Description
Foreliggende oppfinnelse vedrører preservering av røde blodceller i vesentlig fysiologisk nativ tilstand. Mer spesielt angår oppfinnelsen et nytt suspensjonsmedlum for lagring av røde blodlegemer fra mennesker ved ca. 4°C i en tilstand som er egnet for blodtransfusjon. Det nye suspens jonsmedlumet Ifølge oppfinnelsen muliggjør i vesentlig grad å øke lagringstiden for røde blodlegemer i forhold til det som hittil har vært mulig.
Lagring eller preservering av røde blodceller (også betegnet blodlegemer) i et suspensjonsmedlum ved ca. 4°C ifølge standard blodbankprosedyre er velkjent. Såvidt vites er det ingen litteraturreferanser som rapporterer effektene av hverken hypotonisk suspensjon eller tilsetning av penetrerende salter, f.eks. ammoniumsalter når det gjelder å bevare kvaliteten av røde blodlegemer.
Det er imidlertid observert at under langvarig kjølelagrlng gjennomgår humane, røde blodlegemer en formforandrlng med utvikling av splkuler som til sist avknoppes til små vesikler. Dette reduserer overflatearealet til det røde blodlegemet, hvilket antageligvis gjør det mindre fleksibelt og mindre i stand til å passere gjennom miltens filtrerings-system. Dette leder igjen til fjerning av legemer fra sirkulasjonen i løpet av noen få minutter etter transfusjon. Akseptable standarder for transfuserbare røde blodlegemer krever at minst 70% av legemene fremdeles må være i sirkula-sjon 24 timer etter transfusjon.
Foreliggende oppfinnelse overvinner de begrensninger og problemer som er forbundet med den tidligere kjente prosess. Det ble resonnert at økning av overflatespenningen til blodlegemene ved osmotisk svelling teoretisk ville være tilbøyelig til å foregripe utviklingen av splkuler. Derfor ble det utviklet et suspensjonsmedium av hensiktsmessig osmolaritet og membranpermeabllltet som kunne preservere de røde blodlegemene ved 4°C i lengre tidsperioder i en tilstand egnet for transfusjon i overensstemmelse med standarder som er etablert for transfusjon av røde blodlegemer (RBCs). Det ble oppdaget at Innbefatning av visse penetrerende forbindel-ser, slik som ammonlumsalter, fortrinnsvis NH4CI og NH4COOCH3, i et suspensjonsmedlum ikke bare unngår de uheldige virkningene til lav ionisk konsentrasjon, men gir også visse positive nyttevirkninger som beskrevet mer fullstendig i det nedenstående.
Det er derfor et formål med foreliggende oppfinnelse å tilveiebringe et suspensjonsmedlum for røde blodlegemer som vesentlig øker lagringstiden for suspenderte RBCs ved ca. 4°C, mens transfusjonskvaliteten for det lagrede RBCs bibeholdes.
Det er et annet formål med oppfinnelsen å tilveiebringe en fremgangsmåte for lagring av RBCs i lengre tidsrom ved 4°C enn det som har vært mulig ved den konvensjonelle prosess, under opprettholdelse av brukbarhet for slik lagret RBCs for transfusjon hos mennesker.
Andre formål og fordeler vil fremgå fra beskrivelsen av oppfinnelsen i det nedenstående.
Disse og andre formål, trekkene og mange av de medfølgende fordeler ved oppfinnelsen, vil bedre forstås under henvisning til følgende detaljerte beskrivelse tatt i forbindelse med de medfølgende tegninger hvor: Fig.l viser ATP-tnnhold 1 human-RBCs (prosent av opprinnelig verdi) som en funksjon av lagringstid ved 4°C i forskjellige suspens j onsmedia.. Fig. 2 viser prosent in vivo overlevelse av human-RBCs etter •lagring i forskjellige tidsintervaller ved 4°C i forskjellige suspensJonsmedia.
Uten å være bundet til noen spesiell teori eller forklaring, postuleres det at for å foregripe utviklingen av uønskede splkuler i RBCs under kjølelagringen i konvensjonelle suspensjonsmedia, kan økning av overflatespenningen hos blodlegemene ved osmotisk svell ing være nødvendig. Cellene suspenderes derfor i et medium av 180-220 mOsm (milli-osmolar), mer spesielt i et medium av 200 mOsm. Suspensjons-mediets pH-verdi holdes i området fra 6,8 til 7,2, fortrinnsvis omkring 7,0. Siden dette innebærer en reduksjon i ionekonsentrasjon, hvilket kunne lede til en økning i membran-permeabilitet, opprettholdes normal ionekonsentrasjon i blodlegemene ved hjelp av en unik sammensetning som benytter visse penetrerende, ioniserbare salter i suspensjonsmediet, idet det blant slike salter foretrekkes ammonlumacetat og ammoniumklorid ved 10-200 mM konsentrasjon, fortrinnsvis ved ca. 50 mM konsentrasjon. Naturligvis kunne egnede kationer andre enn ammonium og egnede anioner andre enn acetat og klorid også fordelaktig anvendes så lenge slike salter tilfredsstiller kriteriene og formålene ved foreliggende oppfinnelse. Et eksempel på et slikt salt er metylaminhydro-klorid.
Ifølge foreliggende oppfinnelse er det tilveiebragt et sus-pen jonsmedlum for forlenget lagring av røde blodceller ved ca. 4°C, kjennetegnet ved at det innbefatter 80-150 mM glukose, 30-80 mM mannltol, 6-90 mM kallumcitrat, 1-5 mM adenln og 10-200 mM av et penetrerende ammoniumsalt i en vandig oppløsning, og egnede mengder av en- eller tobaslsk kallumfosfat for justering av mediets pH-verdi til 6,8-7,2 og til en osmolarltet på 180-220 mllliosmolar.
