Abstract
만성 염증성 장질환, 특히 크론병과 궤양성 결장염이 있는 환자의 철분 부족 상태의 경구 치료용 약제의 제조를 위한 탄수화물 또는 그 유도체와의 철(Ⅲ) 착화합물의 용도가 개시되어 있다.
Description
본 발명은 탄수화물 또는 그의 유도체, 특히 덱스트린이나 덱스트린의 산화물의 철(Ⅲ)과의 착화합물의 신규한 치료 용도, 다시 말해 만성 염증성 장질환, 특히 크론병 및/또는 궤양성 결장염 환자의 철분 부족 상태 치료용 약제를 제조하기 위한 철(Ⅲ) 착화합물의 신규한 치료 용도에 관한 것이다.
철분 부족은 세계적으로 가장 빈번히 발생하는 미량 원소 부족이다. 세계적으로 대략 20억의 사람들이 철분 부족 또는 철분 부족성 빈혈로 고생하고 있다 (E. M. DeMaeyer, "Preventing and controlling iron deficiency anaemia through primary health care", World Health Organization, Geneva, 1989, ISBN 92 4 154249 7).
WO 95/35113호에는 면역 부족 질환, 특히 AIDS의 치료를 위한 활성 화합물로서 산화철(Ⅲ)의 용도가 기재되어 있다.
DE 1467980호에는 치료학상 이용 가능한 철분의 주사제류 및 이들의 제조 방법이 기재되어 있다.
US 3076798호에는 비경구 투여에 적합한 철(Ⅲ)-폴리말토오스 착화합물의 제조 방법이 기재되어 있다.
WO 04/037865호에는 철분 부족 상태의 치료 또는 예방을 위한 철-탄수화물 착화합물의 용도가 기재되어 있다.
WO 03/087164호에는 철분 부족 상태의 치료 또는 예방을 위한 수소화 덱스트린과의 철 착화합물들이 기재되어 있다.
WO 02/46241호에는 철분 부족 상태의 치료 또는 예방을 위한 철(Ⅲ)-플루란 착화합물 또는 이들의 용도가 기재되어 있다.
WO 99/48533호에는 철분 부족성 빈혈의 치료를 위한, 개별 분자량이 대략 1000 달톤인 수소화 덱스트린을 함유하는 철-덱스트린 화합물이 기재되어 있다.
마슬로브스키 (I. Maslovski)의 문헌 (American Journal of Hematology, Apr. 2005, vol. 78, no. 4, p. 261-264)에는 분자량이 350,000인 수크로오스 중의 철(Ⅲ) 글루콘산염 착화합물인 페르레시트 (Ferrlecit®) 또는 철(Ⅲ)-수크로오스 착화합물인 베노페르 (Venofer®)의 만성 염증성 장질환으로 고생하는 빈혈 환자들의 정맥 주사 치료에 대한 활성이 기재되어 있다.
보데마르 (G. Bodemar) 등의 문헌 (Scandinavian Journal of Gastroenterology, May 2004, vol. 39, p. 454-458)에는 크론병 및 궤양성 결장염 환자의 빈혈의 정맥 주사 치료를 위한 철(Ⅲ)-수크로오스가 기재되어 있다.
DE-A-102 49 552호에는 말토덱스트린과의 철(Ⅲ) 착화합물 및 이들의 빈혈 치료용 용도 (특히 좋기로는 비경구적 용도)가 기재되어 있다.
CH-A-694 197호에는 빈혈을 치료하는 철(Ⅲ) 폴리말토오스 화합물이 기재되어 있으나, 위장관 내에서의 작용 또는 IBD 또는 크론병에 대한 작용에 대한 지시가 없다.
황산철은 비교적 빈번하게 적절하지 않은 복용량으로 인한 위장 장애 또는 치아 변색 등의 부작용을 일으키는 것으로 알려져 있다. 철염 화합물로부터의 철은 철 자유 이온의 수동(受動) 확산을 받기 쉽다. 철은 순환하여 부작용 또는 철 중독을 일으킬 수 있다. 따라서, 흰쥐의 LD50 농도 역시 230 ㎎ 철/㎏으로 비교적 낮다.
오스키 (Oski) 등의 논문 ("Effect of Iron Therapy on Behavior Performance in Nonanemic, Iron-Deficient Infants", PEDIATRICS 1983; volume 71; 877-880)에는 철 덱스트란의 용도가 기재되어 있다. 철 덱스트란의 비경구적 사용은 덱스트란 유도의 아나팔랙시스 쇼크를 일으킬 수 있기 때문에 불리하다.
염증성 장질환 (IBD)으로서는 장 염증과 끊임없이 재발하는 만성 경과에 의하여 특징되는 일군의 위장관 질환이 있다. 통상 IBD는 임상, 방사선학, 내시경 및 조직학적 기준에 의하여 궤양성 결장염 또는 크론병 중의 하나로 특징되어진다. IBD의 병인이 여전히 정의를 요하고 있으나, 최근의 임상 및 실험 연구들은 이들 질환의 유발 인자 및 병인이 다인성이고, 유전, 환경 및 면역 인자들 간의 상호 작용이 연루되어 있다는 것을 보여주고 있다.
