Abstract
HMGB1 단백질의 일부로 이루어지는 적절한 길이를 가지는 단편 펩티드를 합성하고, 그 펩티드가 심근경색 치료 효과를 나타내는 것을 확인하였다.
Description
본 발명은 HMGB1의 단편 펩티드를 포함하는, 심근경색의 신규 치료용 의약 조성물 및 그 사용에 관한 것이다.
관상동맥의 폐색에 의해 심근괴사를 초래하는 급성 심근경색은 예후 불량인 질환으로, 우리나라의 제2위 사망 원인인 심장병의 주요 기초 질환이 되고 있다. 현행 치료법인 급성기 카테테르 치료나 관동맥 바이패스 수술은 심근장해를 경감하여 사망률 저하에 기여하지만, 두절된 관혈류가 재개되었을 때에 발생하는 심근장해(허혈 재환류 장해) 등의 문제가 있어 추가적인 치료법 개발이 요구된다.
최근에 골수 중간엽 줄기세포 이식이 이식 세포의 심근 구성 세포로의 직접 분화에 의한 심근 재생, 생산 사이토카인의 파라크라인 효과에 의한 좌심실 리모델링 억제를 통해 심근경색의 진전을 억제하는 것이 동물 모델을 이용한 기초 실험에서 보고되고, 현재 다양한 투여 경로에서의 자가 세포 이식의 임상시험이 행해지고 있다. 그러나 자가 세포 이식의 문제로서 세포 채취시의 신체적 부담, 세포 배양에서의 비용·인력 문제, 세포 이식까지 시간을 필요로 하는 점 등 앞으로 극복해야 할 과제도 많다. 한편, 최근 손상 조직이 골수 다능성 줄기세포 동원 인자를 혈중에 방출하여 손상부 조직 재생이 유도되는 메커니즘이 확인되고 있다. 골수 줄기세포 유도 인자 투여에 의한 생체 내 손상 조직 재생 유도 메커니즘을 이용한 조직 재생은 종래에 없는 재생 의료의 새로운 발상으로, 줄기세포를 이용한 세포 치료에 비해 인력이 필요 없어 안정 공급이 가능, 더욱이 손상 초기에의 투여가 가능하다는 이점을 가지고 있다고 생각된다.
본 발명자들은 이전의 연구에서 새로운 골수 다능성 줄기세포 동원 인자로서 HMGB1 단백질을 동정(同定)하였다. HMGB1은 비히스톤 핵단백의 주요 성분임과 동시에 손상 부위에 집적되는 수상 세포, 대식 세포 혹은 괴사 세포로부터 세포 밖으로 방출되어 다양한 질환과의 관련이 확인되어 있다.

최근 뇌경색 등의 질환에서 손상된 조직이 골수 다능성 줄기세포 동원 인자를 혈중에 방출함으로써 그 손상부의 조직 재생을 유도한다는 메커니즘이 존재하고, 그 메커니즘이 손상의 추가적인 확대 방지에 기여하고 있는 것이 확인되었다. 그래서, 본 발명자들은 새로운 골수 다능성 줄기세포 동원 인자로서 동정된 HMGB1 단백질 유래의 단편 펩티드를 이용한 심근경색의 신규 치료제 개발을 본 발명의 과제로 하였다.
본 발명자들은 세포 이동 활성을 가지는 HMGB1 단백질의 단편 펩티드(1-44)를 작성하고, 그 단편 펩티드를 전신 투여함으로써 심근경색 모델에서 경색 부위 및 그 근방에 PDGFRα-양성 세포의 집적이 촉진되는 것을 처음으로 밝혔다. 나아가 그 단편 펩티드(1-44) 투여군의 심근경색 모델 동물에서 장기간에 걸친 심장 기능의 개선 효과가 있는 것을 밝혔다. 이는 그 단편 펩티드(1-44)에 의해 생체 내의 손상 조직 재생 유도 메커니즘을 이용한 조직 재생이 유도되어 급성 심근경색의 진전을 억제할 수 있는 것을 시사하고 있다. 따라서, 본 발명의 단편 펩티드가 예후 불량인 인간 급성 심근경색에 대한 유용한 치료제가 될 수 있을 가능성이 나타났다.
본 발명에서는 HMGB1의 단편 펩티드를 포함하는, 심근경색의 신규 치료용 의약 조성물 및 그 사용을 개시한다.
