Description
Farmaceutische samenstelling die als aktieve ingrediënt 3-/2-(dimethylamino)ethyl/-N-methyl-1H-indool-5-methaansulfo-namide bevat.
De uitvinding heeft betrekking op een farmaceu-tische samenstelling die als aktieve ingrediënt 3-/2-(dimethylamino )ethyl7-N-methyl-1H-indool-5-methaansulfonamide bevat, in het bijzonder een samenstelling geschikt voor orale toediening.
3-/2-(diethylamino)ethyl/-N-methyl-1H-indool-5-me-thaansulfonamide, dat voldoet aan formule I van het formuleblad en de fysiologisch aanvaardbare zouten en solvaten daarvan worden beschreven in het Britse octrooischrift 2.162.522. De verbinding van formule I vertoont selectieve vaatvernauwende werking en kan gebruikt worden bij de behandeling van migraine.
Geneesmiddelen worden in het algemeen bij voorkeur oraal toegediend, omdat dit het meest gemakkelijk en aanvaardbaar voor de patiënten is. Helaas hebben orale samenstellingen zekere nadelen bij de behandeling van met hoofdpijn geassocieerde klachten. Deze klachten, in het bijzonder migraine, gaan bijvoorbeeld samen met een gastrointestinale disfunctie in de vorm van een vertraagd legen van de maag. Dit leidt tot zowel een vertraging als een verhindering van de opname van het geneesmiddel, en algemeen wordt aangenomen dat orale geneesmiddelsamenstellingen voor de behandeling van zulke klachten in de vorm van een vloeibaar preparaat moeten worden toegediend.
Een aantal klinische studies hebben de effectiviteit van de verbinding van de formule I bij migraine-patiënten bewezen. Tot nu toe wordt de verbinding altijd toegediend ofwel door middel van een parenterale injectie ofwel in de vorm van een dis-pergeerbare tablet die voor het toedienen in water wordt gedisper-geerd. Deze wijze van toediening werd geacht de potentiële problemen gerelateerd met de gastrointestinale disfunctie van migraine— patiënten te minimaliseren.
Het is echter gebleken dat de verbinding van formule I een bijzonder onaangename smaak heeft. Als de verbinding van formule I oraal wordt toegediend kan dit de misselijkheid en braken die samengaan met migraine verergeren.
De onderhavige uitvinding verschaft een bijzonder geschikte samenstelling geschikt voor orale toediening van de verbinding van formule I.
Zo wordt volgens de onderhavige uitvinding een farmaceutische samenstelling voor orale toediening verschaft die een met een bekledingslaag beklede doseereenheid die 3-/2-(dimethylami-no)ethyl7-N-methyl-1H-indool-5-methaansulfonamide of een farmaceutisch aanvaardbaar zout of solvaat daarvan bevat. De boven gebruikte term "met een bekledingslaag beklede doseereenheid" betekent een harde kern die de aktieve ingrediënt bevat, welke harde kern bedekt is met een bekledingslaag.
De samenstellingen volgens de uitvinding kunnen bijvoorbeeld korrels, tabletten of capsules omvatten. De samenstellingen volgens de uitvinding omvatten bij voorkeur tabletten, en in bijzondere voorkeur geperste tabletten.
