Description

【発明の詳細な説明】 本発明は経皮薬物投与に関する。さらに詳しくは、本発
明は避妊薬投与に関し、なおさらに詳しくは、本発明は
エチニル　エストラジオールとレボノルゲストレルとの
組み合わせの治療有効速度での経皮投与に関する。
本発明は下記の共通に譲渡された同時係属特許出願に開
示された発明に閏する： ガレ（Ｇａｌｅ）等の「プロゲステロン、エストラジオ
ールエステルおよびこれらの混合物の経皮投与ｊ；チェ
ング（Ｃｈｅｎｇ）等の「グリセロール　モノラウレー
トを用いる皮膚透過促進剤組成物１；およびチェンゾ等
のｒスクロース　エステルを用いる皮膚透過促進剤組成
物１（これらの総てが本発明と同じ出願口である） 経皮経路の非経口的薬物投与は多くの利点をイｊしてお
り、多様な薬物またはその他の有効剤を投与するための
経皮系は、例えば米国特許第３．５９８，１２２号、第
３，５９８，１２３号、第４，３７９，４５４す、第４
，２８６，５９２号、第４，３１４，５５７号および第
４．．５６８，３４３号に述べられている、これらの特
許は総て参考文献としてここに関係する。
避妊のための経口複合ビルおよび子宮内デバイスは不便
さと副作用のような、それらの問題のためによく知られ
ている。ここに述べるような、避妊薬の経皮投与はこの
ような問題を解消または軽減するための一つの試みであ
る。
本発明の目的の一つは、経皮系を用いて避妊薬を投与す
ることである。
本発明の他の目的は、エチニル　エストラジオールとレ
ボノルゲストレルを避妊有効速度で同時投与することで
ある。
本発明のさらに他の目的は、グリセロール　モノオレー
トのような皮膚透過促進剤を用いて避妊薬を経皮投与す
ることである。
本発明のさらに他の目的は、エチニル　エストラジオー
ルとレボノルゲストレルを組み合わせて経皮投与する方
法を提供することである。
上記その他の目的は、透過促進剤としてグリセロール　
モノオレートおよび避妊製剤としてエチニル　エストラ
ジオールとレボノルゲストレルの組み合わせを含むポリ
マーマトリックスを用いて経皮系を設計する本発明によ
って達成される。
添付図面を参照して、本発明をさらに詳細に説明する。
本発明は経皮薬物投与の原理を利用して、避妊薬を効果
的に投与するための新規な系を提供する。
皮膚を通しての薬物投与を促進するために、同時投与さ
れる透過促進剤、グリセロール　モノオレー）−（ＧＭ
Ｏ）を用いることが特に重要である０本発明はエチニル
　エストラジオールとレボノルゲストレルの７日間まで
の連続同時投与を可能にする。適当な透過促進剤として
ＧＭＯが用いられる。
第１図には、薬剤と透過促進剤とを含むマトリックスと
しての薬剤／透過促進剤溜め２を含む、本発明による治
療経皮系１を示す、溜め２は不透過性バッキング３によ
って覆われており、不透過性バッキング３の周囲は溜め
２よりもやや大きいサイズであることが好ましい、系１
を皮膚上に維持するための手段４は系の他の要素と一緒
に製造することも、別々に形成することもできる、第１
図の実施Ｓａ！では手段４が接着性オーバーレイの形状
である０本発明に接着性オーバーレイを用いる場合には
、グリセロール　モノオレートが薬剤学的に受容可能な
大ていの接触接着剤の接着性に不利に影響するので、溜
め２の皮膚隣接面に塗布するインライン接着剤の使用が
望ましい、この理由から、不透過性バッキング層３を溜
め２よりもやや大きいサイズにして、溜め２の周囲にグ
リセロール　モノオレート補助剤を全く含まない周辺領
域を形成して、オーバーレイ４中の接着剤と使用中に溜
め２の底面から漏出する透過促進剤との間の不利な相互
作用を予防することが望ましい、貼付前に除去するよう
に形成された、剥離可能な剥離ライナー５が包装製品に
通常含まれる。
種々な層の形成に適した多様な材料が前記特許に開示さ
れている。
ポリマーマトリックスは無水であることが好ましく、適
