Description
Bakgrunn for oppfinnelsen
Foreliggende oppfinnelse vedrører nye 4-oksocykliske urinstoffer og farmasøytiske sammensetninger derav, som er nyttige ved behandling av mennesker eller andre pattedyr med kardial arytmi og/eller kardial fibrillasjon.
De nye 4-oksocykliske urinstoffer ifølge foreliggende oppfinnelse er aktive som antifibrillatoriske og antiarytmiske midler. Foreliggende forbindelser har utbredt virkning mot kardial arytmi og fibrillasjon, og de kan med fordel anvendes for å vesentlig lindre og/eller forhindre arytmi og fibrillasjon. I tillegg har nevnte forbindelser lavere forekomst av noen av de uønskede bivirkninger som mange konvensjonelle antiarytmiske terapeutiske midler har. En ytterligere fordel med forbindelsene som er beskrevet her, er at de har både antifibrillatorisk og antiarytmisk aktivitet; de fleste konvensjonelle terapeutiske midler har vanligvis ikke virkning som antifibrillatoriske midler. I et friskt, strukturelt sunt hjerte karakteriseres den presise, sekvensielle, elektriske aktivering, deretter deaktivering, av hele den kardiale muskel som usvikelig opptrer ved hvert slag, som normal kardial rytme. Arytmier karakteriseres som forekomst av unormal elektrisk aktivitet som kan innvirke på normal kardial rytme. Den unormale elektriske aktivitet kan innvirke på initieringen av, og/eller den ensartede spredning av, den elektriske bølge (dvs. depolarisasjon etterfulgt av repolarisasjon av den kardiale muskel) som får hjertet til å trekke seg sammen. Avbrytelsen av den jevne, periodiske prosess i den kardiale funksjon forbundet med normal kardial rytme ved forekomst av arytmier er i noen tilfeller livstruende.
Arytmier varierer i alvorsgrad fra relativt godartet (bestående av asymptomatiske og sjeldne, premature, ventrikulære komplekser [PVC er] til livstruende (bestående av ventrikulaer f ibrillasjon og undertrykket ventrikulaer takyarytmi). Livstruende arytmier er angitt som den mest utbredte døds-årsak over hele verden. For eksempel er det anslått at plutselig kardial død som et resultat av ventrikulaer fibrillasjon dreper ca. 400.000-600.000 mennesker i USA hvert år, U.S. Department of Health and Human Services (1985) NCHS Monthly Vital Statistics Report 33:8-9.
Arytmier klassifiseres generelt i to typer: 1) supraventrikulære arytmier (f.eks. atrial fibrillasjon og hjertebank) og 2) ventrikulære arytmier (f.eks. ventrikulær takyarytmi og ventrikulær fibrillasjon og hjertebank).
Supraventrikulære arytmier er normalt ikke livstruende. Individer med disse arytmier kan oppleve et stort antall symptomer, av svak til alvorlig intensitet. Disse individer kan fysisk føle tapte slag, ekstraslag og/eller hjertebank, kan iblant føle seg noe lette i hodet eller svimle, og de kan ha åndenød og/eller brystsmerter. Siden denne situasjon faktisk ikke er livstruende normalt, foreskrives ikke mer aggressive terapier, såsom konvensjonelle antiarytmiske medisiner, fordi bivirkningene som normalt forbindes med dem, kanskje ikke er akseptable for en ikke-livstruende tilstand. Imidlertid tolereres de nye forbindelser i henhold til foreliggende oppfinnelse, vanligvis meget bedre enn mange av de konvensjonelle antiarytmiske midler som for tiden er tilgjengelige, og de vil derfor være en akseptabel terapi for mange pasienter som lider av supraventrikulære arytmier, og de vil vesentlig underlette det ubehag disse individer føler.
