Abstract
본 발명은 시험관 내에서 MHC 분자와 결합할 수 있고 항원 처리 없이(예를 들면 아피토프) T 세포에 제시될 수 있는 펩타이드를 제공하는 것으로 이는 마이엘린 희소돌기아교세포 당단백질(MOG) 40-60 잔기 영역의 부분을 포함하는 것이다. 특히 펩타이드는 마이엘린 희소돌기아교세포 당단백질 41-55, 43-57, 44-58 및 45-59에서 선택된 아피토프이다. 또한 본 발명은 상기 펩타이드를 사용하여 질환의 치료 및/또는 예방을 위한 의약 조성물과 이러한 펩타이드의 사용방법을 제공한다.
Description
본 발명은 마이엘린(myelin) 희소돌기아교세포(oligodendrocyte) 당단백질(MOG)로부터 유래된 펩타이드에 관한 것이다. 더욱 상세하게는 본 발명은 MHC 분자와 결합할 수 있고 시험관 내에서 항원 처리 없이 T 세포에 제시될 수 있는 MOG의 40-60 잔기 영역의 일부를 포함하는 펩타이드에 관한 것이다. 본 발명은 질병의 치료 및/또는 예방을 위한 이러한 펩타이드의 용도에 관한 것이다.
다발성 경화증(MS)은 신경 축색의 탈수초(demyelination) 특징을 지니는 중추신경계에 영향을 주는 만성 퇴화성 질환이다. MS는 다양한 신체적 또는 정신적 증상의 원인이 되고 통상 신체적 뿐만 아니라 인지 장애로 진전된다. 질병은 통상 젊은 시기(20-40세)에 발병하고 여성에게 더 흔하게 발병하며 전 세계에 백만 명 이상의 환자를 지니고 있다.
MS의 질병 경과는 유동적이며 일시적으로 정지할 수 있으나 기간 경과에 따라 점차적으로 진전되어진다. MS의 몇 가지 서브타입이 질병의 진전 형태에 따라 구분되어진다.
MS 질환자의 경우 감도 또는 감촉과 같은 감각의 손실, 따끔거림(pricking)또는 지각 감퇴 및 지각 이상과 같은 무감각, 근육 약화, 간헐성 경련, 근육 경련, 또는 움직임의 곤란과 같은 감각의 변화 또는 손실에 기인한 신경학적 증상과 신호로 고통받을 수 있고, 운동 실조증에 따른 조화와 균형의 곤란, 장애로 인한 발성의 곤란 또는 연하 장애로 인한 삼킴의 곤란, 안진 안내섬광 또는 복시를 포함하는 시신경염으로 인한 시각 장애, 피로, 급성 또는 만성 통증, 방광과 장의 곤란에 의한 고통을 겪게 된다.
MS는 면역 관련 장애로 최근 인식되고 있으며 체내의 면역 시스템이 마이엘린을 공격하여 손상시키는 것이다.
MS에 대한 치료법은 아직 알려지지 않았다. 몇 개의 최근 치료법이 공격 후 증상 보존(재발)에 효과적인 것으로 알려지고 있으며 또한 새로운 공격(재발)의 빈도를 예방하거나 감소시키고 장애의 정도를 예방하거나 감소시키는 것으로 알려지고 있다. 그러나 다수의 현재 MS 치료법은 부작용이 발생하고 내성이 있다. 따라서 MS의 증상을 경감시키거나 감소시킬 수 있는 MS의 효과적 치료법에 대한 요구가 존재하고 있다.
본 발명자들은 고정된 항원 제시 세포에 의해 T 세포로 제시될 수 있는 마이엘린 희소돌기아교세포 당단백질로부터 유래된 다수의 펩타이드를 확인하였다. 이러한 펩타이드는 다발성 경화증과 같은 탈수초 질환의 예방 및/또는 처리에 유용한 것이다.
따라서 첫 번째 측면에서 본 발명은 시험관 내에서 MHC 분자와 결합할 수 있고 항원 처리 없이(예를 들면 아피토프) T 세포에 제시될 수 있는 펩타이드를 제공하는 것으로 이는 마이엘린 희소돌기아교세포 당단백질(MOG) 40-60 잔기 영역의 부분을 포함하는 것이다.
펩타이드는 마이엘린 희소돌기아교세포 당단백질 41-59 잔기 영역의 부분을 포함한다.
펩타이드는 MOG 41-55, 43-57, 44-58 및 45-59 마이엘린 희소돌기아교세포 당단백질 부분에서 선택된 것이다.