염증성 장질환이 세계적으로 균등하게 퍼져 있는 것은 아니다. 발전이 덜 된 국가와 비교할 때 선진국에서 발병이 증가하는 경향이 뚜렷하다. 유럽에서의 IBD의 발병 사례는 약 100,000명당 390건 정도이다. 약 5억 8000만명의 유럽 인구에 이 수치를 외삽하면 2200만명으로 추정되는 수의 사람들이 IBD에 영향을 받는 것으로 계산된다 (Loftus EV, Jr., Gastroenterology 2004, 126, 11504-1517). 궤양성 결장염 및 크론병은 사춘기 (older adolescent)와 청년층에서 가장 빈번하게 진단되지만, 어떤 연령층에서도 발생할 수 있다.
궤양성 결장염은 통상 직장에 영향을 미치며, 이어서 인접 부위로 확장되어 결장 전부 또는 일부에 영향을 미치는 점막 질환이다. 영향을 받지 않은 점막이 남지 않을 때까지 확산이 계속된다. 궤양성 결장염의 주요 증상은 심한 설사, 직장 출혈, 점액 분비 및 경련형 복부 통증이다. 증상의 경중도는 그 질환의 정도와 관계있다.
크론병은 입에서부터 항문에 이르는 위장관의 어떤 부위에도 영향을 미칠 수 있으나, 소장 및/또는 결장과 가장 빈번한 관계가 있다. 염증은 전층에, 그리고 드문드문 발생하여, 질환이 있는 장 부위들 사이에는 정상적인 부위가 존재한다. 염증의 결과로서는 장의 다른 루프, 방광, 질 또는 항문 주변 피부 위의 누관, 복부 또는 항문 주변 종기 및 장협착을 들 수 있다. 상기 질환 부위 및 진행은 임상 소견에 영향을 미친다. 가장 빈번히 나타나는 증상은 설사, 경련형 복부 통증, 발열, 식욕 감퇴 및 체중 감소이다.
궤양성 결장염과 크론병의 장외 증상은 눈, 피부 및 관절 등의 여러 기관계 및 간과 쓸개를 포함하는 위장 기관과 관계가 있을 수 있다.
치료로는 항염증제 및 어떤 경우에는 항생제의 투여와 식이요법의 변화를 들 수 있다. 때로는 수술을 요할 수도 있다. 또한, 한편으로는, 역시 유발 인자인 스트레스의 제어를 위하여, 또 다른 한편으로는 종종 만성적인 재발 증상의 결과로서 발생하는 우울증의 치료를 위하여, 때때로 심리 요법이 취해진다 (예컨대, Pschyrembel, Klinisches Worterbuch, 256th edition, de Gruyter, p.302/303, p. 443; http://familydoctor.org or http://www.mayoclinic.com 참조).
철분 부족은 종종 만성 염증성 장질환이 있는 환자에게서 합병증으로서 발생한다. 만성 장출혈 때문에 음식에 의하여 섭취되는 양보다 손실되는 철의 양이 더 많게 될 수 있다. 통상의 경구 철분 제제들, 일반적으로 철(Ⅱ)염들은 종종 심각한 위장 부작용을 일으키어, 환자 순응도가 나쁘다. 철분 경구 치료법은 활성 산소종의 형성을 촉매하여 장 조직의 병소를 악화시킬 수 있다. 자유 철은 활성 산소종 형성의 잠재적인 촉매이기 때문에, 철(Ⅱ) 경구 치료법은 만성적인 염증성 장질환이 있는 환자에게 심지어 해가 되는 수도 있다. 철(Ⅱ) 경구 제제들은 잘 흡수되지 않으므로 배설물 철 농도가 높아지고, 유의적인 함량의 배설물 철은 촉매 활성에 이용 가능하다. 철이 염증이 있는 장점액과 접촉하면, 활성 산소종의 생성을 증가시킬 수 있고, 그 결과로 조직 손상을 악화시키게 될 것이다. 그러므로, 만성적인 염증성 장질환이 있는 환자가 내용력(耐容力)이 있는 철분 제제들을 용이하게 입수할 수 있게 하는 것이 특히 중요하다.
철(Ⅲ)-폴리말토오스 착화합물은 독성이 적은 비이온 형태의 철을 함유한다. 이러한 형태의 화합물을 투여시 부작용이 덜 발생하여, 황산철(Ⅱ)과 비교시 환자 순응도가 개선된다 (Jacobs, P., Wood, L., Bird, AR., Hematol. 2000, 5:77-83). 그러나 여전히 만성적인 염증성 장질환이 있는 환자에 있어서 철(Ⅲ)-말토오스 착화합물의 이용에 관한 어떤 실험이나 보고도 없다.
그러므로, 본 발명자들은 만성 염증성 장질환이 있는 환자의 철분 부족 상태의 치료에 적합한, 내용력이 있는 철 화합물을 용이하게 발견하는 것을 목적으로 하였다.