구체적으로 본 발명자들은 HMGB1 단백질의 1에서 44번째까지의 아미노산으로 이루어지는 펩티드(서열 번호: 3)를 펩티드 합성에 의해 작성하였다. 작성한 각 HMGB1 단편을 심근경색의 치료 효과를 평가할 수 있는 모델 마우스에 투여하여 심근경색에 대한 그 단편의 치료 효과를 확인하였다.
본원은 이 지견에 기초하여 이하의 발명을 제공하는 것이다.

도 1a는 심근경색 작성 7일 후의 경색부(INFARCT), 경계부(BORDER), 비경색부(REMOTE)에서의 PDGFRα-양성 골수 중간엽 줄기세포 수를 나타내는 그래프이다. 단편 펩티드(1-44) 투여군(1-44; N=4)에서는 음성 컨트롤군(CONT; N=4)과 비교하여 PDGFRα-양성 골수 중간엽 줄기세포가 경색·경계·비경색부 모든 영역에서 유의하게 동원되었다(1-WAY ANOVA, *P<0.05 VS. 1-44).
도 1b는 단편 펩티드(1-44) 투여 마우스에서의 경계부에서의 형광 면역 염색상이다. 녹색 형광 양성 세포는 GFP 양성의 골수 유래 세포이고, 또한 적색 형광 양성 세포는 PDGFRα 발현 세포를 표지하고 있다. GFP 양성이고 PDGFRα-양성 세포를 화살표로 나타내었다.

본 발명은 세포 이동 자극 활성을 가지는 HMGB1 단편 펩티드를 함유하는, 심근경색을 치료하기 위해 이용되는 의약 조성물을 제공한다. 본 발명의 심근경색을 치료하기 위해 이용되는 의약 조성물은 본 명세서에 있어서 의약, 약제 또는 약학적 조성물이라고도 표현된다.
본 발명에 있어서, 세포 이동 자극 활성이란 세포 이동을 자극하는 활성을 의미한다. 본 명세서에 있어서, 세포 이동 자극 활성은 세포 이동 유도 활성 또는 세포 유도 활성이라고도 표현된다.
본 발명의 의약 조성물은 투여·첨가 부위를 한정하지 않는다. 즉, 그 조성물은 재생이 필요한 심근경색 부위, 그 경색 부위와는 다른 부위, 혈중 등 어느 부위에 투여되어도 그 효과를 발휘할 수 있다. 예를 들면, 그 조성물을 투여·첨가함으로써 투여·첨가 부위 또는 그 근방 부위에 세포가 동원되어 손상 재생이 유도 또는 촉진된다. 또한 예를 들면, 그 조성물을 경색 부위 또는 그 근방에 투여·첨가함으로써 그 경색 부위에 세포가 동원되어 경색 부위의 재생이 유도 또는 촉진된다. 또한 예를 들면, 그 조성물을 재생이 필요한 조직과는 다른 조직에 투여·첨가함으로써 골수로부터 재생이 필요한 부위에 말초 순환을 통해 골수 세포가 동원되어 재생이 유도 또는 촉진된다. 여기서, 「말초 순환」은 「혈액 순환」, 「말초 순환 혈류」라고도 불린다.
재생이 필요한 조직과 다른 조직으로의 투여는 재생이 필요한 부위 이외의 부위(재생이 필요한 부위와는 다른 부위)에 투여하는 것을 의미한다. 따라서 "재생이 필요한 조직과 다른 조직"은 재생이 필요한 조직과 다른 부위, 재생이 필요한 부위와는 다른 부위, 재생이 필요한 조직에서 떨어진 부위, 재생이 필요한 부위에서 떨어진 부위, 재생이 필요한 부위에서 원위에 있는 부위, 재생이 필요한 조직에서 원위에 있는 조직, 원위부, 원위 조직으로 표현할 수도 있다. 즉, 본 발명의 조성물은 체외로부터 직접 약제를 투여하기 어려운 조직을 재생하기 위해 효과적으로 이용된다. 또한, 재생이 필요한 조직과 다른 조직으로서는 혈액 조직, 근육 조직, 피하 조직, 피내 조직, 복막 등을 예시할 수 있다.
