Abstract
S1P3(Edg-3) 수용체의 길항제가 결합조직 성장인자(CTGF) 축적을 수반하는 안 질환에서 수용체 신호전달의 하향 조절 및 하류의 CTGF의 생산을 감소시키는 방법에 Smad 신호전달을 약화시키기 위해 제공된다. 부적절한 CTGF 축적을 수반하는 안 질환에는 예컨대, 고안압증, 녹내장, 녹내장성 망막병증, 시각신경병증, 황반변성, 당뇨망막병증, 맥락막 혈관신생, 증식 유리체망막병증 및 눈 상처치유가 포함된다. 이러한 질환들은 본 발명의 길항제를 투여함으로써 치료된다.

본 발명은 결합조직 성장인자(CTGF) 축적을 수반하는 안 질환에서 수용체 신호전달의 하향 조절과 하류의 CTGF의 생산 감소를 목적으로 하는 내피 분화 유전자 서브패밀리 3 수용체의 약화를 위한 조성물 분야에 관련한 것이다.
대부분의 안 질환은 세포증식, 생존, 이동, 분화 및 혈관형성을 포함하는 세포 과정과 관련된다. CTGF는 이러한 세포 과정에서 중심 매개체일 것으로 여겨지는 분비 사이토카인이다. 특히, CTGF는 콜라겐Ⅰ과 섬유결합소(fibronectin)의 침전 증가를 통하여 세포외기질의 생산을 증가시키는 것으로 알려져 있다. CTGF의 과발현은 피부경화증(scleroderma), 섬유증식성 질환(fibroproliferative disease), 그리고 세포외기질 성분의 과축적이 있는 흉터 형성과 같은 상태의 주요 원인 요소로 서 연루된다.
잔기둥그물(trabecular meshwork; TM) 부위에서 세포외기질 물질의 과축적은 특정 형태의 녹내장의 특징이고, 그러한 증가는 방수배출에 대한 저항 증가를 유도함으로써 안압(IOP)의 상승을 유도한다고 여겨진다. Fleenor 등에 의한 WO 03/092584로 2003년 11월 13일 공개되고, 알콘사(Alcon, Inc.)에 양도된 국제특허출원(International Patent Application) 제 PCT/US2003/012521호에는 정상 TM 세포와 비교하여 녹내장성 TM 세포에서 CTGF mRNA의 존재가 증가된 것으로 기술되었다. 따라서, CTGF는 잔기둥그물 세포에 의해서 세포외기질 생산에 관여하는 것으로 보인다.
잔기둥그물(TM)은 내피세포, 결합조직, 그리고 각막과 홍채 사이의 각에 위치하는 세포외기질을 포함하며 정상 안압의 유지를 위해 필요한 정상 저항을 제공하는 복합 조직이다. 적절한 안압은 눈의 모양을 유지하고 안방수가 무혈관 각막 및 수정체로 흐르도록 압력차를 제공하는데 필요하다. 일반적으로 녹내장에서 나타나는 과도한 안압은 시신경에 해로운 영향을 주고, 망막신경절세포와 축삭의 손상을 이끌며, 만약 치료되지 않으면 점진적으로 시력상실 및 실명으로 이어진다. 세계적으로 녹내장은 실명의 주요한 원인의 하나이다.
원발녹내장은 해부학적, 생화학적 또는 생리학적 기본을 이루는 안방수 흐름의 장애에 의한 결과이다. 속발녹내장은 눈의 손상이나 외상 또는 이미 존재하는 질병의 결과로 발생한다. 만성 또는 단순 녹내장으로도 알려진 원발개방각 녹내장(POAG)은 미국에서 모든 원발녹내장의 90 퍼센트를 차지한다. POAG는 눈으로부터 액 배출에 대해 비정상적으로 높은 저항을 가져오는 TM에서의 병리학적 변화가 특징이다. 이러한 저항의 결과는 안압의 증가이다.
