Abstract
본 발명은 하나 이상의 제2 치료제들과 병용된, 제1형 당뇨병, 제2형 당뇨병, 손상된 내당력, 고혈당증, 식후 고혈당증, 과체중, 비만(1단계 비만, 2단계 비만, 3단계 비만, 내장 비만 및 복부 비만) 및 대사 증후군을 포함하는 대사성 장애의 치료에 적합한 하나 이상의 SGLT-2 억제제 화합물(들)로 구성된 약제학적 조성물에 대한 것이다.

Description
본 발명은 다른 활성 성분들과 병용된 나트륨 의존성 글루코즈 공동수송체 2(SGLT-2) 억제제, 이들의 약제학적 조성물, 이를 제조하는 방법 및 대사성 질환, 예를 들면, 제1형 당뇨병, 제2형 당뇨병, 손상된 내당력 및 고혈당증 및 관련 질환의 치료에서의 이들의 용도에 관한 것이다.
대사성 질환 및 수반되는 장애들은 점점 만연될 것이다. 특히, 제2형 당뇨병(T2DM) 대사성 질환은 고빈도의 합병증으로 인해 기대 수명의 뚜렷한 감소를 유발하는 점점 만연하는 질환이다. 당뇨병-수반되는 미세혈관 합병증들로 인해, 제2형 당뇨병은 현재 산업화된 세계에서 성인형 시력 상실, 신부전증 및 절단의 가장 흔한 원인이다. 또한, 제2형 당뇨병의 존재는 심혈관 질환 위험의 2배 내지 5배의 증가와 관련된다.

다양한 작용 모드를 목표로 함에도 불구하고, 처음 3개 물질 부류는 혈당 조절과 관련하여 효능 상 현저히 유사하다(평균 기준선이 ≥ 8.0%인 경우, HbA1c의 절대 감소 ∼1.0-1.5%). 그러나, 이들의 작용 모드가 상이하므로, 이들은 병합 치료로 적합하다.
혈당 조절과 관련한 글루코즈 흡수의 특정 억제제(장 α-글루코시다아제 억제제, 원형 아카르보스)의 효능은 상기 언급된 기타 작용 메카니즘의 것보다 덜하다.
신장 여과 및 글루코즈 재흡수는 기타 메카니즘 중에서도 정상 상태 혈장 글루코즈 농도에 기여함으로써 항당뇨병 목표로서 이용된다. 신장의 상피 세포를 통해 여과된 글루코즈의 재흡수는 나트륨 구배를 따라서 근위 세관에서 브러쉬-보더 막에 위치한 나트륨 의존성 글루코즈 공동수송체(SGLT)를 통해 진행된다(1). 발현 패턴 및 물리-화학적 특성이 상이한 적어도 3개의 SGLT 동형이 있다(2). SGLT2는 신장에서 발현되며(3), 반면 SGLTl은 장, 결장, 골격근 및 심근과 같은 기타 세포(4,5)에서 발현된다. 글루코피라노실옥시-치환된 방향족 그룹 및 이의 제조 및 SGLT-2 억제제로서 이들의 가능한 활성은 공개된 국제 공개공보 제98/31697호, 제01/27128호, 제02/083066호, 제03/099836호, 제2004/063209호, 제2004/080990호, 제2004/013118호, 제2004/052902호, 제2004/052903호 및 미국특허 제2003/0114390호로부터 알려졌으며, 이의 내용이 본 명세서에 인용되어 있다.
따라서, 혈당 조절, 질환-조절하는 특성 및 심혈관 이환율 및 사망률의 감소에 대하여 양호한 효능을 갖는 방법, 약제 및 약제학적 조성물에 대한 충족되지 않은 의학적 요구가 있다.
발명의 개요
본 발명은 대사성 질환의 치료에 적합한 하나 이상의 치료제와 병용되는 하나 이상의 SGLT-2 억제제 화합물(들)을 포함하는 약제학적 조성물에 대한 것이다.
더 특히, 본 발명은 SGLT-2 억제제 화합물이, 대사성 장애의 치료를 위해 본 명세서에서 기술된 활성 성분 B 화합물과 병용되는 본 명세서에서 기술된 활성 성분 A 화합물인 약제학적 조성물에 대한 것이다.