또, 본 발명에서 이동이 자극되는 세포 또는 골수로부터 말초혈액으로 동원되는 세포는 미분화 세포, 및 각종 분화 단계에 있는 세포를 들 수 있는데 이들에 한정되지 않는다. 또한 본 발명에서 이동이 자극되는 세포 또는 골수로부터 말초혈액으로 동원되는 세포로서는 줄기세포, 비줄기세포 등을 들 수 있는데 이들에 한정되지 않는다. 줄기세포는 순환성의 줄기세포, 또는 비순환성의 줄기세포가 포함된다. 비순환성의 줄기세포는 조직에 상주하고 있는 조직 줄기세포를 예시할 수 있다. 순환성의 줄기세포로는 혈액 순환성의 줄기세포를 예시할 수 있다.
또 이동이 자극되는 세포 또는 골수로부터 말초혈액으로 동원되는 세포로는 골수유래세포 또는 조혈계 줄기세포를 포함하는데 이에 한정되지 않는다. 본 명세서에서 "조혈계 줄기세포"는 호중구, 호산구, 호염기구, 림프구, 단핵구, 대식 세포 등을 포함하는 백혈구의 외에, 적혈구, 혈소판, 비만 세포, 수지상 세포 등의 혈구계의 세포로 분화 가능한 줄기세포이다. 마커로 인간에서는 CD34-양성, CD133-양성, 마우스에서는 CD34-음성, c-Kit-양성, Sca-1-양성, 가계 마커(Lineage marker)-음성임이 알려져 있다. 또 조혈계 줄기세포는 배양 접시에서 배양하는 경우 단독으로 배양하기가 어려우므로 스트로마 세포와의 공배양이 필요한 것이 특징이다.
본 명세서에서 "골수 세포"란 골수 내에 존재하는 세포를 의미하는 한편 "골수 유래 세포"는 골수로부터 골수 외로 동원된 "골수 세포"을 의미한다. "골수 세포"는 골수 중에 존재하는 조직 전구 세포 집단을 포함하는 세포를 포함한다. 또한 "골수 유래 세포"는 메소앤지오블라스트(mesoangioblast)를 포함한 세포여도 되고 메소앤지오블라스트를 제외한 세포여도 된다.
조직 전구 세포는 혈액계 이외의 특이 조직 세포로 일방향성 분화 능력을 갖는 미분화 세포라고 정의하고 전술한 바와 같이 중간엽조직, 상피계조직, 신경 조직, 실질 장기, 혈관 내피로 분화 능력을 갖는 미분화 세포를 포함한다.
"골수 세포" 및 "골수 유래 세포"는 조혈계 줄기세포 및 이것에 유래하는 백혈구, 적혈구, 혈소판, 골아세포, 섬유세포 등의 분화 세포, 또는 지금까지 골수 중간엽 줄기세포, 혹은 골수 간질 다능성 줄기세포, 혹은 골수 다능성 줄기세포로 불리는 세포로 대표되는 줄기세포다. 본 명세서에서 "골수 줄기세포"란 골수 내에 존재하는 줄기세포를 의미하는 한편 "골수 유래 줄기세포"는 골수에서 골수 외로 동원된 "골수 줄기세포"를 의미한다. 본 발명에서 이동이 자극되는 세포 또는 골수로부터 말초혈액으로 동원되는 세포로서는 "골수 유래 줄기세포"를 들 수 있지만 이에 한정되지 않는다. "골수 세포" 및 "골수 줄기 세포"는 골수 채취(골수 세포 채취), 혹은 말초 혈액 채혈에 의해 단리할 수 있다. 조혈계 줄기세포는 비부착 세포이지만, "골수 세포" 및 "골수 줄기 세포"의 일부는 골수 채취(골수 세포 채취), 말초 혈액 채혈에 의해 얻어진 혈액 중의 단핵구 분획 세포 배양에 의해 부착 세포로서 얻어진다.
