Description
Foreliggende oppfinnelse angår sirupfonnuleringer inneholdende et antihistamin og en aminopolykarboksylsyre. Sirupformuleringer anvendes vanligvis for avlevering av farmasøytiske stoffer, spesielt der stoffene skal avleveres til pediatriske pasienter.
Tradisjonelle siruper er konsentrerte sukkerløsninger (vanligvis sukrose) i renset vann, slik som Sirup NF tilberedt med 850 g sukrose og tilstrekkelig vann for å lage 1000 ml ifølge fremgangsmåten gitt i den offisielle monograf på side 1990 av NF XVII The National Formulary, United States Pharmacopeial Convention, Inc., Rockville, Maryland U.S.A., 1990. For hensikter ifølge foreliggende oppfinnelse vil imidlertid betegnelsen "sirup" også omfatte de flytende formuleringer med en søt smak frembrakt i sin helhet eller delvis ved kunstige søtningsstoffer, slik som sakkarin, sorbitol, aspartam, natriumcyclamat og liknende, for å unngå tannproblemer og medisinske problemer som kan forverres ved høykalorisøtningsstoff. Slik det er vel kjent innen teknikken er siruper oftest smakstilsatt, slik som med frukt- eller mintsmakstoffer, vanligvis i den hensikt å maskere en vond smak forårsaket av tilstedeværelsen av en oppløst eller suspendert farmakologisk aktiv substans. En god smak er spesielt viktig når formuleringen har til hensikt å bli inntatt av barn. Typiske smaksstoffer som vanligvis anvendes i søtede farmasøytiske preparater, mat, godteri, drikkevarer og liknende er også nyttige ifølge oppfinnelsen; disse materialene gir smaker slik som grape, kirsebær, sitrus, fersken, jordbær, tyggegummi, peppermynte og mange andre.
Siruper må ofte inneholde antimikrobielle bestanddeler for å sikre trygg lagring uten frigivelse av patogene muggsopper, gjær, bakterier og liknende; et typisk antimikrobielt middel som antas å være egnet for anvendelse innenfor mat og andre spiselige substanser er natriumbenzoat. I tillegg kan de siruper som ikke inneholder sukker eller som inneholder en blanding av sukker og andre søtningsstoffer, inneholde fortykningsmidler, slik som en hydroksypropylmetylcellulose, der noen former er tilgjengelige fra Dow Chemical Midland, Michigan USA under varemerket METHOCEL for å frembringe en viskøs munnfølelse lik den for en tradisjonell sirup.
Et eksempel på en nåværende markedsført sirup inneholder 1 mg/ml av antihistaminmedikamentet loratadin, sammen med sitronsyre, kunstig smak, glyserin, propylenglykol, natriumbenzoat, sukrose og vann; denne formulering har typisk en pH-verdi mellom ca. 2 og 4. Under spesielle lagringsbetingelser som omfatter kontakt med luft har imidlertid tap av loratadininnhold og en samtidig dannelse av forurensninger oppstått. Tilsvarende problemer kan oppstå med formuleringer inneholdende andre, kjemisk beslektede medikamenter, slik som deskarboetoksyloratadin og azatadin.

Det stadfestes at spormetallinitierte oksidasjonsreaksjoner kan minimaliseres gjennom anvendelse av sitronsyre eller EDTA-kelateringsstofFer.
Det er ønskelig å frembringe en lagringsstabil sirupformulering av loratadin og beslektede antihistaminbestanddeler, som bare inneholder komponenter som erkjennes å være trygge for humant inntak.
OPPSUMMERING AV OPPFINNELSEN
Ifølge oppfinnelsen frembringes det en sirupformulering inneholdende et antihistamin valgt blant loratadin, deskarboetoksyloratadin, azatadin og kombinasjoner av hvilke som helst to eller flere av disse, og 0,05 til 5 mg/ml av en aminopolykarboksylsyre eller et salt av denne. Egnede aminopolykarboksylsyrer omfatter etylendiamintetraeddiksyre ("EDTA") og salter av denne, slik som dinatriumsaltet.
