Description

관절 보철을 고정하기 위한 방법 및 수단
본 발명은 관절 보철을 고정시키기 위한 방법 및 수단에 관한 것이다.
관절 보철을 고정하는 통상적인 방법은 뼈 조직에 보철을 시멘트 접합시키는 것이며, 즉, 관절 보철과 뼈의 벽 사이의 틈을 뼈 시멘트로 채우는 것이다. 시멘트 접합된 보철의 만족스럽지 못한 장기간 고정의 문제점으로 인하여, 현재 보철을 고정시키기 위해 시멘트를 덜 자주 사용하게 한다. 이와 관련하여, 뼈속에 확장된 공동에 삽입되는 보철대의 형태를 공동의 형태와 일치시키고 뼈 조직을 형성시키고 적합한 시간 내에 모든 측면으로부터 보철대 상으로 성장시켜서 보철을 뼈의 벽에 고정시키도록, 뼈의 벽과 보철대의 표면 사이의 가교 거리를 가능한 짧게 하는 것이 매우 중요하다. 물론, 최소 가교 거리는 보철표면과 뼈의 벽 사이의 확실한 물리적 접촉이지만, 뼈 속의 관절 보철에서는 이러한 접촉이 충분하지 않게 일정 지점 이외의 다른 지점에서 일어난다는 것은 가능성이 거의 없다. 뼈 시멘트가 뼈의 벽으로부터 보철 표면을 분리시키기 때문에, 이러한 접촉을 달성시키고 우수한 장기간 고정을 제공하지만, 불량한 단기간 고정을 제공하는 것으로 이루어지는 시멘트를 사용하지 않는 방법을, 우수한 단기간 고정을 제공하지만 불량한 장기간 고정을 제공하는 뼈 시멘트를 사용하는 방법과 결합시키는 것은 어렵다.
본 발명에 따르면, 뼈 조직에 대한 관절 보철의 고정은 생물학적으로 적합한 과립 물질을 사용함으로써 보장되며, 여기에서 과립 물질은 실제로 균일한 입자 크기 분포를 갖고, 실제로 불규칙적이고/이거나 가소성이다. 수술이 완결된 후에, 이들 과립 물질은 서로에 대해 그리고 뼈 조직 및 보철대에 대해 치밀하게 충전되고 고정되어야 한다. 고정 수단으로서 과립 물질을 사용함으로써, 통상적으로 시멘트를 사용하지 않는 조작과 비교하여, 보철대와 뼈의 벽 사이의 더 큰 공차가 허용되며, 동시에 뼈 시멘트의 사용과 관련한 문제점이 해소된다. 이와 같이, 본 발명은 우수한 단기간 고정, 및 우수한 장기간 고정을 달성시키는 데에 있어서의 문제점의 해결책을 제공한다.

바람직하게는, 본 발명에 따르는 방법은 하기의 방식으로 수행된다.
공동은 보철을 적용시키려는 뼈 속에서 확장된다. 생물학적으로 적합한 물질의 과립 물질은 공동 속에 넣어져서 층을 형성하여, 보철은 실제로 전체 보철대가 과립 물질에 의해 둘러싸일 때까지 과립 물질 층 내로 침투한다. 과립 물질을 보유하기 위한 장치는 보철대 둘레에 적용되고, 보철은 임의적으로 보철을 공구로 쳐서 최종적으로 고정된다.

과립 물질에 대한 뼈의 벽, 과립 물질에 대한 과립 물질, 및 보철대에 대한 과립 물질의 맞물림, 즉 고정을 제공하기 위해 과립 물질은 또한 불규칙하고/하거나 가소성이어야 하며, 즉 외력을 받을 경우에 과립 물질의 형태를 변화시키고, 외력의 작용이 중단될 경우에 새로운 형태를 유지할 수 있어야 한다. 고형 과립 물질을 사용할 수 있지만, 특정의, 바람직하게는 고다공성을 갖는 과립 물질이 바람직하다. 다공성 과립 물질은 과립 물질의 용융을 통해 기체 또는 액체를 송풍시킴으로써 공지된 방법으로 얻어진다. 임의적으로 과립 물질은 항생 물질 및 또는 성장-자극제로 충전되거나 피복될 수 있다.
상기 기술된 바람직한 구체예에서, 과립 물질은 과립 물질 층 중에서 유동화되어, 보철대를 확장된 공동 중의 과립 물질 층 내로 침투된다. 유동화는 2-단계로 수행되며, 제1단계는 공동 재에 위치한 층의 과립 물질이 액체로서 작용한다는 점에서, 높은 진동수에서 보철대를 과립 물질 층 내로 침투시킨다. 유동화, 치밀화 및 맞물림을 위해 필요한 진동에 대한 적합한 진동수는 각각의 특정 경우에 당업자에 의해 시험되고, 과립 물질의 형태 및 보철의 형태와 같은 인자에 의존한다. 1 내지 1.5㎜의 입자 크기를 갖는 다공성 티탄 분말의 경우에, 통상적인 유형의 공기식으로 분말화된 진동 골톱을 사용하고, 조임 수단이 제공되는 경우에 우수한 결과가 달성된다.
과립 물질 덩어리는 보철대 둘레에서 생성된 치밀화되고 맞물린 과립물질 층 위에 뼈 시멘트 또는 플라스틱의 칼라 또는 고리를 가함으로써 공동 내에 고정된다. 대안적으로, 뼈 시멘트 또는 동등한 빠르게 응고되는 액체가 과립 물질 상에 부어질 수 있다.
보철(보철대)은 이것이 외부의, 예를 들어 거칠고 고르지 못한 마찰표면을 갖는 한은 공지된 형태를 가질 수 있다. 적합하게는 보철대는 생물학적으로 적합한 성장-자극 물질로 구성되거나, 이러한 물질의 표면층을 가져서 상기 외부 마찰 표면을 형성시킨다.