또한 "골수 세포" 및 "골수 줄기 세포"는 중간엽 줄기세포를 포함하고, 골아 세포(분화를 유도하면 칼슘의 침착을 관찰함으로써 특정 가능), 연골 세포(알시안 블루 염색 양성, 사프라닌-O 염색 양성 등으로 특정 가능), 지방 세포(수단 III 염색 양성에서 특정 가능), 또 섬유 아세포, 평활근 세포, 스트로마 세포, 힘줄 세포 등 기타 중간엽 세포, 또 신경 세포, 상피 세포(예를 들어 표피 각화 세포, 장관 상피 세포는 사이토케라틴 패밀리를 발현한다), 혈관 내피 세포로의 분화 능력을 갖는 것이 바람직하다. 분화 후의 세포는 상기 세포에 한정되는 것이 아니라 간장, 신장, 췌장 등의 실질 장기 세포로의 분화 능력도 포함한다.
본 명세서에서 "골수 중간엽 줄기세포", "골수 간질 다능성 세포" 혹은 "골수 다능성 줄기세포"는 골수 내에 존재하는 세포로서 골수로부터 직접 또는 다른 조직(혈액, 피부, 지방, 및 기타 조직)으로부터 간접적으로 채취되어 배양 접시(플라스틱 또는 유리 제품)에 부착 세포로서 배양, 증식 가능하며, 뼈, 연골, 지방 등 중간엽 조직(간충직계 줄기세포) 혹은 골격근, 심근, 나아가 신경 조직, 및 상피 조직(다능성 줄기세포)로의 분화 능력을 갖는다는 특징을 갖는 세포이며, 골수 세포 채취에 의해 취득할 수 있는 세포이다.
한편, 골수로부터 골수 밖으로 동원된 "골수 유래 골수 중간엽 줄기세포", "골수 유래 골수 간질 다능성 세포" 혹은 "골수 유래 골수 다능성 줄기세포"는 말초 혈액 채혈, 나아가 지방 등 간충직 조직, 피부 등 상피 조직, 뇌 등의 신경 조직으로부터 채취에 의해 취득할 수 있는 세포이다.
또 이들 세포는 채취 후 직접 혹은 한번 배양 접시에 부착시킨 세포를 생체의 손상부에 투여함으로써, 예를 들면 피부를 구성하는 케라틴 세포 등의 상피 조직, 뇌를 구성하는 신경계 조직으로의 분화 능력도 갖는다는 특징을 갖는다.
골수 중간엽 줄기세포, 골수 간질 다능성 줄기세포, 골수 다능성 줄기세포, 혹은 골수로부터 골수 밖으로 동원된 이들 세포는 골아 세포(분화를 유도하면 칼슘의 침착을 관찰하는 것 등으로 특정 가능), 연골 세포(알시안 블루 염색 양성, 사프라닌-O 염색 양성 등으로 특정 가능), 지방 세포(수단 III 염색 양성 등으로 특정 가능)외에, 예를 들면 섬유 아세포, 평활근 세포, 골격근 세포, 스트로마 세포, 힘줄 세포 등 기타 중간엽 세포, 신경 세포, 색소 세포, 표피 세포, 모포 세포(사이토케라틴 패밀리, 헤어케라틴 패밀리 등을 발현한다), 상피계 세포(예를 들어 표피 각화 세포, 장관 상피 세포는 사이토케라틴 패밀리 등을 발현한다), 내피 세포, 나아가 간장, 신장, 췌장 등의 실질 장기 세포로 분화하는 능력을 가지는 것이 바람직하지만 분화 후의 세포는 상기 세포들에 한정되는 것은 아니다.
인간 골수 세포 및 인간 골수 유래 세포로는 골수 채취(골수 세포 채취), 말초 혈액 채혈, 지방 채취에 의해 취득하고 직접 혹은 단핵구 분획을 분리 후에 배양해 부착 세포로서 취득할 수 있는 세포가 예시될 수 있지만, 이들에 한정되는 것은 아니다. 인간 골수 세포, 인간 골수 유도 세포의 마커로는 예컨대 PDGFRα-양성, Lin-음성, CD45-음성, CD44-양성, CD90-양성, CD29-양성, Flk-1-음성, CD105-양성, CD73-양성, CD90-양성, CD71-양성, Stro-1-양성, CD106-양성, CD166-양성, CD31-음성의 전부 또는 일부를 예시할 수 있지만 이들로 한정되는 것은 아니다.