펩타이드는 MOG 41-55이다.
두 번째 측면에서 본 발명은 발명의 첫 번째 측면에 따른 펩타이드의 탈수초(demyelinating) 질환의 치료 및/또는 예방의 용도를 제공하는 것이다.
세 번째 측면에서 본 발명은 발명의 첫 번째 측면에 따른 하나 또는 그 이상의 펩타이드를 포함하는 의약 조성물을 제공하는 것이다.
네 번째 측면에서 본 발명은 발명의 첫 번째 측면에 따른 펩타이드를 개체(subject)에게 투여하는 단계를 포함하는 이를 요구하는 개체 내에서 탈수초 질환의 치료 및/또는 예방을 위한 방법을 제공하는 것이다.
다섯 번째 측면에서 본 발명은 탈수초 질환의 치료 및/또는 예방을 위한 용도의 의약품을 제조하는 것에 의한 발명의 첫 번째 측면에 따른 펩타이드의 용도에 관한 것이다.
또한 본 발명의 두 번째, 네 번째 및 다섯 번째 측면에 있어서 상기 질환은 다발성 경화증이다.

첫 번째 측면에서 본 발명은 펩타이드에 관한 것이다.
펩타이드
'펩타이드'라는 용어는 일반적인 의미에서 인접한 아미노산 사이의 알파-아미노 및 카르복실 그룹이 펩타이드 본드에 의해 각자가 연결되어 있는 일련의 L-아미노산과 같은 잔기를 의미한다. 이 용어는 변형된 펩타이드와 합성 펩타이드 아날로그를 포함한다.
본 발명의 펩타이드는 시험관 내에서 주조직적합성 복합체(MHC) 클래스 Ⅰ 또는 클래스 Ⅱ 분자와 결합할 수 있는 길이를 지닌 것으로 항원 처리 없이 T 세포에 의해 제시될 수 있는 것이다. 다시 말하면 펩타이드란 MHC 분자의 펩타이드 결합 홈에 의해 직접적으로 결합될 수 있는 것으로 하나 또는 모든 말단에 어떠한 트리밍(trimming)도 필요하지 않는 것이다.
MHC 클래스 Ⅰ 분자에 결합하는 펩타이드는 일반적으로 7∼13의 아미노산 길이 바람직하게는 8∼10의 아미노산 길이를 지닌다. 펩타이드의 결합은 펩타이드의 주 사슬 내 원자와 모든 MHC 클래스 Ⅰ 분자의 펩타이드 결합 홈 내에 불변 위치 사이의 두 개의 말단 접촉에 의해 안정화된다. 아미노산이 결합되는 홈의 말단과 펩타이드의 카르복시 말단 모두에 불변 사이트가 존재한다. 펩타이드 길이의 변형은 펩타이드 골격의 꼬임에 의해 수용될 수 있고 프롤린 또는 글리신 잔기는 원하는 유연성을 부여할 수 있다.
MHC 클래스 Ⅱ 분자에 결합하는 펩타이드는 일반적으로 8∼20의 아미노산 길이 바람직하게는 10∼17의 아미노산 길이를 지닌다. 이러한 펩타이드는 MHC 클래스 Ⅰ 펩타이드-결합 홈과는 달리 양 말단이 개방된 MHC 클래스 Ⅱ 펩타이드-결합 홈을 따라 연장되어 변형(conformation)될 수 있다. 주사슬 원자에 의해 주로 유지되는 펩타이드는 펩타이드-결합 홈 선상의 보존된 잔기와 접촉된다.
본 발명의 펩타이드는 화학적 방법을 이용하여 제조된다(Peptide Chemistry, A practical Textbook. Mikos Bodansky, Springer- Verlag, Berlin.). 예를 들어 펩타이드는 고형상 기술(Roberge JY et al (1995) Science 269: 202-204)에 의해 합성되고, 수지로부터 절단되고, 예비 고성능 액체 크로마토그래피에 의해 정제된다(예를 들어 Creighton (1983) Proteins Structures And Molecular Principles, WH Freeman and Co, New York NY). 자동화 합성은 예를 들어 제조사에 의해 제공된 지침에 따라 ABI 43 1 A Peptide Synthesizer(Perkin Elmer)를 이용하여 달성된다.
또한 펩타이드는 재조합 방법 또는 긴 폴리펩타이드로부터의 절단에 의해 제조된다. 예를 들어 펩타이드는 전체 길이 마이엘린 염기성 단백질로부터의 절단에 의해 수득된다. 펩타이드의 조성은 아미노산 분석 또는 서열분석(예를 들어 Edman 분해 절차)에 의해 확인되었다.
