Description
Foreliggende oppfinnelse angår imido- og amidosubstituerte acylhydroksamsyrer, fremgangsmåten for å redusere nivåer eller aktiviteter av cytokiner som tumornekrose-faktor a i pattedyr ved administrering derav, samt farma-søytiske preparater av slike derivater.
Tumomekrosefaktor a, TNFCt, er et cytokin som frigis primært av celler i immunsystemene som respons til visse im-munostimulatorer. Administrert til dyr eller mennesker forårsaker den inflammasjon, feber, kardiovaskulære ef-fekter, hemorhage, koagulering, kakeksi samt akuttfase-responser tilsvarende de som ses under akutte infeksjoner, inflammatoriske sykdommer og sjokktilstander. Eksessiv eller ikke-regulerte TNFct-produksjon har vært implikert i et antall sykdomstilstander. Disse inkluderer endotoksemi og/eller toksisk sjokksyndrom (Tracey et al., "Nature" 330, 662-664 {1987} og Hinshaw et al., "Cir. Shock" 30, 279-292 (1990)), reumatoid artritt, inflammatorisk tarmsykdom, kakeksi (Dezube et al., "Lancet", 335
(8690), 662 (1990)), og lupus. TNFa-konsentrasjoner ut over 12.000 pg/ml er påvist i pulmonæraspirater fra Adult Respiratorisk Distress Syndrom (ARDS) pasienter (Millar, et al., "Lancet" 2(8665), 712-714 (1989)). Systemisk in-fusjon av rekombinant TNFCt resulterte i endringer som typisk ses ved ARDS (Ferrai-Baliviera, et al., "Arch. Surg.", 124(12), 1400-1405 (1989)). TNFOt synes å være involvert i et antall benresorpsjons-sykdommer, inkludert artritt. Aktivert ville leukocyttene gi benresorpsjon. TNFCt bidrar tilsynelatende til denne mekanisme (Bertolini et al., "Nature" 319, 516-518 (1986) og Johnson et al., "Endocrinology" 124(3). 1424-1427
(1989)). TNFCc er også vist å stimulere benresorpsjon og inhibere bendannelse in vitro og in vivo ved stimulering av osteoklastdannelse og aktivering kombinert med inhibering av osteoblastfunksjoner. En annen forbindelse med sykdom er assosiasjonen mellom produksjonen av TNFa av tumor- eller vertsvev og malignanassosiert hyperkalcemi ("Calci. Tissue Int." (US) 46(suppl), S3-10 (1990)). I poding-mot-vertsreaksjoner er økede serum TNFot-nivåer assosiert med en vesentlig komplikasjon etter akutt alloge-nisk benmargtransplantasjon (Holler et al., "Blood", 75(4), 1011-1016 (1990)).
Validering av TNFa-inhibering som en klinisk terapi, har vært påvist ved den terapeutiske anvendelse av TNFa-antistoffer og oppløselige TNFcc-reseptorer. TNFoc-bl okker ing med monoklonale anti-TNFa-antistoffer har vist seg å være fordelaktig ved reumatoid artritt (Elliot et al., "Int. J. Pharmac." 1995 17(2), 141-145). Høye nivåer av TNFa er assosiert med Chrons sykdom (von Dullemen, et al., "Gastroenterology", 1995, 109(1), 129-135) der behandling med oppløselig TNFa-reseptor ga kliniske fordeler.
Cerebral malaria er et letalt hyperakutt neurologisk syndrom assosiert med høye blodnivåer av TNFa og den alvor-ligste komplikasjon som opptrer hos malariapasienter. Eleverte nivåer av serum TNFa korrelerte direkte med al-voret av sykdommen og prognosene hos pasienter med akutte malariaangrep (Grau, et al., "N. Engl. J. Med." 320(24), 1586-1591 (1989)).
TNFa spiller en rolle på området kroniske pulmonare inflammatoriske sykdommer. Ved avsetning av silikapar-tikler fører til silikose, en sykdom med progressiv respiratorisk svikt forårsaket av en fibrotisk reaksjon. Antistoffer mot TNFa blokkerte fullstendig den silikain-duserte lungefibrose hos mus (Pignet, et al., "Nature", 344, 245-247 (1990)). Høye nivåer av TNFa-produksjon (i serum og i isolerte makrofager) er påvist i dyremodeller av silika- og asbestindusert fibrose (Bissonnette, et al., "Inflammation" 13(3), 329-339 (1989)). Alveolære makrofager fra pulmonar sarkoidosepasienter er også funnet å spontant å frigi massive mengder TNFa sammenlignet med makrofager fra normale donorer (Baughman, et al., "J. Lab. Clin. Med." 115(1), 36-42 (1990)).
Forhøyede nivåer av TNFa er implisert i reperfusjonsska-de, den inflammatoriske respons som følger reperfusjon og er en hovedårsak til vevsskade etter blodstrømstap (Ved-der, et al., "PNAS" 87, 2643-2646 (1990)). TNFa endrer også egenskapene til endoteliale celler og har forskjellige prokoagulantaktiviteter, for eksempel å produsere en økning i vevfaktorprokoagulantaktiviteten, å dempe anti-koagulant protein C-veien og å regulere ned ekspresjonen av trombomodulin (Sherry, et al., "J. Cell. Biol." 107, 1269-1277 (1988)). TNFa har proinflammatoriske aktiviteter som sammen med dens tidlige produksjon (under de første trinn av et inflammatorisk evenement) gjør den til en sannsynlig mediator for en vevskade i flere betydelige lidelser inkludert, men ikke begrenset til myokardialt infarkt, slag og sirkulatorisk sjokk. TNFa-indusert eks-presjon av adhesjonsmolekyler (ICAM) eller endoteliale leukocyttadhesjonsmolekyler (ELAM) på endoteliale celler kan være spesielt viktig (Munro, et al., "Am. J. Path" 135{1), 121-132 (1989)).
