Description
453 793 Egenskaperna hos levande vävnad gör den svår att mäta på grunå av dess mjuka, elastiska och eftergivliga egenskaper. Studier har utförts för bestämning av lämpligaste läkningstryck, t.ex. för väv- naderna i inre organ. Det lämpligaste läkningstrycket varierar med olika vävnadstyper. Lungvävnad t.ex. skiljer sig helt från inälvor- nas vävnad. Därför måste vävnadsmätinstrumentet enligt uppfinningen vara konstruerat så att det anbringar det lämpligaste läknings- trycket under vävnadsmätningen för den speciella vävnadstyp som skall mätas. Detta är lätt utförbart och samma principer och mät- ningsförfarande med vävnadsmätningsinstrumentet enligt uppfinningen kan användas för varje typ av vävnad.
Såsom exempel skall vävnadsmätningsinstrumentet enligt uppfinningen beskrivas i en tillämpning för mätning av tjockleken hos inälvs- vävnad. Fackmän har tidigare utvecklat olika typer av agrafftän- ger för intralumenal anastomos. Exempel pà sådana instrument be- skrivs i US-patenten 3 552 626, 4 207 898 och 4 319 576. Dessa in- strument förbinder de rörformiga kroppsdelarna i stort sett ände mot ände, ingriper invändigt i de rörformiga delarna och avlägsnar överskottsdelar av de rörformiga delarna intill den eller de ring- formiga agrafferna som inplanteras medelst instrument med använd- ning av en cylindrisk skalpell eller liknande. Dessa instrument är anpassade för användning av cylindriska agraffpatroner av mer än en storlek och är försedda med en anordning för inställning av ga- pet mellan agraffpatronen och ett ringformigt städ, mot vilket aqrafferna formas. De angränsande rörformiga kroppsdelarna, som skall sammanfogas, är belägna inom gapet. Vävnadsmätinstrumentet enligt uppfinningen är avsett att användas tillsammans med sådana kirurgiska agrafftänger, som är försedda med gapinställningsanord- ning (såsom t.ex. det i det senast nämnda US-patentet beskrivna), vars kalibrering är identisk med den för vävnadsmätinstrumentet.
Enligt uppfinningen åstadkoms ett mätinstrument för vävnadstjock- lek för kirurgiskt bruk, med de i patentkravet 1 angivna särdragen.
Vävnadsmätinstrumentet enligt uppfinningen fäster mot vävnaden med en klämrörelse och gör den aktuella vävnadstjockleksmätningen obe- roende av kirurgen. Under mätförfarandet klämmer instrumentet mot .den vävnad som skall mätas med en kraft, som är i stort sett lika med det OVHD Hämnåê, lämpligaste anläggningstrycket. Detta stämmer 453 793 över ett förutbestämt intervall för vävnadstjocklek. Instrumentet har minst en skala och en visaranordning, medelst vilken vävnadens tjocklek enkelt och direkt kan avläsas av kirurgen. Instrumentets klämkäkar är så formade, att de lätt griper inälvsvävnad med be- gränsad åtkomlighet. Instrumentet kan förses med en andra skala för bestämning av diametern hos en inälvslumen för val av en Cy- lindrisk agraffpatron av rätt diameter.
Uppfinningen beskrivs i det följande med hänvisning till bifogade ritningar, i vilka fig. 1 är en sidovy av en utföringsform av mätinstrumentet för vävnadstjocklek enligt uppfinningen, fig. 2 är en vy framifrån av utföringsformen i fig. 1, sedd från vänsteri.figuren, fig. 3 är en vy bakifrån av instrumentet i fig. 1 sedd från hö- ger i figuren, fig. 4 är ett längdsnitt utmed linjen 4-4 i fig. 1, fig. 5 är en sidovy liknande fig. 1 och visar användningen av in- strumentet i fig. 1 och fig. 6 är en fragmentarisk tvärsnittsvy liknande fig. 4 och visar anordningen med en lateralt förskjuten käke.
En utföringsform av vävnadsmätinstrumentet enligt uppfinningen vi- sas i fig. 1-6, i vilka liknande delar har samma beteckningar.
Först hänvisas till fig. 1-4. Själva instrumentet är allmänt be- tecknat 1 och innefattar en första käke 2 med en bakåt riktad för- längning i form av ett första handtag 3 och en andra käke 4 med en bakåt riktad förlängning i form av ett handtag 5. Vid främre änden slutar käkarna 2 och 4 i plana vävnadsgreppytor 2a resp. 4a.
Som framgår av fig. 1 och 2 har käkarna 2 och 4 en bredd, som unge- fär motsvarar en tredjedel av deras respektive handtag 3 och 5.