DETALJERT BESKRIVELSE AV OPPFINNELSEN
Loratadin er medikamentnavnet gitt til den forbindelse som er kjent som etyl-4-(8-kloro-5,6-dihydro-11 //-benzo[5,6] syklohepta[ 1,2-6]pyridin-11 -y liden)-1 - piperidinkarboksylat og har den empiriske formel C22H23CIN2O2. En strukturformel for denne forbindelse er:

Degradering av sirupfonnuleringer inneholdende loratadin eller beslektede antihistaminer observeres under lagringsstabilitetstesting, slik det viser seg ved reduserende konsentrasjoner av den aktive ingrediens og en samtidig dannelse av forurensninger. To av forurensningene som dannes i loratadinsiruper er blitt identifisert som 2-hydroksymetylloratadin ("2-HML") og 4-hydroksymetylloratadin ("4-HML") mens andre uidentifiserte forurensninger oppstår regelmessig og er samlet blitt betegnet som "gruppe A"; disse stoffene er i antall ca. 5 til 7 og elueres samtidig i en HPLC-analyse ved retensjonstider som antyder en høyere polaritet enn den for loratadin. Alvorlighetsgraden av degraderingen kan minst være omtrent i forhold til volumet i en flaske som ikke er fylt med sirup, dvs. "headspace". Da det ikke er praktisk å fylle hver flaske helt til toppen, er en undersøkelse blitt utført der toppvolumet ble fylt med nitrogengass; resultatene var inkonklusive, muligens på grunn av uunngåelig oksygenpermeabilitet av den polymere flaskekorken.
En annen test ble utført, der vanlig antioksidantadditiver ble inkorporert i sirupen. Ideelt sett vil antioksidanten være løselig i sirupen og er trygg å anvende i matvarer og farmasøytiske preparater. Blant de vannløselige materialer ble askorbinsyre ved 0,1 og 1 mg/ml funnet å redusere degraderingen noe, men medførte også en uakseptabel sterk fargeforandring på produktet, mens natirumbisulfit ga en sterk, ubehagelig lukt til sirupen. De oljeløselige antioksidanter butylert hydroksytoluen og tokoferol var ikke løselig i sirupen, slik at de ble funnet ikke å være akseptable.
Det er blitt funnet at tilsetting av små mengder av en aminopolykarboksylsyre der betegnelsen spesifikt omfatter salter av syrene, kan stabilisere siruper mot degradering. Nyttige aiiiinoporykarboksylsyrer og salter av disse er de som er trygge å innta og har tilstrekkelig løselighet i sirupformuleringene for å danne et stabilt enkeltfasepreparat. Kommersielt tilgjengelige forbindelser som kan anvendes omfatter iminodieddiksyre, metyliminodieddiksyre, nitrilotrieddiksyre, etylendiamintetraeddiksyre ("EDTA"), dietylentriaminpentaeddiksyre, 1,2-diaminosykloheksan-tetraeddiksyre, N-hydroksyetylendiamintrieddiksyre og beslektede forbindelser. Blandinger av to eller flere av de foregående er egnet for anvendelse. Ut fra aspektene om enkel tilgjengelighet, sikkerhet og effektivitet og kostnader foretrekkes for tiden alkalimetallsalter av EDTA, og det resterende av denne beskrivelse vil fokusere på disse materialer.
