Description
Denne oppfinnelse vedrører et strikket tekstilmateriale eller en strikket tekstilvare for kirurgisk bruk, omfattende flere filamenter. Varen fremstilles av et vevabsorberbart materiale, nemlig polyglykolsyre(PGA)-fibre.
Tekstilvaren kan være nyttig i en rekke forskjellige indre kirurgiske operasjoner, f.eks. som tamponeringsmiddel for trykkinnkapsling og reparasjon av traumatisk skadede organer så som milt, lever eller nyre. Varen kan også brukes som null-strekk- eller lav-strekkmateriale, f.eks. ved reparasjon av en bukvegg-skade, brokk, urinleder o.l. Videre kan varen brukes i områder for det kardiovaskulære system og i hjernen. Varen kan også brukes i munnkirurgi, f.eks. for reparasjon av feil og for å påvirke lindring av vev.
Skjønt dimensjonene og vekten av varen bare er begrenset av den praktiske størrelse for tilsiktet bruk, kan dimensjonene fra 10x10 cm til 25x33 cm og med vekt fra 24 til 210 g/m<2> normalt brukes. Åpninger i varen kan være fra nesten null til 6,5 mm. Varen kan også fremstilles som en rørformet eller slangeformet protese.
En tegning som beskriver formen og/eller den geometriske konfigurasjon av tekstilvaren er unødvendig for forståelsen av denne oppfinnelsen. Enhver person som er fagutdannet på området vil vite hvordan varen kan fremstilles og hvordan oppfinnelsen skal utøves ved å lese denne beskrivelse i sin alminnelighet og eksempelet spesielt.
For ytterligere stabilisering av varen, dvs. å eliminere horisontal eller vertikal kant-krølling og for å regulere strekket, kan tekstilvaren varmefikseres eller -herdes ved å holde både lengden og bredden til en bestemt dimensjon innenfor en pinneramme eller klipsramme mens materialet utsettes for temperatur fra 9 0 til 175°C i løpet av 30 sekunder til 15 minutter, fortrinnsvis i vakuum. Varen kan også varmefikseres ved at den holdes både i lengderetningen og bredderetningen på en bestemt dimensjon på en oppvarmet sylinder mens materialet utsettes for en temperatur fra 90 til 17 5°C i løpet av en periode på opp til 4 timer i vakuum.
Fra US patent nr. 3,739,773 er kjent en strikket maskevaré av et garn fremstilt av polyglykolsyre, hvilket garn består av et antall filamenter. Fremstillingsmetoden for selve garnet er også omtalt. Det strikkede stoff som omtales i US-patentet, er imidlertid varmeherdet og ikke strekkbart. Slike egenskaper er uheldige innen det anvendelsesområde dette gjelder. Tekstilvaren ifølge foreliggende oppfinnelse avhjelper den her påviste mangel ved den tidligere kjente teknikk.
Tekstilvaren ifølge oppfinnelsen utmerker seg ved at alle filamenter er fremstilt av en polyglykolsyre, og er buntet eller tvunnet til et garn som er strikket til et strekkbart, ikke-varmeherdet tekstilmateriale, og som har høyere linjedensitet enn 60 denier og inneholder opptil 4 strenger som hver inneholder flere enn 25 filamenter. Garnet kan ha linjedensitet opp til 150 denier, og totalt antall filamenter pr. garn kan være opp til 75.
Ved en utførelse ble tekstilvaren fremstilt på en 14-gauge trikot strikkemaskin hvor strikkemønsteret var frontskinne (2/0 2/4) x 2

Tekstilvarens vekt var 135 til 340 g/m<2>. Varens beskaf-fenhet var 25 til 54 cm/480 rekker.
I en andre utførelse ble tekstilvaren fremstilt på en 48-gauge Raschel strikkemaskin hvor strikkemønsteret var frontskinne 1/0 0/1

Vekten av strikkevaren var 135 til 340 g/m<2>. Beskaffen-heten var 25 til 55 cm/480 rekker.
En tekstilvare ifølge oppfinnelsen som er beskrevet mere fullstendig i eksempelet er en strekkbar strikket tekstilvare. Tekstilvaren kan fremstilles på en 14-gauge trikotmaskin eller alternativt, på en 28-gauge Raschel varpstrikkemaskin med 2 skinner. På disse maskiner varmefikseres ikke tekstilvaren.
Garnet som benyttes til strikking av tekstilvaren kan ha linjedensitet mellom 150 og 600 denier med mellom 4 og 20 vindinger pr. 10 cm. Fortrinnsvis har tekstilvaren låste masker (petinet-rekker) slik at varen ikke kan rakne.
Denne tekstilvare er hovedsakelig nyttig i områder for organreparasjon. Spesielt er tekstilvaren verdifull ved reparasjon av myke organer f.eks. milt, lever eller nyre. Denne tekstilvare kan også være nyttig ved reparasjon av bukspyttkjertelen. Endelig er tekstilvaren meget anvendelig som oppheng eller understøttelse for organer under indre heling av organene. Tekstilvaren kan imidlertid også brukes som en fjernbar understøttelse eller oppheng til andre formål, f.eks. fjernbar fatle for bestrålingsterapi.
En annen tekstilvare som er beskrevet, er semistrekkbar strikket tekstilvare. Denne kan strikkes på en 28-gauge trikotmaskin med 1 skinne. Tekstilvaren kan strikkes av et garn med linjedensitet 60 til 100 denier.
Denne halv-strekkbare tekstilvare er nyttig ved reparasjon av bukvegger, brokk eller i oral kirurgi, men også som forbigående understøttelse eller bandasjemateriale for øking av vevets evne til å gro inn.
Enda en tekstilvare ifølge oppfinnelsen, er varmefiksert uten strekking og stabilisert. Fikseringen kan skje på en spennramme. Varen kan fremstilles på en 24-gauge trikotmaskin eller en 48-gauge Raschel maskin med 2 skinner.
Varen kan ha linjedensitet mellom 75 og 2 50 denier med 1 til 20 vindinger pr. 10 cm. Fortrinnsvis fremstilles varen med varpmasker for bedre stabilitet.
Denne tekstilvare kan være nyttig ved reparasjon av bukvegger. Et annet formål kan være reparasjon av mellomgulv-feil. Enda andre anvendelser kan være rektal understøttelse for kronisk prolaps. Endelig kan denne tekstilvare brukes i forbindelse med luftrørsmalaci.
Et eksempel på en typisk tekstilvare som omfattes av oppfinnelsen, er gitt nedenfor.

1. Strikket tekstilmateriale for kirurgisk bruk, omfattende flere filamenter,
karakterisert ved at alle filamenter er fremstilt av en polyglykolsyre, og er buntet eller tvunnet til et garn som er strikket til et strekkbart, ikke-varmeherdet tekstilmateriale og som har høyere linjedensitet enn 60 denier og inneholder opptil 4 strenger som hver inneholder flere enn 25 filamenter.
2. Strikket tekstilmateriale ifølge krav 1, karakterisert ved at garnet har linjedensitet opp til 150 denier og at det totale antall filamenter pr. garn er opp til 75.