Det er rapportert at TNFa er en potent aktivator for re-trovirusreplikasjon inkludert aktivering av HIV-1. (Duh, et al., "Proe. Nat. Acad. Sei." 86, 5974-5978 (1989); Poll, et al., "Proe. Nat. Acad. Sei." 87, 782-785 (1990); Monto, et al., "Blood" 79, 2670 (1990); Clouse, et al., "J. Immunol." 142, 431-438 (1989); Poll, et al., "AIDS Res. Hum. Retrovirus", 191-197 (1992)). I det minste tre typer eller stammer av HIV (dvs. HIV-1, HIV-2 og HIV-3) er identifisert. Som en konsekvens av HIV-infeksjon blir T-cellemediert immunitet forringet, og infekterte individer manifesterer alvorlige opportunistiske infeksjoner og/eller uvanlige neoplasmer. HIV-entring inn i T-lymfo-cyttene krever T-lymfocyttaktivering. Andre vimser som HIV-1, HIV-2 infekterer T-lymfocytter etter T-celle-aktivering. Denne virusproteinekspresjon og/eller replikasjon medieres eller opprettholdes av denne T-celle-aktivering. Når først en aktivert T-lymfocytt er infek-tert med HIV, må T-lymfocytten fortsette å opprettholdes i en aktivert tilstand for å tillate HIV-genekspresjon og/eller HIV-replikasjon. Cytokiner og spesielt THFa er implisert i aktivert T-cellemediert HIV-proteinekspresjon og/eller -virusreplikasjon ved å spille en rolle ved opprettholdelsen i T-lymfocyttaktivering. Derfor understøt-ter interferens cytokinaktivitet som prevensjon eller in-hiber ing av cytokinproduksjonen, og særlig TNFa, et HIV-infektert individ en begrensning av opprettholdelsen av T-lymfocytt forårsaket av HIV-infeksjon.
Monocytter, makrogater og relaterte celler som kupffer-og glialceller er også implisert i opprettholdelsen av HIV-infeksjonen. Disse celler er på samme måte som T-celler mål for viral replikasjon og viral replikasjon er avhengig av aktiveringstilstanden for cellene. (Rosenberg et al., "The Immunopathogenesis of HIV Infection, Advances in Immunology, 57 (1989).) Cytokiner som TNF a er påvist å aktivere HIV-replikasjon i monocytter og/eller makrofager (Poli, et al., "Proe. Nati. Acad. Sei., 87, 782-784 (1990)), derfor understøtter prevensjon eller in-hiber ing av cytokinproduksjon eller cytokinaktivitet en begrensning av HIV-progresjon for T-celler. Ytterligere studier har identifisert TNFa som en felles faktor ved aktivering av HIV in vitro og har gitt en klar virk-ningsmekanisme via et nukleært regulatorisk protein som finnes i cellecytoplasma (Osborn, et al., "PNAS" 86 2336-2340). Dette funn antyder at en reduksjon av TNFa-syntesen kan ha en antiviral effekt i HIV-infeksjoner ved å redusere transkripsjonen, og således virusproduksjonen.
AIDS viralreplikasjon av latent HIV T-celle og makro-faglinjer kan induseres av TNFa (Folks, et al., "PNAS" 86, 2365-2368 (1989)). En molekylær mekanisme for virus-indusert aktivitet antydes av evnen hos TNFa til å aktivere et genregulatorisk protein, NFKB, som finnes i cellecytoplasma, og som fremmer HIV-replikasjon ved å binde til en viral regulatorisk gensekvens (LTR) (Osborn, et al., "PNAS" 86, 2336-2340 (1989)). TNFa i AIDS-assosiert kakeksi antydes av eleverte serum TNFa og høye nivåer av spontan TNFa-produksjon i perifere blodmonocytter fra pasienter (Wright, et al., "J. Immunol. 141(1), 99-104
(1988)). TNFa har vært implisert i tallrike roller med andre virale infeksjoner som cytomegaliaviruset (CMV), influensaviruset, adenovirus, samt til herpesfamilien av virus av samme grupper som angitt ovenfor.
