Description
Foreliggende oppfinnelse vedrører en fremgangsmåte for fremstilling av et preparat for behandling av urinveisinfeksjoner. Preparatet er basert på anvendelse av nitrofurantoin.
Nitrofurantoin har i mange år med fremgang blitt benyttet
for behandling av urinveisinfeksjoner. Det antas at virk-ningen av denne forbindelse er at den griper inn i mange bakterielle enzymsystemer. Bakterier utvikler bare en begrenset resistens mot nitrofurantoin klinisk. Nitrofurantoin er indikert for behandling av urinveisinfeksjoner forårasket av følsomme stammer av E. coli, enterokokker,
S. aureus og visse følsomme stammer av Klebsiella-arter, Entero-bakteriearter og Proteus-arter.
Anvendelse av nitrofurantoin i de tradisjonelle farmasøy-tiske doseringsformene slik som pulver, tabletter, kapsler, suspensjoner og oppløsninger, har krevet peroral tilførsel av vanligvis fire enhetsdoser pr. dag. Mange pasienter som inntar nitrofurantoin, er eldre mennesker som ofte også inntar mange andre medisiner daglig for annen behandling.
Med hensyn til pasientens velvære og - hvilket er mer vesentlig - for å sikre at pasienten skal reagere på et spesielt behandlingsprogram, er det følgelig betydningsfullt at altallet enhetsdoser av medisiner som administreres peroralt hver dag, skal være så lite som mulig.
Med det formål å rette på disse problemer, har det vært
gjort forsøk av mange forskere på å tilveiebringe klassiske doseringsformer av nitrofurantoin med bibeholdt eller forlenget frigivelse for det formål å minske antallet av daglige perorale administrasjoner som er nødvendige for pasienten. Samtlige tilveiebragte doseringsformer har hatt den ulempe at de ikke betydelig forlenger nivåene for nitrofurantoin i urin mer enn det som oppnås med de i øye-blikket tilgjengelige preparater. Nitrofurantoin synes ikke å være egnet for anvendelse i de klassiske farmasøy-tiske produktene med bibeholdt/forlenget frigivelse.
Dette tilskrives det foretrukne opptak av nitrofurantoin i
et begrenset område av mage-tarmkanalen hos mennesker,
nærmere bestemt duodenum. Samtidig som de fleste forsøk på
å få frem en doseringsform for nitrofurantoin med bibeholdt/ forlenget frigivelse har vært fremgangsrike fra det syns-punkt at frigivelsen av det aktive legemiddelstoff fra doseringsformen går langsommere ved peroral tilførsel (hvilket er bevist ved minsket human biotilgjengelighet for legemiddel-stoffet), har det ikke fremkommet noe bevis for at nitrofurantoin gir et mønster med forsinket utsondring av urin sammenlignet med det som oppnås med tilgjengelige doseringsformer. Det er intet preparat tilgjengelig som tilbyr pasienten et behandlingsprogram av urinveisinfeksjoner med to doser daglig av nitrofurantoin.
Ved hjelp av den ifølge oppfinnelsen fremstilte doseringsform av nitrofurantoin oppnås mulighet for et behandlingsprogram med to doser daglig i slike tilfeller når tilgjengelig tradisjonelle preparater krever et program med 3 til 4 doser daglig. Dette preparat er ingen doseringsform for kontinuerlig be-behandling av typen beredning med bibeholdt eller forlenget frigivelse. Hensikten er i stedet at det skal benyttes intermitterende i form av en eneste oral dose. Doseringsformen er ikke beregnet for varig behandling; den vanlige opp- og nedgående typen av blod/urin-profiler som er forbundet med intermitterende dosert behandling, kan bli tydelig. For-delene med foreliggende doseringsform er at den er praktisk og fordelaktig for pasienten ved at den muliggjør et peroralt behandlingsprogram med minsket antall daglige doser.
Gjennom foreliggende fremgangsmåte tilveiebringes en doseringsform av nitrofurantoin som utgjøres av en enterisk overtrykket tablett som hurtig går i stykker eller løser seg opp og inneholder et lett oppløselig salt av nitrofurantoin. En slik doseringsform representerer en bekvem metode for behandling av pasienten med to doser daglig i kombinasjon med konvensjonelle doseringsformer av nitrofurantoin.