Een bevoorkeurd aspect van de uitvinding verschaft een met een bekledingslaag beklede tablet die een tabletkem die een effectieve hoeveelheid 3-/2-(dimethylamino)ethyl/-N-methyl-1H— indool-5-methaansulfonamide of een farmaceutisch aanvaardbaar zout of solvaat daarvan als aktief ingrediënt bevat en een bekledingslaag op de tabletkern omvat-
Gebleken is dat de onaangename smaak die samengaat met de orale toediening van de verbinding van formule I bij de sa menstellingen volgens de huidige uitvinding geëlimineerd wordt. De bekledingslaag maakt de samenstellingen ook makkelijker hanteerbaar en verminderd potentieel gevaarlijke stofvorming tijdens het verpakken en toedienen van het geneesmiddel. Deze voordelen worden verrassenderwijs verkregen zonder enige significante afname van de biologische beschikbaarheid van de verbinding van formule I in vergelijking met waterige oplossingen of dispergeerbare tabletsamen-stellingen voor orale toediening aan migraine-patiënten. De met een bekledingslaag bedekte tabletten volgens de uitvinding zijn daarom bijzonder effectief bij de behandeling van migraine. Het heeft de voorkeur dat 3-/2-(dimethylamino)ethylT-N-methyl-1H-indool-5-me-thaansulfonamide gebruikt wordt in de samenstellingen volgens de uitvinding in de vorm van een fysiologisch aanvaardbaar zout. Zulke zouten omvatten zouten van anorganische of organische zuren zoals waterstofchloride, waterstofbromide, sulfaat, nitraat, fosfaat, formiaat, fumaraat, maleaat, tartraat en succinaatzouten. Bij de samenstellingen volgens de uitvinding wordt 3-/2-(dimethylamino)-ethyl7-N-methyl-1H-indool-5-methaansulfonamide het meest bij voorkeur in de vorm van het succinaat (1:1) zout gebruikt.
De bekledingslaag omvat een polymeer. Geschikte polymeren omvatten cellulose-ethers, bijvoorbeeld hydroxypropylme-thylcellulose, hydroxypropylcellulose of methylcellulose, en copo-lymeren van methacrylzuur en methylmethacrylaat. De bekledingslaag omvat bij voorkeur hydroxypropylmethylcellulose. De totale hoeveelheid vaste stoffen voor de bekledingslaag worden in het algemeen in een hoeveelheid van 2-5 gew.%, bij voorkeur 3-4 gew.%, gebaseerd op het gewicht van de vaste doseereenheid, aangebracht op de vaste doseereenheid, bijvoorbeeld de tabletkern.
De bekledingslaag kan bovendien farmaceutisch aanvaardbare kleurstoffen of dekstoffen omvatten, bijvoorbeeld in water oplosbare verven, aluminiumlak uit wateroplosbare verven en anorganische kleurstoffen zoals titaandioxyde en ijzeroxyde. Geschikte kleurstoffen of dekstoffen kunnen 5-65 gew.%, bij voorkeur 25-50 gew.%, gebaseerd op het drooggewicht van de bekledingslaag, omvatten.
De deklaag kan ook één of meer weekmakers die gebruikelijk worden toegepast in polymere deklagen bevatten, bijvoorbeeld polyethyleenglycol, propyleenglycol, drnutylsebacaat, minerale olie, sesamolie, diethylftalaat en triacetine. Geschikte weekmakers kunnen 1-40 gew.%, bij voorkeur 5-20 gew.% gebaseerd op het drooggewicht van de bekledingslaag, uitmaken.
Buiten de verbinding van formule I of een fysiologisch aanvaardbaar zout of solvaat daarvan, bevatten samenstellingen volgens de uitvinding bij voorkeur ook farmaceutisch aanvaardbare dragers en ingrediënten zoals bindmiddelen (bijvoorbeeld voor-gelatineerd maïszetmeel, polyvinylpyrrolidon of hydroxypropylme-thylcellulose); vulstoffen (bijvoorbeeld lactose, sucrose, mannitol, maïszetmeel, microkristallijne cellulose of calciumwaterstof-fosfaat); glijmiddelen (bijvoorbeeld stearinezuur, polyethyleenglycol, magnesiumstearaat, talk of siliciumoxyde); ontleedmiddelen (bijvoorbeeld aardappelzetmeel, zetmeelnatriumglycolaat of croscar-mellosenatrium); of bevochtigingsmiddelen (bijvoorbeeld natrium-Laurylsulfaat).