Den första käken 2 och handtaget 3 har en nedåt riktad mittdel 6 med en genomgående öppning Ga. Den andra käken 4 och dess handtag harett par upprättstâende delar 7 och 8 med genomgående, koaxia- la öppningar 7a resp. Ba. Det nedåtriktade elementet 6 på den förs- ta käken 2 och dess handtsg är anordnade att befinna sig mellan de upprättstàende delarna 7 och 8 på den andra käken 4 och dess hand- tag 5 med öppningarna 6a, 7a och 8a koaxiala. En tapp 9 är anord- 453 793 nad i öppningarna för svängbar anordning mellan den första kåken 2 och dess handtag 3 mot den andra käken 4 och dess handtag 5.
Handtaget 3 på den första käken 2 har på insidan en långsträckt fåra eller intryckning 10. Intryckningen 10 är visad i fig. 1 och 4. Vid bakänden av intryckningen 10 är en utskjutande tapp 11 for- mad. Handtaget 5 på den andra kåken 4 har en liknande lângsträckt fâra eller intryckning 12 på sin insida (se fig. 1). Vid bakänåen av intryckningen 12 är en utskjutande tapp 13 anordnad, motsvarande tappen 11 på handtaget 3. En bladfjäder 14 är anordnad mellan hand- tagen 3 och 5. Änden 14a på fjädern 14 är belägen i intryckningen 10 på handtaget 3 och innefattar en öppning 15. Tappen 11 skjuter ge- nom öppningen 15 sâsom fig. 1 och 4 visar. Fjäderns 14 ände 14b är på liknande sätt anordnad i intryckningen på handtaget 5. Fjäder- änden 14b har en öppning 16, genom vilken tappen 13 skjuter ut.
Det är uppenbart, att fjädern 14 förblir på plats mellan handtagen 3 och 5 pâ grund av att dess ändar 14a och 14b är anordnade i hand- tagens intryckningar 10 och 12 under ingrepp av tapparna 11 resp. 13. Fjädern 14 pressar handtagen 3 och 5 åtskils och pressar såle- des käkarna 2 och 4 med deras vävnadsgreppytor 2a och 4a samman.
Pig. 1-3 visar vävnadsmätinstrumentet i dess normala skick då det icke används.
Vid dess distala ände har handtaget 3 ett par nedåtriktade element 17 och 18. Som fig. 4 klarast visar har elementen 17 och 18 i stort sett triangulärt tvärsnitt. Deras motliggande ytor 17a och 18a lig- ger parallellt på avstånd från varandra. Deras utsidor 17b och 18b sluttar bakåt mot varandra. Utsidan 17b på elementet 17 uppvisar en skala 19. Utsidan 18b på elementet 18 är försedd med en identisk skala 20.
Det andra handtaget 5 har en uppâtriktad, bâgformig del 21. Som bäst framgår av fig. 4 har delen 21 en bakàtriktad triangulär kant med en yta 21a i stort sett i samma plan som ytan 17b på elementet 17 samt en andra yta 21b vilken i stort sett ligger i samma plan som ytan 18b på element 18. Delen 21 har också en tunn kantdel 21c, vilken just kan skjuta in mellan ytorna 17a och 18a på elementen 17 och 18. Ytan 21a på elementet 21 uppvisar en indexlinje 22 för samverkan med skalan 19 på elementet 17 (se fig. 1 och 3). 455 793 Pâ liknande sätt uppvisar ytan 21b en indexlinje 23, identisk med indexlinjen 22, avsedd att samverka med skalan 20 på elementßt 13- Skalorna 19 och 20 ger minst en förstoring av 3:1 för att underlät- ta avläsningen. Den visade utföringsformen ger skalorna 19 och 20 en förstoring av 4:1.
För att fullständiga uppbyggnaden kan handtaget 3 vara räfflat så- som vid 3a så att det bekvämt och stadigt kan hållas utan att slinta i kirurgens hand. Handtaget 5 kan vara räfflat på liknande sätt vid 5a. Om så önskas kan handtaget 3 vara försett med ännu en skala 24 (fig. 1). Skalan 24 löper längdledes utmed handtaget 3.
Den andra sidan av handtaget 3 kan vara försett med en identisk skala (ej visad) så att instrumentet kan avläsas från båda sidorna vid användning. Skalan 24 medger att kirurgen gör en snabb bestäm- ning av diametern på ett rörformigt kroppselement som skall häftas, såsom senare skall beskrivas.
Studier har utförts för bestämning av det lämpligaste läkningstryc- ket för suturerad intestinal vävnad. Trycket är omkring 8 g/mmz.