Med det formål å oppnå det ønskede program med oral Inngivning to ganger daglig av nitrofurantoin, må legemidlet frigis fra den enterisk overtrukne doseringsformen og bli tilgjengelig for absorpsjon i mage-tarmkanalen ca. 3-4 timer etter peroral tilførsel. En innledende dose av nitrofurantoin fra konvensjonelle doseringsformer frigis og blir umiddelbart tilgjengelig for absorpsjon. Med tømmingstider fra magen i det typiske tilfellet av 1/2 til 2 timer, befinner den umiddelbart tilgjengelige dosen av nitrofurantoin seg på den primære opptagningsplassen i mage-tarmkanalen, dvs. duodenum, innen 1-2 timer. Med den meget hurtige hastighet (rate) med hvilken legemidlet elimineres fra blodet, f.eks.
i løpet av mindre enn 1 time, kan man vente seg topper med henblikk på konsentrasjonen av nitrofurantoin i urinen innen ca. 2-3 timer.
Ifølge foreliggende oppfinnelse er det tilveiebragt en fremgangsmåte for fremstilling av et oralt, administrerbart preparat for behandling av urinveisinfeksjoner, og denne fremgangsmåten er kjennetegnet ved at et nitrofurantoin-natriumsalt eller et hydrat derav blandes med ett eller flere farmasøytiske fortynningsmidler, at blandingen overføres til doseringsenheter, samt at hver slik enhet forsynes med et enterisk belegg bestående av eller inneholdende anionisk kopolymer av metakrylsyre og metakrylsyremetylester.
Fremstillingen av den med enterisk belegg forsynte doseringsform av nitrofurantoin fremgår fra nedenstående eksempler.
Eksempel I
De ovenfor angitte bestanddeler i eksempel I blandes på en slik måte som er vanlig innen farmasøytisk praksis. Den ferdige blandingen siktes og presses til tabletter med kon-veks kjerne og vekten 488 mg ved direkte pressing under anvendelse av en egnet tablettpresse som gir tabletter med en diameter på ca. 11 mm og høyde på 5,4 mm.
I eksempel I kan det aktive legemiddelstoff ha form av et salt (natrium eller kalium) av nitrofurantoin eller et av dets solvater, f.eks. hydrat. Fortynningsmidlet (farmasøy-tiske hjelpestoffer) kan innbefatte (1) fyllstoff for å øke volumet og forbedre komprimerbarheten, f.eks. laktose, stivelse, sukkeralkoholer, cellulosederivater, kalsiumsul-fat eller -fosfat, (2) sprengmiddel for å findele doseringsformen, f.eks. stivelse, natriumstivelseglykolat, cellulosederivater, alginater, planteslim, brusemiddelblandinger, (3) bindemiddel for å danne granuler eller forbedre sammen-pressbarheten, f.eks. planteslim, sukkerarter, stivelse, cellulosederivater, alginater, polyvinylpyrrolidon, (4) smøremiddel for å minske friksjonen, f.eks. stearinsyre, metallstearater, høytsmeltende vokser og talk, (5) middel for å forbedre oppløsningen, f.eks. overflateaktive midler, alkaliske buffere og (6) glidemiddel for å forbedre risle-evnen, f.eks. stivelse, talk, silikater.
Eksempel II
"Eudragit-S" er en anionisk polymer fremstilt fra metakrylsyre og metakrylsyremetylester.
I eksempel II kan konsentrasjonen av fortynningsmiddel og oppløsningsmiddel i den filmdannende oppløsning varieres for å påvirke tørkehastighetene ved forskjellige prosess-temperaturer og i apparatur. Alternative oppløsnings-midler slik som metanol, etanol eller metylenklorid, samt andre mykningsmidler slik som ricinusolje, glycerol, propy-lenglykol eller ftalatderivater og stoffer som kan danne enteriske filmer slik som shellakk, celluloseacetatftalat, hydroksypropylmetylcellulose, kan benyttes.
Pressede tabletter fra eksempel I overtrekkes enterisk ved sprøyting med "Eudragit-S"-suspensjonen fra eksempel II på overflatene, mens tablettene bringes til å rotere i en kon-vensjonell beholder for belegging. Den tykkelse på belegget som er nødvendig å tilveiebringe en jevn, sammenhengende overflatefordeling, varierer mellom 4,0 og 7,2 mg/cm 2 beregnet som tørt lakkstoff. Beleggets tykkelse kan variere utover dette området avhengig av produksjonsskala og prosess-apparatur. Man kan også anbringe belegget fra en luftsus-pensjon.