또한 마우스 골수 세포, 마우스 골수 유래 세포로는 골수 채취(골수 세포 채취), 말초 혈액 채혈, 지방 채취에 의해 취득하고 직접 혹은 단핵구 분획을 분리 후에 배양해 부착 세포로서 취득할 수 있는 세포가 예시될 수 있지만, 이에 한정되는 것은 아니다. 마우스 골수 세포, 마우스 골수 유래 세포의 마커로는 CD44-양성, PDGFRα-양성, PDGFRβ-양성, CD45-음성, Lin-음성, Sca-1-양성, c-kit-음성, CD90-양성, CD29-양성, Flk-1-음성의 전부 또는 일부를 예시할 수 있지만, 이들로 한정되는 것은 아니다.
본 발명에서 이동이 자극되는 세포로서는 PDGFRα-양성 세포를 들 수 있지만 이에 한정되지 않는다. 또한, 이동이 자극되는 PDGFRα-양성 세포는 특별히 제한되지 않지만, 바람직하게는 골수 유래의 PDGFRα-양성 세포이다. 또, PDGFRα 이외의 마커로는 CD29-양성, CD44-양성, CD90-양성, CD271-양성, CD11b-음성, Flk-1-음성의 전부 또는 일부가 예시하지만, 이들로 한정되는 것은 아니다. PDGFRα-양성 세포로서는 PDGFRα-양성의 골수 유래 세포, PDGFRα-양성의 골수 유래 중간엽 줄기세포, PDGFRα-양성 조직에 상주하고 있는 조직 세포(예를 들면, 섬유 아세포 등을 예시할 수 있다), PDGFRα-양성의 골수 유래 세포로서, 골수 채취(골수 세포 채취), 말초 혈액 채혈에 의해 얻어진 혈액 중의 단핵구 분획 세포 배양에 의해 부착 세포로서 얻어지는 세포 등을 예시할 수 있지만, 이들로 한정되는 것은 아니다.
본 발명에서의 HMGB1 단백질로서는 인간 유래의 HMGB1 단백질로서 서열 번호: 1에 기재된 아미노산 서열을 포함하는 단백질, 그 단백질을 코딩하는 DNA로서 서열 번호: 2에 기재된 염기 서열을 포함하는 DNA를 예시할 수 있지만 이들에 한정되는 것은 아니다.
본 발명에서의 「세포 이동 자극 활성을 가지는 HMGB1 단편 펩티드」는 세포 이동 자극 활성을 가지는 HMGB1 단백질의 일부로 이루어지는 펩티드를 의미한다. 본 발명의 HMGB1 단백질의 일부로 이루어지는 단편 펩티드는 세포 이동 자극 활성을 가지는 한 특별히 제한되지 않지만, 바람직하게는 세포 이동 자극 활성을 가지는 단편 펩티드인 HMGB1 단백질의 1번째에서 44번째까지의 아미노산 서열(서열 번호: 3)을 적어도 포함하는 HMGB1 단편 펩티드이다.
본 발명에서 세포 이동 자극 활성을 가지는 HMGB1 단편 펩티드로서는 이하의 단편을 예시할 수 있지만, 이들에 한정되지 않는다.
본 발명에서 세포 이동 자극 활성을 가지는 HMGB1 단편 펩티드로서는 서열 번호: 3의 아미노산 서열을 포함하는 펩티드인 세포 이동 자극 활성을 가지는 HMGB1 단편 펩티드를 들 수 있다. 이들 단편 펩티드로서는 예를 들어 HMGB1 단편 펩티드(1-44)를 적어도 포함하고 HMGB1 단백질의 전체길이는 포함하지 않는 임의의 HMGB1 유래의 단편 펩티드를 그 상한으로 하는 펩티드를 들 수 있지만, 이들에 한정되지 않는다.
본 발명에서 세포 이동 자극 활성을 가지는 HMGB1 단편 펩티드로서는 HMGB1 단편 펩티드(1-44)를 들 수 있다.
본 발명의 조성물의 투여 방법은 경구 투여 및 비경구 투여를 포함하고 비경구 투여 방법으로서는 구체적으로는 주사 투여, 경비 투여, 경폐 투여, 경피 투여 등을 들 수 있지만 이들에 한정되지 않는다. 주사 투여의 예로서 정맥 내 주사, 근육 내 주사, 복강 내 주사, 피하 주사 등에 의해서 본 발명의 조성물을 전신 또는 국부적(예를 들면, 피하, 피내, 피부 표면, 안구나 안검 결막, 비강 점막, 구강 내 및 소화관 점막, 질과 자궁 내 점막, 혹은 손상 부위 등)으로 투여할 수 있다.