Den nukleære faktor kB, NFKB, er en pleiotropisk, trans-skripsjonen aktivator (Lenardo, et al., "Cell" 1989, 58, 227-29). NFKB har vært implisert som transkripsjonen aktivator i et antall sykdommer og inflammatoriske tilstander og antas å regulere cytokinnivået inkludert, men ikke begrenset til, TNFa og aktiv HIV-transkripsjon (Dbaibo, et al., "J. Biol. Chem." 1993, 17762-66; Duh, et al., "Proe. Nati. Acad. Sei." 1989, 86, 5974-78; Bachelerrie, et al., "Nature" 1991, 350, 709-12; Boswas, et al., "J. Acquired Immune Deficiency Syndrome" 1993, 6, 778-786; Suzuki, et al., "Biochem. And Biophys. Res. Comm." 1993, 193, 277-83; Suzuki, et al., "Biochem. And Biophys. Res Comm." 1992, 189, 1709-15; Suzuki, et al., "Biochem. Mol. Bio. Int." 1993, 31(4), 693-700; Shakhov, et al., "Proe. Nati. Acad. Sei." USA 1990, 171, 35-47; og Staal, et al., "Proe. Nati. Acad. Sei." USA 1990, 87, 9943-47). Således ville det være nyttig å inhibere NFieB-akt iver ing, nukleær translasjon eller binding for å regulere transkripsjonen av ett eller flere cytokingener og via denne modulering og andre mekanismer å være nyttig med henblikk på å inhibere et antall sykdomstilstander.
Mange cellefunksjoner medieres av nivåer av adenosin 35'-cyklisk monofosfat (cAMP). Slike cellulære funksjo-ner kan bidra til inflammatoriske tilstander og sykdommer inkludert astma, inflammasjon og andre tilstander (Lowe and Cheng, "Drugs of the Future", 17(9), 799-807, 1992). Det er vist at en forhøyning i cAMP i inflammatoriske leukocytter inhiberer deres aktivering og den etterføl-gende frigivning av inflammatoriske mediatorer inkludert TNFa og NFKB. Økede nivåer av cAMP fører også til relaksering av den glatte luftveismuskel.
Den primære cellulære muskel for inaktivering av cAMP er nedbrytning av cAMP av en familie av isoenzymer som angis som sykliske nukleotidfosfodiestereaser (PDE) (Beavo and Reitsnyder, "Trends in Pharm.11, 150-155, 1990). Det er ti kjente medlemmer av familien av PDE'er. Det er godt dokumentert at inhiberingen av PDE type IV-(PDE 4-)enzymet er særlig effektivt både ved inhibering av inflammatorisk mediatorfrigivning og relaksering av den glatte luftveimuskel (Verghese, et al., "Journal of Pharmacology and Experimental Therapeutics", 272(3), 1313-1320, 1995).
En reduksjon av TNFa-nivåene og/eller økning av cAMP-nivåene utgjør således en verdifull terapeutisk strategi for behandling av mange inflammatoriske, infeksiøse og immunologiske sykdommer samt malignantsykdommer. Disse inkluderer, men er ikke begrenset til: septisk sjokk, sepsis, endotoksisk sjokk, hemodynamisk sjokk og sepsis syndrom, postiskemisk reperfusjonskade, malaria, mycobak-teriell infeksjon, meningitt, psoriasis og andre dermale sykdommer, kongestive hjertesvikt, fibrotisk sykdom, kakeksi, podingsawising, cancer, tumorvekst, uønsket angiogenese, autoimmunsykdom, opportunistiske infeksjoner i AIDS, reumatoid artritt, reumatoid spondolitt, osteoartritt, andre artrittiske tilstander, inflammatorisk tarmsykdom, Crohns sykdom, ulcerøs kolitt, multippel sklerose, systemisk lupus erytrematose, ENL i leprosi, strålingsskade og hypertoksisk alveolær skade. Tidligere forsøk rettet mot suppresjon av virkningene av TNFa har rangert fra anvendelse av steroider som deksametason og prednisolon til bruken av både polyklonale og monoklonale antistoffer (Beutler, et al., "Science" 234, 470-474
(1985); WO 92/11383).
Angiogenese, prosessen ved utvikling og dannelse av nye blodkar, spiller en viktig rolle i tallrike normale og patologiske fysiologiske evenementer. Angiogenese inntrer som respons på spesifikke signaler og involverer en komp-leks prosess som karakteriseres ved infiltrering av den basale lamina på grunn av vaskulære endoteliale celler i respons på angiogenetiske vekstsignaler, migrering av de endoteliale celler mot signalkilder og etterfølgende pro-liferering og dannelse av kapillarrøret. Blodstrøm gjennom de nydannede kapillarer initieres etter at de endoteliale celler kommer i kontakt med og forbindes med en allerede eksisterende kapillar. Angiogenese kreves for tumorvekst ut over en viss størrelse.
Inhibitoriske innflytelser overveier i den naturlig opp-tredende balansen mellom endogeniske stimulatorer og inhibitorer av angiogenese (Rastinejad, et al., 1989, "Cell" 56:345-355). I de sjeldne tilfeller der neovaskularisering inntrer under normale fysiologiske tilstander som sårheling, organregenerering, embryonisk utvikling og female reproduktive prosesser, er angiogenese stringent regulert og spatialt og temporært avgrenset. Under tilstander med patologisk angiogenese, så vel som tilstander som karakteriserer fast tumorvekst, mangler disse regula-toriske kontroller.
Ikke-regulert angiogenese blir patologisk og understøtter progresjon av mange neoplastiske og ikke-neoplastiske sykdommer. Et antall alvorlige sykdommer domineres av anormal neovaskularisering, inkludert fast tumorvekst og metastaser, artritt, visse typer øyelidelser og psoriasis (Moses, et al., 1991, "Biotech." 9:630-634; Folkman et al., 1995, "N. Engl. J. Med.", 333-1757-1763; Auerbach, et al., 1985, "J. Microvasc. Res." 29:401-411; Folkman, 1985, "Advances in Cancer Research", red. Klein og Wein-house, Academic Press, New York, s. 175-203; Patz, 1982, "Am. J. Opthalmol." 94:715-743; og Folkman, et al., 1983, "Science" 221:719-725). I et antall patologiske tilstander bidrar angiogeneseprosessen til sykdomstilstanden. For eksempel antyder signifikante data at veksten av faste tumorer er avhengig av angiogenese (Folkman og Kags-brun, 1987, "Science" 235:442-447).
