Поставленная задача решается тем, что малоинвазивный способ устранения изолированного остаточного приведения переднего отдела стопы при эквино-варо-аддукционной деформации стоп у детей включает выполнение продольно латерального разреза в проекции 5 плюсневой кости, выход на сухожилие короткой малоберцовой мышцы и его удлинение путем отсечения сухожилия короткой малоберцовой мышцы у места прикрепления в области бугристости 5 плюсневой кости и прикрепления на основание 5 плюсневой кости, фиксацию в положении коррекции. Предлагаемый способ осуществляют следующим образом.The proposed method is as follows.
Производят продольно латеральный разрез в проекции 5 плюсневой кости. Послойно рассекают кожу, подкожно жировую клетчатку, удерживатель и сухожильное влагалище, после чего осуществляют выход на сухожилие (1) короткой малоберцовой мышцы. Далее сухожилие (1) с кортикальной пластинкой отсекают у места прикрепления (2) в области бугристости 5 плюсневой кости. На основании 5 плюсневой кости остеотомом формируют ложе, укладывают в ложе свободный конец сухожилия короткой малоберцовой мышцы с кортикальной пластиной и фиксируют кортикальную пластину к основанию (3) 5 плюсневой кости в положении коррекции деформации стопы. У детей до 3 лет фиксацию кортикальной пластины осуществляют трансоссальным хирургическим швом. У детей старше 3 лет фиксацию кортикальной пластины осуществляют биодеградируемыми фиксаторами. Получают дополнительную коррекцию деформации стопы (стоп). Ложе промывают антисептиком, например раствором хлоргексидина. Проверяют гемостаз послеоперационной раны, послойно накладывают швы на рану в обратном направлении. Далее накладывается асептическая повязка и гипсовая иммобилизация от кончиков пальцев до пахово-ягодичной складки. Пациент Н., 3 года, поступил в детское ортопедическое отделение Новосибирского НИИТО в 2011 г. с диагнозом: Врожденная левосторонняя косолапость, типичная форма, тяжелая степень. Проходил лечение по месту жительства без удовлетворительных результатов. Срок наблюдения по месту жительства составил 6 месяцев. Обратился в ННИИТО с остаточным компонентом деформации. При осмотре выявлено изолированное приведение переднего отдела левой стопы (фиг. 2). Выполнен малоинвазивный способ устранения изолированного остаточного приведения переднего отдела стопы. Срок наблюдения после выполнения заявляемого изобретения и гипсовой иммобилизации составил 7 месяцев (фиг. 3).
