【発明の属する技術分野】本発明はドイツ特許第196 01
340号（引例により本明細書中に含まれる開示内容を有
する米国特許出願第08/783,476号：特願平9-5808号（特
開平9-289996号）））に記載の大腿骨頸内補装具の変更
である人工股関節用大腿骨頸内補装具に関する。
【０００２】
【従来の技術】最近の数１０年の間に、実質的に２つの
処置法が、痛みのかなりの感覚や関節の重大な機能制限
が生じる元になる壊れた大腿骨頭を処置するために有効
であることが分かっている。
【０００３】第１は、患者に矯正器、すなわち、外部支
持装置を付けることができる。この治療の可能性は、装
飾的観点で完全には受け入れられないことに加えて、治
療技術の観点からも最善ではない。
【０００４】広く受け入れられる他の可能性は、患者に
内補装具、つまり股関節ステム内補装具を付けることで
ある。このステムは、大腿骨の、予め穿孔された骨髄領
域に設置され、そこで、セメントを使用して又は使用せ
ず固定される。次に、このような内補装具により、基端
部側での人工球関節の接続が可能となる。
【０００５】正しく後者の可能性が特許文献に強調して
述べられている。ここでは、一例として、DE-U- 94 12
408 U1を指摘しておこう。
【０００６】特許文献に開示されている種々の股関節ス
テム内補装具のなかで、患者の身体内での長時間安定に
関して極めて有益である幾つかの試みがある。しかし乍
ら、遅かれ早かれ、例えば、１０乃至１５年後に、内補
装具に磨損（摩耗並びに／もしくは損傷）が生じること
が認められ、磨損のために、埋め込んだ内補装具を除去
して新しいものに交換する修正処置が必要となる。確か
に、このことは全く問題がない訳ではない。何故なら
ば、骨髄領域の切除及び除去により最初の移植の際に自
然の骨材のかなりの部分が除去されているからである。
一度除去された骨材が修正処置の際に場合によっては存
在しないことがある。このことは、存命中に少なくとも
１回の修正処置を受けねばならないことが最初から想定
できる患者が比較的若い場合には、特に影響がある。
【０００７】公知の股関節ステム内補装具よりも自然の
骨材の切除が僅かですむ内補装具はDE-A-27 24 234 C2
から知られている。この公報に開示されている補装具部
分は、ステムが骨髄領域に延出することなく、大腿骨の
上領域で、しかし小転子の下方で埋め込まれる。プレッ
シャプレート補装具とも呼ばれているこの補装具は、実
質的には、基部側のプレッシャプレートによって、除去
された股関節頭の領域の、切除される大腿骨の皮質に被
さるように延びている。この補装具は、内補装具と大腿
骨との間のすべての機械的力を大腿骨の皮質層に直接に
加え、このことによって、海綿体への許容できないと感
じられる機械的応力を避けるように、大腿骨に横方向に
接触しているプレッシャプレートで支えられる。少量の
骨セメントのみがプレッシャプレートを基端部で取着す
るために必要である。
【０００８】上述したように、この補装具の原理は、大
腿骨の海綿体ができる限り小さい荷重を受けるようにす
るという仮定を基礎にしている。このことは、大腿骨の
皮質上への極端に大きい荷重によって、つまりは、全体
の力が皮質上に掛けられるのではなく、皮質中に導入さ
れることによって、得られる。このことは、今日の知識
によれば、自然の荷重状況に全然対応していない。
【０００９】WO 89/11837 は、更に、全く似たような作
動をする補装具を示している。この公報では、前述の刊
行物に記載の基部側のプレッシャプレートの機能が、特
別に形成された股関節頭によって果たされ、この股関節
頭に内部凹所が設けられ、切除された大腿骨断端は、圧
力の発生と共に、内側で凹所に接触している。
【００１０】見たところでは主負荷を大腿骨の海綿体に