Opprettholdelsen av avaskulariteten i cornea, linse og trabekulærnettverket er avgjørende for synet så vel som for okkulær fysiologi, se for eksempel oversikter av Waltman, et al., 1978, "Am. J. Opthal." 85:704-710 og Gartner, et al., 1978, "Surv. Opthal." 22:291,312. Vanligvis er behandlingen av disse sykdommer, og særlig først når neovaskularisering har oppstått, uttilstrekke-lig og blindhet er ofte resultatet.
En inhibitor for angiogenese kunne ha en betydelig terapeutisk rolle ved å begrense bidragene ved denne prosess til patologisk progresjon av de underliggende sykdomstilstander så vel som å tilveiebringe verdifulle midler for å studere deres etiologi. For eksempel kunne midler som inhiberer tumorneovaskularisering spille en betydelig rolle ved å inhibere metastatisk og fast tumorvekst.
Flere typer forbindelser har vært benyttet for forhindre angiogenese. Taylor, et al., benyttet protamin for å inhibere angiogenese, (Taylor, et al, "Nature" 297:307
(1982)). Toksisiteten hos protamin begrenser den praktis-ke anvendelse som terapeutikum. Folkman, et al. benyttet heparin og steroider for å kontrollere angiogenese (Folkman, et al., "Science" 221:719 (1983) og US-patent 5,001,116 og 4,994,443). Steroider som tetrahydrokortisol og som mangler gluko- og mineralkortikoid aktivitet, er angiogeniske inhibitorer. Interferon-p er også en potent inhibitor for angiogenese indusert av allogeniske milt-celler (Sidky, et al., "Cancer Research" 47:5155-5161
(1987)). Human rekombinant interferon-a ble rapportert å være vellykket benyttet ved behandling av pulmonær he-mangiomatose, en angiogeneseindusert sykdom (White, et al., "New England J. Med." 320:1197-1200 (1989)).
Andre midler som har vært benyttet for inhibere angiogenese, inkluderer askorbinsyreetere og relaterte forbindelser (Japansk Kokai Tokkyo Koho nr. 58-131978). Sulfa-tert polysakkarid DS 4152 viser også angiogenisk inhibering (Japansk Kokai Tokkyo Koho nr. 63-119500). Et fungal-produkt, fumagillin, er et potent angiostatisk middel in vi tro. Forbindelsen er toksisk in vivo, men et syntetisk derivat, AGM 12470, har vært benyttet in vivo for å behandle kollagen II artritt. Fumagillin og o-substituerte fumagillinderivater er beskrevet i EP-patent 0325199A2 og 0357061A1.
I US 5,874,081 beskriver Parish bruken av monoklonale antistoffer for å inhibere angiogenese. I WO 92/12717 beskriver Brem et al. at visse tetrasykliner og særlig minosyklin, klortetrasyklin, demeklosyklin og lymesyklin er brukbare som inhibitorer for angiogenese. Brem et al. beskriver at minosyklin inhiberer angiogenese i en grad sammenlignbar med den kombinasjonsterapien av heparin og kortison ("Cancer Reserach", 51, 672-675, 15. januar 1991). Teicher et al. beskriver at tumorvekst reduseres og antallet metastaser reduseres når det antiangiogeniske middel minosyklin benyttes i forbindelse med cancerkjemo-terapi eller -stråleterapi ("Cancer Research", 52, 6702-6704, 1. desember 1992).
Makrofagindusert angiogenese er kjent for å bli stimulert av TNFCt. Leibovich et al. rapporterte at TNFa induserer in vivo kapillar bloddannelse i rottekornea og de utvik-lende kyllingchorioallantoriske kyllingmembraner ved me-get lave doser og antyder at TNFa er en kandidat for å indusere angiogenese i inflammasjon, sårheling og tumorvekst ("Nature", 329, 630-632 (1987)).
Alle de forskjellige celletyper i kroppen kan transforme-re til benigne eller malignante tumorceller. Det hyppigs-te tumorsetet er lungen fulgt av kolorektal, bryst, pro-stata, blære, pankreas og ovarie. Andre overveiende typer av cancer inkluderer leukemi, sentralnervesystemcancere, hjernecancer, melanoma, lymfoma, erytroleukemi, uterin-cancer, bencancer og hode- og halscancer.
Cancer blir i dag hovedsakelig behandlet med en eller en kombinasjon av tre typer av terapier: kirurgi, stråling og kjemoterapi. Kirurgi involverer massefjerning av sykt vev. Mens kirurgi enkelte ganger er effektiv med henblikk på å fjerne tumorer som er lokalisert på visse seter (f.eks. bryst, kolon og hud), kan kirurgi ikke benyttes ved behandling av tumorer som er lokalisert i andre områ-der (f.eks. ryggraden) eller ved behandling av dissemi-nerte neoplastiske tilstander (f.eks. leukemi). Kjemoterapi involverer oppbryting av cellereplikasjonen eller cellemetabolismen. Kjemoterapi benyttes som oftest ved behandling av leukemi så vel som bryst-, lunge- og tes-tikkelcancer.