본 발명자들은 탄수화물, 특히 폴리말토오스 (말토덱스트린)와의 철(Ⅲ) 착화합물이 특히 내성이 있고 환자 순응도가 높다는 연구로 증명할 수 있었다. 이 연구에 있어서, 황산철(Ⅱ)에 의한 치료시 지질 과산화 마커인 혈장 말론디알데히드의 유의적인 증가가 관찰되었던 것과 반대로, 철(Ⅲ) 착화합물에 의한 치료시에는 어떠한 산화성 스트레스도 발생하지 않는다는 것은 놀라운 사실이었다.
산화성 스트레스, 특히 지질 과산화는 심근경색증, 암 및 아테롬성 동맥 경화증 발병 위험의 증가와 관련이 있다. 저밀도 지단백질 (LDL)의 산화 변형은 아테롬성 동맥 경화증에 원인이 있다. 문헌 (Tuomainen et . al., Nutrition Research, vol. 19, no. 8, pp. 1121-1132, 1999)을 참조. 철(Ⅲ)-폴리말토오스 착화합물은 실제로 페리틴 농도를 서서히 증가시키지만, 헤모글로빈의 합성에 더 효율적으로 이용된다 (T.-P. Tuomainen et al., loc. cit., p. 1127). 본 발명자들은 이러한 결과들을 기초로 하여 본 발명을 제공하였다.
그러므로, 본 발명은 탄수화물 또는 그의 유도체와의 철(Ⅲ) 착화합물의 만성 염증성 장질환이 있는 환자의 철분 부족 상태를 치료하는 약제 제조를 위한 용도를 제공한다.
본 발명에 따르면, 철분 부족 상태는 혈장 중의 헤모글로빈, 철 및 페리틴 수치가 감소되고 트랜스페린의 수치가 증가하는 상태를 의미하는 것으로서, 이는 트랜스페린 포화도를 감소시키는 것으로 이해된다.
본 발명에 따라 치료하여야 할 상태로는 철분 부족으로 인한 빈혈 및 빈혈 없는 철분 부족을 들 수 있다. 이러한 분류는 예컨대 헤모글로빈 수치 및 트랜스페린 포화도(%)에 의하여 행할 수 있다. 헤모글로빈의 대조치는 유식 세포 분석법 또는 시아노헤모글로빈 광도 측정법으로 측정되고, 철, 페리틴 및 트랜스페린의 대조치는 예컨대 레퍼런스 뱅크 [charity Institut fur Laboratoriumsmedizin und Pathobiochemie (http://www.charite.de/ilp/routine/parameter.html)] 및 토마스의 저서 (Labor und Diagnose, TH Book Verlagsgesellschaft, Frankfurt/Main 1998)에 열거되어 있다. 철분이 부족하지 않은 환자의 트랜스페린 포화도는 >16%를 기준으로 하고 있다. 정상치를 아래 표 Ⅲ에 나타내었다.
위크(M. Wick), 핀게라 (W. Pinggera), 레만(P. Lehmann), 아이젠스토프벡셀(Eisenstoffwechsel)의 저서 (Diagnostik und Therapien der Anamien, 4., enlarged edition, Springer Verlag Vienna 1998)에 따르면, 모든 형태의 철분 부족을 임상 화학적으로 탐지하는 것이 가능하다. 이와 관련하여, 일반적으로, 페리틴 농도의 감소는 보상으로서 트랜스페린의 증대와 트랜스페린 포화도의 저하를 수반한다.
만성 염증성 장질환 (IBD)은 소화관의 만성적인 염증, 특히 크론병과 궤양성 결장염을 의미하는 것으로 이해된다.
좋기로는, 본 발명에 따라 이용될 수 있는 탄수화물과의 철(Ⅲ) 착화합물로서는 탄수화물이 풀루란, 올리고머 및/또는 이들의 유도체뿐만 아니라 덱스트란과 그의 유도체, 덱스트린과 그의 유도체로 이루어진 군으로부터 선택되는 것들을 들 수 있다. 전술한 유도체로는, 특히 수소화 유도체들을 들 수 있다. 덱스트린 또는 그의 산화물과의 철(Ⅲ) 착화합물이 특히 좋다. 본 발명에 따른 철(Ⅲ) 착화합물의 제조예들은, 예를 들어 전술한 특허 명세서 DE 14679800호, WO 04037865 A1, US 3076798호, WO 03/087164호 및 WO 02/46241호에서 발견되고, 그 명세서 본문, 특히 제조법과 관련된 부분을 전부 본 명세서에 포함시킨다. 본 발명에 따라 양호하게 사용되는 "덱스트린(dextrins)"이라는 용어는 녹말의 불완전한 가수 분해로 형성되는 D 글루코스 단위의 다양한 저중합체 및 고중합체를 위한 포괄 명칭이다. 또한 덱스트린은 당의 중합에 의하여 제조될 수 있다 (예컨대, WO 02083739 A2, US 20030044513 A1, US 3766165). 덱스트린으로는 말토덱스트린 및 폴리말토오스를 들 수 있는데, 이들은 예컨대 옥수수 녹말 또는 감자 녹말을 알파 아밀라아제로 효소 분해하여 제조되고, DE 값 (덱스트로스 당량)으로 나타내는 가수 분해도에 의하여 특징된다. 본 발명에 따르면, 폴리말토오스도 역시 녹말, 특히 덱스트린의 산 가수 분해에 의하여 얻을 수 있다. 본 발명에 따라 이용될 수 있는 철(Ⅲ) 착화합물의 제조는 일반적으로 철(Ⅱ) 또는 철(Ⅲ)염, 특히 염화철(Ⅲ)을 알칼리 수용액 (pH > 7)에서 덱스트린, 특히 폴리말토오스, 또는 덱스트린 산화물과 반응시킨 다음 단리 정제함으로써 수행된다. 또한, 약산 pH 범위에서도 역시 제조된다. 그러나, 알칼리 pH 값, 예컨대 >10이 좋다.