Ventrikulære arytmier på den annen side er potensielt meget alvorligere, og de klassifiseres i tre grupper: 1) godartet; 2) prognostikalt-signifikant (potensielt dødelig); og 3) livstruende (letal). For individer med godartede arytmier finnes det meget lav risiko for død, hjertearrdannelse og hjertesykdom. Godartede ventrikulære arytmier er relativt vanlige og forekommer for ca. 30% av alle ventrikulær arytmier. Id. Godartede arytmier, såsom premature, ventrikulære komplekser (PVC - er), utgjør minimale risikoer for den enkelte, og de krever sjelden arytmisk terapi. Imidlertid kan PVC ene være hyp-pige og komplekse, eller de er forbundet med tilstrekkelig alarmerende symptomer, slik at den enkelte som opplever dem, ikke er mottakelig for forsikringer om at arytmiene og symptomene ikke er farlige. De er kanskje heller ikke mottakelige for mer konvensjonell behandling, f.eks. beta-blokkerere. I disse tilfeller vil behandling med forbindelsene beskrevet her sannsynligvis være gunstige for disse individer.
Prognostikalt signifikante arytmier er forbundet med en ytterligere klinisk fremstilling av kardial sykdom, såsom mild hjertesvikt, iskemiske sykdommer og/eller kardial frykt. Det er blitt fastslått at ca. 65% av alle ventrikulære arytmier er prognostikalt signifikante. Se f.eks. Morganroth & Bigger, side 1497.
Pasienter med livstruende arytmier kan få synkope (plutselig tap av bevissthet - vanligvis besvimelse - forbundet med utilstrekkelig hjerneperfusjon), kardial stans, hjertesvikt og/eller myokardial iskemi, i nærvær av strukturell hjertesykdom. Livstruende arytmier er relativt uvanlige; sannsynligvis mindre enn 10% av individene som lider av arytmier, lider av en livstruende form. Se Morganroth & Bigger, side 1497. Men på grunn av de dødelige ventrikulære arytmiers livstruende natur og symptomenes alvor, må de behandles aggressivt.
De nye forbindelser i henhold til den foreliggende oppfinnelse, er virksomme mot kardial fibrillasjon og supraventrikulære og ventrikulære arytmier. I tillegg har de nye forbindelser i henhold til foreliggende oppfinnelse, færre bivirkninger, hvilket er blitt tolerert i tradisjonell antiarytmisk terapi av mangel på akseptable alternative terapier. For eksempel forårsaker mange nåværende terapier pulmonær toksisitet, kardial depresjon og nevrologiske virkninger som ikke er spesifikke for kardialt vev. I tillegg er de nye forbindelser i henhold til foreliggende oppfinnelse lett biotilgjengelige. Dette underletter behandling ved oral administrasjon, og de underletter derfor tilgjengeligheten for pasienten. I tillegg er de nye forbindelser i henhold til foreliggende oppfinnelse relativt billige å fremstille, og de har en høy stabilitetsgrad i orale doseringsformer.
Oppsummering av oppfinnelsen
De nye 4-oksocykliske urinstoffer i henhold til foreliggende oppfinnelse, og de farmasøytisk akseptable salter og biohydrolyserbare estere derav, og de har følgende generelle struktur:
hvor (a) X er fenyl; (b) R er C=0; (c) Y er 1,3-fenylen substituert med ett eller to halo-genatomer; (<d>) R1# R2 og R3 er hydrogen; (e) L er -CH=N-; (f) R4 er en alkylenkjede med 1-8 karbonatomer; (g) A er valgt fra di (C-L-Cg) alkylamino eller 4-metylpi-perazino;
(h) R5 er en C<H>2<->gruppe.

De nye 4-oksocykliske urinstoffer i henhold til foreliggende oppfinnelse består av en 4-oksocyklisk urinstoffgruppe forbundet til et ringsystem (X-R-Y) via en forbindende gruppe (L). De 4-oksocykliske urinstoffer har et nitrogenatom i 3-stilling, hvilket er substituert med en amino-holdig gruppe (A) bestående av en aminogruppe separert fra nitrogenatomet i 3-stilling på den 4-oksocykliske urinstoffgruppe ved en avstandsholdende gruppe (R4) .