Kjemoterapeutiske midler kalles ofte antineoplastiske midler. Alkyleringsmidlene antas å virke ved å alkylere og kryssbinde guanin og eventuelt andre baser i DNA og derved stanse celledeling. Typiske alkyleringsmidler inkluderer nitrogensenneper, etyleniminforbindelser, alkyl-sulfater, cisplatin og tallrike nitroureaer. En mangel ved disse forbindelser er at de ikke bare angriper malignante celler, men også andre celler som deler seg naturlig, som de i benmargen, huden, gastrointestinal mukose og fetalvev. Antimetabolitter er typisk reversible eller irreversible enzyminhibitorer eller forbindelser som ellers interfererer med replikasjon, translasjon eller transkripsjon av nukleinsyre. Således ville det være å foretrekke å finne mindre toksiske forbindelser for can-cerbehandling.
Matriksmetalloproteinase (MMP) inhibering har vært assosiert med diverse aktiviteter inkludert inhibering av TNF a (Mohler, et al., "Nature", 370, 218-220 (1994)) og inhiberingen av angiogenese. MMP'er er en familie av ut-skilte og membranbundne sinkendopeptidaser som spiller en nøkkelrolle i både fysiologisk og patologisk vevnedbry-ting (Yu, et al., "Drugs & Aging, 1997, (3): 229-244; Wojtowicz-Praga, et al., "Int. New Drugs", 16:61-75
(1997)). Disse enzymer er kapable til å bryte ned komponenter i den ekstracellulære rnatriks inkludert fibrillære. og ikke-fibrillære kollagener, fibronektin, laminin og membranglykoproteiner. Vanligvis er det en delikat balanse mellom celledeling, matrikssyntese, matriksnedbrytning (under kontroll av cytokiner), vekstfaktorer og celle-matriksinteraksjoner. Under patologiske tilstander kan imidlertid denne balanse forrykkes. Øket MMP-aktivitet er detektert i et vidt spekter av can-cere (Denis, et al., "Invest. New Drugs", 15: 175-185
(1987)). På samme måte som ved TNFa, antas MMP'er å være involvert i den invasive prosess av angiogenese og tumor-metastase.
Foreliggende oppfinnelse er basert på den oppdagelse at visse klasser av forbindelser som er mer detaljert beskrevet her, reduserer nivåene av TNFa, øker cAMP-nivåene, inhiberer fosfodiesteraser (PDE'er og særlig PDE 4), påvirker tumorer og påvirker angiogenese.
Forbindelsene som her beskrives, kan inhibere virkningen av NFKB i nukleus og er således brukbar ved behandling av et antall sykdommer inkludert, men ikke begrenset til reumatoid artritt, reumatoid spondylitt, osteoartritt andre artrittiske tilstander, septisk sjokk, sepsis, endotoksisk sjokk, poding-mot-vertsykdom, spill, inflammatorisk tarmsykdom, Chrons sykdom, ulcerativ kolitt, multippel sklerose, systemisk lupus erytrematose, ENL i leprosy, cancer, HIV, AIDS og opportunistiske infeksjoner i AIDS. TNFa- og NFKB-nivåene påvirkes av en resiprok feedbacksløyfe. Som angitt ovenfor, påvirker forbindelsene ifølge oppfinnelsen nivåene både av TNFa og NFKB. Forbindelsene ifølge oppfinnelsen inhiberer PDE4.

Forbindelsene med formel I benyttes, under overvåking av kvalifiserte profesjonelle, til å inhibere de uønskede virkninger av TNFa og/eller inhibere fosfodiesteraser, og/eller inhibere inflammasjon og eller angiogenese og/eller cancer. Inhibering av fosfodiesteraser type 4 (PDE4 eller PDE IV) er den foretrukne utførelsesform av foreliggende søknad. Forbindelsene kan administreres oralt, rektalt eller parenteralt, alene eller i kombinasjon med andre terapeutiske midler inkludert antibiotika, steroider osv. til pattedyr som trenger behandling.
Forbindelsene ifølge oppfinnelsen kan benyttes topisk med behandling eller profylakse av topiske sykdomstilstander som medieres eller forverres av inflammasjon, ekssesivt TNFa-produksjon, eksessive MMP'er eller der økende cAMP-nivåer vil være nyttige. Noen eksempler inkluderer virale infeksjoner som de som forårsakes av herpes-viruser eller viral konjunktivitt, eller dermale tilstander som psoriasis eller atopisk dermatitt osv.