Andre bestanddeler i suspensjonsmediet kan være bovinserum-albumln, lnosln og pyruvat.
Betegnelsen "vesentlig forlenget" lagring og lignende betyr ganske enkelt at det oppnås levedyktig preservering eller lagring av RBCs i en tidsperiode som er lengre enn den som er mulig ved benyttelse av hittil kjent blodbankprosedyre og konvensjonelle suspensjonsmedia.
En typisk sammensetning for suspensjonsmediet ifølge foreliggende oppfinnelse innbefatter en vandig oppløsning inneholdende:
Andre slike sammensetninger kan lett fremstilles så lenge pH-verdien er ca. 7,0 og osmolariteten er i området omkring 200 mOsm.
Røde blodlegemer som har blitt separert fra sitt plasma under den normale komponentfremstilling, resuspenderes i et omtrent likt volum av dette suspensjonsmediet. Cellesuspensjonen lagres deretter ved 4°±2°C ifølge standard blodbankprosedyre som er beskrevet i "Clinical Practise of Blood Transfusion", utgivere: Petz & Swisher, Churchill-Livingston publishers, N.Y. , 1981, som innbefattes heri som referanse. Kapittel 11, side 281 i nevnte referanse, viser levedyktighetsdata for preservert RBCs ved hjelp av konvensjonelle teknikker.
Ved lagring i et suspensjonsmedlum ifølge foreliggende oppfinnelse foregår en progressiv økning i adenosintrifosfat (ATP) i løpet av de første fire til fem uker av lagringen. ATP-nivåer er tilbøyelig til å korreleres med overlevelse av røde blodlegemer etter transfusjon. Ved fire uker varierer ATP-nivåene fra 115# til så høyt som 25056 med et gjennomsnitt på 265# av den innledende verdi. ATP-nivået faller deretter jevnt med ytterligere lagring og når den opprinnelige verdi ved omkring 11 uker og faller til 5056 av den opprinnelige verdi ved omkring 16 uker.
Tabell 1 viser forskjellige komponenter og konsentrasjoner av oppløst produkt 1 suspensJonsmedia hvori røde blodlegemer ble lagret i opptil 16 uker. Det skal bemerkes at oppløsning nr. 20, som er det foretrukne suspensjonsmedlum ifølge foreliggende oppfinnelse, holder rød blodlegeme-ATP ved et høyere nivå 1 et lengre tidsrom enn noen annen testet oppløsning som vist i fig. 1. Fig. 1 viser ATP-verdier ved 4, 8, 10, 12, 14 og 16 uker for røde blodlegemer suspendert i (a) en isotonisk kallumcitrat-, fosfat-, mannitol-, glukose-, adeninoppløsning, tilsvarende oppløsning nr. 7 i tabell 1; b) den samme oppløsning som (a) med ammoniumacetat tilsatt, tilsvarende oppløsning nr. 14 i tabell 1; c) den samme oppløsning som 9a) med noe kaliumcitrat fjernet for å gjøre den hypotonisk, tilsvarende oppløsning nr. 12 i tabell 1; d) den samme hypotoniske oppløsning som (c) med tilsetning av ammoniumacetat, tilsvarende oppløsning nr. 13 i tabell 1; e) den samme hypotoniske oppløsning som (c) med tilsetning av ammoniumklorid, tilsvarende oppløsning nr. 20 i tabell 1. Resultatene indikerer at det er nyttevirkninger fra både hypotonisiteten og tilsetningen av ammoniumsalter. Nyttevirkningene til hypotonisitet kan knyttes til utsettelsen av spikul-dannelse, men grunnene for økning i ATP-nivåer er ukjent. Mekanismen hvorved ammoniumsaltene i foreliggende oppfinnelse, når disse inkorporeres i suspensjonsmediet, øket ATP, er også ukjent. Fig. 2 viser 24 timers in vivo overlevelsesverdier for røde blodlegemer lagret i oppløsning (c) i tidsrom varierende opptil 125 dager.
Det skal påpekes at det er usannsynlig at en hypotonisk cellesuspensjon inneholdende ammoniumklorid ville være akseptabel for transfusjon. Det anbefales derfor at cellene, forut for transfusjon, bør sedimenteres ved sentrifugering og cellene resuspenderes i en transfuserbar oppløsning, og deretter transfuseres.
Hemolysen av RBCs lagret i overensstemmelse med foreliggende oppfinnelse er fra 0,3 til 3, 8%.
Claims (5)
1. Suspensjonsmedlum for forlenget lagring av røde blodceller ved ca. 4°C, karakterisert ved at det Innbefatter 80-150 mM glukose, 30-80 mM mannitol, 6-90 mM kaliumcitrat, 1-5 mM adenin og 10-200 mM av et penetrerende ammoniumsalt i en vandig oppløsning, og egnede mengder av en- eller tobasisk kaliumfosfat for justering av mediets pH-verdi til 6,8-7,2 og til en osmolaritet på 180-220 milliosmolar.
2. Suspensjonsmedlum ifølge krav 1, karakterisert ved at det penetrerende saltet er valgt fra ammoniumklorid og ammoniumacetat.
3. Suspensjonsmedlum Ifølge krav 1, karakterisert ved at det inneholder 110 mM glukose, 55 mM mannitol, 7,9 mM kaliumcitrat, 2 mM adenin og 50 mM ammoniumklorid eller -acetat.
4. Suspensjonsmedlum Ifølge krav 1, karakterisert ved at pH-verdien holdes ved en verdi på ca. 7.
