조성물 및 치료학적 적용
본원의 실시예는 변형된 예정된 세포사멸 단백질 1(PD-1)을 암호화하는 핵산 서열 및 키메릭 항원 수용체(CAR)를 암호화하는 핵산 서열을 포함하는 분리된 핵산 서열에 관한 것이다. 일부 실시예에서, 변형된 PD-1 및 CAR은 별개의 폴리펩티드인 유전자 산물로서 발현된다. 이러한 경우, CAR은 암세포 상에 존재하는 종양 항원에 특이적이며, 상기 암세포는 PD-L1을 발현한다.

실시예는 또한, CAR 및 변형된 PD-1을 발현하는 유전적으로 변형된 세포(예를 들어, T 세포)에 관한 것이다. 일부 실시예에서, 우성 음성(dominant negative) PD-1은 우성 음성 PD-1이 종양 세포의 PD-L1에 의해 유도된 야생형 PD-1 활성을 억제하도록 T 세포에 도입된다. 특정 실시예에서, 유전적으로 변형된 T 세포는 비기능적(non-functional) PD-1을 발현한다. 예를 들어, 유전적으로 변형된 T 세포는 세포내 도메인을 포함하지 않는, 세포내 도메인과 막횡단 도메인 모두를 포함하지 않는, 또는 본 발명에 기술된 것과 같은 점 돌연변이를 포함하는 PD-1 분자를 발현한다.

예를 들어, 본 발명의 유전적으로 변형된 T 세포의 사이토카인 생산에 대한 PD-L1의 억제 효과는 변형된 PD-1을 암호화하는 핵산 서열의 적어도 일부를 포함하지 않는 T 세포의 사이토카인 생산에 대한 PD-L1의 억제 효과를 적어도 5%, 적어도 10%, 적어도 15%, 적어도 20%, 적어도 30%, 적어도 40%, 적어도 50%, 적어도 60%, 적어도 70%, 적어도 80%, 적어도 90% 감소시키는데 충분하다.

일반적으로, 본원에 기술된 것과 같은 활성화되고 증폭된(expanded) 세포는 면역력이 약화된 개체에서 발생하는 질병의 치료 및 예방에 사용될 수 있다. 특히, 본 발명의 조작된 세포는 암의 치료에 사용된다. 특정 실시예에서, 본 발명의 세포는 암 발병의 위험이 있는 환자의 치료에 사용된다. 따라서, 본 발명은 치료학적 유효량의 본 발명의 조작된 T 세포를 이를 필요로 하는 대상체에 투여하는 것을 포함하는 암의 치료 또는 예방을 위한 방법을 제공한다.

본 발명의 약제학적 조성물은 치료될(또는 예방될) 질병에 적절한 방식으로 투여될 수 있다. 적절한 투여량(dosage)은 임상 시험에 의해 결정될 수 있지만, 투여량과 빈도는 환자의 상태, 환자의 질병의 유형과 심각도와 같은 그러한 인자에 의해 결정될 것이다.

상기 CAR은,

막횡단 도메인, 및
CD3-제타(CD3-zeta) 도메인과 하나 이상의 공자극 분자(costimulatory molecules)의 하나 이상의 신호전달 도메인을 포함하고,
상기 표적 세포는 종양 항원 및 예정된 사멸-리간드 1(programmed death-ligand 1, PD-L1)을 발현하고,