바람직한 양태의 상세한 설명

정의

동물 모델:
본 발명의 범위 내의 상기 언급된 병용물 중의 어느 것은 당해 기술분야에서 알려진 동물 모델에 의해 시험될 수 있다. 다음의 생체 내 실험은 약제학적 조성물의 약리학적으로 관련된 특성을 평가하기에 적합한 본 발명에 따르는 방법을 기술하고 있다.
본 발명에 따르는 약제학적 조성물 및 방법은 유전적으로 고인슐린혈증 또는 당뇨병 동물 예를 들면, db/db 마우스, ob/ob 마우스, 주커(Zucker) 지방 (fa/fa) 래트, 주커 당뇨병 지방 (ZDF) 래트에서 시험될 수 있다. 또한, 이들은 실험적으로 유도된 당뇨병을 갖는 동물 예를 들면, 스트렙토조토신으로 사전 처리된 한위스타(HanWistar) 래트 또는 스프라그 다우리(Sprague Dawley) 래트로 시험될 수 있다.
본 발명에 따르는 병합물의 혈당 조절 효과는 상기 기술된 동물 모델에서 경구 내당력 시험과 병용하여, 대사성 장애만을 치료하기에 적합한 SGLT-2 억제제 화합물(들)과 제2 치료제(들)의 단일 투약 후 시험될 수 있다. 혈중 글루코즈의 시간 경과는 밤새 금식된 동물에게 경구로 글루코즈를 투여한 후에 추적된다. 본 발명에 따르는 병용물은 피크 글루코즈 농도의 감소 또는 글루코즈 AUC의 감소에 의해 측정함으로써 각각의 단일치료제와 비교해 글루코즈 일탈(excursion)이 현저히 향상된다. 또한, SGLT-2 억제제 화합물(들)과 대사성 장애의 치료에 적합한 제2 치료제(들)의 여러 번의 투여 후, 단독으로 또는 상기 기술된 동물 모델과 병용하여 혈당 조절의 효과는 혈중 HbA1c 값을 측정하여 결정될 수 있다.
SGLT-2 억제제 화합물(들)과 대사성 장애를 치료하기에 적합한 제2 치료제(들) 또는 활성 성분 둘 다의 가능한 용량 감소는 상기 기술된 동물 모델과의 병용및 단일치료제의 저 용량의 혈당 조절의 효과에 의해 시험될 수 있다.
본 발명에 따르는 치료용 인슐린으로부터 향상된 독립성은 상기 기술된 동물 모델의 경구 내당력 시험에서 단일 투여 후에 나타낼 수 있다. 혈장 인슐린의 시간 경과는 밤새 금식된 동물에게 글루코즈를 투여한 후에 추적된다.
단일 투여 또는 다수 투여 후 본 발명에 따르는 치료에 의한 활성 GLP-1 수준의 증가는 공복 상태 또는 식후 상태의 상기 기술된 동물 모델의 혈장 중의 GLP-1 수준을 측정하여 결정될 수 있다. 마찬가지로, 혈장 중의 글루카곤의 감소는 동일한 조건 하에서 측정될 수 있다.
β-세포 재생 및 신생에서 대사성 장애를 치료하기에 적합한 SGLT-2 억제제 화합물(들)과 제2 치료제(들)의 병용 효과는 이자 인슐린 함량의 증가를 측정함으로써, 또는 이자 절편의 면역조직 화학염색(immunhistochemical staining) 후에 증대된 β-세포 질량을 생물 형태 계측 분석에 의해 측정함으로써, 또는 분리된 이자섬에서 증가된 글루코즈-자극 인슐린 분비를 측정함으로써 상기 기술된 동물 모델에서 여러 번의 투여 후에 결정될 수 있다.
Claims (15)
- 나트륨 의존성 글루코즈 공동수송체 2(SGLT-2) 억제제 화합물과 제2 치료제를 포함하는, 제2형 당뇨병 치료용 약제학적 배합물(combination)로서,i) 상기 SGLT-2 억제제 화합물이 1-클로로-4-(β-D-글루코피라노스-1-일)-2-[4-((S)-테트라하이드로푸란-3-일옥시)-벤질]-벤젠이고,ii) 상기 제2 치료제가 메트포르민이고,상기 SGLT-2 억제제 화합물 및 상기 제2 치료제가 단일 투여 형태로 존재하고,메트포르민의 1일 용량이 500 내지 2000mg이고 상기 SGLT-2 억제제 화합물의 1일 용량이 10 내지 50mg인, 약제학적 배합물.
- 제1항에 있어서, 상기 제2 치료제의 1일 용량이 500 내지 1000mg인, 약제학적 배합물.