En aminopolykarboksylsyre eller et salt vil som nevnt være til stede i en sirup ved 0,05 mg/ml til 5 mg/ml. Mer foretrukket vil nivået av aminopolykarboksylsyren være 0,1 mg/ml til 1 mg/ml. Som for et hvilket som helst tilsetningsstoff i en formulering som er tilsiktet å inntas, er det ønskelig å inkorporere det laveste nivå som gir det ønskede resultat. Dette nivå kan enkelt bestemmes ved hjelp av en akselerert lagringsstabilitets-test, der pakninger av sluttproduktet lagres ved forhøyede temperaturer i forhold til de vanlige lagringstemperaturer som produktet forventes å bli eksponert for; de foreliggende oppfinnere har anvendt temperaturer opp til 55 °C for denne hensikt, selv om slike temperaturer har tendens til å medføre en liten misfarging (formørkning) av sirupen, sannsynligvis på grunn av noe karamellisering av sukroseinnholdet. Det forventes at de fleste medikamentdegraderingsreaksjoner vil akselereres ved forhøyet temperatur. Ved forhåndsbestemte intervaller åpnes noen av pakningene og analyseres for å bestemme mengden av aktive ingredienser og forurensninger til stede i formuleringen.
Antihistaminsirupformuleringene inneholder ofte også andre medikamenter, for å oppnå mer enn et terapeutisk resultat fra en enkelt dose. Typiske medikamentsubstanser inkludert sammen med antihistaminet er sympatometiske amindekongestanter, slik som pseudoefedrin eller fenylpropanolamin (for å lette øvre luftveiskongesjon som ofte følger med forstyrrelser som rhinitt og øvre luftveisinfeksjoner), og analgesimedikamenter, slik som aspirin, acetaminofen, ibuprofen, naproxen eller ketoprofen (for lindring av smerte og, unntatt acetaminofen, for å redusere inflammasjon). Hostestillende midler, slik som kodein, hydrokodon eller dekstrometorfan, for lindring av hoste, og slimløsende midler slik som guaifenesin, for å øke hosteproduktiviteten, er også inkludert i kombinasjonsprodukter. Enhver av disse tilleggsingredienser, inkludert salter av disse og andre medikamenter fra den samme terapeutiske klasse, er egnet for inkludering i sirupene ifølge foreliggende oppfinnelse.
Oppfinnelsen vil bli ytterligere beskrevet ved hjelp av de følgende eksempler. Der betegnelsen "prosent" er anvendt her i dokumentet, skal det bety vektprosent dersom ikke annet er angitt.
Eksempel 1
En sirup ble formulert til å inneholde de følgende ingredienser, der mengden av alle unntatt vann er uttrykt i milligram.
Denne sirup tilberedes ved den følgende prosedyre: (a) omtrent 80 % av vannet er plassert i et kar, oppvarmes til 75 - 85 °C, tilsatt sukkeret og omrørt for å danne en løsning; (b) sitronsyre tilsettes løsningen og omrøringen fortsettes for å danne en løsning, deretter tilsettes natriumbenzoat og oppløses; (c) løsningen nedkjøles til 30 - 35 °C med kontinuerlig omrøring, og dinatrium-EDTA tilsettes og oppløses; (d) glyserinet tilsettes og omrøringen fortsetter mens løsningen nedkjøles til 25 - 30 °C; (e) i et annet kar dannes propylenglykol og loratadin og omrøres for å danne en løsning (bemerk at anvendelse av mikroniserte loratadinpartikler nedsetter den tid som er nødvendig for å fullføre oppløsningen), deretter tilsettes smaksstoffet og omrøres for å oppnå homogenitet; (f) produktet fra trinn e kombineres med produktet fra trinn d, med omrøring for å sikre homogenitet, og tilstrekkelig vann tilsettes for å oppnå den riktige formuleringsvekt; og (g) den dannede sirup passeres gjennom klargjørende filtre. Sirupen er en klar, fargeløs væske (som enkelt kan farges om ønskelig, slik som ved å tilsette et egnet farmasøytisk akseptabelt vannløselig fargestoff til sukkerløsningen i trinn a) og betegnes Prøve A.
En annen sirup formuleres på tilsvarende måte, med unntak at det i tillegg inneholder 1 mg/ml av dinatriumsaltet av EDTA. Denne betegnes Prøve B.