Forbindelsene kan også benyttes i veterinærbehandling av pattedyr forskjellig fra mennesker og som trenger prevensjon eller inhibering av TNF a-produksjon. TNFa-medierte sykdommer for terapeutisk eller profylaktisk behandling i dyr inkluderer sykdommer som de som er angitt ovenfor, men særlig virale infeksjoner. Oppfinnelsen angår også MMP-inhiberende forbindelser, preparater derav og anvendelse derav ved behandling av sykdom og lidelser assosiert med uønsket produksjon eller aktivitet av MMP'er. Disse forbindelser er i stand til å inhibere konnektiv vevnedbrytning og er brukbare ved terapi eller prevensjon av tilstander som involverer vevnedbrytning. Forbindelsene med formel IV fremstilles lett ved omsetning av en karboksylsyre med formelen:

I hvilken som helst av de foregående reaksjoner kan en nitroforbindelse benyttes der nitrogruppen omdannes til en aminogruppe ved katalytisk hydrogenering eller kjemisk reaksjon. Begge racemater av disse isomerer og de individuelle isomerer i seg selv, så vel som diastereomerer når det er to kirale senter, ligger innenfor rammen av oppfinnelsen. Racernatene kan benyttes som sådanne eller kan separeres i de individuelle isomerer mekanisk ved kroma-tografi ved bruk av en kiral absorbent. Oppfinnelsen angår også de fysiologisk akseptable, ikke-toksiske syreaddisjonssalter med forbindelsen med formel I. Slike salter inkluderer de som avledes fra organiske eller uorganiske syrer som, uten begrensning, hydroklor-syrer, hydrobrom-, fosfor-, svovel-, metansulfon-, ed dik-, vin-, melke-, rav-, sitron-, eple-, malein-, sor-bin-, akonitt-, salisyl, ftal-, embon-, enantinsyre og lignende.
Orale doseringsformer inkluderer tabletter, kapsler og
dragéer og tilsvarende formgitte, pressede farmasøytiske formler inneholdende fra 1 til 100 mg medikament per enhetsdose. Isotoniske saltoppløsninger inneholdende fra 20 til 100 mg/ml kan benyttes for parenteral administrering inkludert intramuskulær, intratekal, intravenøs og intra-arterielle administreringsveier. Rektal administrering kan gjennomføres via bruk av suppositorier som er formu-lert fra konvensjonelle bærere som kakaosmør.
Farmasøytiske preparater omfatter således en eller flere forbindelser med formlene I eller forbindelser av produktet i reaksjon a assosiert med minst en farmasøytisk ak-septabel bærer, diluent eller eksipient. Ved fremstilling av slike preparater ble de aktive bestanddeler vanligvis blandet med eller fortynnet med en eksipient, eller inne-lukket i en slik bærer som kan være i form av en kapsel eller pose. Når eksipiensen tjener som diluent, kan den være et fast, halvfast eller flytende materiale som vir-ker som bærer, vehikkel eller medium for den aktive bestanddel. Således kan preparatene være i form av tabletter, piller, pulvere, eliksirer, suspensjoner, emul-sjoner, oppløsninger, siruper, myke og harde gelatinkapsler, suppositorier, sterile injiserbare oppløsninger og sterilt pakkede pulvere. Eksempler på egnede eksi-pienter er laktose, dekstrose, sukkrose, sorbitol, mannitol, stivelse, akasiegummi, kalsiumsilikat, mikrokrystal-linsk cellulose, polyvinylpyrrolidinon, polyvinyl-pyrrolidon, cellulose, vann, sirup og metylcellulose, og formuleringene kan i tillegg inkludere smøremidler som talkum, magnesiumstearat og mineralolje, fuktemidler, emulgerings- og suspensjonsmidler, preserveringsmidler som metyl- og propylhydroksybenzoater, søtningsmidler og luktestoffer.
Preparatene formuleres fortrinnsvis til enhetsdoseform, noe som betyr fysikalsk diskrete enheter som er egnet som enhetsdosering, eller en på forhånd bestemt brøkdel av en enhetsdose for administrering i et enkelt- eller multip-peldoseregime til menneskelige individer og andre pattedyr, idet hver enhet inneholder en på.forhånd bestemt mengde av aktivt materiale, beregnet for å gi den ønskede terapeutiske virkning i forbindelse med et egnet farma-søytisk drøyemiddel. TNFa-inhibering i LPS-stimulerte, mononukleære, humanpe-rifere blodceller, PBMOer, kan gjennomføres som beskrevet nedenfor. Enzymbundne immunosorbentanalyser, ELISA, for TNFa kan gjennomføres på konvensjonelle måter. PBMCer isoleres fra normaldonorer ved Ficoll-Hypaque densitetssentrifugering. Cellene dyrkes i RPMI supplert med 10% AB+-serum, 2 mM glutamin, 100 U/ml {enheter pr. ml) penicillin og 100 mg/ml streptomycin. Medikamenter oppløses i dimetylsulfoksid {Sigma Chemical), og ytterligere fortynninger gjennomføres i supplert RPMI, et vel-kjent medium. De endelige dimetylsulfoksidkonsentrasjoner i nærvær eller fravær av medikament i PBMC-suspensjonene er 0,25 vektprosent. Medikamenter analyseres som halvlogg eller logg-fortynninger som begynner ved 100 uM. Medikamenter settes til PBMC (10<6> celler/ml) i 96-brønners pla-ter 1 time før tilsetting av LPS. PBMCer {IO<6> celler/ml) i nærvær eller fravær av medikamenter stimuleres ved behandling av 1 ug/ml eller 100 ng/ml LPS fra Salmonella minnesota R595 (List Biological Labs, Campbell, CA). Celler inkuberes ved 37 °C i 18-20 timer. Supernatanter høs-tes og analyseres umiddelbart på TNFa-nivåer eller holdes dypfryst ved -70 °C (i ikke mer enn 4 dager) inntil analyse. Konsentrasjonen av TNFCt i supernatanten bestemmes ved human TNFa ELISA-kit (ENDOGEN, Boston, MA) i henhold til produsentens retningslinj er.