상기 pH는 서서히 또는 점차 증가시키는 것이 좋고, 이는 우선 예컨대 pH 약 3까지 약염기를 첨가함으로써 가능하고 이후 강염기로 중화시킬 수 있다. 이용 가능한 약염기로서는, 예컨대 알칼리 금속 또는 알칼리 토금속 탄산염 또는 중탄산염, 예컨대 탄산나트륨, 탄산칼륨, 중탄산나트륨, 중탄산칼륨 또는 암모니아 등을 들 수 있다. 강염기로서는, 예컨대 알칼리 금속 또는 알칼리 토금속 수산화물, 예컨대 수산화나트륨, 수산화칼륨, 수산화칼슘, 수산화마그네슘 등을 들 수 있다.
상기 반응은 가열함으로써 촉진시킬 수 있다. 예를 들면, 약 15℃에서 비등점까지의 온도가 사용될 수 있다. 온도는 서서히 올리는 것이 좋다. 따라서, 예를 들면, 혼합물을 먼저 15~70℃로 가열하고, 온도를 점차 비등점까지 증가시킬 수 있다.
반응 시간은 예컨대 약 15분에서 수 시간, 예컨대 20분~4시간, 25~70분, 30~60분이다.
반응이 일어나면, 반응 결과 얻은 용액은 예컨대 실온으로 냉각시킬 수 있고 필요에 따라 희석하고 필요에 따라 여과할 수 있다. 냉각 후, 산이나 염기를 첨가하여 pH를 중화점이나 그보다 약간 아래, 예컨대 5~7로 조절할 수 있다. 사용될 수 있는 염기로는 예컨대 상기 반응을 위한 전술한 염기들을 들 수 있다. 산으로서는, 예컨대 염산과 황산을 들 수 있다. 반응 결과 얻은 용액을 정제하여 약제 제조에 직접 사용할 수 있다. 그러나, 예컨대 에탄올 등의 알카놀과 같은 알코올로 침전시키는 등의 방법에 의하여 상기 용액으로부터 철(Ⅲ) 착화합물을 분리하는 것도 역시 가능하다. 그 분리는 분무 건조에 의하여도 역시 수행될 수 있다. 정제는 특히 염 제거를 위한 통상의 방법으로 이루어질 수 있다. 이는 예컨대 역삼투법 등으로 수행될 수 있는데, 이러한 역삼투법은 분무 건조에 앞서 또는 약제에 직접 사용하기에 앞서 수행될 수 있다.
이와 같이 하여 형성된 철(Ⅲ) 착화합물 중의 철분 함량은 예컨대 10~40% wt./wt., 특히 20~35% wt./wt.이다. 이들은 일반적으로 쉽게 물에 용해된다. 예컨대, 철분 함량이 1~20% wt./vol.인 중성 수용액을 이러한 방식으로 제조할 수 있다. 이들 용액은 가열 멸균할 수 있다.
철(Ⅲ)-폴리말토오스 착화합물의 제조에 관하여는 US 3076798호를 참조할 수 있다.
본 발명의 양호한 실시 상태에서는, 철(Ⅲ)-수산화폴리말토오스 착화합물이 사용된다. 이 철(Ⅲ)-말토오스 착화합물은 분자량이 20,000~500,000 달톤 범위인데, 30,000~80,000 달톤인 것이 좋다 [겔침투 크로마토그래피로 측정, 가이서 (Geisser) 등에 의한 문헌 (Arzneim. Forsch/Drug Res. 42(11), 12.1439-1452 (1992), paragraph 2.2.5.]. 특히 양호한 철(Ⅲ)-수산화폴리말토오스 착화합물은 시판품인 스위스 비포르 아게의 말토페르 (Maltofer®)이다. 또 하나의 양호한 실시 상태에서는, 1종 이상의 말토덱스트린의 산화물과의 철(Ⅲ) 착화합물이 사용된다. 이것은 예컨대 철(Ⅲ)염 수용액 및 알칼리 범위의 pH에서 차아염소산 수용액으로 1종 이상의 말토덱스트린을 산화시킨 산화 생성물의 수용액으로부터 얻을 수 있고, 여기서 1종의 말토덱스트린이 사용되는 경우에는 덱스트로스 당량은 5~37이고, 수종의 말토덱스트린의 혼합물이 사용되는 경우에는 그 혼합물의 덱스트로스 당량은 5~37이며, 혼합물에 함유된 개별 덱스트린의 덱스트로스 당량은 2~40이다. 이러한 방식으로 생성된 착화합물의 평균 분자량 Mw는 30~500 킬로 달톤인데, 80~350 킬로 달톤인 것이 좋고, 최대 300 킬로 달톤이 특히 좋다 [겔침투 크로마토그래피로 측정, 가이서 (Geisser) 등에 의한 문헌 (Arzneim. Forsch/Drug Res. 42(11), 12.1439-1452 (1992), paragraph 2.2.5.]. 이러한 관점에서, WO 2004037865 A1을 참조할 수 있는데, 그의 기재 내용을 전부 본 출원에 포함시킨다.