마이엘린 희소돌기아교세포 당단백질(MOG)
마이엘린 희소돌기아교세포 당단백질(MOG)은 단일 세포외 Ig 가변 도메인(Ig-V)을 지니는 타입 Ⅰ 통합 멤브레인 단백질이다. MOG의 아미노산 서열은 동물 종 간에 90% 이상으로 매우 높게 보존되어 있으며 중요한 생물학적 기능의 지표이다. MOG는 희소돌기아교세포 세포 몸통 돌기 뿐만 아니라 수초(myelin sheath)의 가장 외부 박막층(lamellae) 상의 CNS 내에서 특이적으로 발현된다.

아피토프(Apitopes)
적응성 면역 반응에 있어서 T 림프구는 단백질 항원 내부 에피토프를 인식할 수 있다. 항원 제시 세포(APC)는 단백질 항원을 포획하고 짧은 펩타이드 단편으로 분해시킨다. 펩타이드는 세포 내부에서 주조직적합성 복합체(MHC) 클래스 Ⅰ 또는 클래스 Ⅱ 분자에 결합되고 세포 표면으로 운반된다. MHC 분자와 함께 세포 표면에서 제시될 때 펩타이드가 T 세포 에티토프인 경우 T 세포(T 세포 수용체)를 통해 인식되는 것이다.
T 세포 에티토프는 자가 또는 외래에 항원에 대해 적응성 면역 반응의 중심 역할을 한다. 알러지 자가면역질환 이식거부를 포함하는 과민감 질환에서의 T 세포 에티토프의 중심 역할은 실험 모델의 사용을 통해 표시되어 왔다. 애쥬번트와 결합된 T 세포 에티토프의 구조에 기반을 둔 합성 펩타이드의 주입에 의해 염증 또는 알러지 질환을 유발할 수 있다.

Immunol. 28:1251-1261). 또한 관절염, 당뇨병 및 포도막망막염의 실험적 모델(상기 Anderton and Wraith (1998)의 고찰)이다. 이는 또한 EAE 내에서 진행되는 질환의 치료 수단으로 알려져 있다(상기 Anderton and Wraith (1998)).
질병을 치료하거나 예방하기 위한 내성(tolerogenic) 펩타이드의 용도는 상당히 주목되어 왔다. 하나의 이유는 어떤 면역내성 에피토프가 같은 조직 내에서 다른 항원을 위해 T 세포 반응을 하향 조절한다는 것이다. 이러한 현상은 '방관자(bystander) 억제'로 알려져 있으며 이는 주어진 항원 내에서 적어도 하나 이상의 에피토프(바람직하게는 모든 에피토프)에 대한 내성을 유도할 수 있다는 것이다. 이는 특정 면역내성 펩타이드를 이용한다면 하나의 주어진 질환에 대한 하나 이상의 항원에 대해 내성을 유도할 수 있다는 것이다(상기 Anderton and Wraith (1998)). 이는 특정 질환 내에서 모든 병리적 항원을 확인할 필요를 없애주는 것이다.
펩타이드는 상대적으로 낮은 가격으로 인해 치료에 유용한 선택일 수 있다. 또한 펩타이드 동족체는 면역 특성의 변형을 통해 생산될 수 있다. 펩타이드는 MHC 또는 TCR에 대한 그들의 상호반응의 변화를 통해 조절할 수 있다.
이러한 접근법의 하나의 가능한 문제는 T 세포 에피토프로 작용하는 모든 펩타이드가 내성을 유도하지는 않는다는 것이다. 수초 염기성 단백질(MBP) 펩타이드 89-101은 면역 후에 면역우세 항원이며 T 세포 작용성을 위한 프라이밍 및 EAE 유도를 위한 모두의 목적으로 매우 효과적인 면역원이다. 그러나 이 펩타이드는 용해성 형태로 투여시 내성을 유도하는 것은 효과적이지 않다(상기 Anderton and Wraith (1998)).
T 세포 에피토프의 능력에 대한 관찰된 서열을 위한 다수의 설명이 제안되어 왔다(상기 Anderton and Wraith (1998)의 고찰). 특히 HMC에 대한 펩타이드의 친화성과 면역 내성간에 상호관계에 대해 제안되어 왔다(상기 Liu and Wraith (1995)). 그러나 이들은 모두 관찰을 통한 일부만을 나타낼 뿐이다. 예를 들면 MBP[89-101]는 상대적으로 높은 친화성으로 I-AS와 결합하나 관용적이지 않다. 따라서 관용을 유도하는 펩타이드를 쉽게 예측하는 것은 어려운 것이다.