Den 4- oksocvkliske urinstoffgruppe
Den 4-oksocykliske urinstoffgruppe i de nye forbindelser ifølge foreliggende oppfinnelse gir de nye forbindelser ifølge foreliggende oppfinnelse deres karakteristiske navn. Den 4-oksocykliske urinstoffgruppe er en 5-leddet ring. Den 4-oksocykliske urinstoffgruppe er forbundet med resten av molekyllet via nitrogenatomet i 1-stilling på den 4-oksocykliske urinstoffgruppe. Den 4-oksocykliske urinstoffgruppe har følgende struktur:

Definisjoner og anvendelse av betegnelser
Følgende er en liste over definisjoner av uttrykk som er anvendt her. 11 Ct-C8-alkyl11 er en rettkjedet eller forgrenet, mettet hydrokarbonkjede med 1-4 karbonatomer. Foretrukne alkyl-gruppe innbefatter, men er ikke begrenset zi. 1, metyl, etyl, propyl, isopropyl og butyl.
"Halo", "halogen" eller "halid" er et klor-, brom-, fluor-, eller jodatomradikal. Klor, brom og fluor er foretrukne halogenider.
Et "farmasøytisk akseptabelt" salt er et kationisk salt dannet av enhver sur (f.eks. karboksyl) gruppe eller et anionisk salt dannet av enhver basisk (f.eks. amino) gruppe. Mange slike salter er kjent i faget, som beskrevet i World Patent Publication 87/05297, Johnscon et al., publisert 11. september 1987. Foretrukne kationiske salter omfatter alkalimetallsaltene (såsom natrium og kalium) og jordalkalimetallsalter (såsom magnesium og kalsium). Foretrukne anioniske salter omfatter halogensaltene (såsom klorid).
En "biohydrolyserbar ester" er en ester av de 4-oksocykliske urinstoff-forbindelser som ikke innvirker på den antiarytmiske aktivitet av forbindelsene, eller som lett metaboliseres av et menneske eller annet pattedyr og gir et antiarytmisk aktivt 4-oksocyklisk urinstoff. Mange slike estere er kjent i faget, som beskrevet i World Patent Publication 87/05297, Johnston et al., publisert 11. september 1987. Slike estere omfatter lavere alkylestere, lavere acyloksyalkylestere (såsom acetoksylmetyl-, acetoksyetyl-, aminokarbonyloksymetyl-, pivaloyloksymetyl-og pivaloyloksyetylestere), laktonylestere (såsom ftalidyl-og tioftalidylestere) , lavere alkoksyacyloksyalkylestere (såsom metoksykarbonyloksymetyl-, etoksykarbonyloksyetyl-og isopropoksykarbonyloksyetylestere), alkoksyalkylestere, kolinestere og acylaminoalkylestere (såsom acetamidometyl-estere).
Foreliggende oppfinnelse vedrører visse nye 4-oksocykliske urinstoffer og farmasøytiske sammensetninger derav. De
nevnte nye forbindelser og sammensetninger derav kan anvendes for å behandle kardiale arytmier og/eller kardial fibrillasjon i mennesker eller andre pattedyr. Spesifikke forbindelser og sammensetninger som kan anvendes i oppfinnelsen, må følgelig være farmasøytisk akseptable. Anvendt her er en slik "farmasøytisk akseptabel" komponent en
komponent som er egnet for anvendelse på mennesker og/eller andre pattedyr uten utilbørlige bivirkninger (såsom toksisitet, irritasjon og allergisk reaksjon) cg har et rimelig forhold mellom fordel og risiko.