수소화 덱스트린과의 철 착화합물의 제조에 관하여 WO 03/087164호를 참고할 수 있다.
철(Ⅲ)-풀루란 착화합물의 제조에 관하여는 WO 02/46241호를 참고할 수 있다.
본 발명에 따라 사용되는 수산화철(Ⅲ) 착화합물은 경구 투여되는 것이 좋다. 그러나, 원칙적으로 정맥 주사 및 근육내 등 비경구적 투여될 수도 있다. 경구 투여량은 예컨대 10~500 ㎎ 철/일이다. 철분 상태가 개선될 때까지 수 개월 이상을 문제 없이 환자에게 상기 투여량을 투여할 수 있는데, 상태의 개선은 헤모글로빈 수치, 트랜스페린 포화도 및 페리틴 수치에 의하여 반영된다. 상기 경구 투여는 과립제, 캡슐제, 겔제 또는 새세이제 (sachet)로서, 정제, 캡슐제, 수용액제 또는 유액제의 형태인 것이 좋다. 어린이를 위한 용액제 또는 유액제의 이용은 시럽, 주스 또는 알사탕 등이 특히 좋다. 이를 위하여, 철(Ⅲ)-수산화덱스트린 착화합물은 통상의 제약상 담체 또는 보조 물질과 함께 적절한 투여형으로 투여될 수 있다. 통상의 결합제 또는 윤활제, 희석제, 붕해제 등이 이를 위하여 사용될 수 있다.
본 발명에 따른 용도는 만성 염증성 장질환으로 고생하는 어린이, 사춘기 청소년 및 성인에게 사용할 수 있지만, 성인에게 사용하는 것이 좋다.
본 발명에 따른 용도는 특히 철, 헤모글로빈, 페리틴 및 트랜스페리틴 수치의 개선에 있는데, 이에 따라 본 발명에 따른 치료에 의하여 장상태의 임상 질환 활성 표시, 복부 통증 및 구역질이 악화되지 않는다.
도 1은 황산철(Ⅱ) 또는 철(Ⅲ)-폴리말토오스 착화합물에 의한 처리 전과 후의 실시예에서 측정된 혈장 MDA 수치를 나타낸 도표이다. 만성 염증성 장질환 환자의 말론디알데히드 (MDA)의 혈장 수치에 있어서 황상철(Ⅱ)과 철(Ⅲ)-말토덱스트린 착화합물의 효과가 나타나고 있다. 상기 결과는 평균 ± SEM으로 표현되고, p 값은 쌍비교(雙比較)를 위하여 주어진 것이다.
후술하는 실시예를 통하여 본 발명의 활용법을 설명하고 증명한다.
환자
만성 염증성 장질환 (활성 상태 또는 정지 상태의 궤양성 결장염 또는 크론병)과 철분이 부족한 (평균 적혈구 용적 (mean corpuscular volume; MCV) < 80 fl 또는 s-페리틴 < 15 ㎍/ℓ 또는 s-가용성 트랜스페린 수용체 > 1.54 ㎎/ℓ에 의하 여 정의됨) 환자 41명을 무작위 추출법으로 2개 군으로 나누었다. 연구를 시작하기 전에 6주간 철분 치료 또는 수혈을 받은 환자, 연구 시작 2개월 미만 전에 시작한 아자티오프린 치료를 받은 환자, 코발라민 또는 엽산 부족으로 고생하는 환자, 암이나 신장질환이 있는 환자 또는 임신한 환자들은 제외되었다. 혈액, 소변 및 대변 분석과 질환의 임상 진단을 1일과 15일에 행하였다.
투약
1군에서의 치료는 황산철(Ⅱ) (Nycoplus Ferro-Retard® Nycomed Pharma AS, Norway), 14일간 아침에 식사 사이에 1정 (100 ㎎) (Fe2+ 100 ㎎에 해당), 저녁에 1정 투약하였고, 철(Ⅲ)-폴리말토오스 착화합물 (Maltofer Filmtabletten® Vifor International AG, Switzerland)에 의한 2군에서의 치료는, 14일간 아침 식사 중에 하루에 1회 2정 (전체 200 ㎎) (철(Ⅲ) 200 ㎎에 해당) 투약하였다. 상기 정제는 제조자의 권장 내용에 따라 투약하였다. 환자 순응도는 분배 받은 정제의 소비양으로서 정의하는데, 80%를 만족으로 간주하였다.
