60%至~70%的範圍(圖6B至圖6E)。

Claims (26)
- 一種自人類周邊血液單核細胞(PBMC)擴增並單離γδ T細胞之方法,該方法包含:a.獲得人類PBMC;b.使該等人類PBMC在包含唑來膦酸(zoledronic acid)、介白素-2(IL-2)、及介白素-15(IL-15)之培養基中培養以擴增該γδ T細胞;及c.單離該等γδ T細胞。
- 如請求項1所述之方法,其中該唑來膦酸之濃度係約1μM至約20μM。
- 如請求項2所述之方法,其中該唑來膦酸之濃度係約5μM。
- 如請求項1至3中任一項所述之方法,其中該IL-2之濃度係約50IU/mL至約5000IU/mL。
- 如請求項4所述之方法,其中該IL-2之濃度係約100IU/mL至約1000IU/mL。
- 如請求項1至5中任一項所述之方法,其中該IL-2係重組人類IL-2(rhIL-2)。
- 如請求項1至6中任一項所述之方法,其中該IL-15之濃度係約1ng/mL至約100ng/mL。
- 如請求項7所述之方法,其中該IL-15之濃度係約10ng/mL。
- 如請求項1至8中任一項所述之方法,其中該IL-15係重組人類IL-15(rhIL-15)。
- 如請求項1至9中任一項所述之方法,其中該γδ T細胞係Vγ9Vδ2 T細胞。
- 如請求項1至10中任一項所述之方法,其中該等γδ T細胞係藉由流動式細胞測量術、磁性分離、及負向選擇單離。
- 一種單離γδ T細胞,其藉由如請求項11所述之方法產生。
- 一種生成嵌合抗原受體(CAR)-γδ T細胞之方法,該方法包含:a.獲得如請求項12所述之單離γδ T細胞;b.使該γδ T細胞與編碼嵌合抗原受體(CAR)的核酸接觸,該CAR包含:i.胞外結構域;ii.跨膜結構域;及iii.胞內傳訊結構域,其中該CAR可選地進一步包含位於胺基端的信號肽及連接該胞外結構域和該跨膜結構域的鉸鏈區,且其中使該γδ T細胞與編碼該CAR的該核酸接觸生成CAR γδ T細胞。
- 如請求項13所述之方法,其中該CAR包含:i.胞外結構域,其包含抗原結合結構域及/或抗原結合片段;ii.跨膜結構域,其包含CD8α跨膜結構域;iii.胞內傳訊結構域,其包含CD3ζ或4-1BB胞內結構域;iv.信號肽,其包含CD8α信號肽;及v.鉸鏈區,其包含CD8α鉸鏈區。
- 如請求項14所述之方法,其中該CAR包含:i.該跨膜結構域,其具有與SEQ ID NO:1至少90%同一性的胺基酸序列;ii.該胞內結構域,其具有與SEQ ID NO:2或SEQ ID NO:3至少90%同一性的胺基酸序列;iii.該信號肽,其具有與SEQ ID NO:4至少90%同一性的胺基酸序列;及iv.該鉸鏈區,其具有與SEQ ID NO:5至少90%同一性的胺基酸序列。
- 如請求項14或15所述之方法,其中該胞外結構域包含特異性結合腫瘤抗原的抗原結合結構域及/或抗原結合片段。
- 如請求項13至16中任一項所述之方法,其中該CAR包含選自由SEQ ID NO:22至29組成之群組的胺基酸序列。
- 一種CAR-γδ T細胞,其藉由如請求項13至17中任一項所述之方法產生。
- 一種醫藥組成物,其包含如請求項18所述之CAR-γδ T細胞及醫藥上可接受之載劑。
- 一種治療或預防有需要之對象的疾病或病況之方法,該方法包含投予治療有效量的如請求項19所述之醫藥組成物。
- 如請求項20所述之方法,其中該疾病或病況係癌症。
- 如請求項21所述之方法,其中該癌症係選自實體癌症或液體癌症。
- 如請求項22所述之方法,其中該癌症係選自由下列組成之群組:肺癌、胃癌、結腸癌、肝細胞癌、腎細胞癌、膀胱泌尿上皮癌、轉移性黑色素瘤、乳癌、卵巢癌、子宮頸癌、頭頸癌、胰腺癌、子宮內膜 癌、前列腺癌、甲狀腺癌、神經膠質瘤、神經膠質母細胞瘤、及其他實體腫瘤、及非霍奇金氏淋巴瘤(non-Hodgkin’s lymphoma,NHL)、霍奇金氏淋巴瘤/疾病(Hodgkin’s lymphoma/disease,HD)、急性淋巴球性白血病(ALL)、慢性淋巴球性白血病(CLL)、慢性骨髓性白血病(CML)、多發性骨髓瘤(MM)、急性骨髓性白血病(AML)、及其他液體腫瘤。
- 如請求項20所述之方法,其中該疾病或病況係自體免疫疾病。
- 如請求項24所述之方法,其中該自體免疫疾病係選自由下列組成之群組:脫髮、類澱粉變性症、僵直性脊椎炎、Castleman氏病(CD)、乳糜瀉、克隆氏病(Crohn’s disease)、子宮內膜異位、纖維肌痛、腎小球性腎炎、格雷氏病(Graves’ disease)、格巴二氏症候群(Guillain-Barre syndrome)、IgA腎病、狼瘡、萊姆病(lyme disease)、梅尼爾氏病(Meniere’s disease)、多發性硬化症、猝睡症、嗜中性球減少症、乾癬、乾癬性關節炎、類風濕性關節炎、類肉瘤病、硬皮症、第1型糖尿病、潰瘍性結腸炎、及白斑症。
- 一種產生包含CAR-γδ T細胞的醫藥組成物之方法,其中該等方法包含使如請求項18所述之CAR-γδ T細胞與醫藥上可接受之載劑組合以獲得該醫藥組成物。