Farmasøytiske sammensetninger inneholdende nve 4- oksocvkliske urinstoffer
De nye 4-oksocykliske urinstoffer ifølge foreliggende oppfinnelse kan administreres til mennesker eller andre patte dyr ved forskjellige ruter, inklusive, men ikke begrenset til, orale doseringsformer og injeksjoner (intravenøst, intramuskulært, intraperitonealt eller subkutant). Forskjellige andre doseringsformer inneholdende de nye 4-oksocykliske urinstoffer ifølge foreliggende oppfinnelse kan lett formuleres av en fagperson under utnyttelse av de egnede farmasøytiske eksipienter definert nedenfor. Når det gjelder pasienttoleransen, er orale doseringsformer normalt mest foretrukket.
Uttrykket "farmasøytisk sammensetning" anvendt her, betyr en sammensetning bestående av en sikker og effektiv mengde av det 4-oksocykliske urinstoff som aktiv ingrediens, eller blandinger derav, og farmasøytisk akseptable eksipienter.
Uttrykket "sikker og effektiv mengde", anvendt her, betyr en mengde av en forbindelse eller sammensetning som er stor nok til å signifikant og positivt modifisere symptomene og/eller tilstanden som skal behandles, men liten nok for å unngå alvorlige bivirkninger (i et rimelig forhold mellom fordel/risiko) innenfor området av sunn medisinsk bedøm-melse. Den sikre og effektive mengde av den aktive bestanddel for anvendelse i en farmasøytisk sammensetning eller i metoden ifølge oppfinnelsen her, vil variere med den spesielle tilstand som behandles, alderen og den fysiske tilstand hos pasienten som behandles, tilstandens alvor, behandlingens varighet, naturen av samtidig terapi, den spesielle aktive bestanddel som anvendes, den spesielle, farmasøytisk akseptable eksipient som anvendes og lignende faktorer innenfor den behandlende leges kunnskap og eksper-tise .
Uttrykket "farmasøytisk akseptabel eksipient", anvendt her, omfatter ethvert fysiologisk inert, farmakologisk inaktivt materiale som er kjent av en fagperson, hvilket materiale er kompatibelt med de fysikalske og kjemiske egenskaper hos den spesielle aktive, 4-oksocykliske urinstoffingrediens som velges for anvendelsen. Farmasøytisk akseptable eksipienter innbefatter, men er ikke begrenset til, polymerer, harpikser, mykgjørere, fyllstoffer, bindemidler, smøremid-ler, glidemidler, disintegrasjonsmidler, oppløsningsmidler, kooppløsningsmidler, buffersystemer, overflateaktive midler, konserveringsmidler, søtningsstoffer, aromastoffer, farmasøytiske fargestoffer eller pigmenter og viskositetsmidler.
Uttrykket "oral doseringsform" anvendt her, betyr enhver farmasøytisk sammensetning som er beregnet på å bli systemisk administrert til et individ ved å tilføre nevnte sammensetning til den gastrointestinale trakt hos et individ, via munnen på nevnte individ. Vedrørende foreliggende oppfinnelse kan den tilførte form være i form av en tab-lett, belagt eller ikke belagt; oppløsning; suspensjon; eller en kapsel, belagt eller ikke-belagt.
Uttrykket "injeksjon" anvendt her, betyr enhver farmasøy-tisk sammensetning som er beregnet på systemisk administrasjon til et menneske eller et annet pattedyr, via tilførsel av en løsning eller emulsjon inneholdende den aktive bestanddel, ved å stikke hull på huden på nevnte individ for å tilføre nevnte løsning eller emulsjon til individets sirkulasjonssystem ved intravenøs, intramuskulær, subkutan eller intraperitoneal injeksjon.