실험실 연구
야간에 단식 후 제1일과 제15일 아침에 혈액 시료를 취하였다.
혈장 말론디알데히드 (MDA), 혈장 아미노티오페놀류, 혈장 비타민 A, E 및 C 및 혈장 베타 카로텐을 문헌 (Svardal, AM., Manssor, MA., Ueland, PM., Anal. Biochem. 1990; 184:338-346; Vaagenes, H., Muna, ZA., Madsen, L., Berge, RK., Lipids 1998; 33:1131-1137)에 기재된 바와 같이 고성능 액체 크로마토그래피 (HPLC)로 측정하였다.
통상의 실험실 분석법으로서는 혈액 헤모글로빈의 측정, 혈액 망상 적혈구수, 평균 적혈구 용적 (MCV), 평균 혈구내 혈색소량 (mean corpuscular haemoglobin; MCH) 및 평균 혈구내 혈색소 농도 (mean corpuscular haemoglobin concentration; MCHC)의 측정, 혈액 적혈구수, 혈액 백혈구수 및 혈액 혈소판수, 망상적혈구 헤모글로빈 (CHr)의 측정, 색소 감소 적혈구수 (hypochromic red cell count; HYPO), 혈청 페리틴 및 혈청 철의 측정, 총철 결합력 (serum total iron binding capacity), 혈청 용해성 트랜스페린 수용체 및 혈청 C 반응성 단백질 (S-CRP)의 측정, 적혈구 침강 속도 (blood erythrocyte sedimentation rate; B-ESR) 측정 및 혈청 단백질과 혈청 알부민의 측정이 있다.
제1일과 제15일 아침에 소변 시료를 채취하여 크레아티닌 분석을 행하였다. 부틸-히드록시-톨루엔 (BHT)을 소변 2 ㎖에 첨가하여 최종 농도를 20 mM로 하였다. 이어서, 소변 8-이소프로스타글란딘 F2α (8-이소-PGF2α)이 분석될 때까지 시료들을 -80℃에 저장하였다. 노우루즈 자데 (Nourooz-Zadeh) 등의 방법 (Nourooz-Zadeh J., Gopaul NK., Barrow S., Mallet AI., Anggard EE., J. Chromatogr. B. Biomed. Appl. 1995; 667:199-208)에 따른 기체 크로마토그래피/질량 분석법으로 상기 분석을 수행하였으나, 최초의 가수 분해 단계를 생략하고 리 (Lee) 등의 고정상 프로토콜 (Lee CY., Jenner AM., Halliwell B., Biochem. Biophys. Res. Commun. 2004; 320:696-702)을 사용함으로써 소변 매트릭스에 관한 변형을 행하였다.
임상 질환 활성
임상 질환의 상태를 철분 치료 전 (제1일)과 후 (제15일)에 기록하였다. "크론병 활성에 대한 하비-브래드쇼의 단순 지표" (Harvey-Bradshaw Simple Index of Crohn's Disease Activity; Harvey, RF., Bradshaw, JM., Lancet, 1980; 1:514)로 크론병이 있는 환자에 대하여 임상 질환 활성을 평가하였다. 하비-브래드쇼의 단순 지표는 5가지 변수, 즉 일반 건강 상태, 복부 통증, 대변 주기, 복부 중량 및 기타 비장관성 합병증의 5가지 변수에 기초한다. 최대 점수는 25이고, ≥ 5의 점수는 크론병 활성을 의미한다.
궤양성 결장염이 있는 환자에 있어서, "결장염의 임상학적 단순 지표 (Simple Clinical Colitis Activity Index)"를 기록하였다 (Walmsley, RS., Ayres, RC., Pounder, RE., Allan, RN., Gut 1998; 43; 29-32). 결장염의 임상학적 단순 지표는 일반 건강 상태, 주간 및 야간 중의 대변 주기, 배변의 긴급성, 대변 중의 혈액 및 기타 비장관성 합병증의 6 가지 변수에 기초한다. 최대 점수는 20이고, ≥ 4의 점수는 궤양성 결장염 활성을 의미한다.
하비-브래드쇼의 단순 지표와 결장염의 임상학적 단순 지표는 구조와 점수의 주어진 변화의 임상학적 중요성의 관점에서 동일하다. 크론병 및 궤양성 대장염이 있는 환자의 결과가 함께 고려되도록 하기 위하여, 최대 점수에 의하여 나누어진 실질적인 점수로서 활성 점수를 계산하였다.
모든 환자들은 철분 치료의 개시 전 주와 철분 치료 2주 중에 특별한 크론병 활성 지표 (CDAI) 일지 카드 (Best, WR., Becktel, JM., Singleton, JW., Kern, F. Jr., Gastroenterology 1976; 70:439-444)를 완성하였다. CDAI 일지 카드는 매일의 일반 건강 상태, 복부 통증 및 액상이나 매우 묽은 대변 수의 기록으로 구성된다. 7일간의 기록 전체로 각 증상에 대한 점수를 낸다. 점수가 높을수록 환자들은 더 불리한 영향을 받는다. 약제는 연구 중 14일간 투약되었으므로, 2주간의 평균을 분석에 사용하였다. 또한, 환자들은 철분 치료 전과 치료 중의 구역질의 발생을 기록하였다.