프레드니손, 덱사메타손 및 하이드로코르티손과 같은 특정 약제들은 안압을 증가시킴으로서 녹내장을 유도한다고 알려져 있다. 더욱이, 투여 방식이 안압에 영향을 주는 것으로 보인다. 예를 들어, 덱사메타손의 점안투여는 전신투여했을 때보다 더 높은 안압의 증가를 이끈다. 스테로이드의 투여로 발생한 녹내장은 스테로이드-유도 녹내장이라고 한다.
현재 항 녹내장 요법들은 액 배출의 증가를 위한 레이저 섬유주성형술 또는 섬유주절제술과 같은 외과적 처치뿐만 아니라, 안방수 형성을 억제하거나 방수 배출을 유도하는 약물을 사용함으로써 안압을 낮춘다. 약제학적 항 녹내장의 접근은 여러가지 바람직하지 않은 부작용을 나타내어 왔다. 예를 들어, 필로카핀과 같은 축동제는 시야의 흐려짐과 다른 부정적인 국소적 부작용을 유도할 수 있다. 전신적으로 투여된 탄산탈수효소 저해제는 구역, 소화불량, 피로 및 대사산증을 일으킬 수 있다. 또한, 특정 베타-차단제들은 폐조직에서 그들이 베타-2 수용체에 미치는 효과에 기인할 수 있는 폐의 부작용과 관련이 있다. 알파2-작용제는 빠른맥, 부정맥 및 고혈압을 일으킬 수 있다. 이런 부정적인 부작용들은 환자의 순응성 감소 또는 요법의 종료를 초래할 수 있다.
본 명세서에 참고로 인용되는, Fleenor등에 의한 2005년 10월20일 공개된 미국 특허 출원 제 2005/0234075호는, 사람의 잔기둥그물 세포에서 기본적인 CTGF 발현과 TGFβ2-유도 CTGF 발현 모두에 대하여 저해 활성을 갖는 GSK-3와 CDK 저해제 를 제공한다.
황반변성은 고-시력(high-acuity vision)을 가능하게 하는 황반이라는 망막 중심부분의 광수용체의 상실이다. 황반의 변성은 망막색소상피와 혈관맥락막 사이의 막에서 세포외기질 성분의 비정상적인 축적과 관련된다. 이 부스러기같은 물질을 드루젠(drugen)이라고 한다. 드루젠은 안저안구검사에서 발견된다. 정상적인 눈은 황반에 드루젠이 없을 것이나, 망막 주변부에는 드루젠이 풍부할 수 있다. 황반 시력의 어떤 상실도 없을 때 황반에 부드러운 드루젠의 존재는 AMD의 초기 단계로 간주된다.
맥락막 혈관신생은 일반적으로 다른 안 질환들뿐만 아니라 황반변성에서도 발생하고, 맥락막 내피세포의 증식, 세포외기질의 과잉생산, 그리고 섬유혈관 망막하 점막의 형성과 관련된다. 망막색소 상피세포의 증식과 혈관형성인자의 생산은 맥락막 혈관신생에 영향을 미치는 것으로 보인다.
당뇨망막병증은 모세혈관 기저막의 비대와 모세혈관에서 혈관주위세포(pericyte)와 내피세포 사이의 접촉 부족으로 인해 당뇨병에서 발생하는 안 질환이다. 혈관주위세포의 손실은 모세혈관의 누출을 증가시키고 혈액-망막 장막의 파괴를 유도한다.
증식 유리체망막병증은 유리체막 내와 망막의 표면에서 세포막 및 섬유성 막의 세포 증식과 관련이 있다. 망막색소 상피세포의 증식과 이동은 이러한 안 질환에서 일반적이다. 증식 유리체망막병증과 관련된 막은 콜라겐 타입Ⅰ, Ⅱ 및 Ⅳ, 그리고 섬유결합소와 같은 세포외기질 성분을 포함하고, 점차 섬유성으로 된다.