Hastigheten for systemisk tilførsel kan kontrolleres til-fredsstillende av en fagperson ved å regulere én eller flere av følgende faktorer: (a) den egnede aktive bestanddel; (b) de farmasøytisk aktive eksipienter; forutsatt at variasjonene ikke innvirker på den spesielle valgte aktive bestanddels aktivitet; (c) typen av eksipient og den medfølgende ønskelige tyk-kelse og permeabilitet (svelleegenskaper) hos nevnte belegg; (d) de tidsavhengige betingelser for eksipienten i seg selv og/eller i eksipiensene; (e) partikkelstørrelsen på den granulerte aktive bestanddel; og
(f) eksipentenes pH-avhengige betingelser.
Spesielt kan løseligheten, surhetsgraden cg mottakeligheten for hydrolyse hos de forskjellige 4-oksocykliske aktive urinstoffbestanddeler, såsom syreaddisjonssalter, salter dannet med karboksylgruppen, f.eks. alkalimetallsalter, jordalkalimetallsalter, etc, og estere, f.eks. alkyl, alkenyl, aryl, arylalkyl, anvendes som retningslinjer for et riktig valg. I tillegg kan egnede pH-becingelser fast-settes innenfor de orale doseringsformer ved å tilsette en egnet buffer til den aktive bestanddel i henhold til det ønskede frigivelsesmønster.
Som angitt ovenfor, omfatter farmasøytisk akseptable eksipienter, men de er ikke begrenset dertil, narpikser, fyllstoffer, bindemidler, smøremidler, oppløsningsmidler, glidemidler, oppmalingsmidler, kooppløsningsmidler, overflateaktive midler, konserveringsmidler, søtningsstoffer, aromastoffer, buffersysterner, fargestoffer eller pigmenter av farmasøytisk grad og viskositetsmidler.
Det foretrukne oppløsningsmiddel er vann.
Aromastoffer som kan være nyttige her, omfatter de stoffer som er beskrevet i Remington's Pharmaceutical Sciences, 18. utgave, Mack Publishing Company, 1990, sidene 1288-1300. De farmasøytiske sammensetninger som er egnet for bruk, inneholder normalt fra 0 til 2% aromastoff.
Fargestoffer eller pigmenter som er nyttige, innbefatter de som er beskrevet i Handbook of Pharmaceutical Excipients, sidene 81-90, 1986 av the American Pharmaceutical Association & the Pharmaceutical Society of Great Britain. De farmasøytiske sammensetninger inneholder normalt fra 0 til 2% fargestoffer eller pigmenter.
Foretrukne kooppløsningsmidler innbefatter, men er ikke begrenset til, etanol, glyserin, propylenglykol, poly-etylenglykoler. De farmasøytiske sammensetninger ifølge foreliggende oppfinnelse innbefatter fra ca. 0 til 50% kooppløsningsmiddel.
Foretrukne buffersystemer innbefatter, men ikke begrenset til, eddiksyre, borsyre, karbonsyre, fosforsyre, ravsyre, maleinsyre, vinsyre, sitronsyre, benzoesyre, melkesyre, glyserinsyre, glukonsyre, glutarsyre og glutaminsyre og deres natrium-, kalium- og ammoniumsalter. Spesielt foretrukket er fosforsyre, vinsyre, sitronsyre og eddiksyre og deres salter. De farmasøytiske sammensetninger ifølge foreliggende oppfinnelse inneholder vanligvis fra 0 til 5% buffersystemer.
Foretrukne overflateaktive midler innbefatter, men er ikke begrenset til, polyoksyetylensorbitanfettsyreestere, poly-oksyetylenmonoalkyletere, sukrosemonoestere og lanolin-estere og etere, alkylsulfatsalter, natrium-, kalium- og ammoniumsalter av fettsyrer. Den farmasøytiske sammensetning ifølge foreliggende oppfinnelse omfatter 0 til 2% overflateaktivt middel.