본 발명자들은 만약 적당한 크기를 지닌 펩타이드 에피토프가 항원 처리 없이 미성숙된 APC에 의해 제시된다면 이는 면역학적 내성을 유도할 수 있을 것이라고 설명하고 있다(국제특허 출원번호 PCT/GBOl/03702). 어떤 T 세포 에피토프는 관용적이고 또 다른 에피토프는 내성을 유발하지 않는다는 관찰을 통해 어떤 에피토프는 MHC 분자에 의해 그들이 제시되기 전에 항원 처리를 요한다는 사실에 의해 또한 설명될 수 있는 것이다. 추가적 처리를 요하는 이러한 에피토프는 애쥬번트와 결합하여 주입할 때 질병을 유도하는 능력에 관계없이 용해성 형태로 투여할 때 내성을 유도하지 않는 것이다.
추가적 처리를 요구하지 않는 에피토프는 내성을 유도할 수 있다. 또한 이러한 에피토프를 본 발명자들은 '아피토프(apitope)'(Antigen Processing Independent epiTOPES)라고 칭한다.
항원 처리 독립적 제시 시스템(APIPS)
본 발명의 펩타이드는 시험관 내에서 MHC 분자와 결합할 수 있고 항원 처리 없이 T 세포에 제시될 수 있다.
펩타이드가 '무처리(processing free)' 시스템을 사용하여 항원 처리 없이 MHC 분자에 결합할 수 있는지 여부를 측정하는 것이 가능하다. 이러한 시스템은 T 세포에 MHC 분자를 통해 항원을 제시할 수 있다. 그러나 항원을 처리하는 것은 불가능하다.
따라서 펩타이드는 시험관 내에서 MHC 분자에 결합할 수 있는 능력을 측정할 수 있다. 또한 항원 처리 독립적 제시 시스템(APIPS)을 통해 항원 처리 없이 T 세포에 제시될 수 있다.
APIPS의 실시예로는 다음을 포함한다:
a) 고정된 APC(CD28에 대한 항체 존재 또는 부재);
b) MHC 분자 클래스 I 또는 Ⅱ를 포함한 지질막(CD28에 대한 항체 존재 또는 부재); 및
c) 플레이트-결합 형태의 정제된 천연 또는 재조합 MHC(CD28에 대한 항체 존재 또는 부재).
트렁크화된 펩타이드에 대한 반응을 측정함으로서 T 세포 반응을 조사하기 위한 고정된 APC의 이용은 예를 들면 폴리펩타이드 내 최소 에피토프를 조사하는 연구와 같은 분야에서 잘 알려져 있다(Fairchild et al (1996) Int. Immunol. 8:1035-1043). APC는 예를 들어 포름알데하이드(일반적으로 파라포름알데하이드) 또는 글루타르알데하이드를 이용하여 고정된다.
지질막(평면 막 또는 리포솜임)은 인공 지질을 이용하여 제조하거나 APC 유래의 형질막/미세소체 분획일 수 있다.
이용시 APIPS는 조직 배양 플레이트의 웰에 적용한다. 이후 펩타이드 항원이 첨가되고 APIPS의 MHC 부분에 펩타이드와의 결합은 선별된 T 세포주 또는 클론의 첨가에 의해 검출된다. T 세포주 또는 클론의 활성화는 예를 들어 3H-티미딘 통합 또는 사이토카인 분비를 통해 당분야에 알려진 어떠한 방법으로도 측정할 수 있다.
만약 펩타이드가 APIPS에 의해 T 세포에 제시될 능력을 지닌다면 항원 처리 없이 MHC 분자에 결합될 수 있는 것이며 이는 아피토프이다.
내성(Tolerance)
본 발명의 펩타이드는 마이엘린 희소돌기아교세포 당단백질(MOG)에 내성을 유도할 수 있는 것이다.
본 명세서 내에 사용되는 '내성(tolerogenic)'이라는 용어는 내성을 유도할 수 있다는 의미이다.
