Description
Foreliggende oppfinnelse angår en analogifremgangsmåte ved fremstilling av alkali- og/eller jordalkalimetallsalter av L-ot-glyserof osf oryl-D-myo-inositol (heretter kalt GFI) av formel 1, hvor GFI fremstilles/isoleres på i og for seg kjent måte, hvor fremgangsmåten er særpreget ved at GFI deretter omsettes med en base av et alkali- og/eller jordalkalimetall.
Fra et kjemisk synspunkt er GFI strukturelt lik fosfatidyl-inositol (heretter kalt Fl), idet Fl er det dobbeltacylerte produkt med fettsyrer, i hovedsak umettede syrer, ved hydroksygruppene av glyserinresidueet av glyserofosforyl-inositol.
Fl er et molekyl av naturlig opprinnelse som er nesten uoppløselig i vann, og som generelt kan ekstraheres fra bovin hjerne og/eller soyabønner i blanding med fosfatidyletanola-min ("cefalinisk fraksjon") og deretter renset.
Fl viser seg å være relativt ustabil siden de umettede fettsyrekjeder bundet til hydroksygruppene av glyserin-residuet lett undergår peroksyderingsreaksjoner som gir et antall dekomponeringsprodukter.
I motsetning til dette er GFI den deacylerte analog og den er følgelig vannoppløselg som sådan eller som salt, den er stabil og alkali- og jordalkalimetallsaltene derav, spesielt natrium-, kalium-, kalsium- og magnesiumsaltene er krystallinske og spesielt egnet for bruk i farmasøytiske formuleringer.
Fordelene som oppstår ved bruk av slike salter, spesielt kalsiumsaltet, sammenlignet med GFI i fri syreform, omfatter lavere hygroskopisitet, en høyere stabilitet, en bedre mulighet for tilpasning til bruken derav i farmasøytiske blandinger, siden saltene i seg selv, idet de er dårlig hygroskopiske, kan oppbevares i lang tid uten nevneverdig nedbrytning.
GFI eller saltene derav med alkali og/eller jordalkalimetaller, spesielt kalsiumsaltet, blir erholdt ved kon-trollert forsåpning av den acylerte fosfolipidblanding inneholdt i soyabønne, med etterfølgende separasjon med opp-rensning av den erholdte GFI i fri form og eventuelt saltdan-nelse med alkali og/eller jordalkalimetaller.
En kjent fremgangsmåte for fremstilling av GFI er beskrevet i EP-A-0.217.765.
Fra et biokjemisk synspunkt er Fl-katabolisme kjent for å spille en meget viktig rolle i de biokjemiske hendelser som er forbundet med fysiologisk aktivitet i den grad det gjelder metabolisering av fosfor (Ansell G.B. og Dohmen H. ; J. Neurochem. 2, 1, 1957; Sheltaway A. og Dawson R.M.C.; Neurochem. J., 111, 157, 1969).
Når kalsium er tilgjengelig som et støttemateriale for aktiviteten av fosfatidylinositolfosfodiesterase (fosfolipase C) , er dette enzym sannsynligvis det som er hovedansvarlig for Fl-nedbrytning (Friedel R.O., Brown J.D. og Durrell J.; Biochim. biophys. Acta., 144, 684, 1967; Keough K.M.W. og Thompson W.; Biochim. biophys. Acta., 270, 324, 1972; Thompson W.; Can. J. Biochem., 45, 853, 1967; Dawson R.M.C. et al.; Biochem. J., 122, 605, 1971; Irvine R.F.; Biochem. J., 176, 475, 1978), hvorav hovedmetabolittene er diacylgly-serol og inositolfosfat; hvor hele den metabolske cyklus

kan bli representert som følger:
Selv om en dårlig acylering av Fl kan inntreffe under spesielle fysiologiske betingelser (lav Ca<++->konsentrasjon), hvilken acylering blir katalysert av enzymet fosfolipase Ax og gir opphav til lysofosfatidylinositol (Hong S.L. og Deykin D.; J. Biol. Chem., 256, 5215, 1981), gjør spesifisiteten av fosfatidylinositolfosfodiesterase, den allmenne fordeling i animalske celler og den ekstremt høye aktiviteten derav, som også forekommer in vitro (Friedel R.O., Brown J.D. og Durrell J.; Biochim, biophys. Acta., 144, 684, 1967; Keough K.M.W. og Thompson W.; Biochim. biophys. Acta., 270, 324, 1972; Thompson W.; Can. J. Biochem., 45, 853, 1967; Dawson R.M.C. et al.; Biochem. J., 122, 605, 1971; Irvine R.F.; Biochem., J., 176. 475, 1978), dette enzym til den hovedansvarlige for fosfoinositidisk metabolisme.
Det har nå blitt funnet at GFI har en markert aktivitet i perifere nervelidelser av dysmetabolsk eller toksisk opprinnelse og i hjernelidelser av organisk og funksjonell opprinnelse .