Voor de bereiding van samenstellingen volgens de uitvinding wordt 3-/2-(dimethylamino)ethyl/^-N-methyl-1H-indool-5-methaansulfonamide of een fysiologisch aanvaar ernaar zout of solvaat daarvan gemengd met geschikte ingrediënten en, indien gewenst, gegranuleerd. Het heeft de voorkeur dat 3-^/2- (dimethylamino)ethyl/-N-methyl-lH-indool-5-methaansulfonamide met een vulstof wordt gegranuleerd voordat de andere ingrediënten worden toegevoegd. De gebruikte vulstof is met de meeste voorkeur lactose. Onbeklede tabletten kunnen bijvoorbeeld bereid worden door het samenpersen van het poedermengsel of granulaat, waarbij een glijmiddel als hulp bij het tabletteren wordt gebruikt. Samengeperste tabletten hebben de voorkeur.
De vaste doseereenheid wordt dan met een bekledingslaag bekleed, waarbij een suspensie die een geschikt polymeer in een geschikt oplosmiddel bevat wordt gebruikt. De bevoorkeurde oplossing voor het aanbrengen van de bekledingslaag is gezuiverd water maar verschillende soorten organische oplosmiddelen die gewoonlijk gebruikt worden bij deze werkwijze zoals alkoholen, keton- en, ethers en gechloreerde koolwaterstoffen, bijvoorbeeld ethanol, aceton, methyleenchloride en dergelijke kunnen ook worden gebruikt. Het oplosmiddel komt niet voor in het eindprodukt· De hoeveelheid oplosmiddel kan gevarieerd worden afhankelijk van de apparatuur en de omstandigheden die gebruikt worden om een mooi beklede tablet te verkrijgen.
De hoeveelheid 3-/2-(dimethylamino)ethyl7-N-methyl-1H-indool-5-methaansulfonamide, bij voorkeur in de vorm van een fysiologisch aanvaardbaar zout, die gebruikt wordt in de samenstellingen volgens de uitvinding ligt bij voorkeur in het traject van ongeveer 25 mg tot ongeveer 200 mg, meer bij voorkeur van ongeveer 50 mg tot 100 mg, uitgedrukt als het gewicht van de vrije base.
Een verder aspect van de uitvinding omvat een werkwijze voor het behandelen van een zoogdier, met inbegrip van de mens, die lijdt aan of gevoelig is voor met hoofdpijn geassocieerde kwalen zoals clusterhoofdpijn, chronische paroxysmale hemicrania, hoofdpijnen die verband houden met vasculaire kwalen, hoofdpijnen die verband houden met het toedienen of onthouden van stoffen (bijvoorbeeld het niet meer toedienen van geneesmiddelen), spanningshoofdpijn en in het bijzonder migraine, die het oraal toedienen van een farmaceutische samenstelling omvat die een met een bekledings-laag beklede doseereenheid van 3-/2-(dimethylamino)ethyl7-N-me-thyl-1H-indool-5-methaansulfonamide of een farmaceutisch aanvaardbaar zout of solvaat daarvan als een aktieve ingrediënt omvat. Men moet begrijpen dat het verwijzen naar behandeling bedoeld is om zowel preventie als het verzachten van aanwezige symptomen te omvatten.
De exacte therapeutische dosis van de aktieve ingrediënt zal afhankelijk zijn van de leeftijd en conditie van de patiënt en de aard van de te behandelen kwaal, en deze zal uiteindelijk door de behandelend arts bepaald worden.
In het algemeen zullen effectieve doses voor de behandeling van kwalen die verband houden met hoofdpijn, bijvoorbeeld de directe behandeling van migraine, echter liggen in het traject van 10 - 500 mg, bij voorkeur 20 - 300 mg, het meest bij voorkeur 25 - 200 mg, bijvoorbeeld 50 - 100 mg van de aktieve ingrediënt per doseereenheid die in enkele of gedeelde dosis toegediend kan worden bijvoorbeeld 1 tot 4 maal per dag.
De uitvinding wordt verder toegelicht door de volgende niet limitative voorbeelden waarin de aktieve ingrediënt 3-^5-(dimethylamino)ethyl/-N-methyl-1H-indool-5-methaansulfonamide (1:1) succinaat is.