よって果たされる２つの補装具は、FR 26 26 169 A1 及
びFR 26 74 122 A1 から公知である。２つのうちの第１
に挙げた公報では、基端部にプラグ円錐部を具備するね
じボルトを海綿体に捩じ込むことが示されている。この
ことは、極めて短い時間内に、重大な不安定の問題にな
るだろう。２つのうちの第２の公報では、プラグ円錐部
を支持するプレートは一連の骨ねじによって取着され、
これらの骨ねじは海綿体に達している。このことからも
重大な安定性の問題が生じる。何故ならば、海綿体は、
本来、大腿骨の皮質に比較して僅かな点荷重しか受けな
いからである。
【００１１】
【発明が解決しようとする課題】本発明の課題は、大腿
骨への力の導入に関して、前記補装具よりもかなり良好
に自然の状態に適合し、自然の骨材の切除が比較的僅か
で済む人工股関節用大腿骨頸内補装具を提供することで
ある。
【００１２】
【発明が解決しようとする課題】上記課題は、以下の構
成要件（ａ），（ｂ）を有する大腿骨頸内補装具により
解決される。
【００１３】（ａ）大転子の下方の、大腿骨の上領域に
おいてセメントなしに移植可能なシェルであり、このシ
ェルの基端部は人工球関節を収容するアダプタに接続可
能であり、シェルの外側は、少なくとも部分的に、粗目
の三次元の空間格子構造体で覆われている。
【００１４】（ｂ）尾方向に湾曲され、即ち、大腿骨の
長手方向の軸に対して所定角度を有して、大腿骨の一側
から他側に向かって延びており、その先端が下方に向か
って湾曲して延びたステム端部として形成され、少なく
とも部分的に、粗目の三次元の空間格子構造体で覆われ
ているシェル端部。
【００１５】ドイツ特許第196 01 340号とは異なり、本
発明では、大腿骨の皮質を貫通する引張りボルトと協働
する引張りプレートが設けられていない。この貫通によ
って、大腿骨の弱体化が不可避的に生じる。
【００１６】この場合には、シェルの湾曲端部は、むし
ろ、自然な海綿体の皮質側に横方向に接触している部分
なのである。従って、本発明のインプラントは大腿骨頸
の海綿体に「吊下するように」取着されねばならない。
これに対し、ドイツ特許第196 01 340号に記載のインプ
ラントは引張りプレートの形態をなす固定点又は支点を
有する。
【００１７】この場合には、結果は、大腿骨が外側から
は決して弱体化されないことである。更に、構造はドイ
ツ特許第196 01 340号に記載の構造よりも単純である。
人工関節球の取付は対応のアダプタによって同様になさ
れる。
【００１８】他の好ましい改善は副請求項から生じる。
【００１９】
【発明の実施の形態】以下、図面を参照して本発明の実
施の形態を詳述する。本発明では、シェル１´は、基端
から先端に向かって円錐形をなし、前記特開平9-289996
号に記載のシェルに類似の形状を有する。しかし乍ら、
先端領域で、シェル１´は、先端に向かうのに従って、
即ち、下方に湾曲したステム端部３０´を有し、この湾
曲部は、ステム端部３０´の最も外側の端部が大腿骨の
内部の側方領域に位置するような寸法になっている。即
ち、前記シェル１´は、大腿骨の長手方向の軸（図では
垂直な直線として示されている）に対して所定角度を有
して、大腿骨の一側（図では左側）から他側（図では右
側）に向かって延びており、その先端が下方に向かって
湾曲して延びたステム端部（３０´）として形成されて
いる。即ち、ステムは、大腿骨の長手方向の軸に沿って
は延びていない。
【００２０】シェル１´は、ステム端部３０´と同様
に、粗目の三次元の格子構造体（オープンメッシュ）９
´で少くとも部分的に覆われている。本発明の内補装具
に適した粗目の三次元の格子構造体は、例えば、米国特
許第5,178,201 号及び第5,433,750 号から公知である。