当な材料としては、限定する訳ではないが、例えば天然
ゴム、合成ゴムまたはその他の高分子材料、増粘化鉱油
または石油シェリーを挙げることができる０本発明によ
る好ましい実施Ｓａｔは米国特許第４，１４４，３１７
号に述べられている種類のエチレン／酢酸ビニル（ＥＶ
Ａ）コポリマーから、好ましくは酢酸ビニル（ＶＡ）含
量が約ｚｓ−６ｏ％の範囲であるＥＶＡコポリマーから
製造される。酢酸ビニル含量４０％のＥｖＡポリ？−（
ＥＶＡ４０）を用いて、特に良好な結果が得られている
。
好ましい実施態様は基本的には、ポリマー２０−５０％
、粘着付与剤１０−５０％、エチニル　エストラジオー
ル０．０１−１％、レボノルゲストレル０．１−５％お
よび透過促進剤１０−５０％の重量組成を有する一体構
造である。
好ましい系では、ＥＶＡポリマーをエラストマーとして
用いる。適当な粘着付与剤の典型的な例は完全に水素化
された芳香族炭化水素樹脂である。
ｒステイブライトエステル（Ｓ　ｔａｙｂｅｌｉｔｅ　
Ｅｓｔｅｒ”）１の商品名で販売されているパーキュレ
ス社（Ｈｅｒｃｕｌｅｓ、　Ｉ　ｎｃ）（プラウエア州
、ウイルミントン）の製品ラインを用いて、良好な結果
が得られている。特に、ステイブライト　エステル＃５
と＃１０が用いられている。
マトリックス中に飽和よりも過剰な濃度で、すなわち単
位活性で薬物を分散させるのが好ましい。
この過剰量は系の予定の有効期間によって定まる。
しかし、本発明の範囲から逸脱することなく飽和未満の
初期レベルで薬物が存在することも可能である。透過促
進剤好ましくはＧＭＯは、予定投与期間を通して溜め内
にＧＭＯの透過促進濃度を保証するような、充分な濃度
でマトリックス中に分散させるのが望ましい。
第２図では、患者の皮膚１７上に設置した本発明の他の
実施態様を示す、この実施態様では経皮治療系１０は少
なくとも２帯１２と１４とを有する多層状の薬剤／透過
促進剤溜め１１から成る。帯１２は実質的に第１図に関
して述べた通りの薬物量めから成る。帯１４は好ましく
は帯１２の形成に用いたマトリックスと実質的に同じマ
トリックスから成り、不溶な薬物を実質的に含まない透
過促進剤好めから成る。望ましい場合には、帯１２から
皮膚へのＧＭＯ放出速度を制御する速度制御膜を帯１２
と１４との間に用いることもできる。適当な速度制御膜
はＧＭＯに対して帯１２の透過性よりも低い透過性を有
するポリマーから製造することができる。第２図に説明
した系の利点は、帯１２に負荷した薬物が溜め全体に分
散するのではなく皮膚表面に集中することである。これ
は充分な透過促進剤供給を維持しながら、系の薬物量を
減するように作用する。
第１図に関して上述したような不透過性バッキング１５
と接着性オーバーレイ１６とを薬剤／透過促進剤溜め１
１上に重ねる。さらに、剥離可能な剥離ライナー（図示
せず）を第１図に関して述べたように使用前に系に備え
、皮膚１７に貼付する前に除去するのが好ましい、６ 第１図と第２図の実施態様では、キャリヤーまたはマト
リックス材料が滲み出るまたは流動することなくその形
状を維持するために充分な粘性を有する。マトリックス
またはキャリヤーが低粘性の流動可能な物質である場合
には、例えば上述の米国特許第４，３７９，４５４号か
ら技術上公知のように、不透過性バッキングと透過性ま
たは微孔質の皮膚接触膜との間のボウチまたはポケット
の中に組成物を完全に封入することができる。
本発明の経皮治療系は溶液流延およびメルトブレンディ
ングのような先行技術の方法によって製造することがで
きる。主な設計基準は活性な（薬剤含有）層が約５−１
５−１Ｏ厚さであることである。
本発明によって達成される有効避妊のなめに必要な経皮
流量はエチニル　エストラジオール少なくとも１５ｕｇ
／日およびレボノルゲストレルとも５０ｕｇ／日である
。　２０ｃｚ２系て゛は、これらの１日流量値はエチニ