또한 본 발명의 조성물의 투여 방법으로서는 예를 들면, 혈관내 투여(동맥내 투여, 정맥내 투여 등), 혈액내 투여, 근육내 투여, 피하 투여, 피내 투여, 복강내 투여를 예시할 수 있지만 이런 것들로 한정되지 않는다.
또 투여 부위에 제한은 없으며 재생이 필요한 조직 부위 혹은 그 근방, 재생이 필요한 조직과 다른 위치, 또는 재생이 필요한 조직에 대해 원위이고 다른 장소인 부위를 예시할 수 있다. 예를 들면, 혈중(동맥내, 정맥 내 등), 근육, 피하, 피내, 복강 등을 들 수 있는데 이들에 한정되지 않는다.
또 환자의 연령, 증상에 따라 적절히 투여 방법을 선택할 수 있다. 본 발명의 펩티드를 투여하는 경우, 예를 들면, 1회 투여에 있어서 환자의 체중 1 kg당 0.0000001mg에서 1000mg의 범위에서 선택할 수 있다. 혹은, 예를 들면, 환자당 0.00001에서 100000mg/body의 범위에서 투여량을 선택할 수 있다. 본 발명의 펩티드를 분비하는 세포나 그 펩티드를 코딩하는 DNA가 삽입된 유전자 치료용 벡터를 투여하는 경우도 해당 펩티드의 양이 상기 범위 내로 되도록 투여할 수 있다. 그러나 본 발명의 의약 조성물은 이러한 투여량에 제한되는 것은 아니다.
본 발명의 HMGB1 단편 펩티드는 그 펩티드를 코딩하는 DNA를 적당한 발현계에 도입하여 유전자 재조합체(recombinant)로서 얻을 수 있고, 또는 인공적으로 합성할 수도 있다. 또한, 본 발명의 의약 조성물은 통상의 방법에 따라 제제화할 수 있고(예를 들면, Remington's Pharmaceutical Science, latest edition, Mark Publishing Company, Easton, USA), 의약적으로 허용되는 담체나 첨가물을 함께 포함할 수 있다. 예컨대 계면 활성제, 부형제, 착색료, 착향료, 보존료, 안정제, 완충제, 현탁제, 등장화제, 결합제, 붕괴제, 활택제, 유동성 촉진제, 교미제(flavoring agent) 등을 포함하지만, 이들에 제한되지 않고, 기타 상용하는 담체가 적절히 사용될 수 있다. 구체예로는 경질 무수 규산, 유당, 결정 셀룰로스, 만니톨, 전분, 칼멜로스 칼슘, 칼멜로스 나트륨, 하이드록시 프로필 셀룰로오스, 하이드록시프로필메틸셀룰로오스, 폴리비닐아세탈디에틸아미노 아세테이트, 폴리비닐 피롤리돈, 젤라틴, 중쇄지방산 트리그리세라이드, 폴리옥시에틸렌경화캐스터오일 60, 백당, 카르복시메틸 셀룰로오스, 옥수수 전분, 무기 염류 등을 포함한다.
또한 본 명세서에서 인용된 모든 선행 기술 문헌은 참조로서 본 명세서에 편입된다.
본 발명은 하기 실시예를 참조하여 더욱 구체화되지만, 하기 실시예에 한정되는 것은 아니다.
실시예 1
(목적)
마우스 급성기 심근경색 모델을 이용하여 HMGB1 단백질의 단편 펩티드(1-44)의 전신 투여에 의한 치료 효과를 심장 생리학적·병리 조직학적·분자 생물학적 수법을 이용하여 포괄적으로 평가한다.
