Description
Foreliggende oppfinnelse vedrører en kirurgisk artikkel med knutesikkerhet og forbedret knuteomplasseringsegenskaper, hvilken artikkel omfatter en tråd, idet tråden har et bioabsorberbart belegg, og belegget omfatter en statistisk kopolymer.
Det biologisk absorberbare belegg ifølge denne oppfinnelsen har fordeler fremfor tidligere kjente polymerer som er benyttet sammen med kirurgiske suturer eller ligaturer.
Særlig er suturer belagt med det polymere belegg ifølge denne oppfinnelsen mindre stive enn suturer som har et belegg beskrevet i tidligere arbeider.
Dessuten er fremgangsmåter for å overtrekke en biologisk absorberbar kirurgisk artikkel ikke klart beskrevet i tidligere arbeider. Foreliggende kopolymer er oppløselig i aceton. Fremgangsmåter i tidligere arbeider kan anvende en kopolymer
av laktid og glykolid, som ikke er oppløselig i aceton.
Det biologisk absorberbare belegg ifølge denne oppfinnelsen har overlegne og uventede egenskaper i forhold til et kjent kommersielt tilgjengelig kirurgisk sutur- eller ligaturbelegg. F.eks., gir belegget ifølge denne oppfinnelse ikke en tåket overflate på suturen. Belegget kan løses opp i aceton som synes å være mindre skadelig enn andre kjente oppløsningsmidler, f.eks. metylenklorid. Videre, synes egenskaper ved suturer såsom stramming av knuter eller flytting av disse, knutepålitelighet og trekk i vevet, å være lik, om ikke bedre enn suturbelegg omtalt i tidligere arbeider.
Ifølge oppfinnelsen består den innledningsvis nevnte kopolymer av fra 50 til 85 vekt% av ledd med formel (I):
og resten av leddene utgjøres av minst én av formlene (II) til
(VII):
hvori R er H eller CH3,
I én utforming, består belegget av en vilkårlig kopolymer, hvori de gjenværende ledd består av formel (II), hvori R er H.
I en foretrukken utførelsesform utgjør leddene med formel (I) fra 70til 85 vekt-% av den vilkårlige kopolymer.
Fortrinnsvis er den indre viskositeten til kopolymeren 0,2 til 1,4 dl/g (0,5 g/dl i CHC13, 30°C) .
En utforming av oppfinnelsen har et belegg på en biologisk absorberbar sutur eller ligatur. I en spesiell utforming, er suturen eller ligaturen fremstilt fra en polymer laget av én eller flere monomerer valgt fra gruppen som består av laktid, karbonater og laktoner.
Suturen eller ligaturen med belegg på, kan foreligge i multifilament form. I en spesifikk utforming, utgjør belegget ca. 0,1 til5vekt-% av den belagte multifilament sutur eller ligatur. I en mer spesifikk utforming, utgjør belegget fra 0,5 til 3 vekt-% av den belagte multifilament sutur eller ligatur. I den mest spesifikke utforming, kan belegget ut-gjøre opptil 1,5 vekt-% av den belagte multifilament sutur eller ligatur.
En fremgangsmåte for fremstilling av et belegg på en biologisk absorberbar kirurgisk artikkel er også oppfunnet. Fremgangsmåten består i at en statistisk kopolymer der 50 til 85 vekt-% av kopolymeren består av ledd med formel (I)
idet det gjenværende ledd omfattes av minst én av formlene (II) til (VIII) hvori R er H eller CH3,
oppløses i aceton,
en sutur eller ligatur bringes i kontakt med den oppløste kopolymer;
kontakten beholdes mellom nevnte sutur eller ligatur og opp-løst kopolymer inntil kopolymeren på nevnte sutur eller ligatur utgjør fra rundt 0,1 til 5 vekt-% av den belagte sutur eller ligatur;
nevnte overtrukne sutur eller ligatur fjernes fra nevnte oppløste kopolymer; og
kopolymeren tørkes på nevnte sutur eller ligatur.