ル　エストラジオール少なくとも０　、０４ｕｇ　／　
ｃｍ　’　７時およびレボノルゲストレル少なくとも０
　、１４Ｂ／　ｃｍ２／日と換算される。
本発明の系はエチニル　エストラジオールとレボノルゲ
ストレルとを７日間までの長期間にわたって有効投与す
るように設計することができる。有効避妊を達成するに
は、薬物投与が連続的でなければならない、従って、１
つの系を皮膚に貼付して有効期間が終了した場合には、
これを新しい系と取り替える０例えば、７日間系維持と
は７日間毎に新しい系と交換し、この交換を避妊が望ま
しい限り続けることを意味する。
本発明の実施態様と使用法は次の実施例から充分に理解
されるであろう。
実施例　１ 第１図に関して述べたような、エチニル　エストラジオ
ールとレボノルゲストレルを投与するための経皮治療系
をレボノルゲストレル５％、エチニル　エストラジオー
ル０．５％、ＥＶＡ４０　３７．８％、ステイブライト
　エステル＃５　２６．６％および０ＭＯ３０，１％か
ら製造した。インビトロ皮膚流呈を３７℃において７日
間にわたって測定した。次の表は薬物とＧＭＯの両方の
流量データ（１３サンプルの平均）を示す。

各薬物の皮膚透過量は次のように成る： （５ｘｌＯ−コ）（５Ｘ１０コ）＝２５ｕｇ／　ｃｚ２
本発明を特にその好ましい特定の実施態様に関して詳細
に述べたが、本発明の精神と範囲を逸脱しないで変更と
修正を行いうろことは理解されよう。

Claims (11)

    【特許請求の範囲】
  1. （１）エチニルエストラジオールとレボノルゲストレル
    から成る薬剤および皮膚透過促進剤を長期間にわたって
    経皮投与するための医用デバイスであって、 前記薬剤と前記透過促進剤とを含む薬剤溜め手段；およ
    び前記薬剤溜め手段を薬剤と透過促進剤とを皮膚に伝達
    する関係に維持する手段を組み合わせて含むデバイス。
  2. （２）前記薬剤溜め手段がエチニルエストラジオール、
    レボノルゲストレルおよび皮膚透過促進剤を含む単一溜
    め手段である特許請求の範囲第１項記載のデバイス。
  3. （３）前記透過促進剤がグリセロールモノラウレートで
    ある特許請求の範囲第２項記載のデバイス。
  4. （４）前記薬剤溜め手段がエチニルエストラジオールと
    レボノルゲストレルとを含む第１溜めと、皮膚透過促進
    剤を含む第２溜めとから成る２重溜め系である特許請求
    の範囲第１項記載のデバイス。
  5. （５）前記透過促進剤がグリセロールモノラウレートで
    ある特許請求の範囲第４項記載のデバイス。
  6. （６）前記薬剤溜め手段中にエチニルエストラジオール
    とレボノルゲストレルがそれらの飽和濃度よりも過剰な
    量で、すなわちエチニルエストラジオールとレボノルゲ
    ストレルの濃度を前記期間を通して少なくとも飽和レベ
    ルに維持するために充分な量で存在する特許請求の範囲
    第１項記載のデバイス。
  7. （７）前記第２溜めからの透過促進剤の放出を制御する
    手段をさらに含む特許請求の範囲第４項記載のデバイス
    。
  8. （８）前記薬剤溜め手段がエチニルエストラジオールを
    ７日間までの長期間にわたって少なくとも１５ｕｇ／日
    の速度で皮膚をとおして投与するために充分な量で含む
    特許請求の範囲第１項記載のデバイス。
  9. （９）前記薬剤溜め手段がレボノルゲストレルを７日間
    までの長期間にわたって少なくとも５０ｕｇ／日の速度
    で皮膚を通して投与するために充分な量で含む特許請求
    の範囲第１項記載のデバイス。
  10. （１０）前記長期間が前記デバイスの有効期間である特
    許請求の範囲第１項記載のデバイス。
  11. （１１）前記有効期間が７日間までである特許請求の範
    囲第１０項記載のデバイス。