증상의 악화로 상기 약제에 의한 치료가 중단된 환자들을 임상 질환 활성 및 증상 점수의 분석에 포함시켰다. 매일 이들의 질환 활성 점수를 2점씩 증가시키고 증상 점수를 1점씩 증가시켰다.
목적 및 결과
본 연구의 제1차 목적은 산화성 조직 손상에 대한 표지로서 경구 황산철(Ⅱ)과 경구 철(Ⅲ)-폴리말토오스 착화합물의 작용을 비교하는 것이었다. 제1차 결과는 혈장 MDA와 소변 이소-PGF2α이었다. 제2차 목적은 임상 질환 활성과 특이적 증상에 있어서 상기 2 가지 철분 제제들의 작용을 비교하는 것이었다. 철분 부족의 제거에 있어서 임상 효과의 연구를 위한 치료 시간이 너무 짧았다.
통계 분석
상기 2 개군의 내부 및 그들간의 차이를 쌍을 이룬 스튜던트 및 쌍을 이루지 않은 스튜던트 t 시험을 이용하여 평가하고, 평균차와 95% 신뢰 간격을 정하였다. 그 값들을 쌍차(雙差)에 대한 윌콕손 (Wilcoxon) 시험법을 이용하여 분석하고, 중간과 범위를 정하였다. 피셔 통계 분석법 (Fisher Exact test)에 의하여 비율 비교를 평가하였다. 0.05 미만의 p 값은 통계적으로 유의적인 것으로 평가된다. 윈도우 통계 소프트웨어 상품을 위한 그래프패드 프리즘 4 (GraphPad Software, Inc., San Diego, USA)를 이용하여 상기 자료들을 분석하였다.
결과
황산철(Ⅱ) 또는 (n=21) 철(Ⅲ)-폴리말토오스 착화합물 (n=20) 중 어느 한 가지로 치료하기 위하여, 환자 41명을 무작위 추출법으로 나누었다. 37명의 환자들이 프로토콜에 따른 연구를 완성하였다. 이들 환자에 있어서, 정제(錠劑)의 집계 결과, 황산철(Ⅱ)로 치료받은 환자 (100% (82-100)) 및 철(Ⅲ)-폴리말토오스 착화합물로 치료받은 환자 (100% (86-100))에 있어서, 비슷한 순응도를 나타내었다. 3명의 환자들 (크론병 1명, 궤양성 직장염 2명)은 각각 1, 4 및 5일 후에 황산철(Ⅱ)의 복용을 중단하고, 1명의 환자 (크론병)는 1일 후에 철(Ⅲ)-폴리말토오스 착화합물에 의한 치료를 중단하였다. 이들은 모두 과도한 장운동, 복부 통증 및 구역질로 고생하였다. 이들 환자들은 실험 값의 분석으로부터 제외되었으나, 임상 질환 활성 및 증후 점수 분석에는 포함시켰다.
산화성
스트레스에 대한 표지
황산철(Ⅱ)에 의한 치료는 혈장 MDA 값을 뚜렷하게 95 나노몰/ℓ 증가시켰고(CI 18 ~ 171; p = 0.018) (도 1), 소변 이소-PGF2α 값을 194 pg/㎎ 크레아티닌 증가시켰다 (CI 58 ~ 447; p = 0.12). 철(Ⅲ)-폴리말토오스 착화합물에 의한 치료는 혈장 MDA 값 (p = 0.16) (도 1) 또는 소변 이소-PGF2α 값 (p = 0.56)을 유의적으로 변화시키지 못하였다 (표 Ⅱ). 혈장 비타민 A, C 및 E, 베타 카로틴, 글루타티온, 시스테인, 시스테이닐 글리신 및 호모시스테인은 2 가지 치료 후에 모두 변하지 않았다 (표 Ⅱ). 황산철(Ⅱ)과 철(Ⅲ)-폴리말토오스 착화합물에 의한 치료의 비교시, 혈장 MDA (p = 0.08) 및 소변 이소-PGF2α (p = 0.28)의 (전후) 변화는 유의적으로 다르지 않다. 그러나, 상기 2개 군의 평균 혈장 MDA 값은 특정 처리 이후 유의적으로 차이가 났으며 (p = 0.007), 황산철(Ⅱ) 군에서 고(高)MDA 값이 발생하였다 (표 Ⅱ). 소변이나 혈장 변수 중의 어느 것도 임상 활성 지표와 관계가 없다.