Inhibering av fosfodiesterase type 4 (PDE 4) kan også bestemmes i konvensjonelle modeller. For eksempel kan man benytte en modifikasjon av metoden ifølge Hill og Mitchell, der U937-celler (en human promonocytisk cellelin-je) dyrkes til 1 x IO<6> celler/ml og samles ved sentrifugering. En cellepellet på 1 x IO<9> celler vaskes i fosfat-bufret saltoppløsning og fryses så ved -70 °C for senere rensing eller umiddelbart lysering i kald nomogeniser-ingsbuffer (20 mM Tris-HCl, pH 7,1, 3 mM 2-merkapto-etanol, 1 mM magnesiumklorid, 0,1 mM etylenglykol-bis-(p-aminoetyleter)-N,N,N',N'-tetraeddiksyre (EGTA), 1 uM fe-nylmetylsulfonylfluorid (PMSF) og 1 ug/ml leupeptin). Cellene homogeniseres med 20 slag i en Dounce homogeni-sør, og supernatanten inneholdende den cytosoliske frak-sjon oppnås ved sentrifugering. Supernatanten fylles så på en Sefacryl S-200 kolonne som er ekvilibrert i en ho-mogeniser ingsbuf fer . Råfosfodiesterase type 4 enzymet elueres i homogeniseringsbuffer i en hastighet på rundt 0,5 ml/min, og fraksjoner analyseres på fosfodiesterase-aktivitet ved bruk av rolipram. Fraksjoner inneholdende PDE 4 aktivitet (rolipram-sensitiv) slås sammen og alik-voteres for senere bruk.
Fosfodiesteraseanalyse gjennomføres basert på prosedyrer som beskrevet av Hill og Mitchell (Hill og Mitchell, Fa-seb J., 8, A217 (1994)). Analysen gjennomføres i et to-talvolum på 100 ul inneholdende forskjellige konsentrasjoner av forbindelsen av interesse, 50 mM Tris-HCl, pH 7,5, 5 mM magnesiumklorid og 1 uM cAMP, hvorav 1 % er <3>H cAMP. Reaksjonsblandingen inkuberes ved 30 °C i 30 minutter og termineres så ved koking i 2 minutter. Mengden PDE 4-holdig ekstrakt som benyttes for disse forsøk, bestemmes på forhånd, slik at disse reaksjonene ligger innenfor det lineære området og forbruker mindre enn 15 % av det totale substrat. Etter ferdig reaksjon avkjøles prøvene til 4 °C og behandles så med 10 ul 10 mg/ml slangegift i 15 minutter ved 30 °C. Ikke benyttet substrat fjernes så ved tilsetting av 200 ul kvaternær ammoniumionebyttehar-piks (AG1-X8, BioRad) i 15 minutter. Prøver spinnes så ved 3000 omdr./min i 5 minutter og 50 ul av den vandige fase tas for telling. Hvert datapunkt gjennomføres i dup-likat, og aktiviteten uttrykkes som prosentandel av kontroll. ICso-verdiene for forbindelsene bestemmes deretter fra doseresponskurvene for et minimum på 3 uavhengige forsøk.

En blanding av (3-(1,3-dioksoisoindolin-2-yl)-3-(3-etoksy-4-metoksyfenyl)propanhydroksamsyre (5,8 g, 15 mmol) og propionsyreanhydrid (3,93 g, 30,2 mmol) i 170 ml vannfri acetonitril ble omrørt ved romtemperatur under nitrogen over natten. Fjerning av oppløsningsmiddelet under vakuum ga en olje. Oljen ble så omrørt i 25 ml eter. Alle de faste bestanddeler tvinges først gjennom en sikt på 0,25 mm maskevidde. Mannitolen og laktosen blandes, granuleres under tilsetning av gelatinoppløsning, tvinges gjennom en sikt på 2 mm maskevidde og tørkes ved 50 °C og tvinges så nok en gang gjennom en sikt med maskevidde 1,7 mm. (3-(1,3-dioksoisoindolin-2-yl)-3-(3-etoksy-4-metoksy-fenyl)propanoylamino)propanoat, glyein og sakkarin blandes omhyggelig, mannitol, laktosegranulatet, stearinsyren og talkumen tilsettes, og det hele blandes grundig og presses til tabletter med ca. 10 mm diameter og som er konkave på begge sider og med en bruddspor på oversiden.

Tabletter, hver inneholdende 10 mg (3-(1,3-dioksoisoindolin-2-yl)-3-(3-etoksy-4-metoksyf en<y>Dpropan-oylamino)propanoat, ble fremstilt på følgende måte:

De faste bestanddeler tvinges først gjennom en sikt på 0,6 mm maskevidde. Deretter blandes den aktive imid-bestanddel, laktose, talkum, magnesiumstearat og halvparten av stivelsen grundig. Den andre halvpart av stivelsen suspenderes i 65 ml vann og denne suspensjonen settes til en kokende oppløsning av polyetylenglykol i 260 ml vann. Den resulterende pasta settes til de pulverformige stoffer og det hele blandes og granuleres, hvis nødvendig under tilsetting av vann. Granulatet tørkes over natten ved 35 °C og tvinges gjennom en sikt med 1,2 mm maskevidde og presses for å danne tabletter med ca. 10 mm diameter og som er konkave på begge sider og har en bruddspor på oversiden.