내성은 항원에 반응하지 않는 것이다. 자가 항원에 대한 내성은 면역 체계의 필수적인 특징이고 이것이 소실되면 자가면역 질환이 유발될 수 있다. 적응성 면역 체계는 다양한 막대한 감염성 작용제에 반응할 수 있는 능력을 유지하면서 그 자신의 조직 내에 포함된 자가 항원의 자가면역 공격을 회피해야 한다. 이는 대부분 흉선의 아폽토시스성 세포사멸로 미성숙 T 림프구의 민감성(중추 내성)에 의해 상당한 정도로 제어된다. 그러나 모든 자가 항원이 흉선 내에서 탐지되지 않고, 또한 자가-반응성 흉선세포의 사멸도 불완전하게 유지된다. 따라서 말초 조직 내 숙성 자가-반응성 T 림프구에 의해 수득되는 내성(말초 내성) 메커니즘이 존재한다. 중추 및 말초 내성의 메커니즘 고찰은 Anderton et al (1999)(Immunological Reviews 169:123-137)에 기재되어 있다.
내성은 CD4+ T 세포의 일부 이상에서 아네르기의 유도에 의해 유발되고 이를 특징으로 한다. T 세포를 활성화시키기 위해 펩타이드는 T 세포에 2개의 신호를 전달하는 것이 가능한 "전문성(professional)" APC와 연관되어야 한다. 첫 번째 신호(신호 1)는 MHC-펩타이드 복합체에 의해 APC의 세포 표면 상에 전달되고 TCR을 통해 T 세포에 의해 수신된다. 두 번째 신호(신호 2)는 CD80 및 CD86과 같은 동시자극 분자에 의해 APC의 표면 상에 전달되고 CD28에 의해 T 세포의 표면 상에서 수신된다. T 세포가 신호 2의 부재 하에 신호 1을 수신하는 경우 활성화되지 않는 것으로 판단되고, 실제로 아네르기가 된다. 아네르기성 T 세포는 후속 항원 유발에 불응성이고, 다른 면역 반응을 억제하는 것이 가능하다. 아네르기성 T 세포는 T 세포 내성을 매개하는데 관련된 것으로 판단된다.
펩타이드가 성숙된 항원 제시 세포에 의해 처리되어야 하기 때문에 MHC 분자에 의해 제시되기 전에 처리된 펩타이드는 내성을 유도하지 않는다. 성숙된 항원 제시 세포(대식세포, B 세포 및 수지상 세포와 같은)는 항원을 처리할 뿐만 아니라 신호 1 및 2 모두를 T 세포로 전달하여 T 세포 활성화를 유발하는 것이 가능하다. 한편 아피토프는 미성숙 APC 상에서 클래스 Ⅱ MHC에 결합할 수 있을 것이다. 따라서 이들은 동시자극 없이 T 세포에 제시되어 T 세포 아네르기 및 내성을 유발할 수 있을 것이다.
물론 아피토프도 성숙된 APC의 세포 표면에서 MHC 분자에 결합 가능하다. 그러나 면역 체계는 성숙된 APC 보다는 더 많은 미성숙 APC를 포함한다(10% 이하의 수지상 세포가 활성화됨이 제안되었음, Summers et al. (2001) Am. J. Pathol. 159: 285-295). 따라서 아피토프에 대한 디폴트 위치에서 활성화보다는 아네르기/내성이 될 것이다.
펩타이드 투여에 의해 내성이 유도되는 경우 항원-특이적 CD4+ T 세포의 증식 능력이 감소되는 것으로 나타났다. 또한 이들 세포에 의한 IL-2, IFN-γ 및 IL-4 생성은 하향-조절되나 IL-10의 생성은 증가된다. 펩타이드-유도 내성 상태의 마우스 내 IL-10의 중화는 질환에 대한 감수성을 완전하게 회복시키는 것으로 나타났다. 조절된 세포의 대부분은 IL-10을 생성하여 내성 상태를 유지하고 면역 조절을 매개하는 것으로 제시되고 있다(Burkhart et al (1999) Int. Immunol. 11:1625-1634).
따라서 내성 유도는 다음과 같은 다양한 기법에 의해 모니터링 될 수 있다.
(a) 생체 내에서 펩타이드가 목적 에피토프인 질병에 따른 감소된 감수성;
(b) CD4+ T 세포 내 아네르기의 유도(시험관 내 항원에 대한 후속 유발에 의해 검출될 수 있음);
(c) CD4+ T 세포군의 변화
(ⅰ) 증식 감소;
(ⅱ) IL-2, IFN-γ 및 IL-4 생성의 하향-조절; 및
(ⅲ) IL-10 생성의 증가를 포함;
목적 질환
본 발명의 펩타이드는 질병의 치료 및/또는 예방을 위해 사용할 수 있다.