임상 질환 활성 및 증후
임상 질환 활성 점수를 표 III에 나타낸다. 황산철(Ⅱ)로의 치료 (p = 0.45) 또는 철(Ⅲ)-폴리말토오스 착화합물에 의한 치료 (p = 0.80) 중 어느 것도 실질적으로 임상 질환 활성 지표를 변화시키지 못하였고, 치료제간에 변화 차이가 없었으며 (p = 0.81), 황산철(Ⅱ)에 의한 치료 중에 배변 횟수가 증가 (19 (7-106) ~ 24 (7-55); p = 0.0087)하는 반면, 철(Ⅲ)-폴리말토오스 착화합물은 주간 배변의 전체 횟수를 변화시키지 않았다 (17 (7-46) ~ 17 (6-66); p = 0.25). 황산철(Ⅱ)과 철(Ⅲ)-폴리말토오스 착화합물은 모두 일반 건강 상태나 복부 통증 점수 (자료는 나타내지 않음)에 영향을 미치지 않았다. 황산철(Ⅱ)로 치료받은 환자 9/21과 철(Ⅲ)-폴리말토오스 착화합물로 치료받은 환자 7/20에 의하여 구역질의 증가가 기록되었다 (p = 0.75).
통상의 실험실 분석
통상의 실험실 분석 값을 표 Ⅲ에 나타낸다. 황산철(Ⅱ)과 철(Ⅲ)-폴리말토오스 착화합물은 혈액 헤모글로빈을 증가시키지 못하였다. 황산철(Ⅱ)만이 망상 적혈구 헤모글로빈 (1.9 pg, CI 0.01 ~ 3.8; p = 0.049), s-페리틴 (12 ㎍/ℓ, CI 6 ~ 17; p = 0.0003) 및 혈액 망상 적혈구수 (0.016 x 1012/ℓ, CI -0.004 ~ 0.0360; p = 0.10)의 증가와 저색소성 적혈구 (-2.5 %, CI -4.6 ~ -0.3; p = 0.026), 혈청 용해성 트랜스페린 수용체 (-0.21 mg/ℓ, CI -0.31 ~ -0.11; p = 0.0005) 및 혈청 총철 결합력 (-7 마이크로몰/ℓ, CI -10 ~ -4; p < 0.0001)의 감소와 함께, 철분 부족의 생화학 표지에 유의적인 영향을 미쳤다. 철(Ⅲ)-폴리말토오스 착화합물은 단지 혈액 망상 적혈구 수만을 증가시켰다 (0.016 x 1012/ℓ, CI 0.001 ~ 0.030; p = 0.034).
삭제
상기 연구 결과로부터, 만성 염증성 장질환이 있는 환자에 있어서 철(Ⅲ)-폴리말토오스 착화합물에 의한 철분 치료의 양호한 내성이 달성되고, 황산철(Ⅱ)과 비교시 특히 대변 빈도가 감소하였으며, 소수의 환자들은 장질환 때문에 연구를 중단하였다는 것이 분명하다. 또한, 산화성 스트레스는 황산철(Ⅱ)로 치료시보다 본 발명에 따른 치료시에 유의적으로 더 낮다.

Claims (9)
- 탄수화물과의 철(Ⅲ) 착화합물을 활성성분으로서 함유하는, 만성 염증성 장질환이 있는 환자의 철분 부족 상태의 경구 치료용 의약 조성물.
- 제1항에 있어서, 상기 탄수화물은 풀루란, 이들의 올리고머 및 수소화 풀루란뿐만 아니라 덱스트란과 수소화 덱스트란, 덱스트린과 수소화 또는 산화덱스트린으로 이루어진 군으로부터 선택되는 것인 의약 조성물.
- 제1항 또는 제2항에 있어서, 상기 탄수화물은 산화 또는 수소화 덱스트린으로부터 선택되는 것인 의약 조성물.
- 제1항 또는 제2항에 있어서, 상기 철(Ⅲ) 착화합물은 철(Ⅲ)-폴리말토오스 착화합물인 것인 의약 조성물.
- 제4항에 있어서, 상기 철(Ⅲ)-폴리말토오스 착화합물은 분자량이 20,000 ~ 500,000 달톤 범위인 것인 의약 조성물.
- 제1항 또는 제2항에 있어서, 상기 철(Ⅲ) 착화합물은 1종 이상의 말토덱스트린의 산화물과의 철(Ⅲ) 착화합물인 것인 의약 조성물.
- 제6항에 있어서, 상기 철(Ⅲ) 착화합물은 철(Ⅲ)염 수용액 및 알칼리 범위의 pH에서 차아염소산 수용액으로 1종 이상의 말토덱스트린을 산화시킨 산화 생성물의 수용액으로부터 얻을 수 있는 수용성 철-탄수화물 착화합물이고, 1종의 말토덱스트린이 사용되는 경우에는 그 덱스트로스 당량은 5~37이며, 수종의 말토덱스트린의 혼합물이 사용되는 경우에는, 그 혼합물의 덱스트로스 당량은 5~37이고, 상기 혼합물에 함유된 덱스트린 각각의 덱스트로스 당량은 2~40인 것인 의약 조성물.
- 제1항 또는 제2항에 있어서, 과립제, 캡슐제, 겔제 또는 새세이제 (sachet)로서, 정제, 수용액제 또는 유액제의 형태로 제공되는 것인 의약 조성물.
- 제1항 또는 제2항에 있어서, 상기 만성 염증성 장질환은 크론병 또는 궤양성 결장염인 것인 의약 조성물.