Tørrfylte geiatinkapsler, hver inneholdende 100 mg (3-(1,3-dioksoisoindolin-2-yl)-3-(3-etoksy-4-metoksyfenyl)-propanoylamino)propanoat, ble fremstilt på følgende måte:
Bestanddeler ( for 1000 tabletter)
Natriumlaurylsulfat siktes inn i (3-(1,3-dioksoisoindolin-2-yl)-3-(3-etoksy-4-metoksyfenyl)propanoylamino)-propanoat gjennom en sikt med maskevidde 0,2 mm og de to komponenter blandes grundig i 10 minutter. Den mikrokrys-tallinske cellulose tilsettes så gjennom en sikt med maskevidde 0,9 mm og det hele blandes igjen i 10 minutter. Til slutt settes magnesiumstearatet til gjennom en sikt med maskevidde 0,8 mm og etter blanding i ytterligere 3 minutter innføres blandingen i porsjoner på 140 mg hver i langstrakte størrelse 0 tørrfyll gelatinkapsler.

En 0,2 % injeksjons- eller infusjonsoppløsning kan for eksempel fremstilles på følgende måte:
(3-(1,3-dioksoisoindolin-2-yl)-3-(3-etoksy-4-metoksy-fenyl)propanoylamino)propanoat oppløses i 1000 ml vann og filtreres gjennom et mikrofilter. Bufferoppløsningen tilsettes og det hele bringes til 2500 ml med vann. For å preparere enhetsdoseformer innføres porsjoner på 1,0 eller 2,5 ml i glassampuller, hver inneholdende henholdsvis 2,0 eller 5,0 mg imid. EKSEMPEL 25 ( 3-( 4- acetylamino- 1, 3- dioksoisoindolin- 2- yl)- 3-( 3- etoksy-4- metoksyfenyl)- propanoylamino) propanat (3-(4-acetylamino-1,3-dioksoisoindolin-2-yl)-3-(3-etoksy-4-metoksyfenyl)-propanoylamino)propanat ble fremstilt ifølge prosedyren i eksempel 1 fra (3-(4-acetylamino-l,3-dioksoisoindolin-2-yl)-3-(3-etoksy-4-metoksyfenyl)-propanhydroks-aminsyre (lg, 2,26 mmol) og propionsyreanhydrid (0,59 g, 4,53 mmol) i 30 ml vannfritt acetonitril. Produktet ble oppnådd som et hvitt fast stoff (0,96 g, 87 %) : Smp: 147-149 °C;

Farmasøytisk preparat, karakterisert ved at det omfatter en mengde av en forbindelse ifølge krav 1 i form av en i det vesentlige kiralt ren (R)-isomer, en i det vesentlige kiralt ren (S)-isomer eller en racemisk blanding derav, tilstrekkelig til ved administrering i et enkelt eller multippelt doseregime å inhibere uønskede nivåer av minst en av matriksmetallopro-teinaser eller PDE 4 hos pattedyr i kombinasjon med en bærer.
9. Anvendelse av en forbindelse ifølge krav 1 i form av en i det vesentlige kiralt ren (R)-isomer, en i det vesentlige kiralt ren (S)-isomer eller en racemisk blanding derav, for fremstilling av medikamenter for inhibering av uønskede nivåer av TKF-Ct i pattedyr.
10. Anvendelse av en forbindelse ifølge krav 1 i form av en i det vesentlige kiralt ren (R)-isomer, en i det vesentlige kiralt ren (S)-isomer eller en racemisk blanding derav, for fremstilling av medikamenter for inhibering av uønskede matriksmetal-loproteinaser i pattedyr.

Anvendelse av en forbindelse ifølge krav 1 i form av en i det vesentlige kiralt ren (R)-isomer, en i det vesentlige kiralt ren (S)-isomer eller en racemisk blanding derav, for fremstilling av medikamenter for behandling av inflammatorisk sykdom og autoimmun sykdom, særlig artritt, rheumatoid artritt, inflammatorisk tarmsykdom, Krohn"s sykdom, aftøse ulsere, kakeksi, poding-mot-vertsykdom, astma, COPD, psoriasis, atopisk dermatitt, lupus, adult respiratorisk distress syndrom og ervervet immun defektsyndrom, i pattedyr.
12. Anvendelse av en forbindelse ifølge krav 1 i form av en i det vesentlige kiralt ren (R)-isomer, en i det vesentlige kiralt ren (S)-isomer eller en racemisk blanding derav, for fremstilling av medikamenter for behandling av cancer i pattedyr.
13. Anvendelse av et acylhydroksamsyrederivat ifølge krav 1 i form av en i det vesentlige kiralt ren (R)-isomer, en i det vesentlige kiralt ren (S)-isomer eller en racemisk blanding derav, for fremstilling av medikamenter for behandling av uønsket angiogenese i pattedyr.
14. Anvendelse av en forbindelse ifølge krav 1 i form av en i det vesentlige kiralt ren (R)-isomer, en i det vesentlige kiralt ren (S)-isomer eller en racemisk blanding derav, for fremstilling av medikamenter for inhibering av fosfodiesteraser type IV eller PDE 4 i pattedyr.