De følgende eksempler beskriver den beste fremgangsmåte for å fremstille og anvende kopolymerer i denne oppfinnelse. Hvis ikke annet er spesifisert, ble alle indre viskositets-målinger i eksemplene gjennomført ved 3 0°C.
EKSEMPEL 1
Syntese av e- kaprolakton- l- laktid ko<p>olymer e-kaprolakton (212,5 g, 1,86 mol), 1-laktid (37,5 g,0,31 mol), laurylalkohol (4,10 ml, 0,018 mol) og tinn(II)-kloriddihydrat (35,9 mg, 1,59 x 10"<4>mol) ble blandet under røring i en reaktor under nitrogen ved 175°C. Blandingen ble rørt ved 175°C i 3 timer. Den resulterende polymer hadde en sammensetning som bestemt ved 'HNMR, på 84 vekt-% e-kaprolakton og16 vekt-% 1-laktid. Den indre viskositet i det kopolymere var 0,50 dl/g (0,5 g/dl i CHC13) .
EKSEMPEL 2
Syntese av e- kaprolakton- l- laktid kopolymer e-kaprolakton (30,0 g, 0,26 mol), 1-laktid (170,0 g, 1,18 mol), laurylalkohol (5,10g,2,74x10"<2>mol) og tinn(II)kloriddihydrat (0,0162 g, 7,2 x 10"<5>mol) ble blandet under røring i en reaktor under nitrogen ved 18 0°C. Blandingen ble rørt ved 180°C i 3 timer. Den resulterende polymer hadde en sammensetning som bestemt ved'HNMR, på 13 vekt-% e-kaprolakton og 87 vekt-% 1-laktid. ^inh var 0,27 dl/g (0,5 g/dl i CHC13) .
EKSEMPEL 3
Syntese av e- kaprolakton- trimetylenkarbonat- kopolvmer
e-kaprolakton (8,0 g, 0,070 mol), trimetylenkarbonat (2,0 g, 0,020 mol), laurylalkohol (0,283 g, 1,52 x 10"<3>mol) og tinn(II)kloriddihydrat (2,02 mg, 8,91 x 10"<6>mol) ble blandet i en kolbe. Kolben ble spylt med nitrogen, evakuert og forse-glet. Kolben ble varmet ved 135°C i 24 timer. Den resulterende polymer hadde en sammensetning målt ved 'HNMR, på 86 vekt-% e-kaprolakton og 14 vekt-% trimetylenkarbonat. ^inh for den kopolymere var 0,26 dl/g (0,5 g/dl i HFAS).
EKSEMPEL 4
Syntese av e- kaprolakton- trimetylenkarbonatkopolymer
e-kaprolakton (40 g, 0,35 mol), trimetylenkarbonat (10 g, 0,098 mol), laurylalkohol (1,42 g, 5,4 x 10"<3>mol) og tinn(II)-kloriddihydrat (10,1 mg, 4,5 x10"<5>mol) ble blandet i en kolbe og varmet i 24 timer under nitrogen ved 135°C. Den resulterende polymer hadde en indre viskositet på 0,42 dl/g (0,5 g/dl i
HFAS). Sammensetningen ble bestemt ved 'HNMR til å være 86 vekt-% kaprolakton og 14 vekt-% trimetylenkarbonat.
EKSEMPEL 5
Syntese av e- kaprolakton- glvkolidkopolymer e-kaprolakton (170 g, 1,49 mol), glykolid (30 g, 0,26 mol), laurylalkohol (1,37 g, 7,3 x IO"<3>mol) og tinn(II)oktoat (0,052 g, 1,2 x 10"'' mol) ble blandet under røring i en reaktor under nitrogen ved 180°C. Blandingen ble rørt ved 180°C i 4,5 timer. Den resulterende polymer hadde en indre viskositet på 0,68 dl/g (0,5 g/dl i CHC13) . Sammensetningen ble bestemt ved'HNMR å være 85 vekt-% e-kaprolakton og 15 vekt-% glykolid.

Syntese av e- kaprolakton- glykolidkopolymerer
En rekke e-kaprolakton-glykolidkopolymerer ble fremstilt ved dén generelle fremgangsmåte beskrevet i eksempel 5. Spesifikke preparative detaljer og egenskaper ved de resulterende polymerer er oppsummert i tabell 1.

Enkle heltalls opptaksverdier er avrundet til nærmeste hele tall. (2) Denne testen måler en suturs evne til å bli strammet. En løkke legges rundt en stålstav og knyttes med en båtsmannsknop. Knuten strammes til et på forhånd bestemt strekk med en Instron-tester, og strekket fjernes deretter. Etter ny utgangsinnstilling av lengden som kan måles, testes løkken så den brister. Bruddstyrken på løk ken og forlengelse frem til brudd måles. Forlengelsen av materialet på bruddpunktet for knuten, bestemmes separat i en rett strekktest og trekkes fra forlengelsen ved knutens brudd for å få glidningen i mm innenfor knuten opp til bristepunktet. Prøver ble testet umiddelbart etter 30 sekunders ned dypping i saltvannsoppløsning (0,9% NaCl i destillert vann).
Strekkkreftene som brukes for å stramme knutene, og alle andre forhold når det gjelder knytting av knute og tes-ting, er praktiske laboratorieforhold, men trenger ikke tilsvare virkelig kirurgisk praksis. Stramming av knuten tilsvarer ikke nødvendigvis klinisk praksis.
(3) En tråd knyttes til seg selv slik at det dannes en løkke, knuten strammes til en på forhånd bestemt stramming, løkken skjæres over og de avkuttede ender festes i feste-anordningene på en Instron-tester. Bruddstyrke og forlengelse til brudd måles. Maksimal glidning måles for knutene som brytes. Dette defineres som forskjellen mellom den gjennomsnittlige forlengelse til brudd for den knuttede sutur og den gjennomsnittlige forlengelse av en ikke-knyttet tråd, målt ved en belastning lik knutens bruddstyrke. Prøver testes umiddelbart etter 3 0 sekunders neddypping i saltvannsoppløsning. (4) Båtsmannsknoper ble dannet på hånddyppede 1/0 polyglykolsyre flettede tråder ved bruk av et vanlig sutur knytte-brett. Knuten ble deretter ført nedover brettet for å måle knutens friksjon (glipping) mens den føres nedover og å måle styrken som er nødvendig for å starte og holde ved like glidningen. Forkortelsene er: R, glir; L, fes-ter; RC, glir med glipping; RD, glir vanskelig; RU, glir upålitelig; RW, glir lett. Sammenligningene gjøres på tørr sutur og på sutur fuktet med saltvannsoppløsning.
(5) Knuten holder ikke.

Karakteren for et belegg er summen av<11>1"-karakterene delt med det totale antallet av under-søkelsesobj ekter. (4) Umiddelbar knutefasthet bestemmes ved å benytte en buet pinsett til å trekke i de 8 til 10 mm ender på en båtsmannsknop med to ekstra kast. Knuter som er pålitelige når de manipuleres får karakter 1, knuter med et løst øvre kast får karakter 2, knuter med et åpent øvre kast får karakter 3, og knuter som ikke er pålitelige når de manipuleres får karakter 4.