For å eksemplifisere hvor effektivt et enterisk filmbelegg av "Eudragit-S" er, når det gjelder å forsinke absorpsjonen og forlenge utsondringen via urinen av nitrofurantoin hos mennesker, ble biotilgjengeligheten med 18 individer i crossover-utførelse studert, hvorved testdoseringsformene ble sammenlignet med et i handelen tilgjengelig nitrofuran-toinpreparat for kontrollformål ved samme 200 mg-doser.
Doseringsformene ble administrert til friske, ikke-fastende individer som urinerte hver annen time i 14 timer og på nytt ved tidspunktet 24 timer. Nitrofurantoininnholdet i urinen ble målt spektrofotometrisk. Resultatene er angitt i tabell I.

Prosentuell gjenvinning av nitrofurantoin i urinen etter oral administrasjon av tabletter inneholdende natriumnitro-furantoin enterisk overtrukne med "Eudragit-S" og et kommersielt produkt som kontroll.
De i tabell I viste biotilgjengelighetsresultater viser evnen til enteriske filmer av "Eudragit-S" til å forsinke frigivelse og absorpsjon og følgelig urinutsondring av nitrofurantoin sammenlignet med et ikke-enterisk overtrukket kontrollpreparat. Ingen gjenvinning i oppsamlingsinter-vallet 0-2 timer hva angår den enterisk overtrukne tablett-doseringsformen indikerer forsinket opptagelse i mage-tarmkanalen. Høye maksimumsverdier i 4-6-timerintervallet for de enterisk belagte tablettdoseringsformene utgjør ytter-ligere bevis for at filmbestanddelen "Eudragit-S" gir opp-hav til forsinket opptak. Uringjenvinningen av nitrofurantoin fra kontrollen er tydelig i 0-2-timersoppsamlings-intervallet og toppen for nitrofurantoin i urinen tidligere ved 2-4 timer.
Forsinket gastrointestinalt opptak og urinutsondring med enterisk belegg av "Eudragit-S" påvirkes av tykkelsen til
tablettens overflatefilm. Når tykkelsen på den enteriske

filmen øker fra 4,0 mg/cm til 5,6 mg/cm , endres urin-utsondringen av nitrofurantoin med hoveddelen iløpet av 2-6 timer til hoveddelen av nitrofurantoin utsondret iløpet av 4-8 timer.
Inngivelsen av doseringsformen i eksemplene I og II led-saget av eller i kombinasjon med konvensjonelle farmasøy-tiske beredningsformer av nitrofurantoin gjør det mulig å redusere behandlingsprogrammet, hvorved de konvensjonelle formene gjør at nitrofurantoin blir tilgjengelig umiddelbart og den enterisk overtrukne form muliggjør en forsinkelse.
Inngivelsen av en kombinasjon av disse to former kan finne sted på flere forskjellige måter. Fortrinnsvis kan den oppnås ved sprøytebelegging av en sammensetning inneholdende 25-50 mg nitrofurantoin på den enterisk overtrukne doseringsformen i eksemplene I og II. Utover en således over-trukken tablettform kan doseringsformen alternativt bestå av en liten forpakning eller en kapsel inneholdende pH-følsomme enterisk overtrukne nitrofurantoin-natrium-granuler eller tabletter (for dosen med forsinkelse) og ikke overtrukket eller helt enkelt filmovertrukket nitrofurantoin-pulver, -granuler eller -tabletter (for umiddelbare doser) eller også kan doseringsformen bestå av en ved våtgranu-lering, tørrgranulering eller direkte sammenpressing lagdelt tablett eller en tablett inne i en tablett eller kapsel. Hver doseringsform inneholder den umiddelbare frigivelses-dosen og den forsinkede dosen (ved pH-følsomt enterisk belegg) som beskrevet ovenfor.
Claims (1)
- Fremgangsmåte for fremstilling av et oralt administrerbart preparat for behandling av urinveisinfeksjoner, karakterisert ved at et nitrofurantoin-natriumsalt eller et hydrat derav blandes med ett eller flere farmasøytiske fortynningsmidler, at blandingen overføres til doseringsenheter, samt at hver slik enhet forsynes med et enterisk belegg bestående av eller inneholdende anionisk kopolymer av metakrylsyre og metakrylsyremetylester.