(재료 및 방법)
골수 키메라마우스는 3주령 마우스(C57BL6-CAG-GFP 트랜스제닉 마우스)의 대퇴/하퇴골에서 골수 세포를 채취하고, 골수 세포 현탁액을 방사선 조사 후(10GY)의 6주령 마우스(C57BL6)에 꼬리 정맥 투여(300ML/MOUSE)함으로써 작성하였다. 골수 이식 후 60일에 전신 마취 하 반듯이 누운 자세로 좌측 제4 늑간을 크게 벌려 관동맥 좌전하행지 근위부를 결찰하고 광범위 심근경색 모델(좌관동맥 전하행지/회선지 폐색 모델)을 작성하였다. HMGB1 단백질의 저분자화시에 골수 중간엽 줄기세포 동원 활성의 유지를 고려한 후에 HMGB1 단백질의 단편 펩티드(1-44)를 개발하였다. 경색 작성 6, 24, 48, 72, 96h 후에 꼬리 정맥에 의해 HMGB1 단백질의 단편 펩티드(1-44) 5μg/300μl 및 PBS 300μl(음성 대조군)을 5회 투여하였다. 과잉량의 케타민 및 자일라진을 투여하여 안락사시킨 후에 심장을 채취하였다. OTC 컴파운드에 조직을 포매하고 두께 5μm의 동결 절편을 작성하였다. 항마우스 PDGFRα 항체를 이용하여 다음과 같이 면역 조직을 행하였다. 1차 항체는 PDGFRα Rabbit anti-PDGF receptor alpha antibody(Abcam사 제품 AB 61219)를 사용하였다. 2차 항체는 Alexa 555 goat anti-rabbit antibody(Molecular Probes), 핵 염색은 4,6-diamino-2-phenylinodole(DAPI, Molecular Probes)를 사용하였다. 촬영에는 공초점 레이저 현미경(FV300, 올림푸스사 제품)을 사용하였다. 조직에 집적된 골수 유래 중간엽 줄기세포의 수에 대해서는 무작위로 선택한 5개의 절편의 전체 영역에 존재하는 GFP와 PDGFRα 모두가 양성 세포의 수를 계측하여 평균값을 산정하였다. 또한, RNeasy Kit(Qiagen사 제품)를 이용하여 심근 조직으로부터 RNA를 추출하고, Omniscript Reverse Transcriptase(Qiagen사 제품)를 이용하여 역전사 반응을 행하여 cDNA를 얻었다. Gene Amp(R)PCR System 9700(Life Technologies Japan사 제품)을 사용하여 전술한 cDNA를 주형으로 한 실시간 PCR을 행하여 TNFα 및 IL-1β의 mRNA량을 GAPDH의 mRNA량과의 비로서 정량하였다. 심기능의 개선 평가는 Vivid 7 echocardiography system과 12-MHZ transducer(General Electric사 제품)를 사용하여 LVDd(Dd: 좌심실 확장 말기 직경), LVDs(Ds: 좌심실 수축 말기 직경), LVEF(EF: 좌심실 구출률)를 계측함으로써 행하였다.
HMGB1 단백질의 단편 펩티드(1-44)의 전신 투여에 의해 PDGFRα-양성 골수 유래 중간엽 세포는 경색 작성 7일째의 비경색부 및 경계·경색부의 모든 영역에 유의하게 동원되었다(비경색부: HMGBI 단백의 부분 단편(1-44 펩티드), 52±11#*; 음성 대조군, 32±9 CELLS/MM2, N=4 EACH, #P<0.05 VS. 음성 대조군)(도 1a). 또한, HMGB1 단백질의 단편 펩티드(1-44)에서는 비경색부의 염증성 사이토카인(TNFα, IL-1β)이 상당히 낮은 값이었다(TNFα: HMGB1 단백질의 단편 펩티드(1-44), 4±2#*; 음성 대조군, 11±3 GAPDH; N=4 EACH, #P<0.05 VS. 음성 대조군), (IL-1β: HMGBI 단백의 부분 단편(1-44 펩티드), 2±1#*; 음성 컨트롤, 6±1 GAPDH; N=4 EACH, #P<0.05 VS. 음성 컨트롤)(도 1c, d). 56일째의 심장 초음파 검사상, HMGB1 단백질의 단편 펩티드(1-44) 군에서는 심근경색 후의 좌심실 수축 기능 장해가 유의하게 억제되었다(EF: HMGB1 단백질의 단편 펩티드(1-44)(N=8), 26±4#*; 음성 대조군(N=16), 20±4%, #P<0.05 VS. 음성 대조군). 또한, HMGB1 단백질의 단편 펩티드(1-44) 군에서는 좌심실 확대가 유의하게 억제되었다(DD: HMGB1 단백질의 단편 펩티드(1-44)(N=8), 5.8±0.2#*, 음성 대조군(N=16), 6.3±0.2MM, #P<0.05 VS. 음성 대조군)(도 1e).