이러한 질병은 탈수초 질환, 예를 들면 부신 백질 이영양증(adrenoleukodystrophy), 백질 질환(white matter disease)의 소멸 또는 다발성 경화증(MS)이다.
본 발명의 펩타이드는 특히 다발성 경화증(MS)의 치료 및/또는 예방에 유용하다. 다발성 경화증(MS)은 중추신경계 백질을 통해 다양한 탈수초 병변 증가에 의해 특징지어지는 만성 염증 질환이다. 이는 또한 다양한 부위에서 수차례 지속 발병한다(McFarlin and McFarland, 1982 New England J. Medicine 307:1183-1188 and 1246-1251). MS는 자가작동 T 세포에 의해 매개되는 것으로 여겨진다.
의약 조성물
두 번째 측면에서 본 발명은 발명의 첫 번째 측면의 하나 또는 그 이상의 펩타이드를 포함하는 의약 조성물에 관한 것이다.
본 발명자들은 '방관자(bystander) 억제'에도 불구하고 내성을 효과적으로 유도하기 위해 상당한 수의 서로 다른 T 세포 클론의 목표가 필요함을 예측하였다. 따라서 다수의 펩타이드를 질병의 예방 또는 치료를 위해 개체에게 투여하는 것이 필요하다.
의약 조성물은 예를 들면 1 내지 20개의 아피토프를 포함하며 바람직하게는 1 내지 15, 2 내지 8 또는 4 내지 6개의 아피토프를 포함한다.
둘 이상의 아피토프를 위해 의약 조성물은 키트의 형태로 제조될 수 있고 아피토프의 일부 또는 각각은 별개로 또는 동시에 제공될 수 있으며 별도로 또는 연속적으로 투여할 수 있다.
선택적으로 만약 의약 조성물 또는 그의 일부가 다중 용량으로 투여된다면 각각의 용량을 서로 별개로 하여 포장할 수 있다.
의약 조성물은 치료 용량 또는 예방 용량에 적합한 각각의 아피토프를 포함할 수 있으며 선택적으로 약제학적으로 허용되는 담체, 희석제 또는 부형제를 포함할 수 있다.
또한 본 발명의 의약 조성물 내에는 각각의 아피토프를 혼합시키기 위해 적절한 결합제, 윤활제, 현탁화제, 코팅제 또는 용해제를 포함할 수 있다.
투여
펩타이드는 애쥬번트 부재하에서 용해성 형태로 투여할 수 있다.
펩타이드는 점막 경로로 투여할 수 있다.
펩타이드에 대한 연구는 용액 형태인 경우 복강 내 투여(i.p.), 정맥 내 투여(i.v.) 또는 비강 내 투여(i.n.) 또는 경구 투여 방식으로 투여하여 T 세포 내성을 유도할 수 있다 (상기 Anderton and Wraith (1998); 상기 Liu and Wraith (1995); Metzler and Wraith (1999) Immunology 97:257-263).
펩타이드는 비강 내 투여할 수 있다.
마우스에 대한 연구는 내성을 유도하기 위한 펩타이드 투여 기간은 투여자 내의 T 세포 전구체 빈도에 의존하는 것으로 나타났다(상기 Burkhart et al (1999)). 다수의 실험 연구에서 반복된 펩타이드의 용량이 내성을 유도하기 위해 요구된다 (상기 Burkhart et al (1999)). 펩타이드의 정확한 용량은 각각의 개체에 의존적이고 바람직한 실시형태에서 다수의 용량을 투여하는 것이다.
만약 다수의 펩타이드를 동시에 투여한다면 이들은 단일 또는 다중 용량의 투여에 적합한 '칵테일' 형태일 수 있다. 선택적으로 다중 용량을 투여하는 것이 바람직하나 용량간의 펩타이드의 상대 농도가 변동될 수 있다.
바람직한 실시형태에서 상승된 농도의 다수의 용량을 환자에게 투여하기 위해서는 '용량의 단계적 상승'을 위한 프로토콜이 필요하다. 이러한 접근법은 예를 들면 봉독 알러지에 대한 면역 치료 적용시 포스포리파아제 A2 펩타이드의 적용과 같은 것이다(Mtiller et al (1998) J. Allergy Clin Immunol. 101 :747-754 and Akdis et al (1998) J. Clin. Invest. 102:98-106).
실시예
다음 실시예들로 본 발명을 더욱 상세히 설명한다. 그러나 하기 실시예는 본 발명을 제한하는 것으로 해석되지 않는다. 본 발명은 이 실시예에 기재된 특정 실시형태에 특히 관련되어 있다.