Resultatene fra farmakologiske forsøk er angitt nedenfor som eksempler.
EKSEMPEL 1
Aktivitet av GFI i den enteriske autonome neuropati med opphav i diabetes
De neurologiske korrelater av diabetes har blitt klart beskrevet som funksjonelle og morfologiske endringer av både sensoriske og motoriske perifere nerver.
Disse abnormaliteter blir understreket ved vesentlige endringer av aksonal transport med påfølgende aksonal atrofi som blir fulgt av degenerasjon.
Den autonome neuropati hos diabetes er assosiert med gastrointestinale problemer såsom diaré ved diabetes, forstoppelse, redusert gjennomgang i tynntarmen og megakolon, og med endret følsomhet i innvollene.
Alloxan-indusert eksperimentell diabetes viser en bemerkel-sesverdig korrelasjon mellom neuronale og gastrointestinale endringer.
Substans P, met-enkefalin og VIP er inneholdt i enteriske neuroner med forskjellige oppgaver og forskjellige aktivi-teter. Substans P og VIP axoner strekker seg ut oralt, mens enkefalinneuroner strekker seg ut analt. Substans P gir direkte eksitering av enteriske neuroner og glatt muskulatur, mens enkefalin gir hemming av enteriske neuroner og muskel-sammentreking ved en direkte virkning, selv om en indirekte effekt via acetylcholinstimulering ikke kan være utelukket. VIP har relakserende aktivitet på glatt muskulatur. De gastrointestinale funksjoner blir sannsynligvis regulert av det fine samspill blant disse nervekomponenter. I den autonome neuropati i tarmen ved diabetes er slik koordinert aktivitet endret og ødelegger således de gastrointestinale funksjoner.
Forsøket ble utført på Sprague Dawley hann-rotter (med vekt omkring 250 g) oppdelt i 3 grupper: en kontrollgruppe (C) , en diabetesgruppe (D) og en behandlet diabetesgruppe (DDTRp). Diabetes ble indusert ved enkel subkutan injeksjon av 100 mg/kg alloxan (glycemi >400 mg/dl) og ved å starte fra den 7. dag etter diabetesinduksjon ble behandling utført subkutant med 10 mg/kg i (GFI)2Ca i tre måneder.
De følgende parametre ble målt: kroppsvekt, blodglukose, met-enkefalin, substans P og VIP.
Alle disse parametere som blir i hovedsak påvirket i diabetesgruppen ble brakt tilbake i den behandlede diabetesgruppe og viste seg å være lik de hos den respektive kontrollgruppe med statistisk signifikante variasjoner.
Tabell 1 og 2 viser met-enkefalin og substans P-verdiene som ble målt i løpet av forsøket.
Ved siden av dette viser alle rotter i diabetesgruppen D en typisk cataract: det krystallinske materialet var meget sprøtt og smuldret opp under disseksjon. I motsetning til dette var det krystallinske materialet av diabetesgruppe DDTRp klart og normalt motstandsdyktig overfor disseksjons-prosedyre.
EKSEMPEL 2
Aktivitet av GFI i tidlig enterisk autonom neuropati av diabetesopprinnelse
Rotter med eksperimentelt indusert diabetes ble behandlet i henhold til de ovenfor gitte angivelser (se EKSEMPEL 1) for å beregne den gastroenteriske beskyttelse på det tidspunkt hvor den neuropatiske skade er etablert.
Behandlingen ble utført i.p. med 10 mg/kg (GFI)2Ca i 35 dager, ved å starte fra den 7. dagen etter diabetesinduksjon. De observerte parametere (kroppsvekt, blodglukose og met-enkefalin) ble reetablert i den behandlede diabetesgruppe DDTRp på lignende måte som beskrevet i EKSEMPEL 1.
EKSEMPEL 3
Aktivitet av reseptor og andre meddelersubstans i perifere organer i eksperimentell diabetesneuropati
Dyrene, behandlingen, gruppene og doseringene er lik dem angitt i EKSEMPEL 1, mens behandlingsdagene er 35 som i
EKSEMPEL 2.
Den målte parameter var insulinreseptor i lever, ved binding av 12<5>l-insulin, analysert ved metoden til Scatchard (G. Scatchard; Ann. N.Y. Acad. Sei.; 51, 660, 1970), ved å bruke rensede hepatiske plasmamembraner, fremstilt ved metoden til Ray (T.K. Ray; Biochem. Biophys. Acta; 196, 190).
Antallet høyaffinintetsreseptorbindingsseter for <125>I-insulin (Bmaxi) i lever er øket i rotter med diabetes. Behandlingen med (GFI)2Ca forhindrer spesielt denne økning i rotter med diabetes.