25 ml porsjoner av de to sirupene plasseres i 50 ml flintglassampuller, forsegles deretter med gummikorker og aluminiumskapsler. De forseglede ampuller lagres ved 55 °C inntil de tas ut og analyseres ved høy-ytelses væskekromatografi. Resultatene fra analysene er som følger, der "NQ" antyder et resultat under kvantifiseringsgrensen (0,1%), men over deteksjonsgrensen (0,02%):
Disse resultater antydet en betydelig inhibisjon av EDTA av degraderingen av loratadin under de harde lagringsbetingelsene til testen.
EKSEMPEL 2
Prøve A fra det foregående eksempel og tilsvarende tilberedte siruper som også inneholdt 0,1, 0,25, 0,5 eller 0,75 mg/ml dinatrium-EDTA forpakkes som i det foregående eksempel, og lagres og testes på tilsvarende måte. De følgende resultater ble oppnådd, der "ND" antyder en konsentrasjon under deteksjonsgrensen som tidligere uttrykt.
Disse resultater antyder at 0,25 % dinatrium-EDTA ville være et fornuftig nivå for lagringsbeskyttelse av den undersøkte sirup.

"METHOCEL " K100M, K4M og A4M fra Dow Chemical er blant hydroksypropylmetylcellulose produktene som er egnet for anvendelse ifølge oppfinnelsen; erstatning av de ulike tilgjengelige produkter kan forårsake endringer i sirupviskositeten, slik at undersøkelser bør utføres for å bestemme den passende kvalitet og mengde nødvendig for å tilberede en gitt sirup med ønskede egenskaper.

En stabilisert sirup for pediatrisk anvendelse formuleres ifølge den foregående beskrevne generelle prosedyre og inneholder de følgende ingredienser, der mengden av alle unntatt vann er uttrykt i milligram.
Denne sirup er funnet å ha akseptabel lagringsstabilitet.
Claims (10)
1. Sirupformulering,
karakterisert ved at den omfatter et antihistamin valgt blant loratadin, deskarboetoksyloratadin, azatadin og kombinasjoner av hvilke som helst to eller flere av disse, og 0,05 til 5 mg/ml av en aminopolykarboksylsyre eller et salt av denne.
2. Sirupformulering ifølge krav 1,
karakterisert ved at konsentrasjonen av aminopolykarboksylsyren er fra 0,1 til 1 mg/ml.
3. Sirupformulering ifølge krav 1,
karakterisert ved at konsentrasjonen av aminopolykarboksylsyren er fra 0,25 til 0,5 g/ml.
4. Sirupformulering ifølge ethvert av kravene 1 til 3,
karakterisert ved at aminopolykarboksylsyren er utvalgt fra gruppen bestående av: iminodieddiksyre, metyliminodieddiksyre, nitrilotrieddiksyre, etylendiamintetraeddiksyre, dietylentriaminpentaeddiksyre, 1,2-diaminocykloheksantetraeddiksyre, N-hydroksyetylendiamintetraeddiksyre og salter av enhver av disse, og enhver kombinasjon av to eller flere av disse.
5. Sirupformulering ifølge ethvert av kravene 1 til 4,
karakterisert ved at den ytterligere omfatter et dekongestant, et analgetisk middel, et hostestillende middel, et slimløsende middel eller enhver kombinasjon av to eller flere av disse.
6. Sirupformulering ifølge ethvert av kravene 1 til 4,
karakterisert ved at den ytterligere omfatter et dekongestant utvalgt fra gruppen bestående av pesudoefedrin, fenylpropanolamin og deres salter.
7. Sirupformulering ifølge krav 5 eller 5,
karakterisert ved at dekongestantet er pseudoefedrin eller et salt av denne.
8. Sirupformulering ifølge ethvert av kravene 1 til 7,
karakterisert ved at antihistaminet er loratadin.
9. Sirupformulering ifølge ethvert av kravene 1 til 7,
karakterisert ved at antihistaminet er deskarboetoksyloratadin.
10. Sirupformulering ifølge ethvert av kravene 1 til 9,
karakterisert ved at aminopolykarboksylsyren er etylendiamintetraeddiksyre eller et salt av denne.