Vid den som exempel givna tillämpningen, med avseende på vilken vävnadsmätinstrumentet här beskrivs, har det befunnits lämpligt att vid mätning av intestinal vävnad anordna instrumentet för mät- ning av vävnadstjocklek i ett intervall från 0 till 4 mm. Det fram- går av fig. 1 och 3 att skalorna 19 och 20 sträcker sig över detta intervall. Fjädern 14 är utförd som en fjäder för konstant kraft, som pressar käkytorna 2a och 4a vid mätning av vävnad däremellan vid ett tryck av 8 g/mmz under halva arbetsintervallet (dvs vid 2 mm avstånd mellan ytorna) så att den praktiskt taget utövar samma tryck genom hela arbetsintervallet från 0 till 4 mm. Käkarna 2 och 4 är så utformade, att deras ytor 2a och 4a är parallella vid hal- va arbetsintervallet (dvs vid 2 mm åtskillnad mellan ytorna) för att ge största noggrannhet vid mätning av vävnadens tjocklek. In- strumentet 1 kan vara tillverkat av varje material (inklusive plast, metall eller kombinationer därav eller liknande) som är lämpat för en klinisk miljö och steriliserbart i autoklav, etylenoxid, strål- ning eller med någon annan standardmetod.
Pig. 6 är en fragmentarisk vy liknande fig. 4. Utföringsformen i fig_ 6 skiljer sig från den i fig- 4 genom att käken (visad vid 453 793 2b i fig. 6) är lateralt vinklad. Det framgår att vid en sådan utföringsform den andra käken är vinklad på liknande sätt. Instru- mentet enligt uppfinningen försett med sådana vinklade käkar, kan användas i trånga arbetsområden där tillgängligheten begränsar syn- fältet.
Sedan instrumentet har beskrivits skall dess användning skildras i det följande. Kirurgen fattar instrumentet 1 såsom en tång, klämmer om handtagen 3 och 5 så att käkarna 2 och 4 öppnar sig mot verkan av konstantfjädern 14. Instrumentet skall i detta sammanhang beskri- vas vid användning för tjockleksbestämning av en intestinal lumen , visad i fig. 5. Käkarna 2 och 4 klämmer som visat om den intes- tinala lumen 25, varefter kirurgen lossar greppet mot handtagen 3 och 5. Fjädern 14 pressar därvid käkytorna 2a och 4a med ett tryck mot lumen 25 av omkring 8 g/mm2. Eftersom kirurgen icke längre kläm- mer om handtagen 3 och 5, är den direkta tjockleksavläsningen i mm på skalorna 19 och 20 i samverkan med indexlinjerna 22 resp. 23 helt oberoende av kirurgen. Anordningen av skalorna 19 och 20 är sådan, att direkt avläsning kan ske från instrumentets båda sidor eller bakifrån instrumentet.
Det framgår att vid förfarandet i fig. 5 mäts dubbla tjockleken av den intestinala lumenvävnaden. Om två i stort sett identiska lumen skall sammanfogas medelst agrafftången, kan denna avläsning använ- das direkt såsom summan av vävnadstjockleken hos de två lumen som skall sammanfogas. Om de två lumen, som skall sammanfogas, har oli- ka vävnadstjocklek, kan varje lumen mätas såsom fig. 5 visar och resultaten divideras med 2. Detta ger en mätning av vävnadstjock- leken för varje särskild lumen. De resulterande mätningarna av tjockleken adderas sedan för bestämning av rätt agraffhöjd och det lämpliga gapet, som skall inställas mellan instrumentets agraff- patron och städet. Då så är nödvändigt kan en avläsning av en enda vävnadstjocklek göras direkt genom inskjutning av en av käkarna 2 och 4 i lumens hålighet.
Som ovan antytt kan instrumentet vara försett med en skala 24 utmed båda sidorna av handtaget 3. Det framgår att en identisk skala kan anordnas utmed båda sidorna av handtaget 5 om så är-lämpligt.
För mätning av diametern hos lumen 25 läggs lumen i löst skick ut- med ytan på handtaget 3 med en kant vid noll-märket på skalan 24. 453 793 Skalan avläsas sedan vid lumens andra kant. Den verkliga mätnin- gen i detta fall är en mätning av omkring halva Omkretsen på lu- men, som är proportionell mot lumens diameter. Talen på skalan 24 anger olika cirkulära patrondiametrar tillgängliga för användning i agrafftången för intralumenal anastomos, eftersom patronen el- ler huvudet under förfarandet ligger inom den ena av de lumen som skall sammanfogas. Förfarandet gör valet av rätt agraffpatron el- ler huvudstorlek enkelt och eliminerar alla gissningar.
Modifikationer kan utföras i uppfinningen utan avsteg från uppfin- ningstanken.
Claims (4)
1. l.