EKSEMPEL 4
Anti- amnesisk aktivitet av GFI ved å bruke en ett- forsøks passiv unnaåelsestest med elektro- konvulsivt siokk som amnesisk middel
Denne passive unngåelsestest ble utført ved å bruke en vanlig apparatur bestående av et kammer med en gitterbunn og en forhøyet rampe som stikker ut fra forveggen i kammeret. Rampen er belyst mens kammeret er mørkt. Når plassert på rampen, kan rotten komme inn i det mørke kammer gjennom en åpning. Et unngåelsesfotsjokk kan bli avlevert gjennom nettingbunnen i det mørke rommet. Forsøket ble delt opp i løpet av 3 dager : på første dag ble hver rotte underkastet den vanlige opplæringstrening. På andre dag ble den behandlede gruppe tilført oralt (GFI)2Ca ved tre dosenivåer: 3-30-100 mg/kg, og etter 60 min ble trening utført ved hjelp av både fotsjokk som et ytterligere opplæringsmiddel og elek-trosjokk som amnesisk middel. På tredje dag ble opplæring gjentatt under måling av latenstiden som er nødvendig for rotten for å gå inn i rommet.
Forsøket ble utført i sammenligning med Piracetam, tilført ved doser på 3 00-1000 mg/kg pr. os og signifikans av resultatene i favør av GFI ble vist ved å sammenligne dosene på 30-100 mg/kg GFI med dem på 300-1000 mg/kg Piracetam.
EKSEMPEL 5
Aktivitet av GFI på nerveledningshastiqhet
Fischer F-344 hannrotter med en opprinnelig vekt på 271 g ble brukt. Diabetes ble indusert med 70 mg/kg streptozocin, og for å forhindre en ukontrollert forverring av den gene- reile fysiologiske tilstand hos dyrene med diabetes, ble de daglig tilført s.c. injeksjoner på 0,75 U insulin. Dyrene ble oppdelt i tre grupper, C, D og DDTRp (se eksempler 1, 2, 3).
Behandlingen ble utført med (GFI)2Ca 10 mg/kg i.p. i 8 uker, ved å starte fra første dag av diabetes. Den målte parameter var halenerveledningshastighten (CV) som angitt av Spiller et al. (M. Spiiler, W. Dimpfel, H.-U. Tiillner; Arch. Int. Pharmacodyn., 287, 211, 1987). Spesielt ble det beregnet: 1. CV av den proksimale del av den motoriske nerve 2. CV av den proksimale del av den sensoriske nerve
3. CV av den distale del av den sensoriske nerve
CV ble målt før induksjon av diabetes (3 måneder gamle rotter) og etter 8 ukers behandling. Den minket i diabetesgruppe D, noe som bekreftet utviklingen av en perifer diabetesneuropati, mens, som angitt i tabell 3, i DDTRp-gruppen ble den statistisk øket, noe som antyder den terapeutiske effekt av (GFI)2Ca i diabetesneuropati. Det er kjent (D.A. Green, P.V. de Jesus og A.I. Winegrad; J. Clin. Invest., 55, 1326, 1975) at denne typen forsøk ble positivt utført i tidligere behandling av streptozotocin-induserte rotter med diabetes med 650 mg/kg myo-inositol, men dette resultat ble ikke bekreftet av senere forsøk, selv om dosene av myo-inositol alltid var meget høye (J.G. Salway, J.A. Finnegan, D. Barnétt, L. Whitehead, A. Karunanayaka og R.B. Payne; 2, 1282, 1978). Av denne grunn sviktet den mulige terapeutiske bruk av myo-inositol.
Slik det fremgår av de ovenfor angitte resultater, kan GFI passende bli brukt som den aktive bestanddel i farmasøy-tiske blandinger for behandling av perifere nerve sykdommer og av hjernelidelser av organisk eller funksjonell opprinnelse såsom vaskulopatier, Alzheimers lidelse, involutive syndromer hos eldre mennesker og lignende lidelser.
Eksempler på farmasøytiske blandinger som er egnet for oral tilførsel omfatter kapsler, myke kapsler, tabletter, granu-later, pulvere, oppløsninger, poser, former med vedvarende frigjøring inneholdende 10 til 500 mg GFI (som sådan eller som et alkali- eller jordalkalimetallsalt, fortrinnsvis som kalsiumsaltet), pr. enhetsdose som skal tilføres 2-3 ganger om dagen i henhold til diagnosen og pasientens tilstand.
For parenteral tilførsel, både intravenøs og intramuskulær, er passende former frysetørkede ampuller eller sterile oppløsninger inneholdende 2 - 250 mg GFI (som sådan eller som et alkali- eller jordalkalimetallsalt, fortrinnsvis som kalsiumsaltet), pr. enhetsdose, som skal tilføres 1 til 3 ganger pr. dag.
De fysiokjemiske særtrekk av GFI og av kalsiumsaltet derav brukt i de ovenfor beskrevne farmakologiske forsøk og fremstilt i henhold til ovenfor angitte vanlige fremgangsmåter er vist nedenfor som eksempler.

karakterisert ved at GFI etterfølgende omsettes med en base av et alkali- og/eller jordalkalimetall.
2. Analogifremgangsmåte ifølge krav 1, karakterisert ved at det benyttes basisk kalsium ved omsetningen.