Foretrukne konserveringsmidler innbefatter, men er ikke begrenset til, fenol, alkylestere av parahydroksybenzoe-syre, o-fenylfenolbenzoesyre og saltene derav, borsyre og saltene derav, sorbinsyre og saltene derav, klorbutanol, benzylalkohol, timerosal, fenylkvikksølvacetat og -nitrat, nitromersol, benzalkoniumklorid, cetylpyridiniumklorid, metylparaben og propylparaben. Spesielt foretrukket er saltene av benzoesyre, cetylpyridiniumklorid, metylparaben og propylparaben. Sammensetningene ifølge foreliggende oppfinnelse innbefatter normalt fra 0 til 2% konserverings-middel .
Foretrukne søtningsstoffer innbefatter, men er ikke begrenset til, sukrose, glukose, sakkarin, sorbitol, mannitol og aspartan. Spesielt foretrukket er sukrose og sakkarin. Farmasøytiske sammensetninger ifølge foreliggende oppfinnelse innbefatter 0 til 5% søtningsstoff.
Foretrukne viskositetsmidler innbefatter, men er ikke begrenset til, metylcellulose, natriumkarboksymetylcellu-lose, hydroksypropylmetylcellulose, hydroksypropylcellulose, natriumalginat, karbomer, povidon, akasie, guargummi, xantangummi og tragant. Spesielt foretrukket er metylcellulose, karbomer, xantangummi, guargummi, povidon, natrium-karboksymetylcellulose og magnesiumaluminiumsilikat. Sammensetninger ifølge foreliggende oppfinnelse omfatter 0 til 5% viskositetsmiddel.
Foretrukne fyllstoffer innbefatter, men er ikke begrenset til, laktose, mannitol, sorbitol, treverdig kalsiumfosfat, toverdig kalsiumfosfat, kompresjonssukker, stivelse, kalsiumsulfat, dekstro- og mikrokrystallinsk cellulose. Sammensetningene ifølge foreliggende oppfinnelse inneholder fra 0 til 75% fyllstoff.
Foretrukne smøremidler innbefatter, men er ikke begrenset til, magnesiumstearat, stearinsyre og talkum. De farmasøy-tiske sammensetninger ifølge foreliggende oppfinnelse innbefatter 0,5 til 2% smøremiddel.
Foretrukne glidemidler innbefatter, men er ikke begrenset til, talkum og kolloid silisiumdioksid. Sammensetningene ifølge foreliggende oppfinnelse innbefatter fra 1 til 5% glidemiddel.
Foretrukne pulveriseringsmidler omfatter, men er ikke begrenset til, stivelse, natriumstivelseglykolat, kros-povidon, kroskarmelosenatrium og mikrokrystallinsk cellulose. De farmasøytiske sammensetninger ifølge foreliggende oppfinnelse innbefatter fra 4 til 15% pulveriseringsmiddel.
Foretrukne bindemidler innbefatter, men er ikke begrenset til, akasie, tragant, hydroksypropylcellulose, pregelatini-sert stivelse, gelatin, povidon, hydroksypropylcellulose, hydroksypropylmetylcellulose, metylcellulose, sukkeroppløs-ninger, såsom sukrose og sorbitol, og etylcellulose. Sammensetningene ifølge foreliggende oppfinnelse innbefatter 1 til 10% bindemiddel.
Ifølge dette innbefatter de farmasøytiske sammensetninger ifølge foreliggende oppfinnelse fra 15 til 95% av en aktiv 4-oksocyklisk urinstoff-forbindelse eller en blanding derav; 0-2% aromastoff; 0-50% kooppløsningsmiddel; 0-5% buffersystem; 0-2% overflateaktivt middel; 0-2% konser-veringsmiddel; 0-5% søtningsstoff; 0-5% viskositetsmiddel; 0-75% fyllstoff; 0,5-2% smøremiddel; 1-5% glidemiddel; 4-15% pulveriseringsmiddel; og 1-10% bindemiddel.