S1P-3 수용체에 대한 길항작용과 결과적인 CTGF 축적의 저해는 또한 인간이나 포유동물에서 안 질환의 개선이 관찰됨으로써 추론된다. 예를 들어, 연령-관련 황반변성에서 시력 상실의 감소와 반전은 CTGF 축적의 저해를 나타내고, 녹내장 환자의 경우 녹내장으로 발전될 위험에 있는 대상에서 낮아진 안압 및 증상 발병의 지연이나 예방은 CTGF 축적의 저해를 나타낸다.

대상:

추가적인 구체적 예로, 안과용 조성물은 길항제를 약 0.1-100 나노몰(nM) 또는, 더욱 구체적으로 1-10nM의 안내 농도를 제공하도록 제형화된다. 국소용 조성물은 숙련된 임상의의 통상적인 판단에 따라서 하루에 1회 내지 4회 안구 표면에 전달된다. 제형의 pH는 4-9, 또는 4.5 내지 7.4이어야 한다. 전신용 제형은 양 10 내지 1000mg의 길항제를 포함할 수 있다.

유전자 발현의 저해
배양된 인간 잔기둥그물 세포에서 Edg3 수용체 길항작용이 CTGF 유전자 발현에 미치는 영향을 다음과 같이 결정할 수 있었다. 형질전환되거나 또는 형질전환되지 않은 인간 TM 세포 배양물((Pang 등, Curr Eye Res, Vol. 13:51-63, 1994; Steely 등, Invest Ophthalmol Vis Sci, Vol. 33:2242-2250, 1992; Wilson 등, Curr Eye Res, Vol. 12:783-793, 1993; Stamer 등, Curr Eye Res, Vol. 14:611-617, 1995)을 일정 기간 동안 자극량의 스핑고신-1-포스페이트 (S1P)로 처리하거나 처리하지 않았고, 또한 Edg3수용체 길항제로 처리하거나 처리하지 않았다. 분리된 배양물을 필요한 희석된 비히클(들)로 처리하여 대조군으로 사용하였다. 다음으로, 제조자의 지시에 따라서 Qiagen RNeasy 96 시스템(Qiagen)를 사용하여 TM 세포로부터 전체 RNA를 분리하였다.
세포 처리 후에 CTGF의 구별되는 발현을 실질적으로 앞서 기술한 대로(Shepard 등, IVOS, Vol. 42:3173, 2001) ABI Prism® 7700 서열 검출 시스템(Applied Biosystems)을 사용한 정량적 실시간 RT-PCR(QRT-PCR)로 확인하였다. CTGF 증폭을 위한 프라이머는 프라이머 발현 소프트웨어(Primer Express software; Applied Biosystems)를 사용하여 디자인하였고, Fleenor 등에 의한 2004년 10월 8일 출원된 미국특허출원 제10/510,585호로서 2005년 10월 20일 공개된 미국 특허 출원 제20050234075호(본 명세서에서 참조로 인용됨)에서 언급한 Genbank accession # NM 001901.1의 인접 엑손에 아닐링하여 76-bp의 앰플리콘을 만들었다. CTGF의 증폭은 Fleenor등에 의한 공개된 미국 특허 출원 제 20050234075호에서 언급된 대로 18S 리보솜 RNA 유전자(GenBank accession #X03205)에 대해 디자인한 프라이머를 사용하여 18S rRNA 발현을 표준화하여, 69-bp 앰플리콘을 만들었다. CTGF QRT-PCR은 40nM 18S 또는 900nM CTGF 프라이머; 100nM 18S 프로브 또는 100nM CTGF; 5㎕ RNA; 1X Multiscribe와 RNase 저해제 혼합물 (ABI); 및 1X TaqMan® Universal 혼합물 (ABI)로 구성된 50㎕ 최종 부피로 18S 프라이머/프로브 세트와 혼합하여 수행하였다. 온도 순환 조건은 48℃에서 30분 그리고 95℃에서 10분, 이후 95℃에서 15초 및 60℃에서 1분의 40회 순환으로 구성하였다. 데이터 분석은 SDS 스프트웨어 버전 1.9.1 (Applied Biosystems)과 MS Excel 2002 (Microsoft)를 사용하여 수행하였다. 상대적인 RNA 농도의 정량은 PE Biosystems User Bulletin #2에 기술된 바와 같이 델타 델타 Ct 방법을 사용하였다. 증폭산물의 수준은 4중 QRT-PCR 분석의 평균값±SEM으로서 나타내었다. 데이터 분석은 SDS 스프트웨어 버전 1.9.1 (Applied Biosystems)과 MS Excel 97 (Microsoft)를 사용하여 수행한다.

-자극 변화의 저해
Edg3 수용체의 길항작용이 배양된 인간 잔기둥그물 세포에서 세포외기질-관련 단백질의 발현에 미치는 영향을 다음과 같이 결정하였다. 인간 TM세포 배양물을 반복 그룹 및/또는 실험 그룹 및/또는 대조군 그룹으로 나누고, 대조용액이나 희석된 비히클(들)을 포함하는 실험 용액(대조군으로서) 및/또는 S1P (자극제로서) 및/또는 Edg3 수용체 길항제를 첨가하였다. 피브로넥틴, 플라스미노겐 활성제 저해제Ⅰ (PAI-Ⅰ), 콜라겐, 피브릴린, 비트로넥틴, 라미닌, 트롬보스폰딘Ⅰ, 프로테오글리칸 또는 인테그린과 같은 세포외기질-관련 단백질의 수준을 표준 효소면역측정법(ELISA)으로 각 세포 배양군에서 측정하였다. 이러한 분석은 당업자들에게 잘 공지되어 있고, 특정 단백질의 검출을 위한 마커로서 항체 또는 항원과 결합하는 효소를 사용하는 민감한 면역분석법이다. 이러한 의미에서, 실험 용액에 미치는 영향을 결정하기 위하여 다양한 세포외기질-관련 단백질 수준의 그룹간 비교가 가능하다.
처리된 인간 TM 세포 배양물로부터 얻어진 상층액에서 Edg3 수용체 길항작용이 PAI-1의 수준에 미치는 영향의 예를 도 2A 및 도 2B에 나타내었다. 본 연구를 위하여, 인간 TM 세포 배양물을 다양한 양의 내인성 Edg 수용체 작용제 S1P의 존재하 및/또는 S1P와 구조적 유사체인 FTY720의 다양한 양의 존재하에서, Edg3 수용체 서브타입 길항제 CAY10444로 처리하거나 또는 처리하지 않았다. 24시간 후, 분비된 PAI-1 단백질의 수준을 처리된 배양물로부터 얻은 상층액 일부로 ELISA를 하여 결정하였다. 이러한 데이터로부터 두 작용제의 효과가 강력하면서 효과적으로 CAY10444에 의해 길항되는 것이 명백하였다(데이터는 평균과 SEM으로 나타내었다).