(실시예 1) 마이엘린 희소돌기아교세포 당단백질(MOG) 내의 아피토프의 확인
재료 및 방법
항원
세포외 도메인을 대표하는 인간 MOG 단백질은 N-말단 부분의 125개 아미노산을 코드화하는 핵산 서열을 박테리아 발현 벡터(pET 시스템)에 클로닝하였다. 상기 재조합 인간 MOG 세포외 도메인(rhMOGED)을 C-말단 상에 His 태그를 붙여 융합 단백질로서 박테리아 시스템 내에서 발현시켰다.
rhMOGED를 포괄하는 15-머의 중복 펩타이드 패널을 표준 F-moc 화학법으로 합성하였다. 15머의 펩타이드는 5개 아미노산에 의해 대체되고 중복되는 10개의 아미노산에 의해 대체될 수 있다.
T 세포 클론의 생성
MOG-특이 하이브리도마 T 세포 클론을 DR2+ 마우스의 면역화에 의해 생성하였다. DR2+ 마우스는 MOG 중복 펩타이드 풀(pool) 또는 완전한 Freunds 애쥬번트(CFA) 내의 rhMOGED와 함께 면역화된다. 10일 후에 비장과 림프절을 제거한다. CD4+ T 세포를 정제하고 시험관 내에서 항원/CFA로 재자극시킨다. 일주일 후에 활성화 항원-특이 세포의 수를 증가시키기 위한 항원과 함께 T 세포를 시험관 내에서 재자극시킨다.
3일 후 활성화된 T 세포를 흉선종(thymoma) 세포주 BW 5147과 함께 융합시킨다. 하이브리도마 클론을 HAT 선별 배지를 사용하여 선별하고 IL-2 ELISA에 의해 펩타이드 특이성을 스크리닝한다.
증식 에세이
신선한 또는 p-프롬알데히드 고정 Mgar(HLA-DR2 +ve)세포를 T 세포와 함께 혈청 내 펩타이드 또는 혈청 단독으로 48시간 인큐베이션시킨다. T 세포 증식 응답을 IL-2 생성에 의해 측정하였다.
결과
인접한(nested) MOG 펩타이드의 제시에 대한 다양한 MOG-특이 하이브리도마 클론의 반응을 도 1에 나타낸다. 펩타이드 MOG 41-55("ROK5")를 더 이상의 처리가 필요 없는 T 세포에 대한 고정화 APC에 의해 제시될 수 있는 아피토프로 정의한다. 상세한 연구를 영역 25-60의 중복 펩타이드를 사용하여 수행하였고 오직 하나의 아미노산에 의해 배치되는 펩타이드를 사용하였다. 펩타이드 MOG 41-55, 43-57, 44-58 및 45-59를 아피토프로 확인하였다(도 1).
이러한 펩타이드의 서열을 표 1에 나타낸다.
(실시예 2) 체외 내성(tolarance) 에세이
아피토프로 확인된 펩타이드가 항원 내성을 유도할 수 있는지 여부를 측정하기 위해서 체외 내성 연구를 수행하였다. HLA-DR2 형질전환 마우스를 100㎍ ROK5(MOG 41-55) 또는 100㎍ MOG 35-55로 면역화시키거나 -8, -4 및 -2일에 PBS로 처리하였다. 처음 날짜인 0일에 CFA와 결합하여 꼬리를 통해 100㎍ MOG 35-55를 주입하여 마우스를 자극시킨다.
10일에 비장과 림프절을 채취하여 분리된 각각의 세포의 농도(population)를 채취하고 MOG 35-55를 사용하여 회상 증식 에세이를 수립시켰다. 각각의 마우스로부터 수득된 세포를 개별적으로 처리하여 데이터를 개별적으로 수득하고 도 2에 나타내었다. 회상 반응을 위해 세포를 체외에서 처리하였으며 이때 MOG35-55의 용량 범위는 100, 10, 1, 0.1, 0.01, 0㎍이었다. 3H- 티미딘 통합으로 증식을 결정하였다. 자극 지수(항원 없는 증식과 비교된)를 각각의 조건에서 계산하였다.
도 2에 나타난 바와 같이 ROK5(MOG 41-55)는 MOG 35-55의 도전에 응답하여 면역을 억제할 수 있는 것이다.
다양한 사이토카인 프로파일 상에서 ROK5 전처리 효과를 또한 조사하였다. 도 3에 나타난 바와 같이 비장 세포에 의한 IFNγ, IL-2, GM-CSF, IL6의 생성이 ROK5 전처리에 명확히 영향받는 것이다.