Fremgangsmåte for behandling av arytmier med de nye 4- oksocykliske urinstoffer
De nye forbindelser ifølge foreliggende oppfinnelse er virksomme ved behandling av mennesker eller andre pattedyr befenget med supraventrikulære arytmier og ventrikulære arytmier og/eller kardial fibrillasjon. Som angitt ovenfor, unntatt i sjeldne tilfeller, er supraventrikulære arytmier ikke ansett å være livstruende, og de behandles vanligvis ikke aggressivt med konvensjonelle antiarytmiske midler på grunn av deres uønskede bivirkninger. Følgelig behandles denne type arytmi ikke aggressivt for bare å underlette symptomer som karakteriseres milde til alvorlige. Imidlertid tolereres de nye forbindelser ifølge foreliggende oppfinnelse vanligvis meget godt og de har færre av de uønskede bivirkninger som mange av de mer konvensjonelle antiarytmiske medisiner har, og følgelig kan de godt utgjøre en akseptabel terapi for å underlette symptomene hos individer som lider av supraventrikulære arytmier, og som faktisk opplever ubehag, selv om de ikke befinner seg i en livstruende situasjon.
Som angitt ovenfor er de nye 4-oksocykliske urinstoffer ifølge foreliggende oppfinnelse også effektive ved behandling av ventrikulære arytmier som vanligvis er meget alvorligere enn supraventrikulære arytmier, og følgelig krever aggressiv terapi. På grunn av det potensielle alvor ved noen ventrikulære arytmier har det oppstått mange typer av pasientklassifikasjoner.
Individer som lider av godartede ventrikulære arytmier, ligner, fra et filosofisk standpunkt vedrørende om de trenger behandling, de individer som opplever supraventrikulære arytmier. Disse individer har ikke hjertesykdom og kan oppleve svimmelhet og hjertebank, og de lider ofte av en viss følelsesmessig bekymring som stammer fra uvissheten som forårsakes av deres fysiske symptomer. Disse individer lider vanligvis av PVC-tilfeller som normalt er fysikalsk harmløse, men forståelig nok er de opphav til en viss grad av engstelse. De nye 4-oksocykliske urinstoffer ifølge foreliggende oppfinnelse har vanligvis færre av de uønskede bivirkninger som kan ha gjort bruken av mange konvensjonelle antiarytmiske medisiner, som hittil har vært reser-vert for alvorligere og/eller livstruende sykdomstilstan-der, uønsket for disse individer. Imidlertid ville disse individer sannsynligvis ha fordel av terapi som generelt tolereres bedre.
En annen klasse av individer som kan ha fordel av terapi med de nye 4-oksocykliske urinstoffer ifølge foreliggende
oppfinnelse, er de individer som karakteriseres som lidende av "prognostisk signifikante" arytmier. Disse individer har vanligvis hatt et myokardialt infarkt og kan ha PVC-tilfeller og/eller episoder med ikke-undertrykket ventrikulær
takyarytmi, enten symptomatisk eller asymptomatisk. De har ikke samme grad av plutselige, presserende livstruende symptomer som de individer som er beskrevet nedenfor, og de er ikke, ved konvensjonell karakterisering, i fare for øyeblikkelig død eller nesten-død. Det finnes imidlertid en signifikant større risiko for hjertesvikt hos dem enn hos den vanlige befolkning, og risikoen for plutselig død ville følgelig være mindre med terapi med de nye forbindelser ifølge foreliggende oppfinnelse. Det finnes andre individer som kontinuerlig opplever livstruende arytmier og er i fare for å dø plutselig. Disse individer opplever vanligvis tilbakeholdt ventrikulær takyarytmi eller ventrikulær fibrillasjon. De ventrikulære arytmier hos disse individer frembringer vanligvis hemo-dynamisk signifikante tegn eller symptomer, såsom synkope, hjertesvikt, myokardial iskemi eller hypotensjon. Disse pasienter har den høyeste risiko for plutselig kardial død og vanligvis den alvorligste form av underliggende kardial sykdom. Se Morganroth & Bigger, side 1498. De nye forbin-deiser ifølge foreliggende oppfinnelse er effektive, aggressive antiarytmiske terapeutiske midler som egner seg for anvendelse i denne klasse av individer, men de har færre av noen av de uønskede bivirkninger som hittil vanligvis ble tolerert med konvensjonelle antiarytmi-behand-linger, fordi det var nødvendig og det ikke fantes egnede alternativer for å behandle de livstruende arytmier.