Gezuiverd Water Ph Eur tot 100,0++ ++ Het water komt niet voor in het eindprodukt. Het theo retische maximum gewicht van de vaste stof die tijdens het afdekken worden gebruikt is 1 mg/tablet.
% Opaspray white is een geoctrooieerde bekledingslaag suspensie, verkrijgbaar van Colorcon Ltd. UK, die hydroxypropyl-ethylcellulose en titaandioxide bevat.
De aktieve ingrediënt en de lactose werden gemengd en gegranuleerd door het toevoegen van gezuiverd water. De na het mengen verkregen korrels werden gedroogd en gezeefd, en de resulterende korrels werden vervolgens gemengd met de andere ingrediënten van de tabletkern. Het mengsel werd samengeperst tot tabletten. De tabletten werden vervolgens met een bekledingslaag bekleed, waarbij de bekledingslaagsuspensie in een conventionele bekleder werd gebruikt.

De tabletkernen werden bereid zoals beschreven is in voorbeeld 1. De tabletten werden vervolgens met een bekledingslaag bekleed waarbij de bekledingslaagsuspensie die hieronder wordt beschreven wordt gebruikt in een conventionele bekleder.

Gezuiverd water Ph. Eur. tot 100,0++ ++ Het water komt niet voor in het eindprodukt. Het theo retisch maximumgewicht van de vaste stoffen die tijdens het bedekken wordt aangebracht is 9 mg/tablet.
Opadry pink is een geoctrooieerd bekledingslaagmate-riaal, dat verkregen kan worden van Colorcon Ltd, UK dat hydroxy-propylmethylcellulose, titaandioxide, rood ijzeroxide en triacetine bevat.
Claims (10)
1. Farmaceutische samenstelling voor orale toediening dat een met een bekledingslaag beklede vaste doseereenheid omvat die 3-/5-(dimethylamino)ethyI/-N-methyl-lH-indool-5-methaansul~ fonamide of een farmaceutisch aanvaardbaar zout of solvaat daarvan als aktieve ingrediënt bevat.
2. Farmaceutische samenstelling volgens conclusie 1 waarin de aktieve ingrediënt in de vorm van het succinaat (1:1) zout wordt gebruikt·
3. Farmaceutische samenstelling volgens conclusie 1 of 2 met het kenmerk dat deze de vorm heeft van een tablet.
4. Farmaceutische samenstelling volgens conclusie 3 met het kenmerk dat deze de vorm heeft van een samengeperste tablet.
5. Farmaceutische samenstelling volgens één der conclusies 1-4 met het kemerk dat de bekledingslaag een polymeer bevat.
6. Farmaceutische samenstelling volgens conclusie 5, met het kemerk dat het polymeer hydroxypropylmethylcellulose is.
7. Farmaceutische samenstelling volgens één der conclusies 1 - 6, met het kenmerk dat het gewicht van de bekledingslaag 2-5 gew.%, gebaseerd op het gewicht van de vaste doseereenheid, is.
8. Farmaceutische samenstelling volgens één der conclusies 1-7 met het kemerk dat deze 25 - 200 mg actieve ingredient bevat.
9. Werkwijze voor de behandeling van een zoogdier, inbegrepen de mens, dat lijdt aan of is gevoelig voor kwalen die verband houden met hoofdpijn, zoals clusterhoofdpijn, chronische len, hoofdpijn gerelateerd aan verbindingen of het onthouden daarvan (bijvoorbeeld het onthouden van geneesmiddelen), spanningshoofdpijn en in het bijzonder migraine, die een orale toediening van een farmaceutische samenstelling omvat die een met een bekle-dingslaag beklede vaste doseereenheid van 3-£2-(dimethylamino)-ethyl7-N-methyl-1H-indool-5-methaansulfonamide of een farmaceutisch aanvaardbaar zout of solvaat daarvan als aktieve ingrediënt bevat.
10. Werkwijze voor een bereiding van een farmaceutische samenstelling volgens één der conclusies 1-8 die het aanbrengen van een bekledingslaag op een vaste doseereenheid van de aktieve ingrediënt door middel van een bekleedtechniek omvat.