(실시예 3) ROK5 EAE 예방(prevention)
EAE(실험적 자가면역 뇌척수염)는 널리 연구된 다발성 경화증의 모델이고 MOG를 포함하는 수초(myelin) 또는 수초 성분과 강력한 애쥬번트를 함께 사용하여 면역화함으로써 다양한 범위의 유전적으로 취약한 설치류 및 영장류를 포함하는 실험동물에게 유도될 수 있다.
HLA-DR2 형질전환 마우스 내에 MOG-유도 EAE의 예방을 위해 ROK5의 능력을 테스트하기 위하여 질병 모델을 마우스 스트레인 내에서 수립하였다. 질병을 유도하기 위해 뇌척수염 MOG 펩타이드를 마우스 MOG 35-55(mMOG35-55)에 투입하였다. 그리고 ROK5를 다음과 같이 EAE 예방 실험 내에서 측정하였다.
실험 1 : 3 ROK5 용량 전처리 후 mMOG35-55를 면역화시킴.
실험 2 : 5 ROK5 용량 전처리 후 mMOG35-55를 면역화시킴.
각각의 실험은 그룹 당 10마리의 마우스를 사용하였다.
펩타이드 서열:

인간 및 마우스 MOG는 42 잔기 위치의 아미노산 치환이 상이하다. 즉 마우스의 세린이 인간에서 프롤린으로 치환되어 있다. 모든 MOG35-55 펩타이드가 면역원성을 지니지만 마우스 MOG35-55가 더욱 뇌척수염에 면역원성을 지닌다. 따라서 마우스 MOG35-55를 모델 내에서 EAE를 유도할 수 있는 물질로 선별하였다.
생체 내에서 EAE 예방 프로토콜을 도 4A 및 도 4B에 나타내었다.

2개의 그룹을 DR2 형질전환 마우스를 수립하고 0일째에 한 개의 동물 그룹(n=7)에 100㎍의 mMOG35-55를 Complete Freund's 애쥬번트(CFA)(100㎕, s.c, 꼬리 주사 기반)와 혼합하여 투여하였다. 또한 다른 한 동물 그룹(n=8)에는 200㎍의 mMOG35-55를 CFA(100㎕, s.c, 꼬리 주사 기반)와 혼합하여 투여하였다. 0일째와 2일째에 모든 동물에게 Pertussis 독소(PBS 내 200ng, i.p.)를 투여하였다. 매일 임상 EAE를 수립하고 동물 체중을 측정하였다.
도 5A, 5B 및 하기 표 2에 나타난 바와 같이 모든 용량의 mMOG35-55는 좋은 성적으로 EAE를 유도하였다.

예방 실험:
이러한 기반하에 MOG-유도 EAE를 예방할 수 있는 ROK5의 능력을 시험하였다.
A / 첫 번째 실험
첫 번째 실험은 시험 동물을 3차례 전처리 하였다. (3일 간격으로) 각각의 전처리는 마우스 ROK5, 인간 ROK5 또는 PBS, CFA 100㎍ in 100㎕(옆구리 경피투여)를 시행한 후 모든 동물에게 CFA를 포함하는 200㎍의 mMOG35-55를 꼬리 기반으로 주입하여 면역화시켰다. 0일째 및 2일째에 200ng의 Pertussis 독소(PBS 내 200ng, i.p.)를 투여한다.
매일 임상 EAE의 스코어를 측정하고 동물 체중을 측정하였다. 도 6은 마우스 및 인간 ROK5 아피토프가 MOG에 의해 유도된 질병의 심각함을 감소시키는 것을 나타낸다. 흥미롭게도 인간 ROK5가 더욱 효과적이었다.
도 6A 및 6B에 나타난 바와 같이 실험 1은 마우스 및 인간 ROK5 아피토프 모두가 MOG35-55에 의해 유도된 질병의 심각함을 감소시키는 것을 나타낸다.
B / 두 번째 실험
두 번째 실험은 시험 동물을 5차례 전처리 하였다. (3일 간격으로) 각각의 전처리는 인간 ROK5 또는 PBS, CFA 100㎍ in 100㎕(옆구리 경피투여)를 시행한 후 모든 동물에게 CFA를 포함하는 200㎍의 mMOG35-55를 꼬리 기반으로 주입하여 면역화시켰다. 0일째 및 2일째에 200ng의 Pertussis 독소(PBS 내 200ng, i.p.)를 투여한다. 매일 임상 EAE의 스코어를 측정하였다.