뼈의 벽(1)과 골반 관절 보철의 원뿔형 대의 외벽(2) 사이의 공간은 약 1㎜의 크기를 갖는 순수한 티탄의 불규칙한 과립 물질(3)로 충전된다. 과립 물질(3)은 다공성이고, 과립 물질의 용융물을 통해 기체를 송풍시킴으로써 수득된다. 과립 물질은 충전되고 서로에 대해 고정된다. 기계적 맞물림은 보철대의 외벽(2)과 여기에 인접한 과립 물질 사이에 제공될 뿐만 아니라, 뼈의 벽(1)과 여기에 인접한 과립 물질 사이에 제공된다. 외과 수술에 의해 초기에 제거된 환자의 한 조각의 골반 관절 볼로 이루어지는 것이 유리한 "바닥 플러그(4)"는 공동의 하한 경계를 형성하고, 플랜지에 의해 주변뼈의 벽에 맞물린다. 뼈 시멘트의 커버(5)는 상한 경계를 형성한다. 도면에서, 과립 물질 층의 두께는 명료함을 더하기 위해 확대한 것이다.
상기 언급한 치밀화, 맞물림 및 고정, 및 결과적으로 보철 고정은 하기의 방식으로 수행된다.
외과 의사가 통상적인 방식으로, 보철대를 삽입시키려는 대퇴골 부분을 확장시키고 바닥 플러그를 가한 후에, 생성된 공동은 확장된 뼈의 개방부의 바로 아래의 높이까지 고압솥에서 살균시킨 상기 과립 물질(3)로 채워진다. 그 후에, 보철대는 이렇게 해서 얻어진 과립 물질 층 내에 삽입되고 그 안에 침투되어, 칼라(7)와 뼈의 개방부 사이에 단지 수 ㎜가 남게 된다. 보철대를 과립 물질 층에 침투시키기위해, 보철대의 상단(6)에 작용하는 조절 가능한 진동수를 갖고, 적합한 진동수에서 과립 물질 층을 느슨하게 하도록 개조된 진동 공구가 사용되어, 보철대를 층 내로 침투시킨다. 층 내로의 보철대의 침투가 층의 치밀도 때문에 과도하게 느려지는 경우, 외과 의사는 진동수를 증가시킨다. 보철대의 위치가 상기 규정한 바와 같이 수득되면, 과립 물질은 이들이 서로 치밀화되고 고정되는 방식으로 고착될 정도로 처리된다. 이러한 처리는 진동 공구를 사용하여, 보철대에 대한 충돌에 의해 또는 다른 방식에 의해 점진적으로 감소하는 진동수를 보철대에 가함으로써 수행하는 것이 바람직하다.
뼈 시멘트는 칼라(7) 아래의 보철대 둘레의 고리 중에 가해져서, 보철대가 햄머에 의한 타격에 의해 과립 물질 층 내로 침투하게 된다. 이러한 방식은 상기 언급한 맞물림, 고정 및 도포/침투 효과를 향상시킨다. 고리 중의 뼈 시멘트는 경화되어 커버(5)를 형성한다.
가장 적합하게는, 보철대의 초기 진동은 거의 수평인 원형 아아크를 따라 수행되는 왕복 운동인 것으로 발견되었다. 이러한 진동은 보철대 공동중의 과립 물질(3)을 부류시켜서, 층 내에 보철대가 빠르게 침투할 수 있게 한다.
본 발명이 골반 관절 보철과 관련하에 기술되었지만, 당업자는 본 발명의 수단 및 방법에 또한 다른 유형의 보철에 적용될 수 있음을 명백히 알 수 있을 것이다.

Claims (6)

  1. 보철대에 마찰 표면이 제공된 관절 보철을 고정시키는 수단으로서, 생물학적으로 적합한 과립 물질의 균질 혼합물을 포함하며, 이러한 과립 물질은 균일한 입자 크기 분포를 갖고, 불규칙적이고 가소성이고, 5㎜이하의 입자크기를 가지며, 보다 높은 진동수의 진동을 받으면 유동화되고, 보다 낮은 진동수의 진동을 받으면 결합됨을 특징으로 하는 수단.
  2. 제1항에 있어서, 과립 물질의 입자의 크기가 0.5 내지 2㎜임을 특징으로 하는 수단.
  3. 제1항 또는 제2항에 있어서, 생물학적으로 적합한 물질이 티탄/티탄 산화물, 탄탈, 니오븀 및 이들의 합금, 또는 바이오세라믹 물질임을 특징으로 하는 수단.
  4. 제1항 또는 제2항에 있어서, 과립 물질 혼합물이 티탄/티탄 산화물로 구성됨을 특징으로 하는 수단.
  5. 제1항 또는 제2항에 있어서, 혼합물이 티탄/티탄 산화물, 니오븀 및 이들의 합금, 또는 바이오세라믹 물질로부터 선택된 물질과, 마쇄된 뼈 성분을 포함하는 인체-내인성 물질이 조합된 과립 물질로 구성됨을 특징으로 하는 수단.
  6. 제1항에 있어서, 과립 물질이 다공성임을 특징으로 하는 수단.