본 검토에서는 HMGB1 단백질의 단편 펩티드(1-44)의 5일간 투여에 의해 PDGFRα-양성 골수 중간엽 줄기세포가 경색 작성 후 급성기(7일째)의 경색 부위 및 그 근방 영역에 동원되는 것을 증명하였다(도 1a, b). 특히, 단편 펩티드(1-44)의 투여에 의해 PDGFRα-양성 중간엽 줄기세포의 비경색부 심근 조직으로의 동원을 촉진하는 것이 나타났다. 골수 중간엽 줄기세포에는 염증 억제 작용이나 조직 재생 작용이 알려져 있고, 본 실시예에서도 염증 반응을 억제하는(도 1c, d) 것이 인정되었다. 또한, HMGB1 단편 펩티드(1-44)의 투여는 좌심실 수축 기능의 악화, 좌심실 확대 및 좌심실 리모델링을 유의하게 억제하고 심근 기능에서도 유의한 장기 개선 효과가 인정되었다(도 1e).
본 발명에 의해 심근경색을 치료하기 위한 PDGFRα-양성 세포의 동원 활성을 유지한 HMGB1 단편 펩티드의 신규 용도가 제공된다. 본 발명의 HMGB1 단편 펩티드는 전체길이가 215 아미노산으로 이루어지는 HMGB1 단백질에 대해 분자량이 약 20퍼센트인 단편 펩티드를 포함한다. 이러한 단편 펩티드는 펩티드 합성기를 사용한 화학적 합성에 의한 생산이 가능하기 때문에 펩티드를 의약품으로서 제조하는 경우 정제 순도의 향상, 안정적 생산, 비용 삭감이 기대된다.
또한, 전체길이의 HMGB1은 내독소(endotoxin)의 일종인 LPS(Lipopolysaccharide)와의 결합 활성을 가지는 것이 알려져 있지만, 의약품에 LPS가 소량이라도 혼입되면 발열 등을 일으켜 자주 심각한 부작용으로 이어지기 때문에 의약품에 LPS의 혼입은 엄격하게 규제되고 있다. HMGB1이 LPS와 친화성을 가지기 때문에 의약품에 혼입된 LPS를 완전히 제거하기는 곤란하다. 그러나 펩티드화에 의해 LPS와의 친화성이 저하되기 때문에 의약품에 LPS의 혼입도 경감할 수 있다고 예상된다. 그 때문에 본 발명에서 특정된 PDGFRα-양성 세포 동원 부분을 포함하는 HMGB1 단편을 이용함으로써, 한층 더 안전한 심근경색 치료용 의약 조성물의 개발이 가능해진다.
재생이 필요한 심근경색 부위 또는 그 근방 부위에 본 발명의 HMGB1 단편 펩티드를 직접 투여함으로써, 그 경색 또는 그 경색에 의해 초래되는 조직 손상의 재생을 유도 또는 촉진할 수 있다. 나아가 정맥 내 투여 등의 방법으로 재생이 필요한 부위와 다른 부위에 본 발명의 HMGB1 단편 펩티드를 투여함으로써 심근경색의 재생을 유도 또는 촉진할 수도 있다. 이와 같이 본 발명에서는 일반 진료에서 널리 시행되고 있는 정맥 투여에 의한 심근경색 치료를 가능하게 하기 때문에 치료제를 임의의 횟수, 임의의 농도로 안전하고 간편하게 투여하는 것이 가능해졌다. 이 사실은 종래의 치료 방법과 비교하여 본원발명이 매우 뛰어난 측면 중 하나이다.
또한, 현재 재생 의료나 세포 치료의 현장에서는 환자 유래의 희소한 골수 다능성 줄기세포를 생체 밖에서 배양하고 증식시킨 후에 치료에 이용하는데 그 배양 과정은 세포 열화(암화나 세균, 바이러스 등의 오염)의 위험을 수반하기 때문에 충분한 안전 관리가 필요하다. 이에 대조적으로 본 발명에 기초하는 치료제는 세포를 체외로 취출하는 공정이나 인공적 조작을 가하는 공정을 포함하지 않기 때문에 비교적 안전성이 높다고 생각된다. 이 사실도 종래의 치료 방법과 비교하여 본원발명이 뛰어난 측면 중 하나라고 할 수 있다.