Som angitt ovenfor, har de nye antiarytmiske midler ifølge foreliggende oppfinnelse færre av de mange uønskede bivirkninger forbundet med mange konvensjonelle antiarytmiske terapeutiske midler. Disse bivirkninger ombefatter, men er ikke begrenset til, perioder med hjerteeksitasjon under hvert hjerteslag. Konvensjonelle terapeutiske midler har anestetiske og/eller kardiale depressive egenskaper som bare gjør hjertet mindre responsivt, ikke mindre fibrilla-torisk.
Følgelig er de nye 4-oksocykliske urinstoffer ifølge foreliggende oppfinnelse nyttige ved behandling av kardiale arytmier og/eller kardial fibrillasjon i mennesker eller andre pattedyr. For behandling av et menneske eller et annet pattedyr som lider av kardial arytmi og/eller kardial fibrillasjon, kan det administreres til nevnte menneske eller annet pattedyr en sikker og effektiv mengde av en farmasøytisk sammensetning omfattende fra 15 til 90% av et 4-oksocyklisk urinstoff som aktiv bestanddel eller blandinger derav, og fra 10 til 85% farmasøytisk akseptable eksipienter.

♦Arytmiskåring er en sammensatt skåring som redegjør for alvorligheten av arytmien og tiden til første hendelse i en rottemodell hvor arytmi induseres ved koronararterieligering etterfulgt av reperfusjon. Dosering ble gjort via den intrave-nøse rute ved 10 mg/kg med mindre annet er indikert i tabellen. En verdi < 40 ved en 10 mg/kg dose indikerer signifikant aktivitet. Claims (3)
1. 4-oksocykliske urinstoffer og de farmasøytisk akseptable salter og biohydrolyserbare estere derav, karakterisert ved at de har den generelle struktur:
hvor (a) X er fenyl; (b) R er C=0; (c) Y er 1,3-fenylen substituert med ett eller to halo-genatomer; (d) Rx, R2 og R3 er hydrogen; (e) L er -CH=N-; (f) R4 er en alkylenkjede med 1-8 karbonatomer; (g) A er valgt fra di (Cx-C8) alkylamino eller 4-metylpi-perazino; (h) R5 er en CH2-gruppe.
2. Farmasøytisk sammensetning som er nyttig ved behandling av arytmi og fibrillasjon, karakterisert ved at den består av en sikker og effektiv mengde på fra 15 til 90% av et 4-oksocyklisk urinstoffer ifølge krav 1, eller blandinger derav, og fra 10 til 85% farmasøytisk akseptable eksipienter, valgt fra gruppen bestående av 0-2% aromastoffer; 0-50% koløsningsmidler; 0-5% buffersystem; 0-2% surfaktanter; 0-2% konserveringsmidler; 0-5% søtningsstoffer; 0-5% viskositetsmidler; 0-75% fyllstoffer; 0,5-2% smøremidler; 1-5% glidemidler; 4-15% disintegranter; og 1-10% bindemidler.
3. Farmasøytisk sammensetning i henhold til krav 2, karakterisert ved at de farmasøytisk akseptable eksipienter er valgt fra gruppen bestående av polymerer, harpikser, mykgjørere, fyllstoffer, bindemidler, smøremidler, glidemidler, disintegranter, løsningsmidler, koløsningsmidler, buffersystemer, surfaktanter, konserveringsmidler, søtningsstoffer, aromastoffer, fargestoffer eller pigmenter av farmasøytisk grad og viskositetsmidler.