尾及び頭方向に面するシェル１´の外側（一側）での格
子構造体が、例えば約２乃至６ｍｍの編目の大きさを有
する比較的粗い編目を有するのは好ましい。これに対
し、腹方向及び背方向に面するシェル内側（他側）での
格子構造体は、例えば約１乃至２．５ｍｍの編目の大き
さを有する比較的細い編目で形成されるのが好ましい。
【００２１】人工球関節２０´は、特開平9-289996号に
規定されているのと同じ方法で、二重プラグ円錐部とし
て形成されているアダプタ２´と結合されている。この
二重プラグ円錐部は、二重円錐部の共通のベースの回り
に環状のフランジ１１´を有するのは好ましい。シェル
１´の基端部８´は円錐スリーブ１０´を有し、円錐ス
リーブ１０´の形状は圧力嵌めでアダプタ２´を受け入
れて保持するための二重プラグ円錐部の一端に対応して
いる。フランジ１１´の、外側に向いた面は、少なくと
も部分的に、粗目の三次元の空間格子構造体１２´で覆
われていても良い。
【００２２】はっきり分かるように、インプラントのど
の部分も大腿骨４０´の皮質を貫通していないので、こ
の領域では、骨材の弱体化が全然生じない。本発明の大
腿骨頸内補装具を用いることができる場合において、よ
り安定な大腿骨が結果として生じる。
【００２３】発明の概念から逸れないで上記実施の形態
を変更することは当業者には明らかである。本発明が、
開示された特定の実施の形態に限定されることはなく、
請求の範囲によって規定された本発明の精神及び範囲内
での変更をも包含するものであることは理解される。

    (57)【特許請求の範囲】
  1. 【請求項１】 大転子の下方の、大腿骨（４０´）の上
    領域においてセメントなしに移植可能なシェル（１´）
    を有し、このシェル（１´）の基端部（８´）は人工球
    関節（２０´）を収容したアダプタ（２´）に接続可能
    であり、前記シェル（１´）は、大腿骨の長手方向の軸
    に対して所定角度を有して、大腿骨の一側から他側に向
    かって延びており、その先端が下方に向かって湾曲して
    延びたステム端部（３０´）として形成され、前記シェ
    ル（１´）の外側及びこのシェル（１´）の前記ステム
    端部（３０´）は、少なくとも部分的に、粗目の三次元
    の空間格子構造体（９´）で覆われてなる人工股関節用
    大腿骨頸内補装具。
  2. 【請求項２】 前記シェル（１´）は、その基端部（８
    ´）から、湾曲したステム端部（３０´）の根元まで錐
    状にテーパをなして形成されている請求項１に記載の人
    工股関節用大腿骨頸内補装具。
  3. 【請求項３】 前記空間格子構造体（９´）は、シェル
    外側の一側で、約２乃至６ｍｍの編目の大きさを有する
    粗目で形成されている請求項１に記載の人工股関節用大
    腿骨頸内補装具。
  4. 【請求項４】 前記空間格子構造体（９´）は、前記シ
    ェル外側の他側で、約１乃至２．５ｍｍの編目の大きさ
    を有する細目で形成されている請求項１に記載の人工股
    関節用大腿骨頸内補装具。
  5. 【請求項５】 前記球関節（２０´）用の前記アダプタ
    （２´）は、実質的に、二重円錐部の共通のベースの回
    りに環状のフランジ（１１´）を備えた二重円錐部とし
    て形成されており、二重プラグ円錐部の一端に形状が対
    応している円錐スリーブ（１０´）が、前記シェル（１
    ´）の基端領域に設けられている請求項１に記載の人工
    股関節用大腿骨頸内補装具。
  6. 【請求項６】 前記フランジ（１１´）の、外側に向い
    た面は、少なくとも部分的に、粗目の三次元の空間格子
    構造体（１２´）で覆われている請求項５に記載の人工
    股関節用大腿骨頸内補装具。
